Objectives: The drop-out rate among residents across all medical specialties in the Netherlands approximates 12.7%. This implies a capacity loss in the medical workforce, a waste of educational resources and personal damage to individuals. The aim of our study was to investigate reasons for dropping out of residency and the relationship with medical work experience after medical school and prior to residency, which is common among Dutch graduates.Method: A questionnaire listing 28 reasons for drop-out was developed and tested. The questionnaire was sent in a nationwide survey to all residents who drop out between 1 September 2017 and 1 September 2019. The respondents were asked to indicate on a 5-point Likert scale, how they weighed reasons for drop-out. Factor analysis was applied to identify dominant factors. Results: The response rate was 39% (N=129; 99 females) representing all medical specialties. The factor structure of our measure showed 5 factors; high emotional job demands, lack of professional satisfaction, incompatible lifestyle, tensions in working relationships and disappointing career perspectives. Of the respondents 69 (54%) had prior clinical experience as a physician-not-in-training in the same medical specialty before residency. The factor "lack of professional satisfaction" weighed heavier for respondents without prior experience as a reason for drop-out.Conclusion: Of influence on dropping out of residency is the lack of a clear image of the responsibilities as a physician within the residency of choice, fuelled by limited prior experience after medical school and before residency. One third of dropouts confirmed that prior physician experience within the same specialty could have prevented dropping out or prevented choosing this specialty in the first place.

BACKGROUND: Today's nursing school applicants are considered “digital natives.” This study investigated students' views of new health care technologies. METHOD: In a cross-sectional survey among first-year nursing students, 23 common nursing activities and five telehealth nursing activities were presented along with three statements: “I consider this a core task of nursing,” “I look forward to becoming trained in this task,” and “I think I will do very well in performing this task.” RESULTS: Internet-generation nursing students (n = 1,113) reported a significantly (p ⩽ .001) less positive view of telehealth activities than of common nursing activities. Median differences were 0.7 (effect size [ES], −0.54), 0.4 (ES, −0.48), and 0.3 (ES, −0.39), measured on a 7-point scale. CONCLUSION: Internet-generation nursing students do not naturally have a positive view of technology-based health care provision. The results emphasize that adequate technology and telehealth education is still needed for nursing students. An at first sight seemingly coherent, global medical workforce, with clearly recognizable specialities, subspecialties and primary care doctors, appears at a closer look quite variable. Even within the most progressive countries as to the development of medical education, with educators who regularly meet at conferences and share major journals about medical education, the differences in structures and regulations are big. This contribution focuses on the preparation, admission policy, duration, examinations, and national competency frameworks in postgraduate speciality training in Germany, the USA, Canada, the UK, Australia and the Netherlands. While general objectives for postgraduate training programs have not been very clear, only recently competency-frameworks, created in a limited number of countries, serve harmonize objectives. This process appears to be a challenge and the recent creation of milestones for the reporting on progress of individual trainees (in the US and in Canada in different ways) and the adoption of entrustable professional activities, a most recent concept that is quickly spreading internationally as a framework for teaching and assessing in the clinical workplace is an interesting and hopeful development, but time will tell whether true harmonization across countries will happen., Was auf den ersten Blick als kohärente, globale Einheit der in der Medizin Beschäftigten erscheint, mit klar erkennbaren Fachgebieten, Teilfachgebieten und den Hausärzten, ist beim genaueren Hinsehen doch recht unterschiedlich. Auch in den in der Entwicklung der Medizinerausbildung fortschrittlichsten Ländern, in denen sich die Ausbilder regelmäßig auf Konferenzen treffen und die wichtigsten medizinischen Fachzeitschriften lesen, sind die strukturellen und gesetzlichen Unterschiede sehr groß. Dieser Beitrag beschäftigt sich mit der Vorbereitung, Zulassungspolitik, Dauer, den Examina und den nationalen Kompetenzrahmenbedingungen in der Facharztausbildung in Deutschland, USA, Kanada, dem Vereinigten Königreich, Australien und den Niederlanden. Die allgemeinen Ziele der Programme für die Postgraduiertenausbildung waren bisher nicht klar definiert, aber in der letzten Zeit werden übereinstimmende Ziele in Kompetenzrahmen gesetzt, die von einigen Ländern geschaffen wurden. Dieser Prozess stellt sich als schwierig heraus. Zwar wurden kürzlich Meilensteine für das Reporting von Fortschritten einzelner Auszubildender (in den USA und Kanada auf unterschiedliche Weise) geschaffen, und auch das vor Kurzem übernommene Konzept "entrustable professional activities", das sich als Rahmen für das Unterrichten und Bewerten am klinischen Arbeitsplatz schnell international ausbreitet, scheint ein interessanter und hoffnungsvoller Ansatz, aber man wird erst mit der Zeit wissen, ob sich die Länder wirklich einander annähern.

Objective: A well-designed final year may ease the transition from medical school to postgraduate training, if it has enough depth to enable the acquisition of early specialty expertise, while keeping enough breadth to support the graduation as all-round physician. Aim of this article is to describe the design of a multidisciplinary dedicated transitional year (DTY) around the theme of recognition and initial treatment of vitally threatened patients.Methods: Undergraduate and postgraduate training directors from the departments of Anaesthesiology, Cardiology, Emergency Medicine, Intensive Care Medicine and Respiratory Medicine at UMC Utrecht and partnering hospitals have collaboratively developed and implemented a curriculum for a final year focusing on three entrustable professional activities (EPAs) in the domain of acute care. These EPAs represent authentic tasks of starting residents in each of the participating specialties, align student training objectives with postgraduate expectations, and are the primary focus of learning, teaching, and assessment throughout the year. Students are developmentally supported by a mentor and educationally supported by monthly academic half days.Results: Between October 2014 and November 2016,, 47 students chose DTY Acute Care. The set-up of our DTY is inspiring other specialties to develop multidisciplinary DTYs. Attainment of clinical competence, experience of students and staff, and exploration of graduates' early careers are subjects of current research projects.Conclusion: This multidisciplinary dedicated transitional year aims to graduate students with profile-specific competence in acute care. It prepares for residency in a range of specialties., Zielsetzung: Ein gut gestaltetes letztes Studienjahr ("praktisches Jahr" oder "Übergangsjahr") kann den Übergang von Studium zu Facharztweiterbildung vereinfachen, wenn es ausreichend fachliche Tiefe zur frühen Aneignung von fachspezifischer Erfahrung bietet, ohne dabei die vollständige Bandbreite medizinischer Ausbildung zum praktischen Arzt zu vernachlässigen. Ziel dieses Artikels ist es, die Gestaltung eines multidisziplinären letzten Studienjahres ("dedicated transitional year", DTY) rund um die Erkennung und Erstbehandlung von vital bedrohten Patienten zu beschreiben.Methoden: Die Lehrbeauftragten für Studenten- und Facharztausbildung der Anästhesiologie, Kardiologie, Notfallmedizin, Intensivmedizin und Pulmologie der Uniklinik Utrecht (Universitätsmedizinisches Zentrum, UMC) und seiner Lehrkrankenhäuser entwickelten und implementierten gemeinsam ein Curriculum für ein letztes Studienjahr, welches sich auf drei APT ("anvertraubare professionelle Tätigkeit", engl. "entrustable professional activity", EPA) aus der Akutmedizin stützt. Diese APT repräsentieren tatsächliche Tätigkeiten beginnender Ärzte der teilnehmenden Fächer, gleichen die Lernziele der Studenten an diese zukünftigen Tätigkeiten an, und sind der Kern für Lernen, Lehre und Beurteilung während dieses Jahres. Die Studenten werden in ihrer Entwicklung durch einen Mentor unterstützt, und es findet monatlicher Unterricht statt.Ergebnisse: Zwischen Oktober 2014 und November 2016 haben sich 47 Studenten für das DTY Akutmedizin entschieden. Die Einrichtung unseres DTY inspiriert andere Fachrichtungen, ebenfalls multidisziplinäre DTYs zu entwickeln. Since the turn of the twenty-first century, competency-based medical education (CBME) has become a dominant approach to postgraduate medical education in many countries. CBME has a history dating back half a century and is rooted in general educational approaches such as outcome-based education and mastery learning. Despite controversies around the terminology and the CBME approach, important national medical regulatory bodies in Canada, the United States, and other countries have embraced CBME. CBME can be characterized as having two distinct features: a focus on specific domains of competence, and a relative independence of time in training, making it an individualized approach that is particularly applicable in workplace training. It is not the length of training that determines a person's readiness for unsupervised practice, but the attained competence or competencies. This shift in focus makes CBME different from traditional training. In this contribution, definitions of CBME and related concepts are detailed., Seit Beginn des 21. Jahrhunderts ist die kompetenzbasierte medizinische Weiterbildung (Competency-Based Medical Education, CBME) zum dominanten pädagogischen Ansatz in der medizinischen Ausbildung vieler Länder geworden. Mit einer Geschichte, die ein halbes Jahrhundert zurückgeht, ist die CBME in pädagogischen Ansätzen wie ergebnisorientierter Ausbildung und "Mastery Learning" verwurzelt. Trotz der Kontroversen um die Terminologie und den CBME-Ansatz haben wichtige nationale medizinische Aufsichtsbehörden in Kanada, den Vereinigten Staaten und in anderen Ländern eine Bereitschaft für CBME gezeigt. CBME kann durch zwei deutliche Merkmale charakterisiert werden: durch einen Fokus auf spezifische Kompetenzbereiche und durch eine relative Unabhängigkeit vom Zeitpunkt innerhalb der Ausbildung; so wird CBME zu einem individualisierten Ansatz, der besonders beim Lernen am Arbeitsplatz anwendbar ist. Es ist nicht die Länge der Ausbildung, die das Bereitsein des Einzelnen für nicht supervidierte Praxis bestimmt, sondern die gewonnene Kompetenz oder die gewonnenen Kompetenzen. Diese Schwerpunktverlagerung unterscheidet CBME von der traditionellen Ausbildung. In diesem Beitrag werden Definitionen von CBME und verwandten Konzepten ausgeführt.

Objectives: This study adds to the ongoing discussion on how to ease the transition from undergraduate medical training to postgraduate training. In the Netherlands there is no central matching system for admission to residency. Medical school graduates just apply for a position in an open job market. Many choose to acquire general or specialty-specific clinical experiences after the medical degree before residency, to further explore career opportunities and to increase their chances to get into their preferred specialty. To shorten this gap between undergraduate and the start of postgraduate training, the sixth and final year of most Dutch medical schools is designed as a "transitional year". Students work with more clinical responsibilities than in the earlier clerkships, and this year includes many elective options. Our study focuses on these elective options and explores how medical students use these transitional year electives to prepare for transition to postgraduate training.Methods: In 2012-2013 we asked all 274 graduating students at one Dutch medical school to complete an open-answer questionnaire with the following topics: their preferred specialty at the start of the transitional year, electives they chose during this year and reasons for these choices, and whether the transitional year electives changed their career considerations. Questionnaire results were coded by two researchers and were discussed with all members of the research team. Results: A total of 235 students responded (86%). Answers about motivation for choices revealed that most electives where chosen for career orientation and to optimize chances to get into a residency program. Students also focused on additional experiences in specialties related to their preferred specialty. Many students chose electives logically related to each other, e.g. combinations of surgery and radiology. In den Niederlanden gibt es kein zentrales Matching-System für die Zulassung zur Assistenzzeit. Medizinabsolventen bewerben sich einfach auf dem offenen Stellenmarkt um eine Stelle. Viele entscheiden sich dafür, nach dem Medizinexamen vor der Assistenzzeit allgemeine oder fachspezifische klinische Erfahrungen zu sammeln, Karrieremöglichkeiten näher kennenzulernen und ihre Chancen auf ihr gewünschtes Fachgebiet zu erhöhen. Um die Zeit zwischen dem Grundstudium und dem Beginn der Postgraduatiertenausbildung zu verkürzen, ist das sechste und letzte Jahr der meisten holländischen medizinischen Hochschulen als "Übergangsjahr" konzipiert. Die Studenten arbeiten mit größerer klinischer Verantwortung in den Kliniken und können in diesem Jahr viele Wahlmöglichkeiten nutzen. Unser Beitrag konzentriert sich auf diese Wahlmöglichkeiten und untersucht, wie die Medizinstudenten die Wahlmöglichkeiten dieses Übergangsjahres nutzen, um sich auf die Postgraduiertenausbildung vorzubereiten.Methoden: 2012-2013 baten wir alle Studenten einer holländischen medizinischen Hochschule, einen Fragebogen mit folgenden Themen auszufüllen: das von ihnen gewünschte Fachgebiet zu Beginn des Übergangsjahres, Wahlfächer, die sie während dieses Jahres wählen und ihre Gründe dafür und ob die Wahlfächer in dem Übergangsjahr die Überlegungen zu ihrer Laufbahn verändert haben. Die Antworten des Fragebogens wurden von zwei Forschern kodiert und unter allen Mitgliedern des Forschungsteams diskutiert. Ergebnisse: Insgesamt haben 235 Studenten (86%) geantwortet. Background: Telehealth is viewed as a major strategy to address the increasing demand for care and a shrinking care professional population. However, most nurses are not trained or are insufficiently trained to use these technologies effectively. Therefore, the potential of telehealth fails to reach full utilization. A better understanding of nursing telehealth entrustable professional activities (NT-EPAs) and the required competencies can contribute to the development of nursing telehealth education. Method: In a four-round Delphi-study, a panel of experts discussed which NT-EPAs are relevant for nurses and which competencies nurses need to possess to execute these activities effectively. The 51 experts, including nurses, nursing faculty, clients and technicians all familiar with telehealth, were asked to select items from a list of 52 competencies based on the literature and on a previous study. Additionally, the panelists could add competencies based on their experience in practice. The threshold used for consensus was set at 80%. Results: Consensus was achieved on the importance of fourteen NT-EPAs, requiring one or more of the following core competencies; coaching skills, the ability to combine clinical experience with telehealth, communication skills, clinical knowledge, ethical awareness, and a supportive attitude. Each NT-EPA requires a specific set of competencies (at least ten). In total, 52 competencies were identified as essential in telehealth. Discussion/Conclusion: Many competencies for telehealth, including clinical knowledge and communication skills, are not novel competencies. They are fundamental to nursing care as a whole and therefore are also indispensable for telehealth. Additionally, the fourteen NT-EPAs appeared to require additional subject specific competencies, such as the ability to put patients at ease when they feel insecure about using technology. The decision to trust a medical trainee with the critical responsibility to care for a patient is fundamental to clinical training. When carefully and deliberately made, such decisions can serve as significant stimuli for learning and also shape the assessment of trainees. Holding back entrustment decisions too much may hamper the trainee’s development toward unsupervised practice. When carelessly made, however, they jeopardize patient safety. Entrustment decision-making processes, therefore, deserve careful analysis.

Aims: In Germany, the final year of undergraduate medical education ('practice year') consists of three 16-week clinical attachments, two of which are internal medicine and surgery. Students can choose a specific specialty for their third 16-week attachment. Practice year students do not receive specific teaching to prepare them for the National Licensing Examination. It is unknown whether knowledge levels increase during this year. This study aimed at assessing knowledge at the beginning and the end of the final year of medical school. Methods: Three hundred pre-selected United States Medical Licensing Examination type items from ten medical disciplines were reviewed by ten recent medical graduates from the Netherlands and Germany. The resulting test included 150 items and was taken by 77 and 79 final year medical students from Göttingen and Hamburg at the beginning and the end of their practice year, respectively. Results: Cronbach's alpha of the pre- and post-test was 0.75 and 0.68, respectively. Mean percent scores in the pre- and post-test were 63.9±6.9 and 69.4±5.7, respectively (p<0.001; effect size calculated as Cohen's d: 0.87). In individual students, post-test scores were particularly high for items related to their specific chosen specialty.Conclusion: The knowledge test used in this study provides a suitable external tool to assess progress of undergraduate medical students in their knowledge during the practice year. The pre-test may be used to guide individual learning behaviour during this final year of undergraduate education., Zielsetzung: In Deutschland besteht das letzte Jahr des Studiums der Humanmedizin ("Praktisches Jahr", PJ) aus drei Tertialen von je 16 Wochen, von denen eines in der Inneren Medizin und eines in der Chirurgie abzuleisten ist. Die Fachrichtung des dritten Tertials von 16 Wochen kann von den Studierenden frei gewählt werden. Während des Praktischen Jahres findet keine gezielte Vorbereitung auf den schriftlichen Teil des Staatsexamens statt. Es ist unklar, inwieweit die Studierenden während des Praktischen Jahres auch neue Wissensinhalte erlernen. Ziel dieser Studie war es, zu Beginn und am Ende des Praktischen Jahres Wissensinhalte zu überprüfen. Methodik: Zehn Absolventen des Medizinstudiums in Deutschland und den Niederlanden trafen eine Auswahl aus 300 vorselektierten Fragen der US-amerikanischen Abschlussprüfung (USMLE), die zehn verschiedenen Fachrichtungen zugeordnet waren. Die ausgewählten 150 Fragen wurden im Rahmen zweier Tests PJ-Studierenden aus Göttingen und Hamburg vorgelegt: einmal zu Beginn (n=77 Studierende) und einmal am Ende des Praktischen Jahres (n=79). Ergebnisse: Die interne Konsistenz der beiden Tests (Cronbach's alpha) betrug 0,75 (Prätest) bzw. 0,68 (Posttest). Der Anteil richtig beantworteter Fragen betrug im Prätest 63,9±6,9 und im Posttest 69,4±5,7 (p<0,001; Effektstärke als Cohen's d: 0,87). Individuelle Studierende schnitten bei denjenigen Fragen besonders gut ab, die sich auf Inhalte ihres Wahlfachs bezogen.Schlussfolgerung: Der in dieser Studie verwendete Wissenstest eignet sich als externes Instrument zur Messung des Wissenszuwachses von Studierenden im Praktischen Jahr. Zudem kann der Prätest genutzt werden, um Studierende bei der Planung ihres Lernverhaltens während des Praktischen Jahres zu unterstützen.
