Cardiac arrest is the least preventable burden of cardiovascular disease, as treatment depends on timely resuscitation. The incidence of sudden cardiac arrest (SCA) is high, contributing 10-20% of cardiovascular mortality globally. The influenza vaccine reduces the risk of acute cardiovascular events. Little is known about the relationship of influenza infection to cardiac arrest.This study aimed to determine the estimated rate of SCA hospitalisations attributable to influenza in Australian adults. A generalised-additive statistical model was applied in the study. Weekly counts of laboratory-confirmed influenza notifications were used as independent variables in the model.Our estimates showed that the yearly rate of SCA hospitalisations varied, and a significant association with influenza was observed in some years in older adults aged 65 years and over. On average, the annual estimated SCA hospitalisations rate due to influenza in adults aged 50-64 years and ≥ 65 years were 0.7 (95%CI: 0.4, 1.1) and 5.3 (95%CI: 4.4, 6.2) per 100,000 population, respectively.The association between influenza and SCA is evident in adults and the disease burden is significant in older people. Prevention of influenza by vaccination may reduce SCA.

Coronavirus disease 2019 (COVID-19) induced by the severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) has impacted the lives and wellbeing of many people. This globally widespread disease poses a significant public health concern that urges to discover an effective treatment. This review paper discusses the effectiveness of repurposed drugs used to treat COVID-19 and potential novel therapies for COVID-19. Among the various repurposed drugs, remdesivir is the only agent approved by the Food and Drug Administration (FDA) to treat COVID-19. On the other hand, several drugs have been listed in the Emergency Use Authorization (EUA) by the FDA to treat COVID-19, including casirivimab and imdevimab, baricitinib (in combination with remdesivir), bamlanivimab, tocilizumab, and IL-6 inhibitors. In addition, in vitro and clinical studies have suggested cepharanthine, sotrovimab, and XAV-19 as potential treatments to manage COVID-19. Due to inadequate understanding of COVID-19 and the rapid mutation of SARS- CoV-2, COVID-19 remains a threat to global public health, with vaccination considered the most effective method to decrease COVID-19 transmission currently. Nevertheless, with the intense efforts of clinical researchers globally, more promising treatments for COVID-19 will be established in the future.&nbsp

Cardiovascular diseases are the major cause of hospitalisation and death globally. Infections exacerbate cardiovascular events among cardiac patients, contributing to all-cause mortality. Vaccination is a cheap and effective intervention that can prevent infection. In Australia, influenza, pneumococcal and herpes zoster vaccines are recommended and funded for high-risk adults such as cardiac patients. There is high prevalence of high-risk adults in Western Sydney.This study investigates the uptake of influenza, pneumococcal and herpes zoster vaccines in patients admitted with heart failure and atrial fibrillation in a tertiary hospital in Western Sydney and factors associated with the uptake of the vaccines.Consecutive patients' hospitalised between 2014 and 2018 with heart failure or atrial fibrillation as principal diagnoses were identified. Information on patients' social demographic, clinical and vaccination status was collected and described using descriptive analysis. Univariate and multivariate analyses were conducted to determine factors associated with the uptake of the vaccines.Low uptake for pneumococcal (40-45 %) and herpes zoster (15 %) vaccines were found. Prevalence of influenza vaccination was lower among participants younger than 65 (51-72 %) than in older ones (78-96 %). Australia-born participants were more likely to receive pneumococcal vaccine than those born overseas (OR 2.02, 95 % CI 1.05-3.89). Participants 65 years or older and those with comorbidities such as hypertension, COPD and chronic renal impairment were more likely to receive the vaccines.Multidisciplinary strategies are needed to improve access to vaccination, community knowledge, community engagement, and healthcare provider support to provide appropriate care to migrants and younger cardiac patients and reduce morbidity and mortality in this high-risk group.

Background New York City (NYC) emergency departments (EDs) experienced a surge of patients because of coronavirus disease 2019 (COVID‐19) in March 2020. NYC Health and Hospitals established rapid medical screening exams (MSE) and each hospital designated areas to perform their MSE. Five of the 11 hospitals created a forward treatment area (FTA) external to the ED to disposition patients before entering who presented with COVID‐like symptoms. Three hospitals used paper‐based, and 2 used an electronic medical record (EMR)‐based MSE. This study evaluated the effectiveness of safely discharging patients home from the FTA while also evaluating the efficiency of using paper‐based versus EMR‐based MSEs. Methods Charts were reviewed using standardized data extraction templates. Patients discharged from the FTA were contacted by phone, and a structured interview captured additional data regarding subsequent clinical courses. Chi‐square tests were used to compare proportions of patients hospitalized, as well as proportions of patients with vital signs recorded. Mortality rates were compared with Fisher exact test. A logistic regression model with fixed effects to account for clustering at hospitals was used to compare the odds of being sent to the ED for further evaluation based on vital signs and adjusted for age and sex Results Across 5 EDs, 3335 patients were evaluated in their FTAs from March 17, 2020, to April 27, 2020. A total of 970 (29.1%) patients were referred for further evaluation into the ED, of which 203 (20.9%) were hospitalized and 19 (2.0%) died. Of 2302 patients discharged from the FTA, 182 (7.9%) returned to the ED within 7 days, resulting in 42 (1.8%) hospitalizations and 7 (0.3%) deaths. Theory and empirical findings predict that individuals in a negative affective state are more sensitive to unexpected reward loss and less sensitive to unexpected reward gain compared to individuals in a neutral or positive affective state. We explore the use of sensitivity to reward shifts measured during successive contrast tasks as an indicator of affect in zebrafish (Danio rerio). In line with the assumption that exposure to rewarding stimuli induces a relatively positive affective state compared to exposure to stimuli that they do not prefer, we confirmed that zebrafish prefer enriched over barren environments, suggesting that the enriched environment is associated with positive affective states. We trained individuals to swim down a channel for food rewards of differing value and then presented them with unexpected increases or decreases in reward value. Contrary to our hypothesis, individuals conditioned to a high-value reward continued swimming at the same speed when reward value was downshifted, thus showing no successive negative contrast effect and appearing insensitive to reward loss. Individuals whose rewards were upshifted gradually increased their speed, but did not display successive positive contrast effects typical of sensitivity to reward gains. In both cases, housing type did not result in differences in swim time. One potential explanation is that goal-directed control of behaviour is necessary for an animal to show a successive contrast response to unexpected reward gain or loss, and the behaviour of zebrafish in this task was under habitual control, perhaps due to over-training. If so, refinements to task design and training procedures will allow further progress with this assay.

A1 is an in-memory distributed database used by the Bing search engine to support complex queries over structured data. The key enablers for A1 are availability of cheap DRAM and high speed RDMA (Remote Direct Memory Access) networking in commodity hardware. A1 uses FaRM [11,12] as its underlying storage layer and builds the graph abstraction and query engine on top. The combination of in-memory storage and RDMA access requires rethinking how data is allocated, organized and queried in a large distributed system. A single A1 cluster can store tens of billions of vertices and edges and support a throughput of 350+ million of vertex reads per second with end to end query latency in single digit milliseconds. In this paper we describe the A1 data model, RDMA optimized data structures and query execution.

Background: Abnormal patellar height is associated with anterior knee pain and several conditions that affect the patellofemoral joint. The aim of this study was to 1) report the incidence of patella alta and patella baja and 2) investigate whether the normal limits of the Insall-Salvati ratio is applicable in adult Middle-Easterners. Methods: A radiographic review of the lateral radiographs of 736 Middle-Eastern knees were performed. Patellar tendon length (TL) and the patellar length (TP) was digitally measured and the ratios of these measures was used to calculate the Insall-Salvati ratio. Results: The overall mean TL/PL ratio was 1.20±0.17. The Insall-Salvati ratio was higher (p=0.0013) in males (1.22± 0.12) than in females (1.18±0.17). According to our measurement, the recommended levels for defining abnormal patellar position should be 0.86 for patella baja and 1.54 for patella alta. Conclusion: The use of TL/PL ratio demonstrated a higher incidence of patella alta and a higher mean TL/PL ratio compared to other techniques. The normal ranges for the TL/PL differs from western populations and may be attributed to lifestyle differences.

OBJECTIVE: The objective of the present study was to describe the implementation of an organizational learning model and evaluate the effectiveness and usability of an application used to facilitate it in an ED setting. METHODS: This was an implementation case study and technology evaluation. The organizational learning model was implemented using an online Web 2.0 collaborative learning application developed by the investigating team. Online use was tracked over a 9-month period. At the end of the study period, a usability assessment was conducted as well as a semistructured interview of participants to assess perceptions of usefulness and effect on learning capacity in the ED. RESULTS: Over a period of 9 months, a total of 54 individual sites from 74 eligible staff members were created within a specific web domain. There were 251 registered users including users outside the ED, who accessed learning materials within these sites 7494 times. The majority of staff members interviewed agreed or strongly agreed that the collaborative learning application had improved learning capacity within this ED (88%, 95% CI 74-94%). CONCLUSION: We demonstrate the implementation of an organizational learning model based on independent online sites networking together within an organization. This appears to be both usable and acceptable to staff members working in a large ED as a means of knowledge management.

Lanthanide nanocrystals have demonstrated strong potentials in nanomedicine due to its up-conversion and strong magnetic properties, and low toxicity. This talk will focus on strategies in lanthanide nanostructure tailoring to achieve up-conversion color emission tuning, MRI T1 and T2 contrast tuning, and the use of up-conversion fluorescence in drug delivery and cancer cells ablation. The current work reports single-phase, terbium-doped sodium dysprosium fluoride (NaDyF4:Tb3+) nanocrystals with green luminescence and good T2 contrast in 7.0-T MRI phantom and animal imaging. The current nanocrystals demonstrate good potential as a dual modal contrast agent for high-field magnetic resonance (MR) and fluorescence imaging.
