Abstract

To verify whether it is possible, in people with diabetes mellitus (DM) considered at very high cardiovascular (CV) risk, stratify this risk better and identify significant modifiable risk factor (including lifestyle habits) to help patients and clinicians improve CV prevention. People with DM and microvascular diseases or one or more CV risk factors (hypertension, hyperlipidemia, smoking, poor dietary habits, overweight, physical inactivity) included in the Risk and Prevention study were selected. We considered the combined endpoint of non-fatal acute myocardial infarction and stroke and CV death. A multivariate Cox proportional analysis was carried out to identify relevant predictors. We also used the RECPAM method to identify subgroups of patients at higher risk. In our study, the rate of major CV events was lower than expected (5 % in 5 years). Predictors of CV events were age, male, sex, heart failure, previous atherosclerotic disease, atrial fibrillation, insulin treatment, high HbA1c, heart rate and other CV diseases while being physically active was protective. RECPAM analysis indicated that history of atherosclerotic diseases and a low BMI defined worse prognosis (HR 4.51 95 % CI 3.04–6.69). Among subjects with no previous atherosclerotic disease, men with HbA1c more than 8 % were at higher CV risk (HR 2.77; 95 % CI 1.86–4.14) with respect to women. In this population, the rate of major CV events was lower than expected. This prediction model could help clinicians identify people with DM at higher CV risk and support them in achieving goals of physical activity and HbA1c.

4. Variable geometry turbine design for off-highway vehicle organic rankine cycle waste heat recovery

Abstract

Although modern ultra-efficient heavy duty diesel engines exhibit thermal efficiencies of well over 40%, a substantial part of fuel energy will continue being wasted as heat in the exhaust system due to the Diesel cycle limitations. Recovering this potential source of energy could increase the over-all thermal efficiency of the engine as well as reduce the exhaust gas emissions and the operational cost of the heavy duty diesel engine. Organic Rankine Cycle (ORC) is regarded as a promising candi-date technology for transforming exhaust gas waste heat into electricity (or direct power) due to the nature of the untapped source of energy that it allows to be capitalised on, its relative simplicity and small back pressure impact on engine performance and fuel consumption. This study was carried out for an off-highway engine project but its application is generic to heavy duty applications. An ORC model with a radial expander submodel is implemented in a heavy duty diesel engine powertrain, to evaluate the impact of the ORC on fuel consumption and exhaust gas emissions under various engine operating conditions. Finally, the potential benefit of utilizing a variable geometry expander (VGE) in the ORC model is investigated in this study. Compared to its fixed geometry expander equivalent, the VGE shows a wider range of high efficiency operation within the engine operating window and is therefore a promising addition for consideration in ORC application of this type.

Abstract

Waste energy recovery (WER) is a suitable solution to improve the fuel utilization of Internal Combustion Engines (ICEs) by producing an eco-friendly electrical power from an energy source currently wasted. Organic Rankine Cycle (ORC) technology has been developed in the past few years to generate electric power from medium temperature (500 K – 800 K) ICE wasted thermal sources. Working fluid selection represents the first step in the design of an ORC. At the state of the art, authors where not able to select a single optimal organic fluid. This is mainly because of the different thermodynamic conditions of the heat sources which offer wasted thermal energy. This paper proposes a procedure for the ORC system preliminary working fluid selection, which takes into consideration thermodynamics and design parameters of the system components. The study is applied to WER systems specifically designed as bottoming cycles to ICE for transport applications. However, the method is quite general and makes the model easily adaptable to different heat sources. A steady state thermodynamic model of the system is developed via the software MATLAB. A wide variety of organic fluids (OF), such as R245fa, Solkatherm (SES36) and hexane have been investigated to identify the candidate which offers the best recovery opportunity. Regeneration is also included in this work. Results show that recover thermal energy in the regenerator is an essential method to improve power recovery when applying ORC to WER systems. The effect of superheating on the system power output has been investigated as well. It is capable to increase the cycle power output only when coupled with regeneration. The paper shows that the addition of a bottoming ORC to the ICE is convenient both in terms of recovered electric power (up to 14% of the engine nameplate power) and heat source utilization rate (up to 11 % heat source conversion into electricity). In addition, it is shown that water offers lower performance with respect to organic fluids when considering single stage radial expanders.

Abstract

Background Trials have shown a beneficial effect of n-3 polyunsaturated fatty acids in patients with a previous myocardial infarction or heart failure. We evaluated the potential benefit of such therapy in patients with multiple cardiovascular risk factors or atherosclerotic vascular disease who had not had a myocardial infarction. Methods In this double-blind, placebo-controlled clinical trial, we enrolled a cohort of patients who were followed by a network of 860 general practitioners in Italy. Eligible patients were men and women with multiple cardiovascular risk factors or atherosclerotic vascular disease but not myocardial infarction. Patients were randomly assigned to n-3 fatty acids (1 g daily) or placebo (olive oil). The initially specified primary end point was the cumulative rate of death, nonfatal myocardial infarction, and nonfatal stroke. At 1 year, after the event rate was found to be lower than anticipated, the primary end point was revised as time to death from cardiovascular causes or admission to the hospital for cardiovascular causes. Results Of the 12,513 patients enrolled, 6244 were randomly assigned to n-3 fatty acids and 6269 to placebo. With a median of 5 years of follow-up, the primary end point occurred in 1478 of 12,505 patients included in the analysis (11.8%), of whom 733 of 6239 (11.7%) had received n-3 fatty acids and 745 of 6266 (11.9%) had received placebo (adjusted hazard ratio with n-3 fatty acids, 0.97; 95% confidence interval, 0.88 to 1.08; P=0.58). The same null results were observed for all the secondary end points. Conclusions In a large general-practice cohort of patients with multiple cardiovascular risk factors, daily treatment with n-3 fatty acids did not reduce cardiovascular mortality and morbidity. Abstract

A feasibility model of an all-electronic bubble memory system has been constructed. It uses a small 60k bit bubble recorder consisting of 6 chips of 10k bits each mounted in three separate packages operating as a FIFO at a 150 KHz bubble data rate. In addition to serving as a direct tape recorder replacement, the bubble recorder can be programmed for random access to each individual chip for ranom block access operation or for self-checking or by-passing any malfunctioning memory chip. Read and write operations can be performed asynchronously from very low frequency up to basic recording field frequency. A large 50M bit prototype is planned.

The dependence of the photometric width of a track on the degree of development has been investigated in G-5 nuclear plates, developed by the standard procedure. It is found that the width varies with the development only through an additive term, which is independent of the nature and velocity of the particle which produced the track. Abstract

The objective of this basic research effort is to demonstrate the feasibility of a new technology for all solid state large data base memories for airborne/spaceborne applications. The program scope is limited to demonstrating the feasibility of the device functions and bit density capabilities required to meet the system goals. It is the objective of this program to advance one technology into development. Based on an evaluation of the present and potential capabilities of candidate solid state technologies to meet the system goals, magnetic bubble domain technology has been selected as the one to be pursued on this program. The proposed approach makes use of small bubble materials, multilayer, self-biased materials and devices, wafer level integration using hybrid chip designs and fineline lithographic techniques for device fabrication. The effort on small bubble materials research was devoted to: (1) extending the useful temperature range of the 2 micrometers and 1 micrometer bubble diameter materials which were developed in the previous report internal, (2) investigating film growth on 76 mm dia substrates, and (3) demonstrating the capabilities of the materials by device operation. With the successful accomplishment of the above three tasks the small bubbles portion of the materials research effort has been concluded. Materials research has concentrated on the means for improving the performances of self-biased bubble films and on an alternate to CaGe substitution in bubble films.

Abstract

This report presents the results of a system and cost study made to determine the feasibility of developing a bubble domain BORAM chip using existing device components. Due to the short access time requirements the major-minor loop chip organization cannot be used and due to the lack of a decoder of the necessary type with adequate margins, an alternative chip scheme is proposed in which parallel access to storage loops is achieved via separate transfer switches. The feasibility of realizing such a chip is examined from the viewpoint and preliminary designs are proposed. Estimates of the system costs and optimum chip capacity are made based upon a simple yield model for bubble devices. (Author)

Abstract

This report describes the progress made during the first year of this program and covers two major areas of investigation. The first one is a study of the temperature stress/anneal effects on the permalloy thin films that are used for the bubble propagation and detection elements. It has been found that the magnetic, electrical, and physical properties of the film vary significantly with temperature stress/anneal and deposition parameters. It is an objective of this study to optimize the film deposition parameters and to be able to predict the failure modes for bubble devices by providing knowledge of the magnetic behavior of the thin films to accelerated thermal stress. The second major area of investigation is the development of bias field temperature compensating techniques using temperature dependent magnets. A review of bubble memory package concepts and techniques is given which covers many packaging areas that are being studied by Rockwell International. A comparison of several thick and thin chevron bubble sensors is also made. Thin detectors have the highest sensitivity per bubble stretch; however, the detector current is limited by Joule heating. Of the thick types the full conductor shorted chevron was found to have the highest sensitivity. For two active bubble sensors in a bridge detecting 180 deg apart half conductor shorted chevron detectors are used which have slightly smaller sensitivities. Except for the detector comparison no device development was performed on this program. Also no effort was made to develop or improve bubble materials for device applications.

Abstract

Clinically relevant hypocalcemia is a well-documented complication of glucocorticoid administration in people with hypoparathyroidism. The current report describes the phenomenon in a dog. A 7 yr old neutered male Pomeranian was diagnosed with immune-mediated thrombocytopenia, immune-mediated hemolytic anemia, and primary hypoparathyroidism. This dog required long-term parenteral calcium gluconate to prevent clinical hypocalcemia despite appropriate doses of oral calcitriol and calcium carbonate. This is the first description of clinically significant presumptive glucocorticoid induced hypocalcemia in a dog with primary hypoparathyroidism.

Importance: Hearing impairment is common in adults, but few studies have addressed it in the US Hispanic/Latino population., Objective: To determine the prevalence of hearing impairment among US Hispanic/Latino adults of diverse backgrounds and determine associations with potential risk factors., Design, Setting, and Participants: The Hispanic Community Health Study/Study of Latinos (HCHS/SOL) is a population-based sample of Hispanics/Latinos in four US communities (Bronx, New York; Chicago, Illinois; Miami, Florida; and San Diego, California). Examinations were conducted from 2008 through 2011. The HCHS/SOL examined 16,415 self-identified Hispanic/Latino persons aged 18 to 74 years recruited from randomly selected households using a stratified 2-stage area probability sample design based on census block groups and households within block groups., Main Outcomes and Measures: Hearing thresholds were measured by pure-tone audiometry. Hearing impairment was defined as a pure-tone average (PTA) of thresholds at 0.5, 1, 2, and 4 kHz greater than 25 dB hearing level. Bilateral hearing impairment required a PTA greater than 25 dB hearing level in both ears. Multivariable analyses included adjustments for sociodemographic and lifestyle variables, body mass index, and medical conditions., Results: The prevalence of hearing impairment was 15.06% (SE, 0.44%) overall, and 8.24% (SE, 0.33%) had bilateral hearing impairment. The prevalence of hearing impairment was higher among people 45 years and older, ranging by Hispanic/Latino background from 29.35% to 41.20% among men and 17.89% to 32.11% among women. The multivariable-adjusted odds of hearing impairment was greater for participants of Puerto Rican background compared with Mexican background (odds ratio [OR], 1.57 [95% CI, 1.10-2.25]). The odds of hearing impairment were lower with more education (OR, 0.71 [95% CI, 0.59-0.86] for at least high school) and higher income (OR, 0.58 [95% CI, 0.36-0.92] for >$75,000 vs ≤$10,000). Noise exposure (OR, 1.35 [95% CI, 1.07-1.70]), diabetes (OR, 1.57 [95% CI, 1.27-1.94]), and prediabetes (OR, 1.37 [95% CI, 1.12-1.67]) were associated with hearing impairment., Conclusions and Relevance: Hearing impairment is a common problem for older Hispanics/Latinos in these communities and is associated with socioeconomic factors, noise exposure, and abnormal glucose metabolism. Longitudinal studies are needed to determine whether these factors are involved in the etiology of hearing impairment and to identify ways to prevent or delay age-related changes in hearing.

39. Cyclic fatigue resistance of 3 different nickel-titanium reciprocating instruments in artificial canals.

Abstract

Introduction: The purpose of this study was to evaluate the cyclic fatigue resistance of 3 different nickel-titanium reciprocating instruments., Methods: A total of 45 nickel-titanium instruments were tested and divided into 3 experimental groups (n = 15): group 1, WaveOne Primary instruments; group 2, Reciproc R25 instruments; and group 3, Twisted File (TF) Adaptive M-L1 instruments. The instruments were then subjected to cyclic fatigue test on a static model consisting of a metal block with a simulated canal with 60° angle of curvature and a 5-mm radius of curvature. WaveOne Primary, Reciproc R25, and TF Adaptive instruments were activated by using their proprietary movements, WaveOne ALL, Reciproc ALL, and TF Adaptive, respectively. All instruments were activated until fracture occurred, and the time to fracture was recorded visually for each file with a 1/100-second chronometer. Mean number of cycles to failure and standard deviations were calculated for each group, and data were statistically analyzed (P < .05). Instruments were also observed through scanning electron microscopy to evaluate type of fracture., Results: Cyclic fatigue resistance of Reciproc R25 and TF Adaptive M-L1 was significantly higher than that of WaveOne Primary (P = .009 and P = .002, respectively). The results showed no statistically significant difference between TF Adaptive M-L1 and Reciproc R25 (P = .686). Analysis of the fractured portion under scanning electron microscopy indicated that all instruments showed morphologic characteristics of ductile fracture that were due to accumulation of metal fatigue., Conclusions: No statistically significant differences were found between the instruments tested except for WaveOne Primary, which showed the lowest resistance to cyclic fatigue., (Copyright © 2015 American Association of Endodontists. 40. Cutting efficiency of instruments with different movements: a comparative study.

Abstract

Objectives: The aim of the present study was to evaluate the cutting efficiency of two new reciprocating instruments, Twisted File Adaptive and WaveOne Primary., Material and Methods: 10 new Twisted File Adaptive (TF Adaptive) (SybronEndo, Glendora, CA, USA) and 10 new WaveOne Primary files (Dentsply Maillefer, Ballaigues, Switzerland) were activated using a torque-controlled motor, respectively TFA motor (SybronEndo, Glendora, CA, USA) and Silver motor (VDW, Munich, Germany). The device used for the cutting test consisted on a mainframe to which a mobile plastic support for the hand-piece is connected and a stainless-steel block containing a Plexiglas block against which the cutting efficiency of the instruments was tested. The length of the block cut in 1 minute was measured in a computerized program with a precision of 0.1 mm. Mean and standard deviations of each group were calculated and data were statistically analyzed with one-way ANOVA and Bonferroni t test (P < 0.05)., Results: TF Adaptive displayed significantly greater maximum penetration depth than WaveOne Primary (P < 0.05). In fact, TF Adaptive instruments (Group 1) cut the Plexiglas block to a mean depth of 8.7 (SD 0.5) mm, while WaveOne Primary instruments cut the Plexiglas block to a mean depth of 6.4 (SD 0.3) mm., Conclusions: Twisted File Adaptive instruments demonstrated statistically higher cutting efficiency than WaveOne instruments.

Abstract

Introduction: The purpose of this study was to present a new periapical and endodontic status scale (PESS) that is based on the complex periapical index (COPI), which was designed for the identification and classification of periapical bone lesions in cases of apical periodontitis, and the endodontically treated tooth index, which was designed for endodontic treatment quality evaluation by means of cone-beam computed tomographic (CBCT) analysis., Methods: Periapical and endodontic status parameters were selected from the already known indexes and scientific literature for radiologic evaluation. Radiographic images (CBCT imaging, digital orthopantomography [DOR], and digital periapical radiography) from 55 patients were analyzed. All parameters were evaluated on CBCT, DOR, and digital periapical radiographic images by 2 external observers. The statistical analysis was performed with software SPSS version 19.0 (SPSS Inc, Chicago, IL). Chi-square tests were used to compare frequencies of qualitative variables. The level of significance was set at P ≤ .05., Results: Overall intraobserver and interobserver agreements were very good and good, respectively. CBCT analysis found more lesions and lesions of bigger dimension (P < .001). CBCT imaging was also superior in locating lesions in the apical part on the side compared with DOR and in the diagnosis of cortical bone destruction compared with both methods (P < .001). Through CBCT analysis, more root canals and more canals associated with lesions were found. The most informative and reproducible periapical and endodontic status parameters were selected, and a new PESS was proposed., Conclusions: The classification proposed in the present study seems to be reproducible and objective and adds helpful information with respect to the existing indexes. 42. Influence of Different Apical Preparations on Root Canal Cleanliness in Human Molars: a SEM Study.

Abstract

Objectives: To compare the influence of type and dimensions of the apical preparation on the cleanliness of the apical area in molars., Material and Methods: A total of 120 root canals (MB and DB root canals from 30 maxillary molars and mesial root canals from 30 mandibular molars) were instrumented with Mtwo NiTi rotary instruments to a size 25/0.06 taper and were equally divided into three different experimental groups depending on the subsequently apical root canal preparation: Group 1: no further apical preparation, Group 2: apical preparation with Mtwo files to a size 40/0.04 taper, Group 3: apical preparation with Mtwo Apical Files. All root canals were observed through scanning electron microscopy (SEM). Presence of superficial debris and smear layer was evaluated using a score system. Data were statistically analysed using the Kruskal-Wallis and Bonferroni tests with a level of significance set at P < 0.05., Results: Kruskal-Wallis test revealed no differences among groups in the middle and coronal third (P > 0.05), while at the apical level, there was a significant difference for both residual debris and presence of smear layer between Group 1 and both Group 2 (P = 0.003 and P = 0.014) and 3 (P = 0.012 and P = 0.021), while no difference was present between Group 2 and Group 3 (P = 0.871 and P = 0.923)., Conclusions: Cleanliness of the apical third in terms of debris and smear layer was statistically better when an apical preparation was performed to a size 40/0.04 taper or with the use of the Mtwo Apical Files.

Abstract

Introduction: The aim of the present study was to use cone-beam computed tomography (CBCT) to analyze root canal anatomy and symmetry of maxillary and mandibular first and second molar teeth of a white population., Methods: A total of 201 patients who required CBCT examinations as part of their dental diagnosis and treatment were enrolled in the present study. Overall, 596 healthy, untreated, well-developed maxillary and mandibular molar teeth (161 maxillary first molars, 157 maxillary second molars, 117 mandibular first molars, and 161 mandibular second molars) were examined by CBCT to establish the symmetry in root and canal anatomy between right and left sides in the same patient by evaluating the number of roots and root canals and the root canal configuration., Results: Three separate roots with 3 separate canals was the normal anatomy of maxillary first and second molars. Most mandibular first and second molars had 2 separate roots, and the majority had 3 canals. In the present study, first molars, both maxillary and mandibular, exhibited greater asymmetry than the second molars. Maxillary first molars were found to be symmetrical in 71.1% of patients, whereas maxillary second molars were symmetrical in 79.6%. The remaining 28.9% and 20.4% of patients, respectively, showed asymmetry. Around 30% of the mandibular first molars and 20% of the mandibular second molars showed asymmetry., Conclusions: The results of the present study reported a percentage of symmetry that varied from 70%-81%. These variations in symmetry should be taken in high consideration when treating 2 opposite molars in the same patient, because their anatomy may be different in up to 30% of the cases., (Copyright © 2013 American Association of Endodontists.
