1. Formation and phenotypic characterization of CD49a, CD49b and CD103 expressing CD8 T cell populations in human metastatic melanoma

Abstract

Integrins α1β1 (CD49a), α2β1 (CD49b) and αEβ7 (CD103) mediate retention of lymphocytes in peripheral tissues, and their expression is upregulated on tumor infiltrating lymphocytes (TIL) compared to circulating lymphocytes. Little is known about what induces expression of these retention integrins (RI) nor whether RI define subsets in the tumor microenvironment (TME) with a specific phenotype. Human metastatic melanoma-derived CD8 TIL could be grouped into five subpopulations based on RI expression patterns: RIneg, CD49a+ only, CD49a+CD49b+, CD49a+CD103+, or positive for all three RI. A significantly larger fraction of the CD49a+ only subpopulation expressed multiple effector cytokines, whereas CD49a+CD103+ and CD49a+CD49b+ cells expressed IFNγ only. RIneg and CD49a+CD49b+CD103+ CD8 TIL subsets expressed significantly less effector cytokines overall. Interestingly, however, CD49a+CD49b+CD103+ CD8 expressed lowest CD127, and highest levels of perforin and exhaustion markers PD-1 and Tim3, suggesting selective exhaustion rather than conversion to memory. To gain insight into RI expression induction, normal donor PBMC were cultured with T cell receptor (TCR) stimulation and/or cytokines. TCR stimulation alone induced two RI+ cell populations: CD49a single positive and CD49a+CD49b+ cells. TNFα and IL-2 each were capable of inducing these populations. Addition of TGFβ to TCR stimulation generated two additional populations; CD49a+CD49bnegCD103+ and CD49a+CD49b+CD103+. Taken together, our findings identify opportunities to modulate RI expression in the TME by cytokine therapies and to generate subsets with a specific RI repertoire in the interest of augmenting immune therapies for cancer or for modulating other immune-related diseases such as autoimmune diseases.

2. Targeting immunosuppression for enhanced focused ultrasound efficacy in triple negative breast cancer

Abstract

Immunologic rejection of triple-negative breast cancer (TNBC) is rare. Focused Ultrasound (FUS) promotes anti-tumor immunity by inducing tissue destruction and inflammation. We hypothesized that thermally-ablative FUS (tFUS) would increase the immune response to TNBC. However, TNBC is commonly accompanied by the expansion of immunosuppressive myeloid cells. By implanting a metastasizing TNBC cell line, 4T1, into BALB/c mice, we showed that a combinatorial therapy of tFUS and Gemcitabine (GEM), a myeloablative chemotherapy, was able to control primary tumor growth and reduce mortality. This response is dependent on T cell-mediated immunity. While this treatment remarkably resulted in some cures, the effects were not durable in every case; suggesting immunosuppression may be re-established. We hypothesize GEM is synergizing with FUS to immunologically control tumor growth by either depleting myeloid derived suppressor cells (MDSCs) or inducing tumor cell death. To discriminate between these possibilities, we will use a Ly6G-specific 1A8 antibody to deplete the MDSC population. In parallel, we are interrogating the ability of GEM to induce immunogenic cell death in this model. Current studies are focused on understanding alterations and the role of CD4 and CD8 T cells in tumor control after FUS+GEM therapy. Finally, RNAseq analysis has been performed on treated samples to develop hypotheses to explain the effectiveness of dual therapy. Notably, similar therapeutic approaches are now ongoing as a clinical trial at our institution. We expect our studies to reveal the mechanistic basis in which FUS and GEM are eliciting an immunological response resulting in primary tumor growth control and an overall survival advantage. Abstract

Major histocompatibility complex (MHC) class I is a membrane-bound protein complex expressed on nucleated human cells. MHC class I presents intracellular protein fragments to cytotoxic T cells and triggers an activation cascade upon neoantigen detection by these cells. MHC class I loss by tumor cells decreases tumor neoantigen presentation to the immune system and therefore represents a possible mechanism of immunotherapeutic resistance even among cancers that otherwise appear to be good candidates for checkpoint inhibition, such as mismatch repair (MMR)-deficient and PD-L1-positive malignancies. We herein assess MHC class I expression in a range of endometrial carcinomas, including MMR-deficient and PD-L1-positive cancers. Immunohistochemical staining for combined MHC class I A-, B-, and C-heavy chains was performed on 76 cases of endometrial carcinoma and was classified as present, subclonally lost, or diffusely lost. Tumoral PD-L1 expression, PD-L1 combined positive score, and CD3-positive T lymphocytes were also quantified. Forty-two percent of tumors showed loss of MHC class I expression, either in a subclonal (26%) or diffuse (16%) pattern. This included 46% of MMR-deficient and 25% of PD-L1-positive cancers. These findings suggest that tumoral MHC class I status may be an important factor to consider when selecting endometrial cancer patients for checkpoint inhibition.

5. MR image-guided focused ultrasound immune modulation for glioma therapy

Abstract

Glioblastoma (GB) is the most common and malignant brain tumor. Despite standard treatment with surgery, radiation and chemotherapy, its diffuse nature and proclivity for recurrence renders it largely intractable. Immunotherapy (ITx) approaches (e.g. anti-PD1) may hold promise for treating GB; however, the blood-brain (BBB) and blood-tumor (BTB) barriers hinder delivery of systemically administered ITx drugs. A potential approach to enhancing ITx delivery is MRI-guided focused ultrasound (FUS), a non-invasive technique that, when combined with concomitant systemic injection of microbubbles (MB), can transiently disrupt the BBB/BTB and mechanically perturb the tumor microenvironment. Here, we investigate whether localized BBB/BTB disruption with FUS+MB enhances anti-tumor immune responses and inhibits tumor growth, as a prelude to eventual combination with immune checkpoint blockade. One week after FUS+MB (peak negative acoustic pressure=0.6 MPa) treatment of a murine glioma model stably transfected with luciferase (GL261-luc2), CD86 mean fluorescence intensity on dendritic cells (DC) increased ~3-fold in deep cervical lymph nodes, intratumoral CD4+ T cells doubled, and intratumoral CD8+ T cells increased by ~17% (by flow cytometry). Serial bioluminescence imaging of tumors revealed significant reduction in total photon flux as early as 6 days following FUS+MB (p=0.0106), indicating tumor growth inhibition. We conclude that FUS+MB can promote DC maturity and potentially mediate adaptive immunity against glioma, independent of drug delivery. Ongoing studies entail combining FUS+MB with anti-PD-1 delivery to evaluate whether an allied treatment approach can promote an even more robust anti-glioma response.

6. Focused ultrasound ablation as an immunomodulatory strategy for metastatic breast cancer therapy

Metastatic breast cancer (BrCa) is incurable, with a 5 year survival of only 22%. While immunotherapies are capable of generating durable responses in many forms of cancer, immune rejection of BrCa is very rare. Indeed, immunotherapies (e.g. checkpoint blockade) for BrCa are limited by poor functional CD8+ T cell infiltration and immunosuppression in the tumor microenvironment. Thus, adjunct strategies that render BrCa tumors responsive to immunotherapies are desperately needed. Focused ultrasound (FUS) mediates noninvasive acoustic energy deposition into tumors, conferring localized thermal and mechanical damage to targeted tissue. Here, we tested the hypothesis that FUS thermal ablation (TA) can serve as an auto-vaccine for treatment of BrCa with immunotherapy. Murine mammary carcinoma (4T1-HA) tumors were partially ablated with a 3 MHz ultrasound-guided FUS system. In tumor-draining lymph nodes, 1 week after FUS, we observed a ~3-fold increase in mature dendritic cells (CD11c-hi/CD86+), as well as an increasing trend in M1 macrophage representation by flow cytometry. While FUS did not confer changes in intratumoral CD8+ and CD4+ T cells, likely due to an overwhelming immunosuppressive burden imposed by myeloid derived suppressor cells (MDSCs), infiltrating T cells did express PD1. Therefore, we developed a combination treatment approach of FUS + gemcitabine and/or anti-PD1. This combination treatment elicited an ~50% reduction in 4T1-HA tumor growth at 5 days post-FUS ablation, suggesting that partial TA of BrCa tumors with FUS may represent a promising immunoadjuvant approach. Taken together, these results suggest immunoadjuvants abrogating immunosuppression may enable FUS to stimulate anti-tumor immunity in BrCa.

7. Acute virus-specific responses by NK cells shape CD8+ memory T cells and protective immunity

NK cells are cytotoxic to CD8+ T cell effectors following certain viral infections, such that transient NK depletion can increase T cell effector activities. Previous studies have shown that licensed Ly49G2+ (G2) NK cells confer MCMV resistance in mice expressing MHC I Dk, a specific ligand of the inhibitory G2 receptor, which resulted in enhanced adaptive immunity due to accelerated accumulation of virus-specific CD8+ T cells. However, the longer-term impact of licensed NK-mediated virus resistance on adaptive T cell immunity was not previously investigated. Hence, we examined CD8+ T cells for KLRG1, CD127, CD62L and cytokine production at discrete times post-infection. Early on (6-10d), licensed G2+ NK-mediated resistance in Dk mice led to a profound skewing of CD8+ T cells from short-lived effectors (SLEC) into memory precursor effector cells (MPEC). By comparison, mice without Dk-licensed G2+ NK cells were biased towards SLECs throughout 10d, and upon contraction. This T cell differentiation effect endured through 21-56d postinfection such that the frequencies of memory and effector cells reflected differences observed for the strains at d10. We then transferred CD8+ T cells from latently MCMV-infected mice into naïve, G2+ NK cell-depleted hosts and challenged them with high dose MCMV. Remarkably, recipients of CD8+ T cells primed in the presence of licensed G2+ NK cells exhibited significantly lower viral titers than those that received CD8+ T cells primed in non-Dk mice. We infer that licensed NK-mediated viral resistance significantly boosted the formation of memory CD8+ T cells, and that the means of early NK cell activation and virus resistance shapes adaptive immunity.

An MHC H-2k locus is critical in NK cell-mediated resistance to murine CMV (MCMV) in MA/My mice. Recently we found that congenic mouse strains C57L.M-H2k(R7) and C57L.M-H2k(R2) which express different MHC class I D alleles, also exhibit remarkable variation in MCMV resistance. Interestingly, MCMV resistance (Cmvr) in R7 mice corresponds with selective expansion and activation of inhibitory Ly49G2 receptor bearing NK cells after virus infection. Here we examined a potential role for Ly49G2+ NK cells in MCMV resistance, maintenance of splenic dendritic cell subsets and establishment of virus-specific CD8+ T cells. We show that Ly49G2+ NK cells acquire functional competence most efficiently in Dk-expressing R7 animals. In addition, we have observed a loss of splenic CD8α+ and CD11b+ dendritic cells shortly after MCMV infection in susceptible (Cmvs) R2 mice, whereas R7 animals maintain these subsets. We further report that efficient MHC H-2k NK-mediated virus immunity led to a robust and significant expansion of MCMV-specific CD8 T cells by day 6 in R7 mice in contrast with R2 or C57L (Cmvs) animals, which failed to mount efficient CD8 T cell immune responses. We conclude that MHC H-2k enhances virus specific T cell immunity in R7 congenic mice, in addition to establishing functionally competent Ly49G2+ NK cells, which can rapidly respond to MCMV infection.
