Abstract

Dementia is a syndrome characterized by cognitive and neuropsychiatric symptoms associated with progressive functional decline (FD). FD is a core diagnostic criterion for dementia, setting the threshold between its prodromal stages and the full-blown disease. The operationalization of FD continues to generate a great deal of controversy. For instance, the threshold of FD for the diagnosis of dementia varies across diagnostic criteria, supporting the need for standardization of this construct. Moreover, there is a need to reconsider how we are measuring FD to set boundaries between normal aging, mild cognitive impairment, and dementia. In this paper, we propose a multidimensional framework that addresses outstanding issues in the assessment of FD: i) What activities of daily living (ADLs) are necessary to sustain an independent living in aging? ii) How to assess FD in individuals with suspected neurocognitive disorders? iii) To whom is the assessment directed? and iv) How much does FD differentiate healthy aging from mild and major neurocognitive disorders? Importantly, the To Whom Question introduces a person-centered approach that regards patients and caregivers as active agents in the assessment process of FD. Thus, once impaired ADLs have been identified, patients can indicate how significant such impairments are for them in daily life. We envisage that this new framework will guide future strategies to enhance functional assessment and treatment of patients with dementia and their caregivers. [ABSTRACT FROM AUTHOR]

Abstract

According to gerontological thought, an important part of senior citizens' disabilities are products of their social exclusion rather than factors associated with their physical or mental health. How these exclusions come about was the purpose of the research reported in this article. The study was conducted among 682 Chilean university students using a semantic differential test. A generalized negative image on old age was observed. The conclusion is that social images of old age affect the well-being of present and future senior citizens. (Contains 16 figures and 1 table.)

Abstract

Background: Cognitive assessments able to detect impairments as early as neuropathological changes that occur in neurodegenerative diseases initiate are urgently needed. The Visual Short-Term Memory Binding Test (VSTMBT) and the Free and Cued Selective Reminding Test (FCRST) have been recently recommended by the Neurodegenerative Diseases Working Group (Costa et al., 2017) as promising preclinical markers of AD. They have never been used before to assess elderlies with cognitive complain recruited from the community. Method: A total of 271 subjects (37 healthy controls (HC), 112 subjective cognitive complain (SCC), 96 mild cognitive impairment (MCI) and 26 Alzheimer's disease dementia (ADD)), recruited from Geroscience Center for Brain Health and Metabolism (GERO) Cohort and the Memory and Neuropsychiatric Clinic (CMYN), underwent assessment with the VSTMBT and the Visual Version of the FCRST (FCRST-Visual). Two memory loads were used for the VSTMBT (low and high). The ability of these tests to discriminate between groups. Result: Significant differences were found between HC, MCI and ADD using the VSTMBT and the FCRST-Visual version. Notably, the STMB was the only test that "marginally" differentiated between HC from SCC (p = 0.055). Moreover, whereas the FCRST-Visual showed a gradient (HC = SCC) > MCI > AD, the VSTMBT's gradient was HC > (SCC = MCI = ADD) suggesting that the function assessed by the latter test may be sensitive to the very early stages of the disease dropping to levels that cannot decline further. Conclusion: Our results suggest that the VSTMBT and FCSRT are sensitive to the early stages of dementia. Whereas the former detects changes in the early subjective stages, the latter is more sensitive to later objective stages of cognitive decline. The latter but not the former could help monitor disease progression. These results raise important questions about the usefulness of cognitive screening tools to detect and monitor disease progression and to separate normal and abnormal ageing trajectories. We propose the need of cognitive assessments that detect subtle differences as early as neuropathological changes occur in the brain, which will lead to the development of new "cognitive biomarkers" for dementia.

Abstract

Aim: Flexible adjustment or accommodation to loss is healthy; however, little is known about how it can be achieved in old age. We sought to identify and characterize effective coping processes for achieving accommodation at this stage of life. Our aim was to foster the activation of the psychological resources of those who must deal with significant losses to which they will inevitably need to adapt. Method: Qualitative study with a sample of men (n = 16) and women (n = 19) aged 60 years and up. Information was collected through observation and content analysis applied to 35 in-depth interviews, using Atlas-Ti (v7). Results: Eighteen coping processes implemented in response to loss were identified and characterized. Although no single process led to full adjustment by itself, the difference between the participants who accommodated and those who lacked accommodation was reflected in the predominance of certain processes. This approach enabled us to identify 13 effective processes, such as the search for meaning and the use of humor, which were generally used by participants who had achieved accommodation. The processes regarded as ineffective, such as avoidance and procrastination, were used more often by those who displayed insufficient accommodation. Conclusion: It was confirmed that accommodation to losses in old age not only results from "the passage of time"; rather, it can be fostered by people's active efforts. Knowledge of these practices can help practitioners orient mental health interventions for older adults who have difficulty coping in healthy ways and preserving their subjective well-being. El creciente aumento de la población mayor en nuestro país desata una serie de interrogantes con respecto a los mecanismos que favorecen o dificultan su integración social. Nuestra investigación tuvo por objetivo conocer, a través de la aplicación de una encuesta nacional, opiniones, expectativas y evaluaciones sobre diferentes modalidades de inclusión y exclusión social de las personas adultas. Los resultados del estudio revelaron la falta de preparación, a nivel personal y como país, para enfrentar el envejecimiento poblacional, junto con una demanda por soluciones colectivas, principalmente de carácter político-estatal, para asumir los desafíos que acompañan este cambio demográfico The increasing elderly population in our country generates a series of questions regarding the mechanisms that help or hinder their social integration. Our research aimed to reveal, through the implementation of a national survey, opinions, expectations and evaluations of different forms of social inclusion and exclusion of adults. Abstract

En este artículo se reflexiona sobre una manifestación específica de la complejidad social contemporánea: la contradictoria observación de las prácticas solidarias. Se analiza la sociedad chilena que, en un plano general, es descrita como individualista y en la cual primaría el desinterés por los proyectos colectivos. Fundamentamos nuestros argumentos con observaciones -de segundo orden- aplicadas a discursos de gran impacto en la comunicación social y que tratan sobre la solidaridad y sus cambios, así como sobre las conclusiones de estudios locales que hemos realizado sobre estas materias. Levantamos la hipótesis –no conclusiva- que los proyectos modernizadores recientes, con su decidido carácter neoliberal, aunque se funden en promover los intereses propios, la competitividad y el éxito a base de los méritos personales, no anulan los vínculos asociativos y comunitarios, sino que los registran bajo nuevos códigos y, por lo tanto, con otras expectativas. Finalmente, exploramos las proyecciones y requerimientos que tienen las nuevas formas de colaboración social para las organizaciones que se basan o canalizan el trabajo social voluntario.

Esta investigación buscó conocer la mirada que los jóvenes chilenos, en este caso universitarios, tienen sobre la vejez. El trabajo está basado en corrientes del pensamiento gerontológico que destacan que en gran medida las minusvalías de las personas mayores son producto de su exclusión social y de un contexto cultural desfavorable, más que de su estado físico o condición mental. De qué manera se dan estas exclusiones fue el objeto de una investigación realizada entre jóvenes chilenos, a partir de la aplicación de una prueba de diferencial semántico a 682 estudiantes universitarios. El propósito del estudio fue ampliar el conocimiento sobre las imágenes que los jóvenes universitarios chilenos tienen de la vejez y proponer, a la luz de sus resultados, una reflexión sobre algunas de las consecuencias que estas imágenes pueden tener en nuestra sociedad. Los resultados mostraron un predominio de imágenes negativas que son generalizadas y proyectadas sobre la vejez, las que podrían estar afectando las perspectivas de bienestar de los adultos mayores actuales y futuros en un contexto creciente de envejecimiento poblacional y aumento de las expectativas de vida. Esta pesquisa buscou conhecer o olhar dos jovens chilenos, neste caso universitários, sobre a velhice. O trabalho está baseado em correntes de pensamento gerontológico que destacam que, em grande medida, as deficiências das pessoas idosas são produto de sua exclusão social e de um contexto cultural desfavorável, mais do que de seu estado físico ou condição mental. De que maneira se dão estas exclusões foi o objeto de uma pesquisa realizada entre jovens chilenos, a partir da aplicação de uma prova de diferencial semântico a 682 estudantes universitários. O propósito do estudo foi ampliar o conhecimento sobre as imagens que os jovens universitários chilenos têm da velhice e propor, à luz de seus resultados, uma reflexão sobre algumas das conseqüências que estas imagens podem ter em nossa sociedade. Os resultados mostraram um predomínio de imagens negativas que são generalizadas e projetadas sobre a velhice, as quais podem estar afetando as perspectivas de bem-estar dos idosos da atualidade e do futuro, em um contexto crescente de envelhecimento populacional e aumento das expectativas de vida. This investigation searched for the view that the Chilean youth, in this case university students, have on old age. The work is based on the movements of the gerentological thought which to a great extent emphasizes minusvalias in older people which is a product of their social exclusion and unfavorable cultural context, more than of their physical state or mental condition. These exclusions were the object of an investigation made among Chilean youth, by applying a semantic differential test to 682 university students. The intention of the study was to extend the knowledge of the images that the Chilean university students have of old age and propose, a reflection on some of the consequences that these images can have in our society. The results showed a predominance of negative images that are generalized and projected on old age, those that could be affect the perspective of well-being of present and future of the old aged in an increasing context of population aging and increase of the life expectancies.

Abstract

The authors discuss the presence of the notion of autopoiesis, developed by biologist Humberto Maturana and his colleagues, in the social sciences. The study looks at the adoption of a concept that referred strictly to a biological phenomenon and its dissemination from a peripheral country (Chile) to the plane of universalist social theories. The presentation is organized in three parts: the first deals with the emergence of the theory of autopoiesis and aims to pinpoint the context and conditions that favored its being accepted in the social sciences, particularly in Niklas Luhmann's application; the second part presents the results of an analysis applied to the data bases of recent scientific publications that make it possible to specify the generalization of the concept; and finally, the authors develop certain hypotheses to explain this exceptional case of the dissemination of Latin American scientific thought in social theory.
