- Abstract
Background: Lowering low-density lipoprotein (LDL) cholesterol with statin therapy has been shown to reduce the incidence of atherosclerotic events in many types of patient, but it remains uncertain whether it is of net benefit among people with chronic kidney disease (CKD). Method(s): Patients with advanced CKD (blood creatinine >=1.7 mg/dL [>= 150 mumol/L] in men or >=1.5 mg/dL [ >= 130 mumol/L] in women) with no known history of myocardial infarction or coronary revascularization were randomized in a ratio of 4:4:1 to ezetimibe 10 mg plus simvastatin 20 mg daily versus matching placebo versus simvastatin 20 mg daily (with the latter arm rerandomized at 1 year to ezetimibe 10 mg plus simvastatin 20 mg daily vs placebo). The key outcome will be major atherosclerotic events, defined as the combination of myocardial infarction, coronary death, ischemic stroke, or any revascularization procedure. Results A total of 9,438 CKD patients were randomized, of whom 3,056 were on dialysis. Mean age was 61 years, two thirds were male, one fifth had diabetes mellitus, and one sixth had vascular disease. Compared with either placebo or simvastatin alone, allocation to ezetimibe plus simvastatin was not associated with any excess of myopathy, hepatic toxicity, or biliary complications during the first year of follow-up. Compared with placebo, allocation to ezetimibe 10 mg plus simvastatin 20 mg daily yielded average LDL cholesterol differences of 43 mg/dL (1.10 mmol/L) at 1 year and 33 mg/dL (0.85 mmol/L) at 2.5 years. Follow-up is scheduled to continue until August 2010, when all patients will have been followed for at least 4 years. Conclusions SHARP should provide evidence about the efficacy and safety of lowering LDL cholesterol with the combination of ezetimibe and simvastatin among a wide range of patients with CKD.Copyright © 2010, Mosby, Inc. Hospitality and tourism industries have become one of the pivotal industry that contribute to the growth of the nation's economy. However, less studies actually pay attention to the aviation industry where this industry has faced a lot of challenges and crisis in the past decade. Hence, this study aims to examine the predicting factors of service quality on corporate reputation of AirAsia in Malaysia. The study utilized SERVQUAL model to guide the study. A quantitative (survey) research design was employed wherein a total of 200 completed questionnaires was obtained from the Malaysian’s customers of AirAsia, through purposive sampling. Based on the correlation analysis, tangibility, reliability, responsiveness, assurance has a positive and moderate relationship with corporate reputation, while empathy has a positive and high relationship with corporate reputation. In addition, multiple regression analysis showed that responsiveness, assurance, and empathy were the contributing factors to corporate reputation. However, tangibles and reliability were not the predictors of corporate reputation. Conclusion, implications, and suggestion for future study were also discussed.

Atomic emission spectrometry was applied for the determination of potassium and sodium concentrations in crude drugs and decoctions. The content of the two elements and potassiumsodium ratios of medicinal plant extracts were measure as a basis for evaluating diuretic activity. 37. Mediastinal Hibernoma: A Rare Case with Radiologic-Pathologic Correlation.

- Abstract
Hibernomas, especially located in the mediastinum, are extremely rare benign tumors, which are important to consider in the differential diagnosis of a heterogeneously enhancing mass with areas of fat attenuation on imaging of an often incidentally discovered mass. Other common possibilities in the differential include malignant tumors, such as liposarcoma, hence histopathology is usually required to confirm the diagnosis. Hibernomas often follow the distribution of sites of persistence of brown fat in adults, and intrathoracic locations are unusual. We present a very rare case of a mediastinal hibernoma in a 53-year-old woman. She presented to the emergency department with severe, progressive right neck and shoulder pain with radiation down her arm and was found to have a right apical posterior mediastinal mass on imaging. Initial radiographs of the shoulder showed a soft tissue mass within the apical right hemithorax. Further imaging with CT revealed a well circumscribed, heterogeneously enhancing mass with areas of fat attenuation. Pathology confirmed the diagnosis of mediastinal hibernoma, and the mass was completely excised. Fourteen months after surgery, the patient had a normal chest radiograph, and thirty-two months after surgery, she remains asymptomatic.

Tinctures are almost the oldest medicines and their use is substantial in the medication nowadays as well. The antioxidant values by ferric reducing/antioxidant power (FRAP) method and element content by inductively coupled plasma optical emission spectrometry (ICP-OES) were investigated in some tinctures official in the VII. and VIII. Pharmacopoeia Hungarica. The highest FRAP values were found for volatile oil containing Tinctura Aurantii amari epicarpii et mesocarpii, Tinctura Amara and Tinctura Valerianae (764.54 ± 19.90; 757.37 ± 14.46; 826.40 ± 5.89 µmol l⁻¹, respectively). The correlations between the FRAP values and dilution with different alcohol content in Tinctura Chinae, Tinctura Ipecacuanhae normata and Tinctura Strychni were also investigated. Remarkable differences were found between the element concentrations in the different tinctures. The element contents in tinctures are not so high in absolute values nevertheless the presence of essential selenium, zinc, manganese and copper is important since they have key role in the antioxidant system. The common feature of the tinctures seems to be the lithium content. The Ca to Mg concentration ratio was found to be shifted towards magnesium in some of the tinctures that can show a higher Mg absorption which could affect against the proinflammatoric processes in the cases of gastrointestinal diseases.

39. Lymphatic filariasis disseminating to the upper extremity.

- Abstract
Lymphatic filariasis is the most common cause of acquired lymphedema worldwide (Szuba and Rockson, 1998). It is endemic to tropical and subtropical regions, and its effects are devastating. With over 100 million infected persons, it ranks second only to leprosy as the leading cause of permanent and long-term disability. Wuchereria bancrofti is the etiologic agent in 90% of cases. There is a dearth of published MRI findings with pathologically proven active infections, making this entity even more of a diagnostic dilemma. Imaging may provide the first clue that one is dealing with a parasite and may facilitate proper treatment and containment of this disease. This is the first report of pathologic correlation with MRI findings in the extremity in active filariasis. The magnetic resonance images demonstrate an enhancing, infiltrative, mass-like appearance with partial encasement of vasculature that has not been previously described in filariasis. Low signal strands in T2-hyperintense dilated lymphatic channels are seen and may depict live adult worms. We hypothesize that the low signal strands correspond to the collagen rich acellular cuticle. This, in combination with the surrounding hyperintense T2 signal, corresponding to a dilated lymphatic channel, may provide more specific MRI findings for active nematodal infection, which can prompt early biopsy, pathological correlation, and diagnosis.

Introduction: In gastrointestinal diseases folk medicine usually recommends herbs and teas with antiinflammatory activities, especially mucilage and bitter agent containing plants. The use of stomatics as adjuvant therapy in medical practice is frequent., Aim: The authors explored the mineral element content of some herbs, because mineral elements may have significant role in the development and treatment of gastrointestinal diseases, and a close connection between the presence or absence of mineral elements and inflammatory mediators was noted., Methods: The mineral content in Trigonellae foenugraeci semen, Farfarae folium et flos, Taraxaci radix and Cichorii radix and teas prepared from these herbs was determined using inductively coupled plasma optical emission spectrometry. The results were evaluated from nutritional and biomedical points of view., Results: Farfarae folium et flos tea was found to be a rich source of calcium, chromium, manganese and molybdenum. Trigonellae foenugraeci semen, Taraxaci radix and Cichorii radix displayed favourable calcium to magnesium ratio (1-1.5) and they contained other elements (manganese, iron, copper, zinc) which may have significant role in the immune defence system., Conclusion: The beneficial therapeutic effect of the studied herbs may be related to their mineral element content.

- Abstract
Plant drug mixtures are widely used in the adjuvant therapy of type 2 diabetes mellitus for the prevention of complications. The drug mixtures generally contribute to the efficiency of the therapy and may also reduce undesirable side effects. Two herbal extracts (lyophilized aqueous extracts of plant drug mixtures 1: Myrtilli folium, Phaseoli fructus sine seminibus and 2: Myrtilli folium, Phaseoli fructus sine seminibus, Salviae folium) were investigated in in vitro rat models. The content of bioactive constituents (polyphenol, flavonoid and vitamin C) in plant drug mixtures and lyophilized samples was evaluated. The antioxidant activity of lyophilized extracts was determined by measuring the ferric reducing ability of the plant, Fe2+ induced lipid peroxidation (LPO) in rat brain homogenates and NADPH (beta-nicotinamide adenine dinucleotide phosphate reduced form) induced LPO in cerebral microsomes. The antioxidant activity of lyophilized extracts was compared to that of quercetin and rutin. Both teas of lyophilized extracts had significant reducing ability (2694 and 2771 micromol/l) and inhibited LPO (IC50 28.0 and 20.6 microl in NADPH induced LPO, 17.3 and 8.7 microl in Fe2+ induced LPO). The high concentration of polyphenol/flavonoid (12.38-13.00 and 1.45-5.22 g/100 g, respectively) and vitamin C (0.099-0.165 g/100 g) in the herbal extracts is related to their significant antioxidant properties. The tea mixtures have significant nutritional value, since the consumption of 2 or 3 cups of tea a day covers 50% of the daily requirement of vitamin C and it is also relevant polyphenol source. The high polyphenol/flavonoid content may restore the redox imbalance and contribute to the prevention of diabetic complications.

- Abstract
Peripheral primary neuroectodermal tumors (PNET) are uncommon tumors, with an overall incidence of 1% of all sarcomas. Renal PNET is extremely rare. It is an aggressive tumor, with a high incidence of local recurrence and early metastases. Radiological diagnosis can be challenging as it is often difficult to differentiate it from other primary renal neoplasms. Imaging features include a large heterogeneous mass with central low density areas due to necrosis. Intra tumor hemorrhage and calcification are rare. We present a rare case of a large PNET mimicking renal cell carcinoma in a 51 year old female that required arterial embolization and nephrectomy.

Some current oils and their main components were studied to determine their antioxidant values. This was done by using the modified method of ferric reducing ability of plasma. It has been established that volatile oils of medicinal plants have on average a reducing capacity of 3.5-220 mmol/kg oil. The reducing capacities of the main constituents of volatile oils are 0.165-65.5 mmol/kg in concentrated oils. The highest reducing capacity was showd for phellandrene (65.438 +/- 0.166 mmol/kg) and anethole (50.087 +/- 0.160 mmol/kg) while the lowest values were obtained for menthol (0.165 +/- 0.023 mmol/kg) and menthone (0.168 +/- 0.010 mmol/kg). It has been stated that the antioxidant values of the main constituents are lower than those of volatile oils. The reducing capacity of the main constituents of medicinal plant drugs at different concentrations was also determined.

- Abstract
Several novel types of miniaturized electrochemical flow cells are described. The flow cells are fabricated in fluorinated ethylene propylene using a novel technique where channels with inner diameters down to 13 microm are integrated with electrodes. The channel is formed by shrinking and simultaneous melting of a heat shrink/melt tubing around a channel template (a tungsten wire) and electrodes followed by removal of the channel template. The technique allows incorporation of different electrode materials of different sizes. The electrode configuration consists of one or two working electrodes inside the channel and a counter electrode located in the channel outlet reservoir. Electrode configurations with different channel and working electrode sizes, different electrode materials including carbon fibers, glassy carbon rods, poly(tetrafluoroethylene)/carbon composite material, and platinum wires, and different arrangements have been assembled. Hydrodynamic voltammograms in dual-electrode (generator-collector) experiments indicate good potential control for cells with 25-microm channels, while there is some iR drop in cells with 13-microm channels. Cells prepared with a cylindrical working electrode tangent and perpendicular to a flow channel show a flow rate dependence consistent with thin-layer cell behavior. Electrode areas can be made in the range of 10(-10)-10(-8) m2.

From the rose hip seed, which is generally a waste material, valuable oil can be obtained for medicinal use. Various extraction methods have been compared: traditional solvent extraction with ultrasound-, microwave-, sub- and supercritical fluid extraction (SFE). Unsaturated fatty acid (UFA: oleic-, linoleic- and linolenic acid; 16.25-22.11%, 35.94-54.75%, 20.29-26.48%) and polyunsaturated fatty acid (PUFA:linoleic- and linolenic acid) content were over 90% and 60% in the recovered oils. The oils contained different amounts of metals. The concentration of some metals, particularly iron in microwave oil (27.11 microg g(-1)) is undesirable from the aspect of stability. By traditional solvent extraction, oil was obtained in 4.85 wt/wt%. Subcritical FE appeared to be the best method for the recovery of rose hip oil with highest oil yield (6.68 wt/wt%), carotene- (145.3 microg g(-1)) and linoleic acid content (54.75%). Supercritical FE without organic solvent is suitable for mild recovery of oil. The oil was rich in UFA and PUFA (92.7% and 76.25%) and contained the lowest amount of carotene and pheophytin (36.3 and 45.8 microg g(-1)). Oil yield in most new extraction methods (microwave extraction, super- and subcritical FE) was higher than in the case of traditional Soxhlet extraction. The main benefit of supercritical FE with CO2 is the solvent free oil while in the case of other extractions evaporation of the solvent is needed. Although the content of bioactive compounds in oils was different, all oils may be appropriate for medicinal use.

Ground fennel seeds were extracted with supercritical carbon dioxide. Small-scale subsequent extractions of the same sample showed that the composition of volatile compounds was changed with the extension of extraction time and only principal volatile components (limonene, fenchone, methylchavicol, and anethole) were present in the last-extracted sample. Fennel oil was successfully fractionated into the essential oil rich and fatty oil rich products in pilot-scale apparatus using two separators in series. Designed experiments were carried out to map the effects of pressure and temperature in the first separator on the yields and compositions of the products. The minimum level of the total undesired components in both essential oil rich and fatty oil rich products appeared at a pressure of 80-84 bar and a temperature of 31-35 degrees C in the first separator. Supercritical CO(2) extraction of fennel seeds resulted in higher yield (10.0%) than steam distillation (3.0%), almost the same yield as hexane extraction (10.6%), and lower yield than alcohol extraction (15.4%). Analysis of the volatile compounds revealed the significant difference of the composition in distilled oil and oleoresins prepared by CO(2) and solvent extractions. Sensory evaluation showed that the CO(2) extraction product and distilled oil were more intense in odor and taste than alcohol and hexane extracts.

- Abstract
Plant-anatomical and phytochemical investigations were carried out on three Salvia species: S.officinalis L., S.sclarea L., S.pratensis L. It was established that the structure of the glandular hairs of the three species doesn't differ from each other but the characteristics of the covering hairs are different. The covering hairs of sage (Salvia officinalis) consist of 1-4 cells and have protective function. The hairs of the other two Salvia species are bristle hairs. The highest essential oil content was found in sage and the lowest one in S. pratensis. The qualitative and quantitative composition of the essential oils of Salvia species was also various but the qualitative composition of leaf, calix and petal of the same Salvia species was characteristical standard; significant differences were found only in their quantitative composition. Sclareol diterpene alcohol was the main component of the hexane extract obtained from clary sage flowering herb. In polyphenol ingredients Salvia officinalis was the richest. The results demonstrated that the 20% ethanol is the best of the 20, 40, 70% alcoholic solvents, for the extraction of polyphenol compounds. It was established that the rosmarinic acid depside was the main component of polyphenols. Mineral elements were also analysed in the Salvia species leaves as well in the alcoholic and wateric extracts of sage. The magnesium content was considerable in S. pratensis, the zinc content was the highest in S. officinalis.
