Abstract

Background: PCEs are hypothesized to increase affinity for primary care careers. Subjects: Four focus groups (FGs) of randomly chosen medical students stratified by gender and year of medical school ( 40 students invited, 30 attended). Approximately 150 year I medical students surveyed by questionnaire. Design: Students were surveyed before and after year I (average response rate, 92%). FGs assessed year I students after the PCE and year 2 students without a PCE (controls). This pilot PCE included 14 half-days of "shadowing'' an office-based generalist during first semester. Analysis: FG transcripts were coded using a published model of career choice in Internal Medicine. Results: Among many positive comments (e.g., "wonderful role models", "'got me fired up about med school"), some unexpected themes emerged, including discomfort with patient relationships("direct patient contact was intimidating and a “tum off"), boredom ("Internal Medicine ... gotrepetitious", "peds was boring ... so many well-child exams") and primary care backlash ("too much push to primary care'). Learning climate comments were often negative in controls but less so in PCE students. When compared with men, women cared more about role conflict (work/home) issues, and made more positive comments about intellectual stimulation of the PCE. Survey results showed no change in interest in a primary care career after the PCE (44% interested before vs 46% after, P = 0.09). Conclusions: A PCE can have positive and negative influences on career choice. These influences may differ by gender. Students could gain a greater affinity for generalism by better preparation for interacting with patients, careful selection of preceptors, and less pressure to enter primary care.

Abstract

Environmental and socio-economic developments induce land-use changes with potentially negative impacts on human well-being. To counteract undesired developments, a profound understanding of the complex relationships between drivers, land use, and ecosystem services is needed. Yet, national studies examining extended time periods are still rare. Based on the Special Report on land use, land management and climate change by the Austrian Panel on Climate Change (APCC), we use the Driver-Pressure-State-Impact-Response (DPSIR) framework to (1) identify the main drivers of land-use change, (2) describe past and future land-use changes in Austria between 1950 and 2100, (3) report related impacts on ecosystem services, and (4) discuss management responses. Our findings indicate that socio-economic drivers (e.g., economic growth, political systems, and technological developments) have influenced past land-use changes the most. The intensification of agricultural land use and urban sprawl have primarily led to declining ecosystem services in the lowlands. In mountain regions, the abandonment of mountain grassland has prompted a shift from provisioning to regulating services. However, simulations indicate that accelerating climate change will surpass socio-economic drivers in significance towards the end of this century, particularly in intensively used agricultural areas. Although climate change-induced impacts on ecosystem services remain uncertain, it can be expected that the range of land-use management options will be restricted in the future. Consequently, policymaking should prioritize the development of integrated land-use planning to safeguard ecosystem services, accounting for future environmental and socio-economic uncertainties.

Abstract

Concurrent multiscale techniques such as Adaptive Resolution Scheme (AdResS) can offer ample computational advantages over conventional atomistic (AT) molecular dynamics simulations. However, they typically rely on aphysical hybrid regions to maintain numerical stability when high-resolution degrees of freedom (DOFs) are randomly re-inserted at the resolution interface. We propose an Energy Minimized AT (DOF) Insertion (EMATI) method that uses an informed rather than random AT DOF insertion to tackle the root cause of the issue, i.e., overlapping AT potentials. EMATI enables us to directly couple AT and coarse-grained resolutions without any modifications of the interaction potentials. We exemplify AdResS-EMATI in a system of liquid butane and show that it yields improved structural and thermodynamic properties at the interface compared to competing AdResS approaches. Furthermore, our approach extends the applicability of the AdResS without a hybrid region to systems for which force capping is inadequate. [ABSTRACT FROM AUTHOR]

Abstract

Expression of the immediate-early response gene IER2 has been associated with the progression of several types of cancer, but its functional role is poorly understood. We found that increased IER2 expression in human melanoma is associated with shorter overall survival, and subsequently investigated the mechanisms through which IER2 exerts this effect. In experimental melanoma models, sustained expression of IER2 induced senescence in a subset of melanoma cells in a p53/MAPK/AKT-dependent manner. The senescent cells produced a characteristic secretome that included high levels of the extracellular phosphoglycoprotein osteopontin. Nuclear localization of the IER2 protein was critical for both the induction of senescence and osteopontin secretion. Osteopontin secreted by IER2-expressing senescent cells strongly stimulated the migration and invasion of non-senescent melanoma cells. Consistently, we observed coordinate expression of IER2, p53/p21, and osteopontin in primary human melanomas and metastases, highlighting the pathophysiological relevance of IER2-mediated senescence in melanoma progression. Together, our study reveals that sustained IER2 expression drives melanoma invasion and progression through stimulating osteopontin secretion via the stochastic induction of senescence.

Abstract

The main aim of the current study was to present the abilities of widely used crop models to simulate four different field crops (winter wheat, spring barley, silage maize and winter oilseed rape). The 13 models were tested under Central European conditions represented by three locations in the Czech Republic, selected using temperature and precipitation gradients for the target crops in this region. Based on observed crop phenology and yield from 1991 to 2010, performances of individual models and their ensemble were analyzed. Modelling of anthesis and maturity was generally best simulated by the ensemble median (EnsMED) compared to the ensemble mean and individual models. The yield was better simulated by the best models than estimated by an ensemble. Higher accuracy was achieved for spring crops, with the best results for silage maize, while the lowest accuracy was for winter oilseed rape according to the index of agreement (IA). Based on EnsMED, the root mean square errors (RMSEs) for yield was 1365 kg/ha for winter wheat, 1105 kg/ha for spring barley, 1861 kg/ha for silage maize and 969 kg/ha for winter oilseed rape. The AQUACROP and EPIC models performed best in terms of spread around the line of best fit (RMSE, IA). In some cases, the individual models failed. For crop rotation simulations, only models with reasonable accuracy (i.e. without failures) across all included crops within the target environment should be selected. Abstract

A 66-year-old woman was admitted to a hospital burn unit with a peeling rash over 85% of her body. She had a history of chronic kidney failure and arthritis, for which she took corticosteroids. Six weeks before admission, she had started methotrexate for her arthritis. Three weeks later she developed ulcers in her mouth and throat and was admitted to a hospital. She developed skin lesions on her back, which spread to the rest of her body except her legs, and eventually the skin began to peel away. She was given antibiotics and transferred to a burn unit. Blood cultures revealed infection with Klebsiella pneumoniae. CT scans showed lung and brain abnormalities. She died 27 days after admission to the burn unit. At autopsy, her skin had peeled away over 90% of her body. Examination of lung and brain tissue revealed extensive fungal infection with Aspergillus fumigatus. The destruction of her outer skin layer was probably caused by one of the drugs she was taking.

Abstract

There are different BCR-ABL1 fusion genes that are translated into proteins that are different from each other, yet all leukemogenic, causing chronic myeloid leukemia (CML) or acute lymphoblastic leukemia. Their frequency has never been systematically investigated. In a series of 45503 newly diagnosed CML patients reported from 45 countries, it was found that the proportion of e13a2 (also known as b2a2) and of e14a2 (also known as b3a2), including the cases co-expressing e14a2 and e13a2, was 37.9% and 62.1%, respectively. The proportion of these two transcripts was correlated with gender, e13a2 being more frequent in males (39.2%) than in females (36.2%), was correlated with age, decreasing from 39.6% in children and adolescents down to 31.6% in patients ≥ 80 years old, and was not constant worldwide. Other, rare transcripts were reported in 666/34561 patients (1.93%). The proportion of rare transcripts was associated with gender (2.27% in females and 1.69% in males) and with age (from 1.79% in children and adolescents up to 3.84% in patients ≥ 80 years old). These data show that the differences in proportion are not by chance. This is important, as the transcript type is a variable that is suspected to be of prognostic importance for response to treatment, outcome of treatment, and rate of treatment-free remission.

Abstract

A probabilistic crop forecast based on ensembles of crop model output estimates, presented here, offers an ensemble of possible realizations and probabilistic forecasts of green water components, crop yield and green water footprints (WFs) on seasonal scales for selected summer crops. The present paper presents results of an ongoing study related to the application of ensemble forecasting concepts in crop production. Seasonal forecasting of crop water use indicators (evapotranspiration (ET), water productivity, green WF) and yield of rainfed summer crops (maize, spring barley and sunflower), was performed using the AquaCrop model and ensemble weather forecast, provided by The European Centre for Medium-range Weather Forecast. The ensemble of estimates obtained was tested with observation-based simulations to assess the ability of seasonal weather forecasts to ensure that accuracy of the simulation results was the same as for those obtained using observed weather data. Best results are obtained for ensemble forecast for yield, ET, water productivity and green WF for sunflower in Novi Sad (Serbia) and maize in Gross-Enzersdorf (Austria) - average root mean square error (2006-2014) was lt 10% of observation-based values of selected variables. For variables yielding a probability distribution, capacity to reflect the distribution from which their outcomes will be drawn was tested using an Ignorance score. Average Ignorance score, for all locations, crops and variables varied from 1.49 (spring barley ET in Gross-Enzersdorf) to 3.35 (sunflower water productivity in Gross-Enzersdorf).

Abstract

26th European Photovoltaic Solar Energy Conference and Exhibition; 3935-3938, The analysis of high resolution measurement data from Munich showed significant and repeated irradiance peaks. For irradiance enhancement (IE), the observed irradiance is above the theoretically-calculated global irradiance of a clear-sky day (i.e. for the given time with a clear-sky and atmosphere). The increased irradiance is caused mainly by reflections from cumulus clouds. Concurrent with the elevated global irradiance, low ambient and module temperatures were observed, due to cloud coverage between irradiance enhancement peaks. This analysis investigates the essential meteorological observations from these data records. It also depicts the technological effects on a PV system. To illustrate this, the irradiance peaks are classified by their specific characteristics such as magnitude, duration, distribution or slope. Data sets from several Baseline Surface Radiation Network (BSRN) sites, representing various climatic zones are examined to evaluate the magnitude of this effect across Europe and its dependence on general meteorological conditions. It is crucial to understand the PV system response to the monitored IE-effects. The employment of the simulation environment INSEL (for various PV cell types and panels) enables simulations incorporating high resolution measurement data and (commercially available) PV module simulation data. The output power of the modules investigated can be up to 30 % higher than under STC conditions. The system response was also explored by using a solar generator simulator and by evaluating data sets of operating MW PV plants.
