Aim: According to delivery room guidelines, an optimal position is not specified for the stabilization of the baby. This study aimed to define the positions effects on postnatal adaptation parameters and cerebral oxygenation in non-resuscitated neonates. Materials and Methods: A total of 60 neonates delivered by cesarean section stabilized randomly in the supine, right-side, left-side, or prone positions were enrolled. Apgar scores, heart rates (HR), arterial oxygen saturations via pulse oximetry, and perfusion indexes (PI) at the 2
nd , 5th and 10th minutes were recorded. Cerebral regional oxygen saturation (SpO2 ) of the patients was monitored by near-infrared spectroscopy. Results: In the prone position, the 1st minute Apgar score was significantly lower than other groups, but no difference was observed at the 5th minute Apgar scores (1st min Apgarprone, p=0.05). Although there was no statistically significant difference, the prone position had the lowest HR at the 2nd minute, while the supine posture had the greatest HR at the 5th and 10th minutes. While the groups' SpO2 values were similar, the left-side group's perfusion rates increased at the 5th and 10th minute marks (5th and 10th min PIleft-side, p=0.67, p=0.21, respectively). Regional cerebral oxygen saturation (rScO2 ) and cerebral fractional oxygen extraction did not differ significantly between groups at the 5th and 10th minute time intervals. Although right and left rScO2 were found to be higher in the first 5 minutes in the prone position, this elevation did not lead to a statistically significant difference, and right and left rScO2 values were found to be similar in all groups at the 10th minute. Conclusion: Adaptation parameters were not affected by position, except for lower 1st minute Apgar scores in the prone group and higher perfusion indices in the left lateral position. Cerebral perfusion was similar in all groups. The left-side position, which results in a higher PI, may be a good alternative. Studies with larger case series may provide further information. - Abstract
Background: healthcare-acquired infections (HAIs) in the neonatal period cause substantial morbidity, mortality, and healthcare costs. Our purpose was to determine the prevalence of HAIs, antimicrobial susceptibility of causative agents, and the adaptivity of the Centres for Disease Control and Prevention (CDC) criteria in neonatal HAI diagnosis. Methods: a HAI point prevalence survey was conducted in the neonatal intensive care units (NICUs) of 31 hospitals from different geographic regions in Turkey. Results: the Point HAI prevalence was 7.6%. Ventilator-associated pneumonia (VAP) and central line-associated bloodstream infections (CLABSI) and late onset sepsis were predominant. The point prevalence of VAP was 2.1%, and the point prevalence of CLABSI was 1.2% in our study. The most common causative agents in HAIs were Gram-negative rods (43.0%), and the most common agent was Klebsiella spp (24.6%); 81.2% of these species were extended spectrum beta-lactamase (ESBL) (+). Blood culture positivity was seen in 33.3% of samples taken from the umbilical venous catheter, whereas 0.9% of samples of peripherally inserted central catheters (PICCs) were positive. In our study, 60% of patients who had culture positivity in endotracheal aspirate or who had purulent endotracheal secretions did not have any daily FiO2 change (p = 0.67) and also 80% did not have any increase in positive end-expiratory pressure (PEEP) (p = 0.7). On the other hand, 18.1% of patients who had clinical deterioration compatible with VAP did not have endotracheal culture positivity (p = 0.005). Conclusions: neonatal HAIs are frequent adverse events in district and regional hospitals. This at-risk population should be prioritized for HAI surveillance and prevention programs through improved infection prevention practices, and hand hygiene compliance should be conducted. Amaç: Basınç yaraları, yoğun bakım ünitelerinde sık görülen, hastane yatış süresini uzatan, mortaliteyi arttıran ve tedavi giderlerini yükselten önemli bir bakım sorunudur. Yenidoğan dönemine göre geliştirilmiş olan Neonatal Braden Q Basınç Risk Değerlendirme Ölçeği’nin (NBQBRD) ise ülkemizde henüz güvenilirlik ve geçerlilik çalışması bulunmamaktadır. Konu hakkında, yenidoğan dönemine ait ülkemizde tek geçerlilik ve güvenirlilik çalışması yapılmış ve halen kullanılmakta olan Yenidoğan Cilt Risk Değerlendirme (YCRD) Ölçeğidir. Çalışmamızın amacı Türkiye’de ilk defa yenidoğan yoğun bakım ünitesinde yatarak tedavi gören bebeklerde Türkçe NBQBRD Ölçeği’nin ülkemiz için geçerlilik ve güvenirliğini değerlendirmektir. Yöntem: Yenidoğan yoğun bakım ünitesinde yatarak tedavi gören 114 olguda toplam 201 değerlendirme yapılmıştır. Verilerin toplanmasında; hasta özelliklerine ve çalışmayı sürdüren hemşirelerin bireysel özelliklerine ilişkin soru formu ve Türkçe NBQBRD Ölçeği ile birlikte paralel form testi olarak Türkçe YCRD Ölçeği kullanılmıştır. Aynı vaka eş-zamanlı olarak bakım veren hemşiresi ve araştırmacı tarafından değerlendirilmiştir. Bulgular: Çalışmada Türkçe NBQBRD Ölçeği’nin geçerli ve güvenilir olduğu, Türkçe YCRD Ölçeği ile karşılaştırıldığında klinik hemşireleri ve araştırmacı ölçümlerinin iki ölçek arasındaki korelasyon katsayısının yüksek ve aralarındaki ilişkinin ileri düzeyde anlamlı olduğu görülmüştür. Sonuç: Bu sonuçlar yenidoğan yoğun bakım ünitesinde yatmakta olan bebeklerde basınç ülseri risk değerlendirmesinde Türkçe NBQBRD Ölçeği’nin ülkemizde güvenle kullanılabileceğini göstermiştir., Objective: Pressure sore is a common care problem in intensive care units which prolongs hospital stay, increases mortality and treatment costs. Validity and reliability study of neonatal Braden Q scale, which is developed for the neonatal period, has yet not performed in our country. the only validity and reliability study for the neonatal period in Turkey was performed for the Newborn Skin Risk Assessment Scale which is currently in use. the aim of this study was to assess the validity and reliability of Turkish Neonatal Braden Q Scale in infants hospitalized firstly in the neonatal intensive care unit. Method: A total of 201 assessments were made for 114 patients who were hospitalized in the Neonatal Intensive Care Unit of Ege University Faculty of Medicine, Department of Pediatrics. in the collection of data; a questionnaire form concerning characteristics of patients and individual features of study nurses; and Neonatal Braden Q Scale together with Turkish version of Newborn Skin Risk Assessment Scale were used. the same case was evaluated simultaneously by the patient’s nurse and the investigator. Results: in this study, it was found that the Turkish verison of Neonatal Braden Q Scale is reliable and valid. the correlation coefficient was high between the two scales when clinical nurse’s and responsible investigator’s measurements was compared. Conclusion: in conclusion, Turkish version of Neonatal Braden Q Pressure Ulcer Risk Assessment Scale is reliable for the evaluation of pressure ulcer risk in infants hospitalized in the neonatal intensive care units in our country.

Background To evaluate the prevalence, risk factors and treatment of retinopathy of prematurity (ROP) in Turkey and to establish screening criteria for this condition., Methods A prospective cohort study (TR-ROP) was performed between 1 April 2016 and 30 April 2017 in 69 neonatal intensive care units (NICUs). Infants with a birth weight (BW)=1500 g or gestational age (GA) 1500 g or GA> 32 weeks with an unstable clinical course were included in the study. Predictors for the development of ROP were determined by logistic regression analyses., Results The TR-ROP study included 6115 infants: 4964 (81%) with a GA 32 weeks. Overall, 27% had any stage of ROP and 6.7% had severe ROP. A lower BW, smaller GA, total days on oxygen, late-onset sepsis, frequency of red blood cell transfusions and relative weight gain were identified as independent risk factors for severe ROP in infants with a BW=1500 g. Sağlıklı term bebeklerde olduğu gibi preterm ve hasta term bebeklerde de en idealbesin anne sütüdür. Mümkün olan en kısa sürede anne sütü verilmesi amaçlanmalıdır.Yenidoğan yoğun bakım ünitelerinde beslenme desteğinin uygun şekilde sağlanabilmesihem kısa hem de uzun dönemde morbidite ve mortalite üzerine önemlietkilere sahiptir. Birçok çalışmada erken enteral veya oral beslenmenin kısa ve uzundönem gelişmedeki önemi vurgulanmıştır. Ancak tüm çabalara karşın intrauterin dönemdekibüyümeye erişecek büyüme hızını yakalamak her zaman mümkün olmamaktadır.Yenidoğan bebekler için en uygun beslenme şekli kendi annesi tarafındanemzirilmesi iken bunu sağılmış anne sütünün yapay bir yöntemle verilmesi (kaşık,kap veya biberon), donör anne sütü ve formula kullanımı izlemektedir. Preterm bebeklerdebazen anne sütü yetersiz kalmakta mineralizasyon ve yeterli büyüme içingüçlendirme yapılması gerekmektedir.

EgeUn###, Cytomegalovirus (CMV), is the most common cause among congenital infections and is the most seen etiology in long-term sensorineural hearing loss (SNHL) and neurological impairment. Congenital CMV infection (CCMV) was reported in 0.15-2.2% of live-borne neonates in studies from different countries. A significant proportion of infected infants are asymptomatic after birth and might only be detected by routine screening methods during the new born period. The aim of this study was to screen the saliva of live-born neonates with areal-time PCR based method for the detection of CCMV in our hospital. Saliva samples collected in half an hour after birth by dry dacron swabs and were evaluated for CMV DNA (Rt-PCR, Abbott Molecular USA) from 1000 babies born in Ege University Faculty of Medicine Hospital Obstetrics Clinic between October 2015-October 2017. For the confirmation of CCMV, saliva positive newborns were evaluated with the same method for CMV DNA from their urine or blood within 21 days. All newborns were screened for sensorineural hearing tests. Subjects were 497 girls (49.7%) and 503 boys (50.3%), with a mean weight of 3116.8 g and mean of 37.61 birth week. CMV DNA was positive in the saliva of 16 newborns (1.6%). Fourteen newborns were weakly positive for CMV DNA in their saliva and were not confirmed for CCMV infection. Congenital CMV was confirmed in only two (0.2%) with the CMV DNA results in urine and/or blood samples. One of the two newborns with CCMV was symptomatic and had a neurosensorial hearing loss. The other one was asymptomatic. Saliva samples, taken immediately after birth with a noninvasive and easy method for the detection of CMV DNA is very important for diagnosis of CCMV. Positive samples should be confirmed with CMV DNA in urine or blood samples of these newborns. In this study, detection of positivity in saliva samples that were confirmed with other samples of our newborn population for CCMV was 0.2%. The specific diagnosis for CCMV in newborns with a noninvasive and easy collecting sample is important to avoid sequelae and for public health concerns.

- Abstract
Objective: to investigate the early neonatal outcomes of very-low-birth-weight (VLBW) infants discharged home from neonatal intensive care units (NICUs) in Turkey. Material and methods: a prospective cohort study was performed between April 1, 2016 and April 30, 2017. The study included VLBW infants admitted to level III NICUs. Perinatal and neonatal data of all infants born with a birth weight of.1500 g were collected for infants who survived. Results: data from 69 NICUs were obtained. The mean birth weight and gestational age were 1137 +/- 245 g and 29 +/- 2.4 weeks, respectively. During the study period, 78% of VLBW infants survived to discharge and 48% of survived infants had no major neonatal morbidity. VLBW infants who survived were evaluated in terms of major morbidities: bronchopulmonary dysplasia was detected in 23.7% of infants, necrotizing enterocolitis in 9.1%, blood culture proven late-onset sepsis (LOS) in 21.1%, blood culture negative LOS in 21.3%, severe intraventricular hemorrhage in 5.4% and severe retinopathy of prematurity in 11.1%. Hemodynamically significant patent ductus arteriosus was diagnosed in 24.8% of infants. Antenatal steroids were administered to 42.9% of mothers. Conclusion: the present investigation is the first multicenter study to include epidemiological information on VLBW infants in Turkey. Morbidity rate in VLBW infants is a serious concern and higher than those in developed countries. Implementation of oxygen therapy with appropriate monitoring, better antenatal and neonatal care and control of sepsis may reduce the prevalence of neonatal morbidities. Therefore, monitoring standards of neonatal care and implementing quality improvement projects across the country are essential for improving neonatal outcomes in Turkish NICUs., Turkish Neonatal Society

Objective Endotracheal intubation is a frequent procedure performed in neonates with respiratory distress. Clinicians use different methods to estimate the intubation insertion depth, but, unfortunately, the improper insertion results are very high. In this study, we aimed to compare the two different methods (Tochen's formula = weight in kilograms + 6 cm; and nasal septum–tragus length [NTL] + 1 cm) used to determine the endotracheal tube (ETT) insertion depth. Study Design Infants who had intubation indications were enrolled in this study. The intubation tube was fixed using the Tochen formula (Tochen group) or the NTL + 1 cm formula (NTL group). After intubation, the chest radiograph was evaluated (above T1, proper place, and below T2). Results A total of 167 infants (22–42 weeks of gestational age) were included in the study. The proper tube placement rate in both groups was similar (32.4 vs. 30.4% for infants < 34 weeks of gestational age and 56.8 vs. 45.0% in infants > 34 weeks of gestational age). The ETT was frequently placed below T2 at a higher rate in infants with a gestational age of <34 weeks, especially in the NTL group (46% in the Tochen group and 60.7% in the NTL group). Conclusion The NTL + 1 cm formula led to a higher rate of ETT placement below T2, especially in infants with a birth weight of <1,500 g. - Abstract
Background to evaluate the prevalence, risk factors and treatment of retinopathy of prematurity (ROP) in Turkey and to establish screening criteria for this condition. Methods: a prospective cohort study (TR-ROP) was performed between 1 April 2016 and 30 April 2017 in 69 neonatal intensive care units (NICUs). Infants with a birth weight (BW)=1500 g or gestational age (GA)<= 32 weeks and those with a BW> 1500 g or GA> 32 weeks with an unstable clinical course were included in the study. Predictors for the development of ROP were determined by logistic regression analyses. Results: the TR-ROP study included 6115 infants: 4964 (81%) with a GA <= 32 weeks and 1151 (19%) with a GA>32 weeks. Overall, 27% had any stage of ROP and 6.7% had severe ROP. A lower BW, smaller GA, total days on oxygen, late-onset sepsis, frequency of red blood cell transfusions and relative weight gain were identified as independent risk factors for severe ROP in infants with a BW=1500 g. Of all infants, 414 needed treatment and 395 (95.4%) of the treated infants had a BW <= 1500 g. Sixty-six (16%) of the treated infants did not fulfil the Early Treatment for Retinopathy of Prematurity requirements for treatment. Conclusions: screening of infants with a GA <= 34 weeks or a BW<1700 g appears to be appropriate in Turkey. Objective: Tuberous sclerosis complex (TSC) is an autosomal dominant neurocutaneous syndrome. TSC arises from mutations in either TSC1, at 9q34, or TSC2, at 16p13.3. Skin lesions, such as hypomelanotic macules, facial angiofibromas, shagreen patches, and ungual fibromas, are frequently seen in these patients. The present study aims to investigate clinical manifestations, molecular findings and phenotype-genotype correlations in 17 patients with TSC. Materials and Methods: TSC1 and TSC2 molecular analyses were performed on a next-generation sequencing platform (Illumina MiSeq). Variant interpretation was made in accordance with the American College of Medical Genetics 2015 recommendations. Results: Four patients carried a heterozygous mutation in TSC1, while the remaining seven carried mutations in TSC2. Three novel variants in TSC2 were defined. Sequencing failed to detect a mutation in six patients. In only one of these patients, multiplex ligation-dependent probe amplification (MLPA®) could be performed, and a large deletion in the TSC1 gene was detected. A wide spectrum of phenotypic features was noted throughout the study group. Dermatological findings were observed in almost all patients. Conclusion: In this study, in addition to the three novel mutations reported herein, the spectrum of TSC1 and TSC2 gene mutations and their phenotypes were reported. [ABSTRACT FROM AUTHOR]

Aim: Exogenous surfactant may be needed not only for Respiratory Distress syndrome (RDS) treatment; but also, in the management of other pulmonary diseases of infants. In this study, we aimed to investigate the impact of single versus multiple doses of surfactant therapy in pulmonary problems of preterm infants. Materials and Methods: In this study, preterm infants who needed surfactant treatment were retrospectively evaluated. Surfactant therapy for RDS were given as 200 mg/kg poractant or 100 mg/kg beractant and repeated with 100 mg/kg doses when needed Later. Poractant or beractant (100 mg/kg) were given in the treatment of other pulmonary diseases. Results: Totally 64 preterm patients were recruited into this study. Patients in group 1 (43.8%) received a single dose of surfactant; whereas group 2 patients (56.2%) had more than one dose. Mean gestational age and birth weight of infants in group 2 were significantly Lower than group 1 (p<0.05). Intrauterine growth restriction (IUGR) was more common in group 2 (p=0.041). Multiple doses of surfactant were needed for severe RDS, atelectasis, pulmonary hemorrhage and pneumonia. Duration of mechanical ventilation and hospitalization were Longer in group 2 (p<0.05). Mortality rates were higher in group 2 (p=0.011). Conclusion: Preterm infants with earlier gestational age and Lower birth weight; particularly with IUGR may need multiple doses of surfactant due to more severe respiratory problems regardless of antenatal steroid or maternal chorioamnionitis status. Duration of mechanical ventilation, hospitalization and also neonatal mortality remained higher due to disease severity in preterms who needed multiple doses of surfactant. [ABSTRACT FROM AUTHOR]

Amaç: Bu çalışma nekrotizan enterokolit (NEK) tanısı ile izlenen olgularda hastalığın risk faktörleri, karakteristik özellikleri, klinik, laboratuvar bulguları ve mortalite oranının değerlendirilmesi amacıyla yapılmıştır.Gereç ve Yöntemler: Çalışmaya 2010-2011 yıllarında kliniğimizde izlenen 488 hasta yenidoğan içinde NEK tanısı alan 37 bebek dahil edilmiştir.Bulgular: NEK sıklığı tüm yatan hasta popülasyonunda %7,5 olarak bulundu. NEK olgularında ortalama gebelik yaşı, 27,5±2,5 hafta, ortalama doğum ağırlığı, 1044±485 gram idi ve NEK'li olguların %90'ı, Aim: the aim of this study was to evaluate the risk factors, characteristic features, clinical and laboratory findings and mortality rates of necrotizing enterocolitis (NEC) patients.Materials and Methods: Thirty seven patients diagnosed with NEC among the 488 cases followed up in our clinic between 2010-2011 were included into this study.Results: the incidence of NEC was found as 7.5% among the total patient population. Mean gestational age and birth weight of NEC patients were 27.5±2.5 weeks and 1044±485 grams. Ninety percent of NEC patients were born under 1500 grams and 30 weeks. Twenty-four cases were classified as stage 1, and 13 cases were classified as stage 2-3. Stage 2-3 NEC was diagnosed in 9.8% of very low birth weight infants. the mean gestational age, birth weight, gender, delivery type, Apgar scores and the incidence of intrauterine growth restriction, diagnosis and/or treatment of patent ductus arteriosus, retinopathy of prematurity, respiratory distress syndrome, intracranial hemorrhage were similar between patients with different stages of NEC.
