- Abstract
Background: An economic evaluation was performed alongside an RCT investigating flap fixation in reducing seroma formation after mastectomy. The evaluation focused on the first year following mastectomy and assessed cost-effectiveness from a health care and societal perspective. Methods: The economic evaluation was conducted between 2014 and 2018 in four Dutch breast clinics. Patients with an indication for mastectomy or modified radical mastectomy were randomly assigned to: conventional closure (CON), flap fixation with sutures (FFS) or flap fixation with tissue glue (FFG). Health care costs, patient and family costs and costs due to productivity losses were assessed. Outcomes were expressed in incremental cost-effectiveness ratios (ICERs): the incremental cost per quality-adjusted life year (QALY). Bootstrapping techniques, sensitivity and secondary analyses were employed to address uncertainty. Results: The FFS-group yielded most QALYs (0.810; 95%-CI 0.755–0.856), but also incurred the highest mean costs at twelve months (€10.416; 95%-CI 8.231–12.930). CON was the next best alternative with 0.794 QALYs (95%-CI 0.733–0.841) and mean annual costs of €10.051 (95%-CI 8.255–12.044). FFG incurred fewer QALYs and higher costs, when compared to the CON group. The ICER of FFS compared to CON was €22.813/QALY. Applying a willingness to pay threshold in the Netherlands of €20.000/QALY, the probability that FFS was cost-effective was 42%, compared to 37% and 21% for CON and FFG, respectively. Conclusion: The cost-effectiveness of FFS following mastectomy, versus CON and FFG, is uncertain from a societal perspective. Yet, from a health care and hospital perspective FFS is likely to be the most cost-effective intervention.

METHODS AND RESULTS: In the DUTCH-AF registry, patients with newly diagnosed AF (<6 months) are prospectively enrolled. Label adherence to NOAC dosing was assessed using the European Medicines Agency labelling. Factors associated with off-label dosing were explored by multivariable logistic regression analyses. From July 2018 to November 2020, 4500 patients were registered. The mean age was 69.6 ± 10.5 years, and 41.5% were female. Of the 3252 patients in which NOAC label adherence could be assessed, underdosing and overdosing were observed in 4.2% and 2.4%, respectively. In 2916 (89.7%) patients with a full-dose NOAC recommendation, 4.6% were underdosed, with a similar distribution between NOACs. Independent determinants (with 95% confidence interval) were higher age [odds ratio (OR): 1.01 per year, 1.01-1.02], lower renal function (OR: 0.96 per ml/min/1.73 m(2), 0.92-0.98), lower weight (OR: 0.98 per kg, 0.97-1.00), active malignancy (OR: 2.46, 1.19-5.09), anaemia (OR: 1.73, 1.08-2.76), and concomitant use of antiplatelets (OR: 4.93, 2.57-9.46). In the 336 (10.3%) patients with a reduced dose NOAC recommendation, 22.9% were overdosed, most often with rivaroxaban. Independent determinants were lower age (OR: 0.92 per year, 0.88-0.96) and lower renal function (OR: 0.98 per ml/min/1.73 m(2), 0.96-1.00). CONCLUSION: In newly diagnosed Dutch AF patients, off-label dosing of NOACs was seen in only 6.6% of patients, most often underdosing. In this study, determinants of off-label dosing were age, renal function, weight, anaemia, active malignancy, and concomitant use of antiplatelets.

Door het verwerven van geletterdheid kunnen deze kinderen zelfstandig en onafhankelijk leren communiceren en optimaal deelnemen aan onderwijs, sociale interactie en het dagelijks leven in de samenleving. Het onderzoek gerapporteerd in dit artikel was een pilotstudie naar het effect van de toepassing van een op deze doelgroep aangepaste werkwijze voor het technisch leesonderwijs van mkmm-woorden met teksten en oefeningen uit de reguliere leesmethode Veilig leren lezen, de kim-versie. Daarnaast is ook een aangepaste werkwijze toegepast voor het begrijpend leesonderwijs. Het onderzoek is uitgevoerd bij twee kinderen die niet of nauwelijks spreken en gebruik maken van een ooggestuurd elektronisch communicatiehulpmiddel met spraakuitvoer. Beide participanten lieten vooruitgang zien op het automatisch herkennen, decoderen en spellen van de doelwoorden en -zinnen. Bij het begrijpend lezen werden wisselende resultaten verkregen en was er geen duidelijke vooruitgang zichtbaar. Verdere ontwikkeling, toepassing en effectevaluatie van de hier toegepaste werkwijze is noodzakelijk. 23 p.

8th World Conference on Photovoltaic Energy Conversion; 1099-1104, In this work we give an overview of the module monitoring results achieved within the H2020 funded project called HighLite. A dedicated online monitoring platform has been developed for the project to group and treat the data from all partners with homogeneous procedures. Monitoring of advanced prototype modules developed in the project and based on various cells and interconnection approaches, like silicon-heterojunction (SHJ)-shingle, TopCon-shingle, cut interdigital back-contact cells (cut-IBC) are reported. We describe key learnings from monitoring modules for various applications including building-applied PV (BAPV) and building-integrated PV (BIPV). Annual energy yield up to 1900 kWh/kWp is expected in Cadarache (France) based on HighLite PV technology. Temperature corrected performance ratios are used for a ranking of the new technologies against the reference PERC modules.

- Abstract
Background: Real-world data on non-vitamin K oral anticoagulants (NOACs) are essential in determining whether evidence from randomised controlled clinical trials translate into meaningful clinical benefits for patients in everyday practice. RIVER (RIVaroxaban Evaluation in Real life setting) is an ongoing international, prospective registry of patients with newly diagnosed non-valvular atrial fibrillation (NVAF) and at least one investigator-determined risk factor for stroke who received rivaroxaban as an initial treatment for the prevention of thromboembolic stroke. The aim of this paper is to describe the design of the RIVER registry and baseline characteristics of patients with newly diagnosed NVAF who received rivaroxaban as an initial treatment. Methods and results: Between January 2014 and June 2017, RIVER investigators recruited 5072 patients at 309 centres in 17 countries. The aim was to enroll consecutive patients at sites where rivaroxaban was already routinely prescribed for stroke prevention. Each patient is being followed up prospectively for a minimum of 2-years. The registry will capture data on the rate and nature of all thromboembolic events (stroke / systemic embolism), bleeding complications, all-cause mortality and other major cardiovascular events as they occur. Data quality is assured through a combination of remote electronic monitoring and onsite monitoring (including source data verification in 10% of cases). Patients were mostly enrolled by cardiologists (n = 3776, 74.6%), by internal medicine specialists 14.2% (n = 718) and by primary care/general practice physicians 8.2% (n = 417). The mean (SD) age of the population was 69.5 (11.0) years, 44.3% were women. Mean (SD) CHADS2 score was 1.9 (1.2) and CHA2DS2-VASc scores was 3.2 (1.6). Background: Real-world data on non-vitamin K oral anticoagulants (NOACs) are essential in determining whether evidence from randomised controlled clinical trials translate into meaningful clinical benefits for patients in everyday practice. RIVER (RIVaroxaban Evaluation in Real life setting) is an ongoing international, prospective registry of patients with newly diagnosed non-valvular atrial fibrillation (NVAF) and at least one investigator-determined risk factor for stroke who received rivaroxaban as an initial treatment for the prevention of thromboembolic stroke. The aim of this paper is to describe the design of the RIVER registry and baseline characteristics of patients with newly diagnosed NVAF who received rivaroxaban as an initial treatment. Methods and results: Between January 2014 and June 2017, RIVER investigators recruited 5072 patients at 309 centres in 17 countries. The aim was to enroll consecutive patients at sites where rivaroxaban was already routinely prescribed for stroke prevention. Each patient is being followed up prospectively for a minimum of 2-years. The registry will capture data on the rate and nature of all thromboembolic events (stroke / systemic embolism), bleeding complications, all-cause mortality and other major cardiovascular events as they occur. Data quality is assured through a combination of remote electronic monitoring and onsite monitoring (including source data verification in 10% of cases). Patients were mostly enrolled by cardiologists (n = 3776, 74.6%), by internal medicine specialists 14.2% (n = 718) and by primary care/general practice physicians 8.2% (n = 417). The mean (SD) age of the population was 69.5 (11.0) years, 44.3% were women. Mean (SD) CHADS2 score was 1.9 (1.2) and CHA2DS2-VASc scores was 3.2 (1.6). Almost all patients (98.5%) were prescribed with once daily dose of rivaroxaban, most commonly 20 mg (76.5%) and 15 mg (20.0%) as their initial treatment; 17.9% of patients received concomitant antiplatelet therapy. Most patients enrolled in RIVER met the recommended threshold for AC therapy (86.6% for 2012 ESC Guidelines, and 79.8% of patients according to 2016 ESC Guidelines). Conclusions: The RIVER prospective registry will expand our knowledge of how rivaroxaban is prescribed in everyday practice and whether evidence from clinical trials can be translated to the broader cross-section of patients in the real world. 38th European Photovoltaic Solar Energy Conference and Exhibition; 533-537, The EU crystalline silicon (c-Si) PV manufacturing industry has faced strong foreign competition in the last decade. To strive in this competitive environment, the EU c-Si PV manufacturing industry needs to focus on highly performing c-Si PV technologies and on reducing environmental impact (lower CO2 footprint, improved sustainability, recyclability, etc.). In this work, we give an overview of the latest results achieved within the H2020 funded project called HighLite which addresses the work programme LC-SC3-RES-15-2019: Increase the competitiveness of the EU PV manufacturing industry. We report the progress made when implementing novel layers and processes in existing pilot-lines producing high-efficiency n-type silicon heterojunction (SHJ) cells with a shingle layout and interdigitated back-contact (IBC) cells with passivated contacts. In particular, we present the work done together with several industrial partners towards innovative production equipment for assembly of cut-cells into PV modules. Finally, we explain key learnings made so far when tailoring c-Si PV modules for various applications including building-applied PV (BAPV), building-integrated PV (BIPV) and vehicle-integrated PV (VIPV).

Background: Value Based Health Care (VBHC) is a well-known strategy in most countries, amongst whom the Netherlands, to improve patient-relevant outcomes and reduce healthcare costs. However, a methodology to implement VBHC principles in the full care cycle is lacking. This study describes a stepwise approach to implement and continuously improve patient-relevant outcomes in the total care delivery value chain. Methods: Key principles of VBHC are used to develop a stepwise methodology in which healthcare providers of primary, secondary, and tertiary care collaborate in a physician-driven initiative called the Netherlands Heart Network. The methodology incorporates the Plan-Do-Study-Act cycle to ensure continuous improvement of patient-relevant outcomes. To outline the presented methodology, the medical condition atrial fibrillation is used as a proof of concept. Results: Using the stepwise methodology results in an adequate registration of patient-relevant outcomes and a structured evaluation of adherence to prevailing guidelines. Based on the followed methodology, detailed improvements are defined in order to optimize patient-relevant outcomes. Conclusions: The presented methodology provides a description how to implement VBHC principles in the full cycle of care. Since this methodology is a first concept, future research should apply and assess the stepwise methodology in other fields and for different medical conditions. [ABSTRACT FROM AUTHOR]

In this work, ECAs based on different chemistry platforms are presented and the impact of their properties (such as adhesion, bulk electrical conductivity, contact resistivity to Ag ribbons and storage modulus) on module thermocycling reliability (-40/85˚C) and static mechanical load performance is studied. In particular, full-sized modules were built with ECAs with a wide range of storage moduli -- from very flexible, to very rigid materials -- and it was found that materials with the highest storage modulus show a larger power loss upon standard mechanical load tests. In addition, one-cell mini-modules were built to study the effect of the contact resistivity on the reliability performance, with findings suggesting that ECA usage is the main contributor to the series resistance increase upon cycling. Lastly, the work also proves that other materials in the mini-module manufacture process, such as the backsheet, play a critical role in reliability.

38. Anti-inflammatory and combined antiinflammatory/ analgesic medication in the early management of iliotibial band friction syndrome A clinical trial

Forty-three athletes presenting with unilateral iliotibial band friction syndrome (ITBFS) were randomly divided into three groups for the first 7 days of treatment (placebo-controlled, double-blind): 1 - placebo (N = 13); 2 - anti-inflammatory medication (N = 14) (Voharen; Geigy); and 3 - analgesicl anti-inflammatory combined medication (N =. 16) (Myprodol; Rio Ethicals). All subjects rested from day 0 to day 7 and all groups received the same physiotherapy outpatient treatment programme from day 3 to day 7. On days 0, 3 and 7 the subjects performed a functional treadmill running test (maximum 30 minutes) during which they reported pain (scale 0-10; 0 = no pain, 10 = unbearable pain) each minute. Total running distance, total running time and the area under the pain v. time curve was calculated. Daily 24-hour recall pain scores were also recorded. The 24-hour recall pain scores decreased significantly for all the groups over the treatment period. This method of assessing efficacy of treatment therefore failed to show differences between groups. In contrast, during the running test only group 3 improved their total running time and distance from day 0 to day 7, whereas in all the groups the area under the pain v. time curve decreased from day 0 to day 7. All the other groups improved total running time and running distance from day 3 to day 7. All three treatment modalities are effective in the early. treatment of ITBFS but physiotherapy in combination with analgesicl anti-inflammatory medication is superior. A functional running test, which is more sensitive than conventional pain-recall· methods in assessing efficacy of treatment in this type of clinical trial, is described.

- Abstract
Between 2009 and 2014, a research project was carried out by several research institutes and commercial companies in order to study and publish three settlement sites of the Single Grave Culture located in the Western Netherlands. These sites were excavated more than twenty years ago, but unfortunately the results were only scarcely published, and mostly in Dutch. This research project provided an excellent opportunity to unlock the high quality archaeological information. The presence of organic remains and numerous artefacts in cultural layers and the thorough excavation of these layers have produced a wealth of data regarding Late Neolithic behavioural variability in a dynamic wetland landscape. In this article, a summary of the results of this project will be presented.

Helaas zijn er weinig interventies of methoden om deze kinderen te helpen. Bovendien komen ook in deze doelgroep lees- en spellingsproblemen voor, zoals bijvoorbeeld problemen bij het aanleren van het alfabetisch principe. Ondanks meerdere jaren lees- en spellingsonderwijs, slaagde een 12-jarige jongen met meervoudige beperkingen die niet kan spreken er niet in om onbekende woorden te decoderen, terwijl hij woorden wel automatisch kon leren herkennen. In dit onderzoek is het effect geëvalueerd van een interventie, gebaseerd op de Non-verbal Reading Approach, het Whole-to-Part model en de methode 'Zo leer je kinderen lezen en spellen', bij deze jongen. De interventie leidde tot verbetering op alle afhankelijke variabelen, namelijk het correct decoderen van doelwoorden, het correct plaatsen van letters in de doelwoorden, het beheersen van klank-tekenkoppelingen, het correct horen van klanken in doelwoorden, het correct identificeren van doelwoorden en het decoderen van 12 generalisatiewoorden. De effecten bleven in stand in de follow-upmeting na 8 weken. De positieve resultaten die zijn behaald in dit onderzoek laten zien dat het via een gerichte interventie mogelijk is voor een kind dat niet of nauwelijks sprekend is om het alfabetisch principe aan te leren en decodeervaardig-heden te ontwikkelen. 15 p.

Households experience HIV and AIDS in a complex and changing set of environments. These include health and welfare treatment and support services, HIV-related stigma and discrimination, and individual and household social and economic circumstances. This paper documents the experiences of 12 households directly affected by HIV and AIDS in rural KwaZulu Natal, South Africa, between 2002 and 2004. The households were observed during repeated visits over a period of more than a year by ethnographically trained researchers. Field notes were analysed using thematic content analysis to identify themes and sub-themes. This paper focuses on three dimensions of household experience of HIV and AIDS that have received little attention in HIV and AIDS impact studies. First, that experience of HIV and AIDS is cumulative. In an area where population surveys report HIV prevalence rates of over 20% in adults, many households face multiple episodes of HIV-related illness and AIDS deaths. We describe how these challenges affect perceptions and responses within and outside households. Second, while over 50% of all adult deaths are due to AIDS, households continue to face other causes of illness and death. We show how these other causes compound the impact of AIDS, particularly where the deceased was the main income earner and/or primary carer for young children. Third, HIV-related illness and AIDS deaths of household members are only part of the households’ cumulative experience of HIV and AIDS. Illness and death of non-household members, for example, former partners who are parents of children within the households or relatives who provide financial support, also impact negatively on households. We also discuss how measuring multiple episodes of illness and deaths can be recorded in household surveys in order to improve quantitative assessments of the impact of HIV and AIDS.

My dissertation is on foundational questions about the value of human beings. This is a Kantian topic but I develop a proposal in a non-Kantian framework. I argue that to be a Kantian in ethics is to be committed to rationalism, but that the foundations of ethics should take account of the nature of human beings and our circumstances in the world. I develop a non-Kantian theory in which the value of human beings is no different, metaphysically speaking, from the value of other valuable things. Human beings have value, just as anything of value has value: because we are capable of being good-for something or someone. Most fundamentally, I argue that we are capable of being good-for ourselves. I propose that human beings have value in virtue of a capacity for having final ends, and that the capacity for having final ends makes us valuable because it makes us capable of living a good life, a life that is valuable because it is good-for the person who leads it. I show how the value of human beings gives everyone reason to treat human beings in certain ways. In particular, I show how everyone has reason not to destroy the capacity of human beings to have final ends, and, more positively, to help others realise their ends.

Background This study aimed (i) to ascertain the number of treatment referrals and information about protection orders given to survivors of domestic violence presenting for emergency trauma care, as reported at the one-month visit, (ii) to obtain a profile of violent incidents and injuries, and (iii) to assess self-esteem and posttraumatic and depressive symptomatology in the aftermath of injury. Methods A survey of 62 participants presenting in the acute aftermath of domestic violence (as defined by the Domestic Violence Act of 1998) was conducted over 12 weeks at the Trauma and Resuscitation Unit of a Level One trauma centre in an urban public hospital in South Africa.1 Following informed consent, face-to-face structured interviews were conducted during admission and a month later. The following instruments were administered at baseline: a Demographic and Injury Questionnaire, the Beck Depression and Rosenberg Self-Esteem Inventories, and the Davidson Trauma Scale. A psychosocial questionnaire was administered at the one-month follow-up. Results Fifty-eight per cent of the participants were female and 42% were male. Seventy-four per cent of the perpetrators were male. Ninety-five per cent of the participants said that no health professional had informed them about where or how they could find help. Although all were seriously injured, 76% of the participants said only the researcher had asked about their experience. Sixty-six per cent of the cases of domestic violence were related to intimate partner violence. Overall, subjects displayed high levels of depressive and post-traumatic stress symptomatology that had neither been treated nor adequately referred. Conclusion Even though domestic violence poses significant health threats and costs to the health system, it appears to be a neglected area of South African health care. Health professionals should at least be able to identify and intervene within the "open window" period when psychosocial opportunities are pivotal. Jarenlang lag het prehistorische grafveld de Busjop verscholen in de Limburgse bossen. Een ondoordringbare barrière van braamstruiken en varens had het langzaam maar zeker overgroeid. Nu zijn de grafheuvels weer zichtbaar gemaakt, zeventien in totaal. Grafveld de Busjop bij Heythuysen is nu een sterlocatie voor bezoekers aan het Leudal geworden
