Temporaries are used as placeholders before the permanent crowns are installed. If the temporary crown is not fitted properly the tooth can be subject to increased decay and gums can become inflamed causing gingivitis which leads to other more serious problems. A provisional fixed restoration will provide a template for defining tooth contour, esthetics, proximal contacts, ridge contacts and occlusion. Margins made by the indirect technique are considered to be more accurate than those made by the direct technique. The purpose of this in vitro study was to compare the marginal accuracy of provisional crowns made from three different biomaterials using the established indirect method in different environments and to evaluate the effect of water absorption on polymerization shrinkage and the effect of polymerization shrinkage occurring in dry storage for a week. Method:Variables used in this study are: a) Revotek LC-light cured composite b) Protemp II c) poly methyl methacrylate. An aluminium master die was machined with dimensions: 5mm length, 10mm gingival diameter, 5 degree taper, and 1mm shoulder then dental stone die was prepared by making an impression of this experimental model using a poly vinyl siloxane material. Both group A and group B consisted of seven specimens each of DPI, ProtempII, Revotek LC. The specimens were evaluated using measuring microscope (Biolux), which had an eyepiece graticule of 1/10th of an mm. The seven specimens in group A (DPI, ProtempII, Revotek LC) were kept in air at room temperature for one week on the dental stone cast and the readings were tabulated and the mean values were obtained. The seven specimens in group B (DPI, ProtempII, and Revotek LC) were kept in water at room temperature for one week and the readings were tabulated in the same manner and the mean values were obtained. Results: Comparative statistics of the mean values of the specimens kept in air at room temperature showed that DPI had the least marginal discrepancies followed by ProtempII, Revotek LC. The values were highly significant (.002) Comparative statistics of the mean values of the specimens kept in water at room temperature showed that DPI had the least marginal discrepancies followed by Revotek LC, ProtempII. The values were highly significant (.009). Conclusion: After one week in air at room temperature and after one week in water at room temperature DPI recorded the minimal marginal discrepancy. When stored in air at room temperature, DPI had the best marginal adaptation. All of the materials showed evidence of continued polymerization shrinkage after storage in air for a week. Water absorption compensated for polymerization shrinkage in DPI and ProtempII whereas Revotek LC was an exception. Objectives: Assessment of the effect of aging and bonding on the reparability of different temporary crown and bridge materials using a flowable resin composite. Methods: The materials used included two bis-acryl and two polymethylmethacrylate materials. The materials were aged either dry, in distilled water, or in a 75% ethanol/water solution. Each group was divided into three subgroups (n = 6) according to the bonding method: application of a universal adhesive, application of a universal primer followed by a universal adhesive, or no bonding. Materials were repaired with a light-cure flowable resin composite; then, they were subjected to thermocycling and tested by shear bond strength. Immediate implant placement after extraction is a dental implant treatment procedure reported in several case studies with satisfactory results for patients and less crestal bone loss around the implant. This case report describes the method of immediate implant placement after extraction. It directs temporary crown placement in a female patient with failed post-endodontic treatment restoration on the maxillary incisor. The procedure was performed under local anesthesia, preceded by tooth extraction and socket cleaning post-extraction, followed by the implant and bone grafting procedure, then finalized by provisionalization. The immediate implant procedure is a simple and time-saving technique. No second surgery is required, such as delayed implant procedure, but the operator's good analytical, diagnostic, and planning skills in its implementation.

The materials used in temporary restorations can be classified as acrylic or resin composites. PMMA-based acrylics, introduced in the 1930s, are the materials most commonly used for single and multiple provisional restorations. Composites are temporary materials composed of bis-acrylic resin (self-curing, dual-setting or light-curing). The additive manufacture of restorations in three dimensions (3D) has had a significant impact in the field of restorative dentistry in recent years (ABDUO et al., 2014). This technology allows for greater savings, since it provides less waste of material, with the reuse of the resin that has not yet been processed in a new impression (BEUER et al., 2008). In addition, it also allows the printing of several materials at the same time (BERMAN, 2012; ABDUO et al., 2014). Despite this, a crucial point in question is whether these materials maintain a satisfactory level of mechanical and functional properties and, it is important to know their physical-biological properties, as they affect the strength characteristics of restorations, as well as their behavior in the biological environment of the oral cavity.

The aim of this study is to determine the optimal combination of self-curing resins and type of matrix that provides a minimal temperature increase in the pulp chamber during the fabrication of temporary crowns. Material and methods. We designed as experimental model of direct temporary crown fabrication for extracted and than prepared molars. Intrapulpal temperature rise was measured in vitro conditions during polymerization of Protemp II (3M), Protemp 4 (3M), Visalis Temp (Kettenbach), Structur (Voco) and Carbodent (Stoma). Output and peak temperature findings of self-curing resin polymerization were recorded and values ​​of temperature increase in the tooth chamber were calculated. Two types of materials were used to make external anatomical moulds: 1) silicone impression material Panasil Putty Soft of high and low viscosity and Panasil initial contact Light (Kettenbach) to make two-phase impression; 2) transparent thermoplastic polymer Erkodur (Erkodent), sheet of 1.0 mm thick, vacuum pressed. Results and Discussion. We obtained the following finding of the temperature rise inside the pulp chamber (polymer pattern / silicone matrix): Protemp IV (2,2˚C / 0,2˚C), VisalisTemp (3˚C / 0.3˚C), Protemp II (3,3˚C / 0,5˚C), Structur (3,4˚С/0,6˚С), Karbodent (6.7˚C / 3.0˚C). Conclusions. Exothermic effects during intra oral fabrication of temporary crowns can be minimized by polymerization of resins in the silicone mould as this material can absorb and dissipate heat.

Background: Immediate implant placement and provisionalization (IIPP) is considered as a high-risk treatment for aesthetic failure and generally only recommended in case of post-extraction intact sockets and a thick phenotype gingiva. During a prospective clinical cohort study on one-hundred consecutive patients, using this strict flapless immediate implant placement and provisionalization (FIIPP) protocol, we found high and stable aesthetic outcomes (WES/PES = 8.2/12.1) in both intact – and defect sockets, and both thin- as thick gingival phenotype. By means of one case report (Case 1), the total FIIPP treatment is illustrated. Results of two other cases, show that comparable high aesthetic outcomes can be reached in cases with a thin phenotype gingiva or buccal bone defect using the same protocol. Case Presentation: In a 24-year-old male with good general and oral health, root fracture of tooth 21 was diagnosed and FIIPP was indicated. Direct post-extraction, an implant was placed in a palatal position of an intact socket by a flapless approach. A minimum space of 2 mm in front of the implant was created and filled with a bone-substitute prior to implant placement. Subsequently, the implant was restored by use of a titanium abutment and a composite temporary crown. Three months later, the temporary crown was replaced by a customized zirconium abutment and ceramic crown resulting in a high aesthetic outcome. A comparable aesthetic outcome using this protocol can be reached in cases with a thin phenotype gingiva and/or a buccal bone defect (Case 2). Conclusion: Using described surgical FIIPP protocol, high aesthetic outcomes are reached in only 4 visits and within a treatment period of 4 months. A thin phenotype gingiva, or a pre-operative buccal bone defect, does not seem to affect the aesthetic outcome using this treatment method.

To investigate the impact of different cleaning methods on the fracture load and two-body wear of additively manufactured three-unit fixed dental prostheses (FDP) for long-term temporary use, compared to the respective outcomes of milled provisional PMMA FDPs. Shape congruent three-unit FDPs were 3D printed using three different resin-based materials [FPT, GCT, NMF] or milled [TEL] (N = 48, n = 16 per group). After printing, the FDPs were cleaned using: Isopropanol (ISO), Yellow Magic 7 (YEL), or centrifugal force (CEN). Chewing simulation was carried out with a vertical load of 50 N (480,000 × 5 °C/55 °C). Two-body wear and fracture load were measured. Data were analyzed using global univariate ANOVA with partial eta squared, Kruskal-Wallis H, Mann-Whitney U, and Spearman’s rho test (p < 0.05). TEL showed less wear resistance than FPT (p = 0.001) for all cleaning methods tested. Concerning vertical material loss, NMF and GCT were in the same range of value (p = 0.419–0.997), except within FDPs cleaned in ISO (p = 0.021). FPT showed no impact of cleaning method on wear resistance (p = 0.219–0.692). TEL (p < 0.001) showed the highest and FPT (p < 0.001) the lowest fracture load. Regarding the cleaning methods, specimens treated with ISO showed lower fracture load than specimens cleaned with CEN (p = 0.044) or YEL (p = 0.036). The material selection and the cleaning method can have an impact on two-body wear and fracture load results. Printed restorations showed superior two-body wear resistance compared to milled FDPs but lower fracture load values. Regarding cleaning methods, ISO showed a negative effect on fracture load compared to the other methods tested.

Aim: To evaluate the effect of different oral media (artificial saliva, carbonated orange juice and coffee) on the surface roughness and color stability of 3D printed temporary material. Methodology: Twenty one discs samples were designed using additive manufacturing technique (SLA) were divided into 3 groups and stored in artificial saliva, coffee and carbonated orange juice at 37 ℃. Surface roughness was measured before and after immersion using non-contact USB Digital microscope. Color measurements were made before and after immersions using CIE L*a*b*. Data was analyzed by ANOVA followed by Tukey’s pair-wise tests. Results: After two, three and twelve weeks of immersion period there was statistically significant difference between the three tested oral media groups regarding the surface roughness. The highest ΔE values were observed in the coffee (3.58, 4.40 and 10.35 ΔE) followed by Carbonated orange juice mean values (3.29, 3.84 and 7.18 ΔE) while the lowest color changes mean values were for A. Saliva immersed group (1.91, 2.29 and 2.98 ΔE) respectively. Conclusions: 1. All different oral media (except A.saliva) used in the present study affected the surface roughness and the color stability of Interim discs constructed using 3-D printing technique. 2. Manufacturing of interim crowns using 3-D printing technique could be used for short-term provisional restoration. 3. This study has shown a strong positive correlation between color change and surface roughness of provisional restorative material.

BACKGROUND: Clinicians routinely provide provisional crowns following teeth preparation. Three-dimensional (3D) printing technology could be used over conventional methods for better fit as lack of adequate fit would result in plaque accumulation, micro-leakage, teeth sensitivity, caries and periodontal diseases. OBJECTIVE: The aim of the study was to evaluate the marginal and internal fit of provisional crowns fabricated using 3D printing technology and to compare it with that of compression molding and milling methods. METHODS: Ninety study models were fabricated by duplicating metal master models of the maxillary first premolar molar with three different finish line chamfer, rounded shoulder and rounded shoulder with bevel. On each study model, provisional crowns were fabricated using compression molding (Mo. group, n= 30 – by over impression technique), milling (Mi. group, n= 30 – by 5-axis dental milling machine), and 3D printing method (3D-P. group, n= 30 – by 3D printer). Marginal and internal fit of the samples were evaluated by measuring gap using a scanning electron microscope with a magnification of 27 ×, at 7 zones A–G on different finish line models. The data were statistically analysed using one-way analysis of variance (ANOVA) at the 0.05 significance level. The p-values were calculated using Dunnett’s test. RESULTS: The marginal gap was minimal for the 3D-P. group for each finish line with lowest for rounded shoulder with bevel at zone A 30.6 ± 5.3 and at zone G 32.8 ± 5.4. In axial area, i.e. zones B and F, the minimum gap was noticed for the Mo. group and in Occlusal area (cusp and fossa), for zones C–E maximum gap was determined in Mi. group followed by Mo. and 3D-P. groups. CONCLUSIONS: 3D printed provisional crowns have better marginal and internal fit compared to milled and molded provisional crowns.

Aims We conducted a randomized controlled trial to assess the clinical outcomes of two loading protocols involving either immediate or delayed prosthetic temporization of single implants placed at posterior, healed sites. Materials and methods Forty-nine patients in need of single implants at premolar or molar sites were randomized to receive a temporary crown either immediately after implant placement or 3 months later. Randomization was stratified by sex, implant location (premolar/molar) and arch (maxilla/mandible). Final implant screw-retained zirconia crowns with angulated screw channels were delivered at 5 months after surgery. Radiographic bone levels (primary outcome), peri-implant mucosal margin levels and peri-implant probing depths were recorded at baseline, 6 and 12 months after surgery. Results Both treatment arms showed similar patterns of soft tissue and bone re-modelling from the implant platform over 12 months [mean bone level change 1.6 mm (SD 1.0 mm) in the delayed, and 1.2 mm (SD 1.3 mm) in the immediate temporization group], with the majority of changes occurring within the first 6 months. Conclusions Immediate or delayed temporization of single implants placed at posterior healed sites resulted in largely similar 1-year outcomes with respect to peri-implant bone levels and soft tissue changes.

Introduction: A temporary construction serves as a preliminary representation of the type and appearance of a future permanent one that is tailored to the patient’s requirements. Like any prosthetic construction, it should meet the functional requirements, preserve or improve chewing and speech function. No matter how well maintained a prophylactic and functional prosthetic structure is, it will not be evaluated by the patient unless it retains and restores the existing shape, size and color of the natural teeth. Aim: To determine instrumentally to what extent different colorants change the color of temporary constructions. Materials and methods: Two materials for temporary restoration were tested – Protemp II and Protemp IV, of which a total of 100 test specimens, were fabricated. Under the equal storage conditions of room temperature and no direct access to sunlight, they were exposed to five 100-ml staining solutions: Coca-Cola, coffee (espresso), berry tea, orange juice and red wine in five separate containers. Measurements of color changes in the three areas of the tooth were performed using two spectrophotometric devices – Vita EasyShade and SpectroShade, at different time intervals – immediately before placement in the staining solution, at 1, 4, 7, and 14 days. Results: The results were analysed using the SPSS Statistical Processing Program (SPSS Inc., IBM SPSS Statistics) version 21.0. They were converted to a text file with the converter of the same program. In hypothesis testing, a standard value of p ≤ 0.05 was chosen for the level of significance that rejects the null hypothesis. Conclusion: Based on this study, the strong colouring effect of coffee and red wine on these restorations was demonstrated. We can conclude that Protemp IV material showed better color stability compared to Protemp II.

Background: Nowadays, there is an increased demand for aesthetic restorations with healthy peri-implant soft tissue. Immediate implantation and temporization could facilitate the preservation of the original soft tissue architecture. A possible scenario could be the application of PMMA (poli metil-metacrilate) temporary abutments with proper contour of the peri-implant mucosal surface.Description: A 30-year-old female patient visited the dental office. The level of the mucosa and the papilla next to the broken tooth harmonized with the adjacent teeth and soft tissues. The treatment plan aimed to preserve the current soft tissue conditions. A bone level implant (Bone Level Tapered SLActive, Straumann) was placed immediately after the extraction with xenograft bone substitute material (Botiss, Cerabone) to promote the preservation of the bone architecture. Screw-retained temporary crown in infraocclusion was immediately placed on the implant. The temporary crown was fabricated by bonding the extracted tooth to a modified PMMA temporary abutment. After the maturation of the peri-implant mucosa and complete integration of the implant, an open tray A-silicon impression was taken by a customized impression coping. The emergence profile of the tem-porary crown was replicated by pattern resin-modified impression coping. The final restoration was a cementable e.max crown seated on a customized zirconia dioxide abutment. The zirconia dioxide abutment was cemented to its titanium variobase and it resembled the shape of the temporary abutment and seated perfectly into the emergence profile.

Background Artificial intelligence (AI) is a branch of computer science concerned with building smart software or machines capable of performing tasks that typically require human intelligence. We present a protocol for the use of AI to fabricate implant-supported monolithic zirconia crowns (MZCs) cemented on customized hybrid abutments. Methods The study protocol consisted of: (1) intraoral scan of the implant position; (2) design of the individual abutment and temporary crown using computer-aided design (CAD) software; (3) milling of the zirconia abutment and the temporary polymethyl-methacrylate (PMMA) crown, with extraoral cementation of the zirconia abutment on the relative titanium bonding base, to generate an individual hybrid abutment; (4) clinical application of the hybrid abutment and the temporary PMMA crown; (5) intraoral scan of the hybrid abutment; (6) CAD of the final crown with automated margin line design using AI; (7) milling, sintering and characterisation of the final MZC; and (8) clinical application of the MZC. The outcome variables were mathematical (quality of the fabrication of the individual zirconia abutment) and clinical, such as (1) quality of the marginal adaptation, (2) of interproximal contact points and (3) of occlusal contacts, (4) chromatic integration, (5) survival and (6) success of MZCs. A careful statistical analysis was performed. Results 90 patients (35 males, 55 females; mean age 53.3 ± 13.7 years) restored with 106 implant-supported MZCs were included in the study. The follow-up varied from 6 months to 3 years. The quality of the fabrication of individual hybrid abutments revealed a mean deviation of 44 μm (± 6.3) between the original CAD design of the zirconia abutment, and the mesh of the zirconia abutment captured intraorally at the end of the provisionalization. At the delivery of the MZCs, the marginal adaptation, quality of interproximal and occlusal contacts, and aesthetic integration were excellent. The three-year cumulative survival and success of the MZCs were 99.0% and 91.3%, respectively. Conclusions AI seems to represent a reliable tool for the restoration of single implants with MZCs cemented on customised hybrid abutments via a full digital workflow. Further studies are needed to confirm these positive results.

Aim This prospective clinical study evaluated the clinical performance of managing cracked teeth with reversible pulpitis through a combination of internal and external splinting and investigated factors that can affect pulp survival after splinting. Methodology Thirty-four teeth diagnosed with cracks and reversible pulpitis were enrolled and treated with bidirectional crack splinting: 1) immediate splinting with a stainless-steel band; 2) internal splinting with crack line removal and resin filling; and 3) external splinting with a temporary crown followed by final crown setting. If the symptoms remained/recurred, root canal treatment was performed. Patients were followed up at 3, 6, and 12 months, then annually thereafter. Kaplan-Meier survival analysis to calculate the survival of the treated teeth and Cox univariate proportional hazards regression model to investigate prognostic factors were performed. Results Twenty-nine (97%) teeth were followed up for up to 4 years. The pulp survival rate was 72% after banding and 91% after final crown cementation. No tooth was extracted (100% tooth survival rate). In the univariate Cox proportional hazard test, pain on percussion was the only statistically significant factor (hazard ratio = 11.77). Teeth with pain on percussion at the first visit had a pulp survival rate of 46% during the follow-up period. In comparison, their counterparts without pain had a 94% pulp survival rate. Conclusions Bidirectional splinting successfully managed cracked teeth with reversible pulpitis. Pain on percussion (mechanical allodynia) may be an important factor in deciding whether to attempt root canal treatment on symptomatic cracked teeth. A step-by-step approach with bidirectional crack splinting should be encouraged for a cracked tooth with a vital pulp without mechanical allodynia rather than preemptive root canal treatment.

Dental polymers, commonly known as “Dental Resins” were first used in dentistry in 1839, and since then they have emerged as a favorable candidate for restorative dentistry and cosmetic and functional purposes. Many prosthesis and implants made from polymers have been in use for the last three decades and there is a continuous search for more biocompatible and stronger polymer prosthetic materials. Typical applications of polymers in dentistry are impression materials, relining materials, temporary crown materials, denture base materials, obturation materials (endodontic treatment), and filling materials (composite, cements, adhesives). The dental polymers that are to be used in the oral cavity should be harmless to all oral tissues – gingiva, mucosa, pulp, and bone. Furthermore, it should contain no toxic, leachable, or diffusible substance that can be absorbed into the circulatory system, causing systemic toxic responses, including teratogenic or carcinogenic effects. The materials should also be free of agents that could elicit sensitization or an allergic response in a sensitized patient. Rarely, unintended side effects of dental polymers may occur as a result of toxic, irritative, or allergic reactions. The most widely used polymer in prosthodontics is polymethyl-methacrylate resin (PMMA), which is used for fabrication of various dental prostheses and denture liners, temporary crowns and orthodontic appliances. The aim of the current paper is to provide an overview of the current literature on toxicology of dental polymers and to give implications for possible improvements concerning their biocompatibility.

Objectives To follow-up the radiographic bone level changes and the clinical outcomes of immediately provisionalized and conventionally restored implants with a hydrophilic surface following 5 years of function. Materials and methods This was a 5-year follow-up of a prospective, randomized, single-blind controlled study involving 16 of the 24 originally recruited patients in need of a single-tooth replacement in the esthetic area. Implants were either immediately provisionalized with a non-occluding temporary crown (test group, n = 7), or left without a crown (control group, n = 9). In both groups, the definitive restoration was placed 16 weeks after implant placement. Radiographic and clinical parameters were evaluated at 36, 48, and 60 months post-implant placement, together with implant survival and success rates. The esthetic outcomes were measured with the Papilla Fill Index (PFI) and the Pink Esthetic Score (PES). Results At 60 months, similar peri-implant bone loss was observed in the test (-0.42 mm ±0.17 mm) and in the control (-0.37 mm ±0.35 mm) groups. A tendency for an improved esthetic outcome from implant loading to the subsequent follow-ups was noticed in both groups. Both groups presented with high levels of long-term implant survival and success. Conclusions This study supports non-functional immediate provisionalization as a viable long-term option for the management of single-tooth implants in the esthetic area. However, the small sample size does not allow statistical inference at 60 months of follow-up and future adequately powered studies are warranted.

Interim restoration protects abutment teeth and periodontal tissues until prosthetic treatment ends with the fabrication of a definitive prosthesis, restores and maintains the functions and morphology of the stomatognathic system, and preserves the occlusal contact relationship. Temporary prostheses are used for a short time, to confirm the diagnosis and treatment plan. However, because interim restorations are used only briefly, their importance in ensuring the success of definitive prostheses is often overlooked. This review includes a comprehensive literature review of interim restoration of molars and summarizes the current clinical understanding and status of this functionally important area. Peer-reviewed publications were identified by searching PubMed in November 2018. An interim restoration, even when used briefly, can help dentists evaluate many aspects of the treatment plan and contribute to accurate prognosis of prosthetic treatment.

Objetivo: Este trabalho avaliou a rugosidade superficial de uma Resina Acrílica Ativada Quimicamente (RAAQ) e uma Resina Bisacrílica, em diferentes técnicas de manipulação, para confecção de coroas provisórias. Materiais e Métodos: Foram confeccionadas matrizes de silicone (20x10x3mm) que serviram para confecção dos corpos de prova, que formaram 4 grupos: Grupo Panela, Grupo Pincel, Grupo Fabricante e Grupo Bisacrílica (n=12). Após a polimerização, foram realizadas 3 leituras longitudinais e 3 transversais com auxílio de um rugosímetro digital. As leituras foram repetidas após acabamento e polimento dos corpos de prova em Politriz Metalográfica. Os valores de rugosidade foram somados e criada uma média para cada corpo de prova, os quais foram somados novamente dentro do seu grupo criando uma média e desvio padrão, antes e após o polimento. Objective To evaluate, through microcomputed tomography (μCT), the cement film thickness of veneers and crowns made with different provisional materials. Material and Methods A veneer and a crown preparation were performed on a central incisor and a second molar of a dental model, respectively, scanned with an intraoral scanner, and the .stl files were exported to an LCD‐based SLA three‐dimensional (3D)‐Printer. Twenty‐four preparations were 3D‐printed for each veneer and crown and divided into four groups (n = 6/group): (a) Acrylic resin (Acrilico Marche); (b) Bisacrylic resin (Protemp 4); (c) PMMA computer‐aided design and computer‐aided manufacturing (CAD‐CAM) (Vipiblock); and (d) 3D‐printed resin for provisional restorations (Raydent C&B for temporary crown and bridge). Veneers and crowns restorations were performed and cemented with a flowable composite. Each specimen was scanned with a μCT apparatus, files were imported for data analysis, and cement film thickness was quantitatively measured. Data were analyzed by 2‐way ANOVA and Tukey post‐hoc tests (α = .05). Results Crowns presented a thicker cementation film than veneers (P < .05).The bisacrylic resin showed the smallest veneer film thickness, similar to the acrylic resin (P = .151), which was not significantly different than the PMMA CAD/CAM material (P = .153). The 3D printed provisional material showed the thicker film, different than all other materials (P < .05). The bisacrylic resin showed a cement film thickness with a high number of voids in its surface. For crowns cementation, the 3D printed provisional material showed the thicker cementation film, different than all other materials (P < .05). Conclusions Different provisional materials present different film thicknesses. The 3D printed provisional material showed the highest veneer and crown film thicknesses. Veneers film thicknesses were smaller than crowns for all provisional materials. Clinical Significance The 3D printed provisional material studied can be satisfactorily used, presenting appropriate adaptation with the tooth preparation, however, it shows the highest cement film thickness for both veneers and crowns cementations when compared with other provisional materials. A better internal fit, or smaller cement film thickness is obtained by CAD/CAM materials, acrylic and bisacrylic resins. Veneer cementation showed a smaller cement film thickness compared with crown cementation for all provisional materials.

ANAHTAR KELIMELER: Dis asinmasi, dikey boyut, protetik tedavi ABSTRACT Prosthetic Treatment of A Patient With Vertical Dimension Loss: A Case Report Busra Tosun * , Nuran Yanikoglu * INTRODUCTION: Tooth wear is a disease characterized by the loss of hard tissues of the teeth except tooth caries.The underlying causes are also different according to the type of wear and therefore, the treatment options vary accordingly. In cases where the occlusal vertical dimension needs to be increased, some of the treatment options are extension of the crown length, surgical reposition of the alveolar bone, fixed prosthetic treatments, casting overlay prostheses.This case report presents a patient who needs to be increased in vertical dimension, using acrylic temporary crowns, adaptation to the new vertical dimension and treatment with permanent fixed restorations. CASE REPORT: A 65 year-old male patient referred to department of the prostodontics at Ataturk University, faculty of dentistry with inadequate chewing function, complaints of aesthetic appearance and worn in his teeth. The intraoral examination of the patient, revealed that upper and lower teeth were worn halfway through the crowns and the dentin was exposed on the incisal edges. Vertical dimension of rest of the patient was determined using the Niswonger method, oclusal vertical dimension was measured in the case of full closure.By adjusting the freeway space 3mm, teeth were prepared with chamfer style step burs and the vertical size was gradually increased with acrylic temporary crowns. At the end of 5 weeks, after making sure that the patient did not develop any joint pain, the patient's permanent measure was taken with additional silicone and metal supported porcelain bridges were made. CONCLUSION: In cases where the vertical dimension has to be increased, the permenant fixed prosthetic should not be immediately restorated. First, in the long term, whether the adaptation to the new vertical dimension can be achieved or not, should be followed in long term, and then the prosthetic treatment should be completed in a vertical dimension where the patient is comfortable.As in our case, permenant fixed prosthetic treatment by increasing the vertical dimension gradually with temporary crown is one of alternative treatment options. Aim To record the pulp temperature at different tooth sites during fabrication of two different temporary crown systems. Methodology Two temporary crown systems were investigated; a conventional direct fabricated and a preformed thermoplastic resin system. Extracted caries-free human teeth (incisor, premolar and molar) were prepared for full coverage ceramic restoration with roots sectioned below the cemento-enamel junction. Thermocouple wires were secured at the surface of crown material, the cut dentine and inside the pulp cavity. Provisional crowns (n = 10/group) from each system were formed prior to placement in a water bath of 37 °C to simulate pulpal temperature. Temperatures were recorded using a K-type thermocouple data logger to collect the mean and peak temperature during crown fabrication. Statistical analysis was carried out on all tested groups and heat flow was calculated. Results For direct fabricated crowns, the mean rise in pulpal temperature recorded was 0.1 °C with the mean temperature range of 37.3 °C–37.8 °C. For the preformed thermoplastic crowns, the mean rise in pulpal temperature recorded was 37.3 °C–45.1 °C. The increase in temperature was significantly higher (6.5 °C for the incisor group, 7.5 °C for the premolar group, and 6.7 °C for the molar group). For both crown systems, the temperature difference between the three different sites; pulp, crown and tooth surface showed a statistical difference (P < 0.01). A search of the medical and dental records at Evidensia Lørenskog Dyreklinikk, in Lørenskog, Norway, was conducted to identify dogs that received temporary crown extensions (TCEs) to correct linguoverted mandibular canine (LMC) teeth over a 54-month investigation period (2012-2016). Criteria for inclusion into the study were the presence of complete medical and dental records, pre- and postoperative clinical photographs and intraoral radiographs of the affected canine teeth, adequate information pertaining to the procedures performed, and at least 1 follow-up >3 months after appliance removal. Seventy-two dogs with LMC teeth were treated with TCE. Thirty-nine breeds were represented in this study. Mean age at the time of appliance installation was 6.4 (range, 4.7-13.4 months [median, 5.9 months] months). Fifty-three (73.6%) dogs presented with class I malocclusion, 14 (19.5%) dogs with class II malocclusion, and 5 (6.9%) dogs with class III malocclusion. Twenty-five (34.7%) dogs were considered to have mild, 32 (44.4%) dogs to have moderate, and 15 (20.8%) dogs to have severe mandibular canine malocclusion. The TCE was combined with other treatment modalities (active orthodontics, extraction of nonstrategic teeth, gingivectomy, and inclined bite plane and ball therapy) to correct mandibular canine tooth malocclusions in 19 (26.4%) dogs. Fractured or detached crown extensions were seen in 9 (12.5%) dogs. Soft tissue ulceration or inflammation was seen in 7 (9.7%) dogs. The mandibular canine teeth occlusion resolved completely with self-retaining, functional, nontraumatic occlusion in 56 (77.8%) dogs. Fifteen dogs (20.8%) resolved with functional, nontraumatic occlusion, but the mandibular canine teeth were too short to be perfectly self-retained, thus left with 1- to 2-mm crown extensions for permanent retention. In 1 (1.4%) dog, both mandibular canine teeth relapsed almost back to original position. The results show that TCE is a viable treatment option to correct LMC teeth in young dogs.

Traditionally, the fabrication of temporary teeth restorations must go through multiple processes such as moulding, curing and post finishing, which requires extensive expertise of dentists. In addition, the handmade temporary restorations are usually unable to precisely fit the patient’s teeth due to limited formability of material. To address the problem, a three-dimensional (3D) printing technology named temperature controlled mask image projection based stereolithography (TCMIP-SL) is presented for dental materials in this paper, with aim to build customized temporary crown and bridge quickly for its use in dental offices. We first studied the photo-polymerization performance of commercial materials that are commonly used in dental industry. Then we discussed the temperature effect on curing performance and rheology of dental composite material. Based on the studies, we further developed our TCMIP-SL process by integrating a material coating system with controllable heating, so that the coated thin film of dental composite material can be selectively cured using high resolution patterned light beam. Several test cases are performed to demonstrate the TCMIP-SL process can 3D print high viscous temporary crown material with fast speed and high resolution.

Sjögren's syndrome (SS), an autoimmune disorder, affects the exocrine glands, including the lacrimal and salivary glands. It is characterized by symptoms of dry eye and dry mouth. As secretion of saliva decreases, patients with SS experience rampant caries, pain in the oral mucosa, inflammation and hardening of the salivary glands, abnormal taste, dysphagia, and loss of teeth earlier than healthy individuals. A removable partial denture is often used as a prosthesis after tooth loss. Compromised salivary lubrication, however, can produce traumatic ulceration of the mucosa, making use of a removable prosthesis in SS patients painful. In such cases, a dental implant is likely to be requested as an alternative. This report describes dental implant treatment in an SS patient, a 50-year-old woman who presented with the chief complaint of masticatory dysfunction and pain due to a removable partial denture. Eight implants were placed in the maxillary and mandibular first molar tooth and second molar tooth regions. Following a 4-month non-loading period, second-stage surgery and provisional restoration with a screwretained implant temporary crown were performed. Screw-retained superstructures were fitted by means of a customized titanium abutment and zirconia crown as the final restoration. No complications, including inflammation of peri-implant soft tissue or resorption of peri-implant bone, were observed at 3 years following placement of the superstructures.

Objective: Many techniques have been developed to enhance the gingival thickness, gingival level and emergence profile around the implant in the esthetic zone. Introduction: In this study, a buccal rotational flap was used to improve the implant site in the esthetic zone and increase gingival tissue thickness. Methods: Two cases involved the use of a rotational flap during second-stage implant surgery, one case involved the use of a temporary crown with a healing abutment, and another case involved the use of a healing abutment. Result: The cases were followed up until the final crown was placed. The implant site was improved in 2 cases; the gingival thickness increased, the gingival level was enhanced and the emergence profile was developed. Conclusion: Many factors affect the results of a rotational flap; some factors are surgical, while others are prosthetic, biological and anatomical.

Background Immediate loading of single implants is generally considered a reliable procedure. Purpose The objective of the present prospective randomized clinical study was to compare the overall treatment outcome following immediate loading (IL) and delayed loading (DL) of single implants after 1 year of follow-up. Materials and Methods Patients with a missing maxillary tooth (15-25) were randomly assigned to IL or DL. The protocol included implant installation in healed sites, immediate loading, delayed loading, temporary screw-retained restoration, and replacement with a permanent single implant crown. Outcome measures were implant survival, marginal bone level, soft tissue changes, papillae index, pink, and white esthetic score (PES and WES), patient judged aesthetics, and oral health impact profile (OHiP-14). Results Implant survival rate was 100% and 96% for IL and DL, respectively. Implant success rate was 96% and 88% for IL and DL, respectively. Statistically significant lower papilla index scores were found in the IL group at temporary crown and definitive crown placement. An overall statistically significant improvement after 12 months for PES, WES and OHIP-14 was found. Conclusion This prospective randomized study showed that single implants in the maxilla can present satisfactory results with respect to either immediate loading or delayed loading after 12 months.

Background: Temporary crown and bridge materials have to fulfill a couple of important functions within the timeframe between tooth preparations until luting of the definitive restoration. The aim of the current study was to evaluate the color stability and fracture resistance of two fiber‑reinforced provisional fixed partial denture (FPD) materials. Materials and Methods: In this in vitro study Using a plexiglass mold, 96 bar‑shaped specimens (4 mm × 2 mm × 20 mm) were fabricated and divided into four groups (n = 24): nonreinforced composite (NRC) resin, glass fiber‑reinforced composite resin (RC), nonreinforced polymethylmethacrylate (PMMA), and glass fiber‑reinforced PMMA. Values of CIEL*a*b* were recorded for all the samples. Then, the samples were immersed in coffee, chlorhexidine mouthrinse, and distilled water. After 1 day and 1 and 4 weeks, CIEL*a*b* values were recorded again and color differences (ΔE) were calculated. All the specimens immersed in distilled water were then subjected to force to measure their fracture resistance. Data were analyzed with one‑way ANOVA, honestly significant difference Tukey tests, and paired t‑test (α = 0.05). Results: The NRC group, immersed in coffee for 1 month, exhibited the highest ΔE (17.1 ± 0.69) and the lowest ΔE belonged to the RC group immersed in water for 1 day. The RC group, immersed in water, exhibited the highest fracture resistance. Conclusion: Coffee is considered as one of the most important factors affecting color changes in provisional FPDs, either in composite resins or in PMMAs. Fracture resistance of both composite resin and PMMA FPDs revealed no significant differences between the groups; however, there were significant differences between the nonreinforced and fiber‑reinforced FPDs in both groups. Purpose: To present a novel, full-digital protocol for the design and fabrication of implant-supported monolithic translucent zirconia crowns cemented on customized hybrid abutments. Methods: The present retrospective clinical study was based on data from patients who had been treated with single Morse-taper connection implants (Exacone&reg, Leone Implants, Florence, Italy) and were prosthetically restored with monolithic translucent zirconia crowns, cemented on customized hybrid abutments. The full-digital protocol (SCAN-PLAN-MAKE-DONE&reg, ) consisted of 8 phases: (1) intraoral scan of the implant position with scanbody, (2) computer-assisted design (CAD) of the individual abutment (saved as &ldquo, supplementary abutment design&rdquo, in external folder) and temporary crown, (3) milling of the individual zirconia abutment and of the temporary polymethyl-methacrylate (PMMA) crown, with extraoral cementation of the zirconia abutment on the relative titanium bonding base, to generate an individual hybrid abutment, (4) clinical application of the individual hybrid abutment and cementation of the temporary PMMA crown, (5) two months later, intraoral scan of the individual hybrid abutment in position, (6) CAD of the final crown with margin line design on the previously saved &ldquo, superimposed on the second scan of the abutment in position, (7) milling of the final crown in monolithic translucent zirconia, sintering, and characterization, and (8) clinical application of the final crown. All patients were followed for a period of 1 year. The primary outcomes of this study were the marginal adaptation of the final crown (checked clinically and radiographically), the quality of occlusal and interproximal contact points at delivery, and the aesthetic integration, the secondary outcomes were the 1-year survival and success of the implant-supported restoration. An implant-supported restoration was considered successful in the absence of any biological or prosthetic complication, during the entire follow-up period. Results: In total, 25 patients (12 males, 13 females, 26&ndash, 74 years of age, mean age 51.1 &plusmn, 13.3 years) who had been restored with 40 implant-supported monolithic translucent zirconia crowns were included in this study. At delivery, the marginal adaptation was perfect for all crowns. However, there were occlusal issues (2/40 crowns: 5%), interproximal issues (1/40 crowns: 2.5%), and aesthetic issues (1/40 crowns: 2.5%). The overall incidence of issues at delivery was therefore 10% (4/40 crowns). At 1 year, one implant failed, thus the survival of the restorations was 97.5% (39/40 crowns in function). Among the surviving implant-supported restorations, three experienced complications (one loss of connection between the hybrid abutment and the implant, one decementation of the zirconia abutment, and one decementation of the zirconia crown). The success of restorations amounted to 92.4%. Conclusions: The restoration of single Morse-taper connection implants with monolithic translucent zirconia crowns cemented on customized hybrid abutments via the novel SCAN-PLAN-MAKE-DONE&reg, full-digital protocol seems to represent a reliable treatment option. However, further studies on a larger number of patients and dealing with different prosthetic restorations (such as implant-supported fixed partial prostheses) are needed to confirm the validity of this protocol.

Background. To date, insertion torque value (ITV) and implant stability quotient (ISQ) obtained by the Osstell instrument are common clinical methods to assess the initial stability of an implant for a predictable loading procedure. The aim of this current study is to evaluate the ITV and ISQ as stability parameters as part of the decision-making protocol in the adoption of immediate loading in fresh extraction sockets. Materials and Methods. A total of 41 tapered implants were allocated into two groups: the test group (n = 11; 3 males and 8 females; mean age: 62.8 ± 10.7) which received 18 implants as type 1 fresh extraction sockets after teeth removal and the control group (n = 7; 4 males and 3 females; mean age: 65.4 ± 9.7) which received 23 implants placed in healed sockets for a period of at least 3 months. Both the ITV and ISQ data were recorded at the time of insertion (t0). Since ITV (test group) and ITV/ISQ (control group) values were useful for the immediate loading protocol, a screw-retained temporary crown was immediately loaded. ISQ values were recorded after a healing period of 4 months (t1). Results. ITV mean values at t0 in test and control groups were, respectively, 48.61 ± 15.39 and 70.47 ± 14.71, whereas ISQ mean values were 57.55 ± 1.93 and 72.86 ± 5.25, respectively, showing a statistically significant difference (p value t1 in either the test or the control group were 68.68 ± 4.20 and 74.54 ± 4.17, not showing a statistical difference. The implant survival rate was 100% in both groups, and no surgical and prosthetic complications were reported during the study. Conclusion. In conclusion, this study remarked the presence of a residual gap that influenced the ISQ during implant insertion in fresh extraction sockets making this parameter not sufficient for a conclusive decision in the immediate loading, whereas the ITV alone showed to be the best parameter for a final substantial decision.
