1. Clinical and Radiological Evaluation of Patients with Chest Trauma in the 2023 Great Kahramanmaras Earthquake.

Abstract

Objective: On February 6, 2023, two earthquakes of magnitude 7.8 and 7.6 occurred in Kahramanmaras, affecting south and central Türkiye as well as north and west Syria. The objective of this study is to present the clinical and radiological findings of chest trauma patients in a significant disaster like an earthquake. It aims to contribute to early diagnosis and intervention in multidisciplinary treatment approaches, thereby reducing mortality and morbidity rates. Materials and Methods: This study was a cross-sectional type registry research. A total of 6,500 patients who presented to Kayseri City Hospital due to the earthquake between February 6, 2023 and February 20, 2023 were retrospectively reviewed through medical hospital records. Of these, 150 patients with blunt or penetrating chest trauma were included in the study. Results: Among the 6,500 patients screened, chest trauma was identified in 150 cases. The average age was 45.1 years. Of the patients, 40.0% arrived at the hospital within 0-1 days, while 10.7% arrived after 6 or more days, with the longest duration being 11 days. While 30.7% of patients had no rib fractures, 31.3% had fractures on the right side, 10.7% had bilateral fractures, and 23.3% had 5 or more rib fractures. Complications occurred in 61.3% of cases, 69.3% received medical treatment, and 26.7% underwent tube thoracostomy. The patients' hospital stays ranged from 0 to 162 days, with an average of 18 days and a median of 11.5 days. Conclusion: Türkiye and its surrounding regions are prone to frequent earthquakes, and similar events are expected in the future. A multidisciplinary patient approach, starting from the emergency department, plays a crucial role, with thoracic surgery being an integral part of this approach. [ABSTRACT FROM AUTHOR]

5. Accuracy of Metabolic Imaging-guided Transthoracic Biopsy in Lung Cancer

All rights reserved.Objective: To compare the detection of transthoracic tru-cut biopsy performed on metabolically active areas in positron emission tomography (PET) for the diagnosis of lung cancer, compared to only CT scan-guided biopsy. Study Design: Descriptive study. Place and Duration of Study: Department of Thoracic Surgery, Kayseri City Training and Research Hospital, Turkey, between December 2020 to June 2021. Methodology: Patients of suspected lung cancer with pre-transthoracic biopsy chest computerised tomography and without positron emission tomography were included in Group A; while, patients with both and positron emission tomography chest computerised tomography were included in Group B. Based on the CT findings of the patients in Group A, a biopsy was obtained from the most appropriate place. The patients in Group B were evaluated by a nuclear medicine specialist and the place with the highest maximum standardised uptake value before the biopsy was marked and the area to be biopsied was determined. Results: The malignancy detection rate was significantly higher in Group B (48/50 patients, 96%) than in Group A (36/50 patients, 72%, p=0.001). Two lesions in the Group B (4 %) and 14 lesions in the Group A (28 %) were found to give benign results (p=0.001). Biopsy was repeated in one patient of Group B, and in five patients of Group A due to an initial negative diagnosis. The sensitivity of PET/CT in predicting malignant tumor was 96%, with the positive predictive value (PPV) of 98.0%; while the sensitivity of CT was 74.5%, with PPV of 82%. Conclusion: Transthoracic biopsies taken by considering metabolically active areas of the mass in positron emission tomography-guided can both increase diagnosis rate and reduce the complication rate by preventing repeated biopsies.

6. Katater Torakostomi: 5 Yıllık Klinik Deneyimimiz

Abstract

Background: It was aimed to retrospectively examine the data of patients who underwent catheter thoracostomy to determine their demographic and clinical characteristics and to evaluate the effectiveness of the procedure.Materials and Methods: We retrospectively evaluated 613 catheter thoracostomy procedures performed at the Health Practice and Research Center between January 1, 2013 and December 31, 2017.Results: Five hundred and sixty-seven (92.5%) were unilateral and 46 (7.5%) were bilateral catheter thoracostomy. Three hundred and seventy-seven (61.5%) procedures were performed on male patients and 236 (38.5%) procedures were performed on female patients. The mean age was 68 ± 16 (13-105 years old). The operation indications were 202 (33%) thoracic malignancies, 98 (16%) extrathoracic malignancies, and benign pathologies in 313 (51%) patients.Conclusions: Catheter thoracostomy procedure is being used more and more every day in the treatment of pleural effusions due to patient comfort, ease of application and low cost.Key Words: Effusion, Catheter thoracostomy, Pleura, Amaç: Kateter torakostomi uygulanan hastaların verilerini retrospektif olarak inceleyerek demografik ve klinik özelliklerini belirlemek ve işlemin etkinliği ile ilgili değerlendirmelerde bulunmak amaçlandı.Materyal ve Metod: 1 Ocak 2013- 31 Aralık 2017 tarihleri arasında Sağlık Uygulama ve Araştırma Merkezinde uygulanan 613 adet kateter torakostomi işlemini retrospektif olarak değerlendirdik.Bulgular: Beş yüz altmış yedi (%92.5)’si unilateral, 46 (%7.5)’sı ise bilateral kateter torakostomi idi. Abstract

Amaç: Bu çalışmada nadir görülen spontan pnömomediastinuma dikkat çekmek veklinik tecrübelerimizi aktarmak istedik.Materyal ve Metod: Nisan 2016- Nisan 2018 tarihleri arasında Sağlık Uygulama ve AraştırmaMerkezinde yatırılıp tedavi edilen spontan pnömomediastinum’lu 12 hastayıretrospektif olarak değerlendirdik.Bulgular: Spontan pnömomediastinumlu 12 hastanın 9’u erkek (%75) ve 3’ü (%25)kadındı. Hastaların yaş ortalaması 30,5 (15-42 yaş) idi. İki hasta şişme vedispne, 1 hasta öksürük ve kusma, 1 hasta öksürük ve göğüs ağrısı, 3 hastadispne ve göğüs ağrısı, 3 hasta boğaz ve göğüs ağrısı ve 2 hasta diş çekimisonrası boyunda şişme şikâyeti ile acil servise kabul edildi. Hastaların6’sında (%50) sigara hikayesi mevcuttu.Sonuç: Spontan pnömomediastinum, acilde nadir görülen, hayatı tehdit edenkomplikasyonlar gelişebilecek tedavi edilmesi gerekli bir hastalıktır. Patients had a mean age of 30.5years (range 15-42 years), 2 patients admitted to the emergency service withswelling and dyspnea complaints, 1 patient with cough and vomiting, 1 patientwith cough and chest pain, 3 patients with dyspnea and chest pain, 3 patientswith sore throat and chest pain, 2 patients with swelling in the neck after thetooth extraction. 6 (50%) of the patients had a smoking history.Conclusions: Spontaneous pneumomediastinum is adisease that is seen rarely in emergencies and should be treated. Sometimes life-threateningcomplications may develop. 12. Göğüs Cerrahisi Kliniğine Yatırılarak Tedavi Edilen Geriatrik Popülasyondaki 80–100 Yaş Arası Hastaların Analizi; Beş Yıllık Deneyim

Abstract

Amaç: Göğüs Cerrahisi Kliniğine kabul edilen 80 yaş üzerindeki hastaların profilleri, başvuru nedenleri, tanı ve tedavi yaklaşımları ile morbidite ve mortaliteyi etkileyen faktörler ve oranlarının belirlenmesi amaçlanmıştır.Materyal ve Metot: Hastane bilgi yönetim sisteminden hastalara ait datalar retrospektif olarak incelendi. Hastalar demografik bulguları, yatış nedeni, klinik bulgular, eşlik eden kronik rahatsızlıklar, uygulanan tanı ve tedavi yöntemi, yoğun bakım ihtiyacı, toplam yatış süresi ve sonuçlar açısından değerlendirildi.Bulgular: 2013–2018 arasındaki beş yıllık sürede Göğüs Cerrahi kliniğine 80 yaşından büyük 422 hasta yatırılarak tedavi edildi. Hastaların yaş ortalaması 84,7 (80–99 yaş) idi. Plevral efüzyon ve toraks travmasına bağlı nedenler en sık etiyolojik faktörlerdi. Ortalama hastanede yatış süresi 4,4 gündü. Kırk yedi hasta yoğun bakımda takip edildi, 17 (%4) hasta kaybedildi.Sonuç: Nüfusumuz yaşlanmaya devam etmekte ve 80 yaşından büyük hastalarla Göğüs Cerrahisinde daha sık karşılaşmaktayız. Bu hastaların profilleri, başvuru nedenleri, tanı ve tedavi yaklaşımları ile morbidite ve mortaliteyi etkileyen faktörleri bilmemiz Göğüs Cerrahisi kliniklerine yol gösterici olacaktır.

13. A Thorough View of Tube Thoracostomy, The Most Common Surgical Procedure Performed by Thoracic Surgery Clinics: Ten Years' Clinical Experience

Abstract

AMAÇ: Göğüs cerrahisi pratiğinde en sıkuygulanan cerrahi girişimlerden biri olan tüp torakostomi, plevral boşluktakihava ya da sıvının (kan, lenf, püy vs) drenajını sağlamak amacıyla interkostalaralıktan plevral boşluğa kateter yerleştirilmesi işlemidir. Bu çalışmada tüp torakostomi uygulananhastaların demografik ve klinik özelliklerini belirlemek ve işlemle ilgilikomplikasyonları değerlendirmek amaçlandı.MATERYAL VE METOD: Çalışma retrospektif olarak planlandı.Ocak 2008- Aralık 2017 tarihleri arasındaki 10 yıllık sürede herhangi birnedenle hastanemizde tüp torakostomi uygulanan hastalar çalışmaya alındı.Torasik ve kardiyak operasyonlardan sonra uygulanan tüp torakostomilerdışlandı. Olguların demografik verileri, tüp torakostomi uygulanma endikasyonu,tüp torakostomi kalış süresi, klinik bulgular, gelişen komplikasyonlar vetoplam yatış süresi açısından incelendi.BULGULAR: Ocak 2008- Aralık 2017 tarihleri arasındaki 10yıllık sürede 3567 adet tüp torakostomi işlemi yapıldı. 2072 (%58.1) işlemtravmatik, 1495 (%41.9) işlem ise travma dışı nedenlerden dolayı uygulandı.İşlem endikasyonları ise 1665 (%46.7) pnömotoraks, 688 (%19.3) hemotoraks, 324(%9.1) hemopnömotoraks, 752 (%21.1) plevral efüzyon, 130 (%3,6) ampiyem ve 8(%0.2) şilotorakstı.SONUÇ: Tüp torakostomi, göğüs cerrahisinde elektif ve acil şartlardasıkça kullanılan hayat kurtarıcı bir işlemdir. Tecrübeli ellerde dakikalariçinde ve kolayca uygulanan bir yöntem olmasına rağmen dikkat edilmezseoluşacak komplikasyonları ciddidir. Doğru ve dikkatli bir şekilde yapılan tüptorakostomi mükemmel sonuçlar verir.ANAHTAR SÖZCÜKLER: Toraks dren, göğüs tüpü, tüptorakostomi, AIM: Tube thoracostomy, which is one of the mostcommon surgical interventions performed in thoracic surgery, is the procedureof inserting a catheter from the intercostal space into the pleural space so asto provide drainage of the air or fluid (blood, lymph, pus, etc.) in thepleural space. The objectives of this study were to determine the demographicand clinical characteristics of the patients undergoing tube thoracostomy andto assess the complications due to the procedure.METHODS: The study was planned retrospectively. Patientswho underwent tube thoracostomy due to any reason in our hospital during the10-year period between January 2008 and December 2017 were included in thestudy. Tube thoracostomies performed after thoracic and cardiac operations wereexcluded from the study. Demographic data of the patients were evaluated interms of indications for tube thoracostomy, duration of tube thoracostomy, thoracostomy-inducedcomplications, clinical findings, and the total hospitalization duration. RESULTS: A total of 3567 tube thoracostomies were performed during the 10-yearperiod between January 2008 and December 2017. 2072 (58.1%) were traumatic and1495 (41.9%) were due to non-traumatic reasons. The indications of thethoracostomy were 1665 (46.7%) pneumothorax, 688 (19.3%) hemothorax, 324 (9.1%)hemopneumothorax, 752 (21.1%) pleural effusion, 130 (3.6%) empyema and 8 (0.2%)chylothorax.CONCLUSION: Tube thoracostomy is a life-saving procedure frequently used underelective and emergency conditions in the field of thoracic surgery. Althoughthe procedure is easily performed by the experienced surgeons within minutes,its emergent complications may be serious and life-threatening if carefulattention is not paid. 18. Unusual cavitary lesions of the lung: Analysis of patients with traumatic pulmonary pseudocyst.

METHODS: Between January 2014 and January 2017, 30 patients included in this study who were diagnosed with traumatic pulmonary pseudocysts, from 582 patients who were admitted to our hospital with blunt thoracic trauma. To obtain data from the hospital information management system of patients included in this study, permission was obtained from the medical specialization board and the data were analyzed retrospectively. RESULTS: The mean age of the patients with traumatic pulmonary pseudocysts was 40.8 years (range, 7–75 years), and 11 (36.7%) of the patients were under the age of thirty and 19 (63.3%) were over the age of thirty. Twenty-eight (93.3%) patients had lung contusion and among those who had traumatic pulmonary pseudocysts diameter less than 2 cm, in 10 (33.3%) patients, contusion was present. In 27 patients, TPP was seen in thoracic computed tomography and diagnosed in the first 12 hours, but only in 10 of these patients, traumatic pulmonary pseudocysts was seen on chest x-ray. The extrathoracic injury was found in 15 (50%) patients. Twenty-two (73.3%) patients underwent tube thoracostomy. No patient required a thoracotomy. CONCLUSION: Traumatic pneumatoceles are benign lesions and most of them regress with symptomatic treatment and supportive therapy without any specific treatment. [ABSTRACT FROM AUTHOR]

20. Pulmonary Langerhans Cell Histiocytosis X Incidentally Diagnosed in a Non-Smoker Because Of Simultaneous Bilateral Spontaneous Pneumothorax.

Abstract

Introduction: Pulmonary Langerhans cell histiocytosis X (PLCHx) is a rare interstitial lung disease of unknown aetiology and occurs mostly in young smokers. Lung involvement alone accounts for >85% of the cases; other organs may be involved too (5%-15%). Case report: A 19-year-old boy was presented to the emergency service with sudden-onset, sharp, stabbing chest pain and dyspnoea. The patient had no history of smoking. His medical and family histories were negative for any disease. Thoracic radiography showed increased reticulonodular density with a near-total bilateral pneumothorax at the left and partially at the right. Hence, bilateral tube thoracostomy was performed. High-resolution computed tomography (HRCT) showed pronounced thin-walled cystic structures smaller than 10 mm and small millimetre-sized nodules in the upper and middle zones. Extrapulmonary Langerhans cell histiocytosis was excluded based on the radiological and laboratory findings. Based on the clinical and tomography findings, diagnosis of PLCHx was made. Conclusion: Our case highlights that PLCHx should be suspected in non-smokers. Further studies elucidating the aetiology of PLCHx are needed. [ABSTRACT FROM AUTHOR]

26. The role of telomerase in the etiology of primary spontaneous pneumothorax.

Abstract

Background: This study aims to investigate the role of telomerase activity in the risk of primary spontaneous pneumothorax, which is most frequently encountered in the practice of thoracic surgery., Methods: A total of 61 patients (56 males, 5 females; median age: 29.4 years; range, 17 to 43 years) who underwent treatment for primary spontaneous pneumothorax and 19 age- and sex-matched healthy controls (10 males, 9 females; median age: 29.1 years; range, 23 to 43 years) were included in this prospective study between January 2018 - August 2018. Telomerase activity was evaluated with enzyme-linked immunosorbent assay. The correlation between telomerase activity and clinical and demographic parameters was examined., Results: The mean serum telomerase level was 3.4±0.6 ng/mL in the primary spontaneous pneumothorax group and 1.9±0.5 ng/mL in the control group, indicating significantly higher levels in the patient group (p<0.001). There was no significant association between the telomerase levels and presence of blebs and/or bullae on thoracic computed tomography, extent of pneumothorax, laterality (right, left, or bilateral), and pack years of cigarette smoking., Conclusion: Telomerase levels of patients with primary spontaneous pneumothorax are significantly higher than healthy individuals. Abstract

Although there are a lot of data on intrathoracic and extrathoracic injuries associated with adult sternal fractures and how to manage them; but there is not much data on diagnosis and treatment protocols, since they are rare in children. If the child patient has a sternal fracture, he/she probably had a high-energy trauma, so it should be carefully examined whether if there is an accompanying intrathoracic or extrathoracic injury. In this study, we aimed to determine the demographic data, accompanying injuries, treatment modalities and results in children who were admitted to the emergency trauma unit of our hospital and diagnosed with sternal fracture.
