Relapse data registered in the DaTeCa database rely on manual registration. Currently, some safeguarding against missing registrations is attempted by a non-validated register-based algorithm. However, this algorithm is inaccurate and entails time-consuming medical record reviews. We aimed (1) to validate relapse data as registered in the DaTeCa database, and (2) to develop and validate an improved register-based algorithm identifying patients diagnosed with relapse of clinical stage I testicular cancer.Patients and Methods: Patients registered in the DaTeCa database with clinical stage I testicular cancer from 2013 to 2018 were included. Medical record information on relapse data served as a gold standard. A pre-specified algorithm to identify relapse was tested and optimized on a random sample of 250 patients. Indicators of relapse were obtained from pathology codes in the Danish National Pathology Register and from diagnosis and procedure codes in the Danish National Patient Register. We applied the final algorithm to the remaining study population to validate its performance.Results: Of the 1377 included patients, 284 patients relapsed according to the gold standard during a median follow-up time of 5.9 years. The completeness of relapse data registered in the DaTeCa database was 97.2% (95% confidence interval (CI): 95.2– 99.1). The algorithm achieved a sensitivity of 99.6% (95% CI: 98.7– 100), a specificity of 98.9% (95% CI: 98.2– 99.6), and a positive predictive value of 95.9% (95% CI: 93.4– 98.4) in the validation cohort (n = 1127, 233 relapses).Conclusion: The registration of relapse data in the DaTeCa database is accurate, confirming the database as a reliable source for ongoing clinical quality assessments. Applying the provided algorithm to the DaTeCa database will optimize the accuracy of relapse data further, decrease time-consuming medical record review and contribute to important future clinical research.Keywords: testicular cancer, relapse, algorithm, validation, registries

22. Broad-sense heritability and inter-trait relationships in young macadamia architecture, flowering and yield

Abstract

Macadamia in the orchard environment is relatively unaltered from its natural form, and there is considerable scope to change vegetative and reproductive architecture for improving yield and related traits through breeding. An understanding of genetic and environmental control mechanisms and the dynamics between vegetative and reproductive characteristics are useful to identify the most important traits for improvement. In breeding programs, considerations of the heritability of vegetative traits are usually limited to tree scale measurements such as canopy volume. Here we studied the broad-sense heritability (H) of characteristics that constitute the macadamia canopy and yield at multiple architectural scales, and also documented relationships between vegetative and reproductive traits. Cutting-grown clones of 15 macadamia genotypes were subsampled from a breeding trial planted in 2011 in South East Queensland, Australia. At the tree scale, canopy volume had very low H (0), suggesting a strong environmental influence. Detailed measurements of individual canopy components give insight to more complex interactions within the canopy. Some vegetative architectural characteristics, such as branch number, average internode length and growth unit (GU) length had medium H (0.31-0.39), and node number per GU had high H (0.57). Other canopy components such as length of primary branch axes and primary branch cross-sectional area (BCA) displayed very low heritability (0-0.05), and are likely to be mainly controlled by environmental conditions. H values were low for yield and nut counts (0.16-0.21), except nuts per raceme (0.43). Heritability was medium to high for other reproductive characteristics (0.34-0.70) that may indirectly relate to yield. Understanding the relationships within and between vegetative and yield traits at different architectural scales informs the choice of multiple useful traits, and will aid the breeding of future elite macadamia cultivars. 23. Positive association between cholesterol in human seminal plasma and sperm counts:results from a cross-sectional cohort study and immunohistochemical investigations

Background: Cholesterol is essential for cell membrane stability, permeability, and fluidity. Cholesterol is present in seminal plasma, but whether a relationship between the level of cholesterol in seminal plasma and semen quality exists remains to be elucidated. Objectives: To explore the association between cholesterol levels in seminal plasma and serum cholesterols, semen quality, and serum reproductive hormones. Secondly, to explore whether the associations are biologically plausible. Materials and Methods: An association study between cholesterol levels in seminal plasma and semen quality in 403 men, median age 19 years, from the general population. Additionally, an immunohistochemical evaluation of proteins involved in cholesterol metabolism and transport in tissues from the male reproductive tract (testis, epididymis, prostate, and seminal vesicle). Tissue specimens were investigated by immunohistochemistry for markers of cholesterol metabolism and transport (ABCA1, ABCG1, CYP11A1, CYP51A1, HMGCR, LAL, LCAT, LDLR, and SOAT1). Results: Trend analyses showed that total amount of total cholesterol in seminal plasma was positively associated with sperm concentration, total sperm count, sperm motility, and morphology (all p < 0.008, adjusted). Cholesterol concentrations in seminal plasma were neither associated with serum cholesterol and lipid levels nor serum reproductive hormone (FSH, LH, testosterone, estradiol, sex-hormone-binding globulin, inhibin b) levels. All investigated markers of cholesterol metabolism and transport were expressed in the investigated tissue specimens to varying degrees. Discussion: Seminal plasma level of cholesterol was positively associated with semen parameters. The presence of proteins and enzymes involved in cholesterol metabolism in Leydig cells, Sertoli cells, and maturing germ cells in the seminiferous tubules supports the view that cholesterol may be important for spermatogenesis. Abstract

INTRODUCTION & OBJECTIVES: Nodal involvement is a strong prognosticator in penile cancer and lymph node staging is crucial. Sentinel node biopsy (SNB) has proven a useful staging tool with few complications, but evidence rely mostly on single institution publications with a short follow-up. In this study we estimated the diagnostic accuracy of SNB in patients with squamous cell carcinoma of the penis and assessed procedure-related complications in a multicentre setting with long-term follow-up. MATERIAL & METHODS: Data was retrospectively collected from medical records by one medical doctor at the four university centres where the procedure was performed. Thus, the study covers all SNB procedures performed in Denmark in the 11-year period 2000-2010. Included patients were newly diagnosed and had either non-palpable lymph nodes (cN0) in one or both groins or had a palpable mass in the groin, from which aspiration cytology was unable to detect malignancy. Routine ultrasound of cN0 groins was not used. Patients were injected with nanocolloid technetium and a scintigram was recorded before the surgical SNB. A gamma probe and intraoperative palpation of the exposed inguinal wound was used for the detection of lymph nodes. In addition 1 ml of Patent blue was used intradermally in the circumference of the proximal penile shaft just prior to surgery in 3 centres. Diagnostic accuracy was calculated on the basis of histopathology and complete follow-up until October 2014. RESULTS: 409 groins in 222 patients were examined by SNB. Median follow-up of patients who survived was 6.6 years (interquartile range: 5-10). No patient was lost to follow-up. A total of 1004 lymph nodes were removed of which 93 nodes (9.3 %) harboured metastatic tissue at histopathological examination. All groins with positive SNB underwent complementary ipsilateral ILND. Sixteen groins (24 %) contained further positive lymph nodes, while 50 groins (76 %) had no further metastases on histopathological examination of ILND specimen. In total 66 groins (52 patients) had positive SNB and 343 groins had negative SNB. During a median follow-up of 73 (IQR 53- 102) months 8 false negative groins were encountered. Sensitivity was 89.2% (95% CI, 79.8-95.2%) per groin and false negative rate was 10.8% (95% CI, 4.8-20.2%). Remarkably, four of 67 T1G1 patients had a positive SNB. This is interesting considering the current EAU recommendation with no surgical staging of T1G1 tumours. Twenty-eight of 222 (13%) patients experienced 39 postoperative complications of Clavien-Dindo grade I-IIIa to SNB during follow-up. Complications were encountered after 30 of 409 (7%) procedures and were primarily grade I. One patient was readmitted because of severe wound infection, the remaining complications were handled as a part of primary hospitalization, or in the outpatient clinic. 8 patients experienced more than one SNB-related complication. No patient needed to return to the operating room because of SNB related complications. No patient died from complications. CONCLUSIONS: To our knowledge, this is the first complete national study on sentinel node biopsy. Penile cancer sentinel node biopsy with a close follow-up is a reliable lymph node staging and has few complications in a national multicentre setting. Inguinal lymph node dissection which has complication rates of 30 - 70 % was avoided in 76 % of patients. The EAU guideline recommendation with no surgical staging of T1G1 tumours should be reconsidered.

Abstract

Background:Testicular germ cell tumours of young adults, seminoma or non-seminomas, are preceded by a pre-invasive precursor, carcinoma in situ (CIS), understood to arise through differentiation arrest of embryonic germ cells. Knowledge about the malignant transformation of germ cells is currently limited by the lack of experimental models. The aim of this study was to establish an experimental tissue culture model to maintain normal and malignant germ cells within their niche and allow investigation of treatment effects.Methods:Human testis and testis cancer specimens from orchidectomies were cultured in 'hanging drops' and effects of activin A and follistatin treatment were investigated in seminoma cultures.Results:Testis fragments with normal spermatogenesis or CIS cells were cultured for 14 days with sustained proliferation of germ cells and CIS cells and without increased apoptosis. Seminoma cultures survived 7 days, with proliferating cells detectable during the first 5 days. Activin A treatment significantly reduced KIT transcript and protein levels in seminoma cultures, thereby demonstrating a specific treatment response.Conclusions:Hanging drop cultures of human testis and testis cancer samples can be employed to delineate mechanisms governing growth of normal, CIS and tumorigenic germ cells retained within their niche.

42. Individual versus standard dose of rFSH in a mild stimulation protocol for intrauterine insemination: a randomized study

Abstract

Udgivelsesdato: 2009-Oct, BACKGROUND: Controlled ovarian stimulation (COS) and intrauterine insemination (IUI) are often used as the first-line treatment for subfertile couples. To minimize the variability in ovarian response in patients' first treatment cycle, we recently developed a recombinant follicle-stimulating hormone (rFSH) dosage nomogram. The nomogram has now been tested. METHODS: Multicentre randomized controlled trial (RCT) including 228 ovulatory patients scheduled for COS and IUI. Patients were randomized to 'individual' (50-100 IU rFSH/day, n = 113) or 'standard' (75 IU rFSH/day, n = 115) dose. 'Individual' dose was prescribed according to the nomogram, which was based on patients' body weight and antral follicle count. The primary end-point was the proportion of patients with two to three follicles > or = 14 mm (maximum two follicles > or = 18 mm) on the day of hCG (leading follicle = 18 mm). Primary analysis was made by intention-to-treat. RESULTS: In the 'individual' group, 79/113 (70%) of the patients developed two to three follicles versus 64/115 (56%) in the 'standard' group [absolute difference = 14.3 percentage points; 95% confidence interval (CI) 2-26, P = 0.03; absolute difference = 14.4; 95% CI 2-27, P = 0.02, when adjusting for centre]. Among patients with two to three follicles, the proportion of patients with two follicles was 46/79 (58%) in the 'individual' group versus 34/64 (53%) in the 'standard' group, P = 0.54. Ongoing pregnancy rate was 23/113 (20%) in the 'individual' group and 21/115 (18%) in the 'standard' group and the rate of multiple gestations was 1/113 (1%) versus 5/115 (4%), P = 0.21. CONCLUSIONS: This RCT is the first to clinically test a dosage nomogram in ovulatory IUI patients' first rFSH treatment cycle. Dosing according to the nomogram was superior to standard dosing. Udgivelsesdato: 2008-Nov, The objective of this prospective study was to identify predictors of ovarian response in ovulatory patients treated with low-dose recombinant FSH (rFSH), gonadotrophin-releasing hormone antagonist and intrauterine insemination (IUI), and to develop an rFSH dosage nomogram based on the findings. Patients (n = 159) were stimulated with a starting dose of 75 IU rFSH/day. Ten parameters were investigated as possible predictors of the number of mature follicles >or=15 mm: age, spontaneous cycle length, body weight, body mass index, smoking status, total ovarian volume, total number of antral follicles, total Doppler score of the ovarian stromal blood flow, baseline FSH and oestradiol. Simple and multiple linear regressions were used for the statistical analysis. Appropriate ovarian response was defined as two to three mature follicles. Body weight (P = 0.001) and the number of antral follicles (P = 0.004) were the strongest independent predictive factors of the number of mature follicles. In conclusion, body weight and antral follicle count may be used to achieve appropriate ovarian response for IUI in ovulatory patients. Based on this, a simple rFSH dosage nomogram was developed for individual ovarian stimulation prior to IUI.

Abstract

Background: Systematized Nomenclature of Medicine (SNOMED) codes are computerprocessable medical terms used to describe histopathological evaluations. SNOMED codes are not readily usable for analysis. We invented an algorithm that converts prostate SNOMED codes into an analyzable format. We present the methodology and early results from a new national Danish prostate database containing clinical data from all males who had evaluation of prostate tissue from 1995 to 2011. Materials and methods: SNOMED codes were retrieved from the Danish Pathology Register. A total of 26,295 combinations of SNOMED codes were identified. A computer algorithm was developed to transcode SNOMED codes into an analyzable format including procedure (eg, biopsy, transurethral resection, etc), diagnosis, and date of diagnosis. For validation, ~55,000 pathological reports were manually reviewed. Prostate-specific antigen, vital status, causes of death, and tumor-node-metastasis classification were integrated from national registries. Results: Of the 161,525 specimens from 113,801 males identified, 83,379 (51.6%) were sets of prostate biopsies, 56,118 (34.7%) were transurethral/transvesical resections of the prostate (TUR-Ps), and the remaining 22,028 (13.6%) specimens were derived from radical prostatectomies, bladder interventions, etc. A total of 48,078 (42.2%) males had histopathologically verified prostate cancer, and of these, 78.8% and 16.8% were diagnosed on prostate biopsies and TUR-Ps, respectively. Future perspectives: A validated algorithm was successfully developed to convert complex prostate SNOMED codes into clinical useful data. A unique database, including males with both normal and cancerous histopathological data, was created to form the most comprehensive national prostate database to date. 46. Ultrasound characteristics and histological dating of the endometrium in a natural cycle in infertile women compared with fertile controls

Abstract

OBJECTIVE: To compare endometrial dating and ultrasound texture in a natural cycle before IVF and relate these to outcome and to fertile references. DESIGN: Prospective study with a fertile reference group. SETTING: Four university IVF clinics. PATIENT(S): Seventy-five IVF patients and 21 fertile women. INTERVENTION(S): Ultrasound, biopsy, blood sampling. In vitro fertilization-ET in the following cycle. MAIN OUTCOME MEASURE(S): Endometrial evaluation, P, IVF outcome. RESULT(S): At day LH+7, 42% infertile vs. 67% fertile women demonstrated endometria that were in phase (statistically nonsignificant difference). Nine percent had an accelerated endometrium, and 47% (infertile) vs. 24% (fertile) had a delayed endometrium (statistically nonsignificant difference). Statistically significantly fewer women with tubal factor and no hydrosalpinges had an endometrium in phase (20%) than was the case in fertile women (67%). Dating could not predict outcome. Statistically significantly more women in the hydrosalpinx group had a visible midline at day LH+7 compared with the case in other infertile groups. Forty-three percent conceived after IVF-ET. Plasma P was statistically significantly lower in nonpregnant women compared with in women with ongoing pregnancies and with fertile controls. A periovulatory hyperechogenic endometrium resulted in no ongoing pregnancies. CONCLUSION(S): In a natural cycle preceding IVF, a low midluteal P level predicts a low implantation rate. A periovulatory hyperechogenic endometrium or hydrosalpinges visible at ultrasound may have some predictive value. Endometrial dating was of no help.
