Abstract

Background: To identify indicators of guideline-concordant care in lung cancer, to implement such indicators with cancer registry data linked to health databases, and to pilot them in a cohort of patients from the cancer registry of the Milan Province. Methods: Thirty-four indicators were selected by revision of main guidelines by cancer epidemiologists, and then evaluated by a multidisciplinary panel of clinicians involved in lung cancer care and working on the pathway of lung cancer diagnosis and treatment in the Lombardy region, Italy. With a modified Delphi method, they assessed for each indicator the content validity as a quality measure of the care pathway, the degree of modifiability from the health professional, and the relevance to the health professional. Feasibility was assessed using the cancer registry and the routine health records of the Lombardy region. Feasible indicators were then calculated in the cohort of lung cancer patients diagnosed in 2007–2012 derived from the cancer registry of the Milan Province. Criterion validity was assessed reviewing clinical records of a random sample of 114 patients (threshold for acceptable discordance ≤20%). Finally, reliability was evaluated at the provider level. Results: Initially, 34 indicators were proposed for evaluation in the first Delphi round. Of the finally 22 selected indicators, 3 were not feasible because the required information was actually not available. The remaining 19 were calculated on the pilot cohort. After assessment of criterion validity (3 eliminated), 16 indicators were retained in the final set and evaluated for reliability. Conclusion: The developed and piloted set of indicators is now available to implement and monitor, over time, quality initiatives for lung cancer care in the studied health system.

Abstract

Increased lopinavir (LPV) exposure obtained in vivo through combination with low-dose ritonavir may overcome a certain grade of resistance but not all. We sought to analyze LPV variability and possible risk factors. LPV trough plasma concentrations were determined by high-performance liquid chromatography after 1, 4, and 12 weeks from salvage regimens and tested in both univariate and multivariate regression analyses with age, gender, weight, risk factors for HIV acquisition, hepatitis C virus reactivity, hepatitis B surface antigen positivity, baseline aspartate transferase (AST) or alanine transferase (ALT) levels, creatinine, non-nucleoside reverse transcriptase inhibitors (NNRTIs) or tenofovir as concomitant drugs, and NNRTIs administered in the previous regimen. Fifty-six patients were included into the study. Among them, 8 of 56 (14.3%) at week 1, 12 of 56 (21.4%) at week 4, and 9 of 56 (16.1%) at week 12 had suboptimal LPV plasma concentrations, defined as trough concentration less than 4 [micro]g/mL. No correlation was found between LPV trough concentrations and assessed variables. In conclusion, pharmacokinetic variability and low LPV concentrations have been found, supporting the use of therapeutic drug monitoring in those starting this drug.

Abstract

In the present article, new records are given for 18 species (6 native, 9 alien and 3 cryptogenic), belonging to 5 Phyla (i.e. Chlorophyta, Cnidaria, Annelida, Mollusca and Chordata), from 8 Mediterranean countries: Spain: A large population of Polycerella emertoni reported from the Ebro Delta, the presence of Spinimuricea klavereni is confirmed for the first time in the Catalan coast; Italy: the new record of Cryptonome turcica extends its distribution to the western Mediterranean Sea, the findings of Mawia benovici constitute the southernmost record of this species in the Adriatic Sea; Slovenia: second record of the alien seaslug Cuthona perca in the Mediterranean Sea; Montenegro: Styela plicata communities were recorded in Boka Kotorska Bay; Albania: the native great torpedo ray Tetronarce nobiliana recorded for the first time in Albanian waters; Greece: first record of Lagocephalus sceleratus in the Greek side of the Adriatic, the records of Cassiopea andromeda and Pterois miles are first records of these species from the Ionian Sea, the colonial ascidians Aplidium coeruleum and Didemnum protectum are recorded for the first time in the Eastern Mediterranean, an extremely abundant population of Melibe viridis is found in Messolonghi lagoon, the record of Synchiropus sechellensis is the range expansion of this species in Greek Seas, Acetabularia caliculus is recorded for the first time from Greece; Cyprus: first records of Callionymus filamentosus and Haminoea cyanomarginata from the island; Turkey: the presence of Berthellina citrina in the Mediterranean is confirmed. © 2018 Mediterranean Marine Science.

Abstract

A first synoptic and trans-domain overviewof plankton dynamics was conducted across the aquatic sites belonging to the Italian Long-Term Ecological Research Network (LTER-Italy). Based on published studies, checked and complemented with unpublished information, we investigated phytoplankton and zooplankton annual dynamics and long-term changes across domains: from the large subalpine lakes to mountain lakes and artificial lakes, from lagoons to marine coastal ecosystems. This study permitted identifying common and unique environmental drivers and ecological functional processes controlling seasonal and long-termtemporal course. The most relevant patterns of plankton seasonal succession were revealed, showing that the driving factors were nutrient availability, stratification regime, and freshwater inflow. Phytoplankton and mesozooplankton displayed a wide interannual variability at most sites. Unidirectional or linear long-term trends were rarely detected but all sites were impacted across the years by at least one, but in many case several major stressor(s): nutrient inputs, meteo-climatic variability at the local and regional scale, and direct human activities at specific sites. Different climatic and anthropic forcings frequently co-occurred, whereby the responses of plankton communities were the result of this environmental complexity. Overall, the LTER investigations are providing an unparalleled framework of knowledge to evaluate changes in the aquatic pelagic systems and management options

Abstract

Background: Clear cell RCC (ccRCC) accounts for approximately 75% of the renal cancer cases. Surgery treatment seems to be the best efficacious approach for the majority of patients. However, a consistent fraction (30%) of cases progress after surgery with curative intent. It is currently largely debated the use of adjuvant therapy for high-risk patients and the clinical and molecular parameters for stratifying beneficiary categories. In addition, the treatment of advanced forms lacks reliable driver biomarkers for the appropriated therapeutic choice. Thus, renal cancer patient management urges predictive molecular indicators and models for therapy-decision making. Methods: Here, we developed and optimized new models and tools for ameliorating renal cancer patient management. We isolated from fresh tumor specimens heterogeneous multi-clonal populations showing epithelial and mesenchymal characteristics coupled to stem cell phenotype. These cells retained long lasting-tumor-propagating capacity provided a therapy monitoring approach in vitro and in vivo while being able to form parental tumors when orthotopically injected and serially transplanted in immunocompromised murine hosts. Results: In line with recent evidence of multiclonal cancer composition, we optimized in vitro cultures enriched of multiple tumor-propagating populations. Orthotopic xenograft masses recapitulated morphology, grading and malignancy of parental cancers. High-grade but not the low-grade neoplasias, resulted in efficient serial transplantation in mice. Engraftment capacity paralleled grading and recurrence frequency advocating for a prognostic value of our developed model system. Therefore, in search of novel molecular indicators for therapy decision-making, we used Reverse-Phase Protein Arrays (RPPA) to analyze a panel of total and phosphorylated proteins in the isolated populations. Abstract

This study provides new and additional data on morphology and a phylogenetic analysis of the recently described species Pelagia benovici Piraino, Aglieri, Scorrano & Boero, 2014 from the Northern Adriatic (Mediterranean Sea). Comprehensive morphological analyses of diagnostic characters, of which the most significant are marginal tentacles anatomy, basal pillars, gonad pattern, subgenital ostia and exumbrellar sensory pits, revealed significant differences from the currently known genera Sanderia, Chrysaora and Pelagia in the family Pelagiidae. A phylogenetic analysis of mitochondrial genes (COI, 16S rRNA, 12S rRNA) and nuclear ribosomal genes (28S rRNA, ITS1/ITS2 regions), together with cladistic analysis of morphological characters, positioned Pelagia benovici as a sister taxon with Sanderia malayensis, and both share a common ancestor with Chrysaora hysoscella. Pelagia benovici does not share a direct common ancestor with the genus Pelagia, and thus we propose it should not belong to this genus. Therefore, a new genus Mawia, gen. nov. (Semaeostomeae : Pelagiidae) is described, and Pelagia benovici is renamed as Mawia benovici, comb, nov.

Abstract

It is still controversial whether viral hepatitis co-infection can influence antiretroviral plasma drug concentrations and whether drug concentrations are correlated with liver enzyme elevations during highly active antiretroviral therapy. An analysis of data from a cohort of 220 human immunodeficiency virus (HIV)-infected patients was conducted. Univariate and multivariate logistic analyses were performed to identify predictors of plasma drug concentrations. The association of transaminase elevation with higher plasma drug concentrations was explored following stratification of patients into HIV monoinfected and hepatitis C virus (HCV) and/or hepatitis B virus (HBV) co-infected groups. Hepatitis co-infections were independently correlated with drug concentrations above the therapeutic cut-offs at Week 1 (P = 0.06), Week 4 (P = 0.04) and Week 12 (P = 0.005). The apparent effect was independent of the possible impact exerted by other variables such as demographics and medication adherence. The incidence of relevant hypertransaminasaemia was low. Patients with hepatitis co-infections had higher rates of transaminase elevation than monoinfected HIV patients; however, risk of transaminase elevation was not associated with drug concentrations. Abstract

Data from 197 patients for whom highly active antiretroviral therapy (HAART) failed, who started a new regimen chosen under the guide of resistance testing results interpreted by experts, were retrospectively studied, provided that at least 2 determinations of adherence and plasma drug concentrations were performed during the follow-up. Univariate and multivariable logistic regression analyses were conducted, using confirmed virologie response at week 24 as outcome measure (i.e., achievement of undetectable HIV plasma viral load at any time point before week 24 and its maintenance up to week 24). Suboptimal drug concentrations (odds ratio [OR]: 0.3; 95% confidence interval [CI] 0.2-0.7; p = 0.006) and suboptimal adherence (OR: 0.4; 95% CI 0.2-0.8; p = 0.014) were both negative independent predictors of sustained virologic response, while the use of boosted protease inhibitor-containing regimens resulted to be protective (OR: 2.4; 95% CI 1.1-5.3; p = 0.032). © Mary Ann Liebert, Inc.

Abstract

The understanding of the role of the pteropods Limacina helicina in the ecosystem has become of greater interest as the debate on ocean acidification and its consequences for calcifying organisms has increased. Four incubation experiments were carried out in January and February 2006 in Terra Nova Bay Polynya (Ross Sea) to identify the faecal pellets (FPs) produced by L. helicina. Mean FP production rates were 6.1+ 1.3 and 10.9+2.1 pellets day21 individual21 in January and February, respectively. FPs produced by L. helicina had an oval shape with a more lengthened side. The identification of L. helicina faeces allowed us to quantify the amounts of L. helicina FPs in the material collected by sediment traps deployed in the same area from 1998 to 2001. We found that L. helicina FPs flux ranged from 71 103 FP m22 year21 to 362 103 FP m22 year21 and reach maximum values in March–April every year. The FPs flux of this organism contributed 19% of the particle organic carbon flux. The carbon pump may be modified if the L. helicina population declines as a consequence of the predicted acidification in polar and subpolar waters.
