Deficiency of vitamins and minerals is an important public health problem in both developing and industrialized countries. Consequently, food fortification practices are often used to combat nutritional deficiencies in the general populace. In Turkey, women in almost every age group do not meet the recommended daily iron intake. Furthermore, iron intake was found to be insufficient in males and in children (2-5, 6-8 and 9-11 ages). Folate intake is also under daily requirement in women of childbearing age. Serum vitamin D levels of 81.7% of mothers were deficient (19.9ng/mL and below) and 11.6% were insufficient (20-29.9 ng/mL). In a study of 6 to11 month-old infants, 26.8% of children were deficient (14.99ng/mL and below), 14.7% were slightly deficient (15-19.9 ng/mL) and 26.2% were insufficient (20-29.9 ng/mL). The median levels of serum vitamin D in 10,160 samples was found to be 23.8 ng/mL. Of the participants, 6.9% had below 20 ng/mL and 66.7% below 30ng/mL respectively. Fortification is important for eliminating these deficiencies and combating stunting, which is still relatively common in Turkey. The protection and prevention of diseases are easier and more cost-effective than treatment and rehabilitation practices. This report was produced through examining national study results and international practices, with the aim of contributing to regulatory practices to be developed around nutritional deficiencies. Regulations should be made to fortify flour with iron and folic acid; milk and oils with vitamin D and facilitate access to these products., Çeşitli vitamin ve minerallerin yetersizliği; gelişmekte olan ve endüstrileşmiş ülkelerde sağlık ve ekonomik sonuçları ile önemli bir halk sağlığı sorunudur. Besin zenginleştirme uygulamaları birçok ülkede beslenme gereksinmelerinin sağlanmasında pratik bir araç olarak kullanılmaktadır. Ülkemizde kadınlarda hemen hemen her yaş grubunda demir alımının ihtiyacın altında kaldığı ve günlük gereksinimin karşılanamadığı görülmektedir. Erkeklerde özellikle çocuk yaş gruplarında (2-5, 6-8 ve 9-11 yaş grupları) demir alımının yetersiz olduğu tespit edilmiştir. Folat alımı da özellikle kadınlarda doğurganlık çağında ve ileri yaşlarda günlük gereksinimin altındadır. Serum D vitamini düzeyine göre annelerin %81.7’sinde D vitamini eksik (19.9 ng/mL ve altında), %11.6’sında ise yetersiz (20–29.9 ng/mL)’dir. Altı ila onbir aylık süt çocuklarını kapsayan bir çalışmada çocukların D vitamini düzeylerine bakıldığında; %26.8’inin D vitamini eksik (14.9 ng/mL ve altında), %14.7’sinin hafif eksik (15–19.9 ng/mL) ve %26.2’sinin ise yetersiz (20.0–29.9 ng/mL) olduğu bulunmuştur. Türkiye genelinde incelenen 10160 numunenin serum D vitamini düzeylerinin ortancası 23.8 ng/mL olarak saptanmıştır. Bireylerin %6.9’unda D vitamini düzeyi 20 ng/mL’nin. %66.7’sinde ise 30 ng/mL’nin altında bulunmuştur. Zenginleştirme bu yetersizliklerin giderilmesi yanında, halen yüksek olan bodurluk ile mücadele için de önemlidir. Halk sağlığının korunması ve hastalıkların önlenmesi için yapılan tüm çalışmalar, tedavi ve rehabilitasyon uygulamalarından daha kolay ve “maliyet-etkin” yaklaşımlardır. Ülkemizde yapılan araştırma sonuçları ve dünyada yapılan uygulamalar dikkate alınarak hazırlanan rapor ile Türkiye’de bu konuda yapılacak düzenlemelere katkı sağlanması amaçlanmıştır. Unun demir ve folik asit ile; süt ve yağların D vitamini ile zenginleştirilmesi; ayrıca bu ürünlere erişimin kolaylaştırılması için gerekli düzenlemeler yapılmalıdır.

Çakır, Volkan (Arel Author), Berber, Ergül (Arel Author), Background. Factor XI (FXI) deficiency is an autosomal bleeding disease associated with genetic defects in the F11 gene which cause decreased FXI levels or impaired FXI function. An increasing number of mutations has been reported in the FXI mutation database, most of which affect the serine protease domain of the protein. FXI is a heterogeneous disorder associated with a variable bleeding tendency and a variety of causative F11 gene mutations. The molecular basis of FXI deficiency in 14 patients from ten unrelated families in Turkey was analysed to establish genotype-phenotype correlations and inheritance of the mutations in the patients' families. Material and methods. Fourteen index cases with a diagnosis of FXI deficiency and family members of these patients were enrolled into the study. The patients' F11 genes were amplified by polymerase chain reaction and subjected to direct DNA sequencing analysis. The findings were analysed statistically using bivariate correlations, Pearson's correlation coefficient and the non-parametric Mann-Whitney test.Results. Direct DNA sequencing analysis of the F11 genes revealed that all of the 14 patients had a F11 gene mutation. Eight different mutations were identified in the apple 1, apple 2 or serine protease domains, except one which was a splice site mutation. Six of the mutations were recurrent. Two of the mutations were novel missense mutations, p.Val522Gly and p.Cys581Arg, within the catalytic domain. The p.Trp519Stop mutation was observed in two families whereas all the other mutations were specific to a single family. Discussion. Identification of mutations confirmed the genetic heterogeneity of FXI deficiency. Most of the patients with mutations did not have any bleeding complications, whereas some had severe bleeding symptoms. Genetic screening for F11 gene mutations is important to decrease the mortality and morbidity rate associated with FXI deficiency, which can be life-threatening if bleeding occurs in tissues with high fibrinolytic activity.

Tedavide kullanılan metilen mavisi ile dramatik bir düzelme sağlanmış ve hayat kurtarıcı olmuştur., Methemoglobinemia is a disorder characterized by the presence of a higher than normal level of ferric (Fe+3) rarer man ferrous (Fe+2) hemoglobin. It may be in two forms, congenital or acquired methemoglobinemia. It is known that many agents can cause acquired methemoglobinemia. If the blood methemoglobin level reaches 55%, it can be lethal. Prompt care is needed in this situation. Here, a 4-month-old boy with acquired methemoglobinemia due to prilocaine use is presented. Methylene blue was used in the treatment of this case. Aktive protrombin kompleks konsantresi (aPCC) ve rekombinant faktör VIIa (rFVIIa) inhibitör geliştirmiş hemofili hastalarının kanama tedavisinde ve profilaksisinde kullanılmaktadır. Ülkemizde rFVIIa için profilakside endikasyon onayı ve geri ödemesi henüz yoktur. Bu yazıda inhibitörlü hastada aPCC’nin doz ve etkinliği değerlendirilmiştir. Her üç vakamızda da aPCC profilaksisi alırken kanamanın olmaması, iki hastada profilaksi kesildikten sonra tekrarlayan kanamaların görülmesi profilaksi süresinin uzatılabileceğini desteklemektedir., Use of activated prothrombin complex concentrate (aPCC) and recombinant factor VIIa (rFVIIa) is indicated for the treatment and prophylaxis of hemophiliac patients who has developed inhibitors. rFVIIa is an off labeled drug for prophylaxis in Turkey and not covered by health insurance. In this manuscript, the efficacy and dosage for aPCC prophylaxis were evaluated. During the aPCC prophylaxis, bleeding did not occur in any of the three cases. However, two patients developed recurrent bleeding attacks after the prophylaxis ceased. Therefore, we suggest that prophylaxis by aPCC should be prolonged to prevent recurrent bleeding attacks in hemophiliacs who developed inhibitors

In this retrospective study, cyclosporine levels at the second hour (C2 levels) were measured during oral cyclosporine intake in 28 pediatric hematopoietic stem cell transplant patients, and the relations between cyclosporine dosage and C0, C2 levels, C2/C0 ratio, and cyclosporine-related adverse effects were investigated. Cyclosporine levels at the second hour levels were found to be significantly lower in children younger than 7 years old, suggesting age-related differences in absorption and metabolism of the drug. There were statistically significant correlations of both C0 and C2 levels with blood creatinine values. In addition, a statistically significant negative relation was found between C0 and C2 levels and serum potassium levels; this unexpected finding was attributed to multiple drug effects in the early posttransplant period. The common adverse effects of cyclosporine (gingival overgrowth, gynecomastia, and hypertrichosis) were also evaluated in this study, and no correlation was found between those adverse effects and C0, C2 levels, C2/C0 ratio, and cyclosporine dosage. In the present study, despite the highly significant correlation of C2 levels with renal and metabolic effects, in pediatric hematopoietic stem cell transplant patients, measurement of C2 levels as a standard practice did not provide an advantage over C0 monitoring. However, the preliminary results suggest that C2 level monitoring could be useful in selected patients with increased risk of renal toxicity or in states where a better estimation of gastrointestinal absorption is needed

Bu çalışmada lösemili çocuklarda yüksek doz metil prednizolon (YDMPZ) tedavisininserum leptin, insülin düzeylerine ve vücut kitle indeksi (VKI)'ne etkileri araştırılmıştır. Buçalışma, YDMPZ tedavisinin serum leptin düzeylerine etkisinin değerlendirildiği literatürdekiilk çalışmadır. Ayrıca, leptin gen polimorfizmleri içinde promotor bölgede olduğu gösterilenve fonksiyonel bir polimorfizm olan G-2548A polimorfizmi çalışılarak, bu polimorfizminlösemili çocuklarda YDMPZ tedavisine bağlı leptin, insülin ve VKI'ndeki değişime etkileriaraştırılmıştır.Bu çalışma Haziran 2005- Mart 2007 tarihleri arasında Hacettepe Üniversitesi TıpFakültesi, Pediatrik Hematoloji Ünitesi ve Dr.Sami Ulus Çocuk Hastanesi, PediatrikHematoloji Kliniği'ne başvuran, yaşları 0-16 yaş arasında değişen 56 akut lenfoblastik lösemi(ALL) ve 16 akut miyeloblastik lösemi (AML)'li çocukta prospektif olarak yapılmıştır.Herhangi bir hastalığı olmayan, yaş ve cinsiyet dağılımı çalışma grubumuza uyan 70 çocuk isekontrol grubu olarak çalışmaya dahil edilmiştir. Akut lösemi grubundaki hastalara 7 günsüreyle YDMPZ tedavisi (ALL'li çocuklara 20 mg/kg/gün, AML'li çocuklara 30 mg/kg/gün,maksimum 1 gram) uygulanmıştır. Tedavi başında ve 7 günlük YDMPZ tedavisi sonundaserum leptin ve insülin düzeyleri Hacettepe Üniversitesi Tıp Fakültesi Nükleer TıpA.B.D.'nda çalışılmıştır. Ayrıca YDMPZ tedavi öncesinde hastaların vücut ağırlığı ve boyölçümleri yapılmış, vücut kitle indeks'leri (VKİ:VA/Boy2) hesaplanmış ve tedavi sonundaaynı parametreler tekrar ölçülerek aradaki fark değerlendirilmiştir. Verilerin istatistikseldeğerlendirmeleri SPSS software, version 11.0 (SPSS Inc, Chicago, IL, USA) programıkullanılarak yapılmıştır.Lösemi ve kontrol grubundaki çocukların vücut ağırlığı, boy ölçümleri, vücut kitleindeksleri, leptin, insülin düzeyleri ve leptin G-2548A polimorfizmi açısından her iki gruparasında fark bulunmamıştır (p>0.05). Lösemi grubunda YDMPZ tedavisi ile leptin G-2548Apolimorfizmi için normal, heterozigot ve homozigot tüm bireylerin vücut ağırlığı ve VKI'ndeartış olduğu görülmüş, ancak bu artış heterozigot ve homozigot bireylerde istatistiksel olarakanlamlı saptanırken (p0.05).
