ContextPrevious research finds that some state policies regarding alcohol use during pregnancy (alcohol/pregnancy policies) increase low birth weight (LBW) and preterm birth (PTB), decrease prenatal care utilization, and have inconclusive relationships with alcohol use during pregnancy.ObjectiveThis research examines whether effects of 8 alcohol/pregnancy policies vary by education status, hypothesizing that health benefits of policies will be concentrated among women with more education and health harms will be concentrated among women with less education.MethodsThis study uses 1972-2015 Vital Statistics data, 1985-2016 Behavioral Risk Factor Surveillance System data, policy data from National Institute on Alcohol Abuse and Alcoholism's Alcohol Policy Information System and original legal research, and state-level control variables. Analyses include multivariable logistic regressions with education-policy interaction terms as main predictors.ResultsThe impact of alcohol/pregnancy policies varied by education status for PTB and LBW for all policies, for prenatal care use for some policies, and generally did not vary for alcohol use for any policy. Hypotheses were not supported. Five policies had adverse effects on PTB and LBW for high school graduates. Six policies had adverse effects on PTB and LBW for women with more than high school education. In contrast, 2 policies had beneficial effects on PTB and/or LBW for women with less than high school education. For prenatal care, patterns were generally similar, with adverse effects concentrated among women with more education and beneficial effects among women with less education. Although associations between policies and alcohol use during pregnancy varied by education, there was no clear pattern.ConclusionsEffects of alcohol/pregnancy policies on birth outcomes and prenatal care use vary by education status, with women with more education typically experiencing health harms and women with less education either not experiencing the harms or experiencing health benefits. New policy approaches that reduce harms related to alcohol use during pregnancy are needed. Public health professionals should take the lead on identifying and developing policy approaches that reduce harms related to alcohol use during pregnancy.

BACKGROUND:Most states have at least one policy targeting alcohol use during pregnancy. The public health impact of these policies has not been examined. We sought to examine the relationship between state-level policies targeting alcohol use during pregnancy and alcohol use among pregnant women. METHODS:Data include state-level alcohol and pregnancy policy data and individual-level U.S. Behavioral Risk Factor Surveillance System data about pregnant women's alcohol use from 1985 to 2016 (N = 57,194). Supportive policies include mandatory warning signs, priority substance abuse treatment, reporting requirements for data and treatment purposes, and prohibitions on criminal prosecution. Punitive policies include civil commitment, Child Protective Services reporting requirements, and child abuse/neglect. Analyses include logistic regression models that adjust for individual- and state-level controls, include fixed effects for state and year, account for clustering by state, and weight by probability of selection. RESULTS:Relative to having no policies, supportive policy environments were associated with more any drinking, but not binge or heavy drinking. Of individual supportive policies, only the following relationships were statistically significant: mandatory warning signs was associated with lower odds of binge drinking, and priority treatment for pregnant women and women with children was associated with higher odds of any drinking. Relative to no policies, punitive policy environments were also associated with more drinking, but not with binge or heavy drinking. Of individual punitive policies, only child abuse/neglect was associated with lower odds of binge and heavy drinking. Mixed policy environments were not associated with any alcohol outcome. CONCLUSIONS:Most policies targeting alcohol use during pregnancy do not seem to be associated with less alcohol consumption during pregnancy.

Abstract
Background: Prior to the U.S. Comprehensive Addiction and Reovery Act of 2016 (CARA) , policymaking on alcohol and drug use during pregnancy was more concentrated on alcohol than drug use policy - although the overlap between the two types of policy was high. Further, the highest levels of legislative activity were requirements to report pregnant women who used alcohol or other drugs and child abuse/neglect policy.Methods: This research brief uses rigorous legal epidemiology methodology to explore state policy activity on alcohol and other drug use during pregnancy after the U.S. Comprehensive Addiction and Recovery Act of 2016 (CARA) amended the 2010 Child Abuse Prevention and Treatment Act Reauthorization (CAPTA).Results: Since CARA, policymaking has been more concentrated on drug policy than alcohol policy, although the overlap between the two is still high. Further, since CARA, states have concentrated policy activity on priority treatment for drugs and reporting requirements.Conclusions: Even though CARA does not require reporting for the purposes of child welfare investigations, several states adopted such requirements during these years. Maxine Beneba Clarke is an African-Caribbean Australian writer, the award-winning author of many books across a range of genres and a distinguished slam poet. In her long poetic memoir "demerara sugar", she addresses the trauma, politics, genres and sites of intergenerational memory of plantation slavery and its legacies. With the prefix "after" in afterlives of slavery, the essay draws out aspects of Clarke's project: representing the wake of slavery; going in search of memory of the lived experience of enslavement; finding new routes of memory work. Her routes take her from Melbourne to London, Liverpool and Birmingham, through slavery museums, archives, family interview, city streets and air and train journeys. This essay addresses the dynamics of haunting, rememory, and (un)belonging in the poem by exploring the chronotopes of the museum/archive and the city street. [ABSTRACT FROM AUTHOR]

BackgroundPolicies regarding alcohol use during pregnancy continue to be enacted and debated in the United States. Purpose: Alcohol use during pregnancy is a significant public health concern. Nearly all U.S. states have enacted policies targeting alcohol use during pregnancy, but there has been little research examining their impact, particularly across racial/ethnic groups. Methods: Using data from the Behavioral Risk Factor Surveillance System and about eight state-level, pregnancy-specific alcohol policies from 1985 to 2016, the aim of this study was to examine the differential effects of these policies on drinking among pregnant women by race/ethnicity. Results: We found evidence of differential effects for priority treatment, prohibitions on criminal prosecution, and civil commitment policies. In relation to priority treatment policies, effects benefited versus harmed different racial/ethnic groups depending on whether the priority treatment policies were for pregnant women only or if they gave priority to both pregnant women and pregnant women with children. Conclusions: Findings from this study suggest that benefits and harms from these policies do not appear to be equitably distributed across different racial/ethnic groups. Research considering the impact of alcohol/pregnancy policies should consider differential effects by race/ethnicity.

AimsAlcohol consumption during pregnancy remains a public health problem despite >40 years of attention. Little is known about how state policies have evolved and whether policies represent public health goals or efforts to restrict women's reproductive rights.MethodsOur data set includes US state policies from 1970 through 2013 obtained through original legal research and from the National Institute for Alcohol Abuse and Alcoholism's (NIAAA)'s Alcohol Policy Information System. Policies were classified as punitive to women or supportive of them. The association between numbers of punitive policies and supportive policies in 2013 with a measure of state restrictions on reproductive rights and Alcohol Policy Effectiveness Scores (APS) was estimated using a Pearson's correlation.ResultsThe number of states with alcohol and pregnancy policies has increased from 1 in 1974 to 43 in 2013. Through the 1980s, state policy environments were either punitive or supportive. In the 1990s, mixed punitive and supportive policy environments began to be the norm, with punitive policies added to supportive ones. No association was found between the number of supportive policies in 2013 and a measure of reproductive rights policies or the APS, nor was there an association between the number of punitive policies and the APS. The number of punitive policies was positively associated, however, with restrictions on reproductive rights.ConclusionPunitive alcohol and pregnancy policies are associated with efforts to restrict women's reproductive rights rather than effective efforts to curb public health harms due to alcohol use in the general population. Future research should explore the effects of alcohol and pregnancy policies.Short summaryThe number of states with alcohol and pregnancy policies has increased since 1970 (1 in 1974 and 43 in 2013). Alcohol and pregnancy policies are becoming increasingly punitive. - Abstract
The aim of this article is to illustrate the experienced and optimal patient journeys of people with multiple sclerosis. A review of research published in the British Journal of Neuroscience Nursing in 2024 was conducted to build on the insights and provide guidance around each part of the patient journey, from diagnosis to care. A composite patient journey was created based on the experiences of the MS patients interviewed as part of the ALERT study. Many people with multiple sclerosis experience delayed diagnosis and management of bladder and bowel dysfunction, leading to significant physical and psychosocial consequences. The composite patient journey showed common clinical touchpoints where care was sub-optimal. There was a lack of early discussions about bladder and bowel issues, insufficient patient education and missed opportunities for intervention. Early diagnosis, patient education and integrated care pathways are essential for managing neurogenic bladder and bowel dysfunction in people with multiple sclerosis. By addressing these issues proactively, healthcare providers can improve patient outcomes and quality of life. Policy initiatives should focus on increasing awareness, education and developing clinical guidelines to support effective bladder and bowel management. Using an original 2021 survey of mayors in U.S. mayors in cities of 10,000 in population and above, we explore the individual costs of psychological and physical violence. These costs include emotional distress, diverted attention from the job, and considerations about leaving the job. Overall, we find that 95 percent of mayors faced violence in office. Among those who did, the more violence reported, the more likely they were to have faced such costs. Further, those mayors (mostly women) who faced sexualized violence were more likely to have reported those effects. All told, violence against officeholders has individual effects as well as systemic ones. If violence keeps mayors from being able to do their job fully or causes them to leave it, the political costs may be quite grave in terms of losing experienced representatives—and it may also have disparate effects on particular groups of officeholders, such as women. Plastic waste and the bee population are two topics often discussed in relation to the environment. A practical activity in a primary school classroom linked to these areas can help to fulfil some of the requirements of the science curriculum. A study of the life of bees shows the importance of pollinators and highlights the concern many people have for keeping the environment clean and safe for the wildlife. In this article, the author outlines how making beeswax wraps can support learning science in school or at home.

25. Taking Teachers out of the Equation: Constructions of Teachers in Education Policy Documents over a Ten-Year Period

Education policy documents recently have placed great emphasis on teacher quality in the belief that "education of the highest quality requires teachers of the highest quality" (Department of Employment Education Training and Youth Affairs 2000). This paper traces the discourses on teachers constructed in policy documents in order to establish what is meant by "teachers of the highest quality". It employs Critical Discourse Analysis to investigate the discursive construction of teachers' professional identities in three policy documents released over the last decade. This analysis finds that, despite recognition being given to the importance of teachers in all three documents, teachers' professional autonomy is effectively curtailed as they are increasingly being "taken out of the equation" in education policy decision-making. The paper concludes with suggestions for ways in which teachers may challenge these constructions and work to reconstruct teachers as active voices in the policy-making process. (Contains 1 endnote.)

This article describes U.S. state policies related to alcohol use during pregnancy, using data from the National Institute on Alcohol Abuse and Alcoholism (NIAAA) Alcohol Policy Information System (APIS). Specifically, this study examines trends in policies enacted by states over time and types of policies enacted across states in the U.S., with a focus on whether laws were supportive or punitive toward women. Findings revealed substantial variability in characteristics of policies (19 primarily supportive, 12 primarily punitive, 12 with a mixed approach, and 8 with no policies). Findings underscore the need to examine possible consequences of policies, especially of punitive policies and "mixed" approaches.

The publication of NHS operational planning guidance is a significant moment in the health service calendar as it sets out the NHS priorities for the coming year and dictates what the strategic focus should be for health services. The devolved nations also publish similar policy documents, which outline the key priorities for NHS spending and focus. These policy documents are important for all neurology services as they set out the key areas of activity for healthcare and what services will be measured against. This article explores the important role of Parkinson's Nurse Specialists (PNS) in the management of Parkinson's disease (PD). PD is a degenerative brain disease that affects both motor and non-motor symptoms, impacting a person's quality of life and increasing care requirements. The PNS role has evolved in the UK, providing clinical, educational, research, and consultation support to patients. While there is limited evidence on the efficacy of the PNS role, studies have shown positive outcomes in terms of patient well-being and quality of life. However, there is a global shortage of PNS, particularly in low and middle-income countries, due to factors such as nurse shortages and inadequate funding. Although evidence-based parenting programs are widely available, they remain inaccessible to many families of children with social, emotional and behavioural problems, particularly in remote settings. In partnership with Aboriginal communities in the remote Fitzroy Valley, Western Australia, we introduced and evaluated the Indigenous Triple P – Positive Parenting Program, with additional Stepping Stones Triple P strategies (for children with complex needs). Implementation followed community consultation and program adaptation to acknowledge local culture, social complexities and language. The initiative was named Jandu Yani U, meaning "for all families" in the local Bunuba language. Local practitioners (parent coaches) were trained to deliver Triple P. Parents and carers (henceforth carers) of children up to 15 years of age were invited to participate. Families attending the program reported on child behaviour, parenting practices, self-efficacy, wellbeing and empowerment pre- and post-intervention and at 8-month follow-up using standardised assessment tools delivered by local community navigators. Of 30 participating families, 100% completed Triple P and reported the program was culturally appropriate. Of the index children, 93% were Aboriginal, 63% boys, median age 6.83 years. Following the program, carers reported high levels of empowerment and reaching goals for behaviour change (p <.001) and less use of unhelpful parenting strategies (over-reactivity and hostility; p <.05), and this maintained at 8-month follow-up (p <.001). At follow-up, improvement in carer self-efficacy (p <.001) and reduced anxiety (p <.001) were documented. Significant improvements in prosocial behaviour, reductions in challenging behaviour and decreased intensity and frequency of behavioural problems (all p <.05) were reported in children at follow-up. The average level of goal attainment was 65% at post, which was maintained at follow-up. This study demonstrates the feasibility of delivering Indigenous Triple P to Aboriginal families in remote settings, and the importance of partnership with communities. Program tailoring, flexible delivery, use of Aboriginal parent coaches and provision of practical support to families resulted in a culturally acceptable parent support program. With local Aboriginal co-design, this initiative should be generalisable to similar remote communities elsewhere. What is already known about the topic: Aboriginal and Torres Strait Islander families are more often exposed to stressful life events than non-Indigenous families, with children overrepresented in hospitalisation rates, poor health outcomes, early school dropout, out-of-home care, involvement with the justice system and youth suicide. There is a significant evidence base that parenting programs, such as Triple P, built on the principles of social learning and self-regulation, can be effective in addressing emotional and behavioural problems in children in the short and longer term. Despite the availability of evidence-based parenting programs, a minority of parents of children with behavioural and emotional problems access such programs, and access by Aboriginal and Torres Strait Islander families is lower. What this topic adds: This study determined the feasibility of delivering a community tailored, evidence-based parenting program to families in a very remote Western Australian Aboriginal community. Families reported significant reductions in unhelpful parenting strategies, carer anxiety and challenging child behaviours and significant increases in parenting confidence and empowerment and prosocial child behaviour following the program. Co-design methods, community-led modifications to resources, two-way learning with Aboriginal and non-Indigenous trainers, training local parent coaches, flexible program delivery and long-term practical support were essential for delivering a culturally acceptable parenting program implementation strategy that could be adapted for remote communities elsewhere. The publication of our NHS Long Term Workforce Plan (NHS England, 2023) is perhaps one of the most seminal moments in the 75-year history of the NHS. This is the first time the government has asked the NHS to come up with a comprehensive workforce plan; a once-in-a-generation opportunity to put staffing on a sustainable footing and improve patient care. The future outlook for the nursing workforce holds both promise and challenges as the healthcare landscape continues to evolve. With ongoing shifts in demographics, technology, and healthcare delivery models, nurses play a crucial role in providing quality patient care. But does the NHS know how many nurses are currently working in neurology? Are there enough to manage the care that is required? Sue Thomas highlights the current challenges facing the neurology and nursing workforce, trends in nursing employment, the impact of technology and highlights the lack of information about our current neurology nursing numbers and suggests that strategies are needed to address neurology nursing shortages. Climate change is a significant global health threat, projected to cause over 250000 deaths annually by 2050, as a result of factors such as extreme heat, flooding and poor air quality caused by pollution. The NHS, as a major healthcare provider, contributes around 4% of the UK's total emissions and in October 2020, became the world's first health service to commit to reaching net zero carbon, in response to the profound and growing threat to health posed by climate change. Nurses, as frontline healthcare providers, are uniquely positioned to drive sustainability initiatives within healthcare settings, reducing environmental impact while improving patient outcomes. - Abstract
This guide provides background information and youth activities for youth leaders and classroom teachers interested in integrating waste management issues into current educational programming. Five parts suggest ideas for action that youth can take at home, in their group, or in the community to solve waste management problems. Part 1 introduces young people to six waste management issues: waste source reduction, reuse, recycling, composting, incineration, and landfilling. Part 2 presents 12 activities for use in class and at home that address the six issues discussed in Part 1. Each activity includes preparation facts, steps on implementing the activity, questions that stimulate participant reflection, and ideas on taking action at home, within the group, and in the community. Part 3 focuses on actions children can take to solve solid waste problems. The actions are separated into three categories and include: (1) 14 actions for groups; (2) over 100 actions to reduce, reuse, and recycle waste for projects related to animal sciences, expressive arts, home economics, mechanical sciences, natural resources, personal growth and development, and plant sciences; and (3) 56 community service ideas. Part 4, a resources section, lists major organizational affiliation; resource people; curriculum, posters and games; periodicals; books; and audio visual materials. 50. Mature Age Professionals: Factors Influencing Their Decision to Make a Career Change into Teaching

- Abstract
This paper presents the early findings from a study that addresses the topic of mature age professionals making a career change into the secondary teaching profession by undertaking a postgraduate coursework initial teacher education program. The paper specifically addresses the factors that affect the decision for mature age professionals to make such a change. Education systems worldwide are encouraging mature age professionals to make a career change into secondary teaching to fill teacher shortages in specialist subject areas. However, the literature shows there has been little recent research about this aspect of mature age career changers. The study on which this paper is based utilised a qualitative, narrative inquiry approach to explore the stories of mature age professionals who were making a career change into teaching. The paper focuses on the resonances across the stories of 17 mature age professionals as they outline the factors that influenced their decision to make a career change into teaching. The findings of this study would be of interest to educational policy makers and academic administrators responsible for the recruitment of mature age professionals into secondary teaching.
