Contains fulltext : 291061.pdf (Publisher’s version ) (Open Access), BACKGROUND: Dementia-friendly communities (DFCs) are seen as key to the inclusion and participation of people with dementia and carers. Dementia-friendly initiatives (DFIs) are important building blocks for the growth of DFCs. The collaboration between different stakeholders is a central aspect in developing and sustaining DFIs. AIM: This study tests and refines an initial theory about collaborating for DFIs with special attention for the involvement of people with dementia and their carers during the collaboration for DFIs. The realist approach is used for deepening contextual aspects, mechanisms, outcomes, and its explanatory power. METHODS: A participatory case study design using qualitative data (focus groups, observations, reflections, minutes from meetings, and exit interviews) was executed in four Dutch municipalities that have ambitions to become dementia- friendly communities. RESULTS: The refined theory on the collaboration for DFIs incorporates contextual aspects such as diversity, shared insights, and clarity. It draws attention to the importance of mechanisms such as the recognition of efforts and progress, informal distributed leadership, interdependency, belonging, significance, and commitment. These mechanisms resonate with feeling useful and feeling collectively powerful in the collaboration. The outcomes of collaboration were activation, getting new ideas, and fun. Our findings address how stakeholders' routines and perspectives impact the involvement of people with dementia and their carers during collaboration. CONCLUSION: This study provides detailed information about collaboration for DFIs. The collaboration for DFIs is largely influenced by feeling useful and collectively powerful. Further research is needed to understand how these mechanisms can be triggered with the involvement of people with dementia and their carers in the heart of the collaboration.

Background: The spiral ganglion hypothesis suggests that pathogenic variants in genes preferentially expressed in the spiral ganglion nerves (SGN), may lead to poor cochlear implant (CI) performance. It was long thought that TMPRSS3 was particularly expressed in the SGNs. However, this is not in line with recent reviews evaluating CI performance in subjects with TMPRSS3-associated sensorineural hearing loss (SNHL) reporting overall beneficial outcomes. These outcomes are, however, based on variable follow-up times of, in general, 1 year or less. Therefore, we aimed to 1. evaluate long-term outcomes after CI implantation of speech recognition in quiet in subjects with TMPRSS3-associated SNHL, and 2. test the spiral ganglion hypothesis using the TMPRSS3-group. Methods: This retrospective, multicentre study evaluated long-term CI performance in a Dutch population with TMPRSS3-associated SNHL. The phoneme scores at 70 dB with CI in the TMPRSS3-group were compared to a control group of fully genotyped cochlear implant users with post-lingual SNHL without genes affecting the SGN, or severe anatomical inner ear malformations. CI-recipients with a phoneme score ≤ 70% at least 1-year post-implantation were considered poor performers and were evaluated in more detail. Results: The TMPRSS3 group consisted of 29 subjects (N = 33 ears), and the control group of 62 subjects (N = 67 ears). For the TMPRSS3-group, we found an average phoneme score of 89% after 5 years, which remained stable up to 10 years post-implantation. At both 5 and 10-year follow-up, no difference was found in speech recognition in quiet between both groups (p = 0.830 and p = 0.987, respectively). Despite these overall adequate CI outcomes, six CI recipients had a phoneme score of ≤ 70% and were considered poor performers. Contains fulltext : 286715.pdf (Publisher’s version ) (Open Access), INTRODUCTION: Different multimodal pain management strategies following total hip arthroplasty(THA) and total knee arthroplasty (TKA) surgery are used in clinical practice. The optimal pain management strategy, however, remains unclear. This study aims to evaluate the differences in perioperative multimodal pain management strategies for THA and TKA in the Netherlands, and studies the associations between patient- and therapy related factors and pain outcomes. METHODS: Data from the Dutch hospitals in the PAIN OUT network were used in this study. Demographic data, pain management strategy including perioperative medication use and anesthetic techniques were recorded and used in a multivariable regression analysis to study the association with maximum pain intensity, the duration of severe pain, pain interference in bed and postoperative nausea. RESULTS: In 343 hip arthroplasty patients and 301 knee arthroplasty patients in seven hospitals, respectively 28 and 35 different combinations of analgesic regimens were used. The number of different drugs prescribed was not related to postoperative pain intensity. Female sex, younger age and spinal anesthesia were associated with higher postoperative maximum pain scores (Numeric Rating Scale (NRS) > 5). Hip surgery and ketamine use were associated with lower postoperative pain scores. The use of non-steroidal anti-inflammatory drugs (NSAIDs) and gabapentinoids, higher age, higher body mass index (BMI) and male gender were associated with less postoperative nausea (NRS < 3). CONCLUSION: In conclusion, our study demonstrated a large diversity of analgesic strategies following total joint arthroplasties in the Netherlands. Although no ideal strategy was identified, the use of NSAIDs, ketamine and dexamethasone were associated with less pain and less side effects.

Item does not contain fulltext, INTRODUCTION/AIMS: Oxaliplatin often causes acute or chronic peripheral neuropathy in patients with an intestinal or pancreatic tumor, but in-depth insights in its influence on quality of life (QoL) are lacking. We explored the influence of acute oxaliplatin-induced peripheral neuropathy (OIPN) on daily QoL in these patients. METHODS: We performed semistructured interviews with a purposive sample of patients receiving oxaliplatin and possibly experiencing acute OIPN. Interviews were audio-recorded, transcribed verbatim, and coded by two researchers. Data were analyzed by using the constant comparative method for content analysis with ATLAS.ti software. RESULTS: After nine patients, saturation took place. In total, 11 patients were interviewed. Four themes were extracted from the data: (1) adverse effects, (2) physical (un)well-being, (3) emotional aspects, and (4) treatment aspects. All participants were suffering from acute OIPN to a certain extent, leading to restrictions in daily activities such as household chores, but also to a decrease in mobility and independency. Other adverse effects such as general malaise and gastrointestinal side effects also influenced the participants' well-being, as did the diagnosis and prognosis of their disease. CONCLUSION: Acute OIPN, together with other side effects of chemotherapeutic treatment and the difficulties that come with the diagnosis of cancer and its prognosis, largely influences patients' daily QoL. Managing expectations (by patient education) seems important.

Met 'natuurlijk kapitaal' wordt de waarde uitgedrukt die de natuur heeft voor de samenleving, zowel geldelijk als niet-geldelijk. Het draagt bij aan het welzijn en de welvaart van de mens, bijvoorbeeld in de vorm van schone lucht, drinkwater en voedsel, maar ook door duinen die ervoor zorgen dat ons land niet overstroomt. Op dit moment wordt niet duurzaam omgegaan met natuurlijk kapitaal in de wereld, met schaarste van grondstoffen, overbevissing, uitputting van de bodem en klimaatverandering als gevolg. Het RIVM heeft onderzocht of de nieuw in te voeren Omgevingswet regels stelt om duurzaam met natuurlijk kapitaal om te gaan. Deze wet blijkt overheden wel de mogelijkheid te bieden om aandacht te besteden aan natuurlijk kapitaal, maar stelt daarvoor weinig regels. Doelen om het natuurlijk kapitaal te beschermen zouden het beste in de omgevingsvisie kunnen worden opgenomen; in dit instrument van de Omgevingswet leggen overheden hun doelen voor de omgeving vast. Duidelijke en concrete regels in de Omgevingswet zijn belangrijk omdat in een samenleving het natuurlijk kapitaal niet automatisch goed wordt beheerd. Dat komt doordat iedereen er vrijelijk gebruik van kan maken zonder ervoor te hoeven betalen. Ook zijn schadelijke effecten of de eindigheid van het natuurlijk kapitaal pas na een lange tijd merkbaar, zoals bij het gebruik van fossiele brandstoffen. Deze geleidelijke processen zijn niet of moeilijk terug te draaien. Om dit in goede banen te leiden, is het dus noodzakelijk tijdig maatregelen te nemen. Dit is ook van belang om aan internationale verplichtingen te voldoen, zoals het Klimaatakkoord van Parijs. Om duurzaam met het natuurlijk kapitaal om te kunnen gaan, hebben overheden voldoende informatie nodig over hun natuurlijk kapitaal, bijvoorbeeld om natuurlijk kapitaal in de omgevingsvisie op te nemen. Daarnaast biedt het overheden handvatten voor keuzes om hun grondgebied duurzaam(er) te beheren. - Abstract
Background: RNA interference (RNAi) is a natural gene modulation process that was recently approved for clinical use and has great potential for treatment of many pathological conditions. The surface receptor FAS and tumor suppressor p53 are implicated in ischemic liver damage. Both may be silenced via RNAi with limited off-target effects. We determined the efficiency of FAS siRNA delivery with lipid nanoparticles during normothermic (37°C) and hypothermic (4°C) ex vivo machine perfusion. We also show silencing p53 via siRNA reduces inflammation and caspase expression during liver ischemia. Methods: Rat portal veins were cannulated (Fig. 1A) and perfused at 10mmHg on a closed-loop pump circuit regulated by circulating water bath (Fig. 1B). Controls were perfused with Williams E+10U insulin and lipid nanoparticles (Fig. 1C). Normothermic livers were perfused with medium plus nanoparticle/FAS-siRNA complex with 3‘-AlexaFluor-555 modification at 37°C. Hypothermic livers were perfused similarly at 4°C. Biopsies were collected at 4 hours, formalin fixed, stained, then imaged with confocal microscopy. For p53 studies, siRNA was delivered 24 hours before hilum clamping. Tissue/blood was collected 3 days after siRNA injection. Results: Compared to empty vector controls (Fig. 2A-D), normothermic (Fig. 2E-H) and hypothermic livers (Fig. 2I-L) perfused with FAS siRNA demonstrate diffuse uptake in sinusoids and surrounding central veins. SiRNA is observed together in sinusoid membranes and in hepatocyte cytoplasm. Livers pretreated with p53 siRNA showed reduced cellular infiltration, vacuolization, and fewer caspase-3 positive cells. Conclusions: By incorporating RNAi into preservation solutions or delivering before procurement, we may reliably improve the quality of livers during the ischemic period. It will be important to determine the ideal perfusion temperature. Further studies will quantify uptake with peptide-nucleic acid hybridization at both perfusion temperatures and the degree of FAS or p53 silencing in a transplant model.

Ook zijn schadelijke effecten of de eindigheid van het natuurlijk kapitaal pas na een lange tijd merkbaar, zoals bij het gebruik van fossiele brandstoffen. Deze geleidelijke processen zijn niet of moeilijk terug te draaien. Om dit in goede banen te leiden, is het dus noodzakelijk tijdig maatregelen te nemen. Dit is ook van belang om aan internationale verplichtingen te voldoen, zoals het Klimaatakkoord van Parijs. Om duurzaam met het natuurlijk kapitaal om te kunnen gaan, hebben overheden voldoende informatie nodig over hun natuurlijk kapitaal, bijvoorbeeld om natuurlijk kapitaal in de omgevingsvisie op te nemen. Daarnaast biedt het overheden handvatten voor keuzes om hun grondgebied duurzaam(er) te beheren. Deze informatie is nu niet altijd gemakkelijk toegankelijk; de, Natural capital consists of all renewable and non-renewable resources and processes from the environment that provides products or services. It contributes to the welfare and prosperity of human beings, for example in the form of clean air, drinking water and food, but also by dunes that ensure that our country does not overflow. At present, the natural capital in the world is used in an unsustainable way, with resource scarcity, overfishing, soil depletion and climate change as a result. RIVM has investigated whether the new environmental and planning act introduces rules to use the natural capital sustainably. The new law offers the opportunity to pay attention to natural capital, but proposes few rules therefor. Objectives to sustain the natural capital can be included best in the 'Omgevingsvisie' (Environmental & Planning Vision) in which governments set their goals for the area. Clear and concrete rules in the environmental and planning act are important because the natural capital is not managed automatically well. That's because everyone can freely use it without having to pay for it. Also, adverse effects or the finite nature of natural capital are only noticeably after a long time, as in the use of fossil fuels. These gradual processes are difficult or impossible to reverse. Therefore it is necessary to take measures timely in order to manage the natural capital properly. This is also important in order to meet international obligations, such as the Climate Agreement of Paris. Governments need adequate information in order to manage their natural capital sustainably and include specific objectives in their Environmental & Planning Vision. In addition, good information offers the government direction to manage their territory sustainably. Background: The spiral ganglion hypothesis suggests that pathogenic variants in genes preferentially expressed in the spiral ganglion nerves (SGN), may lead to poor cochlear implant (CI) performance. It was long thought that TMPRSS3 was particularly expressed in the SGNs. However, this is not in line with recent reviews evaluating CI performance in subjects with TMPRSS3-associated sensorineural hearing loss (SNHL) reporting overall beneficial outcomes. These outcomes are, however, based on variable follow-up times of, in general, 1 year or less. Therefore, we aimed to 1. evaluate long-term outcomes after CI implantation of speech recognition in quiet in subjects with TMPRSS3-associated SNHL, and 2. test the spiral ganglion hypothesis using the TMPRSS3-group. Methods: This retrospective, multicentre study evaluated long-term CI performance in a Dutch population with TMPRSS3-associated SNHL. The phoneme scores at 70 dB with CI in the TMPRSS3-group were compared to a control group of fully genotyped cochlear implant users with post-lingual SNHL without genes affecting the SGN, or severe anatomical inner ear malformations. CI-recipients with a phoneme score ≤ 70% at least 1-year post-implantation were considered poor performers and were evaluated in more detail. Results: The TMPRSS3 groupconsisted of 29 subjects (N = 33 ears), and the control group of 62 subjects (N = 67 ears). For the TMPRSS3-group, we found an average phoneme score of 89% after 5 years, which remained stable up to 10 years post-implantation. At both 5 and 10-year follow-up, no difference was found in speech recognition in quiet between both groups (p= 0.830 and p= 0.987, respectively). Despite these overall adequate CI outcomes, six CI recipients had a phoneme score of ≤ 70% and were considered poor performers. The latter was observed in subjects with residual hearing post-implantation or older age at implantation. Conclusion: Subjects with TMPRSS3-associated SNHL have adequate and stable long-term outcomes after cochlear implantation, equal to the performance of genotyped patient with affected genes not expressed in the SGN. These findings are not in line with the spiral ganglion hypothesis. However, more recent studies showed that TMPRSS3 is mainly expressed in the hair cells with only limited SGN expression. Therefore, we cannot confirm nor refute the spiral ganglion hypothesis.

Medio dit jaar gaat de eerste online versie van de Atlas Natuurlijk Kapitaal ‘live’. Deze basisversie van de Atlas geeft informatie over het natuurlijk kapitaal van Nederland. U vindt er kaarten over ecosystemen, hun kwaliteit, én hun capaciteit om ecosysteemdiensten te leveren. De Atlas laat ook inspirerende voorbeelden zien van duurzaam gebruik van ons natuurlijk kapitaal. Deze eerste versie biedt informatie voor waterbeheerders en gebiedsontwikkelaars en voor rapportage aan de Europese Commissie. Daarna zal de Atlas Natuurlijk Kapitaal ook informatie voor andere doelgroepen bieden, zoals het bedrijfsleven en vergunningverleners. Op termijn zal de atlas een kennisbasis vormen voor duurzame besluitvorming in de leefomgeving.

DESIGN: Cost effectiveness study alongside a single blind randomised controlled trial. SETTING: Memory clinic, day clinic of a geriatrics department, and participants' homes. Patients 135 patients aged > or =65 with mild to moderate dementia living in the community and their primary care givers. INTERVENTION: 10 sessions of occupational therapy over five weeks, including cognitive and behavioural interventions, to train patients in the use of aids to compensate for cognitive decline and care givers in coping behaviours and supervision. MAIN OUTCOME MEASURES: Incremental cost effectiveness ratio expressed as the difference in mean total care costs per successful treatment (that is, a combined patient and care giver outcome measure of clinically relevant improvement on process, performance, and competence scales) at three months after randomisation. Bootstrap methods used to determine confidence intervals for these measures. RESULTS: The intervention cost 1183 euros (848 pounds sterling, $1738) (95% confidence interval 1128 euros (808 pounds sterling, $1657) to 1239 euros (888 pounds sterling, $1820)) per patient and primary care giver unit at three months. Visits to general practitioners and hospital doctors cost the same in both groups but total mean costs were 1748 euros (1279 pounds sterling, $2621) lower in the intervention group, with the main cost savings in informal care. There was a significant difference in proportions of successful treatments of 36% at three months. The number needed to treat for successful treatment at three months was 2.8 (2.7 to 2.9). CONCLUSIONS: Community occupational therapy intervention for patients with dementia and their care givers is successful and cost effective, especially in terms of informal care giving.

PARTICIPANTS: 135 patients aged > or =65 with mild to moderate dementia living in the community and their primary care givers. INTERVENTIONS: 10 sessions of occupational therapy over five weeks, including cognitive and behavioural interventions, to train patients in the use of aids to compensate for cognitive decline and care givers in coping behaviours and supervision. MAIN OUTCOME MEASURES: Patients' daily functioning assessed with the assessment of motor and process skills (AMPS) and the performance scale of the interview of deterioration in daily activities in dementia (IDDD). Care giver burden assessed with the sense of competence questionnaire (SCQ). Participants were evaluated at baseline, six weeks, and three months. RESULTS: Scores improved significantly relative to baseline in patients and care givers in the intervention group compared with the controls (differences were 1.5 (95% confidence interval 1.3 to 1.7) for the process scale; -11.7 (-13.6 to -9.7) for the performance scale; and (11.0; 9.2 to 12.8) for the competence scale). This improvement was still significant at three months. The number needed to treat to reach a clinically relevant improvement in motor and process skills score was 1.3 (1.2 to 1.4) at six weeks. Effect sizes were 2.5, 2.3, and 1.2, respectively, at six weeks and 2.7, 2.4, and 0.8, respectively, at 12 weeks. CONCLUSIONS: Occupational therapy improved patients' daily functioning and reduced the burden on the care giver, despite the patients' limited learning ability. Echte overlast in de vorm van lawaai, stof en stank behoort in het merendeel van de gevallen tot het verleden. Maar er dienen zich nieuwe en complexere uitdagingen aan: klimaatverandering, het doorbreken van de stikstofcyclus en het grote verlies aan biodiversiteit. Deze mondiale problemen lossen we met het klassieke milieubeleid niet op, daarvoor zijn wijzigingen van het economisch bestel nodig. De kern van ons leefmilieu, de ecosystemen, bieden ons allerlei diensten die van essentieel belang zijn voor ons welzijn. Deze ecosysteemdiensten hebben momenteel nauwelijks een positie in ons economisch verkeer, er hangt geen prijskaartje aan. Daardoor kunnen deze diensten schijnbaar kosteloos worden vernietigd. Schijnbaar, want uiteindelijk zal de prijs worden betaald in de vorm van welvaartverlies voor de hele maatschappij. We zullen er dus voor moeten zorgen dat ecosysteemdiensten beter in het economisch verkeer tot hun recht komen. Dat kan onder andere door de kennis en het bewustzijn te vergroten, door doelen te stellen en te bewaken en door klassieke economische instrumenten in te zetten.
