9. Shoulder recovery for head and neck cancer patients after unilateral neck dissection: a pilot exploratory study

Abstract

An established side-effect of neck dissection (ND) for head and neck (HNC) tumour management includes shoulder dysfunction (SD), which can impact quality of life (QOL). Shoulder strength and range of movement (ROM) are key parameters to be monitored in SD. However, such evaluations are not routinely conducted in the clinical setting. The aim of this study was to evaluate objectively the impact of ND on shoulder functions. Methods: This is a pilot exploratory study in a tertiary cancer centre. Five participants with unilateral ND and advanced HNC, completed the study. Outcome measures consisted of self-reported QOL questionnaires, C2–T1 dermatomes and shoulder ROM and strength testing. Data was collected at baseline, 1.5-months after surgery and 6-months after diagnosis (after adjuvant treatment completion). Results: Most outcome measures on the surgically affected side were negatively impacted post-operatively, with varied recovery seen at follow-up. Sensory loss was noted at C3–4 dermatome levels. Shoulder ROM and strength was reduced on the surficial side for all participants, with some recovery after six months except for two participants. Conclusion: Results of SD after ND are diverse and unique to each patient. Findings from this pilot study indicate that regular rehabilitation/exercise may facilitate recovery of shoulder function post HNC surgery. However, customised rehabilitation may yield better outcomes. Future studies with a larger sample are indicated to validate the findings of this study.

10. Quantifying the effectiveness of ultraviolet-C light at inactivating airborne Mycobacterium abscessus.

Abstract

Background: Mycobacterium abscessus (MABS) group are environmental organisms that can cause infection in people with cystic fibrosis (CF) and other suppurative lung diseases. There is potential for person-to-person airborne transmission of MABS among people with CF attending the same care centre. Ultraviolet light (band C, UV-C) is used for Mycobacterium tuberculosis control indoors; however, no studies have assessed UV-C for airborne MABS.Aim: To determine whether a range of UV-C doses increased the inactivation of airborne MABS, compared with no-UVC conditions.Methods: MABS was generated by a vibrating mesh nebulizer located within a 400 L rotating drum sampler, and then exposed to an array of 265 nm UV-C light-emitting diodes (LED). A six-stage Andersen Cascade Impactor was used to collect aerosols. Standard microbiological protocols were used for enumerating MABS, and these quantified the effectiveness of UV-C doses (in triplicate). UV-C effectiveness was estimated using the difference between inactivation with and without UV-C.Findings: Sixteen tests were performed, with UV-C doses ranging from 276 to 1104 μW s/cm2. Mean (±SD) UV-C effectiveness ranged from 47.1% (±13.4) to 83.6% (±3.3). UV-C led to significantly greater inactivation of MABS (all P-values ≤0.045) than natural decay at all doses assessed. Using an indoor model of the hospital environment, it was estimated that UV-C doses in the range studied here could be safely delivered in clinical settings where patients and staff are present.Conclusion: This study provides empirical in-vitro evidence that nebulized MABS are susceptible to UV-C inactivation. Abstract

In the event of a novel influenza A virus pandemic, prophylaxis mediated by antibodies provides an adjunct control option to vaccines and antivirals. This strategy is particularly pertinent to unvaccinated populations at risk during the lag time to produce and distribute an effective vaccine. Therefore, development of effective prophylactic therapies is of high importance. Although previous approaches have used systemic delivery of monoclonal antibodies or convalescent sera, available supply is a serious limitation. Here, we have investigated intranasal delivery of influenza-specific ovine polyclonal IgG antibodies for their efficacy against homologous influenza virus challenge in a mouse model. Both influenza-specific IgG and F(ab’)2 reduced clinical scores, body weight loss and lung viral loads in mice treated 1 hour before virus exposure. Full protection from disease was also observed when antibody was delivered up to 3 days prior to virus infection. Furthermore, effective prophylaxis was independent of a strong innate immune response. This strategy presents a further option for prophylactic intervention against influenza A virus using ruminants to generate a bulk supply that could potentially be used in a pandemic setting, to slow virus transmission and reduce morbidity associated with a high cytokine phenotype.

35. Real-Time Cure Monitoring of Unsaturated Polyester Resin from Ultra-Violet Curing

Abstract

Real time Fourier transform infrared (RTFT-IR) spectroscopy reveals the influence of the nature of the photoinitiator on the kinetics for the polymerization reaction. Real time cure monitoring was used to determine the polymerization rate of ultra-violet cured unsaturated polyester resins (UPR1:2-1:5) based on palm oil and containing styrene as a cross-linking agent in the presence of IRGACURE 184 photoinitiator. Firstly, variable types of UPR1:2-1:5 were prepared using various ratios of monoglyceride (MG) monomer to maleic anhydride which used as a source of double bond using polycondensation technique. RTFT-IR spectroscopy was used to characterize the ultra-violet curing kinetics for all the systems. This technique offered a powerful approach for monitoring changes in the chemical properties of the system during the ultra-violet curing. Pseudo first order kinetics for all UPR1:2-1:5 curable systems were determined and the rate constant values and regression coefficients were calculated. Furthermore, the thermal behaviour and morphological features for the photo-fabricated UPR1:2-1:5 systems were examined. The thermal analyses for the cured films were evaluated by TGA, DTG and DSC in nitrogen atmosphere at a heating rate of 10°C/min. All the formulations showed similar degradation pattern at 40 % and 50 % weight losses (Td = 40% and Td =50% respectively) except polymer UPR1:5 which had somewhat lower degree in the same range. In addition, the morphological properties for photo-fabricated UPR1:2,1:3,1:5 were investigated by using scanning electron microscopy.
