2. Adverse Childhood Experiences, Risk of Opioid Misuse and Its Pathway among Students at a Public University

- Abstract
Objective: We examine role of ACEs and pathways to risk of opioid misuse among young adults. Participants and Methods: A cross-sectional survey of validated measures of ACEs, risk of opioid misuse, and health conditions with a sample of 1,402 students from a large public university followed by multivariate logistic regression and pathway analysis. Results: Majority (61%) of participants reported at least one ACE. A dose-response relationship between numbers of ACEs with risk for opioid misuse was present. Compared to participants with no ACEs, participants with [greater than or equal to]4 ACEs and 0-3 ACEs were 2.93 (95% CI: 1.95, 4.39; p < 0.001) and 1.96 (95% CI: 1.46, 2.65; p < 0.001) times more likely to be at risk for opioid misuse, respectively. Having at least one existing or past health condition significantly mediated the association. Conclusions: Our findings suggest need to include assessment of ACEs as a screening criterion for opioid prescription and administration among college-aged individuals.

Floral characteristics of 27 mango varieties were studied during February-May, 2024. Distinct variations were found among the studied varieties. Significant variation were observed in term of number of male flower per inflorescence, number of hermaphrodite flower per inflorescence, length of inflorescence and width of inflorescence ranging from 57 to 1737, 7 to 461, 19 cm to 40.6 cm, 6.4 cm to 25.7 cm respectively. The result disclosed that in all varieties flowers were of pentamerous type and inflorescence position were found terminal. The number of male flower per inflorescence were highest in Bombay (1737). In term of number of hermaphrodite flowers per inflorescence, chausa has highest number (461). In overall, male flower per inflorescence were more than hermaphrodite flower per inflorescence across all varieties except Amrapali, Jarda and Chausa. Ratna has the longest inflorescence (40.6 cm) and widest inflorescence (25.7 cm). From the study, it can be inferred that chausa will have more fruit set as it has more hermaphrodite flowers but seeing the overall floral character Ratna is more superior as it has longest and widest inflorescence and second highest in term of number of hermaphrodite flower among the studied varieties. The findings of the study will be beneficial for breeding purposes while developing new varieties of superior quality. [ABSTRACT FROM AUTHOR]

This mixed methods study assessed the perceptions of food choices among SNAP participants and SNAP-eligible non-participants when exposed to two alternative SNAP policy options: restrictions on sugar-sweetened beverages (SSBs) and incentives for fruit and vegetable (FV) purchases. Twelve focus groups that included a randomized food choice experiment were conducted in seven counties in Georgia, involving 73 participants. Participants had a favorable view toward incentivizing FV; responses toward SSB restrictions were mixed. FV incentives were associated with greater spending on FV and lower spending on SSBs, and SSB restriction was associated with lower spending on SSBs. [ABSTRACT FROM AUTHOR]

The present study investigates the spatial correlation between built and natural environments that promote physical activity (PA) and children's fitness. Previous work has been conducted on various geographic scales such as from a county level. However, evaluations from a school attendance zone level, which are the policy and program administration units for schools, have yet to be investigated. Using a factor analysis, 24 PA-related environmental variables were reduced to six environmental factors and then run factors with geographically weighted regression (GWR) at the level of the elementary school attendance zones (ESAZ) in Georgia. The outside-school environmental factors (park, recreation, terrain, nature and camping) and inside school environmental factor (school PA time) were constructed by ESAZ. We found that of all the PA environment factors, the most highly associated with promoting PA and children's fitness were inside-school PA environment. The outside factors of the park and terrain environment indicated strong local geographic variation in youth health. The result showed local variation in the association between PA environment and fitness among elementary school children in Georgia and highlighted the geographically customized efforts for different regions' policy changes to improve child health. 38. Evolution of HIV-1 Surveillance Drug Resistance Mutations Over 10 Years in New South Wales, Australia.

- Abstract
New South Wales has the greatest burden of HIV in Australia, with 2012 and 2013 recording the highest rates of new diagnoses in 20 years. Concurrently, there has been significant changes in antiretroviral treatments and testing paradigms. We compiled a statewide resistance database to characterize changes in HIV-1 resistance mutations over time. Genotypic antiretroviral resistance testing (GART) was performed on request at three reference laboratories using commercial and in-house methods. In total, 7629 HIV-1 polymerase sequences obtained from GART from 2004 to 2013 were retrospectively collated, reformatted, de-identified, and analyzed using Stanford HIVdb program 7.0 and the 2009 World Health Organization (WHO) surveillance drug resistance mutations (SDRMs). Analyses were performed on subgroups of known treatment naives, treatment experienced, and seroconverters. There has been a decrease in overall rates of prevalent drug resistance mutations from 57.8% in 2004 to 21% in 2013. Dual and triple class resistance mutations have decreased from 32.7% in 2004 to 5.8% in 2013 and 16.4% to 1.2%, respectively. In treatment-naive individuals (n = 450), the frequency of protease inhibitor (PI) mutations remains low at 2.7%. In seroconverters, rates of transmitted drug resistance (TDR) are 6.6%, 3%, 3%, and 1.5% for overall, PI, non-nucleoside reverse transcription inhibitor (NNRTI), and NRTI, respectively. In treatment experienced, rates remain stable with 36.0%, 18.9%, 29.1%, and 6.4% for overall, NNRTI, NRTI, and PI mutations. The most common mutations in treatment experienced occurred at position M184, T215 (NRTI); K103 (NNRTI); I54 (PI). Apparent decreases in prevalent SDRMs can be attributed to changes in GART testing indications over time. In treatment-naive and -experienced subgroups, rates have been stable with low rates of TDR in seroconverters. [ABSTRACT FROM AUTHOR]

BACKGROUND: Global genetic diversity of HIV-1 is a major challenge to the development of HIV vaccines. We aimed to estimate the regional and global distribution of HIV-1 subtypes and recombinants during 1990-2015. METHODS: We searched PubMed, EMBASE (Ovid), CINAHL (Ebscohost), and Global Health (Ovid) for HIV-1 subtyping studies published between Jan 1, 1990, and Dec 31, 2015. We collected additional unpublished HIV-1 subtyping data through a global survey. We included prevalence studies with HIV-1 subtyping data collected during 1990-2015. We grouped countries into 14 regions and analysed data for four time periods (1990-99, 2000-04, 2005-09, and 2010-15). The distribution of HIV-1 subtypes, circulating recombinant forms (CRFs), and unique recombinant forms (URFs) in individual countries was weighted according to the UNAIDS estimates of the number of people living with HIV (PLHIV) in each country to generate regional and global estimates of HIV-1 diversity in each time period. The primary outcome was the number of samples designated as HIV-1 subtypes A, B, C, D, F, G, H, J, K, CRFs, and URFs. The systematic review is registered with PROSPERO, number CRD42017067164. FINDINGS: This systematic review and global survey yielded 2203 datasets with 383 519 samples from 116 countries in 1990-2015. Globally, subtype C accounted for 46·6% (16 280 897/34 921 639 of PLHIV) of all HIV-1 infections in 2010-15. Subtype B was responsible for 12·1% (4 235 299/34 921 639) of infections, followed by subtype A (10·3%; 3 587 003/34 921 639), CRF02_AG (7·7%; 2 705 110/34 921 639), CRF01_AE (5·3%; 1 840 982/34 921 639), subtype G (4·6%; 1 591 276/34 921 639), and subtype D (2·7%; 926 255/34 921 639). Subtypes F, H, J, and K combined accounted for 0·9% (311 332/34 921 639) of infections. Other CRFs accounted for 3·7% (1 309 082/34 921 639), bringing the proportion of all CRFs to 16·7% (5 844 113/34 921 639). URFs constituted 6·1% (2 134 405/34 921 639), resulting in recombinants accounting for 22·8% (7 978 517/34 921 639) of all global HIV-1 infections. The distribution of HIV-1 subtypes and recombinants changed over time in countries, regions, and globally. At a global level during 2005-15, subtype B increased, subtypes A and D were stable, and subtypes C and G and CRF02_AG decreased. CRF01_AE, other CRFs, and URFs increased, leading to a consistent increase in the global proportion of recombinants over time. INTERPRETATION: Global and regional HIV diversity is complex and evolving, and is a major challenge to HIV vaccine development. Surveillance of the global molecular epidemiology of HIV-1 remains crucial for the design, testing, and implementation of HIV vaccines. Abstract Background Influenza A, including avian influenza, is a major public health threat in developed and developing countries. Rapid and accurate detection is a key component of strategies to contain spread of infection, and the efficient diagnosis of influenza-like-illness is essential to protect health infrastructure in the event of a major influenza outbreak. Methods We developed a multiplexed PCR (MT-PCR) assay for the simultaneous diagnosis of respiratory viruses causing influenza-like illness, including the specific recognition of influenza A haemagglutinin subtypes H1, H3, and H5. We tested several hundred clinical specimens in two diagnostic reference laboratories and compared the results with standard techniques. Results The sensitivity and specificity of these assays was higher than individual assays based on direct antigen detection and standard PCR against a range of control templates and in several hundred clinical specimens. The MT-PCR assays provided differential diagnoses as well as potentially useful quantitation of virus in clinical samples. Conclusions MT-PCR is a potentially powerful tool for the differential diagnosis of influenza-like illness in the clinical diagnostic laboratory.
