2. Factors associated with moral sensitivity among nurses in adult intensive care units: A cross‐sectional study in China.

Abstract

Background: Moral sensitivity may play a role in dealing with ethical issues in ICUs. However, the moral sensitivity of adult ICU nurses in China and its predictors have not been well investigated. Aim: To analyse the moral sensitivity of intensive care unit (ICU) nurses and its predictors. Study Design: A cross‐sectional study through convenience sampling was conducted in the ICUs of five hospitals in China between November and December 2022. A total of 331 ICU nurses were recruited. Data were obtained using a self‐developed socio‐demographic characteristic questionnaire, the Moral Sensitivity Questionnaire, the Jefferson Scale of Empathy‐Health Professionals and the Organizational Climate Scale for Nursing. The participating institutions received a link to the tools used for online data collection, which they then forwarded to nursing staff. Results: The valid response rate was 96.98% (n = 321). The total mean moral sensitivity score of nurses was 43.04 ± 5.95. The mean scores for empathic ability and organizational climate for nursing were 91.97 ± 17.88 and 101.28 ± 14.77, respectively. Regression analyses revealed that the primary factors associated with moral sensitivity among critical care nurses included work tenure (p <.05), empathic ability (p <.001) and organizational climate (p <.001). Conclusions: High levels of moral sensitivity were found in critical care nurses in China. Work tenure, empathic ability and organizational climate were significant predictors of moral sensitivity in critical care nurses. Relevance to Clinical Practice: To enhance the ethical sensitivity of ICU nurses, we suggest to strengthen the training of novice nurses so that they can better face the moral dilemma in clinical practice. In addition, organizational managers should also take measures to create a positive and harmonious working atmosphere, promote the application of moral knowledge in nursing practice and enhance their moral sensitivity. Abstract

Aims: Establishing a nomogram to estimate the probability of oral mucosal membrane pressure injury of endotracheal tube‐intubated hospitalized patients in intensive care unit. Design: Multicentre prospective cohort study. Methods: Using Lasso regression and COX regression, variable selection was performed on demographic, clinical and laboratory data of 1037 ICU endotracheal tube‐intubated hospitalized patients from West China Hospital, to construct a nomogram. External validation was conducted on 484 ICU endotracheal tube‐intubated patients from People's Hospital of Zhongjiang County. Results: Among 38 potential predictors, five variables emerged as independent predictors, integrated into the nomogram: administration of antibiotics, nutritional therapy duration, agitation, hypotension and albumin levels. Conclusions: We established a nomogram based on the hospital characteristics of ICU endotracheal tube‐intubated patients, aiding in the prediction of the occurrence of oral mucosal membrane pressure injury. Reporting Method: The study followed TRIPOD guidelines. Relevance to Clinical Practice: The nomogram we developed can assist clinical worker in better identifying at‐risk patients and risk factors. It enables the implementation of evidence‐based nursing interventions in care to prevent the development of oral mucosal membrane pressure injury. Abstract

Aim: To investigate the ability of critical care nurses to identify pressure injury and incontinence‐associated dermatitis and analyse the possible influencing factors. Design: Cross‐sectional survey. Methods: This study was conducted at 24 hospitals across 12 provinces in China. A self‐made electronic questionnaire was used. Nurses identified and judged injuries according to the information provided. Results: The average identification score for pressure injury and incontinence‐associated dermatitis was 9.00 ± 3.51 points, and only 2.16% of nurses scored ≥16 points. The average correct identification rate for pressure injury and incontinence‐associated dermatitis was 45%. The correct identification rate for stage 1 pressure injury was the highest, while those for stage 3, stage 4, deep tissue pressure injury and unstageable pressure injury were all lower than 50%; incontinence‐associated dermatitis was also easily misjudged. Nurses' educational backgrounds, professional titles, job positions, hospital levels and learning frequency were the factors that affected their ability to identify pressure injury and incontinence‐associated dermatitis. Abstract

This research project develops self-assembled porphyrin micro-nano structures and their metal nanocomposites for efficient solar H2 evolution. The project is composed of three inter-related tasks. The first task is to develop the synthetic methods for preparing the binary porphyrin micro-nano structures by ionic self-assembly under various aqueous solution conditions. Although the porphyrin molecules have been widely studied in artificial photosynthesis, the porphyrin monomers absorb light in a limited range of the solar spectrum and are unstable in solution. In using ionic selfassembly of two oppositely charged porphyrin molecules, we successfully synthesized the more robust micro/nano clover-like structures to replace the porphyrin monomer, which also have red-shifted light absorption due to J-aggreate formation. In addition, the microscale porphyrin structures can be designed with photochemical and catalytic properties, which is useful in solar energy harvesting and conversion to desired hydrogen. Effect of reaction time, growth temperature, ionic strength, and metal interchange on the self-assembly process of these micro-nano structures is studied. I have also investigated the ability of these porphyrin micro-nano structures to photocatalytically reduce aqueous gold(I) and platinum(IV) complexes to initiate and form nanostructures decorated with metal nanostructures on their surfaces. Photocatalytic reduction of both gold(I) thiosulfate and thiourea complexes lead to the metal-clover porphyrin nanocomposites. The platinized micro-nano clovers catalyze H2 generation utilizing visible light and a sacrificial electron donor at pH 3. The photoconductive ZnTPPS4-/SnT(N-EtOH-4-Py)P4+ porphyrin micro-nano structures are able to deliver electrons directly to the platinum nanoparticles at the surface with sufficient reducing power to generate hydrogen directly. 49. Diverse Dyes-Embedded Staphylococcus aureusas Potential Biocarriers for Enhancing Sensitivity in Biosensing

Abstract

Nanomaterials-based immunochromatographic assays (ICAs) have gained great commercial success in real-life point-of-care testing (POCT). Exploring novel carriers of ICAs with improved signaling and sustained activity favors the development of sensitive POCT. Herein a potent signal biotag, colored Staphylococcus aureus(SA), was created for ICA carriers through a mild self-assembly strategy, providing high luminance and abundant specific binding sites for immobilization of monoclonal antibodies (mAbs). The biocompatible SA-dyes (SADs) retained both an intact surface structure for mAbs labeling (Fc portion) and the superior bioactivity of immobilized mAbs (affinity constant was about 109M–1), thus waiving the intrinsic limitations of traditional nanomaterials and endowing high sensitivity. Proof-of-concept was demonstrated by employing Congo red- or/and fluorescein isothiocyanate-embedded SA (SACR, SAFITC, and SACR–SAFITC) as ICA carriers to detect zearalenone (ZEN) through colorimetric or/and fluorimetric signals. Furthermore, the ICAs satisfied the clinical requirement perfectly, including limit of detection (0.013 ng/mL, which was at least an 85-fold improvement over that of traditional gold nanoparticles-based ICA), linearity (R2> 0.98), reproducibility (RSD < 8%), selectivity, and stability. Importantly, the proposed biosensors could be well-applied in four real samples for ZEN monitoring with satisfactory recoveries, correlating well with the results from liquid chromatography–tandem mass spectrometry (LC-MS). This work also proved a universal design for tailoring coloration bands for SAD–ICA detection of multiple analytes.

A comparative hydrogen evolution study of all of the clover-like structures from the Zn and Sn complexes of TPPS and T(N-EtOH-4-Py)P indicate that the porphyrin solids produce hydrogen more efficiently and durably than individual porphyrins in solution, suggesting that the collective properties of the micro-nano solids enhance the H2 production reaction and stablize the porphyrin light-harvesting materials.
