Abstract

Due to a strong patriotic resistance movement, the Italian conquest of Ethiopia and their colonizing ambitions were hindered by the women at the time. But academicians did not write their roles adequately. For this reason, it is crucial to emphasize that this study closes the historiographical vacuum on the history of women in the provinces. From this point on, the study made an effort to look into the invaluable contributions made by women to Bure Damot’s five-year patriotic resistance movement (1936–1941). As direct participants and Yewust arbegna (inner patriots), the notable women patriots ultimately contributed to Ethiopia’s independence in 1941. Since then, the roles of women have become increasingly valuable since they are portrayed in more ways than men are. In the resistance movement women played a crucial role by cooking food, offering logistical support, serving as active inner patriots, and other ways, while men actively participated in the movement against the enemy. Both published and unpublished historical materials were used in the study of the heroine’s roles as combatant and non-combatant patriots. Crucial sources included elderly people with rudimentary knowledge of the resistance effort in the study area and informants who were patriots. In order to document the untold history of women in the patriotic resistance movement in Bure Damot, 1936–1941, sources were gathered and then analyzed.

Abstract

AbstractFirm patriotic opposition followed the Italian conquest of Addis Ababa, the capital city of Ethiopia, and other metropolitan centers in 1936. Even though Ethiopians had fought against the fascist army, some of them provided various forms of support to the opposing forces. The scholars did not write the brief history of the Italian collaborators, known as bandas. These bandas served as both combatant and non-combatant combatants for the Italian troops. On the other hand, some scholars had enumerated that Italian collaborators were later the patriots of their surrounding province, thus, led for the sovereignty of Ethiopia. Different sources, however, attested to the fact that Ethiopian bandas, despite their past in Qolla Dega Damot Awraja, which the scholars did not write and document, were essential to the Italians’ ability to hinder the nationalist resistance effort. Thus, between 1936 and 1941, the history of Ethiopia’s traitors in Qolla Dega Damot Awrajja was meticulously researched and documented in this work. For political and financial reasons, the Italian collaborators in Qolla Dega Damot Awrajja had served their masters, the fascist troops. Therefore, methodologically, the brief history of the bandas of Qolla Dega Damot Awrajja was documented using a variety of historical published and unpublished sources. Furthermore, oral sources from informants who witnessed the events firsthand and who were knowledgeable about the subject were also considered relevant sources. Once the sources are delivered, they are qualitatively analyzed to record the background information on the previously specified subjects.

Information is lacking regarding the views of patients and concerned healthcare professionals on the informed consent process.Objective: This study is aimed to explore patients’ and healthcare professionals’ perceived barriers during informed consent process and identify suggested solutions for improvement.Methods: Patients who underwent elective surgery, anesthetists, and surgeons were the subjects of a qualitative study employing focused group discussions and in-depth interviews. The study participants were chosen through the use of purposeful sampling.Results: The main barriers identified by the patients include: inadequate explanation about the intended procedure, family’s influence in the decision-making, fear of surgery, fear of light/power interruption, inadequate time for discussion, and not letting the family members attend the discussion. On the other hand, healthcare professionals also identified various barriers, which includes: inability of patients to understand the information, limited time to discuss with patients in detail, poorly designed informed consent form, poor awareness of patients, fear of patient refusal for surgery if the risks and associated health problems are explained, lack of adequate investigation to confidently explain about the disease condition, use of medical jargons, poor documentation habit of professionals and lack of legal system regarding ethical dilemmas.Conclusions and Recommendation: Patients and healthcare professionals have identified avoidable barriers that need the attention of concerned health professionals, educators, and the healthcare delivery system.Keywords: anesthesia, barriers, informed consent, preoperative, surgery

Due to an inadequate healthcare system in low-income nations like Ethiopia, the pandemic has had a devastating impact. Despite this, information on the severity of COVID-19 and related difficulties in Ethiopia is sparse. Therefore, we aimed to determine the survival time to severity and predictors of COVID-19 in Northwest Ethiopia.Methods: A prospective follow-up study was conducted among 202 adult COVID-19 patients in the South Gondar zone treatment centers. Data were entered using EpiData version 3.1 and then exported to Stata 16 for analysis. Kaplan–Meier was used to estimate mean survival time, and Log rank tests were used to compare survival time between explanatory variable groups. A cox-proportional hazards regression model with a 95% confidence interval and a p-value of 0.05 was used to identify covariates associated with the outcome variable.Results: The patients’ average age was 41.2 years. With an IQR of 4– 7 days, the median time to COVID-19 severity was 5 days. The overall COVID-19 severity rate was 6.35 (95% CI: 5.17– 7.86) per 100 person-days observed. Senior adult age group (51– 59 years) (AHR = 3.59, 95% CI: 1.05, 12.23), elderly age group (≥ 60 years) (AHR = 2.11, 95% CI: 1.09, 12.67), comorbidity (AHR = 3.26, 95% CI: 1.48, 7.18), high blood pressure at admission (AHR = 4.36, 95% CI: 1.99, 9.54), and high temperature at admission (AHR = 5.60, 95% CI: 2.55, 12.46) were significantly associated with COVID-19 severity time.Conclusion and Recommendation: Patients with COVID-19 had a short median severity time, and factors like older age, comorbidity, high temperature, and high blood pressure were all independent predictors of severity time. Patients with high body temperature, blood pressure, comorbidity, and advanced age should be the focus of interventions to reduce progression time and improve clinical outcomes.Keywords: COVID-19, time to severity, predictors, South Gondar zone, Ethiopia

Despite the high burden of difficulty, the survival status and the predictors for mortality are not yet determined in Ethiopia. Studies related to this area are scarce. Therefore, we aimed to estimate the survival status and predictors of mortality among breast cancer patients in Northwest Ethiopia.Methods: A retrospective cohort study design was carried out from September 2015 to August 2020 among 482 women who had breast cancer in Northwest Ethiopia. A systematic sampling technique was employed to select the required representative sample. The Cox regression model was used to identify the predictors of mortality among breast cancer patients.Results: For this study, 482 participants had followed for 8824 person-months total analysis time or at-risk time. In our findings, the overall survival of breast cancer patients at the end of two and five years was 54.24% and 25.8%, respectively. In the multivariable Cox regression model, age, stage of BC, menopausal status, and surgical therapy were significant predictors of death.Conclusion: The overall survival after two years was 54.24%, and after five years was 25.8%. This result is lower than the recently published report and indicates that in LMIC, especially in rural cancer centers, the infrastructure and resources for routine screening mammography are often unavailable. Therefore, there is a need to promote early diagnosis of BC at each level of health-care delivery point.Keywords: breast cancer, survival, predictors, Ethiopia

Therefore, this study aimed to assess the magnitude and associated factors of head injury at Debre Tabor Teaching and Referral Hospital in south Gondar zone, North-central Ethiopia.Methods: A cross-sectional study was conducted on 370 trauma patients at Debre Tabor Teaching and Referral Hospital from November 1 to December 30, 2019, using a systematic random sampling technique. The data were collected through a face-to-face interview. The data were analyzed using SPSS version 23. Bivariate and multivariable logistic regression analyses were performed to identify factors associated with head injury. P-value < 0.05 was used to declare statistical significance.Results: The mean (±SD) age of patients was 41.4 (± 11.6) years. The prevalence of head injury was 39.7%, 95% CI (34.9– 44.9%). Two hundred sixty-five (72%) were male and 259 (70%) of all trauma patients were from rural residents. The study also found that younger age (20– 24 years) [AOR=1.2; 95% CI (1.29– 8.86)], being male [(AOR=2.02; 95% CI (1.31– 6.24)], alcohol use [(AOR=6.31; 95% CI (2.03– 16.08)], and rural residence [(AOR= 1.40; 95% CI (1.13– 6.94)] were the factors associated with head injury.Conclusion: Like other studies done in Ethiopia, head injury is a major problem in the study area. The study also revealed that socio-demographic and behavioral factors are mainly associated with head injury. Therefore, appropriate prevention strategies should be devised and implemented against the contributing factors both at the individual and community level to minimize the risk of head injury.Keywords: head injury, prevalence, trauma, Ethiopia

7. Log-Gaussian Cox Processes for Spatiotemporal Traffic Fatality Estimation in Addis Ababa

Abstract

We investigate the spatiotemporal dynamics of traffic accidents in Addis Ababa, Ethiopia, using 2016--2019 data. We formulate the traffic accident intensity as a log-Gaussian Cox Process and model it as a spatiotemporal point process with and without fixed and random effect components that incorporate possible covariates and spatial correlation information. The covariate includes population density and distance of accident locations from schools, from markets, from bus stops and from worship places. We estimate the posterior of the state variables using integrated nested Laplace approximations with stochastic partial differential equations approach by considering Mat\`ern prior. Deviance and Watanabe - Akaike information criteria are used to check the performance of the models. We implement the methodology to map traffic accident intensity over Addis Ababa entirely and on its road networks and visualize the potential traffic accident hotspot areas. The comparison of the observation with the model output reveals that the covariates considered has significant effect for the accident intensity. Moreover, the information criteria results reveal the model with covariate performs well compared with the model without covariates. We obtained temporal correlation of the log-intensity as 0.78 indicating the existence of similar traffic fatality trend in space during the study period.

However, evidence-based information about patient delay in presentation and contributing factors to diagnosis of breast cancer in Ethiopia is scarce.Methods: Institution-based cross-sectional study was conducted at oncology units of the University of Gondar and Felege Hiwot specialized hospitals. A total of 371 female breast cancer patients who were newly diagnosed from September 2019 to April 30, 2020 were included. Data were entered using EPI info version 7.2 and analyzed in SPSS version 23. Descriptive statistics was used to summarize socio-demographic and clinical characteristic of the patients. Multivariable logistic regression at a P-value< 0.05 significance level was used to identify predictors of patient delay.Results: A total of 281 (75.7%) patients had long patient delay of ≥ 90 days (3 months) with the average patient delay time of 8 months, and advanced stage diagnosis was found on 264 (71.2%) of patients. The median age of patients was 40 years. Rural residence (AOR=3.72; 95% CI=1.82– 7.61), illiterate (AOR=3.8; 95% CI=1.71– 8.64), having a painless wound (AOR=3.32; 95% CI=1.93, 5.72), travel distance ≥ 5 km (AOR=1.66; 95% CI=1.09– 3.00), having no lump/swelling in the armpit (AOR=6.16; 95% CI=2.80– 13.54), and no history of any breast problem before (AOR=2.46; 95% CI=(1.43– 4.22) were predictors for long patient delay.Conclusion: Long patient delay and advanced stage diagnosis of breast cancer are higher in our study. Travel distance ≥ 5 km, rural residence, no history of any breast problem before, having no lump/swelling in the arm pit, a painless lump in the breast, and being illiterate were important predictors for patient delay. Therefore, public awareness programs about breast cancer should be designed to prevent patient delay in presentation and to promote early detection of cases before advancement.Keywords: breast cancer, patient delay, predictors, Ethiopia

16. Impact of Parental Migration on Education and Behavioural Outcomes of Children Left behind in Southern Wollo

Abstract

The migration of parents is believed to be for the sake of children and families left behind. However, its impact on children left behind has been overlooked in Southern Wollo, Ethioipia. The impact of parental migration on the education and behavioral outcomes of children left behind has to be investigated in the migration-prone area. The objective of this study was to examine the impact of the migration of parent(s) on the education and behavioral outcomes of children left behind. A total of 622 children of migrating parents and children living with both parents were selected for this study. Data were gathered by using a strength and difficulty questionnaire (SDQ) from children of non-migrating parents and children whose either one or both parents migrated abroad in two weredas of southern Wollo, Ethiopia through the use of a stratified sampling method. Morover18 teachers were selected for an interview and focus group discussion to explore their perception of the influence of migration on the education and literacy of children. The results found that the mean score of LBC with parental migration was 15.86 (SD= 3.18) as compared to children living with intact families with a mean score of 12.06 (SD=3.20). The results of the study demonstrated that a significant mean difference was found between children of migrating parents and children living with both parents (F(1,620)= 219.25, p<0.01). Interview and FGD results also demonstrated that children experienced isolation, longing, sadness, and lack of motivation in schooling following their parents' migration. To mitigate the problem families, government, and non-government organizations that have a stake in children well-being and education that can be affected by migration should discharge their responsibilities accordingly. It is necessary to look at the hidden costs of parental migration on children left behind.

25. Gut microbiota patterns associated with duration of diarrhea in children under five years of age in Ethiopia

Abstract

Introduction: Diarrhea claims about 500,000 lives annually among children under five years of age and about 90% of those deaths occur in Sub-Saharan Africa and South Asia. Childhood mortality due to acute diarrhea (AD; less than 7 days of duration) is decreasing, but prolonged (ProD; 7-13 days of duration) and persistent (PD; more than 14 days of duration) diarrhea combined (ProPD) are responsible for increased morbidity and mortality in low- and middle- income countries (LMICs). Objectives: This study aimed to characterize the gut microbiota (GM) composition of children with acute diarrhea (AD) and prolonged or persistent diarrhea (ProPD) by comparing GM composition between these two groups and with non-diarrheal controls in a case-control study. Methods: The study included fecal samples from 1321 children 0-59 months old. After preprocessing based on sequencing depth, we analyzed the GM of 1313 children comparing 554 AD cases, 95 ProPD cases, and 663 frequency matched, non-diarrheal controls. All children were enrolled from Jimma, Ethiopia. The GM composition was determined using 16S rRNA gene (V4 region) amplicon sequencing. The observed zero-radius Operational Taxonomic Units (zOTUs) and Shannon diversity index were assessed, and Bray-Curtis dissimilarity metrics were generated and differences between groups evaluated by PERMANOVA. DESeq2 was used to determine taxa being differentially abundant between diarrhea subcategories and non-diarrheal controls. Results: All diarrhea cases (AD and ProPD) combined and each group separately were associated with a decrease in bacterial diversity compared to non-diarrheal controls. Relative to non-diarrheal controls, the GM of children with AD or ProPD was enriched in Escherichia spp., Campylobacter spp., and Streptococcus spp. The GM of children with AD or ProPD was characterized by a decreased relative abundance of Prevotella copri, Faecalibacterium prausnitzii and Dialister succinatiphilus compared to non-diarrheal controls. Similarly, compared to AD cases, ProPD cases were characterized by a decreased relative abundance of important gut commensals such as F. prausnitzii, Akkermansia muciniphila, Eubacterium hadrum and Blautia spp. Conclusion: The GM of children with AD and ProPD was enriched in putative pathogens as well as facultative anaerobes, while obligate anaerobes were reduced. The GM of children with ProPD was characterized by a more pronounced reduction of obligate anaerobic gut commensals compared to AD, suggesting that prolonged duration of diarrhea is accompanied by a depletion of healthy gut commensals. This indicates a potential role for targeted dietary supplements to restore gut eubiosis and/ or the use of next generation probiotics to improve ProPD treatment.

Abstract

Background The aftermath of childbirth plays a vital role in addressing unmet family planning requirements and is instrumental in mitigating the risks associated with pregnancies that are closely spaced. Recognizing factors that indicate immediate postpartum utilization of long-acting reversible contraceptives is essential for crafting focused initiatives. This helps meet unaddressed family planning needs and contributes to the reduction of maternal and child mortality rates in the country. Therefore, this research aimed to pinpoint factors influencing the utilization of long-acting reversible contraceptives immediately after childbirth among women delivering in public health facilities in northwest Ethiopia. Methods A facility-based unmatched case-control study was carried out, involving 342 postpartum women (comprising 108 cases and 216 controls) who delivered in public health facilities in northwest Ethiopia. Data collection involved employing a structured interviewer-administered questionnaire, and binary logistic regression analysis was conducted. The statistically significant association between independent factors and the outcome variable was assessed using the adjusted odds ratio with a 95% confidence interval, considering a p-value less than 0.05. Result The educational status of women at secondary level and above (AOR=2.314, 95% CI=1.645-5.043), adherence to ANC follow-up (AOR=2.930, 95% CI=1.829-4.769), lack of desire to have a child (AOR=1.421, 95% CI=1.026-2.006), receiving counseling (AOR=3.029, 95% CI=2.085-5.988), and possessing knowledge (AOR=1.642, 95% CI=1.086-3.186) were identified as significantly correlated with immediate postpartum LARC utilization at p Conclusion In this investigation, the utilization of immediate postpartum long-acting reversible contraceptives (LARC) was significantly linked to women’s educational attainment, attendance at antenatal care (ANC), their desire for additional children, counseling, and knowledge about immediate postpartum LARC. To promote family planning in the postnatal phase, it is recommended to prioritize maternal education, offer comprehensive counseling and health education, and enhance adherence to targeted ANC services.

Abstract

Recent advances in the application of spectral bands from satellite observations and machine learning algorithms (MLA) in the Google Earth Engine (GEE) cloud-computing platform have been demonstrated to enhance the accuracy of mapping forest resources. This study presents a novel method for mapping the natural distribution of highland bamboo (Oldeania alpina) using spectral bands and three machine learning algorithms, namely random forest, gradient tree boosting, and classification and regression tree. First, spectral bands, vegetation indices and textural features including contrast, entropy and inverse difference moment were derived from the Sentinel-2 elevation data. Second, observations were categorized as the dry season (December–February), short rainy season (March-May), main (long) rainy season (June-August), wet season (September–November) and annual composite. Third, the machine learning algorithms were tested using 1882 ground control points collected from field and high spatial resolution images. Finally, the best-performing machine learning algorithm was used to classify and map bamboo forests. The five land cover categories identified were bamboo stands, natural forest, other vegetation, non-vegetated areas and water bodies. The result shows that the random forest classifier is a robust algorithm with an overall accuracy of 94% considering all the annual composite’s spectral, vegetation and textural variables. The highland bamboo coverage (91 km2) in the Andracha district provided valuable insights. Bamboo stands were mostly distributed in the southern and southeastern parts of the district. Here, we show that image compositing and multiple input parameters using machine learning techniques can overcome challenges facing land cover classification, which can hinder accurate mapping of Afromontane forests. We conclude that the incorporation of vegetation indices and textural features in land cover classification increases accuracy of mapping natural highland bamboo coverage and distribution. The application of this new method is a promising prospect not only in Ethiopia but also in Afromontane forests elsewhere in Africa.
