30. Vagotomy upregulates expression of the N-methyl-D-aspartate receptor NR2D subunit in the stomach.

Abstract

Background: N-methyl-D-aspartate (NMDA) receptor is the major excitatory neurotransmitter receptor in the central nervous system. Recent evidence has pointed to expression of NMDA receptor in peripheral non-neuronal tissues and organs; however, little is known about the expression of the receptor in the stomach. The present study identified what types of NMDA receptor subunits are expressed in the rat stomach and examined whether vagotomy affects their expression.Methods: To see the expression and distribution of the mRNAs and proteins for the NMDA receptor subunits, real-time reverse transcription-polymerase chain reaction, Western blotting, in situ hybridization, and immunohistochemistry were carried out on rat stomach with and without vagotomy.Results: Of the NMDA receptor subunits, NR1, NR2 (2A, 2B, 2C, 2D), and NR3 (3A, 3B), all NR subunit mRNAs except for the NR2B subunit mRNA were expressed in the intact rat stomach, with huge expression of NR2D mRNA. Expression of NR2D subunit mRNA and protein significantly increased after vagotomy. Increased expression was found in mucosal cells, the myenteric plexus, and the outer membrane of the stomach with vagotomy.Conclusions: Vagotomy upregulates expression of the NR2D subunit in the stomach and therefore, the NR2D subunit may be implicated in gastric motility. 41. Genital human papillomavirus infection in female university students as determined by a PCR-based method.

Abstract

The presence of genital human papillomavirus (HPV) was determined at cervical and vulvar sites using two methods, the Food and Drug Administration-approved ViraPap test and polymerase chain reaction (PCR) DNA amplification technology, in 467 women presenting to a university health service for a routine annual gynecologic examination. The PCR system afforded the sensitive detection of a broad spectrum of genital HPV types. Using PCR, we found that 46% of the study population was infected with HPV; the ViraPap test showed a prevalence of 11% infected. PCR analyses demonstrated that 69% of the HPV-positive women were infected at both genital sites. Subsequent HPV-type determination showed that 33% of the study population had HPV types 6, 11, 16, 18, 31, 33, 35, 39, 45, 51, 52, or other previously isolated types, and 13% had yet unidentified types. Almost all (92%) of the women diagnosed by Papanicolaou smear with condylomatous atypia or dysplasia (n = 12) were HPV positive. The PCR method proved to be an informative and rapid way to detect HPV in large numbers of clinical samples. 43. Analysis Using a Linear Viscoelastic Model of the in Vitro Osmotic Kinetics of Polydisperse Synthetic Colloids

Abstract

This study clarifies the contribution to overall osmotic kinetics of colloid osmotic pressure (Π) and the interaction of synthetic colloids with the membrane. Solutions (6%) of dextran with weight average molecular weight (MWw) 68 800 (DEX 70), dextran with MWw 40 000 (DEX 40), hydroxyethyl starch with MWw 70 000 (HES 70), gelatin with MWw 60 000 and albumin were tested. An osmotic flow cell fitted with membranes of molecular weight cutoff size 30 000 or 50 000 was used to measure time-dependent changes in Π for each of these solutions. A linear viscoelastic model was fitted to the curve describing changes to Π as a function of time. Values of total effective Π for DEX 40 and DEX 70 were larger than those for HES 70, gelatin, and albumin. As an index of solute−solvent exchange rate at the membrane surface, these values were in the order DEX 40 > DEX 70, HES 70 > gelatin, albumin. The findings suggest that DEX 40 may be preferable for the temporary restoration of plasma volume because of a heightened initial osmotic force. In contrast, the osmotic force exerted by gelatin is slower to increase but is likely to be longer lasting in vivo as a result of the inhibition of gelatin from penetrating the capillary membrane due to its interaction with negatively charged groups in the endothelial glycocalyx.

47. Intraoperative Awake Tracheal Intubation Using the Airway Scope™in Caesarean Section

Abstract

The Airway Scope™, a novel videolaryngoscope used for tracheal intubation, is minimally invasive and can be used in conscious patients. The parturient with a potentially difficult airway should sometimes be intubated while awake, without anaesthesia or neuromuscular block. Two pregnant women who experienced massive postpartum haemorrhage during caesarean section underwent unscheduled intraoperative tracheal intubation using the Airway Scope. They were conscious and were intubated with minimal local anaesthesia so as to prevent cardiovascular compromise. We believe the Airway Scope is useful for anaesthetic procedures in the parturient who has haemodynamic instability.
