Abstract

Background: 18F-fluciclovine is a synthetic amino acid positron emission tomography (PET) radiotracer that is approved for use in prostate cancer. In this clinical study, we characterised the kinetic model best describing the uptake of 18F-fluciclovine in breast cancer and assessed differences in tracer kinetics and static parameters for different breast cancer receptor subtypes and tumour grades.Methods: Thirty-nine patients with pathologically proven breast cancer underwent 20-min dynamic PET/computed tomography imaging following the administration of 18F-fluciclovine. Uptake into primary breast tumours was evaluated using one- and two-tissue reversible compartmental kinetic models and static parameters.Results: A reversible one-tissue compartment model was shown to best describe tracer uptake in breast cancer. No significant differences were seen in kinetic or static parameters for different tumour receptor subtypes or grades. Kinetic and static parameters showed a good correlation.Conclusions: 18F-fluciclovine has potential in the imaging of primary breast cancer, but kinetic analysis may not have additional value over static measures of tracer uptake.Clinical Trial Registration: NCT03036943. Abstract

[18F]1-­amino-­3-­fluorocyclobutane 1-­carboxylic acid (18F-­fluciclovine) is a synthetic amino acid developed as a PET radiotracer for cancer imaging. Upon project commencement, 18F-­fluciclovine was in late phase of pharmaceutical development for recurrent prostate cancer imaging, requiring validation of clinical utility. Growing prospect in targeting of tumour metabolism in breast cancer treatment set the premise to investigate the role of amino acid imaging in this prevalent disease. The aims of this project were to: (1) establish the transport mechanisms of fluciclovine in breast cancer;; (2) explore the utility of fluciclovine in early assessment of response to treatments of related metabolic and biological targets;; (3) run a clinical study to investigate how 18F-­fluciclovine uptake can be used to characterise breast cancer biology;; and (4) run a multicentre clinical trial to investigate the impact of 18F-­fluciclovine imaging on clinical management of men with recurrent prostate cancer. Fluciclovine transport in breast cancer was found to be predominantly mediated by the high-­affinity glutamine transporter ASCT2. Increased fluciclovine uptake was demonstrated in cell lines sensitive to glutaminase inhibition with CB-­839 (known to result in build-­up of intra-­cellular glutamine);; and the opposite with bithionol and metformin which are known to reduce intra-­cellular glutamine. These results were recapitulated in animal models, and collectively raise the potential for fluciclovine to be used as an imaging biomarker of glutamine pool, in addition to early response assessment in therapies augmenting glutamine metabolism. The FRONTIER study was established to investigate how 18F-­fluciclovine can be used to characterise breast cancer biology and is ongoing. Initial data demonstrate a trend of highest uptake in triple-­receptor negative, followed by ER+ then HER2+ tumours, complementing in vitro data. The FALCON multicentre trial investigated the impact of 18F-­fluciclovine imaging on clinical management of recurrent prostate cancer. In the 32-­patient Oxford cohort, 44% of patients had their intended management plan revised following 18F-­fluciclovine PET/CT. Initial short term follow-­up data demonstrate a higher proportion of short-­term responders to 18F-­fluciclovine guided radiotherapy (71%, 5/7) compared to standard radiotherapy (53%, 8/15). This encouraging trend will require ratification with trial-­wide data. Across the trial, interim analysis found management was revised post-­scan in 52/85 (61%) patients, exceeding a pre-­ set criteria for overwhelming effectiveness (45/85), leading to early closure.

26. Are all urgent nephrostomies that urgent?.

Abstract

Purpose: To investigate if the presence of systemic inflammatory response syndrome (SIRS) in patients with urinary tract obstruction at time of presentation to the emergency department correlates with a subsequent positive urine culture and to determine if it could be used as a tool to predict the urgency of a percutaneous nephrostomy (PCN). The secondary aim of the study was to assess the incidence of in-hour and after-hour emergent PCNs performed in a tertiary center which has both interventional radiological and urological expertise. Methods and materials: Emergent adult PCN cases referred from the emergency department between 2011 and 2016 were identified retrospectively. Urine culture results, clinical features of SIRS, timing of procedure, and radiological findings were analyzed. Result(s): A total of 110 patients met the inclusion criteria. There were a total of 60 male and 50 female patients with a mean age of 62 (age range 18-99 years). Ninety-two (83%) PCN patients were referred and performed on after-hours with the remaining 18 patients performed in-hours. There were 53 (48%) patients that met the criteria for obstruction with SIRS and 57 (52%) patients with obstruction without SIRS. The number of after-hours PCN performed in both groups was equal at 46 patients each. The most common cause of obstruction in the SIRS group was stone related in 31 (58%) patients. Whereas, the most common cause of obstruction in the non-SIRS group was due to malignancy in 26 (45%) cases. An elevated temperature and heart rate were the most common features of SIRS at 96 and 83% respectively in the SIRS group. An organism was isolated in 35 (67%) out of the 52 urine culture results obtained for the SIRS group. No organism was isolated in 42 (95%) out of the 44 urine culture results obtained in the non-SIRS group. Abstract

Anti-AMPA (anti-alpha-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid) receptor limbic encephalitis is a rare autoimmune syndrome. It can be associated with an underlying malignancy, such as lung, breast, or thymus. We are reporting a case of a 19-year-old patient who presented with a 2-week history of confusion and short-term memory loss. A magnetic resonance imaging of brain showed increased T2 hyperintensity within the hippocampi bilaterally. Cerebrospinal fluid analysis was positive for anti-AMPA receptor antibodies. A computed tomography revealed multiple pulmonary metastases as well as an expansile lucent and sclerotic lesion in the ilium, which was subsequently biopsied. Histopathology confirmed a diagnosis of Ewing sarcoma (ES). Fluorescence in situ hybridization testing of the specimen identified EWSR1 (22q12.2) signal rearrangements in 80% of cells scored. To date, this is the first case report describing anti-AMPA receptor limbic/paraneoplastic encephalitis as a presenting feature of ES. Abstract

Poly(lactic acid)/poly(methyl methacrylate) (PLA/PMMA) blends were prepared by melt compounding technique. The miscibility of PLA/PMMA blends at various blending ratios (i.e. 80/20, 60/40, 40/60, and 20/80) was investigated using a dynamic mechanical analyzer (DMA) and solvent uptake experiment. The solvent uptake experiment was conducted to estimate the interaction of PLA/PMMA blends based on the calculation of Flory–Huggins interaction parameter (χ12). DMA results revealed that only single glass transition temperature (Tg) existed along the PLA/PMMA blends. Smallest χ12 (i.e. −0.03) was found on the PLA/PMMA20 blend, suggesting interaction between PLA and PMMA at this composition. The incorporation of PMMA slightly improved the UV protection properties of the PLA/PMMA blend, while maintaining their optical transparency. [ABSTRACT FROM AUTHOR]

Abstract

We investigated if previously demonstrated inhibition of fluciclovine (18F) in vitro could be replicated in a PC3-Luc xenograft mouse model. Following intratumoral injection of 2-aminobicyclo-(2,2,1)-heptane-2-carboxylic acid (BCH), alpha-(methylamino)isobutyric acid (MeAIB) or saline, fluciclovine PET tumor-to-background activity was 43.6 (± 5.4)% and 25.3 (± 5.2)% lower in BCH (n = 6) and MeAIB (n = 5) injected PC3 Luc xenografts, respectively, compared to saline-injected controls (n = 2). Abstract

This paper presents a methodology for extracting road edge and lane information for smart and intelligent navigation of vehicles. The range information provided by a fast laser range-measuring device is processed by an extended Kalman filter to extract the road edge or curb information. The resultant road edge information is used to aid in the extraction of the lane boundary from a CCD camera image. Hough Transform (HT) is used to extract the candidate lane boundary edges, and the most probable lane boundary is determined using an Active Line Model based on minimizing an appropriate Energy function. Experimental results are presented to demonstrate the effectiveness of the combined Laser and Vision strategy for road-edge and lane boundary detection.

Abstract

The primary focus is on the development of an intelligent control scheme, which is insensitive to parametric uncertainty, load and parameter fluctuations and most importantly amenable for real time implementation. In this paper, we present an effective uncoupled direct fuzzy PD/PI control scheme for an outdoor AGV, which is a converted electrically powered golf-car. The controller performance is assessed against the required performance criteria and also against another effective nonlinear control method known as computed torque technique (CTT). It is established through simulations that the fuzzy logic controller (FLC) yields good performance even under uncertain and variable parameters in the model, unlike the CTT. And in terms of real-time implementation the availability of custom fuzzy chips and the reduced computational complexity of the fuzzy controller as against the CTT, makes the fuzzy controller, an ideal choice amongst the two schemes.
