31. Triplication of α-globin genes is responsible for unusual α113Leu/α113His-globin chain ratios in sheep

Abstract

By investigations at the DNA and protein level, it has been shown that in sheep a previously detected, presumed quantitative allele of theIIa113His gene, displaying a reduced efficiency (called theIIa113His? gene), is carried by a chromosome bearing three a-globin loci. In particular, five sheep having an a113Leu/a113His-chain ratio of about 13:1 (13:1 phenotype) possessed the —Ia113Leu—IIa113Leu—/—Ia113Leu—IIa113Leu —IIIa113His? genotype. One sheep showing a a113Leu/a113His-chain ratio of about 3:1 (3:1 phenotype) had the —Ia113Leu—IIa113His—/—Ia113Leu—IIa113Leu —IIIa113His? genotype, while one sheep having a chain ratio of about 6:1 (6:1 phenotype) carried the —Ia113Leu—IIa113Leu—IIa113His?—/—Ia113Leu—IIa113Leu —IIIa113His? genotype. Nineteen sheep, displaying the common phenotypes, all possessed the aa/aa gene arrangement. Furthermore, the possible location of the gene with reduced efficiency and the expression of the three genes in the triple a-globin loci chromosome are discussed.

Abstract

Fifty-four patients affected by primary adrenocortical insufficiency have been retrospectively studied. Eighteen were affected by a tuberculous Addison's disease, 24 by an idiopathic Addison's disease and 12 by an unclassifiable form. Modality of arise, clinical presentation and association with other diseases are reviewed. The most useful laboratory and instrumental tests for the diagnosis are discussed and a diagnostic flow-chart is proposed.

Abstract

Torsade de pointes is a polymorphic ventricular tachycardia, associated with prolonged QT interval and characterized by twisting of the mean electrical axis of the QRS complexes around an isoelectric line. The long QT syndrome can be divided into two categories, congenital and acquired. The congenital long QT syndrome may be caused by some gene mutation, whereas the acquired form is usually associated with drugs and electrolyte imbalance. It usually remains asymptomatic or causes presyncope, although it may degenerate into ventricular fibrillation and may cause sudden death. The different presentation depends on the polymorphism that characterizes genotypic and phenotypic expression of proteic channel subunits, and on drug toxicity that provoke subunit dysfunction. The case report presented here is an example of prolonged QT interval syndrome in a patient with cocaine abuse and electrolyte disturbances.
