Objective: To investigate the association of movement patterns evaluated by the Dynamic Movement Assessment (DMA) with the occurrence of musculoskeletal injuries in navy cadets., Design: Cohort study., Setting: Participants were filmed performing the six functional tests of the DMA (deep squat, step up, single-leg squat, hop, plank and side-plank tests)., Participants: 240 navy cadets., Main Outcome Measurements: Depending on the number of certain movement patterns on the Dynamic Movement Assessment (DMA), participants were classified as high, moderate, medium or low risk of developing injuries. Predictive associations between injuries and risk classification were examined using logistic regression analysis., Results: Considering sex, previous injuries and the type of secondary school as covariates, participants who were classified as high risk were not more likely to develop injuries. Non-military high school was an independent risk factor for any injuries (OR = 3.14, 95% CI [1.43,6.91]; OR = 4.57, 95% CI [1.92,10.83]), overuse injuries (OR = 2.58, 95% CI [1.05,6.30]; OR = 2.55, 95% CI [1.06, 6.14]) and acute injuries (OR = 4.88, 95% CI [1.19,19.99]), respectively. Previous musculoskeletal symptoms also increased the chance of AI (OR = 4.45, 95% CI [1.15,17.18]; OR = 5.91, 95% CI [1.13,30.88])., Conclusions: Movement patterns evaluated by DMA are not associated with an increased risk of injuries. Objective: To investigate predictors of low back and lower extremity musculoskeletal injury in a cohort of Naval cadets., Design: Prospective Cohort Study METHODS: 545 Naval cadets (Males, n = 394, 72%) were followed-up over eight months. Nine variables were investigated as predictors: history of musculoskeletal symptoms in the last 12 months, ankle dorsiflexion range of motion, sit and reach test, isometric hip abduction and external rotation strength, alignment during the single leg squat test, single leg hop test for distance, prone plank and side plank tests. All injuries that required medical attention were registered. Predictive associations were examined using univariable and multivariable logistic regression analyses., Results: The incidence of all injuries was 7%. Cadets who failed the 60-second plank test (OR = 3.3; 95% CI, 1.2-8.8, P = 0.04), had ≤18 cm in the sit and reach test (OR = 4.0; 95% CI, 1.4-11.2, P = 0.01), or reported pain in the last 12 months in two or more body regions (OR = 2.7; 95% CI, 1.02-7.3, P = 0.04), had greater odds of sustaining an overuse injury. No predictors were identified for acute injuries., Conclusion: Decreased trunk endurance on the prone plank test, reduced posterior chain flexibility on the sit and reach test, and a history of pain reported in two or more sites in the last 12 months were predictors of overuse injuries in Naval cadets. Abstract: Lopes, TJA, Simic, M, Alves, DdS, Bunn, PdS, Rodrigues, AI, Terra, BdS, Lima, MdS, Ribeiro, FM, Vilão, P, and Pappas, E. Physical performance measures of flexibility, hip strength, lower limb power, and trunk endurance in healthy navy cadets: Normative data and differences between sex and limb dominance. J Strength Cond Res 35(2): 458-464, 2021-The objectives were to provide normative data on commonly used physical performance tests that may be associated with musculoskeletal injuries in Navy cadets and assess for sex and limb dominance differences. A large cohort of Navy cadets were assessed for physical performance tests of flexibility (ankle dorsiflexion range of motion and sit and reach), isometric hip strength, lower limb power (single-leg hop), and trunk endurance (plank and side plank tests). Besides providing normative data tables, sex and limb dominance differences were assessed by a 2-way mixed analysis of variance. A total of 545 Brazilian Navy cadets (394 men) representing 79% of the cadets in the Academy participated. Normative reference values were reported as mean ± SD, 95% confidence interval and percentiles. For tests of muscle strength, power, and endurance, men performed better than women (p < 0.001). For flexibility tests, women achieved greater distances than men for the sit and reach test (p < 0.001), but no difference for ankle dorsiflexion (p = 0.51). Overall, there were no clinically relevant differences between limbs. In conclusion, normative data for commonly used physical performance tests were provided. Although no clinically relevant side-to-side differences were found, men presented higher values for lower limb strength and power, as well as trunk endurance than women, whereas women demonstrated increased flexibility. Valuable normative data are provided to professionals who work with young, active populations from the injury prevention or rehabilitation perspective because the current study may help professionals to identify athletes or cadets whose performance is outside the normative values and may be at risk for injury., (Copyright © 2018 National Strength and Conditioning Association.)

Purpose: We analyzed the impact of sex, performance level and substantial speed reductions (SSR) on pacing in the VI Rio 24-h Marines Ultramarathon. This will provide insights into the importance of minimizing speed variations in relation to optimal pacing in endurance events. Methods: Runners (30 males and 21 females), classified as high- (HP) and low-performance (LP) ran the race while having their time recorded every 400 m. The pacing was analyzed as the first 10% (initial epoch), the following 80% (intermediate epoch) and the last 10% of the race (final epoch). The time percentage spent at speeds <3.5 km·h -1 (SSR), 3.5 to 5.9 km·h -1 (walking speed), 6.0 to 8.0 km·h -1 (walk-to-running transition speed) and > 8.0 km·h -1 (running speed) was calculated. Results: Runners showed a reverse J-shaped pacing ( P < 0.001) regardless of sex and performance level, although male ( P < 0.004) and HP runners ( P < 0.001) have preserved a higher mean speed throughout the race. Male and HP runners spent more time at running speed ( P < 0.001) and less time at SSR ( P < 0.001) than female and LP runners. Total distance was inversely correlated with the number of SSR and speed CV in male ( r = -0.47 and r = -0.64, respectively) and female ( r = -0.61 and r = -0.47, respectively). Conclusion: Male, HP runners showed less SSR, conserving a higher mean speed with less variation throughout the race. Results suggest that conservative pacing strategies, with lower speeds in the beginning and higher speeds toward the end, may be the most adequate for different endurance running disciplines. Background: Shoulder pain is common among patients with musculoskeletal pain and the prevalence of patients with subacromial pain syndrome (SAPS) is high. Despite the high prevalence, there is a lack of an extensive evaluation of the proprioception acuity in patients with SAPS. Knowledge of the proprioceptive deficit would assist clinicians in the proper treatment and may offer an alternative explanation for the mechanisms underlying SAPS, which are poorly understood., Objective: To compare the proprioceptive function of the shoulder in patients with SAPS and matched controls., Study Design: Matched case-control study., Setting: Physical Functional Rehabilitation Service of an outpatient clinic., Participants: A total of 32 consecutive patients with SAPS who sought physical therapy for shoulder pain and 32 healthy participants (control group) matched for age, sex, and handedness., Interventions: All participants completed a questionnaire containing sociodemographic information, pain intensity and characteristics, the Numerical Pain Rating Scale, and the Shoulder Pain and Disability Index., Main Outcome Measurements: The proprioceptive assessment was performed through kinesthesia, passive joint position sense (PJPS), and the active joint position sense (AJPS)., Results: The groups showed no statistically significant differences in kinesthesia, PJPS, and AJPS for internal or external rotation. The proprioceptive acuity was not associated with pain intensity or functional disability in patients with SAPS., Conclusions: Participants with SAPS did not present proprioceptive deficits in a pain-free motion of medial and lateral rotation when compared to their matched controls. The proprioceptive deficit may not be involved with the mechanisms underlying SAPS and seems not to interfere with the clinical outcomes of patients with SAPS., Level of Evidence: Level III., (© 2019 American Academy of Physical Medicine and Rehabilitation.)

The incidence of fungal infections is considered a serious public health problem worldwide. The limited number of antimycotic drugs available to treat human and animal mycosis, the undesirable side effects and toxicities of the currently available drugs, and the emergence of fungal resistance emphasizes the urgent need for more effective antimycotic medicines. In this paper, we describe a rapid, simple, and efficient synthetic route for preparation of the antifungal agent butenafine on a multigram scale. This novel synthetic route also facilitated the preparation of 17 butenafine analogues using Schiff bases as precursors in three steps or less. All the synthesized compounds were evaluated against the yeast, Cryptococcus neoformans / C. gattii species complexes and the filamentous fungi Trichophyton rubrum and Microsporum gypseum . Amine 4bd , a demethylated analogue of butenafine, and its corresponding hydrochloride salt showed low toxicity in vitro and in vivo while maintaining inhibitory activity against filamentous fungi.

Background: Physical exercise interventions have been extensively advocated for the treatment of obesity; however, clinical trials evaluating the effectiveness of exercise interventions on weight control show controversial results. Compensatory mechanisms through a decrease in energy expenditure and/or an increase in caloric consumption is a possible explanation. Several physiological mechanisms involved in the energy balance could explain compensatory mechanisms, but the influences of physical exercise on these adjustments are still unclear. Therefore, the present trial aims to evaluate the effects of exercise on non-exercise physical activity energy expenditure, energy intake and appetite sensations among active overweight/obese adults, as well as, to investigate hormonal changes associated with physical exercise., Methods: This study is a randomized controlled trial with parallel, three-group experimental arms. Eighty-one overweight/obese adults will be randomly allocated (1:1:1 ratio) to a vigorous exercise group, moderate exercise group or control group. The trial will be conducted at a military institution and the intervention groups will be submitted to exercise sessions in the evening, three times a week for 65 min, during a 2-week period. The primary outcome will be total spontaneous physical activity energy expenditure during a 2-week period. Secondary outcomes will be caloric intake, appetite sensations and laboratorial biomarkers. Intention-to-treat analysis will be performed using linear mixed-effects models to evaluate the effect of treatment-by-time interaction on primary and secondary outcomes. Data analysis will be performed using SAS 9.3 and statistical significance will be set at p < 0.05., Discussion: The results of the present study will help to understand the effect of physical exercise training on subsequent non-exercise physical activity, appetite and energy intake as well as understand the physiological mechanisms underlying a possible compensatory phenomenon, supporting the development of more effective interventions for prevention and treatment of obesity., Trial Registration: Physical Exercise and Energy Balance trial registry, trial registration number: NCT 03138187 . Background: Musculoskeletal disorders are common among military personnel, especially during the initial basic training period. Prior studies have reported the prevalence rate of overall musculoskeletal symptoms or injuries in different military population and nationalities, especially from North America and Europe; however, very limited information regarding the military population of South America exists. Although Brazil has one of the biggest military forces worldwide (≈335,000 military personnel), currently, to our knowledge, there is no study reporting musculoskeletal symptoms or injury statistics in the Brazilian Armed Forces. Thus, the aims of this study were to describe the 12-month prevalence rate of self-reported musculoskeletal symptoms in cadets and to compare this prevalence rate between sexes and school years., Methods: We conducted a cross-sectional study that took place from January to March 2016. Participants were Navy cadets, of both sexes and from three different school years of a Brazilian Merchant Navy Academy. All volunteers completed an adapted version of the Brazilian Nordic Musculoskeletal Questionnaire that assessed the past 12-month prevalence of musculoskeletal symptoms over eight body regions of the trunk and lower extremity. The Pearson's χ 2 test was conducted to compare prevalence of symptoms per body region between sexes and among the 3 school years. The study has been approved by the Naval Hospital's ethical committee., Results: A total of 545 cadets (394 males), corresponding to 79% of all 688 cadets enrolled at the Merchant Navy Academy, volunteered to participate on this study. Among all cadets, 266 (49%) reported symptoms in at least one body region in the past 12-months. The knee with 116 (21%) and lower back with 96 (18%), were the most prevalent regions. In terms of sex differences, there was higher prevalence of symptoms among females 90 (60%) than males 176 (45%). Furthermore, females reported almost double the prevalence for lower back symptoms (27% vs. 14%, p = 0.001) and 11% higher prevalence of knee symptoms (29% vs. 18%, p = 0.006) than males. Finally, it is important to highlight that cadets from the second (127 [65%]) and third (77 [55%]) school years had higher prevalence of symptoms than cadets from the first year (62 [29%]). The knee and lower back were consistently the two most prevalent regions among all school years, but shin symptoms increased from 3% to 17% (p < 0.001) between the first and second school years., Conclusion: The prevalence of musculoskeletal symptoms in the Brazilian Merchant Navy Academy doubles between initial enrollment and the beginning of the second school year. Females have consistently higher rates of symptoms than males, particularly reporting higher prevalence of knee and lower back pain which are the two most prevalent regions in this population. Prevention efforts should concentrate on the basic training period in an attempt to decrease the prevalence of musculoskeletal symptoms in this population. Since the disclosure of Biginelli reaction by the chemist Pietro Biginelli, functionalized 3,4-dihydropyrimidin-2(1H)-ones/thiones (DHPMs) have emerged as prototypes for the design of compounds with a broad variety of biological activities. This mini-review describes over 100 Biginelli adducts demonstrated to be promising anticancer, inhibitors of calcium channel, anti-inflammatory, antimicrobial and antioxidant agents. Thus, this compilation presents the most notable in vitro and in vivo results for such fascinating class of organic compounds.

Nature is an irrefutable source of inspiration for the modern man in many aspects. The observation and understanding of nature have allowed the development of new materials, new sources of energies, new drugs etc. Specifically, natural products provide a great contribution to the development of new agents for the treatment of infections and antitumor agents. However, obtaining natural products directly from animals, fungi, bacteria, plants etc has been considered not enough to attend the high demand by pharmaceutical industries. In this regard, various strategies based on biotechnological processes or synthetic approaches have been developed. In this scenario the total synthesis can be undoubtedly a useful and powerful tool for obtaining higher amounts of natural products and/or structural modifications thereof. Herein, we emphasize successful examples of total synthesis of galanthamine, morphine, paclitaxel and podophyllotoxin - natural products approved as pharmaceuticals.

The purpose was to compare 24 participants' acute physiological and affective responses to two aerobic prescriptions in three sessions. Anthropometry, International Physical Activity Questionnaire, and VO2max were recorded. In subsequent visits two aerobic prescriptions were randomly applied: one based on VO2max (PBVO2max) and another based on physical activity level (PBPA). Physiological and affective variables were measured in each session. The PBVO2max showed lower risk for dropout than the PBPA. An effect size analysis showed higher ratings on the Feeling Scale in the PBVO2max session at the end of exercise. After categorizing participants by fitness (High, Medium, and Low) according to VO2max, significant differences were observed for the training impulse between Low and High fitness categories, indicating PBVO2Max were sensitive in distinguishing levels of fitness. The PBVO2max, compared to PBPA, seems to provide better physiological and affective responses. VO2max (even if estimated) is recommended as the basis for aerobic prescriptions.
