Diseases of the respiratory system ,RC705-779 - Abstract
This second of two parts continues with the development of a framework for understanding air quality issues and their relationship to human health. Recognized health effects associated with air pollution are described and current controversies regarding ozone and PM10 are briefly outlined. Epidemiological methods of investigating air quality effects are discussed, comparing recent landmark studies in Canada. Comparative prevalence studies do not reflect the state of the art in air pollution epidemiology but are frequently cited and conducted in Canada as if they were definitive. The implications of setting air quality standards and objectives on this basis or to meet arbitrary levels of risk of health effects are examined. The current state of the art does not support risk-based air quality standards. A policy of continuous improvement is most protective of both human health and the environment.

Diseases of the respiratory system ,RC705-779 - Abstract
There is a fundamental reevaluation of the association between air quality and human health taking place. This reevaluation is motivated by several recent developments: increasing interest in air quality as an environmental issue; interest in the unanswered questions regarding the epidemiology of asthma; and the reduced prevalence of the principal hazard to respiratory health, cigarette smoking, the control of which invites interest in second-order determinants of health. This article attempts to provide a framework for understanding air quality issues that pertain to human health. The objective is to provide the specialist in respiratory medicine with an overview that will assist in educating patients and in responding to their inquiries, and to equip the physician to respond to requests for assistance or interpretation when called upon to comment on public policy issues involving air pollution. The implications of setting air quality standards or objectives to meet arbitrary levels of risk of health effects are examined. The current state of the art does not support risk-based air quality standards. A policy of continuous improvement is most protective of both human health and the environment.

8. Trends in Mortality from Chronic Obstructive Pulmonary Disease in Alberta: Back to the Future?

Diseases of the respiratory system ,RC705-779 - Abstract
Trends in mortality from chronic obstructive pulmonary disease (COPD) in Alberta over 60 years, from 1927 to 1987, for ages 15 and above or both sexes, were examined. There was a striking decline in mortality among older adults in the 1930s and 1940s. a nadir that lasted almost 10 years in the 1950s, and a striking increase thereafter. By 1970, most age groups had returned to levels of the 1930s. This overall trend was observed in both the younger age groups (aged 15 to 50) and older adults, although mortality from COPD in the former disproportionately reflected asthma-related deaths. Subsequently, mortality climbed still higher in older age groups, but not in the younger age groups. The sustained rise in mortality in older age groups after the Second World War is presumably related to smoking habits. Historical trends in Alberta were then compared with Canada as a whole for both sexes over 50 years of age. Although Alberta had a much lower mortality from COPD than Canada as a whole, this difference disappeared by 1980. There is no obvious explanation that would explain all of the observed trends, but they appear more likely to be a consequence of social and environmental conditions, including changes in health-related behaviour, than of major changes in medical management at the time.

The American Thoracic Society (ATS), in collaboration with George Mason University, surveyed international members of the society to assess perceptions, clinical experiences, and preferred policy responses related to global climate change. A recruitment email was sent by the ATS President in October 2015 to 5,013 international members. Subsequently, four reminder emails were sent to nonrespondents. Responses were received from 489 members in 68 countries; the response rate was 9.8%. Half of respondents reported working in countries in Asia (25%) or Europe (25%), with the remainder in South America (18%), North America (Canada and Mexico) (18%), Australia or New Zealand (9%), and Africa (6%). Survey estimate confidence intervals were ± 5% or smaller. A high percentage of international ATS survey respondents judged that climate change is happening (96%), that it is driven by human activity (70%), and that it is relevant to patient care ("a great deal"/"a moderate amount") (80%). A majority of respondents also indicated they are already observing health impacts of climate change among their patients; most commonly as increases in chronic disease severity from air pollution (88%), allergic symptoms from exposure to plants or mold (72%), and severe weather injuries (69%). An even larger majority anticipated seeing these climate-related health impacts in the next two decades. Respondents further indicated that physicians and physician organizations should play an active role in educating patients, the public, and policy makers on the human health effects of climate change. International ATS respondents, like their counterparts in the U.S., observed that human health is already adversely affected by climate change, and support responses to address this situation.

The American Thoracic Society (ATS), in collaboration with George Mason University, surveyed a random sample of ATS members to assess their perceptions of, clinical experiences with, and preferred policy responses to climate change. An e-mail containing an invitation from the ATS President and a link to an online survey was sent to 5,500 randomly selected U.S. members; up to four reminder e-mails were sent to nonrespondents. Responses were received from members in 49 states and the District of Columbia (n = 915); the response rate was 17%. Geographic distribution of respondents mirrored that of the sample. Survey estimates' confidence intervals were ±3.5% or smaller. Results indicate that a large majority of ATS members have concluded that climate change is happening (89%), that it is driven by human activity (68%), and that it is relevant to patient care ("a great deal"/"a moderate amount") (65%). A majority of respondents indicated they were already observing health impacts of climate change among their patients, most commonly as increases in chronic disease severity from air pollution (77%), allergic symptoms from exposure to plants or mold (58%), and severe weather injuries (57%). A larger majority anticipated seeing these climate-related health impacts in the next 2 decades. Respondents indicated that physicians and physician organizations should play an active role in educating patients, the public, and policy makers on the human health effects of climate change. Overall, ATS members are observing that human health is already adversely affected by climate change and support responses to address this situation.

From 1943 to 1979, miners and factory workers in more than two hundred work sites globally were subjected to mandatory medical treatments by their employers as an unproven, and ultimately ineffective, treatment to prevent the lung disease silicosis. The treatments involved inhaling finely ground aluminum dust known as McIntyre Powder, blown into miners’ change rooms each shift using compressed air systems. Tens of thousands of industrial laborers were exposed to McIntyre Powder, yet their story is scarcely known, and the possible health impacts of their aluminum treatments were rarely studied. This paper integrates the history of the aluminum prophylaxis program and its control by the northern Ontario mining industry with the lived experience of one of the affected miners, whose daughter created a voluntary registry which documents health issues in exposed miners, and stimulated research that found a link to her father’s Parkinson disease.

Geography, Planning and Development ,General Environmental Science - Abstract
The concept of an observatory for examining and monitoring trends for research in the social sciences and health arose from analogy with the facilities of astronomy and the realization that close and consistent observation was essential to understand interactions among determinants of health in a community. Their theoretical foundations are largely grounded in the "population health model," an intellectual framework developed in 1974 and associated with a movement called "the new public health." We developed an observatory for an affluent, unexceptional, suburban jurisdiction ("County 20") in the province of Alberta, in western Canada, and used it to study and monitor both normative and emergent issues in environmental health. The observatory was conceived as a utility, or multi-purpose vehicle, for describing and responding to emerging community issues quickly and gathering data and qualitative findings as required. Case studies on issues of local importance allowed us to observe the dynamics of community response comprehensively, using quantitative and qualitative methods as appropriate. Valuable insights gleaned from the cases studied pertained to risk perception, the NIMBY syndrome, and reorganizing and auditing public health services. It did less well when the object of study was further removed from the community of interest, when there was no clear action item to be recommended, and when sponsors had expectations for certainty that could not be supported by available data sources. The observatory eventually was merged into a community health outreach program. The local observatory model is a practical, low-cost, and sustainable model for capturing generalizable and specifically local case study experience on a small scale but is limited in its effectiveness and by the scope of its coverage. It can be an initial step toward longitudinal and community monitoring studies on a larger scale. The observatory model can be used to elevate the level of case studies, impose rigor on analysis, and compile case studies for comparative and analytical studies.

Students in environmental studies, sciences, and sustainability need some degree of health literacy to pursue their own interests because health issues have played a disproportionate role in shaping environmental history, regulation, and interventions. Those entering careers in practice, advocacy, and deep study of environmental issues need further depth to have the capacity to evaluate the plausibility and context of health claims, to advocate responsibly, and to address health disparities. How much education is required for a minimum foundation and in which areas of content? A workshop held at the 2018 Annual Meeting of the Association of Environmental Studies and Sciences focused on minimum health content required for undergraduate and master’s-level graduate students to deal with issues involving health. The goal was a set of recommendations or guidelines for cognitive frameworks and fund of knowledge related to human health that may be considered fundamental in education of these students. Four sets of findings were formulated, addressing four key dimensions: (1) Participants agreed that the health content of current curricula in environmental studies, sciences, and sustainability is sparse. (2) The purpose of introducing health into education in these fields is to reflect the influence or significance of health problems on or arising from environmental justice, regulation and ecosystem management, or sustainability practice, respectively, and to provide the student with a context for understanding health issues. (3) A high-level working knowledge of the essentials of toxicology, epidemiology, risk science, planetary health, and related fields was prioritized, but mastery was deemed unnecessary for education at this level. (4) Teaching human and animal health is facilitated by analogy or reference to instruction already incorporated in the curriculum. For example, toxicology can be taught by analogy to ecotoxicology, because the same principles apply, and basic concepts of epidemiology build on population studies. Integrative case studies were suggested as a teaching tool, and the One Health (human and veterinary health) paradigm was recommended as a natural bridge.
