- Abstract
Autistic people experience more health conditions and earlier mortality. This study investigated views about a primary care health check for autistic adults to inform its design. Fifty-one people participated in consultation groups and interviews, comprising autistic adults (some with co-occurring intellectual disabilities), adults with intellectual disabilities, supporters and health professionals. Participants wanted the health check to cover physical and mental health and social functioning. They emphasised the importance of sharing information about individual needs and associated adjustments before the health check. They highlighted the need to change the way healthcare services communicate with autistic people, such as reducing phone contact and booking appointments online. They wanted individual choice in how the health check was completed, with video call or email offered alongside face-to-face. Participants raised the need for further training of primary care staff on autism, to highlight the diversity of experiences of autistic people and ways in which difficulties, such as pain, may present differently. Clinicians raised questions about the capacity of mental health and social care services to meet the additional needs potentially identified through the health check. This study represents a key step in the development and co-design of a UK primary care health check for autistic people.

16. Profile of Mothers of Children with a Disability Who Seek Support for Mental Health and Wellbeing

- Abstract
This paper investigated the characteristics of mothers of children with a disability who registered for a mental health and wellbeing workshop. The questionnaire measured mental health, health-related behaviours, empowerment, family cohesion, wellbeing and child-related variables. Regression analysis identified factors associated with depressive symptoms and positive wellbeing. Fifty-seven percent of participants (N = 171) had depressive symptoms within the clinical range. Higher symptoms were associated with reduced: empowerment (r = -0.39, p < 0.01); positive-wellbeing (r = -0.66, p < 0.05); and healthy activity (r = -0.41, p < 0.001). Low positive wellbeing ([beta] = 0.55, p < 0.001) was the strongest predictor of depressive symptoms. Family cohesion ([beta] = 0.25, p < 0.001), was the strongest predictor of positive-wellbeing. Future health and wellbeing interventions that support mothers with high care responsibilities should include psycho-education and strategies to address healthy maternal and family-related behaviour changes.

- Abstract
Limited interventions exist that support student's school participation. This paper describes a theoretical model of school participation and the iterative process that led to the development of an intervention that aims to improve the school participation of students on the autism spectrum and their typically developing peers. Literature on autism, school participation and intervention research were integrated to develop a theoretical model. Focus groups, a Delphi study, online surveys, and reference group consultation helped to develop and refine the intervention. A novel school-based intervention was developed. The impetus to develop interventions with a strong theoretical rationale is discussed.

- Abstract
School participation and connectedness have significant implications for student outcomes while at school and in later life. The need to develop evidence-based interventions to proactively support students with autism spectrum disorder (ASD) is imperative. A two-round Delphi technique was used to gain expert consensus to inform the development of a school-based intervention to improve the school participation and connectedness of elementary students with ASD. Seventy-six expert clinicians, educators, and researchers completed Round 1 and 65 completed Round 2. Consensus was achieved on the application of a conceptual framework of participation in Round 1, which informed the theoretical rationale of the intervention. Consensus on the importance of proposed classroom modules and the feasibility of proposed intervention techniques was achieved in Round 2. The process of gaining expert perspectives to develop an evidence-based intervention provides greater confidence that the intervention will be effective in achieving meaningful outcomes for students with ASD.

We present a policy and process framework for secure environments for productive data science research projects at scale, by combining prevailing data security threat and risk profiles into five sensitivity tiers, and, at each tier, specifying recommended policies for data classification, data ingress, software ingress, data egress, user access, user device control, and analysis environments. By presenting design patterns for security choices for each tier, and using software defined infrastructure so that a different, independent, secure research environment can be instantiated for each project appropriate to its classification, we hope to maximise researcher productivity and minimise risk, allowing research organisations to operate with confidence.

- Abstract
Healthy Mothers Healthy Families (HMHF) is a program that educates and empowers mothers of children with disabilities to improve health behaviours. Outcomes were investigated in this study. A pre, post-test design was implemented using online questionnaires including the Health promoting activities scale (HPAS) and the Depressional anxiety stress scales (DASS). Mothers (N = 71) experienced improvements in HPAS scores, p < 0.001. Mental health symptomatology reduced: depressive symptoms (p = 0.005), Anxiety symptoms (p = 0.005) and stress (p = 0.002). Wellbeing improved (p < 0.001). Mothers also reported that their child with a disability experienced an increase in quality of life (p = 0.042). Mothers' lifestyles goals improved: managing stress; dietary changes; leisure; self-perception and others. HMHF is an effective intervention with improved health status and outcomes for mothers.

24. Interventions to Improve the Mental Health of Mothers of Children with a Disability: Systematic Review, Meta-Analysis and Description of Interventions

To investigate the effectiveness of interventions that aim to improve the mental health of mothers of children with disabilities. Seven databases were searched. Interventions incorporated primarily cognitive-behavioural, psychoeducation, mindfulness or support-group approaches. The Template-for-Intervention-Description-and-Replication guided descriptions. Meta-analyses using a random effect model of randomized controlled trials assessed intervention effects on parenting stress and mental health. Of the 1591 retrieved papers, 31 met criteria to be appraised and 17 were included in the meta-analysis. Cognitive-behavioural approaches reduced parenting stress [2 studies, n = 64, pooled Standardized-Mean-Difference (SMD) = 0.86, 95% CI (0.43, 1.29)] and improved mental health [3 studies, n = 186, pooled SMD = 1.14, 95% CI (0.12, 2.17)], psychoeducation approaches improved mental health [2 studies, n = 165, SMD = 0.60, 95% CI (0.17, 1.03)]. Cognitive-behavioural and psychoeducation interventions are effective. Further research and clinical guidelines are warranted.

Ageing is a complex process that occurs in every living organism and is the result of interactions between genetics and the environment. As we age, the risk of developing a number of diseases like Alzheimer's disease, cardiovascular disease and cancer increases significantly. However, not everyone will develop any of these illnesses, and may instead undergo 'healthy ageing', a process broadly defined in humans as "ageing in the absence of illness". Healthy ageing can be promoted by, amongst other things, regular exercise and caloric restriction. Previous research has shown that caloric restriction can delay the onset of the negative impacts of ageing, and also extend maximum lifespan. These effects are regulated via the activity of the NAD+ dependent deacetylase SIRT1, and overexpression of SIRT1 can have the same effect as caloric restriction. Exercise also promotes improved health with ageing, thought the underlying mechanisms are not fully understood. Here, we aimed to investigate the impact of long-term exercise in an ageing mouse model, and how changing the expression of the NAD+ biosynthetic enzyme NMNAT1 could impact on ageing in both sedentary and voluntary exercise conditions. For this we compared two colonies of mice: transgenic overexpression of NMNAT1 (TG) and heterozygous knockout of NMNAT1 (Het). We also included the wildtype (WT) littermates from both colonies. We focused on four key areas that are known to be affected in ageing: behaviour, adult neurogenesis, miRNAs as blood biomarkers of healthy ageing and hippocampal gene expression. In order to ensure that our behavioural results were ecologically relevant, we first determined how the phase of the light cycle under which behavioural tests are performed can alter the behaviour outcomes in mice that had access to a running wheel. Within the literature there is conflicting data on how exercise affects behaviour in a mouse, mainly focused on the effect on anxiety. Half of the mice were given access to a running wheel for one month before the behavioural tests. At the end they all underwent a series of behavioural tests, either under white light or in the dark. When the tests were performed in the light it appears as though the mice that exercise go into the open areas of the elevated plus maze less which can be interpreted as the mice being more anxious. However, when the same test was performed in the dark this did not happen. We also found that all mice showed increased activity in all of the tests when under dark conditions, as mice are nocturnal and are more active at night. From these results we decided to perform all other behavioural tests in the dark phase of the light/dark cycle. Afterwards, we focused on the impact of exercise and NMNAT1 genotype on ageing. Male mice of all three genotypes (WT, Het and TG) grew to 18 months of age under group housed conditions. At both 11 and 18 months of age, all of the mice underwent a series of behavioural tests for cognition, anxiety and locomotor activity. From 12 to 18 months of age, the mice either had access to a running wheel or remained sedentary but with a broken wheel to control for environmental enrichment. All mice received an injection of BrdU at 17 months of age. We also had a group of young (2-month old) male and female mice from all three genotypes, and we compared the levels of adult neurogenesis in the young and old mice. In WT mice, exercise tended to increase/maintain cognitive ability with age. In the brains of these mice, we found an increase in the number of new cells in the brain becoming neurons and surviving long enough to mature and potentially integrate into the neuronal network. Exercise also increased the expression of miR-21-5p in the blood plasma of WT mice, a miRNA that has previously been shown to have a role in the regulation of neural stem/progenitor cell (NPC) proliferation in the adult brain. When we compared the levels of adult neurogenesis in the young and aged mice of all three genotypes, we saw a significant decrease with age in all groups. This comparison has not been done in Het and TG mice before, and we saw some slight variations between the different genotypes with age. When comparing adult neurogenesis in the WT and Het mice, we saw a significant increase in the NPC proliferation and immature neuron numbers in the Het mice. However, exercise reduced the number of proliferating cells in the Het mice. In the brains of TG mice, we saw an increase in the number of new cells becoming immature neurons and the number of cells surviving long enough to mature. Exercise had no significant effect on adult neurogenesis in TG mice. We saw a significant increase in miR-21-5p in the TG mice in comparison to the WT mice, similar to the increase seen with exercise in the WT mice. The expression of miR-92a-5p and miR-21a-3p was significantly decreased in TG mice that had exercised. Both of these miRNAs have been previously linked with regulating adult neurogenesis. Finally, we saw a significant increase in the expression of BDNF in the hippocampus of TG mice that had exercised. We saw no significant changes with exercise or genotype in the hippocampal expression of NMNAT2, SIRT1, NAMPT, Ezh2 or Utx. Overall, we concluded that prolonged voluntary exercise has a significant impact on adult neurogenesis and circulating miRNA expression in aged mice but does not significantly affect the behaviour of the mice or gene expression in the hippocampus. Changing the expression of NMNAT1 also impacts adult neurogenesis and circulating miRNA expression in aged mice and can impact the effect that exercise has in aged mice. Further research is required to fully understand the underlying mechanisms behind the changes we have found in this study, both with exercise and the change in NMNAT1 expression.

Introduction: In Australia, children with cerebral palsy and complex disability receive funded supports through the National Disability Insurance Scheme (NDIS). This individualised funding scheme requires parents to navigate and advocate on behalf of their child, supported by expert reports, recommendations, and allied health services. Supports aim to enable participation in all areas of daily life, which may be otherwise largely inaccessible to children with complex disability and their families. This study aimed to explore the experiences of families of children with complex disability after 3 years accessing the NDIS. Methods: A qualitative research design with a demographic questionnaire and in‐depth interview was undertaken. Purposive sampling was used to recruit participants from one organisation providing occupational therapy and other allied health services. Data analysis implemented Braun and Clarke's thematic approach to examine the experiences of participants. Consumer and Community Involvement: This research was conducted with a registered National Disability Insurance Scheme provider to give voice to parent consumers who raise children with complex disability. Findings: Seven mothers and one father (N = 8) of children with complex disability were interviewed. Most parents reported increased success and satisfaction navigating the scheme. Five overall themes were generated from the data: pivotal roles of families, parental empowerment, life‐changing equipment, the fallibility of the scheme, and a critical scheme. Conclusion: Parents reported reliance on the scheme for their child's basic daily care and a more enriched life for their child and family. Parents were grateful for the scheme but experienced inconsistencies, navigation difficulties, and variable choice and control. Most parents had fears about the sustainability of the scheme, translating into uncertainty about their child's future. Allied health professionals, including occupational therapists, are key advocates for children with complex disability and their families. Collaboration through sharing knowledge and skills to support children, their families, and carers is key to empowering parents to navigate the NDIS. PLAIN LANGUAGE SUMMARY The National Disability Insurance Scheme (NDIS) provides funding for people with permanent and significant disability. Children with cerebral palsy (and other complex disability) are lifetime users of the NDIS. For children with complex disability, their families are crucial to ensuring that their daily needs are being met, including providing medication. Previous research indicated that parents rely on the NDIS to support their children; however, there have been various challenges such as long wait times for equipment and difficulty understanding how to use the scheme. This study explored the experiences of families of children with complex disability, after more than 3 years of being an NDIS participant. Eight parents from one therapy service provider completed a short questionnaire about themselves, their child, and their family, followed by an interview with the first author. Four authors (occupational therapists) worked together to design and implement this study. The findings highlighted several key points: the important role of parents as caregivers; parents became more knowledgeable and confident to navigate the NDIS with time; equipment funded by the NDIS was life‐changing; the NDIS has ongoing issues; and the crucial nature of the NDIS. Occupational therapists can be extremely important to families, including with supporting families to navigate the NDIS and advocating for them. Occupational therapists must stay current with their knowledge of the NDIS as they provide lifetime support, including prescribing equipment, technology, and home modifications. This Practice and Policy article examines return‐to‐office mandates, the latest human resource controversy. These mandates are an organisational directive for employees who have been working from home to return to working in their employer's premise. Drawing on the literature and our research on working from home and hybrid working, we consider whether mandates may prevent proximity bias. We conclude that mandates requiring employees to return to the office or caps which limit working from home are not only unnecessary, but may have negative consequences. In particular, mandates may cause employee resentment, while caps limit flexibility and autonomy. We therefore do not advocate the use of these mechanisms, and recommend that managers and teams negotiate the appropriate balance of home and office working arrangements. Points for practitioners: Mechanisms which force employees into the office can be seen as an easy and effective way to mitigate proximity bias. However, they can lead to employee resentment.Similarly, capping the number of days employees can work from home can also result in negative consequences, including reduced flexibility and employee autonomy.Enabling managers and teams to collaboratively determine their own in office/working from home arrangements will maintain flexibility and prevent employee resentment.Preventing proximity bias can be achieved through increasing awareness about this emerging form of bias; harnessing communication technologies to moderate visibility regardless of where work is performed; and ensuring performance management systems are based on quantifiable and objective metrics. - Abstract
Goal 2 of the "APA Goals for Undergraduate Major in Psychology," Scientific Inquiry and Critical Thinking, addresses the development of scientific reasoning and problem-solving, including effective research methods, in undergraduate psychology students. These skills serve as the foundation of not only introductory courses but also the entire psychology curriculum. In this article, we address why these skills are critical to help students interpret information and assess claims and review the current practice of measuring student reasoning in this domain. The reach of Goal 2 is broad, including the application of scientific reasoning to interpret psychological phenomena, the demonstration of psychology information literacy, the interpretation and execution of basic psychological research, and an understanding of sociocultural influences in scientific inquiry. We discuss the value of both formative and summative assessments in this area, offer examples of assessment tools currently in use, and provide a list of nationally normed assessments in critical thinking and scientific reasoning. We conclude with a discussion of limitations in current assessment practices and suggest possible future directions.

35. Optimisation through innovation : judicial exercise of discretionary remedial power to enforce the state's positive human rights duties

This thesis bridges remedial theory and practice to explore how the judicial exercise of discretionary remedial power can be optimised to enforce the state's positive human rights duties. Recognising both the difficulty and the importance of remedying breaches of the state's positive duties, this thesis advances a constructive project that illuminates not just the risks but also the potential rewards of discretionary remedial power. Part I develops a theoretical account of the optimal exercise of discretionary remedial power that is attentive to both the challenges and the responsive nature of the judicial task in remedying breaches of the state's positive duties. I argue for a principled exercise of discretionary remedial power that involves the contextualised prioritisation and optimisation of four remedial principles: (1) Effective Relief, (2) Systemic Compliance, (3) Institutional Responsiveness, and (4) Institutional Complementarity. Remedial decision-making should not only assign a conditional order of preference to the remedial principles when they are in tension, but also optimise all four principles as far as possible in any particular case. Part II uses a case study method to illuminate how this imperative of optimisation can be achieved through remedial innovation. Using the evaluative framework developed in Part I of the thesis, I trace the trajectory of remedial development in two case studies: the litigation on education financing run by the Campaign for Fiscal Equity in New York State, and the education litigation on school infrastructure, furniture and teachers run by the Legal Resources Centre in South Africa. In drawing out the potential of the featured remedial innovations for addressing the distinctive remedial challenges posed by positive duties, my thesis demonstrates how the exercise of discretionary remedial power can be optimised through innovation.

45. Online sexual grooming : the role of offender motivation and grooming strategies

This thesis explores the strategies used by online sexual groomers and the role of offence motivation. Chapter 1 introduces the phenomenon, highlighting the importance of understanding the process of grooming in order to effectively safeguard young people in an ever-expanding online world. Chapter 2 explores the psychometric properties and utility of the Multiphasic Sex Inventory to assess Internet offenders. Following a discussion of the strengths and weaknesses of online sexual grooming research methodology, a systematic re-view of literature investigating grooming narratives is presented in Chapter 3. The review identifies that in-depth qualitative findings require replication with larger statistically significant sample sizes and different grooming offence motives require further attention. Chapter 4 presents a research study analysing narrative themes in the grooming transcripts of 75 contact-driven and 75 fantasy-driven offenders. Six narrative themes are identified which complement the Self-Regulation Model of online sexual grooming (Elliott, 2015). The narrative theme of Sexual Desensitisation is found to contribute to a predictive model for offence motive. The thesis and its utility is discussed in Chapter 5. This thesis directly supports and justifies the introduction of a new sexual communications law in April 2017 criminalising non-contact online grooming interactions.

46. Rawls' 'difference principle' : a test for social justice in contemporary social policy

This thesis addresses a number of topics which are not normally combined: John Rawls, homelessness, and social policy. The research is a piece of applied philosophy which seeks to address claims about a disconnection between philosophical frameworks and tangible political problems. It argues that there is a role for philosophers, and philosophical frameworks, within the policy-making process. By applying philosophy to policy we can create normative accounts of the development and impact of legislation. The philosophical framework used within this research is John Rawls’ conception of justice as fairness. The work outlines the liberal principle of legitimacy and argues that Rawls’ concept of reasonableness can, and should, be used to justify the intervention of policy in individuals’ lives. The concept of reasonableness can be used as a regulatory mechanism in order to test whether social policy meets a standard of social justice. The metric used for this standard is whether the worst off in society have the capacity to create and pursue a conception of the good, a central capability of citizens as effective agents. Through modifying or emphasising certain Rawlsian concepts, the research develops the Difference Principle as a regulatory test to be applied to social policy. The final part of the research provides this initial application. The Housing (Wales) Act is used as a case study to assess whether the test as developed is able to create normative interpretations of specific pieces of legislation. It identifies a particular legal tool – the Pereira Test – as problematic on a normative account. The application of the Difference Principle demonstrates that this particular legal tool undermines individuals’ ability to safeguard their fundamental interests, and their capacity to create and pursue a conception of the good.

47. Healthy Mothers Healthy Families Workshop Intervention: A Preliminary Investigation of Healthy Lifestyle Changes for Mothers of a Child with a Disability

Substantial evidence identifies mothers of children with a disability as having a higher risk for compromised health outcomes and lifestyle restrictions secondary to caring responsibilities. Healthy Mothers Healthy Families (HMHF) is an evidence informed health and empowerment group-based workshop program. Methods: HMHF features health education and lifestyle redesign content. Mixed methods evaluated the program. This paper presents a pretest-postest time series design to evaluate outcomes over 8 months. Results: Mothers reported significant change across 4 time points including participation in healthy activity (p = 0.017), depression, anxiety, stress symptoms (p = 0.017, 0.016, 0.037 respectively) and empowerment (p = 0.009). Conclusion: Coupled with qualitative findings, these results suggest that HMHF is effective at improving health and wellbeing outcomes for mothers of children with a disability.

Drawing on developments in the application of occupational science to issues of homelessness and tenancy sustainability, this paper will argue for the recognition of authentic, everyday relationships as a core part of tenancy support. Data from a small-scale evaluation of a volunteer-led homelessness support service will be explored through a framework of social support to show the importance of companionship in enabling individuals to exit homelessness. The data show that individuals valued the provision of social support from volunteers in everyday locations, as it replicated authentic relationships which individuals had sometimes lost through their experiences of homelessness. This paper argues that the facilitation of authentic, everyday relationships should be considered an important part of homelessness support. [ABSTRACT FROM AUTHOR]

Importance: High stress and compromised mental health are well-documented among mothers of children with disabilities, but less is known about the factors that influence their participation and well-being, especially among Chinese mothers. Identifying these factors could lead to strategies to mitigate potential risks to maternal well-being. Objective: To explore the relationships among participation in health-promoting activities, perceived stigma, perceived support, and well-being of ethnic Chinese mothers who have children with disabilities. Design: Cross-sectional survey. Setting: Community. Participants: Ethnic Chinese mothers (N = 261) with children with disabilities of any age residing in Australia, Singapore, or Taiwan. Outcomes and Measures: Health Promoting Activities Scale, Personal Wellbeing Index, Warwick–Edinburgh Mental Wellbeing Scale; Kessler Psychological Distress Scale, Parental Perceptions of Public Attitudes Scale, and Multidimensional Scale of Perceived Social Support. Results: Moderate to strong correlations were found between mental well-being and all measures. The frequency of participation in health-promoting activities was a predictor of mental well-being (β =.21, p =.001). Perceived support was the strongest predictor of participation in health-promoting activities (β =.25, p =.005), mental well-being (β =.39, p <.001), and personal well-being (β =.45, p <.001). Perceived stigma predicted psychological distress (β =.32, p <.001) and mental well-being (β = −0.29, p <.001). Conclusions and Relevance: Perceived support and stigma affected mothers' participation in health-promoting activities and well-being. Therapists can facilitate mothers' engagement in health-promoting activities and develop strategies to address stigma and enhance support. Plain-Language Summary: This research highlights factors that affect the well-being of ethnic Chinese mothers who have children with disabilities and provides cultural insights for occupational therapists who work with Chinese families. The article also encourages pediatric occupational therapists to address the occupational needs and well-being of mothers in parallel with therapy for their children. We discuss occupation-focused strategies, family-centered perspectives, and health promotion approaches in relation to improving the participation and well-being of ethnic Chinese mothers in their cultural context. Introduction Method Consumer and Community Involvement Results Conclusion Parents of children with additional needs experience compromised physical and mental health and higher stress, and their ability to engage in meaningful occupations is impacted by their additional caregiving tasks. Online interventions targeting mental health, stress, and well‐being for parents of children with additional needs have potential to increase supports alongside occupational therapy direct services. The aims of this review are to systematically identify and synthesise evidence of effectiveness of online interventions aimed at improving mental health, stress, and well‐being of parents of children with additional needs.Eight databases were searched up to July 2024 to identify online interventions for parents of children with additional needs. Studies were appraised for methodological quality. Standardised effect sizes were calculated, and meta‐analyses of randomised control trials (RCTs) were conducted on outcomes of mental health, well‐being, and stress.There was no consumer or community involvement.Systematic screening identified 30 papers that met inclusion criteria. Most were RCTs or controlled clinical trials (CCTs) with ‘moderate’ quality ratings: Three were designed by occupational therapists. There was substantial variation in intervention types, methods of delivery, outcomes, and outcome measures used. The pooled standardised effect size estimates (
ES ) and the lower and upper confidence intervals (CI ) of online interventions from RCTs post‐interventions were significant for mental healthES = 0.47, 95%CI (0.18, 0.77),p = 0.002; stressES = 1.27, 95%CI (0.56, 1.98),p = 0.000; and well‐beingES = 0.65, 95%CI (1.2, 0.06),p = 0.03, respectively.The online interventions that aimed to improve mental health, stress, and well‐being included in this meta‐analysis were effective. Occupational therapists supporting families play an important role in guiding parents of children with additional needs to evidence‐based interventions to support them with addressing their own mental health and well‐being needs.
