Abstract

This study characterized chemical structures of tetraploid jiaogulan saponins, and investigated their cholesterol-lowering effects and mechanisms in hamsters fed a high-fat diet (HFD). Nine saponins, including five reported for the first time, were obtained from total jiaogulan saponins (TJS) and elucidated for their chemical structures according to 2D NMR and HR-MS data. Administration of TJS effectively decreased plasma total cholesterol by 17.4% and 32.6%, triacylglycerols by 22.8% and 47.7% and LDL-C by 22.0% and 40.2%, respectively. Moreover, supplementation with TJS significantly increased faecal excretion of total cholesterol and bile acids by 43.8% and 64.4%, respectively. TJS significantly up-regulated intestinal mRNA levels of ABCG5/8, LXRα, hepatic mRNA CYP7A1 and ABCG5 in hamsters on HFD. In addition, TJS decreased cholesterol micelle solubility and suppressed cholesterol uptake in cultured Caco-2 cells. Taking together, TJS may reduce plasma cholesterol by increasing faecal cholesterol excretion and through modulation of genes involved in cholesterol absorption and metabolism.

Abstract

Abstract Background Seneca Valley virus (SVV) has emerged in multiple countries in recent years. SVV infection can cause vesicular lesions clinically indistinguishable from those caused by other vesicular disease viruses, such as foot-and-mouth disease virus (FMDV), swine vesicular disease virus (SVDV), vesicular stomatitis virus (VSV), and vesicular exanthema of swine virus (VESV). Sensitive and specific RT-PCR assays for the SVV detection is necessary for differential diagnosis. Real-time RT-PCR (rRT-PCR) has been used for the detection of many RNA viruses. The insulated isothermal PCR (iiPCR) on a portable POCKIT™ device is user friendly for on-site pathogen detection. In the present study, SVV rRT-PCR and RT-iiPCR were developed and validated. Results Neither the SVV rRT-PCR nor the RT-iiPCR cross-reacted with any of the vesicular disease viruses (20 FMDV, two SVDV, six VSV, and two VESV strains), classical swine fever virus (four strains), and 15 other common swine viruses. Analytical sensitivities of the SVV rRT-PCR and RT-iiPCR were determined using serial dilutions of in vitro transcribed RNA as well as viral RNA extracted from a historical SVV isolate and a contemporary SVV isolate. Diagnostic performances were further evaluated using 125 swine samples by two approaches. First, nucleic acids were extracted from the 125 samples using the MagMAX™ kit and then tested by both RT-PCR methods. One sample was negative by the rRT-PCR but positive by the RT-iiPCR, resulting in a 99.20% agreement (124/125; 95% CI: 96.59–100%, κ = 0.98). Second, the 125 samples were tested by the taco™ mini extraction/RT-iiPCR and by the MagMAX™ extraction/rRT-PCR system in parallel. Two samples were positive by the MagMAX™/rRT-PCR system but negative by the taco™ mini/RT-iiPCR system, resulting in a 98.40% agreement (123/125; 95% CI: 95.39–100%, κ = 0.97). The two samples with discrepant results had relatively high CT values. Conclusions The SVV rRT-PCR and RT-iiPCR developed in this study are very sensitive and specific and have comparable diagnostic performances for SVV RNA detection. The SVV rRT-PCR can be adopted for SVV detection in laboratories. The SVV RT-iiPCR in a simple field-deployable system could serve as a tool to help diagnose vesicular diseases in swine at points of need.

Abstract

Background: Oral activated factor XI (FXIa) inhibitors may modulate coagulation to prevent thromboembolic events without substantially increasing bleeding. We explored the pharmacodynamics, safety, and efficacy of the oral FXIa inhibitor asundexian for secondary prevention after acute myocardial infarction (MI). Methods: We randomized 1601 patients with recent acute MI to oral asundexian 10, 20, or 50 mg or placebo once daily for 6 to 12 months in a double-blind, placebo-controlled, phase 2, dose-ranging trial. Patients were randomized within 5 days of their qualifying MI and received dual antiplatelet therapy with aspirin plus a P2Y12 inhibitor. The effect of asundexian on FXIa inhibition was assessed at 4 weeks. The prespecified main safety outcome was Bleeding Academic Research Consortium type 2, 3, or 5 bleeding comparing all pooled asundexian doses with placebo. The prespecified efficacy outcome was a composite of cardiovascular death, MI, stroke, or stent thrombosis comparing pooled asundexian 20 and 50 mg doses with placebo. Results: The median age was 68 years, 23% of participants were women, 51% had ST-segment–elevation MI, 80% were treated with aspirin plus ticagrelor or prasugrel, and 99% underwent percutaneous coronary intervention before randomization. Asundexian caused dose-related inhibition of FXIa activity, with 50 mg resulting in >90% inhibition. Over a median follow-up of 368 days, the main safety outcome occurred in 30 (7.6%), 32 (8.1%), 42 (10.5%), and 36 (9.0%) patients receiving asundexian 10 mg, 20 mg, or 50 mg, or placebo, respectively (pooled asundexian versus placebo: hazard ratio, 0.98 [90% CI, 0.71–1.35]). The efficacy outcome occurred in 27 (6.8%), 24 (6.0%), 22 (5.5%), and 22 (5.5%) patients assigned asundexian 10 mg, 20 mg, or 50 mg, or placebo, respectively (pooled asundexian 20 and 50 mg versus placebo: hazard ratio, 1.05 [90% CI, 0.69–1.61]). Conclusions: In patients with recent acute MI, 3 doses of asundexian, when added to aspirin plus a P2Y12 inhibitor, resulted in dose-dependent, near-complete inhibition of FXIa activity without a significant increase in bleeding and a low rate of ischemic events. These data support the investigation of asundexian at a dose of 50 mg daily in an adequately powered clinical trial of patients who experienced acute MI. Abstract

As language models grow ever larger, the need for large-scale high-quality text datasets has never been more pressing, especially in multilingual settings. The BigScience workshop, a 1-year international and multidisciplinary initiative, was formed with the goal of researching and training large language models as a values-driven undertaking, putting issues of ethics, harm, and governance in the foreground. This paper documents the data creation and curation efforts undertaken by BigScience to assemble the Responsible Open-science Open-collaboration Text Sources (ROOTS) corpus, a 1.6TB dataset spanning 59 languages that was used to train the 176-billion-parameter BigScience Large Open-science Open-access Multilingual (BLOOM) language model. We further release a large initial subset of the corpus and analyses thereof, and hope to empower large-scale monolingual and multilingual modeling projects with both the data and the processing tools, as well as stimulate research around this large multilingual corpus., Comment: NeurIPS 2022, Datasets and Benchmarks Track

Abstract

Background: Despite advances in surgery and pharmacotherapy, there remains significant residual ischemic risk after coronary artery bypass grafting surgery. Methods: In REDUCE-IT (Reduction of Cardiovascular Events With Icosapent Ethyl–Intervention Trial), a multicenter, placebo-controlled, double-blind trial, statin-treated patients with controlled low-density lipoprotein cholesterol and mild to moderate hypertriglyceridemia were randomized to 4 g daily of icosapent ethyl or placebo. They experienced a 25% reduction in risk of a primary efficacy end point (composite of cardiovascular death, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina) and a 26% reduction in risk of a key secondary efficacy end point (composite of cardiovascular death, myocardial infarction, or stroke) when compared with placebo. The current analysis reports on the subgroup of patients from the trial with a history of coronary artery bypass grafting. Results: Of the 8179 patients randomized in REDUCE-IT, a total of 1837 (22.5%) had a history of coronary artery bypass grafting, with 897 patients randomized to icosapent ethyl and 940 to placebo. Baseline characteristics were similar between treatment groups. Randomization to icosapent ethyl was associated with a significant reduction in the primary end point (hazard ratio [HR], 0.76 [95% CI, 0.63–0.92]; P =0.004), in the key secondary end point (HR, 0.69 [95% CI, 0.56–0.87]; P =0.001), and in total (first plus subsequent or recurrent) ischemic events (rate ratio, 0.64 [95% CI, 0.50–0.81]; P =0.0002) compared with placebo. This yielded an absolute risk reduction of 6.2% (95% CI, 2.3%–10.2%) in first events, with a number needed to treat of 16 (95% CI, 10–44) during a median follow-up time of 4.8 years. Safety findings were similar to the overall study: beyond an increased rate of atrial fibrillation/flutter requiring hospitalization for at least 24 hours (5.0% vs 3.1%; P =0.03) and a nonsignificant increase in bleeding, occurrences of adverse events were comparable between groups. Conclusions: In REDUCE-IT patients with a history of coronary artery bypass grafting, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events. Abstract

Private and public clouds require users to specify requests for resources such as CPU and memory (RAM) to be provisioned for their applications. The values of these requests do not necessarily relate to the application's run-time requirements, but only help the cloud infrastructure resource manager to map requested resources to physical resources. If an application exceeds these values, it might be throttled or even terminated. As a consequence, requested values are often overestimated, resulting in poor resource utilization in the cloud infrastructure. Autoscaling is a technique used to overcome these problems. We observed that Kubernetes Vertical Pod Autoscaler (VPA) might be using an autoscaling strategy that performs poorly on workloads that periodically change. Our experimental results show that compared to VPA, predictive methods based on Holt-Winters exponential smoothing (HW) and Long Short-Term Memory (LSTM) can decrease CPU slack by over 40% while avoiding CPU insufficiency for various CPU workloads. Furthermore, LSTM has been shown to generate stabler predictions compared to that of HW, which allowed for more robust scaling decisions.

Abstract

We propose a model of a closed economy based upon the System of National Accounts (United Nations [11]) and Stock Flow Consistent theory (Godley and Lavoie [2]) to model observed complex dynamics in real economies. We find that our model displays cyclic behavior while approaching an equilibrium, and simulates the effects of various shocks. We also find that a more egalitarian economy appears to be both yield a higher rate of return and be more stable (Marx [5]-Piketty[9] thesis).

We explore the distribution of gross domestic product (GDP) and per capita GDP among different countries in order to elucidate differences in the dynamics of their economies. An initial analysis of GDP data and per capita GDP data from 1980 and 2016 (and many years in between) typically finds three scaling regions—signatures of likely differences in dynamics. The GDP of the largest ~25 economies (nations, EU) follows a power law GDP ~1/rank (c.f. Garlaschelli et al. in Eur Phys J B 57:159–164 [1]); followed by a second scaling region in which GDP falls off exponentially with rank and finally a third scaling region in which the GDP falls off exponentially with the square of rank. This broad pattern holds despite significant changes in technology (enormous growth in computing power, “intelligent” automation, the Internet), the size of the world economy, emergence of new economic powers such as China, and world trade (almost free communication, containerized shipping yielding sharp declines in shipping costs, trade partnerships, growth of the EU, multinationals displacing the traditional economic role of nation-states). Thus, empirically, these patterns may be universal in which case one approach to growth of less developed economies (in the second and third scaling regions of per capita GDP) may be to identify and target causative differences between these economies and those in the first (power law) scaling region.

Abstract

The high power computing for AI and Data Centers are advancing rapidly and power efficiency becomes critical. Ways of power integration are being explored, with inductor being one of the most challenging topics. Embedding inductor in organic substrate with magnetic material is a popular approach, for the benefit of lower cost, good scalability, and future integrating roadmap. Modeling of power inductors is studied, and comparison between measurement and simulation are used to validate the modeling and verify predictions. By studying various conductor geometry and magnetic material location, an optimized inductor design approach is shown. Novel magnetic epoxy material is chosen with lamination process introduced. It is found that embedded power inductor with high inductance per area can be implemented the methods described in this paper, and a co-design approach can be used to design the power electronics using the embedded inductor with magnetic material. 28. A Smartphone App to Manage Cirrhotic Ascites Among Outpatients: Feasibility Study (Preprint)

Abstract

BACKGROUND Ascites is a common, painful, and serious complication of cirrhosis. Body weight is a reliable proxy for ascites volume; therefore, daily weight monitoring is recommended to optimize ascites management. OBJECTIVE This study aims to evaluate the feasibility of a smartphone app in facilitating outpatient ascites management. METHODS In this feasibility study, patients with cirrhotic ascites requiring active management were identified in both inpatient and outpatient settings. Patients were provided with a Bluetooth-connected scale, which transmitted weight data to a smartphone app and then via the internet to an electronic medical record (EMR). Weights were monitored every weekday. In the event of a weight change of ≥5 lbs in 1 week, patients were called and administered a short symptom questionnaire, and providers received an email alert. The primary outcomes of this study were the percentage of enrolled days during which weight data were successfully transmitted to an EMR and the percentage of weight alerts that prompted responses by the provider. RESULTS In this study, 25 patients were enrolled: 12 (48%) were male, and the mean age was 58 (SD 13; range 35-81) years. A total of 18 (72%) inpatients were enrolled. Weight data were successfully transmitted to an EMR during 71.2% (697/979) of the study enrollment days, with technology issues reported on 16.5% (162/979) of the days. Of a total of 79 weight change alerts fired, 41 (52%) were triggered by weight loss and 38 (48%) were by weight gain. Providers responded in some fashion to 66 (84%) of the weight alerts and intervened in response to 45 (57%) of the alerts, for example, by contacting the patient, scheduling clinic or paracentesis appointments, modifying the diuretic dose, or requesting a laboratory workup. Providers responded equally to weight increase and decrease alerts (P=.87). The staff called patients a mean of 3.7 (SD 3.5) times per patient, and the number of phone calls correlated with technology issues (r=0.60; P=.002). A total of 60% (15/25) of the patients chose to extend their participation beyond 30 days. A total of 17 patient readmissions occurred during the study period, with only 4 (24%) related to ascites. CONCLUSIONS We demonstrated the feasibility of a smartphone app to facilitate the management of ascites and reported excellent rates of patient and provider engagement. This innovation could enable early therapeutic intervention, thereby decreasing the burden of morbidity and mortality among patients with cirrhosis.

30. Development of TaqMan probe-based insulated isothermal PCR (iiPCR) for sensitive and specific on-site pathogen detection.

Abstract

Insulated isothermal PCR (iiPCR), established on the basis of Ralyeigh-Bénard convection, is a rapid and low-cost platform for nucleic acid amplification. However, the method used for signal detection, namely gel electrophoresis, has limited the application of iiPCR. In this study, TaqMan probe-based iiPCR system was developed to obviate the need of post-amplification processing. This system includes an optical detection module, which was designed and integrated into the iiPCR device to detect fluorescent signals generated by the probe. TaqMan probe-iiPCR assays targeting white spot syndrome virus (WSSV) and infectious myonecrosis virus were developed for preliminary evaluation of this system. Significant elevation of fluorescent signals was detected consistently among positive iiPCR reactions in both assays, correlating with amplicon detection by gel electrophoresis analysis. After condition optimization, a threshold value of S/N (fluorescent intensity(after)/fluorescent intensity(before)) for positive reactions was defined for WSSV TaqMan probe-iiPCR on the basis of 20 blank reactions. WSSV TaqMan probe-iiPCR generated positive S/Ns from as low as 10(1) copies of standard DNA and lightly infected Litopenaeus vannamei. Compared with an OIE-certified nested PCR, WSSV TaqMan probe-iiPCR showed a sensitivity of 100% and a specificity of 96.67% in 120 WSSV-free or lightly infected shrimp samples. Generating positive signals specifically and sensitively, TaqMan probe-iiPCR system has a potential as a low-cost and rapid on-site diagnostics method.

Abstract

Background Control of equine infectious anaemia (EIA) currently depends on serological diagnosis of infected equids. However, recently infected equids may not produce detectable anti-EIAV antibodies up to 157 days post infection and so present a high transmission risk. Therefore, direct nucleic acid detection methods are urgently needed to improve EIAV surveillance and management programs in counties where the disease is endemic. Objectives To evaluate a field-deployable, reverse transcription-insulated isothermal PCR (RT-iiPCR) assay targeting the conserved 5' untranslated region (5' UTR)/exon 1 of the tat gene of EIAV. Study design The analytical and clinical performance of the newly developed EIAV RT-iiPCR was evaluated by comparison with a EIAV real-time RT-PCR (RT-qPCR) along with the AGID test. Methods Analytical sensitivity was determined using in vitro transcribed RNA containing the target area of the 5' UTR/tat gene and samples from two EIAV-positive horses. Specificity was verified using nine common equine viruses. Clinical performance was evaluated by comparison with EIAV RT-qPCR and AGID using samples derived from 196 inapparent EIAV carrier horses. Results EIAV RT-iiPCR did not react with other commonly encountered equine viruses and had equivalent sensitivity (95% detection limit of eight genome equivalents), with a concordance of 95.41% to conventional EIAV RT-qPCR. However, the RT-qPCR and RT-iiPCR had sensitivities of 43.75 and 50.00%, respectively, when compared to the AGID test. Main limitations Low viral loads commonly encountered in inapparent EIAV carriers may limit the diagnostic sensitivity of RT-PCR-based tests. Conclusions Although EIAV RT-iiPCR is not sufficiently sensitive to replace the current AGID test, it can augment control efforts by identifying recently exposed or "serologically silent" equids, particularly as the latter often represent a significant transmission risk because of high viral loads. Furthermore, the relatively low cost and field-deployable design enable utilisation of EIAV RT-iiPCR even in remote regions.

Abstract

Bovine leukemia virus (BLV) is a contagious, oncogenic deltaretrovirus of cattle with a worldwide distribution. In the US, over 40% of dairy cows are infected with the virus, and evidence of its economic impact is growing. This study evaluated the performance of a field-deployable automatic nucleic acid-extraction/insulated isothermal PCR (iiPCR) system for on-site BLV-proviral DNA detection in dairy cows compared with a conventional laboratory real-time PCR (rt-PCR). Assay performance was verified in parallel tests of 36 archived blood samples with 100% agreement (κ = 1.0; n = 36) between the iiPCR and conventional rt-PCR systems, and the limit of detection of the iiPCR assay was estimated to be 4 copies (genome equivalent) per reaction. The field-deployable iiPCR system was subsequently used on-farm to test freshly collected blood samples, and showed 100% agreement (κ = 1.0; n = 32) with the laboratory rt-PCR system. Fresh blood samples were collected on a second farm and tested on both systems, also with 100% agreement (κ = 1.0; n = 34). The field-deployable iiPCR/POCKIT™ combo system performs as well as a conventional laboratory-based rt-PCR system for detection of BLV proviral DNA in whole blood and may be a useful tool for on-farm evaluation of BLV-infection status in dairy cattle.

Abstract

OBJECTIVES To compare bivalirudin to heparin during non-primary percutaneous coronary intervention (PCI). BACKGROUND The optimal anticoagulant to support PCI remains uncertain. METHODS We performed a propensity score-based analysis comparing clinical outcomes of patients receiving heparin to those receiving bivalirudin during non-primary PCI. RESULTS Of 18,867 patients in the Cardiovascular Patient Outcomes Research Team Non-Primary PCI (CPORT-E) trial, we selected 7,913 patients undergoing non-staged PCI of whom 57.3% received heparin and 42.7% received bivalirudin. In-hospital myocardial infarction occurred in 4.4% of patients receiving bivalirudin and 3.0% of patients receiving heparin (relative risk [RR] 1.5, 95% confidence interval [CI] 1.1-2.1, P = 0.022); this difference persisted at 6 weeks (5.0% vs. 3.6%, RR 1.4, 95% CI 1.0-1.8, P = 0.041). There was no difference in all-cause mortality either in-hospital (0.2% vs. 0.1% for heparin vs. bivalirudin, P = 0.887) or at 6 weeks (0.5% vs. 0.7%, P = 0.567). Abstract

Background Seneca Valley virus (SVV) has emerged in multiple countries in recent years. SVV infection can cause vesicular lesions clinically indistinguishable from those caused by other vesicular disease viruses, such as foot-and-mouth disease virus (FMDV), swine vesicular disease virus (SVDV), vesicular stomatitis virus (VSV), and vesicular exanthema of swine virus (VESV). Sensitive and specific RT-PCR assays for the SVV detection is necessary for differential diagnosis. Real-time RT-PCR (rRT-PCR) has been used for the detection of many RNA viruses. The insulated isothermal PCR (iiPCR) on a portable POCKIT™ device is user friendly for on-site pathogen detection. In the present study, SVV rRT-PCR and RT-iiPCR were developed and validated. Results Neither the SVV rRT-PCR nor the RT-iiPCR cross-reacted with any of the vesicular disease viruses (20 FMDV, two SVDV, six VSV, and two VESV strains), classical swine fever virus (four strains), and 15 other common swine viruses. Analytical sensitivities of the SVV rRT-PCR and RT-iiPCR were determined using serial dilutions of in vitro transcribed RNA as well as viral RNA extracted from a historical SVV isolate and a contemporary SVV isolate. Diagnostic performances were further evaluated using 125 swine samples by two approaches. First, nucleic acids were extracted from the 125 samples using the MagMAX™ kit and then tested by both RT-PCR methods. One sample was negative by the rRT-PCR but positive by the RT-iiPCR, resulting in a 99.20% agreement (124/125; 95% CI: 96.59–100%, κ = 0.98). Second, the 125 samples were tested by the taco™ mini extraction/RT-iiPCR and by the MagMAX™ extraction/rRT-PCR system in parallel. Two samples were positive by the MagMAX™/rRT-PCR system but negative by the taco™ mini/RT-iiPCR system, resulting in a 98.40% agreement (123/125; 95% CI: 95.39–100%, κ = 0.97). The two samples with discrepant results had relatively high CT values. Conclusions The SVV rRT-PCR and RT-iiPCR developed in this study are very sensitive and specific and have comparable diagnostic performances for SVV RNA detection. The SVV rRT-PCR can be adopted for SVV detection in laboratories. The SVV RT-iiPCR in a simple field-deployable system could serve as a tool to help diagnose vesicular diseases in swine at points of need.

Abstract

Introduction: Trimethylamine-N-oxide (TMAO), a diet-derived, gut microbial-host co-metabolite, has been associated with adverse cardiovascular outcomes in patient populations. However, the evidence is lacking from prospective studies conducted in general populations and in non-Western populations. Hypothesis: TMAO level is associated with risk of coronary heart disease (CHD) in general populations. Methods: We examined associations of urinary TMAO and its precursors (i.e., choline, betaine, and carnitine) with risk of CHD in a case-control study nested within two prospective cohorts of Chinese adults, including 275 incident CHD cases and 275 individually matched controls. Results: Urinary TMAO, but not its precursors, was associated with risk of CHD. Odds ratio (OR, 95% CI) for the highest vs. lowest quartiles of TMAO was 1.91 (1.08-3.35; p-trend=0.008) after adjusting for CHD risk factors including obesity, diet, lifestyles, and metabolic diseases and 1.75 (0.96-3.18; p-trend=0.03) after further adjusting for potential confounders/mediators including central obesity, dyslipidemia, low-grade inflammation, and intakes of seafood and deep-fried meat/fish, which were associated with TMAO level in our study. OR was 1.30 (1.03-1.63) per standard deviation increase in log-TMAO in the fully-adjusted model. History of diabetes modified the TMAO-CHD association. A high TMAO level (≥ vs. Conclusions: Our study suggests that TMAO may play a role in the development of CHD, highlighting the importance of diet-gut microbiota-host interplay in cardiometabolic health.

Abstract

Dengue virus (DENV) infection is considered a major public health problem in developing tropical countries where the virus is endemic and continues to cause major disease outbreaks every year. Here, we describe the development of a novel, inexpensive, and user-friendly diagnostic assay based on a reverse transcription-insulated isothermal PCR (RT-iiPCR) method for the detection of all four serotypes of DENV in clinical samples. The diagnostic performance of the newly established pan-DENV RT-iiPCR assay targeting a conserved 3′ untranslated region of the viral genome was evaluated. The limit of detection with a 95% confidence was estimated to be 10 copies of in vitro -transcribed (IVT) RNA. Sensitivity analysis using RNA prepared from 10-fold serial dilutions of tissue culture fluid containing DENVs suggested that the RT-iiPCR assay was comparable to the multiplex real-time quantitative RT-PCR (qRT-PCR) assay for DENV-1, -3, and -4 detection but 10-fold less sensitive for DENV-2 detection. Subsequently, plasma collected from patients suspected of dengue virus infection ( n = 220) and individuals not suspected of dengue virus infection ( n = 45) were tested by the RT-iiPCR and compared to original test results using a DENV NS1 antigen rapid test and the qRT-PCR. The diagnostic agreement of the pan-DENV RT-iiPCR, NS1 antigen rapid test, and qRT-PCR tests was 93.9%, 84.5%, and 97.4%, respectively, compared to the composite reference results. This new RT-iiPCR assay along with the portable POCKIT nucleic acid analyzer could provide a highly reliable, sensitive, and specific point-of-need diagnostic assay for the diagnosis of DENV in clinics and hospitals in developing countries.

Rapid diagnosis is critical for the implementation of efficient control strategies before and during PEDV and PDCoV outbreaks. Insulated isothermal PCR (iiPCR) on the portable POCKIT™ device is user friendly for on-site pathogen detection. In the present study, a singleplex PEDV RT-iiPCR, a singleplex PDCoV RT-iiPCR, and a duplex PEDV/PDCoV real-time RT-PCR (rRT-PCR) commercial reagents targeting the M gene were compared to an N gene-based PEDV rRT-PCR and an M gene-based PDCoV rRT-PCR that were previously published and used as reference PCRs. All PCR assays were highly specific and did not cross react with other porcine enteric pathogens. Analytical sensitivities of the PEDV RT-iiPCR, PDCoV RT-iiPCR and duplex PEDV/PDCoV rRT-PCR were determined using in vitro transcribed RNA as well as viral RNA extracted from ten-fold serial dilutions of PEDV and PDCoV cell culture isolates. Performance of each PCR assay was further evaluated using 170 clinical samples (86 fecal swabs, 24 feces, 19 intestines, and 41 oral fluids). Compared to the reference PEDV rRT-PCR, the sensitivity, specificity and accuracy of the PEDV RT-iiPCR were 97.73%, 98.78%, and 98.24%, respectively, and those of the duplex PEDV/PDCoV rRT-PCR were 98.86%, 96.34%, and 97.65%, respectively. Compared to the reference PDCoV rRT-PCR, the sensitivity, specificity and accuracy of the PDCoV RT-iiPCR were 100%, 100%, and 100%, respectively, and those of the PEDV/PDCoV duplex rRT-PCR were 96.34%, 100%, and 98.24%, respectively. Overall, all three new PCR assays were comparable to the reference rRT-PCRs for detection of PEDV and/or PDCoV. The PEDV and PDCoV RT-iiPCRs are potentially useful tools for on-site detection and the duplex PEDV/PDCoV rRT-PCR provides a convenient method to simultaneously detect the two viruses and differentiate PEDV from PDCoV.

Aircraft accident investigation has played a pivotal role in improving the safety of aviation. Advances in recorder technology, specifically Cockpit Voice Recorders (CVRs) and Flight Data Recorders (FDR) have made a huge contribution to the understanding of occurrences for accident investigators. However, even these recorders have limitations such as the evidence they provide about pilots' situation awareness or behaviours. Supplementing audio and data recordings with video has been discussed for many years and whilst there continues to be debate among regulators, operators, manufacturers and pilot unions, the International Civil Aviation Organization (ICAO) has mandated Airborne Image Recorders (AIR) from 2023. The purpose of installing such systems is to provide evidence of crew operational behaviours in terms of both human-human and human-computer interactions (HCI) on the flight deck. Video alone is unlikely to provide sufficient evidence for investigators. This study examines the additional value that eye-tracking technology may provide through the case study of an accident involving an Airbus A330-300 aircraft which experienced a rejected landing. Currently, the investigation of such events, where crew interaction with automation is critical to their situation awareness, relies heavily on interview data. Such data may be unavailable (in the case of serious injury) or unreliable (based on hindsight bias). By integrating eye tracking technology into an AIR, accident investigators will potentially gain a better understanding of pilots’ visual scan patterns across flight deck instrumentation. This has implications for flight deck and procedural design as well as training and simulation.

Abstract

This paper describes a package design and associated manufacturing assembly process that stack a module on top of another module using a frame board. The module on the top could be in molded or open frame form. In addition, this manufacturing process is compatible with the conformal shielding process important in RF applications. The complete module-on-module can be coated seamlessly and connected to the ground planes of the two substrates. The module may be a part of product line that requires more functionality over the same foot print. There is the need to houses more components than its predecessor. Such example includes the standard memory package-on-package. However, if the module on top is not of commodity type, or when the volume is not big enough to develop a standard package, this module-on-module architecture and process could be a good solution. Alternatively, one could fix the design of bottom module , and design multiple choices of the top module. When stacked together, the two modules form various different configurations in order to serve different functionalities. The structure of the module on module is shown in figure 1 below: Figure 1 Structure of the Module on Module On the frame board, there are vertical via where vertical electrical and power connection can be made. The frame board is mounted on the main board-II by surface mount process with underfill option. In the figure, a molded module on the top is shown, and an overall coating is applied to the whole module-on-module. Figure 2 Manufacturing Process of the Module-on-module The assembly process of the module-on-module is shown in Figure 2. First, the bottom module is manufactured by a one side SMT process and sawed, with the frame board soldered and underfilled. Then, the top module is SMT and molded in a panel form on the one side, and the bottom module is SMT and underfilled on the other side. Last, the complete module is sawed and conformal shielded by the sputtering process. The design of the ground plans of the substrates, together with that of the frame board, makes it such that the coating is connected to the ground planes of the boards. It is important to study the warpage of such a module-on-module to make sure that it meets the JDEC standards. It is important to assure that the signal integrity of the electrical connections, including RF connections, inside of the frame board meets the requirement. Both simulation and measurement are made to test vehicles of the module-on-module. It is found that the simulation matches the measurement nicely. This module structure can be used for a variety of applications.

Abstract

Middle East respiratory syndrome (MERS) is an emerging zoonotic viral respiratory disease that was first identified in Saudi Arabia in 2012. In 2015, the largest MERS outbreak outside of the Middle East region occurred in the Republic of Korea. The rapid nosocomial transmission of MERS-coronavirus (MERS-CoV) in Korean health care settings highlighted the importance and urgent need for a rapid and reliable on-site diagnostic assay to implement effective control and preventive measures. Here, the evaluation and validation of two newly developed reverse transcription–insulated isothermal PCR (RT-iiPCR) methods targeting the ORF1a and upE genes of MERS-CoV are described. Compared with World Health Organization-recommended singleplex real-time quantitative RT-PCR (RT-qPCR) assays, both RT-iiPCR assays had comparable analytical sensitivity for the detection of MERS-CoV RNA in tissue culture fluid and in sputum samples spiked with infectious virus. Furthermore, clinical evaluation was performed with sputum samples collected from subjects with acute and chronic respiratory illnesses, including patients infected with MERS-CoV. The overall agreement values between the two RT-iiPCR assays and the reference RT-qPCR assays were 98.06% (95% CI, 94.43%–100%; κ = 0.96) and 99.03% (95% CI, 95.88%–100%; κ = 0.99) for ORF1a and upE assays, respectively. The ORF1a and upE MERS-CoV RT-iiPCR assays coupled with a field-deployable system provide a platform for a highly sensitive and specific on-site tool for diagnosis of MERS-CoV infections.

Abstract

Consumption of Salmonella-contaminated foods, such as poultry and fresh eggs, is known to be one of the main causes of salmonellosis. Conventional PCR methods, including real-time PCR for rapid detection of Salmonella, in general require skilled technicians and costly instruments. A recently developed novel convective PCR, insulated isothermal PCR (iiPCR), is carried out in polycarbonate capillary tubes. In this study, we designed TaqMan probes and PCR primers based on the yrfH gene encoding a heat shock protein for the iiPCR detection of Salmonella in chicken meat samples. The TaqMan probe was labeled with 6-carboxyfluorescein and 6-carboxytetramethylrhodamine at the 5' and 3' ends, respectively. The PCR amplicon was 133 bp. A typical run of this iiPCR assay was completed within 1 h. Specific PCR products were obtained for 148 strains representing 49 serotypes of Salmonella tested. Under the same conditions, false-positive results were not obtained for 98 non-Salmonella strains tested, including strains of Enterobacteriaceae closely related to Salmonella. For chicken meat samples, with a 5-h enrichment step Salmonella at as low as 10⁰ CFU/g of poultry meat could be detected. Because the amplification signals from the probes are detectable at 520 nm, identification of the PCR products by gel electrophoresis is not required. Compared with conventional PCR, the iiPCR system requires less expertise and provides an economical, reliable, and rapid tool for result interpretation. Detection results can be obtained within 8 h, including the enrichment and DNA extraction steps.

Optical networking is evolving from classical service-provider base data-center centric (DCC) internetworking environment with massive capacity, hence demanding novel optical switching and interconnecting technologies. The traditional telecom networks are under a flattening transformation to meet challenges from DCC networks for massive capacity serving in order of multi-Pb/s. We present proposed distributed and concentric data center based networks and the essential optical interconnection technologies, from the photonic kernels to electronic and optoelectronic server clusters, in both passive and active structures. Optical switching devices and integrated matrices are proposed composing of tunable (bandwidth and center wavelength) optical filters and switches as well as resonant microring modulators (μRM)(switching and spectral demux/mux) for multi-wavelength flexible-bandwidth optical channels of aggregate capacity reaching Ebps. The design principles and some experimental results are also reported.

Abstract

Tremendous efforts have been developed for multi-Tbps over ultra-long distance and metro and access optical networks. With the exponential increase demand on data transmission, storage and serving, especially the 5G wireless access scenarios, the optical Internet networking has evolved to data-center based optical networks pressuring on novel and economical access transmission systems. This paper reports (1) Experimental platforms and transmission techniques employing band-limited optical components operating at 10G for 100G based at 28G baud. Advanced modulation formats such as PAM-4, DMT, duo-binary etc are reported and their advantages and disadvantages are analyzed so as to achieve multi-Tbps optical transmission systems for access inter- and intra- data-centered-based networks; (2) Integrated multi-Tbps combining comb laser sources and micro-ring modulators meeting the required performance for access systems are reported. Ten-sub-carrier quantum dot com lasers are employed in association with wideband optical intensity modulators to demonstrate the feasibility of such sources and integrated micro-ring modulators acting as a combined function of demultiplexing/multiplexing and modulation, hence compactness and economy scale. Under the use of multi-level modulation and direct detection at 56 GBd an aggregate of higher than 2Tbps and even 3Tbps can be achieved by interleaved two comb lasers of 16 sub-carrier lines; (3) Finally the fundamental designs of ultra-compacts flexible filters and switching integrated components based on Si photonics for multi Tera-bps active interconnection are presented. Experimental results on multi-channels transmissions and performances of optical switching matrices and effects on that of data channels are proposed.

The effects of a rose-flower extract, Rosa damascena, on the mortality rate of Drosophila melanogaster was evaluated in this study. R. damascena is a potent antioxidant that has many therapeutic uses in addition to its perfuming effects. Supplementing Drosophila with this rose extract resulted in a statistically significant decrease in mortality rate in male and female flies. Moreover, the observed anti-aging effects were not associated with common confounds of anti-aging properties, such as a decrease in fecundity or metabolic rate.

Abstract

Genome-wide association studies (GWAS) have uncovered numerous genetic variants (SNPs) that are associated with blood pressure (BP). Genetic variants may lead to BP changes by acting on intermediate molecular phenotypes such as coded protein sequence or gene expression, which in turn affect BP variability. Therefore, characterizing genes whose expression is associated with BP may reveal cellular processes involved in BP regulation and uncover how transcripts mediate genetic and environmental effects on BP variability. A meta-analysis of results from six studies of global gene expression profiles of BP and hypertension in whole blood was performed in 7017 individuals who were not receiving antihypertensive drug treatment. We identified 34 genes that were differentially expressed in relation to BP (Bonferroni-corrected p, Author Summary The focus of blood pressure (BP) GWAS has been the identification of common DNA sequence variants associated with the phenotype; this approach provides only one dimension of molecular information about BP. While it is a critical dimension, analyzing DNA variation alone is not sufficient for achieving an understanding of the multidimensional complexity of BP physiology. The top loci identified by GWAS explain only about 1 percent of inter-individual BP variability. In this study, we performed a meta-analysis of gene expression profiles in relation to BP and hypertension in 7017 individuals from six studies. We identified 34 differentially expressed genes for BP, and discovered that the top BP signature genes explain 5%–9% of BP variability. We further linked BP gene expression signature genes with BP GWAS results by integrating expression associated SNPs (eSNPs) and discovered that one of the top BP loci from GWAS, rs3184504 in SH2B3, is a trans regulator of expression of 6 of the top 34 BP signature genes. Our study, in conjunction with prior GWAS, provides a deeper understanding of the molecular and genetic basis of BP regulation, and identifies several potential targets and pathways for the treatment and prevention of hypertension and its sequelae.

50. Electrical performance characterization for novel multiple compartments shielding and verification on LTE modem SiP

Abstract

This paper describes the performance characterization for a novel compartment shielding process. The CPS process is developed for molded base SiP module which can form multiple arbitrary shape shielded areas. This compartment shielding structure not only occupies the smallest space itself, the good shielding performance can reduce the leakage and increase more placement area by shortening the keep out distance at the sides and edges near the wall between compartments. It reduces the interference between shielded sub-circuits e.g. digital, analog and RF circuits. The compartment shielding structure is seamlessly connected to the grounding of the substrate, hence has superior isolation performance. The shielding wall which isolates the compartments is made by laser scribing and trench filling. First the model of geometry of CPS wall is built up, boundary condition is set and material parameters are assigned. SE performance of CPS is simulated by HFSS software using Finite Element Method (FEM). Manual Mesh is used to improve the accuracy of simulation. Visualized 3D EM field is extracted for analyzing how and to what extent the defects of process degrading the SE. Besides the shielding effectiveness, the conductivity of filling material and the contact resistance between that material and ground pad beneath the CPS trench is studied and correlated to the shielding performance. Finally we improve the shielding performance of CPS and the simulation model we built up is close to measurement. Moreover, a LTE modem with two shielding trenches is made and its performance is verified. This single mode/dual bands LTE modem module with three compartments accommodate digital, RF Tx and RF Rx circuit with size 23×26×1.9 mm only. The RF performance of this modem is excellent, and meets all 3GPP LTE Category 3 and carrier's requirements, passes FCC/CE EMI, GCF and PTCRB certification. It is found that this compartment shielding process is good for consumer grade SiP module both in technology and in commercial terms.
