13. Managing Dissociation in Imagery Rescripting for Voice Hearers With Trauma: Lessons From a Case Series

Trauma, voice hearing, and dissociation tend to be closely linked. Cognitive models of voice hearing largely agree that traumatic events may predispose people to voice hearing via dissociative processes. While treating trauma in voice hearers may help to reduce voice distress and frequency, dissociation may be a barrier to this therapeutic work. This case series reports on the dissociative experiences of a subsample of voice hearing clients who reported dissociation during Imagery Rescripting (ImRs) for trauma (N = 6, 50% of original sample) in the case series study reported on previously (Paulik et al., 2019). The aims in the current paper were to explore the impact of dissociation on outcomes, the type of dissociative experiences encountered, where in the ImRs protocol they occurred, and the use of therapeutic techniques to address them. We found that clients who dissociated during therapy showed reductions in their trauma intrusions and voice-related distress and frequency. However, when compared to nondissociators, these benefits took more sessions to achieve. The most common types of dissociation were flashbacks, losing control over the image, emotional detachment, and trance/absorbed state. These were most likely to occur at points during the therapy where negative affect was heightened. Grounding and soothing techniques, as well as strategies to reduce the level of negative affect, were reported effective by participants in preventing or interrupting dissociation. We recommend that dissociation should not be a barrier to implementing imagery rescripting with this group.

El presente artículo se deriva de una investigación documental, la cual tuvo como objetivo analizar el concepto de pareja y las habilidades del terapeuta de pareja, que se identificaron en fuentes documentales de terapia contemporánea. En primer lugar, se logró identificar los cambios que se han dado en la conceptualización de la pareja, ya que la pareja como concepto ha tenido una fuerte implicación histórica y cultural, evidenciándose nuevas tipologías de pareja en la contemporaneidad. También fue posible encontrar que la pareja ha sido leída como sistema desde el trabajo terapéutico con la influencia de la cibernética de segundo orden. En segundo lugar, se hallaron algunas de las habilidades que un terapeuta de pareja debe desarrollar para el éxito de su intervención, entre las que se encuentran la identificación de motivo de consulta, el establecimiento de la alianza terapéutica y el desarrollo del self del terapeuta.

El presente articulo corresponde a un ensayo teórico que explora el trabajo centrado en la persona con clientes que presentan ideación suicida. Inicialmente se presenta un panorama general sobre el suicido, luego, se plantea una mirada comprensiva sobre el fenómeno de la ideación suicida desde el enfoque centrado en la persona, y, finalmente, se plantean algunas reflexiones sobre el proceso psicoterapéutico para trabajar con personas que presentan ideación suicida. Se concluye que este enfoque puede contribuir al trabajo terapéutico en personas que presentan ideación suicida, y, por lo mismo, la necesidad de realizar mayores investigaciones sobre el tema.

Metaphors play an important role in contemporary approaches to grief therapy by helping clients (re)construe their continuing relationship with the deceased. Relevant studies have illustrated the substantive elements of metaphors (i.e. sources, targets, and mappings) in this regard, often focusing on localized bursts of intense metaphoric activity. This paper highlights the extended nature of metaphoric conceptualizations and their relationship with key meaning-making processes, following the principle of ‘correspondent analysis’ as a collaborative move between language analysts and therapy practitioners. Three specific phenomena are detailed through a mixed methods analysis of 18 motivated segments in a single session of grief therapy: (i) sporadic sources, (ii) persistent sources, and (iii) metaphor ‘chaining’ across embodied and verbal activity. Their respective links to treatment objectives and processes demonstrate how metaphor theoretic constructs dovetail with therapeutic work, and suggest future avenues for modeling unfolding metaphoric activity as a time series.

RESUMO A adolescência é um período marcado por diversas mudanças biopsicossociais, como, por exemplo, a corporal. Quando essa mudança não corresponde ao padrão de beleza estabelecido socialmente, o adolescente precisa lidar com frustrações e preconceitos, o que pode levá-lo ao adoecimento. A presente pesquisa é um estudo de caso de uma adolescente que sofria de Bulimia Nervosa. Neste artigo apresentaremos os resultados da psicoterapia psicodramática, principalmente do trabalho terapêutico da criança interna ferida e de lógicas afetivas de conduta que favoreceram a melhoria de seu transtorno alimentar. Também analisaremos como o corpo carrega as cenas socioculturais, na busca de afetos nas relações. Ao reviver essas cenas, na realidade suplementar, a paciente pode ressignificar experiências de sofrimentos que liberaram a espontaneidade-criatividade e desenvolveram modalidades vinculares menos sofridas. ABSTRACT Adolescence is a period marked by several biopsychosocial changes, such as the body. When this change does not correspond to the socially established standard of beauty, adolescents need to deal with frustrations and prejudices, which can lead them to become ill. This research is a case study of an adolescent who suffered from Bulimia Nervosa. In this article we will present the results of psychodramatic psychotherapy, especially the therapeutic work of the wounded inner child and the affective logic of conduct that favored the improvement of her eating disorder. We will also analyze how the body carries the socio-cultural scenes, in the search for affections in relationships. By reliving these scenes, in surplus reality, the patient can re-signify experiences of suffering that have released spontaneity-creativity and develop less suffered vincular modalities. RESUMEN La adolescencia es un periodo marcado por numerosos cambios bio-psicosociales, como, por ejemplo, el corporal. Cuando este cambio no corresponde al canon de belleza establecido socialmente, el adolescente debe enfrentarse a las frustraciones y prejuicios, que pueden llevarlo a enfermarse. Esta investigación es un estudio de caso de una adolescente que sufría de Bulimia Nerviosa. En el presente artículo se presentarán los resultados de la psicoterapia psicodramática, principalmente del trabajo terapéutico de la herida infantil y de la lógica afectiva de conducta que han favorecido la mejoría de su trastorno alimentario. También se analizará cómo un cuerpo carga con las escenas socioculturales, en la búsqueda de afecto en las relaciones. Al revivir esas escenas en la realidad suplementaria, la paciente logró resignificar experiencias de sufrimiento que liberaron tanto la espontaneidad como la creatividad y desarrollaron modalidades vinculares menos dolorosas.

Although psychosis and psychoanalysis have been closely linked since the earliest Freudian theories, the psychoanalytic treatment of psychotic states still presents major difficulties. The establishment of a therapeutic situation allowing the deployment of psychic work is still uncertain. The resumption of the work of S. Freud and DW. Winnicott associated with the development of the theorization of a metapsychology of the process of symbolization by the French school (Green, Roussillon) allows a revival of the reflection on the stakes of the establishment of the therapeutic situation. This theorization of representative processes makes it possible to consider the psychotherapeutic approach to psychoses from the angle of a "psychopathology of the psychic apparatus" and thus to model the psychic dynamics involved in psychotic problems which initially appear in a chaotic form. The apparent failure of the psychic processes, in their linking and transforming activity with which the psychotic states are confronted, can then be thought of as the seat of a particular psychic work that can provide essential support to the therapeutic work. Two cases studies illustrate these processes.

During the COVID-19 pandemic, eye movement desensitization and reprocessing (EMDR) therapists have been faced with the choice of either abruptly terminating therapy with their clients or moving to working remotely, usually by videoconferencing This poses particular challenges to therapists who may have never worked online The standard EMDR protocol can be effectively delivered remotely when therapists are aware of how to make necessary adaptations Based on clinical and supervision practice, this article describes ways in which EMDR therapists can adapt their ways of working in order to work effectively with clients remotely This ensures that clients can still benefit from this effective and evidence-based treatment during a global health crisis It suggests a number of ways in which bilateral stimulation can be achieved remotely, as well as discussing special considerations which arise with online therapeutic work It considers adaptations which need to be made at each stage of EMDR therapy It uses case examples from children and adults to illustrate how these adaptations work in real life The case examples discussed in this article are illustrative of the techniques and adaptations necessary for remote EMDR and therefore reflect composites rather than individuals © Copyright 2021 EMDR International Association

Recent investigations have elucidated the influence of the strong Black woman (SBW) ideal on the mental health and treatment-seeking behaviors of Black women in the United States. However, factors related to how the SBW ideal affects Black women’s positive and negative attitudes toward seeking professional psychological help have yet to be identified. The current study fills this void in the literature through a qualitative examination of how the SBW ideal affects Black women’s mental health utilization. Analyses were based on 62 participants, including college students and community members, ranging in age from 18 to 72 years. The present study advances the idea that endorsement of the SBW ideal affects Black women’s mental utilization. These results offer evidence and clarification of the impact of the SBW ideal on Black women’s mental health and identity-specific points of intervention for mental health practitioners conducting therapeutic work with Black women. Authors provide practice recommendations for practitioners and suggestions for future research.

Aims and methodThe aim of this study was to compare the efficacy of direct therapy and indirect consultation for treating mental health difficulties among looked after children (LAC), and also to identify any demographic or clinical predictor variables for outcomes in this cohort. A retrospective evaluation of mental health outcomes for 104 LAC was conducted. All children received network consultation in combination with or without direct therapeutic work. Outcomes were compared between the groups with and without direct therapeutic intervention.ResultsThose receiving both treatments displayed significantly greater Strengths and Difficulties Questionnaire (SDQ)-rated improvements than those receiving just consultation. Nonetheless, improvements in scores for the latter group were significant relative to baseline. Treatment duration, younger age at referral and start SDQ were all correlated with positive outcomes, while number of address changes predicted higher start SDQ scores.Clinical implicationsDespite the retrospective design of this study, its results can be considered as preliminary findings to guide treatment decisions in LAC.

This paper discusses the inclusion of ideas from mindfulness, embodiment, and associated brain research in poststructural family therapies. Connecting research and practice across diverse fields is presented as enriching therapeutic work, increasing clinical effectiveness, and broadening options for families seeking our assistance. Ways of incorporating mindfulness and embodiment are described and offer distinctively novel practices not typically used in traditional forms of poststructural therapies. These innovative practices are illustrated by clinical work with a mother and her five-year- daughter who survived a dangerous health crisis and an extremely painful hospitalization. Therapeutic work with this family exemplifies how traumatic experiences are not always accessible through linguistic means alone. Working with interpersonal mind-body practices can uniquely contribute to relaxing the sympathetic nervous system and activating the social engagement branch of the parasympathetic nervous system. The inclusion of such practices can provide valuable opportunities to facilitate the emergence of preferred experiences of self for all family members.Este artículo trata de la inclusión de las ideas de conciencia plena y personificación, y de la investigación neurológica asociada en las terapias familiares posestructurales. La conexión de la investigación y la práctica entre áreas diversas se presenta como un trabajo terapéutico enriquecedor que aumenta la eficacia clínica y amplía las opciones para las familias que buscan nuestra asistencia. Se describen las maneras de incorporar la conciencia plena y la personificación y se ofrecen prácticas distintivamente innovadoras que normalmente no se usan en las formas tradicionales de las terapias posestructurales. Estas prácticas innovadoras se ilustran mediante el trabajo clínico con una madre y su hija de cinco años que sobrevivieron a una emergencia de salud peligrosa y a una hospitalización sumamente dolorosa. El trabajo terapéutico con esta familia ejemplifica cómo no siempre es posible acceder a las experiencias traumáticas por medios lingüísticos solamente. El trabajo con prácticas interpersonales centradas en el cuerpo y la mente puede contribuir de manera única a relajar el sistema nervioso simpático y a activar la rama de la participación social del sistema nervioso parasimpático. Animated film characters represent various attitudes, points of view and issues, including social problems. Many of them are shown as carriers of socially approved character traits, who have an important mission or fight for the greater good. It must be emphasised that a film hero, as the key element of the film plot is the centre of attention for a viewer. The holistic reception, including all the values, norms and principles, proves to be an effective tool during educational and therapeutic work. Thanks to considerations into the social dimension of activities, based on the example of particular characters, it is possible to appreciate in animated films the significance of the issues which are still unnoticed and regarded as childish. The development of this topic is the result of the researcher’s own research into the social message of animated works.

In this paper the author proposes that a central task of psychotherapeutic work is to “stay close to the terror,” particularly when working with those patients whose inner world is populated by often dissociated states of traumatic horror. The paper explores a range of psychoanalytic, Jungian, and trauma theory that might assist in guiding psychotherapists regarding how we might engage with this central task, particularly given the often terrifying intrapsychic, interpsychic, and interpersonal disturbances such therapeutic work entails, for both patient and therapist.

The objective of the study is to find out whether MTWP addressed to enhance the work skills and its success in the client return to work, level of work satisfaction & self-performance level post injured wrist. A different subject prospective, experimental, flexible pre-and-posttest intervention design followed by descriptive analysis with questionnaire has been used for the purpose of this study. Purpose This paper outlines the concept of the 'Compassionate Kitbag', a novel multi-sensory-based means of helping draw together the various elements of compassionate mind training and processes within compassion-focused therapy (CFT), to help clients cultivate and facilitate their capacities for compassion. Building on the work of Lucre and Corten (2013, Psychology and Psychotherapy: Theory, Research and Practice, 86, 387), this is the first published work exploring this concept and the theoretical underpinnings, with a specific focus on how this can be used to support people with attachment and relational trauma. Methods A narrative review of the literature on multi-sensory stimulation in facilitating people's capacities for compassion was conducted, coupled with a review of the literature of the use of non-human and transitional objects as explained by attachment theory and the broader scientific research underpinning the CFT model. Results Clinical examples of how to use the Compassionate Kitbag to help stimulate compassionate therapeutic processes demonstrate the benefits of and how to begin utilizing this approach in compassion-focused work with clients with complex needs. Conclusions The Compassionate Kitbag's potential therapeutic value lies in offering multifarious creative and tangible means of accessing compassion to a wide range of individuals whom are typically fearful of, blocked, and/or resistant to compassion. Further research into the wider application of the concept of the Compassionate Kitbag is needed. Practitioner points Many patients with ruptured and/or traumatized early attachment relationships can find more traditional talking therapies difficult to access. Compassion-focused therapy (CFT) can offer an evolutionary-based understanding of interpersonal difficulties which can be helpful for such patients. Creatively harnessing and utilizing multi-sensory and non-linguistic social signals in CFT is key to exercising the care-giving and care-receiving social mentalities that facilitate compassionate flow. There is considerable evidence to support the use of a multi-sensory component to the therapeutic work to help patients cultivate and facilitate their capacities for compassion. The Compassionate Kitbag can be a way of supporting patients to create concrete representations of compassion in the context of the compassionate mind training component of the therapeutic work. Some of these objects can become transitional objects which can aid the therapeutic work.

Objectives: Family caregivers of people with dementia often report feelings of guilt. However, the number of studies analyzing guilt and the factors associated with its appearance are scarce. The aim of this study is to explore the subjective experience of guilt in the family care of people with dementia. Methods: A qualitative analysis of the narratives of 13 family caregivers of people with dementia about their feelings of guilt was done. Results: Seven categories for understanding guilt in caregiving were obtained: guilt derived from actions themselves; guilt derived from one’s limitations; guilt for feeling negative emotions; guilt associated with the change in the relationship with the person cared for; guilt for neglecting other areas; guilt induced by the person cared for, and guilt induced by others. The results showed the existence of cases in which guilt is absent by distress-avoiding processes. Conclusions: Guilt is a relevant variable in understanding caregiver distress, and its analysis is necessary for therapeutic work in the field of care. Technology and informatics become part of our daily life. Conversely, the on-line therapeutic work is a reality presenting new possibilities and challenges for the professionals. Hardware and software turned more sophisticated as well as the new technological solutions to surpass communication barriers. Several studies reveal that "computerized psychology" is a poorly investigated field, which makes it a very promising study area. This paper presents a work in progress project, which consists on the development of an application that will facilitate the interaction among psychologists and patients. The application has two goals. The first one will allow to a professional in the field of psychology, to obtain a preliminary analysis of the patient's condition, even before the face-to-face appointment. A registered patient may submit narratives in which he reports his situation, and the application will subsequently treat the text and will analyze it. Then, a graphical presentation focused, fundamentally, on user's five senses indicators (hearing, smell, touch, vision and taste) will be designed. Finally, the data analysis will be presented to the assigned psychologist. The second goal is to evaluate, automatically, the patient tests. Games and other activities will be reported as prescriptions. The presented application was designed based on the opinion of the psychologists interviewed. Since the communication process will be text and the patient will be free to send it, any time, to the psychologist, it is our conviction that patients will adhere to use it. Questionnaires are tools of interest in the evaluation of pelvic floor dysfunctions, but their success depends on their readability. Evaluating symptoms associated with such dysfunctions through questionnaires validated in Spanish with adequate readability indices will be useful for subsequent therapeutic work with these patients. The objective of this study is to evaluate the readability of symptomatology questionnaires on pelvic floor dysfunctions adapted to Spanish. This descriptive study included a total of 19 questionnaires, whose readability was analyzed according to four indices: Fernández-Huerta, Szigriszt-Pazos, Inflesz and readability µ (mu). In total, 50% of the questionnaires for fecal incontinence symptoms were found to have inadequate scores in terms of readability, according to the Inflesz index. If we take the readability mu index as a reference, the number of questionnaires that do not meet the minimum readability limit are as follows: 20% in urinary incontinence, 50% in fecal incontinence, 66.6% in sexual function, 100% in general pelvic floor, 25% in overactive bladder and 100% in benign prostatic hyperplasia. Therefore, it is necessary to review and adapt health questionnaires on pelvic floor dysfunctions to improve their readability and ease of understanding by conducting studies with the people who fill out these questionnaires.

Questo articolo approfondisce il tema del rapporto tra ricerca in psicoterapia e pratica clinica. Nonostante gli avanzamenti nell’ambito della ricerca negli ultimi venti anni, permangono ancora diversi ostacoli nel processo di disseminazione di tale conoscenza. a tal fine, verranno messe in evidenza le differenze tra l’evidence-based research, volta a determinare quale trattamento terapeutico sia più efficace per i singoli disturbi psichiatrici e la practice-based research, che prova ad ottenere dei risultati più vicini alla pratica professionale quotidiana, focalizzandosi su aspetti come la relazione terapeutica, le caratteristiche del terapeuta e del paziente ed i fattori contestuali. È possibile costruire una pratica clinica che non sia sganciata dalle evidenze scientifiche su vari aspetti del lavoro terapeutico. infine, verrà evidenziato come le terapie psicodinamiche abbiano ottenuto varie prove di efficacia che le rendono comparabili a trattamenti di altri orientamenti, anche se la complessità dello studio della relazione terapeutica in tale setting richiede uno sforzo sempre maggiore per integrare ricerca quantitativa e qualitativa.

Il trattamento di Clara descritto dalla dott.ssa Marina Amore fornisce nuove intuizioni sul ruolo delle funzioni somatiche nello sviluppo dei disturbi emotivi e nella comunicazione terapeutica. Secondo la teoria del processo referenziale le descrizioni di specifiche esperienze corporee possono servire a costruire connessioni con il modo simbolico e verbale quando le narrazioni che coinvolgono persone ed eventi della vita non sono accessibili. Durante i 10 anni di trattamento, i racconti di Clara dei suoi gravi sintomi nevralgici e delle sue esperienze corporee ed emotive nelle sue interazioni con la dott.ssa Amore le permisero alla fine di parlare delle esperienze passate e presenti con i suoi genitori e le altre persone, e di ricostruire schemi delle emozioni che erano state dissociate. Le sensazioni corporee della dott.ssa Amore nelle sue interazioni con Clara l’hanno guidata nel lavoro terapeutico e hanno inoltre aperto una nuova comprensione delle esperienze nella sua stessa vita. Il processo referenziale può essere visto come un processo che ha avuto luogo in profondità tra e dentro i due partecipanti mentre riconoscevano il processo di crescita di Clara e lavoravano anche per risolvere il loro senso di perdita nella fase finale.
