Abstract

Chronic hepatitis B virus (HBV) infection is a global health problem that presents as a spectrum of liver disease, reflecting an interplay between the virus and the host immune system. HBV genomes exist as episomal covalently closed circular DNA (cccDNA) or chromosomal integrants. The relative contribution of these genomes to the viral transcriptome in chronic hepatitis B (CHB) is not well-understood. We developed a qPCR method to estimate the abundance of HBV cccDNA- and integrant-derived viral transcripts and applied this to a cohort of patients diagnosed with CHB in the HBe antigen negative phase of disease. We noted a variable pattern of HBV transcripts from both DNA templates, with preS1/S2 mRNAs predominating and a significant association between increasing age and the expression of integrant-derived mRNAs, but not with inflammatory status. In contrast, cccDNA-derived transcripts were associated with markers of liver inflammation. Analysis of the inflammatory hepatic transcriptome identified 24 genes significantly associated with cccDNA transcriptional activity. Our study uncovers an immune gene signature that associates with HBV cccDNA transcription and increases our understanding of viral persistence.

Abstract

International audience; Hepatitis B is leading cause of liver related morbidity in Asia with predominant genotypes B and C in East-Asia. Data on Serum, intrahepatic viral-markers, and long-term follow-up of prevalent genotypes (GT) B and C in patients with biopsy proven advanced fibrosis are sparse. To compare serum, intrahepatic viral-markers and development of hepatocellular carcinoma (HCC) in GT-B and C in patients with advanced fibrosis (Ishak \textgreater/= 4). Sixty-three treatment-naive patients identified with advanced fibrosis on liver-biopsy performed between 1998 and 2000 at Singapore General Hospital. FFPE tissue was available for 59 patients and serum for 42 patients. HBV-DNA was quantified in serum and liver while qHBsAg quantified in serum. Patients were followed-up till December 2015. The median age was 47 +/- 16 years, with 77.7% males. About 19 were GT-B, 43 patients were GT-C, and 1 had both GT-B and C. Mean follow-up was 13.5 years. The median serum HBV-DNA was 6.25 +/- 2.17 and 6.58 +/- 1.85 log IU/ml, serum HBsAg was 3.29 +/- 0.80 and 3.45 +/- 1.85 log IU/ml, and intrahepatic HBV-DNA was 0.52 +/- 3.73 copies/cell and 0.4 +/- 1.37 copies/cell in the GT-B and C, respectively (P \textgreater 0.1 in all). Complete cirrhosis (Ishak-6) was present in 47.6%, Ishak-5 fibrosis in 33.3%, and Ishak-4 fibrosis in 19% at recruitment. On follow-up HCC developed in 8/43 in GT-C and in 3/19 GT-B (P = 0.86). Advanced age and cirrhosis were significant factors for development of HCC. No difference in serum HBV-DNA, qHBsAg or intrahepatic HBV-DNA was seen in the two genotypes. Il Commissario designato all’agricoltura e allo sviluppo rurale, Phil Hogan, alla audizione della Commissione agricoltura del Parlamento europeo, e nella sua prima uscita ufficiale in Italia, al Forum Internazionale della Coldiretti, ha espresso come massima priorità quella di dare attuazione alla Politica agricola comune dopo la sua recente riforma, specie per quel che riguarda i pagamenti diretti, il greening, lo sviluppo rurale, la politica di qualità e l’Ocm ortofrutta, con l’impegno di rivederla entro i primi dodici mesi di funzionamento al fine di poterla semplificare. Un annuncio accolto con favore e rilanciato dal ministro per le Politiche agricole che ha parlato di cinque priorità da affrontare nell’immediato futuro: semplificazione della PAC, «che ha prodotto nuova burocrazia», revisione degli strumenti e dei budget per la gestione dei rischi di mercato, tracciabilità ed etichettatura dei prodotti in Europa, interventi sul credito per i giovani, piano per accompagnare il settore lattierocaseario dopo la fine delle quote latte. Tutti temi condivisi anche da Paolo De Castro, parlamentare europeo da poco nominato referente della Commissione Agricoltura del Parlamento europeo per i negoziati di libero scambio tra Unione Europea e Stati Uniti (TTIP), che aggiunge: «In questi anni è mancato il gioco di squadra, non si è riusciti a far comprendere le potenzialità dell’agroalimentare per la crescita economica del continente». A livello di dichiarazioni di molti dei massimi esponenti del mondo politico e agricolo si potrebbe essere rassicurati, non altrettanto se si considerano le notizie e le riflessioni che arrivano da altre componenti del comparto agro-alimentare.

Abstract

The early and mid-career researchers (EMCRs) of scientific communities represent the forefront of research and the future direction in which a field takes. The opinions of this key demographic are not commonly aggregated to audit fields and precisely demonstrate where challenges lie for the future. To address this, we initiated the inaugural International Emerging Researchers Workshop for the global Hepatitis B and Hepatitis D scientific community (75 individuals). The cohort was split into small discussion groups and the significant problems, challenges, and future directions were assessed. Here, we summarise the outcome of these discussions and outline the future directions suggested by the EMCR community.
