Abstract

Background: Allergic rhinitis (AR) is a highly prevalent disease. We aimed to assess the symptoms that physicians who see patients with AR perceive as the most bothersome in their patients. Methods: We performed a cross-sectional study based on an online questionnaire sent to all members of the Allergic Rhinitis and its Impact on Asthma (ARIA) initiative. The survey included questions on the physicians' perceptions of patients’ AR symptoms as well as of their own AR symptoms. Results: Among 401 respondents, 155 (38.7%) reported having AR. ARIA members reported nasal symptoms to be the most frequent (89.7%) and bothersome (80.0%) symptoms experienced by themselves. Likewise, nasal symptoms were reported by ARIA members as the most frequent (94.8% in members with AR vs 96.0% in members without AR) and bothersome (57.0% in members with AR vs 67.9% in members without AR) in their patients. We found a significant association (p = 0.001) between physicians’ own symptoms and those perceived as the most bothersome in their patients. Conclusion: Physicians perceive nasal symptoms to be the most frequent and the most bothersome symptoms in AR patients. The physicians' personal experiences with AR may influence their perception of patients’ symptoms.

2. Poliomielite e seus efeitos tardios: viver ao ritmo da doença

Abstract

Introdução: Muitos sobreviventes da poliomielite, após período de estabilidade clínica, desenvolvem complicações relacionadas com a doença. Este caso demonstra a necessidade do médico de família familiarizar-se com as múltiplas vertentes das repercussões tardias da poliomielite e reconhecê-las como entidade nosológica. Descrição do caso: Doente, 69 anos, apresenta sequelas de poliomielite. A narrativa de vida evidencia o quanto a luta pela superação da deficiência foi central na sua trajetória. Inicia, décadas após a doença aguda, queixas progressivas de fraqueza muscular, fadiga, mialgias, poliartralgias, parestesias das mãos e depressão, condicionando incapacidade funcional crescente, bem como intolerância ao frio, incontinência urinária e perturbação do sono, por vezes associada a fasciculações. O exame objetivo e exames auxiliares de diagnóstico evidenciam lesão neurogénea crónica nos membros inferiores, síndroma do túnel cárpico e diversas doenças músculoesqueléticas. Face a novos sintomas, velhos sentimentos relacionados com a deficiência, que a própria julgava ultrapassados, são despertados. O plano terapêutico envolve uma equipa multidisciplinar e inclui intervenções que incidem sobre: controlo dos sintomas e da evolução da doença; tratamento da psicopatologia coexistente; promoção de estilos de vida salutogénicos; adaptação da habitação e prescrição de ajudas técnicas; combate ao isolamento; mobilização de recursos informais, de serviços de saúde e sociais; transmissão de informação sobre a doença e problemas com ela relacionados. Comentário: Os efeitos tardios da poliomielite podem ser minimizados nos seus sintomas mediante determinadas orientações. A sua gestão efetiva exige um olhar plural e holístico, cabendo ao médico de família identificar o cluster de sinais e sintomas presentes, bem como as suas repercussões psicossociais e, consoante estes, definir um plano terapêutico individualizado.

3. Adoecer longe de casa: desafios psicossociais e coping

Abstract

Introdução: A adaptação a um diagnóstico de cancro tem aspetos comuns a todos os doentes, não obstante o percurso individualizado de cada um. No presente caso ilustramos como os desafios colocados à doente foram moldados pelas suas circunstâncias específicas. Descrição do caso: Mulher de 53 anos, empresária, natural e residente num país africano, a quem foi diagnosticado cancro da mama durante uma estadia em Portugal. A doença e as suas repercussões foram percebidas de forma muito negativa pela doente. A reação inicial foi claramente aversiva. Longe de casa para se tratar, nunca se sentiu integrada (apesar de acompanhada por familiares). Experimentou dificuldades financeiras, agravadas pela instabilidade política no seu país de origem. Com o decorrer do tempo ficou a conhecer melhor a doença, ultrapassou alguns desafios e adaptou-se às alterações na sua vida. Acabou por aceitar as suas novas circunstâncias, sendo capaz de projetar o seu futuro de forma construtiva. Comentário: A distância geográfica entre os locais de residência e de tratamento aumenta as exigências psicossociais do adoecer. Este caso demonstra a adoção de uma variedade de estratégias de coping, em relação com avaliações e reavaliações das circunstâncias. O coping tem múltiplas funções, incluindo a regulação emocional e a gestão dos problemas que causam distress. O coping permite, com base na resiliência pessoal, o controlo e resolução dos problemas ou a aceitação das circunstâncias quando a mudança não é possível. Este caso mostra como os desafios da doença podem ser ultrapassados, mesmo em situações difíceis. O médico de família pode acompanhar a adaptação da pessoa à doença crónica e promover a sua funcionalidade. Isto implica gerir emoções, incluindo a modificação das emoções negativas da doente relativamente à doença.

Abstract

The spread of the interest and the need for process reference models, specifically for small and medium software development organizations, has been a catalyst for generating ISO/IEC 29110 Software Engineering — Lifecycle profiles for Very Small Entities. Based on the Mexican standard NMX-I-059-NYCE-2005, better known as MoProSoft, ISO/IEC 29110 is the first international standard specifically designed for very small entities. Thanks to the COMPETISOFT Project and MoProSoft, the background knowledge and models adoption experience have been introduced in Latin America. In Mexico more than 300 organizations have been evaluated in NMX-I-059-NYCE-2005, in 2009 MoProSoft became a national standard in Peru, while COMPETISOFT included the experience and viewpoints of 13 countries and 23 research groups of the region. As a whole, it gives small software development organizations in the region an advantage in adopting an international standard. This paper clarifies the gap between ISO/IEC 29110 and MoProSoft level 2. As a result of a theoretical and practical review both standards have been mapped defining the coverage level between processes, considering the defined tasks, work products and roles. Several recommendations have been suggested to bridge the gap between these standards. Finally, the effort needed to adopt the Basic Profile of the new international standard starting from the Mexican standard has been estimated. The results have been extended to also consider organizations that adopted COMPETISOFT as their process reference model.

6. Depressão e comorbilidade: Um caso clínico

Abstract

Introdução: É frequente a coexistência de depressão com doenças crónicas. Esta associação leva a pior evolução, tanto do quadro psiquiátrico, como da doença crónica. Contudo, nem sempre o diagnóstico da depressão é evidente e, mesmo quando correctamente identificada, muitas vezes é subtratada, quer por razões imputáveis ao médico, quer por má adesão do doente à terapêutica. Descrição do caso: Maria é grande utilizadora da consulta, a que recorre por tonturas, acufenos, fadiga e dores osteoarticulares. Os problemas de saúde identificados incluem entidades com potencial papel etiológico nestes sintomas, que a doente hipervaloriza. No entanto, esta cumpria irregularmente a terapêutica farmacológica e mantinha hábitos de vida prejudiciais ao controlo desses problemas de saúde. Após múltiplas consultas de padrão semelhante, admitiu, quando questionada, irritabilidade, anedonia, tendência para o isolamento social, hipersónia e humor depressivo que atribuía ao sofrimento físico. Após várias tentativas de introdução de terapêutica antidepressiva, que a doente abandonava após escassos dias alegando efeitos secundários, a marcação de consultas quinzenais resultou em maior adesão à terapêutica, antidepressiva e outra, menos queixas e melhor controlo das suas doenças crónicas. Comentário: A ênfase nos sintomas somáticos e o facto de serem explicáveis no contexto da comorbilidade da doente e, assim, retirados como critérios de suspeição de depressão, levaram a que esta se mantivesse mascarada ao longo do tempo. O médico deve ter presente a frequência da depressão associada a doenças crónicas, de modo a conseguir melhor acuidade diagnóstica. A abordagem inclusiva, que contabiliza sintomas independentemente de serem explicáveis pela patologia coexistente, diminui o risco de não se diagnosticar um quadro depressivo oligossintomático, não obstante poder gerar falsos-positivos. Pelo contrário, a abordagem excludente utilizada durante algum tempo, adiou o diagnóstico (falso-negativo) e o tratamento, com repercussões no controlo da globalidade dos problemas de saúde e na qualidade de vida da doente.

Abstract

A identificação das características necessárias ao exercício, com qualidade, das funções de orientador da formação médica pós-graduada constitui uma necessidade do próprio profissional e das instituições responsáveis pela sua qualificação profissional. Importa, assim, definir um Perfil de Competências do orientador dos médicos em formação que corresponda às necessidades de um processo de formação (especialização) que decorre sobretudo em contexto real de trabalho. Para a definição do Perfil de Competências, os autores fizeram um exercício de análise das funções e tarefas do orientador, identificando as competências que sustentam o seu desempenho. O perfil identificado foi submetido a um grupo de profissionais para validação e, em todas as fases de elaboração, à apreciação de auditores externos. Neste perfil identificam-se e sistematizam-se as características do orientador em três tipos: pré-requisitos, atributos e competências pedagógicas. O Perfil de Competências, constituindo um instrumento útil de auto-avaliação, permite também às instituições responsáveis pela qualificação profissional uma supervisão do exercício pedagógico, contribuindo para o apoio e desenvolvimento técnico individual.

Abstract

La eficacia clínica y anticonceptiva de la inserción de la membrana intra-uterina (MIU) después del tratamiento de un aborto, inevitable o incompleto, fue estudiada en 127 casos. A pesar que la inserción del MIU parece ser un procedimiento seguro sin mayores complicaciones, este dispositivo está asociado con altas tasas de embarazo y retiro por metrorragia y/o dolor y no es adecuado para ser usado después de un aborto incompleto o inevitable. Las tasas acumulativas por tipo de terminación por 100 usuarios a los seis meses de uso fueron de 4.3 por embarazo, 3.9 por expulsiones, y 14.4 por retiros por metrorragia y/o dolor.

BACKGROUND: Validated questionnaires are used to assess asthma control over the past 1-4 weeks from reporting. However, they do not adequately capture asthma control in patients with fluctuating symptoms. Using the Mobile Airways Sentinel Network for airway diseases (MASK-air) app, we developed and validated an electronic daily asthma control score (e-DASTHMA).METHODS: We used MASK-air data (freely available to users in 27 countries) to develop and assess different daily control scores for asthma. Data-driven control scores were developed based on asthma symptoms reported by a visual analogue scale (VAS) and self-reported asthma medication use. We included the daily monitoring data from all MASK-air users aged 16-90 years (or older than 13 years to 90 years in countries with a lower age of digital consent) who had used the app in at least 3 different calendar months and had reported at least 1 day of asthma medication use. For each score, we assessed construct validity, test-retest reliability, responsiveness, and accuracy. We used VASs on dyspnoea and work disturbance, EQ-5D-VAS, Control of Allergic Rhinitis and Asthma Test (CARAT), CARAT asthma, and Work Productivity and Activity Impairment: Allergy Specific (WPAI:AS) questionnaires as comparators. We performed an internal validation using MASK-air data from Jan 1 to Oct 12, 2022, and an external validation using a cohort of patients with physician-diagnosed asthma (the INSPIRERS cohort) who had had their diagnosis and control (Global Initiative for Asthma [GINA] classification) of asthma ascertained by a physician.FINDINGS: We studied 135 635 days of MASK-air data from 1662 users from May 21, 2015, to Dec 31, 2021. The scores were strongly correlated with VAS dyspnoea (Spearman correlation coefficient range 0·68-0·82) and moderately correlated with work comparators and quality-of-life-related comparators (for WPAI:AS work, we observed Spearman correlation coefficients of 0·59-0·68). They also displayed high test-retest reliability (intraclass correlation coefficients range 0·79-0·95) and moderate-to-high responsiveness (correlation coefficient range 0·69-0·79; effect size measures range 0·57-0·99 in the comparison with VAS dyspnoea). The best-performing score displayed a strong correlation with the effect of asthma on work and school activities in the INSPIRERS cohort (Spearman correlation coefficients 0·70; 95% CI 0·61-0·78) and good accuracy for the identification of patients with uncontrolled or partly controlled asthma according to GINA (area under the receiver operating curve 0·73; 95% CI 0·68-0·78).INTERPRETATION: e-DASTHMA is a good tool for the daily assessment of asthma control. This tool can be used as an endpoint in clinical trials as well as in clinical practice to assess fluctuations in asthma control and guide treatment optimisation.FUNDING: None.

Abstract

Drug repurposing is a major field of value-added medicine. It involves investigating and evaluating existing drugs for new therapeutic purposes that address unmet healthcare needs. Several unmet needs in allergic rhinitis could be improved by drug repurposing. This could be game-changing for disease management. Current medications for allergic rhinitis are centered on continuous long-term treatment, and medication registration is based on randomized controlled trials carried out for a minimum of 14 days with adherence of 70% or greater. A new way of treating allergic rhinitis is to propose as-needed treatment depending on symptoms, rather than classical continuous treatment. This rostrum will discuss existing clinical trials on as-needed treatment for allergic rhinitis and real-world data obtained by the mobile health app MASK-air, which focuses on digitally-enabled, patient-centered care pathways.

Abstract

Biomarkers for the diagnosis, treatment and follow-up of patients with rhinitis and/or asthma are urgently needed. Although some biologic biomarkers exist in specialist care for asthma, they cannot be largely used in primary care. There are no validated biomarkers in rhinitis or allergen immunotherapy (AIT) that can be used in clinical practice. The digital transformation of health and health care (including mHealth) places the patient at the center of the health system and is likely to optimize the practice of allergy. Allergic Rhinitis and its Impact on Asthma (ARIA) and EAACI (European Academy of Allergy and Clinical Immunology) developed a Task Force aimed at proposing patient-reported outcome measures (PROMs) as digital biomarkers that can be easily used for different purposes in rhinitis and asthma. It first defined control digital biomarkers that should make a bridge between clinical practice, randomized controlled trials, observational real-life studies and allergen challenges. Using the MASK-air app as a model, a daily electronic combined symptom-medication score for allergic diseases (CSMS) or for asthma (e-DASTHMA), combined with a monthly control questionnaire, was embedded in a strategy similar to the diabetes approach for disease control. To mimic real-life, it secondly proposed quality-of-life digital biomarkers including daily EQ-5D visual analogue scales and the bi-weekly RhinAsthma Patient Perspective (RAAP). The potential implications for the management of allergic respiratory diseases were proposed.

Abstract

Background: Validated combined symptom-medication scores (CSMSs) are needed to investigate the effects of allergic rhinitis treatments. This study aimed to use real-life data from the MASK-air® app to generate and validate hypothesis- and data-driven CSMSs. Methods: We used MASK-air® data to assess the concurrent validity, test-retest reliability and responsiveness of one hypothesis-driven CSMS (modified CSMS: mCSMS), one mixed hypothesis- and data-driven score (mixed score), and several data-driven CSMSs. The latter were generated with MASK-air® data following cluster analysis and regression models or factor analysis. These CSMSs were compared with scales measuring (i) the impact of rhinitis on work productivity (visual analogue scale [VAS] of work of MASK-air®, and Work Productivity and Activity Impairment: Allergy Specific [WPAI-AS]), (ii) quality-of-life (EQ-5D VAS) and (iii) control of allergic diseases (Control of Allergic Rhinitis and Asthma Test [CARAT]). Results: We assessed 317,176 days of MASK-air® use from 17,780 users aged 16-90 years, in 25 countries. The mCSMS and the factor analyses-based CSMSs displayed poorer validity and responsiveness compared to the remaining CSMSs. The latter displayed moderate-to-strong correlations with the tested comparators, high test-retest reliability and moderate-to-large responsiveness. Among data-driven CSMSs, a better performance was observed for cluster analyses-based CSMSs. High accuracy (capacity of discriminating different levels of rhinitis control) was observed for the latter (AUC-ROC = 0.904) and for the mixed CSMS (AUC-ROC = 0.820). Conclusion: The mixed CSMS and the cluster-based CSMSs presented medium-high validity, reliability and accuracy, rendering them as candidates for primary endpoints in future rhinitis trials.

La investigación aborda al patrimonio cultural ferroviario hallado fuera de resguardo institucional por sus condiciones de extensión, que constituyen rutas culturales ferroviarias con las condiciones necesarias desde su instancia material y memoria colectiva, para su denominación como paisajes patrimoniales. La ruta cultural ferroviaria refiere a su mirada socioterritorial, su denominación se fundamenta como elemento cultural vigente en la construcción de ciudadanías. Problematizamos desde la teoría crítica para la investigación del Ferrocarril San Rafael y Atlixco (1898-1930), desde la mirada compleja del espacio que implica el entorno, el patrimonio material y su evocación, desde la memoria colectiva contemporánea.

Abstract

MASK-air® , a validated mHealth app (Medical Device regulation Class IIa) has enabled large observational implementation studies in over 58,000 people with allergic rhinitis and/or asthma. It can help to address unmet patient needs in rhinitis and asthma care. MASK-air® is a Good Practice of DG Santé on digitally-enabled, patient-centred care. It is also a candidate Good Practice of OECD (Organisation for Economic Co-operation and Development). MASK-air® data has enabled novel phenotype discovery and characterisation, as well as novel insights into the management of allergic rhinitis. MASK-air® data show that most rhinitis patients (i) are not adherent and do not follow guidelines, (ii) use as-needed treatment, (iii) do not take medication when they are well, (iv) increase their treatment based on symptoms and (v) do not use the recommended treatment. The data also show that control (symptoms, work productivity, educational performance) is not always improved by medications. A combined symptom-medication score (ARIA-EAACI-CSMS) has been validated for clinical practice and trials. Abstract

Introduction: MASK-air® is an app whose aim is to reduce the global burden of allergic rhinitis and asthma. A transfer of innovative practices was performed to disseminate and implement MASK-air® in European regions. The aim of the study was to examine the implementation of the MASK-air® app in older adults of the Puglia TWINNING in order to investigate (i) the rate of acceptance in this population, (ii) the reasons for refusal and (iii) the evaluation of the app after its use. Methods: All consecutive geriatric patients aged between 65 and 90 years were included by the outpatient clinic of the Bari Geriatric Immunoallergology Unit. After a 1-h training session, older adults used the app for 6 months. A 6-item questionnaire was developed by our unit to evaluate the impact of the app on the management of the disease and its treatment. Results: Among the 174 recruited patients, 102 accepted to use the app (mean age, SD: 72.4 ± 4.6 years), 6 were lost to follow-up, and 63 had a low education level. The reasons given not to use the app included lack of interest (11%), lack of access to a smartphone or tablet (53%), low computer literacy (28%), and distrust (8%). At follow-up, the overall satisfaction was high (89%), the patient considered MASK-air® “advantageous” (95%), compliance to treatment was improved (81%), and the rate of loss to follow-up had decreased to 6%. Conclusion: Older adults with a low level of education can use the MASK-air® app after a short training session.

Abstract

Anaphylaxis is common in old-age adults but is insufficiently understood by physicians, and may be underdiagnosed. This review discusses the specificities of anaphylaxis in this age group and stresses the importance of adrenaline in its management.Data from the European Anaphylaxis Registry on elderly patients is a major finding. Other findings include the prevention of possible anaphylactic reactions in coronavirus disease 2019 vaccination as well as some new epidemiologic data.The most common risk factors are hymenoptera venom and food and drug allergy. Cardiovascular symptoms are the most important ones to reverse in old-age adults, especially due to the multiple comorbidities. Anaphylaxis in old-age adults has a more severe outcome than in younger ones. Polypharmacy is a specific factor to be considered. The Airway, Breathing, Circulation, Disability, Exposure (ABCDE) algorithm is applicable in all clinical emergencies for immediate assessment and treatment, and should be considered for all patients. Adrenaline is the mainstay of the management of the condition. There are no absolute contraindications to the prescription of self-injectable adrenaline in elderly individuals at risk of anaphylaxis.

Abstract

In clinical and epidemiological studies, cut-offs of Patient-Reported Outcome Measures (PROMs) can be used to classify patients into groups of statistical and clinical relevance. However, visual analog scale (VAS) cut-offs in MASK-air® have not been tested.To calculate cut-offs for VAS global, nasal, ocular, and asthma symptoms.In a cross-sectional study design of all MASK-air® participants, we compared (i) approaches based on the percentiles (tertiles or quartiles) of VAS distributions, and (ii) data-driven approaches based on clusters of data from two comparators (VAS work and VAS sleep). We then performed sensitivity analyses for individual countries and for VAS levels corresponding to full allergy control. Finally, we tested the different approaches using MASK-air® real-world cross-sectional and longitudinal data to assess the most relevant cut-offs.We assessed 395,223 days from 23,201 MASK-air® users with self-reported allergic rhinitis. The percentile-oriented approach resulted in lower cut-off values than the data-driven approach. We obtained consistent results in the data-driven approach. Following the latter, the proposed cut-off differentiating "controlled" and "partly-controlled" patients was similar to the cut-off value which had been arbitrarily used (20/100). However, a lower cut-off was obtained to differentiate between "partly-controlled" and "uncontrolled" patients (35 versus the arbitrarily-used value of 50/100).Using a data-driven approach, we were able to define cut-off values for MASK-air® VASs on allergy and asthma symptoms. This may allow for a better classification of rhinitis and asthma patients according to different levels of control, supporting improved disease management.

Abstract

Developmental instability (DI) reflects an organism’s inability to develop an ideal phenotype when challenged by genetic and environmental insults. DI can be estimated via the proxy measure of fluctuating asymmetry (FA), i.e., the small random deviations from perfect bilateral symmetry observed in the morphology of paired traits. The mechanisms involved in the genesis of FA in human populations are relatively unknown, though animal research indicates that hormonal processes may be involved. As maternal stress during pregnancy is detrimental to various developmental processes, elevated prenatal cortisol may represent a causal factor in the subsequent emergence of an asymmetrical phenotype. The main purpose of this pre-registered meta-analysis based on three empirical studies was to investigate whether mid-trimester amniotic cortisol levels predict subsequent FA in finger lengths of infants from Germany, Portugal, and the UK. No statistically significant relationships were observed, and meta-analytic combination of the effect size estimates yielded a null result. We did, however, detect significant positive correlations between the cortisol present in the amniotic fluid and maternal plasma in the Portuguese cohort, and observed that FA in the German cohort was significantly lower at 70-months than at either 9- or 20-months. Taken together, the current findings run contrary to animal research showing that elevated prenatal corticosterone exposure leads to increased FA. However, this may be because a single cortisol assay obtained via amniocentesis is an inadequate proxy for average gestational exposure, and/or that prenatal cortisol levels at an earlier (i.e., first rather than second trimester) stage of pregnancy is what explains variance in subsequent FA.

Abstract

The current COVID-19 pandemic influences many aspects of personal and social interaction, including patient contacts with health care providers and the manner in which allergy care is provided and maintained. Allergen-specific immunotherapy (AIT) is one of the most important treatment options for IgE-mediated allergies and is based on inducing an appropriate immune response in the allergic patient. This manuscript outlines the EAACI recommendations regarding AIT during the COVID-19 pandemic and aims at supporting allergists and all physicians performing AIT in their current daily practice with clear recommendations on how to perform treatment during the pandemic and in SARS-CoV-2 infected patients.

Background: Different treatments exist for allergic rhinitis (AR), including pharmacotherapy and allergen immunotherapy (AIT), but they have not been compared using direct patient data (i.e., “real-world data”). We aimed to compare AR pharmacological treatments on (i) daily symptoms, (ii) frequency of use in co-medication, (iii) visual analogue scales (VASs) on allergy symptom control considering the minimal important difference (MID) and (iv) the effect of AIT. Methods: We assessed the MASK-air® app data (May 2015–December 2020) by users self-reporting AR (16–90 years). We compared eight AR medication schemes on reported VAS of allergy symptoms, clustering data by the patient and controlling for confounding factors. We compared (i) allergy symptoms between patients with and without AIT and (ii) different drug classes used in co-medication. Results: We analysed 269,837 days from 10,860 users. Most days (52.7%) involved medication use. Median VAS levels were significantly higher in co-medication than in monotherapy (including the fixed combination azelastine-fluticasone) schemes. In adjusted models, azelastine-fluticasone was associated with lower average VAS global allergy symptoms than all other medication schemes, while the contrary was observed for oral corticosteroids. AIT was associated with a decrease in allergy symptoms in some medication schemes. A difference larger than the MID compared to no treatment was observed for oral steroids. Azelastine-fluticasone was the drug class with the lowest chance of being used in co-medication (adjusted OR = 0.75; 95% CI = 0.71–0.80). Conclusion: Median VAS levels were higher in co-medication than in monotherapy. Abstract

Co-medication is common among patients with allergic rhinitis (AR), but its dimension and patterns are unknown. This is particularly relevant since AR is understood differently across European countries, as reflected by rhinitis-related search patterns in Google Trends. This study aims to assess AR co-medication and its regional patterns in Europe, using real-world data.We analysed 2015-2020 MASK-airWe analysed 222,024 days (13,122 users), including 63,887 days (28.8%) under monotherapy and 38,315 (17.3%) under co-medication. The median 'VAS Global Symptoms' was 7 for no medication days, 14 for monotherapy and 21 for co-medication (p .001). Medication use peaked during the spring, with similar patterns across different European regions (defined geographically or by Google Trends). Oral HAllergic rhinitis medication patterns are similar across European regions. One third of treatment days involved co-medication. These findings suggest that patients treat themselves according to their symptoms (irrespective of how they understand AR) and that co-medication use is driven by symptom severity.

Data de publicació electrònica: 03-11-2022 Introduction: Data from mHealth apps can provide valuable information on rhinitis control and treatment patterns. However, in MASK-air®, these data have only been analyzed cross-sectionally, without considering the changes of symptoms over time. We analyzed data from MASK-air® longitudinally, clustering weeks according to reported rhinitis symptoms. Methods: We analyzed MASK-air® data, assessing the weeks for which patients had answered a rhinitis daily questionnaire on all 7 days. We firstly used k-means clustering algorithms for longitudinal data to define clusters of weeks according to the trajectories of reported daily rhinitis symptoms. Clustering was applied separately for weeks when medication was reported or not. We compared obtained clusters on symptoms and rhinitis medication patterns. We then used the latent class mixture model to assess the robustness of results. Results: We analyzed 113,239 days (16,177 complete weeks) from 2590 patients (mean age ± SD = 39.1 ± 13.7 years). The first clustering algorithm identified ten clusters among weeks with medication use: seven with low variability in rhinitis control during the week and three with highly-variable control. Clusters with poorly-controlled rhinitis displayed a higher frequency of rhinitis co-medication, a more frequent change of medication schemes and more pronounced seasonal patterns. Six clusters were identified in weeks when no rhinitis medication was used, displaying similar control patterns. The second clustering method provided similar results. Moreover, patients displayed consistent levels of rhinitis control, reporting several weeks with similar levels of control. Conclusions: We identified 16 patterns of weekly rhinitis control. Co-medication and medication change schemes were common in uncontrolled weeks, reinforcing the hypothesis that patients treat themselves according to their symptoms.

Abstract

Introduction: Allergic rhinitis (AR) is a disease characterized by IgE-mediated hypersensitivity responses akin to allergic asthma. Although common in children and young adults, AR can be particularly vexing in the elderly: several studies have underlined its impact on the patient’s self-perceived health-related quality of life (HR-QoL). Available literature data on AR-affected elderly patients remain sparse and often focused on specific characteristics. mHealth solutions such as MASK-air® can be used in assessing salient clinical characteristics and unique shifts in self-perceived HR-QoL in old age people. With this pilot study, we aim to assess these variables in two cohorts of AR-affected elderly patients – one actively involved in the daily use of mHealth applications and the other having never used such a solution – by applying a widespread, validated, and standardized tool. Methods: AR-affected patients aged ≥65 years accessing the outpatient clinic of the Bari Geriatric Immuno-allergology Unit between March and July 2021 were enrolled and assigned to “mHealth” (MASK-air®) and “non-mHealth” cohorts accordingly. Each participant was given a 19-item questionnaire delivered via a custom software solution, with the EuroQoL EQ-5D-5L used to assess HR-QoL. Results: 93 patients (51 mHealth users, 43 non-mHealth users) were enrolled. AR was often either standalone or associated with asthma and conjunctivitis, and 57.4% of the participants reported a negative influence of AR on daily activities. Analysis of HR-QoL showed significantly worse scores in mobility and anxiety/depression dimensions for female patients regardless of app usage, while male non-mHealth users had worse self-care scores. Female mHealth users had worse scores for the self-care and activity dimensions, whereas female non-mHealth users showed worse scores on the pain scale. In general, mHealth users showed a greater degree of anxiety/depression when compared to non-mHealth users, relating to a greater awareness of their health status. Conclusion: The use of an mHealth solution, along with a concise, clinically-validated, comprehensive HR-QoL assessment toolset such as the EQ-5D, can prove beneficial in defining the unique characteristics of AR in the elderly. It can enable a detailed exploration of the impact on specific aspects of quality of life in old age. Raising patient awareness towards a health condition can improve compliance to treatment as well as follow-up. A lack of uniformity in approach, along with missing data pertaining to the general population are critical issues that require further studies. A more thorough diffusion of mHealth usage is also necessary among the geriatric population.

Abstract

The prevalence of asthma in Italy is estimated to be around 4%; it affects approximately 2,000,000 citizens, and up to 80-90% of patients have mild-to-moderate asthma. Despite the clinical relevance of mild-to-moderate asthma, longitudinal observational data are very limited, including data on disease progression (worsening vs. improvement), the response to treatment, and prognosis. Studies are needed to develop long-term, observational, real-life research in large cohorts. The primary outcomes of this study will be based on prospective observation and the epidemiological evolution of mild and moderate asthma. Secondary outcomes will include patient-reported outcomes, treatments over time, disease-related functional and inflammatory patterns, and environmental and life-style influences.This study, called the Mild/Moderate Asthma Network of Italy (MANI), is a research initiative launched by the Italian Respiratory Society and the Italian Society of Allergology, Asthma and Clinical Immunology. MANI is a cluster-based, real world, cross-sectional, prospective, observational cohort study that includes 20,000 patients with mild-to-moderate asthma. Abstract

Objetivo: Identificar los factores que influyen en el hábito de fumar en estudiantes de la Facultad de Enfermería de la Universidad Nacional “San Luis Gonzaga” de Ica. Material y métodos: Estudio descriptivo – transversal. Constituido por 143 estudiantes del I al X Ciclo. Haciendo uso de un cuestionario validado de 12 reactivos de opción múltiples y dicotómicas. Resultados: Factores socioculturales: El motivo que indujo a fumar fue por curiosidad 58%, por presión de grupo 16,8%, considerarse independiente 7,0%, por presencia de fumadores en familia 4,2%. Factores económicos: 84,6% gastan menos de S/5 nuevos soles, y 15,4% gastan más de S/5 Nuevos soles con un monto anual para la compra de cigarrillos 84,6%, y 22,4% semanal o mensual . Factores personales: Consumo de drogas es muy bajo con 1,5%, siendo estos los que fuman ocasionalmente. Respecto al consumo de alcohol el 24,5% beben licor y en su mayoría son los que fuman cigarrillos. Falta de práctica del deporte constituye 44,1%, por conflictos familiares y fracaso o abandono familiar el 30,1% y 25,4% respectivamente. Grupo e táreo predominante fue entre 16 – 20 años el 62,2%, sexo femenino mayoritariamente con 79,7%, edad de inicio fue de 15 años a más con 65% y en menor porcentaje respondieron que iniciaron a los 10 años con 3,5%. Frecuencia de fumar fue ocasional 62,2%. El 26,93% tienen el hábito de fumar (143). Conclusiones: Los factores socioculturales, económicos y personales influyen en el hábito de fumar en estudiantes de la Facultad de Enfermería de la Universidad Nacional San Luis Gonzaga de Ica.

Abstract

La satisfacción del estudiante es descrita como elemento clave en la valoración de la calidad de la educación. Objetivo: Determinar la satisfacción de los estudiantes con la calidad de enseñanza de los Docentes de la Facultad de Enfermería de la Universidad Nacional “San Luis Gonzaga” de Ica. Material y métodos: Estudio descriptivo, transversal, diseño no experimental. Constituido por 143 estudiantes del I al X Ciclo. Se hizo uso de un cuestionario validado por expertos internacionales de 36 reactivos tipo escala Likert. Resultados: los estudiantes mayoritariamente manifestaron estar satisfechos con la calidad de enseñanza de los docentes 62,94%, 24,48% poco satisfecho, 4,90% muy satisfecho, 4,20 insatisfechos y 3,50 muy insatisfechos. Con un promedio ponderado estimado de 4 en indicadores: Nivel de importancia y grado de satisfacción en aspectos: contenido de la unidad y grado de cumplimiento, metodología, desempeño del profesor y del estudiante, y 3 en infraestructura disponible. Conclusiones: Los estudiantes de la Facultad de Enfermería se sienten satisfechos con la calidad de enseñanza de los docentes con un promedio general estimado de 4. Con la contrastación de la hipótesis y aplicando la prueba estadística de T Student, se acepta la hipótesis de investigación (H1).

Abstract

La identidad profesional de los estudiantes de enfermería, su proyección se da en la medida que asimila holísticamente su razón de ser, que es el cuidado hacia las personas sin dejar a un lado el humanismo. Objetivo: determinar la identidad que tienen los estudiantes de la Facultad de Enfermería de la Universidad Nacional “San Luis Gonzaga” de Ica de la profesión. Material y métodos: Estudio descriptivo, transversal no experimental, conformado por 200 estudiantes del VI al X Ciclo. Se utilizó un cuestionario validado por expertos internacionales con 19 reactivos tipo escala Likert. Resultados: Los estudiantes de la Facultad de Enfermería se sienten identificados con la profesión con un 46%, muy identificados con 40%, parcialmente o poco identificados con 11%, y 1,50% no identificados o no muy identificados respectivamente. Con un promedio ponderado estimado de 4 en indicadores: autoconcepto en aspectos autoimagen y autorrealización, motivación en metas educacionales y profesionales, relaciones interpersonales en reconocimiento profesional. Conclusiones: Los estudiantes de la Facultad de Enfermería se sienten identificados con la profesión seleccionada, con un promedio general estimado de 4, con la contrastación de la hipótesis y aplicando la prueba estadística T Student se acepta la hipótesis de investigación (H1).

Abstract

O bjetivo: Determinar las relaciones humanas entre el profesional de enfermeria y paciente en la satisfaccion de necesidades en el servicio de cirugia del Hospital “Santa Maria del Socorro” de Ica. Material y metodos: Diseno descriptivo – transversal, muestra conformada de 134 pacientes hospitalizados en el servicio de cirugia del Hospital “Santa Maria del Socorro” de Ica, mayores de 15 anos. Como instrumento se utilizo un cuestionario previamente validado con preguntas cerradas de opcion multiple. Para confiabilidad del instrumento se aplico una prueba piloto al 15% de la muestra, no incluyendose los 134 pacientes, permitiendo determinar caracteristicas finales del instrumento. Se hizo uso del programa de analisis de datos tabulados (EPIDAD 2,1). Resultados: Grupo etareo predominante fue de 26 a 30 anos (17,9%), siendo el sexo masculino mayoritario con 51,5%, grado de instruccion secundaria completa y estado civil solteros 32,1%; 38,1% respectivamente. Las relaciones humanas en la satisfaccion de necesidades referente a los principios de la comunicacion terapeutica: Interes y permiso fue de muy bueno con un promedio de 2,7 cercano al puntaje esperado que fue 3, en cuanto a las relaciones humanas de honestidad, coherencia, proteccion, satisfaccion de necesidades sociales y asistencia, resulto bueno con 2,6 los 4 primeros y el ultimo con 2,5 de promedio ponderado; en relacion al principio de aceptacion, necesidades biologicas y emocionales con puntajes ponderados mas bajos 2.3, 2,2 y 2,1 respectivamente poco, adecuado o regular. Conclusiones: Las relaciones humanas entre el profesional de enfermeria y paciente en la satisfaccion de necesidades resultaron buenas con un promedio general de 2,5. Objetivo: Determinar el nivel de conocimiento que tienen los estudiantes de la Facultad de Enfermería de la Universidad Nacional “San Luís Gonzaga” de Ica, sobre detección precoz de cáncer de mama. Material y métodos: Diseño descriptivo transversal. Muestra constituida por 136 estudiantes del I al VIII ciclo, cuestionario validado con preguntas cerradas dicotómicas y de opción múltiple. Se utilizó el coeficiente de confiabilidad Alfa-Cronbach, obteniéndose un valor de 0,75, así como la prueba de chi cuadrado (x2) para determinar significancia entre los indicadores y los ciclos de estudios. Resultados: Predomino sexo femenino 92,6%. Conocimiento de la práctica de autoexamen de mama: 72,8%, frecuencia mensual de práctica 61,8% desconocen momento pos menstrual de realizarlo 78,7%, conoce otros medios de diagnóstico 76,5% (Mamografía, TAC), conoce medio de diagnóstico definitivo (Biopsia) 23,5%, conoce edad más frecuente de aparición 71,3% (40 a 50 años), y factores de riesgo: (edad y sexo) 94,1%, fuente de información fue la difusión masiva 44,1%. Prueba estadística de chi cuadrado (x2), se encontró significancia entre frecuencia, momento, edad más frecuente de aparición, medios de diagnóstico, principal fuente de información, y los ciclos de estudios (x2) P < 0,01 los dos primeros y (x2) P < 0,05 los tres siguientes. No encontrándose significancia: factores de riesgo (edad y sexo), antecedentes de práctica de autoexamen de mamas, posición para autoexamen y los ciclos de estudios: (x2) P > 0,05. Conclusiones: Existe conocimiento medio de los estudiantes de enfermería sobre detección precoz de cáncer de mama, con promedio porcentual de respuestas correctas 69,35%.

Abstract

Introduction: Anisakiasis is a zoonosis of parasitic origin whose diffusion seems to be continuously increasing. Objective: The aim of this study was to evaluate the benefits of a fish-free diet in patients allergic to Anisakis simplex as well as underlining the importance of awareness and prevention. Furthermore, we aimed to investigate the critical issues related to the spread of anisakiasis in relation to eating habits. Methods: Patients were assessed by means of skin prick tests (SPTs) and targeted laboratory testing, with an 18-month-long fish-free diet being recommended in cases of severe sensitization. The degree of awareness about anisakiasis was evaluated from interviews. Patients were subjected to follow-up visits after 18 months. Results: A total of 70 cases of sensitization to A. simplex were evaluated. The Interview answers highlighted a general state of misinformation among patients and healthy subjects along with a remarkable underestimation of anisakiasis-related risks. An overall lack of care regarding eating habits and diet plans also emerged. In 21 patients affected by severe sensitization, clinical and laboratory evaluations were repeated after 18 months of the subjects being on a fish-free diet. There was a remarkable improvement in serum IgE levels and clinical symptoms. Conclusion: Data analysis proved the need to implement new and more effective awareness-raising and prevention campaigns in order to reduce the incidence of anisakiasis. It is crucial to establish an adequate diet therapy for sensitized patients. Evaluation of cytokine patterns suggests how a polyphenol-rich regime can activate regulatory T cell function and possibly reduce the allergic and inflammatory components of the disease.

Abstract

Allergen immunotherapy (AIT) is a proven therapeutic option for the treatment of allergic rhinitis and/or asthma. Many guidelines or national practice guidelines have been produced but the evidence- based method varies, many are complex and none propose care pathways. This paper reviews care pathways for AIT using strict criteria and provides simple recommendations that can be used by all stakeholders including health professionals. The decision to prescribe AIT for the patient should be individualized and based on the relevance of the allergens, the persistence of symptoms despite appropriate medications according to guidelines as well as on the availability of good-quality and efficacious extracts. Allergen extracts cannot be regarded as generics. Immunotherapy is selected by specialists for stratified patients. There are no currently available validated biomarkers that can predict AIT success. In adolescents and adults, AIT should be reserved for patients with moderate/severe rhinitis or for those with moderate asthma who, despite appropriate pharmacotherapy and adherence, continue to exhibit exacerbations that appear to be related to allergen exposure, except in some specific cases. Immunotherapy may be even more advantageous in patients with multimorbidity. In children, AIT may prevent asthma onset in patients with rhinitis. mHealth tools are promising for the stratification and follow up of patients.

Abstract

Background: Several studies have suggested an impact of allergic rhinitis on academic productivity. However, large studies with real-world data (RWD) are not available. Objective: To use RWD to assess the impact of allergic rhinitis on academic performance (measured through a visual analog scale [VAS] education and the Work Productivity and Activity Impairment Questionnaire plus Classroom Impairment Questions: Allergy Specific [WPAI+CIQ:AS] questionnaire), and to identify factors associated with the impact of allergic rhinitis on academic performance. Methods: We assessed data from the MASK-air mHealth app of users aged 13 to 29 years with allergic rhinitis. We assessed the correlation between variables measuring the impact of allergies on academic performance (VAS education, WPAI+CIQ:AS impact of allergy symptoms on academic performance, and WPAI+CIQ:AS percentage of education hours lost due to allergies) and other variables. In addition, we identified factors associated with the impact of allergic symptoms on academic productivity through multivariable mixed models. Results: A total of 13,454 days (from 1970 patients) were studied. VAS education was strongly correlated with the WPAI+CIQ:AS impact of allergy symptoms on academic productivity (Spearman correlation coefficient = 0.71 [95% confidence interval (CI) = 0.58; 0.80]), VAS global allergy symptoms (0.70 [95% CI = 0.68; 0.71]), and VAS nose (0.66 [95% CI = 0.65; 0.68]). In multivariable regression models, immunotherapy showed a strong negative association with VAS education (regression coefficient = -2.32 [95% CI = -4.04; -0.59]). Poor rhinitis control, measured by the combined symptom-medication score, was associated with worse VAS education (regression coefficient = 0.88 [95% CI = 0.88; 0.92]), higher impact on academic productivity (regression coefficient = 0.69 [95% CI = 0.49; 0.90]), and higher percentage of missed education hours due to allergy (regression coefficient = 0.44 [95% CI = 0.25; 0.63]). Conclusion: Allergy symptoms and worse rhinitis control are associated with worse academic productivity, whereas immunotherapy is associated with higher productivity.

Abstract

Allergic rhinitis and asthma represent global health problems for all age groups. ARIA (Allergic Rhinitis and its Impact on Asthma) project was initiated during a WHO workshop in 1999 In its 2010 and 2017 Revisions, ARIA has developed clinical practice guidelines for the management of allergic rhinitis and asthma co-morbidities based on GRADE (Grading of Recommendation, Assessment, Development and Evaluation). The 2019 Revision embedded real-life data into the GRADE recommendations to develop next-generation guidelines. Care pathways for allergen immunotherapy proposed a multi-sectoral approach. This paper reviews these two recent documents adding the impact of ARIA in Italy.

Abstract

The study aimed to determine the risk factors in tutoring and dropout in students in a public university methodology: hypothetical deductive, type and quantitative, descriptive and cross-sectional design. The sample consisted of 105 students from the 1st to the 8th semester 2018–2019. 95% female predominance, internal tutoring risk factors are present as motivation only in 48%, self-esteem 38%, food 49%, external factors such as aggression 57%, teacher support 15%, the use of the Web without filter the 85 % they are present. The results of the research led us to conclude that internal and external risk factors are present in the tutoring and dropping out of students.

Abstract

Background Although there are many asymptomatic patients, one of the problems of COVID-19 is early recognition of the disease. COVID-19 symptoms are polymorphic and may include upper respiratory symptoms. However, COVID-19 symptoms may be mistaken with the common cold or allergic rhinitis. An ARIA-EAACI study group attempted to differentiate upper respiratory symptoms between the three diseases. Methods A modified Delphi process was used. The ARIA members who were seeing COVID-19 patients were asked to fill in a questionnaire on the upper airway symptoms of COVID-19, common cold and allergic rhinitis. Results Among the 192 ARIA members who were invited to respond to the questionnaire, 89 responded and 87 questionnaires were analysed. The consensus was then reported. A two-way ANOVA revealed significant differences in the symptom intensity between the three diseases (p < .001). Conclusions This modified Delphi approach enabled the differentiation of upper respiratory symptoms between COVID-19, the common cold and allergic rhinitis. An electronic algorithm will be devised using the questionnaire.

Abstract

There are large between- and within-country variations in COVID-19 death rates. Some very low death rate settings such as Eastern Asia, Central Europe, the Balkans and Africa have a common feature of eating large quantities of fermented foods whose intake is associated with the activation of the Nrf2 (Nuclear factor (erythroid-derived 2)-like 2) anti-oxidant transcription factor. There are many Nrf2-interacting nutrients (berberine, curcumin, epigallocatechin gallate, genistein, quercetin, resveratrol, sulforaphane) that all act similarly to reduce insulin resistance, endothelial damage, lung injury and cytokine storm. They also act on the same mechanisms (mTOR: Mammalian target of rapamycin, PPARγ:Peroxisome proliferator-activated receptor, NFκB: Nuclear factor kappa B, ERK: Extracellular signal-regulated kinases and eIF2α:Elongation initiation factor 2α). They may as a result be important in mitigating the severity of COVID-19, acting through the endoplasmic reticulum stress or ACE-Angiotensin-II-AT1R axis (AT1R) pathway. Many Nrf2-interacting nutrients are also interacting with TRPA1 and/or TRPV1. Interestingly, geographical areas with very low COVID-19 mortality are those with the lowest prevalence of obesity (Sub-Saharan Africa and Asia). It is tempting to propose that Nrf2-interacting foods and nutrients can re-balance insulin resistance and have a significant effect on COVID-19 severity. It is therefore possible that the intake of these foods may restore an optimal natural balance for the Nrf2 pathway and may be of interest in the mitigation of COVID-19 severity.

Abstract

International audience; In all societies, the burden and cost of allergic and chronic respiratory diseases are increasing rapidly. Most economies are struggling to deliver modern health care effectively. There is a need to support the transformation of the health care system for integrated care with organizational health literacy. MASK (Mobile Airways Sentinel NetworK) (1), a new development of the ARIA (Allergic Rhinitis and its Impact on Asthma) initiative, and POLLAR (Impact of Air POLLution on Asthma and Rhinitis, EIT Health) (2), in collaboration with professional and patient organizations in the field of allergy and airway diseases, are proposing real-life integrated care pathways (ICPs) (3)-centred around the patient with rhinitis and using mHealth monitoring of environmental exposure (4).An expert meeting took place at the Pasteur Institute in Paris, December 3, 2018. The aim was to discuss next-generation care pathways: (I) Patient participation, health literacy and self-care through technology-assisted “patient activation”; (II) Implementation of care pathways by pharmacists and (III) Next-generation guidelines assessing the recommendations of GRADE guidelines in rhinitis and asthma using real-world evidence (RWE) assessed by mobile technology.The EU (5) and global political agendas are of great importance in supporting health care transformation. MASK has been recognized by DG Santé as a Good Practice (6) in the field of digitally-enabled, integrated, person-centred care.The one-day meeting objectives were clear (Figure 1). The meeting was followed by a workshop. The present paper reports the background of the two-day meeting.

Abstract

Summary Psoriasis is a multifactorial disease that can be related to genetic, environmental and immunological causes. Therefore, not only a single factor but different aspects contribute to the onset of the disease, varying from individual to individual. It would be characterized by an abnormal proliferation and differentiation of keratinocytes, mediated by a dysregulation in the auto-immune T cell response in which several cytokines participate, including Interleukin (IL)-17, IL-17A, IL-12, IL-22, IL-23. These cells and cytokines are responsible for the aggression on skin cells, inflammation and accelerated reproduction of the cells of the epidermis. Due to the chronic inflammation, psoriasis is frequently associated with other concomitant non-dermatological morbid conditions such as arthropathy which can be complicated by a disabling evolution. Psoriasis is also frequently associated with comorbidities such as Cardiovascular Diseases (CVD), hyperlipidemia, diabetes and obesity. The knowledge of common inflammatory pathways and of the potential links between psoriasis and other diseases should encourage dermatologists to a multidisciplinary approach to psoriasis and to an optimal management also in the light of new therapeutic possibilities.
