Abstract

Aims: The heterozygous phospholamban (PLN) mutation R14del (PLN R14del+/−) is associated with a severe arrhythmogenic cardiomyopathy (ACM) developing in the adult. “Superinhibition” of SERCA2a by PLN R14del is widely assumed to underlie the pathogenesis, but alternative mechanisms such abnormal energy metabolism have also been reported. This work aims to (1) to evaluate Ca2+ dynamics and energy metabolism in a transgenic (TG) mouse model of the mutation prior to cardiomyopathy development; (2) to test whether they are causally connected. Methods: Ca2+ dynamics, energy metabolism parameters, reporters of mitochondrial integrity, energy, and redox homeostasis were measured in ventricular myocytes of 8–12 weeks-old, phenotypically silent, TG mice. Mutation effects were compared to pharmacological PLN antagonism and analyzed during modulation of sarcoplasmic reticulum (SR) and cytosolic Ca2+ compartments. Transcripts and proteins of relevant signaling pathways were evaluated. Results: The mutation was characterized by hyperdynamic Ca2+ handling, compatible with a loss of SERCA2a inhibition by PLN. All components of energy metabolism were depressed; myocyte energy charge was preserved under quiescence but reduced during stimulation. Cytosolic Ca2+ buffering or SERCA2a blockade reduced O2 consumption with larger effect in the mutant. Signaling changes suggest cellular adaptation to perturbed Ca2+ dynamics and response to stress. Conclusions: (1) PLN R14del+/− loses its ability to inhibit SERCA2a, which argues against SERCA2a superinhibition as a pathogenetic mechanism; (2) depressed energy metabolism, its enhanced dependency on Ca2+ and activation of signaling responses point to an early involvement of metabolic stress in the pathogenesis of this ACM model.

Abstract

The ARAPUCA is a novel concept for liquid argon scintillation light detection which has been proposed for the photon detection system of the Deep Underground Neutrino Experiment. The test in liquid argon of one of the first ARAPUCA prototypes is presented in this work, where the working principle is experimentally demonstrated. The prototype has an acceptance window of 9 cm$^2$ and is read-out by a single SiPM with active area of 0.36 cm$^2$. Its global detection efficiency was estimated by exposing it to a $^{238}U$ $\alpha$ source and to cosmic rays and was found to be 1.15\% $\pm$ 0.15\%, in good agreement with the prediction of a detailed Monte Carlo simulation of the device. Several other ARAPUCA prototypes of bigger dimensions and read-out by arrays of SiPMs have been built and are actually under test. In particular 32 ARAPUCA cells have been installed inside the protoDUNE detector, which is being assembled at CERN and will be operated in the second half of 2018.

Abstract

Objective: Data on ANCA-associated vasculitis (AAV) induced by anti-thyroid drugs (ATD) are scarce. We aimed to describe the characteristics and outcome of these patients in comparison to primary AAV. Methods: We performed a retrospective multicentre study including patients with ATD-induced AAV. We focused on ATD-induced microscopic polyangiitis (MPA) and compared them with primary MPA by matching each case with four controls by gender and year of diagnosis. Results: Forty-five patients with ATD-induced AAV of whom 24 MPA were included. ANCA were positive in 44 patients (98%), including myeloperoxidase (MPO)-ANCA in 21 (47%), proteinase 3 (PR3)-ANCA in six (13%), and double positive MPO- and PR3-ANCA in 15 (33%). Main clinical manifestations were skin involvement (64%), arthralgia (51%) and glomerulonephritis (20%). ATD was discontinued in 98% of cases, allowing vasculitis remission in seven (16%). All the remaining patients achieved remission after glucocorticoids, in combination with rituximab in 11 (30%) or cyclophosphamide in four (11%). ATD were reintroduced in seven cases (16%) without any subsequent relapse. Compared with 96 matched primary MPA, ATD-induced MPA were younger at diagnosis (48 vs 65 years, P < 0.001), had more frequent cutaneous involvement (54 vs 25%, P 1⁄4 0.007), but less frequent kidney (38 vs 73%, P 1⁄4 0.02), and a lower risk of relapse (adjusted HR 0.07; 95% CI 0.01, 0.65, P 1⁄4 0.019). Conclusion: ATD-induced AAV were mainly MPA with MPO-ANCA, but double MPO- and PR3-ANCA positivity was frequent. The most common manifestations were skin and musculoskeletal manifestations. ATD-induced MPA were less severe and showed a lower risk of relapse than primary MPA.

Albeit all components of energy metabolism were depressed at rest, functional signs of mitochondrial damage or energy starvation were absent and cell energy charge was preserved. The response of mitochondrial O2 consumption to SERCA2a blockade was lost in mutant myocytes (SR-mitochondrial uncoupling) and ER-stress signalling was activated. Conclusions: 1) PLN R14del+/-loses its ability to inhibit SERCA2a, which argues against SERCA2a superinhibition as a mechanism of ACM; 2) depression of resting energy metabolism may at least partly reflect impairment of SR-mitochondrial coupling; 3) ER-stress may be an early factor in the pathogenesis.

16. A PROMOÇÃO DA SAÚDE ÚNICA: PREVENÇÃO DE ZOONOSES NO MUNICÍPIO DE NOVA IGUAÇÚ – RJ

Abstract

A Pandemia de COVID-19 trouxe para o centro das pautas temas relacionados à saúde e ao ambiente, entre outros, especialmente por ser uma doença derivada de uma zoonose. As zoonoses são infecções transmitidas entre animais e humanos que podem ter sua propagação facilitada pelo manejo sanitário precário, falta de controle de vetores, baixa adesão às campanhas de vacinação, falta de vermifugação dos animais, manutenção de animais silvestres em cativeiro irregularmente ou de sua inclusão na dieta. Sua prevenção visa o bem-estar entre seres humanos, animais e ambiente, proposto pela Saúde Única e pode ser alcançada por meio da promoção de debates e reflexões sobre o tema. Esta pesquisa visa reconhecer a adesão às campanhas de vacinação, o conhecimento e promover a conscientização da população sobre profilaxia e controle de zoonoses. O percurso metodológico contou com questionário semiestruturado aplicado entre julho e agosto de 2022, no município de Nova Iguaçu. Os resultados foram analisados pelo teste do qui-quadrado e indicam o desconhecimento da população sobre zoonoses, que, embora existam campanhas públicas de vacinação, estas não alcançam 100% da população de animais domésticos na região. Existe a necessidade de mobilização social sobre o tema por meio de ações em espaços formais e não formais de educação através da cooperação entre intuições de ensino de diferentes níveis e o poder público. Estas ações visam orientar os tutores de animais domésticos sobre o manejo sanitário de acordo com as espécies; o uso de equipamentos de segurança e proteção quando necessário; o controle de vetores de zoonoses; os programas de vacinação e vermifugação dos animais; e a necessidade de notificação às autoridades sanitárias de casos específicos de contaminação. A ampliação do conhecimento da população sobre zoonoses tem como objetivo a redução de sua propagação e a promoção do bem-estar humano, animal e ambiental.

Abstract

Objectives To assess the sensitivity to change of ultrasound halo features and their association with disease activity and glucocorticoid (GC) treatment in patients with newly diagnosed giant cell arteritis (GCA). Methods Prospective study of patients with ultrasound-confirmed GCA who underwent serial ultrasound assessments of the temporal artery (TA) and axillary artery (AX) at fixed time points. The number of segments with halo and maximum halo intima-media thickness (IMT) was recorded. Time points in which >80% of patients were assessed were considered for analysis. Halo features at disease presentation and first relapse were compared. Results 49 patients were assessed at 354 visits. Halo sensitivity to change was assessed at weeks 1, 3, 6, 12 and 24 and showed a significant standardised mean difference between all time points and baseline for the TA halo features but only after week 6 for the AX halo features. The number of TA segments with halo and sum and maximum TA halo IMT showed a significant correlation with erythrocyte sedimentation rate (0.41, 0.44 and 0.48), C reactive protein (0.34, 0.39 and 0.41), Birmingham Vasculitis Activity Score (0.29, 0.36 and 0.35) and GC cumulative dose (-0.34,-0.37 and-0.32); no significant correlation was found for the AX halo features. Halo sign was present in 94% of first disease relapses but with a lower mean number of segments with halo and sum of halo IMT compared with disease onset (2.93±1.59 mm vs 4.85±1.51 mm, p=0.0012; 2.01±1.13 mm vs 4.49±1.95 mm, p=0.0012). Conclusions Ultrasound is a useful imaging tool to assess disease activity and response to treatment in patients with GCA.

Abstract

Cattle are animals that stay in the herd for a large period of the day in the open air pasture. This daily routine leaves them exposed and susceptible to infectious diseases. One of them is the Bovine Parasitic Sadness which is considered one of the greatest cattle health problems due its high rate of mortality and morbidity. In this work preliminary results are presented from calf inoculated with anaplasmosis. Blood samples were collected and analyzed through Fourier Transform Near Infrared spectroscopy (FT-NIR). The animal was monitored for 60 days until its recovery. This exploratory work shows the potentiality of the FT-NIR in the detection, follow-up and recovery of the anaplasmosis. The possibility of prevention of this disease in the herd via a fast optical analysis may bring a great breakthrough in disease control in dairy and beef cattle. Abstract

OBJECTIVES: Only a third of patients with eosinophilic granulomatosis with polyangiitis (EGPA) are ANCA-positive, mainly directed against MPO. ANCA directed against PR3 are rarely found in EGPA. We aimed to examine the significance of PR3-ANCA in EGPA. METHODS: We set up a retrospective European multicentre cohort including 845 patients. Baseline characteristics and outcomes were analysed and compared according to ANCA status. RESULTS: ANCA status was available for 734 patients: 508 (69.2%) ANCA-negative, 210 (28.6%) MPO-ANCA and 16 (2.2%) PR3-ANCA. At baseline, PR3-ANCA patients, compared with those with MPO-ANCA and ANCA-negative, less frequently had active asthma (69% vs 91% and 93%, P = 0.003, respectively) and peripheral neuropathy (31% vs 71% and 47%, P < 0.0001), more frequently had cutaneous manifestations (63% vs 38% and 34%, P = 0.03) and pulmonary nodules (25% vs 10% and 8%, P = 0.046), and lower median eosinophil count (1450 vs 5400 and 3224/mm3, P < 0.0001). Vasculitis relapse-free survival was shorter for PR3-ANCA (hazard ratio 6.05, P = 0.005) and MPO-ANCA patients (hazard ratio 1.88, P = 0.0002) compared with ANCA-negative patients. CONCLUSION: PR3-ANCA EGPA patients differ from those with MPO-ANCA and negative ANCA, and share clinical features with granulomatosis with polyangiitis. This suggests that PR3-ANCA EGPA could be a particular form of PR3-ANCA-associated vasculitis.

Abstract

Objective: Eosinophilic granulomatosis with polyangiitis (EGPA) is a systemic small-vessel vasculitis characterized by asthma, hypereosinophilia and ANCA positivity in 40% of patients. Renal involvement is rare and poorly described, leading to this renal biopsy-proven based study in a large EGPA cohort. Methods: We conducted a retrospective multicentre study including patients fulfilling the 1990 ACR criteria and/or the 2012 revised Chapel Hill Consensus Conference criteria for EGPA and/or the modified criteria of the MIRRA trial, with biopsy-proven nephropathy. Results: Sixty-three patients [27 women, median age 60 years (18-83)] were included. Renal disease was present at vasculitis diagnosis in 54 patients (86%). ANCA were positive in 53 cases (84%) with anti-MPO specificity in 44 (83%). All patients had late-onset asthma. Peripheral neuropathy was present in 29 cases (46%), alveolar haemorrhage in 10 (16%). The most common renal presentation was acute renal failure (75%). Renal biopsy revealed pauci-immune necrotizing GN in 49 cases (78%). Membranous nephropathy (10%) and membranoproliferative GN (3%) were mostly observed in ANCA-negative patients. Pure acute interstitial nephritis was found in six cases (10%); important interstitial inflammation was observed in 28 (44%). All patients received steroids with adjunctive immunosuppression in 54 cases (86%). After a median follow-up of 51 months (1-296), 58 patients (92%) were alive, nine (14%) were on chronic dialysis and two (3%) had undergone kidney transplantation. Conclusion: Necrotizing pauci-immune GN is the most common renal presentation in ANCA-positive EGPA. ANCA-negative patients had frequent atypical renal presentation with other glomerulopathies such as membranous nephropathy. An important eosinophilic interstitial infiltration was observed in almost 50% of cases.
