Abstract

To compare the clinical presentation of ADHD between youth with autism spectrum disorder (ASD) and ADHD and a sample of youth with ADHD only.A psychiatrically referred sample of autism spectrum disorder (ASD) youth with ADHD attending a specialized ambulatory program for ASD ( n = 107) and a sample of youth with ADHD attending a general child psychiatry ambulatory clinic ( n = 74) were compared.Seventy-six percent of youth with ASD met Diagnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV) criteria for ADHD. The clinical presentation of ADHD in youth with ASD was predominantly similar to its typical presentation including age at onset (3.5 ± 1.7 vs. 4.0 ± 1.9; p = .12), distribution of diagnostic subtypes, the qualitative and quantitative symptom profile, and symptom severity. Combined subtype was the most frequent presentation of ADHD in ASD youth.Despite the robust presentation of ADHD, a significant majority of ASD youth with ADHD failed to receive appropriate ADHD treatment (41% vs. 24%; p = .02). A high rate of comorbidity with ADHD was observed in psychiatrically referred youth with ASD, with a clinical presentation typical of the disorder.

29. Abstract P521: Dairy Consumption and Risk of Cardiovascular Disease and Mortality in the Million Veteran Program

Abstract

Background:Dietary habits are a major determinant of cardiovascular disease (CVD) and mortality risk. However, data on the role of dairy intake on the risk of CVD and death has been inconsistent.Objective:The primary objective is to assess whether total consumption of dairy products is associated with a lower risk of CVD and/or total mortality among US Veterans.Methods:In a prospective cohort study of US Veterans from the Million Veteran Program (MVP), dairy intake was collected through a self-reported food frequency questionnaire at baseline. Incidence of composite CVD (including MI, stroke, and cardiovascular death) and total mortality were measured via electronic health records and the National Death Index database. Cox Proportional hazard models were used to compute the multivariable adjusted hazard ratio of CVD and total mortality.Results:A total of 188,833 Veterans were studied with a mean age of 63.3 ± 12.3 years. After a mean follow-up of 3.2 years for CVD and 3.3 years for total mortality, 9,898 cases of CVD and 6,826 deaths occurred. Median dairy consumption was 1.2 servings/day. In a multivariable Cox regression adjusted for age, gender, race, body mass index (BMI), smoking, alcohol use, exercise, diet quality, diabetes mellitus, and level of education; hazard ratios (95% CI) for CVD were 1.00 (0.89-1.14) for dairy intake of 1-2 servings/week, 1.05 (0.92-1.20) for 2-3 servings/week, 1.01 (0.90-1.12) for 3-5 servings/week, 0.97 (0.87-1.08) for 5-7 servings/week, 0.96 (0.87-1.05) for 1-2 servings/day, 1.02 (0.91-1.13) for 2-3 servings/day, 1.02 (0.90-1.15) for 3-4 servings/day, and 1.11 (0.98-1.26) for 4+ servings/day as compared to <1 serving/week. For total mortality, the hazard ratios across consecutive categories of dairy intake were 0.85 (0.73-0.99), 0.83 (0.70-0.99), 0.87 (0.76-0.99), 0.84 (0.74-0.96), 0.94 (0.84-1.06), 1.10 (0.97-1.25), 1.12 (1.04-1.39), and 1.36 (1.18-1.58).Conclusion:Dairy intake was not associated with composite CVD. However, there was suggestive evidence for J-shaped relation of dairy intake with total mortality.

Abstract

Introduction: Familial hypercholesterolemia (FH) phenotype is defined in part by LDL-Cholesterol (LDL-C) ≥190mg/dL, a strong risk factor for cardiovascular disease (CVD). The 2013 AHA/ACC guidelines recommend high intensity statins for primary prevention irrespective of global risk assessment. We sought to determine predictors of any statin initiation in patients with the FH phenotype within the Veteran's Health Administration (VHA). Methods: Inclusion criteria consisted of care at the VHA (2002-2014), age ≥21 years, absence of prevalent CVD, baseline LDL-C of ≥190 mg/dL, and no prior evidence of statin use. Baseline characteristics were ascertained from electronic health records for new cases of FH phenotype. We define baseline LDL-C quartiles as Q1=190-195, Q2=196-202, Q3=203-215, and Q4 >215 (mg/dL). We constructed multivariable logistic regression models to determine the odds of the initiation of any statin within 90 days of maximum LDL-C. Results: Over 12 years, we identified a cohort of 165,110 Veterans (age=53±12, men=91%; white race=74%). Statins were initiated in 88,878 (in 2002-2013=53%, in 2014=62%) of these patients. Comorbidities included smoking history (80%), hypertension (37%), diabetes (6%), chronic kidney disease (3%), and cancer (15%). In multivariable adjusted logistic regression models, significant predictors of statin initiation were older age, diabetes, hypertension, white race, smoking, baseline LDL-C quartile, and recent calendar year. Veterans with kidney disease, thyroid disease, cancer, and black race had lower odds of statin initiation (Table 1). Conclusions: A majority (62%) of the Veteran population with an LDL phenotype consistent with FH initiate statins, and they are more likely to be older, white, have a high baseline LDL-C, have hypertension and have diabetes. The odds of statin initiation have increased in recent years and this may reflect VHA changes in practice concordant with the ACC/AHA guidelines. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: Previous studies relating weight to post-stroke mortality have shown conflicting results, with some showing lower risk of mortality among overweight or obese individuals. We studied the association between BMI and post-stroke mortality in the U. S. Veteran population. Methods: We used a nationwide sample of hospitalizations in the Veteran's Health Administration (VHA) system (2002-2009) with admission ICD-9 codes for ischemic stroke in patients without a history of stroke. BMI (kg/m2) was calculated as the average of all BMI measurements 1-18 months prior to stroke. All-cause mortality was obtained from the National Death Index (NDI). Hazard ratios (HR) of mortality for standard BMI categories were estimated from Cox models with high normal weight (BMI 22.5-25) as reference. Analyses were adjusted for age, sex, vascular risk factors, PAD, cancer, dementia, COPD, smoking, CKD, and heart disease. Sensitivity analyses were performed on a subgroup without conditions that might affect weight and survival including smoking, cancer, dementia, or heart disease. The effect of BMI as a continuous measure on all-cause mortality was analyzed using splines, with BMI=23 as the reference point. Results: Of 19,409 incident stroke cases, mean age was 68±12 years, 97% were male, and average BMI was 28.8±5.9. There were 1,321 patients included in sensitivity analyses (age 64±12 years, 96% male).
