Abstract

The Met Office in the United Kingdom has developed a completely new probabilistic postprocessing system called IMPROVER to operate on outputs from its operational numerical weather prediction (NWP) forecasts and precipitation nowcasts. The aim is to improve weather forecast information to the public and other stakeholders while better exploiting the current and future generations of underpinning kilometer-scale NWP ensembles. We wish to provide seamless forecasts from nowcasting to medium range, provide consistency between gridded and site-specific forecasts, and be able to verify every stage of the processing. The software is written in a modern modular framework that is easy to maintain, develop, and share. IMPROVER allows forecast information to be provided with greater spatial and temporal detail and a faster update frequency than previous postprocessing. Independent probabilistic processing chains are constructed for each meteorological variable consisting of a series of processing stages that operate on predefined grids and blend outputs from several NWP inputs to give a frequently updated, probabilistic forecast solution. Probabilistic information is produced as standard, with the option of extracting a most likely or yes–no outcome if required. Verification can be performed at all stages, although it is only currently switched on for the most significant stages when run in real time. IMPROVER has been producing real-time output since March 2021 and became operational in spring 2022.

Abstract

The aurora is a readily visible phenomenon of interest to many members of the public. However, the aurora and associated phenomena can also significantly impact communications, ground-based infrastructure, and high-altitude radiation exposure. Forecasting the location of the auroral oval is therefore a key component of space weather forecast operations. A version of the OVATION-Prime 2013 auroral precipitation model (Newell et al., 2014, https://doi.org/10.1002/2014sw001056) was used by the UK Met Office Space Weather Operations Centre (MOSWOC). The operational implementation of the OVATION-Prime 2013 model at the UK Met Office delivered a 30-min forecast of the location of the auroral oval and the probability of observing the aurora. Using weather forecast evaluation techniques, we evaluate the ability of the OVATION-Prime 2013 model forecasts to predict the location and probability of the aurora occurring by comparing the forecasts with auroral boundaries determined from data from the IMAGE satellite between 2000 and 2002. Our analysis shows that the operational model performs well at predicting the location of the auroral oval, with a relative operating characteristic (ROC) score of 0.82. The model performance is reduced in the dayside local time sectors (ROC score = 0.59) and during periods of higher geomagnetic activity (ROC score of 0.55 for Kp = 8). As a probabilistic forecast, OVATION-Prime 2013 tends to underpredict the occurrence of aurora by a factor of 1.1–6, while probabilities of over 90% are overpredicted.

4. Assessing model performance by weather regime transitions

Abstract

Having the ability to stratify a model’s performance by weather type is not only beneficial to a weather forecaster when making decisions, but it is also important for end users, whether they be scientists looking to improve the model, or a customer wishing to know the value of a forecast under a specific set of circumstances.At the MET Office, Decider is a tool which assigns a dominant weather type to a set of ensemble members, to predict the probability of a weather type occurring. The weather type is chosen from either a set of 30 or 8 sub-types, where a weather type is pre-determined objectively by clustering a 154 year record of sea level pressure anomaly fields. There is also a record of daily weather type classifications derived from analysis fields and so information of model performance for these weather types could be invaluable in reducing model error if combined with the predictions from Decider.Early trials of assessing model performance by weather type revealed that larger errors occur when the weather type persisted for a single day, rather than longer timescales, and so this suggests that it would be beneficial to examine weather type transition periods.To examine this, we expand the weather type methodology to include multiple time periods. The current methodology uses 12Z analyses to identify the weather type, and so we first assess model performance as a sensitivity study to the analysis time.Transition days are identified when the weather type changes during a pre-defined validation period, which allows separation into either night/day weather type transitions, or a change in weather type over a full 24-hour period.We will present early results of this work and demonstrate the impact of model performance when stratifying by regime transitions.

Abstract

Objective Outdoor free play is important for preschoolers' physical activity, health, and development. Certain temperamental characteristics are associated with obesity, nutrition, and sedentary behaviors in preschoolers, but the relationship between temperament and outdoor play has not been examined. This study examined whether there is an association between temperament and outdoor play in young children. Methods Healthy children aged 1 to 5 years recruited to The Applied Research Group for Kids (TARGet Kids!), a community-based primary care research network, from July 2008 to September 2013 were included. Parent-reported child temperament was assessed using the Childhood Behavior Questionnaire. Outdoor free play and other potential confounding variables were assessed through validated questionnaires. Multivariable linear regression was used to determine the association between temperament and outdoor play, adjusted for potential confounders. Results There were 3393 children with data on outdoor play. The association between negative affectivity and outdoor play was moderated by sex; in boys, for every 1-point increase in negative affectivity score, mean outdoor play decreased by 4.7 minutes per day. There was no significant association in girls. Surgency was associated with outdoor play; for every 1-point increase in surgency/extraversion, outdoor play increased by 4.6 minutes per day. Conclusions Young children's temperamental characteristics were associated with their participation in outdoor free play. Consideration of temperament could enhance interventions and strategies to increase outdoor play in young children. Longitudinal studies are needed to elucidate the relationship between children's early temperament and physical activity.

Abstract

Background Huntington's disease is caused by a CAG repeat expansion in the huntingtin gene, HTT. Age at onset has been used as a quantitative phenotype in genetic analysis looking for Huntington's disease modifiers, but is hard to define and not always available. Therefore, we aimed to generate a novel measure of disease progression and to identify genetic markers associated with this progression measure. Methods We generated a progression score on the basis of principal component analysis of prospectively acquired longitudinal changes in motor, cognitive, and imaging measures in the 218 indivduals in the TRACK-HD cohort of Huntington's disease gene mutation carriers (data collected 2008–11). We generated a parallel progression score using data from 1773 previously genotyped participants from the European Huntington's Disease Network REGISTRY study of Huntington's disease mutation carriers (data collected 2003–13). We did a genome-wide association analyses in terms of progression for 216 TRACK-HD participants and 1773 REGISTRY participants, then a meta-analysis of these results was undertaken. Findings Longitudinal motor, cognitive, and imaging scores were correlated with each other in TRACK-HD participants, justifying use of a single, cross-domain measure of disease progression in both studies. The TRACK-HD and REGISTRY progression measures were correlated with each other (r=0·674), and with age at onset (TRACK-HD, r=0·315; REGISTRY, r=0·234). The meta-analysis of progression in TRACK-HD and REGISTRY gave a genome-wide significant signal (p=1·12 × 10−10) on chromosome 5 spanning three genes: MSH3, DHFR, and MTRNR2L2. The genes in this locus were associated with progression in TRACK-HD (MSH3 p=2·94 × 10−8 DHFR p=8·37 × 10−7 MTRNR2L2 p=2·15 × 10−9) and to a lesser extent in REGISTRY (MSH3 p=9·36 × 10−4 DHFR p=8·45 × 10−4 MTRNR2L2 p=1·20 × 10−3). The lead single nucleotide polymorphism (SNP) in TRACK-HD (rs557874766) was genome-wide significant in the meta-analysis (p=1·58 × 10−8), and encodes an aminoacid change (Pro67Ala) in MSH3. In TRACK-HD, each copy of the minor allele at this SNP was associated with a 0·4 units per year (95% CI 0·16–0·66) reduction in the rate of change of the Unified Huntington's Disease Rating Scale (UHDRS) Total Motor Score, and a reduction of 0·12 units per year (95% CI 0·06–0·18) in the rate of change of UHDRS Total Functional Capacity score. These associations remained significant after adjusting for age of onset. Interpretation The multidomain progression measure in TRACK-HD was associated with a functional variant that was genome-wide significant in our meta-analysis. The association in only 216 participants implies that the progression measure is a sensitive reflection of disease burden, that the effect size at this locus is large, or both. Knockout of Msh3 reduces somatic expansion in Huntington's disease mouse models, suggesting this mechanism as an area for future therapeutic investigation. Abstract

To examine the association between body mass index (BMI) and iron deficiency in early childhood, while considering the influence of low-grade systemic inflammation.Healthy children ages 1-3 years were included in a cross-sectional analysis. Age- and sex-standardized World Health Organization BMI z score (zBMI) was calculated using height/length and weight measurements; iron status was assessed by serum ferritin; inflammation was assessed by C-reactive protein (CRP). Children with CRP ≥10 mg/L were excluded because this may indicate acute systemic inflammation. Adjusted multivariable regression analyses were used to investigate the association between zBMI and both serum ferritin (µg/L), and iron deficiency (serum ferritin12 µg/L). We performed prespecified subgroup analyses according to CRP level (normal [≤1.0 mg/L] and low-grade inflammation [1.0 mg/L to10.0 mg/L]).Of 1607 children included, 20% were categorized as with zBMI1, 13% had iron deficiency, and 18% had low-grade inflammation. Higher zBMI was associated with lower serum ferritin (-1.51 µg/L, 95% CI -2.23, -0.76, P .0001) and increased odds of iron deficiency (OR 1.28, 95% CI 1.10, 1.50, P = .002). Though there was no interaction between zBMI and CRP for the adjusted linear regression model (P = .79) or logistic regression model (P = .43), children with low-grade inflammation had a higher serum ferritin (P .0001).Higher zBMI is associated with increased risk for iron deficiency in children between 1 and 3 years, and should be considered as a risk factor in targeted screening. Abstract

AIM To explore the nature, process and consequences of giving information to children aged three to 11 years before and during venepuncture. METHOD This was a non-participant, observational, qualitative study of 11 children and four healthcare professionals before and during venepuncture. The healthcare professionals were: a sister, a healthcare assistant, a junior doctor and a staff nurse. FINDINGS There was little assessment of children's levels of knowledge of venepuncture and they received no information before the procedure. During the procedure, the children under six years of age only received information about the local anaesthetic cream during the application of the cream, not the venepuncture; the older children received information about the application of the cream and the venepuncture during both procedures. Practitioners used different language with the younger children when applying the local anaesthetic cream. Although the younger children did not appear distressed by the application of the cream, they appeared to be distressed by the venepuncture--more so than the older children--protesting and crying during the procedure. CONCLUSION Some of the distress associated with venepuncture can be avoided if children are given information about the procedure in a way they can understand.

Abstract

There are currently around 32,300 registered sex offenders in the United Kingdom for whom recent years have seen the implementation of extensive supervision strategies, such as Multi-Agency Public Protection Arrangements (MAPPA) and licence conditions. Concurrently, the last two decades have also seen heightened alertness to the use of communications technologies in the commission of child sexual abuse. This paper describes a 6-month pilot project investigating the management of computer use for registered sex offenders using e-Safety software, which examines PCs for specific inappropriate words and phrases, the results of which are monitored remotely by risk-management officers. The findings of the pilot project are discussed in the context of both the potential benefits for the detection of future inappropriate computer-related behaviour and also the potential benefits for promoting and identifying positive computer use by offenders. Finally, we discuss further development of the use of this software in sex offender management.

Abstract

Objective To evaluate the association between breastfeeding duration and early childhood cardiometabolic risk. Study design A cross-sectional study of 1539 healthy children, 3-6 years of age, recruited through The Applied Research Group for Kids! practice-based research network between October 2009 and August 2015. Adjusted multivariable linear regression was used to examine the association between breastfeeding duration and cardiometabolic risk z score and individual cardiometabolic risk factors of waist circumference, systolic blood pressure, glucose, high-density lipoprotein cholesterol, and triglycerides. Results The mean breastfeeding duration was 12.5 months (SD = 8.4). Breastfeeding duration was associated with lower cardiometabolic risk z score (beta = −0.03; 95% CI −0.05, −0.01). In analysis of cardiometabolic risk factors, each additional 3 months of breastfeeding was associated with a 0.13 cm (95% CI −0.20, −0.05) lower waist circumference and 0.16 mm Hg (95% CI −0.30, −0.02) lower systolic blood pressure. Compared with children who breastfed for 6-12 months, those who breastfed for 12-24 months had a lower systolic blood pressure of 1.07 mm Hg (95% CI −2.04, −0.10). There was no association between breastfeeding duration and cardiometabolic risk for those who breastfed beyond 24 months. Conclusions Breastfeeding duration is associated with lower cardiometabolic risk, although the magnitude of association is small. Causation cannot be inferred. Breastfeeding beyond 24 months may not have an added benefit for cardiometabolic health.

To compare the electrophysiological activity in submandibular hyolaryngeal muscles during performance of 2 exercises that incorporate resistance against muscular contraction.Within-subject repeated-measures design.Academic research laboratory.Healthy, young adult women (N=26; mean age, 24.1y) without a history of dysphagia, cervical spine conditions, neurologic disease, or head/neck cancer.Participants performed 2 isometric exercises requiring contraction against resistance to the submandibular hyolaryngeal muscles: one requiring jaw opening against a semirigid brace (chin-to-chest [CtC] exercise) and one requiring a chin tuck against an air-inflated rubber ball (chin tuck against resistance [CTAR] exercise). Measures of electrophysiology using surface electromyography (sEMG) were obtained during exercise performance.Microvolts as measured from sEMG electrode sensors placed on the skin surface above the hyolaryngeal muscles (surface of skin above geniohyoid, mylohyoid, and anterior digastric). Dependent variables included peak contraction amplitude (in μV) and mean contraction amplitude (in μV) across 10 seconds of sustained contraction.Significant effects of exercise on peak and mean contraction amplitudes were present when both exercises were compared with baseline sEMG activity. (P.001 for both). Normalized values of peak contraction amplitude and mean contraction amplitude during performance of CtC were not significantly different compared with CTAR.This study provides supporting evidence for the influence of 2 published exercises on motor unit recruitment in the submandibular hyolaryngeal muscles, both of which have been previously proposed as rehabilitative modalities. Theoretical and clinical implications are discussed.

Abstract

This anonymous survey sent to every battalion and brigade commander at Fort Hood, Texas assessed the new concept of residency-trained operational physicians that had completed military unique curriculum series training. Commanders provided anonymous professional information about themselves and their assigned physicians and evaluated their physicians in 22 areas of medical and military competence using a linear scoring method from 2 (indicating poor) to 6 (indicating outstanding). Main effects and interaction effects were analyzed using multiple analysis of variance. The commanders' overall confidence in their physicians was 5.4, suggesting general satisfaction. Prior-service physicians were rated higher on several factors if the commander had combat experience. Non-prior-service physicians were rated the same on nearly all variables regardless of the commander's experience. Commanders with combat experience rated their assigned physicians significantly higher than did commanders without combat experience. Despite high ratings, several candid remarks suggest there is still room for improvement.

16. Suspected dietary supplement injuries in special operations soldiers

Abstract

Evidence suggests that a number of Special Operations Soldiers are using dietary supplements to augment their physical training programs and that some of these supplements are not entirely benign. This article presents a series of case reports of Soldiers who suffered adverse effects that may be at least partially attributable to the use of dietary supplements. Given that many Special Operations Soldiers train at the same level as world class athletes and the use of supplements is common among world class athletes, the use of supplements is not likely to stop. To this end, the purpose of this article is to provide awareness of the problem, discuss some of the harmful effects of dietary supplements, make a recommendation for education to help reduce the number of injuries resulting from the use of dietary supplements, and recommend that scientific studies be done to prove the benefits and risks of taking dietary supplements.

Abstract

Retrospective analysis of patient records from two 1st Battalion, 7th Special Forces Group combat rotations in Operation Enduring Freedom reveals a high volume of medical activity over a wide range of medical issues managed by Special Forces Medical Sergeants (MOS 18Ds). The initial training curriculum for 18Ds has been modified to provide graduating 18Ds with a refresher course and updated credentialing before reporting to their first unit. However, due to the high operational tempo, subsequent biannual refresher training has proven difficult for at least one Special Forces unit. Units must plan ahead between deployments to balance medic recredentialing with unit pre-mission training and individual non-medical training.
