PURPOSE The purpose of this study was to examine racial differences in patient portal activation and research participation among patients with prostate cancer. MATERIALS AND METHODS Participants were African American and White patients with prostate cancer who were treated with radical prostatectomy (n = 218). Patient portal activation was determined using electronic health records, and research participation was measured based on completion of a social determinants survey. RESULTS Thirty-one percent of patients completed the social determinants survey and enrolled in the study and 66% activated a patient portal. The likelihood of enrolling in the study was reduced with greater levels of social deprivation (odds ratio [OR], 0.70; 95% CI, 0.50 to 0.98; P = .04). Social deprivation also had a signification independent association with patient portal activation along with racial background. African American patients (OR, 0.48; 95% CI, 0.23 to 0.91; P = .02) and those with greater social deprivation (OR, 0.58; 95% CI, 0.42 to 0.82; P = .002) had a lower likelihood of activating a patient portal compared with White patients and those with lower social deprivation. CONCLUSION Although the majority of patients with prostate cancer activated their patient portal, rates of patient portal activation were lower among African American patients and those who lived in areas with greater social deprivation. Greater efforts are needed to promote patient portal activation among African American patients with prostate cancer and address access to health information technology among those who live in socially disadvantaged geographic areas.

Background There is a need for objective biomarkers of sports-related concussion that are unaffected by physical and cognitive exertion. Electroencephalography-based biomarkers such as steady-state visually evoked potentials (SSVEPs) have been proposed as one such biomarker. The aim of this study was to investigate the effects of cognitive and physical exertion on SSVEP signal-to-noise ratio (SNR). Methods This study involved two experiments. The first experiment was performed in a controlled laboratory environment and involved a treadmill run designed to induce physical fatigue and a Stroop task designed to induce mental fatigue, completed in a randomized order on two separate visits. SSVEPs were evoked using a 15-Hz strobe using a Nurochek headset before and after each task. Changes in the 15-Hz SSVEP SNR and self-reported fatigue (visual analog scales) were assessed. In the second experiment, SSVEP SNR was measured before and after real-world boxing matches. Paired t-tests compared pre- and post-task SSVEP SNR and fatigue scores. Results Eighteen participants were recruited for experiment 1. Following the treadmill run, participants reported higher physical fatigue, mental fatigue, and overall fatigue ( p ≤ 0.005; d ≥ 0.90). Following the Stroop task, participants reported higher mental fatigue and overall fatigue ( p Conclusions The results of both experiments demonstrate that SSVEP SNR scores were not different following the treadmill run, Stroop task or amateur boxing match. These findings provide preliminary evidence that SSVEP fidelity may not be significantly affected by physical and cognitive exertion paradigms.

Highlights • En bloc resection with centralization is a viable approach in low-resource areas. • Multidisciplinary surgical approaches are necessary in low-resource settings. • Loss of motion can be compensated with proper rehabilitation measures., Introduction Giant cell tumor of bone (GCTB) is a benign-aggressive tumor that has a high-rate of recurrence with curettage resection alone. Patients with GCTB in underserved regions of the world can have progression of the tumor with significant disability due to a lack of specialty care. We present a case of an en bloc resection of an aggressive, recurrent GCTB of the radius with excellent function and no evidence of tumor recurrence two years after surgery. Presentation of case A 22-year-old right-hand dominant female in Haiti developed an aggressive recurrence of a giant cell tumor of bone (GCTB) of the distal radius. Treatment consisted of en bloc resection of the distal radius with the proximal row of the carpus and centralization of the ulna. At two-year follow-up, the patient maintained good functional capacity with no clinical or radiological evidence of recurrence. Discussion GCTB can cause significant destruction of the bone and articular surface if not treated adequately. Treatment options should be considered carefully in underserved regions of the world based on the resources available. This case exemplifies that complex limb-salvage surgery is possible when coordination of care between international and local surgeons is provided with an emphasis on continuity of care post-operatively. Conclusion En bloc resection with centralization of the ulna remains a viable technique to address aggressive GCTB of the distal radius and can be appropriate in resource-limited settings.
