La gestione mirata della temperatura (TTM) è un intervento neuroprotettivo cruciale impiegato nelle unità di terapia intensiva per migliorare la prognosi dei pazienti dopo un arresto cardiaco. Il brivido, un effetto collaterale comune della TTM, mette in discussione i suoi benefici neuroprotettivi. Questa scoping review mira a valutare la letteratura disponibile sulle terapie non farmacologiche per il brivido nei pazienti con TTM, identificando le lacune e le potenziali aree di ricerca future. Sono stati analizzati 13 studi sul tema, che hanno coinvolto 743 soggetti, con particolare attenzione alla TTM neuroprotettiva. La revisione ha evidenziato la natura incoerente della letteratura esistente sui trattamenti non farmacologici del brivido, sottolineando la necessità di interventi efficaci supportati da studi clinici. Le coperte ad aria forzata sono emerse come un approccio frequentemente esplorato ed efficace. Lo studio fornisce una base per la ricerca futura sulle terapie non farmacologiche, sottolineando l'importanza di una valutazione oggettiva del brivido e di strumenti di misurazione validati.

En este artículo se reflexiona sobre la forma en que la pandemia derivada del COVID-19 ha alterado determinados procesos asistenciales frente a la muerte. La rápida propagación incontrolada de la enfermedad y la falta de respuesta organizativa en términos de salud pública pronto condujeron a una desproporción entre la demanda asistencial en cuidados intensivos y los recursos humanos y materiales para satisfacerla. En ocasiones, los médicos se han visto en el dilema de decidir qué pacientes tratar y cuales excluir en las unidades de cuidados intensivos. Tanto en España como en Italia se ha usado el triaje y se han publicado algunas recomendaciones que han suscitado ciertas críticas en el ámbito médico, ético y legal. Se plantea el interrogante de que estas medidas excepcionales se puedan aplicar cuando se recupera la normalidad socio sanitaria.

Introducción: Las enfermedades reumatológicas no son infrecuentes en la población ge- neral. Este grupo de pacientes suele requerir hospitalización por diversas causas y hasta un tercio puede ingresar a la Unidad de Cuidados Intensivos (UCI). El pronóstico puede ser desfavorable, sobretodo en presencia de compromiso renal. Objetivo: Caracterizar a los pacientes reumatológicos ingresados a Cuidados Intensivos y determinar la mortalidad. Métodos: Se realizó un estudio descriptivo, retrospectivo de corte transversal. Los datos fueron obtenidos de las historias clínicas de los pacientes con enfermedades reumatológi- cas ingresados al Departamento de Cuidados Intensivos Adultos del Hospital de Clínicas desde enero de 2021 hasta diciembre de 2022. Resultados: Veintinueve pacientes con enfermedades reumatológicas ingresaron en los dos años de estudio. Con una mediana de edad de 37 años (RIC 22,5), veintitrés fueron mujeres (79,31%). Tenían lupus eritematoso sistémico veinte pacientes (68,9%); cinco con artritis reumatoide (17,2%) y los cuatros pacientes restantes tenían enfermedad mixta del tejido conectivo, Síndrome de Sjogren, Síndrome de Superposición y Vasculitis de Churg Strauss (3,44%). Las comorbilidades más frecuentes fueron: hipertensión arterial (55%) y nefropatía (41,3%). Las causas de ingreso a la UCI fueron médicas (62%) y quirúrgicas (38%). Entre las médicas, la insuficiencia respiratoria aguda fue el principal motivo (44%), seguido por shock (27%) e infecciones sobre agregadas (11,1%). La puntuación media de APACHE II (Age and Chronic Health Evaluation) fue 17 ± 10; SOFA al ingreso (Sequential Organ Failure Assessment): 6 ± 8,5 y en los pacientes lúpicos la Escala de SLEDAI (Sys- temic Lupus Erythematosus Disease Activity Measure): 10,6 ± 8,3. Recibieron asistencia respiratoria mecánica (ARM) 19 pacientes (65,5%) con una mediana de días de ARM de 6 (RIC 9). Utilizaron drogas vasoactivas 65,5% y hemodiálisis 17,2%. Se administró metil- prednisolona a 10 de los pacientes (37,9%), además 4 de ellos requirieron ciclofosfamida y 3 inmunoglobulinas. Durante su hospitalización desarrollaron falla renal aguda 13 (44,8%) y sobreinfecciones12 pacientes (41,3%), de los cuales en 6 se aislaron Gram positivos en sangre. La mediana de estancia fue de 6 días (RIC:10) y la mortalidad en UCI fue del 37,9% (11 pacientes). Conclusiones: En nuestro estudio la población fue principalmente de mujeres jóvenes con comorbilidades, que ingresaron graves a terapia intensiva, la mayoría por causas médicas. Dos tercios requirieron ARM y drogas vasoactivas. Presentaron complicaciones como la falla renal aguda y sobreinfecciones bacterianas. La mortalidad fue del 38%, solo un poco mayor que la correspondiente al escore de gravedad del ingreso.

Objective: To describe and analyze, at various levels, the communication exchanges that took place in a group chat of intensivists using the instant messaging application WhatsApp during the first stage of the COVID-19 pandemic, specifically on the day the first case was confirmed in Mexico. An intense situation showing the three dimensions of the theoretical framework is stood out. Materials and methods: A qualitative research of WhatsApp messages shared between 239 intensivists at the beginning of the pandemic, specifically throughout February 27, 2020. A narrative analysis was used to interpret a fragment of a chat. A schema with three dimensions (analysis of shared information, standards of social action in COVID-19 patient care and emotional expressions) was developed to code and classify the messages. Results: The communication exchanges via WhatsApp made it possible to give meaning to the emerging knowledge about COVID-19 in the narrative plots. They also influenced the implementation of appropriate actions in hospital environments and helped modulate emotions in front of the pandemic. In addition, it fostered bonds of solidarity and empathy between intensivists to face personal and social suffering with resilience. Conclusions: The chat messages reflected human relationships and the deep concerns of people in crisis situations. The study provided insight into the forms and meanings of communication exchanges with the use of technological devices in times of crisis to guide the implementation of actions in emerging situations such as the COVID-19 pandemic. WhatsApp responded to the need for information, with scientific and truthful data, about the pandemic. RESUMO Trata-se de um estudo exploratório através do procedimento técnico de levantamento bibliográfico e análise documental de dados extraídos dos sites do Ministério da Saúde e Instituto Brasileiro de Geografia e Estatística. O objetivo foi analisar a cobertura assistencial em terapia intensiva por grandes regiões geográficas brasileiras, a partir de conceitos da Bioética de Intervenção. Foi possível estimar a necessidade de leitos de unidade de terapia intensiva através da Portaria nº 1.101 do Ministério da Saúde, que corresponde a 4 a 10% do total de leitos hospitalares. Identificou-se que as regiões Norte e Nordeste, as mais vulneráveis, possuem as menores coberturas em terapia intensiva do país. Nesse contexto de política e saúde, a Bioética de Intervenção emerge com pertinência, promovendo o discurso a favor dos vulneráveis.

The prescription of medications under conditions of hospitalization in the Intensive Care Unit (ICU) is twice as high as in the ward, favoring more frequent drug interactions and increasing the risk of adverse events associated with medications. The objective of the present study was to review the scientific literature in order to understand the profile of Drug Interactions (DI) and characterize the most prevalent and potentially toxic pharmacological interactions in ICU patients. To this end, a bibliographical search was conducted in the MEDLINE, LILACS and EMBASE databases. After selecting studies published between 2017 and 2023, 24 articles were included in the present review. The results of the studies analyzed indicate that the majority of interactions are due to pharmacodynamic mechanisms, with cardiovascular drugs being the main responsible for DI. Combinations of clopidogrel and aspirin, clopidogrel and pantoprazole, followed by the combination of azithromycin and salbutamol, were the combinations most commonly related to DIs. Understanding the main DIs helps healthcare professionals optimize therapy and obtain better results for patients, preventing the occurrence of a potentially fatal error at some point during their ICU stay. The assistance of appropriate software and the participation of the clinical pharmacist as responsible for monitoring DIs and notifying the doctor about possible problems arising from prescriptions can reduce the incidence of adverse effects related to DIs under ICU admission conditions. Background. Critically ill patients may develop health problems related to their illness, injury, mechanical ventilation or other treatments. Such problems cannot be totally prevented and can continue after the patient leaves the hospital. Aim. To explore the level of anxiety, depression, post-traumatic stress Intensive Care Unit and quality of life in Intensive Care Unit survivors. A comparison analysis between Covid-19 patients and Non-Covid-19 patients was conducted. Methods. A prospective observational study exploring psychological, emotional, and behavioral difficulties experienced in patients admitted to Intensive Care Unit was performed. The study was conducted in an Italian adult 8-bed Intensive Care Unit, from July 2020 to April 2021, and followed-up until 25th May 2022. Data were collected during Intensive Care Unit stay (data collection of demographic and clinical characteristics) and 6 and 12 months after Intensive Care Unit discharge (interviews). Results. A total of 143 patients participated in the study, of which 54 were admitted for Covid-19 (37.76%). Depression symptoms were observed more among Non-Covid-19 patients compared to Covid-19 patients at six months (p= .037) and 12 months (p< .001) after Intensive Care Unit discharge. The quality of life perceived by the Intensive Care Unit patients surveyed improved between 6 and 12 months after discharge (Eq-VAS mean=62.03, + 11.2 vs Eq-VAS mean=66.6, + 9.8) (p=.034). Six months after Intensive Care Unit discharge, the mean of the perception of quality of life, for Covid-19 patients was 63.91 (sd ± 9.30), greater than Non-Covid-19 patients of the same period, which was 60.18 (sd ± 8.63) (p= .038). Conclusions Within 1 year from the acute infection, most hospital survivors of Covid-19 had good physical and functional recovery over time with better outcomes than other Intensive Care Unit patients and had returned to their original work and life. A pneumonia consiste em uma inflamação do parênquima pulmonar, causada por agentes infecciosos ou não. O diagnóstico é dado pelos sinais clínicos, como tosse, espirros e dispneia e pela radiografia e cultura e antibiograma de lavado bronco alveolar (LBA). O presente trabalho relata o caso de um felino, sem raça definida, macho, de um ano de idade, atendido com queixa principal de dispneia. Foi necessária abordagem emergencial devido a dificuldade respiratória e saturação de oxigênio de 71%. O paciente foi internado em unidade de terapia intensiva (UTI) em ventilação mecânica. O diagnóstico de pneumonia por Staphylococcus coagulase negativa foi confirmado pela cultura e antibiograma de LBA. O animal permaneceu internado durante quatro dias em que foi instituída terapia com Ceftriaxona 25 mg/kg/TID, Dexametasona 0,2 mg/kg/TID, Terbutalina 0,01 mg/kg/TID, reposição hidroeletrolítica e tratamento sintomático, com o uso de antitérmico e analgésicos. Foram realizados diagnósticos diferenciais para doenças retrovirais, micoplasmose e possíveis afecções concomitantes. Após o início do tratamento, o paciente apresentou melhora clínica, foi retirado da ventilação mecânica e após a estabilização dos parâmetros vitais recebeu alta. A rápida abordagem emergencial, diagnóstico correto e tratamento em terapia intensiva foi fundamental para o sucesso da recuperação.

Resumo Fundamento Alguns estudos demonstraram uma maior prevalência de óbitos em portadores de fatores de risco cardiovascular (FRC) durante internação por COVID-19. Objetivos Avaliar o impacto do alto risco cardiovascular em pacientes internados em terapia intensiva por COVID-19 Métodos Estudo retrospectivo com pacientes admitidos em terapia intensiva, com diagnóstico confirmado de COVID-19 por RT-PCR e com pelo menos uma dosagem de troponina durante a internação. Os critérios para definição de paciente de alto risco cardiovascular (ARC) foram: histórico de doença cardiovascular estabelecida (infarto, AVC ou doença arterial periférica), diabetes, doença renal crônica com clearance < 60ml/min ou presença de 3 FRC (hipertensão, tabagismo, dislipidemia ou idade > 65 anos). Introdução: O novo coronavírus (SARS-CoV-2) é um patógeno que acomete principalmente o sistema respiratório, sendo o causador da COVID-19. Esse vírus pode afetar todos os grupos populacionais, entretanto indivíduos obesos são mais suscetíveis a evoluir com um quadro mais grave da doença. A obesidade pode ocasionar alterações do sistema imunológico, aumento do risco de infecções e aumento da taxa de mortalidade por COVID-19. Diante de maiores complicações relacionadas com a obesidade e a COVID-19, é necessário o tratamento específico para esses indivíduos no ambiente hospitalar. Portanto, o objetivo do estudo foi desenvolver um protocolo de atendimento nutricional a pacientes obesos com diagnóstico de COVID-19. Metodologia: A busca dos trabalhos foi realizada em base de dados PubMed, MEDLINE, LILACS, SciELO e Google Scholar, utilizando os descritores coronavirus, COVID-19, SARS-CoV-2, SARS-CoV como termo principal, associados a obesity, body composition, infflamation, adipose tissue, nutrition, critical illness. Resultados: foi estabelecido um protocolo para realização da avaliação e triagem de risco nutricional, com definição da via de alimentação pela equipe médica com o auxílio da equipe multiprofissional. As estimativas das necessidades calóricas e proteicas devem ser baseadas de acordo com a avaliação nutricional. Discussão: A avaliação, triagem e terapia nutricional adequadas são fundamentais para melhor recuperação de pacientes obesos. Devido a redução da ingestão alimentar e os sintomas gastrointestinais ocasionada pela COVID-19, é fundamental monitorar os sintomas e a tolerância a dieta ofertada. Conclusão: um protocolo de atendimento nutricional com padronizações de condutas nutricionais pode garantir menores complicações e contribuir para a redução da taxa de mortalidade.

Introduction: The quality of life of critically ill patients who survive treatment in intensive care units is lower than that of the general population. Baseline health status and severity of clinical condition on admission to intensive care are risk factors for quality of life and functionality. Objective: To analyze the level of knowledge on quality of life and functionality of intensive care survivors. Materials and methods: An exploratory review was conducted by searching studies published between January 2010 and May 2020 in Scielo, PubMed, Science Direct, ProQuest, Redalyc, Dialnet, OVID, and Scopus databases. The study was conducted according to the PRISMA statement. Full texts that met the inclusion criteria were reviewed and analyzed in the final selection of articles. Results: Of 1814 articles identified, 65 articles describing the quality of life and functionality in post-intensive care patients were screened. Finally, 16 were included to analyze the article's characteristics, population characteristics, and variables of analysis for assessing the quality of life and functionality of post-intensive care survivors. Conclusions: Studies on quality of life and functionality in intensive care survivors were conducted mainly in Europe between 2010 and 2016. They are primarily prospective observational studies correlating factors determining mental and physical health after intensive care discharge. Multiple scales were applied; the most used were the SF-36 and the EQ-5D to assess the quality of life and the Barthel Index to determine functional status in patients discharged from intensive care. Introducción. La electromiografía de superficie (EMGs) en pacientes críticos se propone como una herramienta no invasiva, de fácil uso para el diagnóstico, el seguimiento de la debilidad muscular adquirida en UCI (DMA-UCI) y la identificación de patrones motores intencionales y automáticos. El objetivo de esta revisión fue mapear y presentar información publicada relacionado con el uso de la electromiografía de superficie (EMGs) en pacientes adultos bajo cuidado intensivo. Metodología. Revisión exploratoria con el método del Instituto Joanna Briggs, concebido por Arskey y O Malley. Se realizo una búsqueda en las bases de datos Cochrane Database, PubMed, SciELO, BVS, CINAHL, PEDro, EBSCO y búsqueda manual, con el uso de palabras clave y términos del tesauro MeSH en idiomas español, inglés y portugués, sin límites de tiempo. Los resultados se presentan de forma descriptiva. Se incluyeron estudios de revisión, estudios de resultados primarios y posters de eventos. Resultados. Se identificaron 1.146 estudios, de los cuales se excluyeron 1.135; mediante búsqueda manual se identificaron dos y finalmente se incluyeron nueve . Los estudios fueron publicados en Colombia, Francia, USA, Italia, Holanda, Rusia, Corea y Uruguay. Los usos reportados corresponden al apoyo en el diagnóstico (30%) y seguimiento de la DMA-UCI (40%). De los estudios encontrados, 33,3% (n=3) fueron estudios transversales, 22,2% (n=2) reportes de caso, 22,2% (n=2) estudios de revisión y 11,1% (n=1) para cada uno de los siguientes diseños: serie de casos y resumen de artículo. Conclusiones. El uso de la EMGs en el paciente crítico se ha enfocado principalmente en el seguimiento y diagnostico de la debilidad muscular adquirida en UCI así como en el seguimiento de estrategias médicas durante la hospitalización. La evidencia revisada describe el uso de la EMGs en el paciente crítico, como una herramienta de evaluación y seguimiento de la activación muscular respiratoria y periférica, no invasiva y a la cabecera del paciente; este técnica evidencia un interés creciente en el uso de herramientas objetivas para el diagnóstico y el seguimiento de la función muscular estos pacientes.

OBJETIVO: Avaliar etiologia, complicações, tratamento, tempo de internação – hospitalar e em centro de terapia intensiva – e mortalidade de todos os pacientes internados por pancreatite aguda no centro de tratamento intensivo do Hospital de Clínicas de Porto Alegre, no período de janeiro de 1990 a dezembro de 1999. MATERIAIS E MÉTODOS: Realizamos um estudo de coorte histórico, no qual foram avaliados 57 pacientes, 37% do sexo feminino e 63% do sexo masculino, com média de idade de 48 ± 17 anos. Os pacientes foram divididos em dois grupos – sobreviventes (n=26;45,6%) e não-sobreviventes (n=31;54,4%) –, e foram comparados quanto a tempo de internação, critérios de Ranson e de Glasgow modificados, APACHE II (acute physiology and chronic health evaluation), falências orgânicas, procedimentos cirúrgicos, nutrição parenteral e antibióticos recebidos. RESULTADOS: As etiologias mais freqüentes foram alcoólica (37%) e biliar (31%). A mortalidade foi de 54,4%. Os sobreviventes apresentaram maior tempo de internação que os não-sobreviventes (47 ± 36 dias contra 21 ± 20 dias). Os não-sobreviventes apresentaram maiores taxas de falências orgânicas (respiratória, renal e cardiovascular) e maior número de critérios de Ranson e de Glasgow modificados, quando comparados aos sobreviventes. Os parâmetros restantes foram semelhantes entre os dois grupos. CONCLUSÕES: Para melhor avaliar os motivos da alta taxa de mortalidade identificada neste grupo, neste período, seria necessário um trabalho prospectivo com melhor controle dos fatores interferentes e que incluísse ainda a avaliação dos casos de pancreatite aguda com critérios de gravidade que não são admitidos no centro de tratamento intensivo.

There are great risks of patients hospitalized in Intensive Care Units (ICU) progressing to death, requiring early conduct in palliative care in this population. However, many professionals are not prepared for this reality and feel insecure. With this, the need to assess the knowledge in palliative care of a multiprofessional team in Intensive Care emerged. The aim of this study was to assess the perception and level of knowledge in palliative care in ICU professionals. This is a cross-sectional, descriptive, and single-center study, carried out through an online questionnaire from December 2020 to January 2021. It was found that more than 87% had knowledge of basic concepts, with 15.6% did training involving the theme: palliative care. 82.3% reported that they were prepared to deal with this patient profile, but 75.8% reported that the team was not. 28.2% stated that they were prepared to deliver bad news, while a higher percentage, 34.4%, reported that the team would be prepared. Despite the formal incapacity, the professionals considered themselves prepared, while the team would be less prepared. Objetivos:Descrever o perfil demográfico dos pacientes, mensurar a carga de trabalho da enfermagem por meio da aplicação do instrumento Nursing Activities Score (NAS) e apresentar o dimensionamento recomendado para a equipe de enfermagem. Método: estudo transversal, prospectivo e descritivo, desenvolvido na unidade de terapia intensiva, com 10 leitos, de um hospital Universitário do Rio de Janeiro entre maio e junho de 2017, tendo 138 avaliações, em 12 pacientes, num total de 20 dias. Resultados: Houve predominância do sexo feminino (67%), com idade mediana de 60,5 anos, e tempo de internação em média de 11,58 dias, o percentual do NAS médio observado foi de 76,9%, correspondendo a 18,4 horas de assistência de enfermagem nas 24 horas, com recomendação de no mínimo 10 profissionais de enfermagem por turno, sendo 52% enfermeiros. Conclusão: A aplicação do NAS ajustado às recomendações do Conselho Federal de Enfermagem contribui para o dimensionamento adequado dos profissionais de enfermagem.

Resumo Objetivo avaliar o conhecimento dos profissionais de enfermagem baseado em simulação in situ sobre o reconhecimento de sepse. Estudo descritivo, quantitativo, realizado em um hospital do interior do estado de São Paulo com profissionais da equipe de enfermagem da Unidade de Terapia Intensiva. Os participantes foram submetidos a um pré-teste para avaliação de conhecimento em seguida aula expositiva dialogada e posterior simulação clínica. Após 30 dias foi aplicado o pós-teste. Houve contribuição da simulação na aquisição e retenção de conhecimento dos profissionais de enfermagem. A média de acerto no pré-teste foi maior na categoria enfermeiro (63,89%). Objective: To report the experience of educational intervention on pressure injury prevention with nursing professionals in intensive care. Methods: Experience report, developed in six intensive care units in the northeast region, from September to October 2018, after ethical approval, opinion 2.495.293. Results: The educational intervention was based on the Constructivist Theory of Meaningful Learning and emphasized the new guidelines of the National Pressure Ulcer Advory Panel and discussion of clinical cases. 18 short meetings were held with the participation of 29 nurses and 62 nursing technicians. Introduction: there are few studies on pediatric patients with severe intoxications requiring management in intensive care units (ICUs). Objective: to describe the clinical characteristics of patients treated in the ICU for intentional or accidental intoxications. Material and methods: observational, descriptive, and retrospective study of 26 patients, younger than 17 years of age, admitted to the ICU for severe intoxication in a private hospital. Results: mean age was 9.6 ± 6 years, 58% were female. In 15 patients the intoxication was intentional (57.6%) and 11 accidental (42.4%). Intoxications caused by medication predominated in both groups. The intentional ones were more frequent in the group of 11-15 years, and in women with a history of psychiatric disorders. While the accidental ones, the majority were in patients from one to five years old, without distinction in sex or important personal or family history. In general, in the 26 patients the management was supportive and no mortality was recorded. Conclusions: it seems that there are differences in the characteristics of patients who are admitted to an ICU for care, with intentional intoxications or those that occur accidentally. Introdução: Adoecimento e morte de crianças são eventos pouco aceitos na sociedade. Objetivo: Analisar a percepção de médicos e enfermeiros de terapia intensiva sobre a morte de crianças. Métodos: Estudo de abordagem qualitativa, com médicos e enfermeiros de três Unidades de Terapia Intensiva (UTI Pediátrica, UTI Neonatal e UTI Cardiológica) de um hospital de ensino. Foram realizadas entrevistas semiestruturadas gravadas, transcritas e analisadas segundo análise de conteúdo na modalidade temática. Resultados: Foram entrevistados 14 profissionais, com idade entre 28-53 anos, que possuíam tempo mínimo de dois anos de atuação em terapia intensiva e média de 43 horas semanais de trabalho. Os sentimentos e atitudes diante da morte emergiram da análise das falas e foram categorizados nos seguintes temas: percepção da morte na infância, obstinação terapêutica e sofrimento no encontro com as famílias. Os profissionais reconheceram lacunas na formação durante a graduação e pós-graduação para lidar com a morte de crianças. Sofrimento, ansiedade, culpa, frustração e impotência foram relatados, demonstrando a complexidade que envolve situações de morte de criança. A condição clínica e a capacidade de interação da criança influenciaram nas atitudes e na própria maneira do profissional lidar com a morte. A espiritualidade foi um importante mecanismo de enfrentamento. Conclusão: A morte de crianças em UTI provoca sentimentos ambivalentes em médicos e enfermeiros responsáveis pelo seu cuidado. Os profissionais sofrem pela morte de alguém que ainda não desfrutou da vida, porém a aceitam quando pensam no fim de uma vida marcada por sofrimentos e limitações impostos pela doença. O contato com o tema morte durante a formação profissional e nas discussões regulares dos serviços podem auxiliar na forma de lidar com esse evento. A espiritualidade foi um importante recurso de enfrentamento.

RESUMO As infecções do trato respiratório inferior associadas à ventilação mecânica são uma das complicações mais frequentes em pacientes em ventilação mecânica. Há muitos anos, a traqueobronquite associada à ventilação mecânica tem sido considerada uma doença que não demanda antibioticoterapia. Na última década, diversos estudos demonstraram que a traqueobronquite associada à ventilação mecânica deve ser considerada um processo intermediário que leva à pneumonia associada à ventilação mecânica, uma vez que apesar de ter impacto limitado sobre a mortalidade dos pacientes gravemente enfermos internados nas unidades de terapia intensiva, em contrapartida, demonstra associação significativa com o aumento dos custos hospitalares desses pacientes, assim como do tempo de internação na unidade de terapia intensiva e hospitalar, do uso de antibióticos, e da duração da ventilação mecânica. Embora ainda necessitemos de evidências científicas mais robustas, especialmente no que tange às modalidades terapêuticas, os dados atuais a respeito da traqueobronquite associada à ventilação mecânica salientam que há desfechos suficientemente importantes que exigem vigilância epidemiológica e controle clínico adequados.

Este estudo teve como objetivo identificar aspectos de afeto à segurança do paciente na passagem de plantão da equipe de enfermagem em Unidades de Terapia Intensivas Pediátricas. Estudo transversal, descritivo e quantitativo. A coleta de dados deu-se por observação não participante das Passagens de Plantão (PP) durante sete dias consecutivos nas Unidades de Terapia Intensivas Pediátricas de três hospitais universitários públicos do Paraná, Brasil. Utilizou-se o instrumento Passagem de Plantão da Equipe de Enfermagem em Unidade de Cuidado Intensivo Neonatal, adaptado ao estudo. Aos dados tabulados, empregou-se análise estatística descritiva. Houve prevalência do método verbal (75,9%), sem anotações escritas; não participação dos acompanhantes (75,9%) e frequentes interrupções na passagem de plantão (53,7%). A identificação do paciente; seu estado de saúde e evolução foram aspectos positivos à segurança do paciente nas PP. Embora apresentem espaço para melhoria, de modo geral, as PP mostraram-se consoantes ao preconizado.
