Abstract

Particular interest is now being given to the best treatment of papillary thyroid microcarcinoma (PTMC) due to its significantly increased incidence. PTMC typically shows indolent organic behavior but, in a low percentage of cases, it can express a relative aggressive behavior. Several risk factors have been shown to negatively influence the rate of regional recurrences and metastases such as tumor diameter, Romaage, sex, multifocality, capsular invasion, extracellular diffusion, lymph node metastases, histological variants, mutated Braf and incidentality. The identification of patients with aggressive PTMCs among the majority with low risk lesions is very important to plan an adequate clinical management, thus the most appropriate surgical treatment. The latter includes thyroid lobectomy and total thyroidectomy with central compartment lymphadenectomy, though several studies did not show statistically significant differences in terms of recurrence and mortality rates between the two techniques. At last, it seems crucial to better define those biological features able to improve selection making process of patients with PTMCs aiming to reserve more radical surgery to those patients carrying more aggressive clinicopathologic features and worse prognosis.

Abstract

This book is the 2015-2017 report of the RINTC project, which has been a joint project of ReLUIS and EUCENTRE. The project intended to assess the seismic risk of code conforming structures in Italy. The results presented herein are, in some cases, different from those presented in the previous years and from preliminary literature derived from this project. This is because, as discussed in the body of text, hypotheses and choices have been continuously revised until consolidation; this process might be continuing even after the end of the project. In general, all computed annual structural failure rates closely reflect specific assumptions on design, modeling and analysis of the considered structures, and this has to be always taken into account when discussing the results of this project. The working group of the project (listed below) consists of the following six sub-groups, each of which explores (i) seismic risk assessment of code-conforming structures, designs, modelling, and analyses of (ii) masonry buildings, (iii) precast reinforced concrete buildings, (iv) reinforced concrete buildings, (v) steel buildings, and (vi) base-isolated reinforced concrete buildings. The book is structured such that each chapter presents the research results from the corresponding sub-group: Chapter (i) introduces the objectives and framework of the project; Chapters (ii) ? (vi) develop code-conforming designs, modelling and analyses of buildings for each structural type; Chapter (vii) concludes the study presenting the final results of the computed failure rates for the structures examined in the preceding chapters. Moreover, readers can refer to Appendices A to C for the details of the considerations on (a) usability-preventing limit state, (b) soil-structure interaction, and (c) model-uncertainty, respectively.

Fecal incontinence (FI) is a common condition that has devastating consequences for patients’ QOL. In some patients, the conventional functional pelvic floor electrical stimulation has been effective but is an invasive and embarrassing treatment. The object of the study was to evaluate the feasibility of functional extracorporeal magnetic stimulation (FMS) in strengthening the pelvic floor muscles without an anal plug and the embarrassment of undressing.

Abstract

AIMS: Biomarkers have proven their ability in the evaluation of cardiopulmonary diseases. We investigated the utility of concentrations of the biomarker procalcitonin (PCT) alone and with clinical variables for the diagnosis of pneumonia in patients presenting to emergency departments (EDs) with a chief complaint of shortness of breath. METHODS AND RESULTS: The BACH trial was a prospective, international, study of 1641 patients presenting to EDs with dyspnoea. Blood samples were analysed for PCT and other biomarkers. Relevant clinical data were also captured. Patient outcomes were assessed at 90 days. The diagnosis of pneumonia was made using strictly validated guidelines. A model using PCT was more accurate [area under the curve (AUC) 72.3%] than any other individual clinical variable for the diagnosis of pneumonia in all patients, in those with obstructive lung disease, and in those with acute heart failure (AHF). Combining physician estimates of the probability of pneumonia with PCT values increased the accuracy to >86% for the diagnosis of pneumonia in all patients. Patients with a diagnosis of AHF and an elevated PCT concentration (>0.21 ng/mL) had a worse outcome if not treated with antibiotics (P = 0.046), while patients with low PCT values (<0.05 ng/mL) had a better outcome if they did not receive antibiotic therapy (P = 0.049). CONCLUSION: Procalcitonin may aid in the diagnosis of pneumonia, particularly in cases with high diagnostic uncertainty. Importantly, PCT may aid in the decision to administer antibiotic therapy to patients presenting with AHF in which clinical uncertainty exists regarding a superimposed bacterial infection.

43. Cold-formed thin-walled steel structures as vertical addition and energetic retrofitting systems of existing masonry buildings

Abstract

According to the current trend for sustainable constructions in urban areas, the present paper deals with the analysis of vertical addition systems for energetic retrofitting of existing masonry buildings. Starting from the current European (EN 1998-3:2005) and Italian (NTC 08) technical codes for the seismic assessment of existing structures, first a FEM model has been implemented to investigate the structural performances of masonry units with variable storeys (1, 2 and 3) and material strength (fk = 1, 3 and 6 MPa). Later on, traditional (reinforced concrete, masonry and steel) and innovative (glued laminated timber and cold-formed thin-walled steel) technologies have been proposed as a solution for vertical addition of the studied buildings. Therefore, a numerical campaign of linear dynamic analyses has been undertaken on the examined structures aiming at selecting the best vertical addition solution. The achieved numerical analysis results have provided cold-formed steel systems as the dominant solution for improving the energetic behaviour of the inspected existing masonry buildings. Finally, the achieved numerical results have been confirmed by a consistent and reliable multicriteria decision-making (MCDM) TOPSIS method, where the proposed vertical addition systems have been compared with each other in terms of structural, environmental and economic performance parameters.

Abstract

Prevalence and risk factors of Leishmania infantum, Dirofilaria spp. and other potentially zoonotic or canine-specific endoparasite infections were assessed in 639 kennel dogs from central Italy. To this end, individual blood and fecal samples were examined using parasitological, immunological and molecular techniques. The presence of compatible clinical pictures, as well as age and gender were considered as putative risks factors. To evaluate risk factors, multivariable analysis with logistic regression and univariable analysis with a Chi square test and a Fischer’s exact test were performed. Overall, 52.6% of dogs (95% CI 48.6-56.5) were found positive, while 39.6% of dogs (95% CI 35.8-43.5) were infected by potentially zoonotic species. Leishmania infantum and Dirofilaria repens showed prevalences of 2.5% (95% CI 1.5-4.1) and 2.8% (95% CI 1.7-4.5), respectively. The prevalence of cardiorespiratory parasites was 7.8% (95% CI 5.9-10.3) and included the species Angiostrongylus vasorum, Eucoleus aerophilus, Eucoleus boehmi and D. immitis; the latter showed a prevalence of 0.2% (95% CI 0.001-1). Intestinal parasites were significantly prevalent (38.8%, 95% CI 35-42.7) and they consisted mainly of species of major zoonotic concern, including ancylostomatids, Toxocara canis, Giardia duodenalis, Dipylidium caninum, Taeniidae, Strongyloides stercoralis and Cryptosporidium parvum. Endoparasites were significantly prevalent in clinically suspected dogs. Leishmania infantum and cardiorespiratory nematodes were prevalent in older dogs, while intestinal parasites were prevalent in younger dogs. Results show high dog and public health risks in kennels in central Italy, and suggest the need for more effective control measures.

Abstract

The recent description of a prion disease (PD) case in a free-ranging bottlenose dolphin (Tursiops truncatus) prompted us to carry out an extensive search for the disease-associated isoform (PrPSc) of the cellular prion protein (PrPC) in the brain and in a range of lymphoid tissues from 23 striped dolphins (Stenella coeruleoalba), 5 bottlenose dolphins and 2 Risso s dolphins (Grampus griseus) found stranded between 2007 and 2012 along the Italian coastline. Three striped dolphins and one bottlenose dolphin showed microscopic lesions of encephalitis, with no evidence of spongiform brain lesions being detected in any of the 30 free-ranging cetaceans investigated herein. Nevertheless, we could still observe a prominent PrPC immunoreactivity in the brain as well as in lymphoid tissues from these dolphins. Although immunohistochemical and Western blot investigations yielded negative results for PrPSc deposition in all tissues from the dolphins under study, the reported occurrence of a spontaneous PD case in a wild dolphin is an intriguing issue and a matter of concern for both prion biology and intra/inter-species transmissibility, as well as for cetacean conservation medicine.
