An expanding array of image-guided spine interventions have the potential to provide immediate and effective pain relief. Innovations in spine intervention have proceeded rapidly, with clinical adoption of new techniques at times occurring before the development of bodies of evidence to establish efficacy. Although new spine interventions have been evaluated by clinical trials, acceptance of results has been hindered by controversies regarding trial methodology. This article explores controversial aspects of four categories of image-guided interventions for painful conditions: spine interventions for postdural puncture headache resulting from prior lumbar procedures; epidural steroid injections for cervical and lumbar radiculopathy; facet and sacroiliac joint pain interventions; and vertebral augmentations for compression fractures. For each intervention, we summarize the available literature with an emphasis on persistent controversies and discuss how current areas of disagreement and challenge may shape future research and innovation. Despite the ongoing areas of debate for various aspects of these procedures, effective treatments continue to emerge and show promise for aiding relief on a range of debilitating conditions.

The history of deglaciation in the Finger Lakes region since the Valley Heads readvance is questioned by recent research in the Cayuga basin, which concludes that, instead of forming a series of proglacial lakes, drainage during the Mackinaw Interstade was into the Laurentide ice sheet. First suspected in the Dryden–Virgil Valley where there is an absence of a lake outlet or surficial lacustrine deposits, this conclusion was explicitly revealed in the Sixmile–Willseyville trough where ice margin channels funneled water into the ice front. Further support was found in the Cayuga Inlet Valley, where a kettle kame terrane sloped northward into the ice front. Northward drainage was preceded by southerly drainage, with reversal occurring about 16.3 kyr ago. Multi-channel seismic profiles at the south end of Lake Cayuga reveal a south-sourced subaqueous sedimentary fan at the base of the lacustrine sequence. This fan is correlated with a coarse and heterogeneous clastic sequence penetrated in water wells in the City of Ithaca and requires northward drainage into a subglacial lake, which precludes the existence of proglacial lakes Ithaca, Newberry, and Hall. The proposed subglacial flow path is through the Cayuga trough, exiting the ice front eastward in the Mohawk Valley. Subglacial drainage from the Cayuga trough probably was part of a regional subglacial drainage system during the Mackinaw Interstade. Studies north of Lake Ontario have led to the proposal of a subglacial lake in the Ontario basin at that time, which likely also drained into the Mohawk Valley.

Most ruptured cerebral aneurysms are small (7 mm). Evidence suggests low rupture rates for such lesions (1% per year). Population studies demonstrate a prevalence rate of 3.2%. This study simultaneously estimates the prevalence of aneurysms in a single geographic population while reporting the observed rate of aneurysmal subarachnoid hemorrhage (aSAH) in the same geographic region composed of a poor urban minority demographic.This is an institutional review board-approved, Health Insurance Portability and Accountability Act of 1996-compliant retrospective study performed between 2005 and 2011 at a single center. Part 1 used the electronic medical record to identify all patients with a magnetic resonance angiography demonstrating a cerebral aneurysm. Part 2 used the electronic medical record to identify all patients from the same geographic area presenting with aSAH during the study period.A total of 11,160 subjects had a magnetic resonance angiography from the study area. In this group, 422 intradural cerebral aneurysms were incidentally discovered. Ninety-one percent were less than 10 mm (mean 5.49, standard deviation 4.6). Twenty-one percent were aneurysms of the anterior communicating artery complex. Fourteen percent were of posterior communicating artery origin. A total of 237 patients had aSAH. Ninety-two percent of the aneurysms were less than 10 mm (mean 6 mm, standard deviation 3.2 mm). Both groups were composed of poor urban minority patients.The observed annual rate of rupture of small anterior circulation aneurysms in this study was .06%-.15% per year. The extrapolated population prevalence of such aneurysms (4.0%-1.5%) may explain the observed rate of rupture of these small aneurysms in a poor urban minority population.

Background: Epidural steroid injections (ESIs) are a common method for treating lower back pain, which is one of the most prevalent health-related complaints in the adult U.S. population. Although the safety of CT-guided ESIs has been extensively studied in adults, there is limited data concerning the procedure’s safety profile in an older patient population. Objective: This retrospective study analyzed safety data among a single-center cohort of patients > 65 years-old who received one or more CT-guided interlaminar ESIs from 2012 to 2015. Study Design: An Institutional Review Board (IRB)-approved retrospective chart review. Setting: University hospital center. Methods: A total of 688 CT-guided ESI procedures were evaluated and a linear regression analysis was conducted to examine the relationship between dose length product (DLP), body mass index (BMI), procedure duration, and kVp/mA settings. Further analysis was performed on a sample of long procedure time, average-DLP and high-DLP procedures. Results: Average age was 75.77 years, with 44% having a BMI > 30. The mean DLP was 55.58 mGy x cm and the mean procedure duration was 5.94 minutes. All procedures were technically successful and no complications were observed during or after any of the procedures, including at one-month follow-up office visits. The kVp and mA settings were the strongest predictors of DLP, followed by procedure time. The high-DLP cases had a greater number of needle placement series, more intervertebral disc spaces included in each planning series and higher machine settings (kVp 120; mA 87.5) than the average-DLP cases (kVp 100; mA 49.9). Limitations: This study is limited by its retrospective design. Conclusion: CT-guided interlaminar ESIs can be performed safely, with low procedure times, relatively low DLP’s and without complications in an older patient population. Peer review is a cornerstone of quality improvement programs and serves to support the peer learning process. Peer review in radiology incorporates the review of diagnostic imaging interpretation, interventional procedures, communication, and the evaluation of untoward patient events. A just culture is an environment in which errors and near-miss events are evaluated in a deliberately nonpunitive framework, avoiding a culture of blame and responsibility and focusing instead on error prevention and fostering a culture of continuous quality improvement. Adoption of a just culture requires careful attention to detail and relies on continuous coaching of individuals and teams to ensure future systems improvements and a culture of safety. The authors describe the practical implementation of a just culture framework for peer review in an academic radiology department and highlight its application to interpretive, noninterpretive, and procedural domains through case examples.

BACKGROUND The choice of the recipient vessels for microsurgical reconstruction is based on proximity to the defect, vessel caliber, anatomic reliability, and ease of dissection. Traditionally in head and neck reconstruction, the superficial temporal artery and vein (STA/V) have been reserved as secondary recipient vessels. Others, including the facial artery (FA) and vein have been the preferred choice for facial reconstruction. METHODS A retrospective analysis of all consecutive head and neck reconstructions using the STA/V by the senior author (E.S.G.) over a 5-year period was performed. Perioperative data were reviewed for all the patients. In a subset of patients, radiographic analysis was used to delineate STA and FA anatomy including vessel diameter and distances to standard anatomic landmarks. RESULTS A total of 31 patients had 32 microsurgical reconstructions using the STA/V as recipient vessels. Radiographic analysis revealed no significant difference between the STA and FA diameters. Image guidance for spine pain control procedures, including epidural steroid injection, nerve root block, and facet block, can be performed with either computed tomography (CT) or conventional fluoroscopy. CT has the advantage of improved anatomic localization and use of air for contrast; however, there are concerns that CT leads to higher radiation dose and longer procedure time.To evaluate procedure time and radiation dose for multiple types of spine pain control procedures performed under CT guidance.Retrospective evaluation.Department of radiology in single academic medical center.Institutional review board approval was obtained. We reviewed CT-guided spine procedures performed over a 12-month period from January 2012 to December 2012. Procedure type, procedure time, and dose-length product were recorded. Patient age and gender were recorded for each case; additionally, demographic and medical history data were obtained for a sub-group of patients.Nine hundred ninety-four studies (performed in 699 patients) were reviewed, including 585 epidural steroid injections, 228 nerve root blocks, and 90 facet blocks. For all studies, procedure time averaged 7:34 ± 5:05, and dose-length product averaged 75 mGy·cm ± 61. Additional medical history (available for 483 patients) revealed high rate of obesity (body mass index [BMI] = 30 ± 6.8, with 76% of patients overweight [BMI25] and 42% obese [BMI30]), and frequent medical comorbidities (including hypertension [n = 179], diabetes [n = 101], renal failure [n = 30], and heart failure [n = 17]).This study was performed retrospectively, and limited to a single institution.These findings add to the growing evidence that CT guidance is a safe and effective technique for epidural steroid injection. These results further demonstrate that other spine intervention procedures, including nerve root block and facet block, can also be performed under CT guidance with short procedure time and reasonable levels of radiation exposure. This approach can be effectively used in a patient population with a high rate of obesity and medical comorbidities.

Sacral insufficiency fractures are a common cause of severe low back pain and immobilization in patients with osteoporosis or cancer. Diagnosis is often delayed due to the relative insensitivity of conventional radiography. Current management options range from analgesia and physical therapy to surgical fixation. Advances in interventional neuroradiological techniques using sacroplasty have resulted in a safe and effective percutaneous treatment option for patients with minimal associated morbidity. Recently, coccygeoplasty for treating coccyx subluxation/fracture has been described.

This article reviews the imaging of lumbar spinal fusion and its major indications. The most common procedures are described for the purpose of allowing understanding of postoperative imaging. Imaging options are reviewed for preoperative workup, intraoperative guidance, and postoperative purposes. Examples of hardware integrity, fusion, and loosening are provided.

The last several decades have witnessed exponential advances in the technology and use of spinal cord neuromodulation for the treatment of chronic refractory pain syndromes in patients whom medical and surgical management have been exhausted. This chapter focuses on the technical aspects of percutaneous placement of SCN devices, emphasizing patient selection, technique, and complications.

Bilateral internal carotid artery dissection is a rare event associated with pregnancy, especially in a patient without any predisposing risk factors.A 34-year-old woman presented with postpartum unilateral weakness, headaches, and blurry vision 14 days after vaginal delivery. Radiologic imaging revealed bilateral cervical internal carotid artery dissections and cerebral infarctions. She was treated with anticoagulation and showed radiographic and clinical improvement.The pathophysiology of cervical artery dissection appears multifactorial, with evidence suggesting environmental and genetic contributions. Intimal injury related to the Valsalva maneuver during labor as well as hemodynamic and hormonal changes related to pregnancy are presumed causes of peripartum spontaneous carotid artery dissection. Antithrombotic therapy for at least 3 to 6 months after dissection and follow-up neuroimaging are suggested.

The aim of this study was to characterize cumulative exposure to ionizing radiation from diagnostic imaging (CEDI) in adult patients and investigate its relationship to a patient's socioeconomic status and comorbid medical conditions.A retrospective cohort study was conducted of 54,015 patients seen within the outpatient clinic system of an academic, tertiary care, urban medical center during the month of January 2006, estimating the CEDI from all procedures performed within 3 years of the index visit (until January 2009). Socioeconomic status was estimated from census tract geocoding. Comorbid medical conditions were identified from the electronic medical record.A total of 9,537 adult patients were seen within the index month and underwent imaging tests. Eighty percent were living in areas with10% poverty. Thirty-six percent of men and 43% of women had diagnoses from the Elixhauser list. Mean CEDI values were 10 ± 19.09 mSv for those from areas with10% poverty and 8.9 ± 22.42 mSv for those living in areas with10% poverty. Poverty and comorbidities covaried. Estimated CEDI within groups of patients with the same comorbidity was not associated with socioeconomic status.At this institution, there is a high prevalence of patients living in poverty. Those living in poverty are at higher risk for comorbid conditions that are associated with increased CEDI. However, controlling for comorbidity, socioeconomic status was no longer predictive of CEDI.

Head CT scans are considered the imaging modality of choice to screen patients with head trauma for neurocranial injuries; however, widespread CT imaging is not recommended and much research has been conducted to establish objective clinical predictors of intracranial injury (ICI) in order to optimize the use of neuroimaging in children with minor head trauma. To evaluate whether a strict application of the New Orleans Criteria (NOC), Canadian CT Head Rule (CCHR) and National Emergency X-Radiography Utilization Study II (NEXUS II) in pediatric patients with head trauma presenting to a non-trauma center (level II) could reduce the number of cranial CT scans performed without missing clinically significant ICI. We conducted an IRB-approved retrospective analysis of pediatric patients with head trauma who received a cranial CT scan between Jan. 1, 2001, and Sept. 1, 2008, and identified which patients would have required a scan based on the criteria of the above listed decision instruments. We then determined the sensitivities, specificities and negative predictive values of these aids. In our cohort of 2,101 patients, 92 (4.4%) had positive head CT findings. The sensitivities for the NOC, CCHR and NEXUS II were 96.7% (95%CI 93.1–100), 65.2% (95%CI 55.5–74.9) and 78.3% (95%CI 69.9–86.7), respectively, and their negative predictive values were 98.7%, 97.6% and 97.2%, respectively. In contrast, the specificities for these aids were 11.2% (95%CI 9.8–12.6), 64.2% (95%CI 62.1–66.3) and 34.2% (95%CI 32.1–36.3), respectively. Therefore, in our population it would have been possible to scan at least 10.9% fewer patients. The number of cranial CT scans conducted in our pediatric cohort with head trauma would have been reduced had any of the three clinical decision aids been applied. Therefore, we recommend that further validation and adoption of pediatric head CT decision aids in non-trauma centers be considered to ultimately increase patient safety while reducing medical expense.

Computed tomography (CT)-guided percutaneous drainage is a minimally invasive procedure that allows for accurate diagnosis and therapy with minimal complications. The drawback is that CT guidance carries a significant amount of radiation exposure. CT-guided percutaneous drainages have been widely used in adults and have been gaining momentum within the pediatric population. Through a thorough review of our institution's (Montefiore Medical Center) CT-guided percutaneous drainages within our pediatric patients, we assessed the radiation exposure per study as well as which studies were deemed possible under ultrasound guidance as a possible alternative.

Background: Hospital admissions for back pain are prolonged, costly, and common. Epidural steroid injections are frequently performed in an outpatient setting with an excellent safety and efficacy profile. Objectives: The purpose was to review data from patients with severe pain that did not respond to aggressive medical treatment in the emergency department (ED) and determine the effectiveness of an interlaminar epidural steroid injection (ESI) in this patient population. Study Design: Retrospective matched cohort design. Setting: Single urban emergency department at a tertiary referral center. Methods: A retrospective cohort comparison pairing 2 groups that both failed aggressive pain control in the ED was performed. The epidural injection group (1ESI) received an interlaminar ESI while in the ED. The standard therapy group (2ST) was admitted for medical pain management. Groups were matched for pain intensity, age, and symptom duration. Results: Thirty-five patients in 1ESI (NRS 8.8, 5 – 10, 0.35), and 28 patients in 2ST (NRS 8.9, 4 – 10, 1.7). Pain score after ESI 0.33 (0 – 2, 0.6); all were discharged. Pain score on day 1 of hospital admission for 2ST was 8.7 (7 – 10, 1.5). Total ED time was 8 hours for 1ESI and 13 hours for 2ST (P < 0.002). 1ESI patients received less narcotics while in the ED (P < 0.002) and were discharged home with less narcotics than 2ST (< 0.002). Average inpatient length of stay (LOS) for 2ST was 5 (1.5 – 15, 3.3) days. Cost of care was over 6 times greater for those patients admitted for pain management (P < 0.001). Limitations: Retrospective design, non-randomized sample, and a small patient population. Conclusion: An ED patient cohort with severe refractory pain was treated with an interlaminar ESI after failing maximal medical pain management while in the ED. Complete pain relief was achieved safely and rapidly. The need for inpatient admission was eliminated after injection. Costs were lower in the group that received an epidural injection. Narcotic requirements upon discharge were decreased as well. Key words: Low back pain, epidural steroid injection, emergency department, hospital admission

Historically, ESIs were performed without any imaging guidance, resulting in erroneous placement in up to 30% of injections. Fluoroscopic imaging is now used to guide most procedures. Recently, several reports have described the use of CT to guide ESIs instead of fluoroscopy. CT provides the ability to use air as contrast to localize the epidural space. This retrospective review will discuss findings in 1000 CT-guided ESIs with air localization.

The screening and detection of recurrent medulloblastoma presents the clinician with significant diagnostic challenges, including the risk of misdiagnosis. The authors present the case of a young girl with a history of a treated standard-risk medulloblastoma that highlights the risk of assuming recurrence has occurred when clinical and/or imaging changes are observed. This girl developed both new clinical deficits and had radiographic evidence of recurrence. She subsequently experienced a complete resolution of symptoms and radiographic findings with steroids alone.

BACKGROUND AND PURPOSE: Hippocampal malrotation (HIMAL) is a failure of hippocampal inversion that occurs during normal fetal development and has been seen on MR imaging examinations of people with epilepsy, but it has not been studied in patients without epilepsy. We intended to evaluate the prevalence of HIMAL in MR imaging examinations of patients without seizures to better understand the significance of HIMAL in the population with seizure. MATERIALS AND METHODS: A total of 497 MR imaging examinations with thin-section imaging through the temporal lobes of patients referred for conditions other than seizures were reviewed. The examinations were performed on 1.5T magnets. Sagittal T1-weighted and coronal T2-weighted images were used to evaluate each MR image for the distinctive features of HIMAL. As previously described in the literature, the criteria for HIMAL include unilateral involvement and incomplete rotation of a hippocampus that is normal in size and signal intensity but abnormally rounded in shape, with blurred inner structure. In addition, ipsilateral findings of an atypical collateral sulcus angle and atypical position and size of the fornix were noted. The corpus callosum is normal, and the temporal lobe remains normal in size, though the temporal horn may appear enlarged. RESULTS: None of the patients’ examinations fulfilled all of the HIMAL criteria. Six studies satisfied 2 or more criteria, which included an abnormally rounded hippocampus and a vertical collateral sulcus. These HIMAL findings were all seen on the left. Forniceal asymmetry was the most prevalent abnormality, with 289 patients manifesting a low position of 1 fornix. CONCLUSIONS: Hippocampal malrotation is a rare finding in patients without seizures. HIMAL is therefore likely to be a pathologic finding.

BACKGROUND AND PURPOSE: CT guidance is increasingly being used to localize the epidural space during epidural steroid injections. A common concern is that CT may be associated with significantly higher radiation doses compared with conventional fluoroscopy. The goal of this retrospective study was to determine the average dose-length product and effective dose delivered while interlaminar epidural steroid injections are performed and allow comparison with other modalities. MATERIALS AND METHODS: A total of 281 patients who had undergone 345 consecutive CT-guided epidural steroid injections of the lumbar spine were evaluated for radiation exposure. The dose-length product for each scan was derived from the CT dose index volume and scan length. Effective dose was then calculated from the dose-length product and a κ coefficient of 0.015. Procedure time was calculated from the PACS time stamp on the scout image to the last CT image of the last image series. RESULTS: The average dose-length product across all procedures was 89.6 ± 3.33 mGy·cm, which represents an effective dose of 1.34 ± 0.05 mSv. No complications from the procedure were observed, and average procedure time was 8 minutes. CONCLUSIONS: The use of a stationary table and an intermittent scanning technique allow for short procedures and doses that are significantly lower than those of conventional diagnostic CT scans. Furthermore, because CT dose index overestimates radiation dose in stationary table procedures, the actual radiation dose may be even lower than stated here.

Background and purpose In the past 50 years, fluoroscopic guidance has been used to improve upon lumbar puncture (LP) technique that was unchanged for over a century. Recently, CT has seen increasing use as a guidance modality due to its ability to demonstrate soft tissue contrast and provide millimeter accuracy with needle targeting. This study compared procedure time and radiation dosages for fluoroscopic and CT guided LP. Materials and methods This institutional review board and Health Insurance Portability and Accountability Act (HIPAA) compliant study was a retrospective review of a consecutive cohort referred for image guided LP. For CT, 45 patients aged 49 years (range 20–78, SD 14) with body mass index (BMI) values of 33 kg/m 2 (range 20–50, SD 12) were included. For fluoroscopy, 100 patients aged 47 years (range 18–88, SD 17) with BMI values of 29 kg/m 2 (range 15–56, SD 9) were included. CT procedure time was determined using picture archiving and communication system (PACS) image time stamps. Radiation dose was determined using the CT dose report and effective dose conversion factors. Fluoroscopic procedure time was determined from nursing. Fluoroscopic radiation dose was calculated from dose–area product (DAP) and fluoroscopy times, with effective dosage calculated using simulation software. Results For CT, procedure time average was 14 min (range 5–42, SD 8.5). Average dose–length product was 120 mGy×cm (range 39–211, SD 43) and average effective dose was 1.98 mSv (range 0.2–8.18, SD 4.4). For fluoroscopy, procedure time averaged 12 min (range 12–30, SD 6). Average DAP was 10 Gy×cm 2 (range 0.1–70, SD 11) and effective dose estimate averaged 2.9 mSv (range 0.9–9.4, SD 1.9). There were no unsuccessful taps or complications. Conclusions Both fluoroscopic and CT guidance may be used to perform an LP in an obese population with a short procedure time and low radiation dose.

Absent sella turcica is an extremely rare and dramatic radiographic finding. It may be isolated or occur in the presence of other anomalies, often involving the adenohypophysis. Our evaluation of a female infant with multiple anomalies including absence of the sella turcica, a normal pituitary in the craniopharyngeal canal, normal pituitary function, choanal atresia and anomalies of the appendiceal skeleton prompted a review of the occurrence and biology of an absent sella turcica.

BACKGROUND AND PURPOSE: Cervical steroid injections are a minimally invasive means of providing pain relief to patients with cervical radiculopathy. CT guidance offers many potential advantages. We developed a technique with the patient in the lateral position with a lateral needle trajectory to minimize the required needle depth from skin to target and a near-vertical needle trajectory. The aim of this study was to analyze the cohort for complications, procedural time, and effective radiation dose. MATERIALS AND METHODS: This was a retrospective evaluation of a single-center patient cohort. PACS images from the procedures were reviewed for needle depth, procedural time, and CTDI(vol). An anatomically relevant conversion factor was used to calculate the effective dose. RESULTS: One hundred sixteen cases from 110 patients were identified. The average patient age was 55 years. There were no complications. In 50% of cases, C5–6 was targeted. The average time was 6 minutes, and the average effective radiation dose, 0.51 mSv (0.21–2.56 mSv). Needle-insertion length from the skin to the target was highly correlated with a need for >3 needle repositioning adjustments and scan series (ρ = 0.52, P < .001) and increased procedural time (ρ = 0.42, P < .001). The angle of needle insertion relative to the floor was significantly correlated with an increased number of needle adjustments for depths >25 mm and a longer procedural time (ρ = 0.29, P = .01) but not for depths

To characterize cumulative radiation exposure from diagnostic imaging (CEDI) in pediatric patients and to investigate its relationship to patients' socioeconomic status and comorbid medical conditions.A retrospective cohort study of19,000 pediatric patients seen within the outpatient clinic system of an academic tertiary care urban medical center during the month of January 2006 was conducted to estimate CEDI from all procedures performed within 3 years of the index visit (until January 2009). Socioeconomic status was estimated from census tract geocoding. Comorbid medical conditions were identified from the electronic medical record.A total of 19,063 patients underwent imaging tests within the index month. The mean age was 8.9 ± 6.3 years. Most had private insurance (56%), with 36% receiving Medicaid and 8% private payers. Our population lived in census tracts in which 27 ± 16% of the population were below the federal poverty level with 62% living in areas in which 20% of residents were living below the poverty level. There were differences in CEDI (P.0001) by age, insurance type, and percentage poverty in the census tract of residence but not among racial groups (P = .6508). The association between poverty and CEDI was generally explained by the 26 Elixhauser diagnoses, with the exception of rheumatoid arthritis.Patients living in areas of greater poverty were exposed over time to more radiation from diagnostic testing than those living in areas with lower percentages of residents living in poverty. This association was explained almost entirely by the presence of disease burden. No direct association was found between socioeconomic status and CEDI.

Objective (a) To evaluate the efficacy of doxycycline as a percutaneous sclerotherapy agent in pediatric head and neck macrocystic lymphatic malformations (LM) and (b) to review the literature with regard to recent developments in the treatment of lymphatic malformations using sclerotherapy. Methods We reviewed the medical records and imaging studies of all patients who underwent percutaneous sclerotherapy of macrocystic LM of the head and neck at our institution between June 2005 and May 2010. All studies were reviewed and procedures performed by a single interventional neuroradiologist using computed tomography (CT) guidance. LM were individually cannulated, the contents aspirated, and then injected with doxycycline at concentrations of 10–20 mg/ml. Response to sclerotherapy was determined clinically. Results Seven patients underwent a total of eight sclerotherapy treatments during the study period. Of the six patients with appropriate follow-up, 67% have experienced complete or near-complete clinical resolution of their LM (1243 days mean follow-up), while 33% have developed recurrent swelling after an initial response following a single doxycycline injection (53 days mean follow-up). Conclusions Our institutional results, in combination with recently published findings, support the moderate efficacy and excellent safety profile of percutaneous doxycycline sclerotherapy for macrocystic lymphatic malformations.

Purpose The purpose of this study was to review the use of covered stents in patients with squamous cell carcinoma of the head and neck threatening bilateral neurovascular structures. Methods The radiology information system was searched for all patients with bilateral head and neck carcinoma treated with covered stents in the carotid vasculature from 2006 through 2009. Five patients (one woman) of mean age 60.5 years (range 45–69) were identified. All had carotid blowout syndrome after treatment for primary squamous cell carcinoma of the head and neck with subsequent tumor recurrence or metastases immediately threatening bilateral carotid vasculature. Covered stents were placed. Long-term follow-up included clinical progress, verification of stent patency and detection of tumor progression via ultrasound or contrast-enhanced CT after the first month and then every 3–6 months. All patients were maintained on antiplatelet medication after treatment. Results Covered stents were safely deployed in all patients. Mean survival was 5 months with one outlier surviving for 3 years. There were no subsequent uncontrollable hemorrhages. Conclusion The use of covered stents for avoidance of catastrophic hemorrhage following treatment in patients with head and neck tumors with bilaterally threatened carotid arteries was successful.

Objective To determine if the application of radiofrequency ablation to advanced head and neck cancer (HNC) would result in local control of the tumor. Design Radiofrequency ablation was applied to advanced head and neck malignant tumors in the participants of this nonrandomized controlled trial. Setting Academic tertiary care medical center. Participants Twenty-one participants with recurrent and/or unresectable HNC who failed treatment with surgery, radiation, and/or chemotherapy were selected for the trial. Patients deemed appropriate for curative standard radiation or surgery were not accepted as participants. Intervention Radiofrequency ablation was applied to head and neck tumors under general anesthesia and computed tomographic scan guidance. Main Outcome Measures The primary end point was local control. Computed tomographic scan tumor measurements were used to assess response by standard response evaluation criteria in solid tumors (RECIST) guidelines. Secondary outcome measures included survival and quality of life. Results Eight of 13 participants had stable disease after intervention. Median survival was 127 days, and an improvement in University of Washington quality-of-life scores was noted. Adverse outcomes included 1 death due to carotid hemorrhage and 2 strokes. Conclusion Radiofrequency ablation is a palliative treatment alternative that shows promise in addressing the challenges of local control and quality of life in patients with incurable HNC who have failed standard curative treatment.

Vertebral augmentation procedures have been used as an effective treatment for back pain due to vertebral compression fracture from different causes, including metastatic disease and osteoporosis. However, diagnosis and localization of the causative vertebral level(s) may be difficult using any single imaging modality. In this retrospective study, we assessed the ability of preprocedural bone scans to determine the appropriate level(s) of subsequent vertebroplasty.The study consisted of a retrospective chart review of 44 patients who underwent bone scintigraphs and vertebroplasties at Montefiore Medical Center from 2002 to 2008. Online medical records and reports were used to determine the level of vertebroplasty and the preprocedure bone scan levels. The data was reviewed by a panel consisting of a neuroradiologist and nuclear medicine physician for agreement.Our retrospective study assessed the utility of bone scans as a diagnostic tool to identify candidate vertebral levels for vertebroplasty and determined that bone scans were positive in 78.3% of all fractures that subsequently underwent vertebral augmentation procedures. Although no significant difference was seen in the positivity of bone scans to subsequent vertebroplasty levels between benign or metastatic compression fractures (79% vs. 76%), a difference was observed when single level vertebroplasty patients were compared with multiple-level vertebroplasty patients (87% vs. 69%). Our results support the use of whole body bone scintigraphy as a diagnostic tool prior to vertebroplasty, especially in cases where a single vertebral level is involved or MRI might be contraindicated.

The authors describe a technique for lumbar drain placement using CT guidance. Midline or paramidline interlaminar approaches to the thecal sac can be used. The major advantage to CT guidance is direct visualization of the needle tip in relation to the thecal sac. This technical approach is a safe and rapid alternative to fluoroscopic guidance for the placement of lumbar drains in patients in whom standard lumbar drain placement techniques have failed.

Purpose To evaluate the safety and effectiveness of computed tomography (CT)-guided radiofrequency (RF) ablation in the palliative treatment of recurrent advanced head and neck cancers. Materials and Methods From November 2002 to January 2005, the authors identified 14 patients (median age, 61 years) with 14 recurrent advanced primary head and neck malignancies who underwent 27 CT-guided RF ablation applications during 20 sessions at their institution. RF ablation was performed in all patients with the intent of palliative therapy. Radiologic tumor response was assessed by using Response Evaluation Criteria in Solid Tumors. Patients were assessed clinically by means of University of Washington Head and Neck Quality of Life questionnaires. Results Technical success in tumor targeting and electrode deployment was 100%. University of Washington quality of life surveys completed by six of 14 patients (43%) showed an index increase by a median of 3.1 percentage points, with four of six patients (67%) demonstrating improvement. Three major complications (in 27 applications, 11%) occurred 7 days to 2 weeks after the procedure. These included stroke, carotid blowout leading to death, and threatened carotid blowout with subsequent stroke. Retrospective analysis of intraprocedural CT scans revealed that the retractable electrodes were within 1 cm of the carotid artery during ablation in these cases. Conclusions RF ablation in patients with advanced head and neck malignancies is feasible and effective for palliation. CT-guidance provides accurate probe placement and electrode deployment. The energy level used and proximity of the ablation sphere to the carotid artery may predispose to vascular complications.

The current standard of care for acute ischemic stroke is intravenous recombinant tissue type plasminogen activator (rt-PA) for eligible patients who present within 3–4.5 h from stroke onset.1 ,2 There is firm evidence supporting its efficacy when administered to patients suspected of having acute ischemic stroke and without imaging evidence of intracranial hemorrhage.3 ,4 For patients with major artery occlusions the use of intra-arterial therapy has grown significantly,5 but remains controversial as clear outcome benefits have not been prospectively demonstrated despite higher rates of recanalization compared with intravenous rt-PA.6 ,7 Proper patient selection is paramount in all interventions. An integral component in this effort is identifying clinically proven imaging biomarkers.8 The foundational principle of reperfusion therapy is that timely vessel recanalization will stop the progression of cell apoptosis in the ischemic penumbra and limit final infarct size. For this reason, the focus of ischemic stroke imaging has been the physiologic characterization of the affected brain parenchyma. The ideal patient possesses a major artery occlusion, a small infarct (core), as well as a significant volume of threatened but still viable tissue (penumbra).
