1. Expect the unexpected? Challenges of prospectively exploring stakeholder engagement in research

Abstract

Abstract Stakeholder engagement in research is increasingly viewed as making a major contribution to assisting impact. This paper draws on a longitudinal, prospective impact study exploring stakeholder engagement in a 3-year tobacco control research project which used stakeholder engagement in the development, testing and dissemination of its return on investment tool. The paper presents the challenges of data collection when undertaking prospective research on stakeholder engagement in health-related research. The impact study used mixed methods of data collection to explore stakeholder engagement in the target project, comprising surveys, interviews and observations of meetings and events involving stakeholders. Stakeholder engagement that actually occurred in the target project, and the data collection methods of stakeholder engagement that were actually used in the impact study, varied substantially from those intended in each case. Data collection for the impact study was dependent on the target project’s stakeholder engagement, which became substantially reduced. Modifications to data collection for the impact study were required. One of the reasons for the reduction of stakeholder engagement was linked to constraints on the target project to meet non-negotiable deadlines. Another factor was concerns about overburdening stakeholders. The knock-on effect of reduced stakeholder engagement highlighted the impact study’s lack of control over its data collection, which was related to the prospective nature of the study. The authors acknowledge that an advantage of a retrospective approach over a prospective one is researchers knowing about data availability and accessibility from the outset. However, the prospective approach of the impact study enabled insight into stakeholder engagement in real-time: how and when it occurred, the challenges, and the experiences of stakeholders and the target project researchers. While the quantity of data gathered was considerably less than anticipated, the quality of data was rich and enabled the impact study objectives to be achieved. With increasing emphasis on public engagement in research and calls for engagement to be evaluated for impact, this paper aims—by highlighting the challenges the impact study experienced—to provide some insight to future research that seeks to respond to those calls.

2. Helping Middle School Students Acknowledge and Navigate Anxiety: An Action Research

Abstract

Teachers and school counselors have the responsibility not only to build a positive school climate in which students want to perform and stretch themselves academically and socially, but also to create an environment in which students know that their teachers and school counselors are aware of their needs, anxiety, and other factors preventing them from reaching their potential. In the present study, we used an action research design to help students acknowledge their anxiety and know how to navigate it. The results indicate that most students exhibited lower anxiety before and during the test, while their anxiety level was the highest after taking a test. Moreover, the power analysis (p-value) shows that students’ anxiety levels before, during, and after a test did not did not significantly impact each other. Challenges regarding anxiety interventions include: 1) students who used fidgets as toys rather than sensory tools, 2) teachers reluctant to implement coping skills continuously for fear of losing instructional time, and 3) the school’s limited funding for purchasing a wider variety of coping strategies. Suggestions to school administrators and future research studies are discussed.

Abstract

New surgical procedures can expose patients to harm and should be carefully evaluated before widespread use. The InSpace balloon (Stryker, USA) is an innovative surgical device used to treat people with rotator cuff tears that cannot be repaired. We aimed to determine the effectiveness of the InSpace balloon for people with irreparable rotator cuff tears.We conducted a double-blind, group-sequential, adaptive randomised controlled trial in 24 hospitals in the UK, comparing arthroscopic debridement of the subacromial space with biceps tenotomy (debridement only group) with the same procedure but including insertion of the InSpace balloon (debridement with device group). Participants had an irreparable rotator cuff tear, which had not resolved with conservative treatment, and they had symptoms warranting surgery. Eligibility was confirmed intraoperatively before randomly assigning (1:1) participants to a treatment group using a remote computer system. Participants and assessors were masked to group assignment. Masking was achieved by using identical incisions for both procedures, blinding the operation note, and a consistent rehabilitation programme was offered regardless of group allocation. The primary outcome was the Oxford Shoulder Score at 12 months. Pre-trial simulations using data from early and late timepoints informed stopping boundaries for two interim analyses. The primary analysis was on a modified intention-to-treat basis, adjusted for the planned interim analysis. The trial was registered with ISRCTN, ISRCTN17825590.Between June 1, 2018, and July 30, 2020, we assessed 385 people for eligibility, of which 317 were eligible. 249 (79%) people consented for inclusion in the study. 117 participants were randomly allocated to a treatment group, 61 participants to the debridement only group and 56 to the debridement with device group. A predefined stopping boundary was met at the first interim analysis and recruitment stopped with 117 participants randomised. 43% of participants were female, 57% were male. We obtained primary outcome data for 114 (97%) participants. The mean Oxford Shoulder Score at 12 months was 34·3 (SD 11·1) in the debridement only group and 30·3 (10·9) in the debridement with device group (mean difference adjusted for adaptive design -4·2 [95% CI -8·2 to -0·26];p=0·037) favouring control. There was no difference in adverse events between the two groups.In an efficient, adaptive trial design, our results favoured the debridement only group. We do not recommend the InSpace balloon for the treatment of irreparable rotator cuff tears.Efficacy and Mechanism Evaluation Programme, a Medical Research Council and National Institute for Health and Care Research partnership.

Abstract

Polyploidy results from whole-genome duplication and is a unique form of heritable variation with pronounced evolutionary implications. Different ploidy levels, or cytotypes, can exist within a single species, and such systems provide an opportunity to assess how ploidy variation alters phenotypic novelty, adaptability, and fitness, which can, in turn, drive the development of unique ecological niches that promote the coexistence of multiple cytotypes. Switchgrass, Panicum virgatum, is a widespread, perennial C4 grass in North America with multiple naturally occurring cytotypes, primarily tetraploids (4×) and octoploids (8×). Using a combination of genomic, quantitative genetic, landscape, and niche modeling approaches, we detect divergent levels of genetic admixture, evidence of niche differentiation, and differential environmental sensitivity between switchgrass cytotypes. Taken together, these findings support a generalist (8×)–specialist (4×) trade-off. Our results indicate that the 8× represent a unique combination of genetic variation that has allowed the expansion of switchgrass’ ecological niche and thus putatively represents a valuable breeding resource.

Abstract

For nearly 50 years, the vision of using single molecules in circuits has been seen as providing the ultimate miniaturization of electronic chips. An advanced example of such a molecular electronics chip is presented here, with the important distinction that the molecular circuit elements play the role of general-purpose single-molecule sensors. The device consists of a semiconductor chip with a scalable array architecture. Each array element contains a synthetic molecular wire assembled to span nanoelectrodes in a current monitoring circuit. A central conjugation site is used to attach a single probe molecule that defines the target of the sensor. The chip digitizes the resulting picoamp-scale current-versus-time readout from each sensor element of the array at a rate of 1,000 frames per second. This provides detailed electrical signatures of the single-molecule interactions between the probe and targets present in a solution-phase test sample. This platform is used to measure the interaction kinetics of single molecules, without the use of labels, in a massively parallel fashion. To demonstrate broad applicability, examples are shown for probe molecule binding, including DNA oligos, aptamers, antibodies, and antigens, and the activity of enzymes relevant to diagnostics and sequencing, including a CRISPR/Cas enzyme binding a target DNA, and a DNA polymerase enzyme incorporating nucleotides as it copies a DNA template. All of these applications are accomplished with high sensitivity and resolution, on a manufacturable, scalable, all-electronic semiconductor chip device, thereby bringing the power of modern chips to these diverse areas of biosensing.

The ease or perceived ease of getting a medical card for the clinic was positively associated with the timing of prenatal care. Abstract

Background The Hispanic/Latino population in Forsyth County, North Carolina, is growing quickly and experiencing significant disparities in access to care and health outcomes. Assessing community perceptions and utilization of health care resources in order to improve health equity among Hispanics/Latinos at both the county and state levels is critical. Methods Our community engagement process was guided by the Community Health Assets Mapping Partnerships (CHAMP) approach, which helps identify gaps in health care availability and areas for immediate action to improve access to and quality of health care. Specifically, we invited and encouraged the Hispanic/Latino population to participate in 4 different workshops conducted in Spanish or English. Participants were identified as either health care providers, defined as anyone who provides health care or a related service, or health care seekers, defined as anyone who utilizes such services. Results The most commonly cited challenges to access to care were cost of health care, documentation status, lack of public transportation, racism, lack of care, lack of respect, and education/language. These data were utilized to drive continued engagement with the Hispanic community, and action steps were outlined. Limitations While participation in the workshops was acceptable, greater representation of health care seekers and community providers is needed. Conclusions This process is fundamental to multilevel initiatives under way to develop trust and improve relationships between the Hispanic/Latino community and local health care entities in Forsyth County. Follow-through on recommended action steps will continue to further identify disparities, close gaps in care, and potentially impact local and state policies with regard to improving the health status of the Hispanic/Latino community.

Abstract

Background: cancellation of planned surgery impacts substantially on patients and health systems. This study describes the incidence and reasons for cancellation of inpatient surgery in the UK NHS. Methods: we conducted a prospective observational cohort study over 7 consecutive days in March 2017 in 245 NHS hospitals. Occurrences and reasons for previous surgical cancellations were recorded. Using multilevel logistic regression, we identified patient- and hospital-level factors associated with cancellation due to inadequate bed capacity. Results: we analysed data from 14 936 patients undergoing planned surgery. A total of 1499 patients (10.0%) reported previous cancellation for the same procedure; contemporaneous hospital census data indicated that 13.9% patients attending inpatient operations were cancelled on the day of surgery. Non-clinical reasons, predominantly inadequate bed capacity, accounted for a large proportion of previous cancellations. Independent risk factors for cancellation due to inadequate bed capacity included requirement for postoperative critical care [odds ratio (OR)=2.92; 95% confidence interval (CI), 2.12–4.02; PConclusions: a significant proportion of patients presenting for surgery have experienced a previous cancellation for the same procedure. Cancer surgery is relatively protected, but bed capacity, including postoperative critical care requirements, are significant risk factors for previous cancellations.

Abstract

Wiskott–Aldrich syndrome (WAS) is a rare X-linked primary immunodeficiency due to mutations in the WAS gene expressed in hematopoietic cells. Hematopoietic stem cell transplantation (HSCT) is the treatment of choice when an appropriate human leukocyte antigen-matched donor is available. The use of the extracorporeal membrane oxygenation (ECMO) circuit to infuse donor cells for HSCT has not been previously published in the literature. We describe a case of a child who had successful engraftment after HSCT with infusion of the donor stem cells through the ECMO circuit.

Abstract

Background The European-study on Quantifying Utility of Investment in Protection from Tobacco (EQUIPT) project aimed to study transferability of economic evidence by co-creating the Tobacco Return On Investment (ROI) tool, previously developed in the United Kingdom, for four sample countries (Germany, Hungary, Spain and the Netherlands). The EQUIPT tool provides policymakers and stakeholders with customized information about the economic and wider returns on the investment in evidence-based tobacco control, including smoking cessation interventions. A Stakeholder Interview Survey was developed to engage with the stakeholders in early phases of the development and country adaptation of the ROI tool. The survey assessed stakeholders’ information needs, awareness about underlying principles used in economic analyses, opinion about the importance, effectiveness and cost-effectiveness of tobacco control interventions, and willingness to use a Health Technology Assessment (HTA) tool such as the ROI tool. Methods A cross sectional study using a mixed method approach was conducted among participating stakeholders in the sample countries and the United Kingdom. The individual questionnaire contained open-ended questions as well as single choice and 7- or 3-point Likert-scale questions. The results corresponding to the priority and needs assessment and to the awareness of stakeholders about underlying principles used in economic analysis are analysed by country and stakeholder categories. Results Stakeholders considered it important that the decisions on the investments in tobacco control interventions should be supported by scientific evidence, including prevalence of smoking, cost of smoking, quality of life, mortality due to smoking, and effectiveness, cost-effectiveness and budget impact of smoking cessation interventions. The proposed ROI tool was required to provide this granularity of information. The majority of the stakeholders were aware of the general principles of economic analyses used in decision making contexts but they did not appear to have in-depth knowledge about specific technical details. Generally, stakeholders’ answers showed larger variability by country than by stakeholder category. Conclusions Stakeholders across different European countries viewed the use of HTA evidence to be an important factor in their decision-making process. Further, they considered themselves to be capable of interpreting the results from a ROI tool and were highly motivated to use it. 16. Does the engagement of clinicians and organisations in research improve healthcare performance: a three-stage review

Abstract

Objective There is a widely held assumption that engagement by clinicians and healthcare organisations in research improves healthcare performance at various levels, but little direct empirical evidence has previously been collated. The objective of this study was to address the question: Does research engagement (by clinicians and organisations) improve healthcare performance? Methods An hourglass-shaped review was developed, consisting of three stages: (1) a planning and mapping stage; (2) a focused review concentrating on the core question of whether or not research engagement improves healthcare performance; and (3) a wider (but less systematic) review of papers identified during the two earlier stages, focusing on mechanisms. Results Of the 33 papers included in the focused review, 28 identified improvements in health services performance. Seven out of these papers reported some improvement in health outcomes, with others reporting improved processes of care. The wider review demonstrated that mechanisms such as collaborative and action research can encourage some progress along the pathway from research engagement towards improved healthcare performance. Organisations that have deliberately integrated the research function into organisational structures demonstrate how research engagement can, among other factors, contribute to improved healthcare performance. Conclusions Current evidence suggests that there is an association between the engagement of individuals and healthcare organisations in research and improvements in healthcare performance. The mechanisms through which research engagement might improve healthcare performance overlap and rarely act in isolation, and their effectiveness often depends on the context in which they operate.

Abstract

We analyzed HIV-1 genome sequences from 68 newly infected volunteers in the STEP HIV-1 vaccine trial. To determine whether the vaccine exerted selective T cell pressure on breakthrough viruses, we identified potential T cell epitopes in the founder sequences and compared them to epitopes in the vaccine. We found greater distances to the vaccine sequence for sequences from vaccine recipients than from placebo recipients. The most significant signature site distinguishing vaccine from placebo recipients was Gag amino acid 84, a site encompassed by several epitopes contained in the vaccine and restricted by human leukocyte antigen (HLA) alleles common in the study cohort. Moreover, the extended divergence was confined to the vaccine components of the virus (HIV-1 Gag, Pol and Nef) and not found in other HIV-1 proteins. These results represent what is to our knowledge the first evidence of selective pressure from vaccine-induced T cell responses on HIV-1 infection in humans.

Abstract

To develop a 'close to patient' peripatetic intravenous service (PIVS) for delivery of specialist osteoporosis care in a community setting without increasing cost and with a reduced carbon footprint.Cost and feasibility of a PIVS for intravenous (i.v.) bisphosphonate treatment were modelled using UK National Health Service costings and then tested in the field for 1 year. Average patient mileage to peripatetic sites was compared with mileage travelled if treated at the base hospital (current practice). The method of travel to hospital (current practice) or peripatetic sites (new study) was ascertained together with patients' preference for the new or the current system. Peripatetic sites were researched and those with suitable facilities selected. Data for fuel consumption were based on a usage of 1 litre per 14.5 km.The main outcome measure was cost comparison between hospital and peripatetic services. Others included patient satisfaction, miles saved, method of travel to the clinic and changes in CO(2) emissions.Cost per patient, including drugs, lies between pound557 and pound622 annually for 1000 and 500 patients, respectively which is cost-neutral compared with hospital attendance. PIVS was rated more convenient by 98% of patients. Hospital transport was significantly reduced and the total monthly saving of 2000 miles has reduced CO(2) emission by 6072 kg p.a. No medical emergency occurred in 410 infusions.PIVS is cost neutral compared with a conventional service, leads to a better patient experience, a significant cutback in hospital transport costs and a reduction of the NHS carbon footprint. However not all drugs may be suitable for this service: the area served was rural with large distances and poor public transport and mileage savings may not accrue in urban areas. Insurance was not included in the calculation of costs.

Abstract

Despite an increased number of economic evaluations of tobacco control interventions, the uptake by stakeholders continues to be limited. Understanding the underlying mechanism in adopting such economic decision-support tools by stakeholders is therefore important. By applying the I-Change Model, this study aims to identify which factors determine potential uptake of an economic decision-support tool, i.e., the Return on Investment tool.Stakeholders (decision-makers, purchasers of services/pharma products, professionals/service providers, evidence generators and advocates of health promotion) were interviewed in five countries, using an I-Change based questionnaire. MANOVA's were conducted to assess differences between intenders and non-intenders regarding beliefs. A multiple regression analysis was conducted to identify the main explanatory variables of intention to use an economic decision-support tool.Ninety-three stakeholders participated. Significant differences in beliefs were found between non-intenders and intenders: risk perception, attitude, social support, and self-efficacy towards using the tool. Regression showed that demographics, pre-motivational, and motivational factors explained 69% of the variation in intention.This study is the first to provide a theoretical framework to understand differences in beliefs between stakeholders who do or do not intend to use economic decision-support tools, and empirically corroborating the framework. This contributes to our understanding of the facilitators and barriers to the uptake of these studies.

Abstract

The purpose of this cross-sectional descriptive study was to explore factors that influence the timing of the first prenatal care visit of pregnant adolescents. Although researchers agree that barriers to prenatal care need to be identified and interventions formulated, barriers have not been well defined for any specific population. Increased availability and adequacy of health care resources, more family system support, and higher self-care agency scores are hypothesized to predict earlier prenatal care. A convenience sample of 51 pregnant adolescents from a university clinic was surveyed. The study found that the availability/adequacy of health care resources was the most significant predictor of the initiation of prenatal care (p = .03). Of the various health care resources assessed, only the ease in attaining a medical card was positively related to the onset of prenatal care. Practitioners can be instrumental in disseminating information about the availability of health care resources and in easing access to those resources through local and national political involvement.In order to identify the factors that influence the gestational age when pregnant adolescents seek prenatal care, data were gathered from a convenience sample of 51 pregnant adolescents who were younger than 18, nullipara, received a government-sponsored medical card, and could read at the sixth-grade level or higher. Availability and adequacy of resources and the family support system were assessed using a modified version of Slusher's conditioning factor data collection tool, and self-care agency was measured using the Denyes Self-Care Agency Instrument. Sociodemographic data were also collected, and gestational age at initial prenatal screening was based on calculations of the estimated date of delivery. In the analysis of structural and nonstructural barriers to initiation of prenatal care, the only significance found was associated with the structural barriers of availability and adequacy of health care resources (the ease of attaining a medical card). The perceived difficulty of dealing with the medical-card system, rather than actual unavailability of funding, created the barrier to seeking care. There was no support for the hypotheses that increased family support or higher self-care agency would be associated with earlier onset of care. An interesting, but not statistically significant, association showed that the subsample with the lowest reported family support and health care resources (the Black adolescents) had the highest self-care scores and earliest prenatal care initiation. It is concluded that additional research is needed on the actual and perceived barriers to prenatal care faced by adolescents and that health care providers can play a role in improving the current Medicaid system.

INTRODUCTION AND OBJECTIVES: The VCUG is an invasive procedure that is uncomfortable and distressing for the pediatric patient. We aim to assess parent preparation prior to VCUG, identify any correlation between being informed, or having had a previous VCUG experience, and parent/patient distress level, and to investigate perceived changes in children’s behavior after the VCUG. METHODS: We conducted an IRB-approved prospective survey study. Three separate surveys were administered to parents of 45 children: the “pre-VCUG survey” was administered before the VCUG, the “post-VCUG survey” was administered within 30 minutes after the study, and the “2-week post-VCUG survey” was a telephone survey administered 2-4 weeks after the VCUG. The first two surveys were investigator-generated; the “2-week post-VCUG survey” was a modified version of a validated survey, Posthospital Behavioral Questionnaire. Survey items were both 5-point scaled and open-ended questions. Demographics, previous experience with VCUG, and having been educated about VCUG were assessed. Statistical analyses were done using STATA 7.0. RESULTS: Our populations consisted of 32 female and 13 male patients (mean age: 2.8 years) Thirty-two parents/guardians identified themselves as white and 13 as non-white. Nineteen parents/ guardians interviewed had children who had undergone a previous VCUG. Twelve parents (27%) reported having had no explanation about the VCUG by a health-care provider. Fifteen percent of non-white interviewees had received an explanation compared to 84.9% of white interviewees. Parents with previous VCUGs experience and those who had been educated anticipated more pain (3.58 vs. 1.88, p 0.001; 0.77 vs. 3.31, p value 0.001 respectively). However, informed parents accurately predicted the perceived pain level their child. Of the 35 parents who completed the 2-week post-VCUG survey, 11 (31%) noticed changes in their child’s behavior. Eight of these eleven parents were worried about those changes. CONCLUSIONS: We conclude that a significant number of parents are uninformed about the VCUG, which influences their expectations for the procedure. A target population to consider improving access to information is the non-white population. Although education prior to the VCUG or a previous VCUG experience can increase parent anticipated pain for their child, which may contribute to increased stress for parent and child prior to the VCUG, it is important to inform parents about the procedure so they have reasonable expectations.

Abstract

The image of nurse-midwifery forms a barrier to utilization that is just as, if not more, formidable as laws restricting practice. In this paper, the issue of image for its impact on the practice of nurse-midwifery is analyzed. Historical, technological, consumer-oriented, and nursing-oriented factors that have affected midwifery's image and the profession's current status are explored. Recommendations to clarify the role and image, including political involvement, networking strategies, and research, are discussed. Although nurse-midwives, who emphasize cost-effective, quality, primary care, should be in the forefront of health care delivery, they are still widely misperceived, underutilized, and unrecognized as mainstream health care providers. In this era of health care reform, it is essential that the role and image of the nurse-midwife be clearly defined to the general public, maternity care consumer, fellow health care providers, and legislators.

Abstract

Background Management of osteoporosis is imperfect because patients may not start, persist or comply with treatment. This study was aimed to identify baseline variables associated with women failing to start, persist or comply with bisphosphonate treatment. Methods 254 women >50 years old were selected 5 years after having a bone densitometry (bone mineral density (BMD)) diagnosis of osteoporosis. At the outset, information about osteoporosis was sent to the general practitioner (GP). Women were not under pressure at the outset to start or comply, and they and their GP were unaware that follow-up studies would take place. Patient survival was identified from the National Health Service Strategic Tracing Service, prescription data from GP records and baseline data from the initial questionnaire. Persistence was defined as at least one prescription issued per year and compliance as having a medicine possession ratio of ≥80% for each of 5 years. Results 38% failed to start treatment. Failure was associated with higher BMD Z score and residence in a nursing/residential home. Half of those starting and alive at 5 years persisted with treatment, whereas only 23% achieved a medicine possession ratio of ≥80%. Persistence was associated with a lower comorbidity index and compliance with a lower BMD Z score and a fall before starting treatment. Conclusions Treatment was low, especially in nursing/residential homes where known low treatment prevalence appears to be associated with non-initiation. The degree of depression of BMD (not just low BMD) was associated with better initiation and compliance. The association of falls with compliance suggests that fall clinics may be able to play a part in improving osteoporosis management.

This paper is intended for Help Desk managers who want to learn how to expand their Help Desk without outpacing themselves and also for institutions that may be just starting out with centralized support or are thinking of opening a help desk.We will explain in detail how, from Winter of 2003 to Summer of 2005, Webster University went from having no Help Desk to having one that services over 40,000 people, provides international support and is primarily staffed by students.Our two major focuses will be how we have succeeded with a staff consisting of 90% student workers and also on how we were able to resolve territory conflicts that go along with support centralization.The evolution of centralized service, its pitfalls and successes, will be detailed using examples of our relationships with other support departments within the University system, including how we addressed territoriality, knowledge hoarding and being a new department in an already established IT infrastructure.We will look at how student workers give Webster University Help Desk its unique blend of personalized service with competent technical support; also, the challenges we faced, both internally and externally with students in regards to staffing, consistency of support, training and responsibility management.In conclusion, we will look at where the Webster University Help Desk is moving in the future: remote support via LANDesk, 24x7 support and resident student computing support.

Abstract

Background: The usefulness of urinary markers of bone turnover in monitoring therapy depends on their within-person variability compared with their responses to therapy. The aim of this study was to assess the performance of two such markers on this basis. Methods: We measured variation, during a whole year, of cross-linked N-terminal telopeptide of collagen I (NTx) and urinary deoxypyridinoline (DPD) as ratios to creatinine concentration and after log-transformation of the ratios in untreated women stratified into three bone density classes, of which the lowest was osteoporotic. We also measured changes in bone mineral density at the lumbar spine (LSBMD) and hip (FNBMD) in untreated women with normal bones and in those with moderate osteopenia and calculated the reference change value (RCV; or least significant change) at P Results: After 12 months on bisphosphonates, LSBMD changed more than RCV (2.55%) in 47% of women compared with 44% of those on HRT and 13% of those on calcium. Response of FNBMD was less. Log NTx (RCV= −28%) responded to bisphosphonates in 78%, regardless of BMD, but less often to HRT (67%). Log DPD (RCV= −30%) responded to bisphosphonates less frequently (31% at 12 months). Conclusions: NTx has advantages over DPD in monitoring therapy for osteoporosis when mailed urine samples are used.

Abstract

AbstractObjective:To develop a ‘close to patient’ peripatetic intravenous service (PIVS) for delivery of specialist osteoporosis care in a community setting without increasing cost and with a reduced carbon footprint.Research design and methods:Cost and feasibility of a PIVS for intravenous (i.v.) bisphosphonate treatment were modelled using UK National Health Service costings and then tested in the field for 1 year. Average patient mileage to peripatetic sites was compared with mileage travelled if treated at the base hospital (current practice). The method of travel to hospital (current practice) or peripatetic sites (new study) was ascertained together with patients’ preference for the new or the current system. Peripatetic sites were researched and those with suitable facilities selected. Data for fuel consumption were based on a usage of 1 litre per 14.5 km.Main outcome measures:The main outcome measure was cost comparison between hospital and peripatetic services. Others included patient satisfaction, miles saved, method of travel to the clinic and changes in CO2emissions.Results:Cost per patient, including drugs, lies between £557 and £622 annually for 1000 and 500 patients, respectively which is cost-neutral compared with hospital attendance. PIVS was rated more convenient by 98 of patients. Hospital transport was significantly reduced and the total monthly saving of 2000 miles has reduced CO2emission by 6072 kg p.a. No medical emergency occurred in 410 infusions.Conclusions:PIVS is cost neutral compared with a conventional service, leads to a better patient experience, a significant cutback in hospital transport costs and a reduction of the NHS carbon footprint. However not all drugs may be suitable for this service: the area served was rural with large distances and poor public transport and mileage savings may not accrue in urban areas. Exposure of Phleum pratense L. (timothy grass) to SO2 was found to reduce the dry weight of the roots before any changes in the shoot could be detected. These effects were evident after treatment with SO2 concentrations of 0·11 ppm for 3 weeks and 0·06 ppm for 6 weeks, or with doses of 0·40 and 0·80 ppm for infrequent short periods which simulated pollution episodes in the field. When 14CO2 was supplied to the first fully mature leaf of the main tiller, the transport of labelled assimilates to the root was reduced in SO2-treated plants, and the proportions of 14C going to some developing regions of the shoot were increased. While these observations support the view that translocation is affected by SO2 pollution, they are not readily explained by a direct inhibition of steps in assimilate transport such as phloem loading, because the redistribution of assimilates within the plant requires a continuation of the basic processes of translocation.

Abstract

The sensitivity of timothy grass Phleum pratense L. to SO 2 was affected by two factors which influenced the rate of growth; namely, light and temperature. When either lower temperatures or smaller photon flux densities were employed to reduce the rate of growth, there appeared to be similar increases in sensitivity to SO 2 . It is therefore postulated that growth rate per se may be a determining factor in the sensitivity of this species to SO 2 . The dry weight of the roots was consistently reduced by SO 2 throughout the range of growth conditions employed here; there was thus no evidence that environmental conditions altered the sensitivity of the roots to SO 2 stress. Effects of SO 2 on shoot:root ratio therefore depend on environmental conditions. Growth analysis techniques were employed to define the responses of P. pratense to SO 2 more precisely. The effect on the roots preceded that on the shoots and it appeared that the dry weight of the shoots was maintained at the expense of the roots. The net assimilation rate was reduced by an average of 14·7% by 0·12 ppm SO 2 , and this effect was fairly consistent during growth. Some of the growth responses detected were interpreted as compensatory in nature, in response to the fall in net assimilation rate.
