Abstract

Introduction and purpose: Palliative care is designed to ensure proper comfort in living and dying, primarily by alleviating suffering in patients for whom medical interventions have proved ineffective. In situations where suffering is unbearable, palliative care offers "last resort" treatment, namely palliative sedation. This involves the intentional administration of sedative drugs to the patient to reduce the level of consciousness, even inducing sleep, in order to control suffering caused by conditions that are resistant to other treatments. The aim of this article is to present various perspectives on palliative sedation. Description of the state knowledge: Topics discussed include: cessation of hydration and nutrition, loss of patient control over decision-making in treatment progression after the induction of pharmacological coma, the principle of double effect, complications arising from the use of indiscriminate polypharmacy, and views on life shortening through palliative sedation or its comparison to euthanasia. The distinction between palliative sedation and euthanasia is explored, with arguments on both sides contributing to ongoing debates. The paper concludes by advocating for comprehensive guidelines and interdisciplinary assessments to ensure ethical and patient-centered decision-making in the application of palliative sedation in terminally ill patients. Summary: A discussion is necessary on the creation of acceptable and appropriate procedures that would not arouse the controversies currently associated with the use of palliative sedation. 3. Using deep reinforcement learning to search reachability properties in systems specified through graph transformation.

Abstract

Today, model checking is one of the essential techniques in the verification of software systems. This technique can verify some properties such as reachability in which the entire state space is searched to find the desired state. However, model checking may lead to the state space explosion problem in which all states cannot be generated due to the exponential resource usage. Although the results of recent model checking approaches are promising, there is still room for improvement in terms of accuracy and the number of explored states. In this paper, using deep reinforcement learning and two neural networks, we propose an approach to increase the accuracy of the generated witnesses and reduce the use of hardware resources. In this approach, at first, an agent starts to explore the state space without any knowledge and gradually identifies the proper and improper actions by receiving different rewards/penalties from the environment to achieve the goal. Once the dataset is fulfilled with the agent's experiences, two neural networks evaluate the quality of each operation in each state, and afterwards, the best action is selected. The significant difficulties and challenges in the implementation are encoding the states, feature engineering, feature selection, reward engineering, handling invalid actions, and configuring the neural network. Finally, the proposed approach has been implemented in the Groove toolset, and as a result, in most of the case studies, it overcame the problem of state space explosion. Also, this approach outperforms the existing solutions in terms of generating shorter witnesses and exploring fewer states. On average, the proposed approach is nearly 400% better than other approaches in exploring fewer states and 300% better than the others in generating shorter witnesses. Abstract

The concept of futile therapy is an attempt to solve the problem that arose on the sidelines of the success of intensive therapy. Both the theoretical bases of futile therapy and the ways of implementing it on the hospital ward raise much controversy. This paper aims to present practical aspects of the implementation of the concept of futile therapy in Polish intensive care units. Increasingly, the ICUs are struggling with prolonged maintenance of organ functions that do not bring therapeutic benefit to the patient (futile therapy). In 2014, under the leadership of Professor Kübler, guidelines for ICUs were established for dealing with the ineffectiveness of maintaining organ functions in patients incapable of providing informed declarations of will. As part of the implementation of futile therapy, it is possible to either withdraw from conducted therapies or withhold from taking new ones. The guidelines are the first official document in Poland containing instructions on how to proceed in futile therapy. The constantly increasing number of futile therapy protocols suggests that the guidelines, despite not being part of the common law, significantly facilitate the work of ICU doctors. In everyday practice, doctors face the difficulty of distinguishing between the situation in which the same therapeutic techniques either aid the treatment of the patient or unnecessarily prolong the process of dying. The futile therapy can take up many forms, this article focuses on practical aspects of the implementation of futile therapy in five different clinical cases.

Abstract

INTRODUÇÃO:Os cuidados paliativos (CP) são caracterizados como cuidados que visam melhorar a qualidade de vida dos pacientes em estado terminal, com ênfase no alívio da dor e de outros sintomas físicos, além do suporte psicológico e emocional para o paciente e sua família.OBJETIVO:Explorar a prática da enfermagem em cuidados paliativos, discutindo as melhores práticas, e desafios.METODOLOGIA:Trata-se de uma revisão narrativa de literatura com o intuito de impulsionar a análise da temática proposta. Foi realizada no mês de fevereiro a abril de 2023, por meio das bases de dados SciELO (Scientific Electronic Library Online), Google Scholar, OPAS (Organização Pan-americana da saúde),e a BVS (Biblioteca virtual em Saúde). Os critérios de inclusão para compor esse estudo foram artigos originais que abordassem a temática deste estudo, que está disponível integralmente online e gratuitamente nos idiomas de português e inglês a partir do período de 2016 a 2023. Os critérios de exclusão foram artigos que não estivessem disponíveis integralmente e publicados a mais de dez anos.RESULTADO E DISCUSSÃO:Destaca-se que após a análise do material encontrado, foi realizada a leitura na íntegra que foram utilizados para construção desse estudo. É notório que a prática de enfermagem frente aos cuidados paliativos seja de fundamental importância para os pacientes em estado terminal, tendo em vista que suas condutas promovem o bem-estar do paciente e sua família, através da comunicação, acolhimento, suporte psicológico e alívio da dor, e através da humanização no cuidado.CONSIDERAÇÕES FINAIS:Os CP têm se mostrado uma maneira eficaz de promover o bem-estar dos pacientes em estado terminal. Os enfermeiros nesse contexto possuem um papel relevante para promoção do cuidado, sendo suporte para família desde o diagnóstico até o final da vida.

Abstract

O objetivo deste artigo é interpretar dados obtidos num estudo de caso de tipo fenomenológico, que pesquisou o sofrimento de dezesseis cuidadores principais (familiares) de doentes em estado terminal, hospitalizados. Os dados desse estudo são aqui interpretados à luz do pré-paradigma salutogénico de A. Antonovsky, que promove a saúde, e não a doença, cumprindo finalidades proclamadas pela OMS, nomeadamente em Ottawa. A metodologia utilizada foi qualitativa, com recurso a interpretação hermenêutica, em conjugação com análise de conteúdo (baseada nas categorias mais importantes da conceptualização salutogénica). Os resultados obtidos revelam que todos os familiares identificaram e utilizaram vários Recursos Gerais de Resistência (GRR), passíveis de serem compreendidos à luz das três metacategorias: "compreensibilidade", "gerenciamento" e "significação". Verificou-se também que a utilização/criação dos GRR tem implícita a existência de sentidos de coerência fortes, por parte dos entrevistados, tal como enunciado por Antonovsky. Os resultados possibilitam compreender que existem áreas de formação de profissionais de saúde que podem ser estimuladas em situações similares à estudada. Algumas dessas áreas são a comunicação e a gestão emocional. Os resultados apontam também para a necessidade de investimento em ações de educação para a saúde que promovam o empoderamento psicológico e comunitário dos indivíduos e dos grupos, em geral.The aim of this paper is to interpret data obtained in a phenomenological case study that researched the suffering of sixteen main caregivers (family members) of terminally ill hospitalized patients. The data of that study are interpreted here in light of A. Antonovsky's salutogenic pre-paradigm, which promotes health, not disease, fulfilling the objectives proclaimed by WHO, particularly in Ottawa. The methodology that was used was qualitative with hermeneutic interpretation, combined with content analysis (based on the most important categories of the salutogenic conceptualization). The results show that all the family members identified and utilized various General Resistance Resources (GRR), which can be understood in light of three metacategories: 'comprehensibility', 'manageability' and 'meaningfulness'. It was also found that the use/creation of the GRR implies the existence of strong senses of coherence on the part of the respondents, as stated by Antonovsky. The results allow us to understand that there are areas of health professionals' training that can be stimulated in situations similar to the studied one, such as communication and emotional management, among others. They also point to the need for investment in health education activities, promoting psychological and community empowerment.
