Abstract

Summary: Background: Surgical care holds significant importance in healthcare, especially in low and middle-income countries, as at least 50% of the 4.2 million deaths within the initial 30 days following surgery take place in these countries. The Lancet Commission on Global Surgery proposed six indicators to enhance surgical care. In Colombia, studies have been made using secondary data. However, strategies to reduce perioperative mortality have not been implemented. This study aims to describe the fourth indicator, perioperative mortality rate (POMR), with primary data in Colombia. Methods: A multicentre prospective cohort study was conducted across 54 centres (hospitals) in Colombia. Each centre selected a 7-day recruitment period between 05/2022 and 01/2023. Inclusion criteria involved patients over 18 years of age undergoing surgical procedures in operating rooms. Data quality was ensured through a verification guideline and statistical analysis using mixed-effects multilevel modelling with a case mix analysis of mortality by procedure-related, patient-related, and hospital-related conditions. Findings: 3807 patients were included with a median age of 48 (IQR 32–64), 80.3% were classified as ASA I or II, and 27% of the procedures had a low-surgical complexity. Leading procedures were Orthopedics (19.2%) and Gynaecology/Obstetrics (17.7%). According to the Clavien–Dindo scale, postoperative complications were distributed in major complications (11.7%, 10.68–12.76) and any complication (31.6%, 30.09–33.07). POMR stood at 1.9% (1.48–2.37), with elective and emergency surgery mortalities at 0.7% (0.40–1.23) and 3% (2.3–3.89) respectively. Interpretation: The POMR was higher than the ratio reported in previous national studies, even when patients had a low–risk profile and low-complexity procedures. The present research represents significant public health progress with valuable insights for national decision-makers to improve the quality of surgical care. Abstract

Anaerobic digestion (AD) is a widely applied technology for renewable energy generation using biogas as energy vector. The existing microbial consortium in this technology allows for the use of several types of biomass as substrates. A promising alternative for the production of high-value products (e.g., mixed volatile fatty acids–VFAs, hydrogen) is the use of modified AD. There are several techniques to achieve this objective by modifying the operating conditions of the process. The literature has described the best AD routes for generating renewable energy or high-value products based on specific substrate types and operating conditions. Few studies have reported the integral fraction valorization of the AD process applying the biorefinery concept. This article provides an analysis of the different routes that favor the production of energy carriers and high-value products involving key issues related to operating conditions and substrates. Moreover, AD is addressed through the biorefinery concept. Finally, a case study is presented where renewable energy and mixed VFAs are generated by applying the biorefinery concept in a number of proposed scenarios using organic kitchen food waste (OKFW) as feedstock. The case study involves an experimental and simulation stage. Then, the economic feasibility of the proposed scenarios is evaluated. In conclusion, AD is a promising and economically feasible technology to produce valuable products from several types of waste materials.

Abstract

Objetivo. Describir los factores asociados a las amputaciones en la población colombiana adulta mayor de 60 años evaluados en la Encuesta SABE 2015, frente a posibles desenlaces adversos en salud. Materiales y métodos. Estudio transversal, retrospectivo y analítico consistente en un análisis secundario de la encuesta SABE (Salud, Bienestar y Envejecimiento) Colombia 2015. Para este trabajo, se excluyeron a las personas que requirieron de un informante sustituto durante la entrevista, personas sin amputaciones o con amputaciones de miembro superior e inferior simultáneamente. La variable dependiente fueron los desenlaces adversos en salud en personas mayores con amputaciones (síntomas depresivos, problemas de movilidad, mala autopercepción de salud, discriminación por discapacidad, caídas en el último año, si en la última caída necesito o no ayuda y hospitalizaciones en el último año). Se realizó análisis descriptivo, bivariado y regresión logística multivariada. Resultados. Abstract

Multipotent mesenchymal stromal cells (MSCs) have been described as bone marrow stromal cells, which can form cartilage, bone or hematopoietic supportive stroma. In 2006, the International Society for Cell Therapy (ISCT) established a set of minimal characteristics to define MSCs. According to their criteria, these cells must express CD73, CD90 and CD105 surface markers; however, it is now known they do not represent true stemness epitopes. The objective of the present work was to determine the surface markers for human MSCs associated with skeletal tissue reported in the literature (1994–2021). To this end, we performed a scoping review for hMSCs in axial and appendicular skeleton. Our findings determined the most widely used markers were CD105 (82.9%), CD90 (75.0%) and CD73 (52.0%) for studies performed in vitro as proposed by the ISCT, followed by CD44 (42.1%), CD166 (30.9%), CD29 (27.6%), STRO-1 (17.7%), CD146 (15.1%) and CD271 (7.9%) in bone marrow and cartilage. On the other hand, only 4% of the articles evaluated in situ cell surface markers. Even though most studies use the ISCT criteria, most publications in adult tissues don't evaluate the characteristics that establish a stem cell (self-renewal and differentiation), which will be necessary to distinguish between a stem cell and progenitor populations. Collectively, MSCs require further understanding of their characteristics if they are intended for clinical use.

5. Alternatives for the Valorization of Avocado Waste Generated in the Different Links of the Value Chain Based on a Life-Cycle Analysis Approach

Abstract

This work evaluates the sustainability of small-scale biorefineries as a potential enterprise alternative to be introduced in rural areas based on experimental and simulation data. Four scenarios were evaluated: the first scenario involves the production of guacamole, the second involves the production of animal feed, and the third and fourth scenarios involve the extraction of bioactive compounds and the production of avocado oil or animal feed, respectively. In addition, all scenarios produce biogas and fertilizer. Each of the scenarios were evaluated considering the technical, economic, environmental, and social aspects. As a main result, the first scenario showed the lowest operating and investment costs, as well as the lowest economic profitability (profit margin 35%). On the other hand, the third and fourth scenarios present the highest investment and operating expenses (OpEx USD 6.2 million per year and CapEx USD 1.0 million), but their profit margins are in the 60–70% range. Furthermore, a life-cycle assessment (LCA) was carried out and allows inferring that the transformer link presents the highest environmental impact of the entire value chain and that the carbon footprint for all scenarios ranges between 1.01–2.41 kg CO2 eq per kg avocado. Similarly, the social impact methodology shows that the proposed scenarios do not present any social risk. Thus, the biorefinery for animal feed, bioactive compounds, biogas, and fertilizer was selected as the best option to be implemented in Caldas.

Abstract

El objetivo de este estudio fue comprender los modelos de Educación para la Salud en programas de Salud Sexual y Reproductiva (SSR) para jóvenes en Medellín 2004-2010. Se desarrolló a través de metodología cualitativa con teoría fundamentada, realizando entrevistas en profundidad a 8 jóvenes y 12 operadores técnicos (16 entrevistas en la primera fase y 4 en la segunda), las cuales fueron codificadas para extraer las categorías de las cuales emergen los resultados. Se estableció que en los modelos de educación presentes en los programas no se tuvieron en cuenta las necesidades de los jóvenes para su formación en SSR. Existe un amplio desarrollo en políticas públicas de juventud, pero los programas de SSR dirigidos a jóvenes siguen teniendo como único sustento las políticas públicas de salud, por esto se deben tener en cuenta las necesidades de los jóvenes para garantizar más éxito en la ejecución.

Tropical forests are a source of several high-value products that provide livelihood to small communities in different regions. Exotic fruits such as annatto are feedstock sources containing bioactive compounds with important applications in the food industry. Nevertheless, the integral use of annatto for community improvement and the crop’s contribution to carbon sequestration in tropical forests have not been analyzed. This paper aims to demonstrate the economic and environmental performance of small-scale alternatives to obtain natural colorants using annatto seed. The extraction of natural colorants (bixin and norbixin) was analyzed using ethanol and NaOH as solvents. The experimental results were used to simulate two scenarios. Scenario one involved bixin production, and scenario two comprised bixin and norbixin production. The economic and environmental assessments were performed considering the life cycle assessment (LCA) methodology based on a Colombian context. The best extraction yield was 72.65 mg g−1 for bixin and 193.82 mg g−1 for norbixin. From a simulation perspective, scenario two showed the best economic performance since a payback period of 3.1 years was obtained. The LCA showed a high CO2 sequestration potential (6.5 kg CO2 eq kg−1 seed) of the annatto crop. Moreover, the solvents used during the colorant extraction proved to be the most environmentally representative. Nevertheless, the CO2 sequestration of the crop continues to exceed the emissions generated by the process. This work demonstrates that the annatto is an alternative for small communities to reach equilibrium between the economic and environmental of the tropical forest.

Abstract

RESUMEN: Esta investigación buscó establecer el significado que tienen los hombres homosexuales sobre sus comportamientos de riesgo de contraer sexualmente el VIH/SIDA. Con un enfoque cualitativo, se entrevistó a 19 hombres homosexuales de la ciudad de Medellín, Colombia. Se utilizó la teoría fundamentada, partiendo del interaccionismo simbólico, para buscar la relación entre los significados y los comportamientos. Se desarrollaron procesos de codificación, categorización e interpretación. Los resultados muestran que (a) la clandestinidad de los encuentros sexuales hace que el riesgo se asuma como una forma de vivir el afecto y (b) existe una asociación inversa entre el afecto y el riesgo asumido: a mayor presencia de afecto mayor evitación del riesgo, consecuente con la información sobre prevención, y a mayor ausencia de afecto mayor disposición al riesgo, buscando el reconocimiento y negando la información sobre protección.
