Abstract This study aims to: (1) examine the impact of cybersecurity disclosure on banks’ performance and (2) explore whether the existence of a chief risk officer (CRO), an information technology (IT) committee, and a board of directors (BOD)’ size moderates the association between cybersecurity disclosure and bank performance. The study used manual textual analysis to measure cybersecurity disclosure in a sample of listed banks in the MENA region countries based on data from 2019 to 2021. The data were collected from annual reports and financial statements of banks available at Orbis Bank Focus database. The study employed a random effect regression model to test the hypotheses and discuss the results. The findings show that banks in the MENA region are increasingly interested in disclosing cybersecurity information, where cybersecurity disclosure over the sample years is increasing from 17% in 2019 to 19.6% in 2021. In addition, the results show that cybersecurity disclosure has a positive and significant influence on bank performance. Furthermore, the findings indicate that the presence of a CRO moderates the relationship between cybersecurity disclosure and bank performance. These findings show that depending largely on a bank's CRO to handle complex and dynamic risks can have serious consequences for decision making processes connected to managing cybersecurity risk and disclosure. This paper creates a new research paradigm by focusing on the disclosure of cybersecurity information in the MENA banking sector, where exploring the moderating role of the CRO, IT committee, and board size in enhancing the cybersecurity disclosure-bank performance relationship is lacking. The findings provide practical implications for various stakeholders, where it reveals the current practices of cybersecurity disclosure of banks in the MENA region with the objective of minimizing information asymmetry, maintaining public trust, and identifying potential risks of financial distress. In addition, the results direct the attention of banks and regulators toward the role of CRO in risk governance, particularly in managing cyber risks within the banking industry.

Purpose – This study examines the correlation between ownership structure (OS) and financial reporting integrity (FRI), with emphasis on the impact of earnings quality (EQ) in the Egyptian context. Design/methodology/approach – The study uses data from 472 firm-year observations of Egyptian publicly listed companies between 2014 and 2021 and carried out descriptive statistics, correlation tests, multiple regression analysis and two-stage least squares (2SLS) to test the hypotheses. Findings – The results revealed that blockholders and institutional ownership significantly enhance reporting integrity through effective oversight and monitoring. The findings underscore the vital role of concentrated OS in overseeing reporting practices and mitigating managerial opportunism, thereby improving the transparency and reliability of financial disclosures in Egypt. Practical implications – The findings enrich the literature on corporate governance and financial reporting quality and have important implications for policymakers, regulators and corporate stakeholders. Originality/value – This work contributes valuable insights on how OS and EQ can bolster FRI, offering crucial information for combating financial crises and facilitating smooth business operations in Egypt.

The management of advanced metastasized breast cancer (BC) is a clinically challenging entity with a wide spectrum of novel therapeutics being introduced to the market. Such agents have remodeled BC treatment landscape and prolonged patients’ survival. Over the past decade, a growing body of literature has shed lights on CDK4/6 involvement in oncogenesis and the role of its inhibitors in clinical use with palbociclib being the prototype drug. We present a case of a 58-year-old post-menopausal Middle-Eastern woman diagnosed with stage IV HR+/HER2- breast cancer with extensive bone metastasis. The lesions were widely distributed across the axial skeleton including base of the skull, sternum, ribs, left iliac bone, right inferior pubic ramus, cervical, thoracic, and lumbosacral vertebrae. The patient was started on therapeutic doses of letrozole and zoledronic acid in conjunction with adjuvant radiotherapy. A significant partial response was achieved reaching 70% remission followed by sternum disease progression. A decision was made to switch letrozole for tamoxifen which resulted in disease stability. Due to postmenopausal bleeding, tamoxifen was held and letrozole was reintroduced leading to regimen failure and disease advancement. Palbociclib and fulvestrant were started accordingly, yielding a remarkable metabolic response of all bone metastatic lesions (stable disease) after three months of the regimen initiation. The aforementioned stable disease status continued for approximately three years up to this point.

Summary: Background: Primary sclerosing cholangitis (PSC) is one of the leading indications for liver transplantation in Europe, and a major risk factor for cancer in inflammatory bowel disease (IBD). However, it is not known how the epidemiology of PSC will change as that of IBD evolves. The aim of this study is to provide nationwide statistics on the past and current prevalence of PSC and IBD across England, and forecast how this is likely to change over time. Methods: We accessed and analysed a nationwide population-based administrative healthcare registry, which houses prospectively accrued data since April 1st 2001. In so doing, the past and current prevalence of PSC-IBD and IBD alone was determined among 18–60-year-olds in England, alongside average annual percentage change rates (AAPC), between the 1st of January 2015 and 2020. Past and current prevalence data, alongside trends in incidence and event-free survival rates, were then used to forecast future prevalence between 2021 and 2027. Findings: In 2015, the prevalence of PSC with prior IBD diagnosis was 5.0 per 100,000 population, rising to 5.7 when including those with IBD diagnosed after PSC. In 2020, prevalence increased to 7.6 (8.6 accounting for IBD developing after PSC), yielding an AAPC of 8.8. In 2027, PSC-IBD prevalence is forecast to be 11.7 (95% prediction interval [PI]: 10.8–12.7), and 13.3 when accounting for IBD developing after PSC (AAPC: 6.4; 95% PI: 5.3–7.5). Comparatively, the prevalence of IBD alone rose among 18–60-year-olds from 384.3 in 2015 to 538.7 in 2020 (AAPC 7.0), and forecast to increase to 742.5 by 2027 (95% PI: 736.4–748.0; AAPC: 4.7, 95% PI: 4.6–4.8). Interpretation: The rate of growth in PSC-IBD is predicted to exceed IBD-alone. Further research is needed to understand changes in disease epidemiology, including aetiological drivers of developing (invariably progressive) liver disease in IBD, and the implications of rising case burden on health care resources. Background Ulcerative colitis is an autoimmune reaction to imbalanced colonic microbiome in genetically predisposed patients. There have been four published randomised controlled trials of faecal microbiota transplant versus placebo in ulcerative colitis; three being positive. However, the methodology used varied and mechanistic data were sparse. Objective To identify an optimal faecal microbiota transplant delivery route to test in a future efficacy-powered randomised controlled trial. Design Prospective, multicentre, open-label, parallel-group, randomised pilot study. Participants were randomised in a 1 : 1 ratio to faecal microbiota transplant by nasogastric or by colonoscopy, minimised by partial Mayo score and current smoking status. Setting Three secondary care hospitals in the United Kingdom. Participants Thirty adult patients with mild to moderately active ulcerative colitis (partial Mayo score of ≥4 and ≤8). Interventions Faecal microbiota transplant samples were prepared at the University of Birmingham Microbiome Treatment Centre and administered to participants either via a nasogastric tube on 4 consecutive days repeated after a month (nasogastric) or by colonoscopy followed by 7 weekly enemas (COLON). Each treatment course was from a single matched donor. Participants and researchers underwent qualitative interviews regarding the faecal microbiota transplant experience. Main outcome measure Composite assessment of qualitative and quantitative data based on efficacy, acceptability and safety. Clinical response (primary measure of efficacy) was defined as a ≥3 point and ≥30% reduction in full Mayo score from randomisation to week 8 and a ≥1 point reduction in rectal bleeding subscore or absolute rectal bleeding subscore of 0 or 1. Results Thirty ulcerative colitis patients were randomised: 16 to nasogastric; 14 to COLON. Seven participants in the nasogastric arm and two in the COLON arm withdrew from treatment and clinical follow-up. Eight of 16 (50%) nasogastric participants were adherent compared to 12/14 (86%) COLON participants. Clinical response data were available for 20 of 30 randomised participants. Clinical response was seen in 2/8 (25%) nasogastric participants versus 9/12 (75%) COLON participants: adjusted risk ratio 2.94 (95% CI 0.84 to 10.30). Adverse events were reported in 11/16 (69%) nasogastric participants versus 11/14 (79%) COLON participants. There were three serious adverse events in two nasogastric participants. Faecal microbiota transplant was deemed an acceptable treatment for ulcerative colitis, with greater participant and clinician acceptability for the COLON route. Faecal microbiota transplant responders had a greater reduction in faecal calprotectin compared to non-responders (t-test p = 0.03) and increase in faecal microbiome diversity (Shannon diversity p < 0.01). In the whole cohort, a negative association between faecal calprotectin and diversity was seen (Kendall’s tau z = −2.8231, p = 0.004757). There were increases in faecal acetate (p = 0.05) and butyrate (p = 0.03) after faecal microbiota transplant intervention, but these were not associated with clinical response. A limitation was that this was an open-label pilot study, which was not powered to assess for efficacy of faecal microbiota transplant. Conclusions Faecal microbiota transplant delivered by the colonic route was considered safe and better tolerated than the nasogastric route. The colonic route was preferred by both participants and clinicians. Mechanistic signals indicated reduction in colonic inflammation and increased microbiome diversity in faecal microbiota transplant responders. Plain language summary Ulcerative colitis, a chronic inflammatory condition affecting the colon, results from the body’s immune response to imbalance in colonic microbes (microbiota). Faecal microbiota transplantation involves transferring processed donated stool samples from carefully screened healthy donors to ulcerative colitis patients aiming to correct the imbalance and reduce inflammation. There have been four trials comparing faecal microbiota transplant with placebo (dummy no treatment) to treat ulcerative colitis, and one which investigated faecal microbiota transplant with dietary intervention. Three of these studies showed encouraging benefit for faecal microbiota transplant. However, these studies varied in the methods used, with faecal microbiota transplant delivered either to the stomach or colon, or both. The aim of this study was to identify the best route of faecal microbiota transplant administration before undertaking a large-scale trial comparing faecal microbiota transplant with placebo. Between March 2018 and 30 April 2019 ulcerative colitis patients from three hospitals were randomly allocated to receive faecal microbiota transplant via either a tube inserted through the nose into the stomach on 4 consecutive days repeated after a month (nasogastric group; 16 participants) or by colonoscopy followed by 7 weekly enemas (COLON group; 14 participants). Faecal microbiota transplant was well accepted, with both patients with ulcerative colitis and healthcare staff stating a preference for the colonic route. Only 8 of 16 in the nasogastric group completed the study compared to 12 of 14 in the COLON group. Altogether, 9/12 (75%) in the COLON group improved compared with 2/8 (25%) in the nasogastric group. The majority of participants [11/14 (79%) COLON vs. 11/16 (69%) nasogastric] had mild, short-lived side effects following faecal microbiota transplant. There were three serious adverse events. Faecal calprotectin (indicating colonic inflammation) fell in responders and stool samples showed an increase in the number of microbial species after faecal microbiota transplant. At the end of the study, a recommendation was made by the Independent Oversight Committee to proceed to a large placebo-controlled randomised trial using the colonic route. Scientific summary Background Ulcerative colitis (UC) results from an exaggerated immune response to an imbalanced (dysbiotic) colonic microbiome in genetically predisposed patients. At the inception of this study, there had been four published randomised controlled trials (RCTs) comparing faecal microbiota transplantation (FMT) with placebo to treat UC in adults. Of these four seminal studies, three involved delivery of FMT via the colonic route and one a combination of delivery to the foregut and colon. This latter study was the only one of the four not to show a difference in efficacy compared to placebo. Foregut-delivered FMT is extensively and successfully used in clinical practice to treat Clostridioides difficile infection (CDI). From the results of the UC trials, FMT appeared to have therapeutic potential for UC. However, the methodology employed in these studies was varied and, taking into account the efficacy of FMT delivered to the foregut to treat CDI, there was uncertainty regarding the best route of delivery in the context of UC. Objectives The aims of this pilot study were as follows: to determine which FMT administration route [nasogastric (NG) or colonic (COLON)] should be investigated in a randomised double-blind, placebo-controlled trial to determine whether a full-scale RCT was feasible. In order to achieve these aims, the pilot study had the following clinical objectives. To assess the following: whether FMT by the NG route induces clinical response in patients with active UC whether FMT by the COLON route induces clinical response in patients with active UC tolerability and safety which route of FMT delivery (if any) was suitable to investigate in a full-scale RCT. The aims of the qualitative research were to assess the following: patient and clinician acceptability of FMT (NG route) patient and clinician acceptability of FMT (COLON route). The aims of the nested-mechanistic substudy were to assess the following: whether FMT by either route is associated with a change in faecal calprotectin as a surrogate marker of colonic inflammation changes in the colonic microbiome induced by FMT via each route changes in the metabolome [short-chain fatty acids (SCFAs)] induced by FMT via each route effect of diet (donors) time from stool donation to treatment. Methods In this prospective, multicentre, open-label, parallel group, randomised pilot study, adult patients with mild to moderate active UC (partial Mayo score of ≥4 and ≤8) were recruited from three centres in the UK (Birmingham, Glasgow and St Mark’s, London). Patients were randomised to receive FMT delivered either via a NG tube for delivery to the stomach (foregut-NG) on 4 consecutive days repeated after a month or by a combination of delivery through a colonoscope followed by 7 weekly enemas (hindgut-COLON). All participants underwent a treatment schedule using FMT derived, in each case, from a single donor. The primary outcome was a composite assessment of both qualitative and quantitative data based on efficacy, acceptability and safety. Clinical response (primary measure of efficacy) was defined as a ≥3 point and a ≥30% reduction in full Mayo score from randomisation to week 8 and a ≥1 point reduction in the rectal bleeding subscore or an absolute rectal bleeding subscore of 0 or 1. Patient and clinician acceptability of FMT and preference of treatment route was assessed through semi-structured qualitative research interviews. Interviews also explored reasons for adherence to treatment allocation and patients’ perspectives on FMT and trial experience. Participants were interviewed face to face or by telephone at two time points, first following the screening visit and prior to randomisation, and second after the 12-week follow-up period. Safety was assessed by the number of participants experiencing adverse events (AEs) and serious adverse events (SAEs). For the translational research, stool samples were taken from participants at baseline and then at 2 weekly intervals throughout the study to assess microbiome diversity (by 16S rRNA sequencing) and to examine the effect of FMT on faecal calprotectin. Donor stool samples were also analysed by 16S rRNA sequencing. Biopsy samples were taken from the colon at the time of endoscopy (baseline and week 8) to analyse mucosa-associated microbiome by 16S rRNA sequencing. SCFA levels in stool samples were also measured throughout the study and blood was taken from participants to look for changes in C-reactive protein (CRP) in response to FMT. Donors were asked to fill in a dietary questionnaire in order to look for associations between dietary components in their habitual diet and the efficacy of their donated FMT samples. Results Thirty patients with UC were randomised between March 2018 and April 2019; 16 to NG; 14 to COLON route. Seven participants in the NG arm and two participants in the colonic arm withdrew from the study. Clinical response was achieved in a higher proportion of participants in the colonic arm [9/12 (75%)] compared with the NG arm [2/8 (25%)]; [adjusted risk ratio 2.94 (95% confidence interval, 0.84 to 10.30)]. In the colonic arm, 12/14 (86%) participants were considered adherent compared with 8/16 (50%) in the NG arm. AEs were reported in 11/14 (79%) participants in the colonic arm and 11/16 (69%) in the NG arm. There were three SAEs in two participants in the NG arm, and none in the colonic arm. Qualitative research found a high level of enthusiasm for the trial among both patients and staff. Patients were more positive about the colonic treatment route than the NG route, both before randomisation and after the patients had been through the study. Lower calprotectin levels, implying reduced colonic inflammation, averaged over the last half of the treatment course (weeks 4, 6 and 8), were seen in the responders (N = 11, mean 508.6 mg/kg) versus the non-responders (N = 9, mean 853.6 mg/kg) (p = 0.03). Furthermore, there was a negative association between faecal microbiome alpha diversity [rarefied total operational taxonomic unit (OTU) richness] and calprotectin levels (p < 0.01). This is the first time that change in faecal calprotectin, a widely used clinical inflammatory biomarker in UC, has been reported in response to FMT in inflammatory bowel disease (IBD). With respect to SCFAs, there were significant and large increases observed in the concentration of acetate (45% increase, p = 0.05) and butyrate (57% increase, p = 0.03) across all participants (N = 19) from baseline to week 2 following FMT treatment. This effect was also observed over the course of the entire FMT treatment period from baseline to 8 weeks (N = 16, acetate 21% increase, p = 0.02; butyrate 39% increase, p = 0.08). Significant changes were not observed for the other SCFAs measured, notably propionate (2 weeks p = 0.46, 8 weeks p = 0.36). The observed increase in acetate and butyrate was not associated with clinical response, and in fact when restricted to the responder group (N = 9) changes at both 2 and 8 weeks were no longer significant, although increases were still observed. Across all participants that completed FMT to day 56, and for whom baseline and week 8 DNA was available (N = 13), there was no significant change in faecal microbiome alpha diversity, as measured by rarefied Shannon diversity of 3% OTUs (p = 0.43). However, when restricted to the colonic arm only (N = 8), there was a significant increase in alpha diversity (p = 0.01) observed after FMT. Again, for those participants in the colonic arm that responded to FMT, there was a significant increase in alpha diversity (rarefied Shannon diversity, p < 0.01) compared to those who did not. No differences were seen in alpha diversity of mucosa-associated bacteria pre and post treatment. Donor microbiome diversity, as measured by rarefied 16S rRNA gene Shannon diversity, varied substantially between donors. There was a non-significant positive association between clinical response and donor diversity (p = 0.19) with the treatment arm taken into account. In this pilot study, CRP values were lower at baseline in the colonic arm (4.2 mg/l) compared with the NG arm (11.3 mg/l). Similarly, CRP values at weeks 4, 6 and 8 were also lower in the colonic arm than in the NG arm. With regard to data derived from diet questionnaires administered to donors, higher milk and milk product intakes were associated with higher alpha diversity, but this was not statistically significant. The mean number of days from stool donation to FMT treatment was similar in participants who achieved a clinical response compared with those who did not achieve a clinical response. There was no association between time from stool donation and treatment, and clinical response when added to the primary intention-to-treat analysis model. Conclusion This pilot study suggests that in patients with active UC, FMT delivered via the colonic route appears to be safe and better tolerated with signals suggesting greater efficacy compared to the NG route. The pilot study was welcomed by patients and clinicians, and FMT appears, at least in the short term, to be safe and well tolerated. On review of these data, the Independent Oversight Committee recommended progressing to the planned placebo-controlled randomised trial, which would be powered to assess efficacy, and would have enabled more detailed mechanistic studies to assess for strains engrafting from donors to recipients, which are associated with clinical response. Patients remain very interested in FMT as a potential treatment for UC as this microbiome manipulation affords an alternative therapy to immune system suppression as a means to treat UC. The trial management group involved in this pilot believe that further studies to assess the efficacy of FMT in UC with a focus on understanding mechanisms are warranted as the first step towards developing precision live biotherapeutics for IBD. The aim of the present quantitative assessment of the phytoplankton community along the Garmat-Ali River and to establish its relationship with the physical and chemical factors based on the changes occurring in the aquatic environment in Basra Governorate, including the decreasing of fresh water, this is due to the low discharge of fresh water coming from the upstream countries, rise in temperatures due to global warming, increased salt concentrations and other factors. Four locations were chosen in the region: Al-Najibiya, Al-Mashhab, Al-Salal and Al-Nagara station. The samples were collected quarterly in 2022. The results of the environmental variables showed that the water temperature was ranging from 11.8 - 33.4 °C, salinity from 2.6 - 9.5 PSU, hydrogen ion concentration -pH- from 7.3-8.6, dissolved oxygen from 1.6-11.7mg/l. total dissolved solids from 2350-9871 ppm, turbidity from 1.39-76.8 NTU, reactive phosphate from 0.01-0.21 mg/l. reactive nitrate from 0.51-19.9mg/l. and chlorophyll-a-from8.62-41.06mg/m3. Statistical analysis of the physical and chemical parameters was performed using SPSS statistics v.19 for one-way analysis of variance. Quantitative data showed that the total numbers of phytoplankton at all the study sites and in all seasons was 120120 cell/l. Al-Salal site showed the highest numerical density of 15,000 cell/l. in spring, while Al-Najibiya site represent the lowest numerical density of 2,500 cell/l. in winter.

Abstract The objectives of this study are to (i) examine the relationship between accounting conservatism and climate-related risk disclosure (CRRD) in the context of Egypt, and (ii) look into the moderating role of earnings quality (EQ) in such a relationship. To operationalize CRRD reported by Egyptian firms listed in the SP/EGX ESG index across the years from 2018 to 2022, the study uses a manual content analysis as a base for analysis and employs two (OLS) regression models to test the study’ hypotheses. EQ is measured using the cross-sectional modified Jones model, while accounting conservatism is gauged using the C-Score. Additionally, endogeneity checks in robustness tests are performed using system GMM. The findings show no impact of CRRD on the conservative accounting practices of Egyptian firms. However, there is a sizable adverse effect when EQ is used to moderate this association. This finding is consistent with the notion that firms report high levels of climate impacts and have fewer information asymmetry issues. This paper extends disclosure indexes by highlighting the frequent keywords of CRRD used in the annual narrative reports of Egyptian firms. Hence, this study is among the first to explore the moderation impact of EQ on the relationship between CRRD and accounting conservatism in the Egyptian setting.

Purpose – This paper aims to examine the impact of environmental, social and governance (ESG) disclosure and firm value (FV), as well as, pinpoints the role of the audit quality (AQ) as a moderating variable on such impact; where the authors hypothesize that AQ modulates the relationship between ESG disclosure and the FV. Design/methodology/approach – Data of a sample of firms listed on the Egyptian Stock Exchange Market (EGX) were collected over the period of 2017–2021 and analyzed using the regression and 2SLS models. Findings – The results suggested that: (1) the ESG has a significant positive impact on the FV in the EGX, and (2) AQ has a significant impact, as a moderating variable, on the relationship between ESG disclosure and FV. Research limitations/implications – The findings would help the Egyptian market authorities in realizing the importance of integrating ESG information within the financial reports of the listed firms. The findings could also help in developing effective disclosure procedures to provide shareholders with useful information. Originality/value – This paper contributes to the literature regarding the ESG disclosure components and the FV value by considering AQ in testing such relationship.

Abstract This paper aims at investigating the effect of board of directors attributes on real earnings management (REM). A panel data of 78 Egyptian listed companies was collected over the period 2008–2017 to test the hypotheses. The results of the system generalized method of moment model revealed that the board size is negatively and significantly correlated with REM proxies, except for abnormal cash flows from operations (ABCFO) measure. Whereas, board meetings are positively and significantly related to REM except for ABCFO. Furthermore, board independence and chief executive officer duality provided varying results due to different REM proxies that have been used in this paper. The results of this study highlight the fact that there is no unified corporate governance (CG) system that fits all countries; therefore, each country should form its CG code in a way that takes into consideration its economic, political, legal, and institutional needs. Furthermore, regulators have the motivation to enhance relevant regulations and rules and maintaining a well-organized regulation system, where this would help in improving the effectiveness of the board as well as protect the investors by reducing the level of earnings manipulation. In investment activities, investors should take into account the attributes of a company’s board to avoid investing in firms that are more liable to conduct earnings management; consequently they could maximize the benefits of investments.

Abstract Background and Aim To evaluate the demographic and prognostic significance of gastrointestinal (GI) symptoms in patients with coronavirus disease 2019 (COVID‐19). Methods A systematic search of electronic information sources was conducted. Combined overall effect sizes were calculated using random‐effects models for baseline demographic factors and outcomes including mortality, intensive care unit (ICU) admission, and length of hospital stay. Results Twenty‐four comparative observational studies reporting a total of 51 522 COVID‐19 patients with (n = 6544) or without (n = 44 978) GI symptoms were identified. The patients with GI symptoms were of comparable age (mean difference [MD]: 0.25, 95% confidence interval [CI] −2.42 to 2.92, P = 0.86), rate of pre‐existing hypertension (odds ratio [OR]: 1.11, 95% CI 0.86–1.42, P = 0.42), diabetes mellitus (OR: 1.14, 95% CI 0.91–1.44, P = 0.26), and coronary artery disease (OR: 1.00, 95% CI 0.86–1.16, P = 0.98) compared with those without GI symptoms. However, there were significantly more male patients in the GI symptoms group (OR: 0.85, 95% CI 0.75–0.95, P = 0.005). The presence of GI symptoms was associated with similar risk of mortality (OR: 0.73; 95% CI 0.47–1.13, P = 0.16), ICU admission (OR: 1.15; 95% CI 0.67–1.96, P = 0.62), and length of hospital stay (MD: 0.43; 95% CI −0.73 to 1.60, P = 0.47) when compared with their absence. Conclusion Meta‐analysis of the best possible available evidence demonstrated that GI symptoms in COVID‐19 patients do not seem to affect patients with any specific demographic patterns and may not have any important prognostic significance. Although no randomized studies can be conducted on this topic, future high‐quality studies can provide stronger evidence to further understand the impact of GI symptoms on outcomes of COVID‐19 patients.

Abstract This paper aims to examine the moderating effect of the narrative risk disclosure quality on the association between firm performance and the cost of equity capital in the Egyptian setting. Manual content analysis and factorial principal component techniques are used to quantify the quality dimensions of the narrative risk disclosures. The weighted average cost of equity is used to estimate the firms’ costs of equity. A cross-sectional analysis was conducted over three years (2018–2020) for a sample of 73 non-financial firms listed on the Egyptian Stock Exchange (EGX100). Multiple OLS regression models are employed to test the hypotheses. The results reveal a negative association between firm performance and the cost of equity and, while such association strengthened when adding the narrative risk disclosure quality as a moderator variable. This suggests that risk disclosure is important to stockholders’ investment decision-making in the Egyptian context. Based on the dearth of literature related to the economic reverberations of narrative risk disclosure quality in emerging economies, this study contributes to the risk reporting literature by providing evidence on the moderating effect of the narrative risk disclosure quality and its reverberations on the firm’s cost of equity capital in one of the emerging economies as Egypt. With regard to the findings of this study, we expect to contribute to the practice and theory by providing new and different insights about the moderating effect of narrative risk disclosure on the association between firm performance and cost of equity capital.

One male specimen of the species Metapenaeus stebbingi Nobili, 1904 was first recorded in November 2021 from the Iraqi territorial waters, Northwest of the Arabian Gulf. Therefore, the investigated shrimp species nominally corresponds to the original description of M. stebbing through the main diagnostic features. It must be adopted in the spread and expansion of species, a new record of the area, and a distinctive addition to the species of commercial shrimp within the Iraqi marine waters.

Abstract This paper aims to (i) investigate the impact of political connections (PC) and related party transactions (RPTs) on the firm value (FV), and (ii) test the moderating effect of RPTs on such relationship, while controlling for the firms’ corporate governance mechanisms. Based on 315 observations from publicly listed firms in Egypt, our results reveal that (i) there is a positive relationship between PC and the FV, (ii) there is a negative relationship between RPTs and the FV, and (iii) the existing of RPTs as a moderating variable enhances the impact of the politically connected companies on the FV. The findings suggest that the Egyptian firms are much affected by the politically connected board of directors or owners, and this significantly exists when associated with related party transactions, despite that, the corporate governance practices could mitigate such effects.

Summary: Background: Faecal microbiota transplantation (FMT) has previously been explored as a treatment for ulcerative colitis (UC) however, biomarkers that predict and / or are associated with clinical response are poorly defined. The aim of this systematic review was to identify donor and recipient clinical, microbial and metabolomic predictive biomarkers of response to FMT in UC. Methods: A systematic search of the relevant literature of studies exploring FMT in UC was conducted. Data on microbial diversity, taxonomic changes, metabolic changes, donor and recipient microbiota relationship and baseline predictors was examined. Findings: 2852 studies were screened, and 25 papers were included in this systematic review. Following FMT, alpha diversity was seen to increase in responders along with increases in the abundance of Clostridiales clusters (order) and Bacteroides genus. Metabolomic analysis revealed short chain fatty acid (SCFA) production as a marker of FMT success. Donors or FMT batches with higher microbial alpha diversity and a greater abundance of taxa belonging to certain Bacteroides and Clostridia clusters were associated with clinical response to FMT. Baseline clinical predictors of response in patients with UC included younger age, less severe disease and possibly shorter disease duration. Baseline recipient microbial predictors at response consisted of higher faecal species richness, greater abundance of Candida and donor microbial profile similarity. Interpretation: Distinct changes in gut microbiota profiles post-FMT indicate that certain baseline characteristics along with specific microbial and metabolomic alterations may predispose patients towards a successful therapeutic outcome. Opportunities towards a biomarker led precision medicine approach with FMT should be explored in future clinical studies. Funding: There no specific funding to declare.

Toxic substances have a deleterious effect on biological systems if accrued in ecosystems beyond their acceptable limit. A natural ecosystem can become contaminated due to the excessive release of toxic substances by various anthropogenic and natural activities, which necessitates rehabilitation of the environmental contamination. Phytoremediation is an eco-friendly and cost-efficient method of biotechnological mitigation for the remediation of polluted ecosystems and revegetation of contaminated sites. The information provided in this review was collected by utilizing various sources of research information, such as ResearchGate, Google Scholar, the Scopus database and other relevant resources. In this review paper, we discuss (i) various organic and inorganic contaminants; (ii) sources of contamination and their adverse effects on terrestrial and aquatic life; (iii) approaches to the phytoremediation process, including phytoextraction, rhizoremediation, phytostabilization, phytovolatilization, rhizofiltration, phytodegradation, phytodesalination and phytohydraulics, and their underlying mechanisms; (iv) the functions of various microbes and plant enzymes in the biodegradation process and their potential applications; and (v) advantages and limitations of the phytoremediation technique. The reported research aimed to adequately appraise the efficacy of the phytoremediation treatment and facilitate a thorough understanding of specific contaminants and their underlying biodegradation pathways. Detailed procedures and information regarding characteristics of ideal plants, sources of heavy metal contamination, rhizodegradation techniques, suitable species and removal of these contaminants are put forward for further application. Scientists, planners and policymakers should focus on evaluating possible risk-free alternative techniques to restore polluted soil, air and water bodies by involving local inhabitants and concerned stakeholders.

Pampus argenteus (Euphrasen, 1788) is one of the most commercially important fish in the North Gulf region, Iraq in particular. It's stocks were critically endangered. The study showed the semi-artificial spawning. The mature brooders were collected from the region corresponding to the Khor Al-Zubair. The semiartificial spawning was done by mixing the sperm and ova of the live fish. It was found that fishes collected in the afternoon were most suitable for semi-artificial spawning. This research was able to diagnose the place and time of spawning fish, and time getting semen and its preservation, time to get mature eggs, access to the mechanism of action and necessary equipment. In addition to planning for subsequent phases of fertilized eggs rearing, hatching and larval rearing.

Despite being known about for more than 30 years, finding an effective therapeutic strategy against it remains a challenge.Aim: There are no US studies evaluating the efficacy of a Levofloxacin based therapy for H. pylori infection. We here intend to study the efficacy of Levofloxacin based triple antibiotic regimen as compared to Clarithromycin based triple therapy and Bismuth based quadruple therapy in our patient population.Methods: This is a retrospective single center observational study. Patients with Helicobacter pylori infection who underwent treatment for H. pylori with one of the three therapies, i.e. Clarithromycin triple, Bismuth Quadruple or Levofloxacin triple, were included in the study and the eradication rates were compared. The confirmation of the H. pylori was done 4 weeks after the completion of anti-microbial therapy.Results: A total of 177 individuals underwent the H. pylori treatment in our retrospective review. Of these, 54% (n=97) of patients were treated with Clarithromycin based triple therapy (Group 1), 35% (n=63) were treated with Levofloxacin based regimen (Group 2), and the remaining 11% (n=17) were treated with Bismuth based quadruple therapy (Group 3). The eradication rates were significantly higher in patients treated with Clarithromycin based triple therapy as compared to Levofloxacin based triple therapy and Bismuth quadruple therapy (78.3% vs 49.2% vs 41.1% P=0.001).Conclusion: In conclusion, our study shows significantly lower eradication rates with Levofloxacin triple therapy among a selected US population. Thus, it may not be a good first-line therapy among this US population and the Clarithromycin based regimen may still be used successfully.Keywords: Helicobacter pylori, clarithromycin, levofloxacin, quadruple therapy, peptic ulcer

This study includes an experimental hatchery design for the oriental river prawn Macrobrachium nipponense from the brackish water of Basrah region (Al-Mashab Marsh). This hatchery consist of five parts: maturation ponds (closed system), hatching of zoug jars, rearing of ponds to larva (nursery and concrete ponds), water tanks and food live of pond (fiberglass). The production capacity was 53,504-190,000 Zoea larvae, 32,355-99,560 Postlarvae. While production of the out ponds (cage two in mud and fiberglass ponds) were estimated between 4,347-24,098 larvae and 2,527-18,828 Postlarvae. Depending on results the total area of the earthen pond and the fiberglass tank, the potential production of Zoea larvae were estimated to be between 36,559-15,300,000 larvae, and between 21,248-11,954,400 Postlarvae. While the total production in all culture systems were 12,600,188-15,760,462 larvae, and between 9,753,349-12,197,630 Postlarvae.

The feeding behavior experiment showed that the juveniles of Macrobrachium nipponense mostly and frequently occurred on the artificial net covered wall of the experimental tanks and in all other result feeding of juvenile Macrobrachium nipponense was pelagic and benthic during 2 and 24 hour, in three parts of pond (net wall- A, water column- B and bottom-C) on bottom of ponds smooth and sand-clay, also feeding on pallet. Also showed near of found ratio was 80%, 9%, 11% and 73%, 12%, 14% during 2 hour in parts of pond of smooth and sand-clay respectively, but during 24 hour 72%, 12%, 16% and 67%, 12%, 21% in parts of pond of smooth and sand-clay respectively.

Shrimp Latreutes anoplonyx sample were collected from the northwest Arabian Gulf that found symbiotic interaction with the Medusa white Rhizostoma sp.. The diagnostic feature of the species is the present of one tooth on dorsal region of the carapace. The rostrum longer than antennular and flagella. The rostrum in the male is curved to down ward above the eye, while in the female more wider and curved upward. the teeth found on both sides of the rostrum, about 9 – 16 on dorsal side and very high and smooth, while in ventral side they were between 7 – 10 and more distance in the mid and high density in the top. The largest for female was 4 cm in length and the average shrimp lengths were between 1.2 – 4.0 cm.

The literature has mixed findings regarding the impact of climate change on agricultural production, probably due to different choices of dependent variables. Based on panel data analysis of 140 countries from an extended period, i.e., 1961 to 2013, this study applies the explicit method of using TFP as the dependent variable, but also delivers estimations with the implicit approach of using agricultural output as the dependent variable, so as to be able to directly compare the results with the mainstream trends in the existing literature. We found that using agricultural TFP as the dependent variable generates more consistent and robust results. We also found a strong negative impact of climate change on agricultural productivity. A one-unit downward deviation of precipitation (i.e., drought) and one unit of upward deviation of temperature (i.e., global warming) decrease the TFP by 0.530 and 0.494, respectively. As we have one of the largest sample sizes when it comes to studying the effect of climate change, we carried out an exploration by dividing the sample into different categories of developed vs. developing countries and cold vs. warm climates, as well as by splitting the time series into two distinct periods. We found that climate change has had a heterogeneous impact on countries with different development levels, with negative impacts on developing countries and positive effects on developed countries, which leads to the rising concern of the impact of climate change on inequality. This heterogeneity and the uneven impact found in this study strongly suggest the need for countries to work together to coordinate and mitigate these adverse effects rather than to adapt to the situation by focusing on the input variations, as the latter will increase the inequality effect of climate change and add to an already unstable global environment.

The globalization of businesses has induced the globalization of boards, and to conform to this trend in the globalization of boards, Chinese firms have had to hire foreign independent directors (FID) as a counterpart to domestic independent directors (DID). Agency theory suggests that an FID contributes to a firm’s financial performance by monitoring and advising the firm’s management. This article explores whether the FID indeed predicts the financial performance of the firm as output and input. Output financial performance refers to the return on investment, and input performance refers to the attraction of foreign investors to the firm after hiring the FID. The evidence from the top 200 Chinese firms shows that the FID does not affect the firm’s financial performance in terms of ROA (return on assets) or ROI (return on investment). However, the FID positively correlates with foreign investment, suggesting that the FID outperforms the DID in attracting foreign investment. These findings point to an alternative explanation, one of which aligns with institutional theory and isomorphism. Institutional theory explains that FID appointments serve conformance rationality more than they serve technical performance rationality. Instead of financial performance pressure, normative conformance pressure influences Chinese firms to adapt to global norms and standards. Such memetic practices support their search for international legitimacy.

This study is based on rearing of juveniles of the shrimp Metapenaeus affinis collected from shatt Al-Ararb in Garmmat Ali river. The period of juveniles existence during the study was found extend from November to July in 2008. During the catch and the transportation of shrimps from the field to laboratory in rearing ponds. Several essential experiments were conducted aiming to understand the feeding habit and food preference of juveniles. Three temperatures were tested (15, 20 and 25 °C) for food consumption. In both cases (i.e. the live food Artemia franciscana and the artificial diet), the food consumption was highest at 25 °C. However, direct increase was found between temperatures and food consumption at this temperature. The elementary canal fullness and was excretion at the three temperatures was examined too. The time (minutes) was found to be shorter when shrimps feed on A. franciscana (live food) compared with the artificial diet, the value were 51.4 - 194.15 for A. franciscana between 15 °C and 25 °C, while were 56.6 - 202.05 for artificial diet at similar that temperatures.

Result disclosure of clinical trial posts a conflicting logic between private secrecy and public interest. Despite ethical and legal requirements for disclosing clinical trial results, clinical trials’ sponsors tend to withhold the results. We explored the location, timing, and rationale behind the withheld clinical trial results. Based on the entrepreneurial orientation (EO) perspective, we propose that organizational EO contingencies moderate the disclosure decision. We used the completed clinical trial projects in China by foreign and domestic sponsors. First, we found that a unit increase in the sponsor’s experience can increase the disclosure about 1.01 times. Second, we found that industrial enterprises disclose results about 3.7 times more than universities do. Third, we found that foreign clinical trial projects in China tend to disclose 3.9 times more than domestic projects. We link these findings to two types of audience. First, we inform the academic community on the theory and empirics regarding risk-taking behavior in the biopharmaceutical industry’s clinical trial activity. Second, we address the general audiences concerned about the ethical and socioeconomic wellbeing of the public.

South Korea imports a large amount of agricultural and aquatic food products from China, which meets its food security. However, the import from China raises food safety questions, leading to food safety apprehension. We explored the source of the Korean consumer’s apprehension. Based on the apprehension reduction theory (ART) developed from interviews with Korean consumers in the first stage of the study, we conducted a survey to assess the social media as an indirect source of information and direct experience of the consumer in the second stage of the study. We received 504 responses, of which 1/3 of the respondents had visited China in the last year. Using FSS (Food Safety Satisfaction) as the dependent variable (1— low to 5— high ), we link information from the social media vis-à-vis direct experience and made three discoveries. (a) The information quantity of social media increases the consumer’s apprehension, partially refuting the ART. (ii) FSS increased in response to information flow from the direct experience of the consumer with Chinese imported food. (c) The direct information from experience mediates the effects of indirect information (social media) on apprehension about agricultural and aquatic product imports. We made three inferences. First, information quantity and quality have separated roles in the ART. Second, social media increases the free-market style information flow, turning legitimate products to illegitimate and vice versa. Third, the collective irrationality from the information quantity needs institutional bricolage to legitimize the chaotic nature of the untamed information.

Recruiters and researchers of TMT (top management teams) tend to emphasize the human capital and social capital of the executive in the interfirm migration, but they ignore the role of the former employer’s prestige. We address this issue to argue that the former employer’s organizational prestige attracts the recruiter’s attention to the TMT, and we propose that the TMT from high-prestige organization attracts high rewards than the TMT from a low-prestige organization. We used data from the biotechnology sector on 1,468 TMTs, their 1,482 hiring events, 783 recruiters, 168 source employers—in the interfirm movement of the TMTs from 1997 to 2005. We used three measures for predictors of the organizational prestige of the former employer, and the regression analysis shows some reflections of those predictors of rewards of the hired executive. The result shows that the organizational age, size, and alliance network of the former employer emit value signals to the recruiter. These components of organizational prestige predict increase in the rewards after controlling for the human capital of the TMT. The study contributes to organizational context as a predictor of value, institutional theory, and general implications for practice and policy.

Earthwork construction involves excavation, hauling, placing and compaction of soil, gravel, and other materials that exist on the soil surface. The soil volume varies depending whether the soil is bank, loose or compacted material. Therefore, the final generated volume of earthwork should be adjusted for volume changes during the above states by applying shrinkage and swell-correction percentages. Estimating these correction factors by engineering expertise or selecting predetermined tables without extensive knowledge of the local soils has proven to be costly and may be misleading. Accordingly, the current study was initiated to develop the database of swell/ shrinkage percentages for various soil materials and link them to other soil properties. Standard procedures were applied for determining soil density at different states along with the physical, chemical and geotechnical properties for soils obtained from 39 surveyed projects within and on the outskirts of Erbil city. The obtained data were subjected to different statistical analysis and the results indicated that the swell percentage ranged from 36.10 -55.7% for clays, 18.40 – 69.20% for silts and 11.90 – 54.5% for gravels. Shrinkage percentage ranged from 9.20 – 16.5% for clays, 4.40 – 20.20% for silts and 0.80 - 23.5% for gravel. Overall, within each group, the swell percent was superior to the shrinkage percent. Additionally, the swell percent was characterized by having a higher coefficient of variation compared to that of shrinkage percent. The in situ soil density and clay content have emerged to be the most effective soil properties for predicting swell percent. On the other hand, the influential variables for predicting shrinkage percent were in situ soil density and the maximum dry density. The mean absolute error of prediction of swell and shrinkage percentages were 6.79 and 1.17 respectively, indicating that shrinkage percent can be predicted more accurately compared with swell percent.

The inter-temporal and inter-spatial institutional change in China have precipitated the internationalisation trends in Chinese higher education, leading to Sino-foreign inter-university alliances. Does the institutional development reflect on the contractual duration differences before and after these institutional changes? This article addressed to answer the duration decision. The inter-temporal institutional development measures the (i) WTO membership and (ii) educational policy in 2003. The inter-spatial institutional change represents the national cultural differences based on indices. The analysis of 717 alliances between 327 foreign partners and 262 Chinese universities shows support for both types of institutional developments. The results on inter-temporal institutional change show that the post-institutional change contractual duration has decreased compared to pre-institutional change. The result of the inter-spatial institutional differences shows mixed results. Power distance predicts longer duration. Likewise, masculine cultures, compared to the feminine culture, show a longer duration of the alliance contract. Instead, the long-term orientation (marginally significant) shows decreased duration. Individualist and uncertainty avoidance cultures show no significant difference, which may be due to the policy changes that have accounted for the individualist and uncertainty avoidance cultures.

Specimens of the snapping shrimp Alpheus edwardsii, were collected from the intertidal zone of Faw and Shatt Al-Basrah channel in Basrah city, Iraq during October 2016. Photographs by digital camera and measurements were presented. Guides and keys were used for identification of this species.

Specimens of the The Brachyuran crab Atergatis roseus (Ruppell, 1830), were collected for first times from Iraqi coast, south Al-Faw, Basrah city, Iraq, in coast of northwest of Arabian Gulf. Morphological features and distribution pattern of this species are highlighted and a figure is provided. The material was mostly collected from the shallow subtidal and intertidal areas using trawl net and hand.

Faecal microbiota transplant (FMT) has now been established into clinical guidelines for the treatment of recurrent and refractory Clostridioides difficile infection (CDI). Its therapeutic application in inflammatory bowel disease (IBD) is currently at an early stage. To date, there have been four randomized controlled trials for FMT in IBD and a multitude of observational studies. However, significant gaps in our knowledge regarding optimum methods for FMT preparation, technical aspects and logistics of its administration, as well as mechanistic underpinnings, still remain. In this article, we aim to highlight these gaps by reviewing evidence and making key recommendations on the direction of future studies in this field. In addition, we provide an overview of the current evidence of potential mechanisms of FMT in treating IBD.

Small family businesses (SFBs) encounter disruption during the intergenerational succession unless understood and managed effectively. Even before the succession process begins, the founder’s apprehension about the succession rises to a critical level, and yet a little research has dealt with this issue. We address the issue of the founder’s apprehension through this qualitative study by tracing the causes, contexts, and contours through the accounts of the founder in Thailand. We used 18 in-depth interviews with founders whose business types, their intergenerational succession planning, and regional contexts had similarities. A bricolage between family business as a rational device and a social device reveals whether and how the founder’s mental structures and situated-attention reflected on the focal concept of “apprehension.” We note several findings. First, a combination of cognitive scripts and situated attention altered the founder’s identity vis-à-vis the heir. Following from the functional, relational, locational, temporal, and structural narratives, the founder’s interpreted distance from that of the heir suggests that the discretionary power of the founder varies. Second, this variation translates into apprehension in an order. Third, based on the order of the distance between the founder and heir, the functional and structural narrative take the first and second positions. Third, theoretically, we link the functional context to cognitive and structural context to normative perspectives.

The rainfed region in Iraq comprises an area of more than 5 million ha of forest, grazing and farmland areas. Except the plains, the region suffers from moderate to severe water erosion due mainly to overgrazing and land mismanagement. Due to population growth and the shortage in water resources, an expansion in land used for agriculture in the region is expected. Terracing is an option when utilizing sloping land for agricultural production. A terrace design criterion was developed for the region in which terrace spacing was determined using the Revised Universal Soil Loss Equation (RUSLE); terrace channel specifications were determined using conventional hydraulic computations. Analyses showed that terracing is feasible on rolling and hilly sloping land in the high rainfall zone (seasonal rainfall >600 mm) where economic crops are grown to offset the high cost of terrace construction and maintenance. In the medium and low rainfall zones (seasonal rainfall 400–600 mm and 300–400 mm), terracing for water erosion control is generally not needed on cultivated land less than 10% in slope where wheat and barley crops are normally grown; however, pioneer research projects are needed to assess the feasibility of terraces of the level (detention) type to conserve rain water in these two zones for a more successful rainfed farming venture.

Specimens of grapsoid crab Metaplax indica were collected from the intertidal zone of the lower reaches of Shatt Al-Arab at Fao region, NW of the Arabian Gulf , Basrah, Iraq 2012. A note on the morphological features of this species and a photograph is provided to confirm the identification of the crab.

In this study, we evaluated whether the quality of bowel preparation differed significantly among populations with a high risk of CRC compared with that among the general population after matching for potential confounding factors.Methods: Hispanic and African American patients who underwent routine screening or surveillance colonoscopies in an outpatient setting between 2003 and 2013 were included in this retrospective study. Patients who underwent colonoscopies for emergent indications and repeat routine screening colonoscopies because of prior history of inadequate bowel preparation were excluded from this study. The patients were divided into three groups: patients having an average risk of being diagnosed with CRC (group 1); patients having a high risk of being diagnosed with CRC because of a personal history of adenomatous polyps (group 2); and patients having a high risk of being diagnosed with CRC because of a family history of CRC in first-degree relatives (group 3). All the patients were given preprocedural counseling and written instructions for bowel preparation. Data on demographic information, method of bowel preparation, quality of bowel preparation, comorbidities, and prescription medications were collected.Results: In all, 834 patients had a "high-risk for CRC" surveillance colonoscopy in view of their personal history of adenomatous polyps and were included in group 2. In total, 250 patients had a "high-risk for CRC" screening colonoscopy in view of their family history of CRC in first-degree relatives and were included in group 3. Further, 1,000 patients were selected to serve as controls (after matching for age, sex and ethnicity) and were included in group 1. Bowel preparation was graded as good, fair, or poor by the endoscopist performing the study. We observed a significantly higher number of good bowel preparations in group 2 and group 3 (P=0.0001) when compared with group 1 (controls) after adjusting for comorbidities and usage of prescription medication that could potentially cause colonic dysmotility. These differences were significant in both Hispanic and African American patients.Conclusion: Our study showed that perception of CRC risk significantly influenced the bowel preparation behaviors of patients belonging to minority populations, with a significantly greater number of patients with a high risk of CRC having adequate bowel preparations. Understanding animal movements is an important factor for the development of meaningful surveillance and control programs, but also for the development of disease spread models. We analysed the Danish pig movement network using static and temporal network analysis tools to provide deeper insight in the connection between holdings dealing with pigs, such as breeding and multiplier herds, production herds, slaughterhouses or traders. Pig movements, which occurred between 1st January 2006 and 31st December 2015 in Denmark, were summarized to investigate temporal trends such as the number of active holdings, the number of registered movements and the number of pigs moved. To identify holdings and holding types with potentially higher risk for introduction or spread of diseases via pig movements, we determined loyalty patterns, annual network components and contact chains for the 24 registered holding types. The total number of active holdings as well as the number of pig movements decreased during the study period while the holding sizes increased. Around 60-90% of connections between two pig holdings were present in two consecutive years and around one third of the connections persisted within the considered time period. Weaner herds showed the highest level of in-loyalty, whereas we observed an intermediate level of in-loyalty for all breeding sites and for production herds. Boar stations, production herds and trade herds showed a high level of out-loyalty. Production herds constituted the highest proportion of holdings in the largest strongly connected component. All production sites showed low levels of in-going contact chains and we observed a high level of out-going contact chain for breeding and multiplier herds. Except for livestock auctions, all transit sites also showed low levels of out-going contact chains. Our results reflect the pyramidal structure of the underlying network. Based on the considered disease, the time frame for the calculation of network measurements needs to be adapted. Using these adapted values for loyalty and contact chains might help to identify holdings with high potential of spreading diseases and thus limit the outbreak size or support control or eradication of the considered pathogen.

Vinaya Gaduputi, Molham Abdulsamad, Sailaja Sakam, Naeem Abbas, Hassan Tariq, Ariyo IhimoyanBronx Lebanon Hospital Center, Department of Medicine, Bronx, NY, USABackground: The aim of this study was to investigate whether systemic vascular resistance (SVR) correlates with validated prospective scoring systems such as Model for End-stage Liver Disease (MELD) and its modifications.Methods: Patients with cirrhosis, who were admitted to hospital with decompensation (as defined by development of ascites, hepatic encephalopathy, and variceal bleeding) and underwent echocardiography were included in this study. Laboratory data required for computing MELD score, serum bilirubin, serum creatinine, international normalized ratio, and serum sodium were collected for every patient. We tabulated hemodynamic and echocardiography parameters that enabled calculation of SVR. We analyzed the correlation between SVR and each of the individual prognostic scores.Results: A total of 771 patients with a diagnosis of decompensated cirrhosis were included in the study. Two hundred and sixty-two patients were found to have a low sodium level (135 mEq/L). In the patients with hyponatremia, we found statistically significant inverse correlations between SVR and validated liver severity models. However, these correlations were not seen in patients with normonatremia.Conclusion: We observed a statistically significant inverse correlation between SVR and all the validated liver disease severity models used in this study among patients with hyponatremia but not in those with normonatremia. It is clear from epidemiological studies that excess iron is associated with increased risk of colorectal cancer; however, questions regarding the mechanism of how iron increases cancer risk, the source of the excess iron (circulating or luminal), and whether iron reduction represents a potential therapeutic option remain unanswered. In this study, we show that after Apc deletion, the cellular iron acquisition proteins TfR1 and DMT1 are rapidly induced. Conversely, restoration of APC reduces cellular iron due to repression of these proteins. To test the functional importance of these findings, we performed in vivo investigations of the impact of iron levels on intestinal tumorigenesis. Strikingly, depletion of luminal (but not systemic) iron strongly suppressed murine intestinal tumorigenesis, whereas increased luminal iron strongly promoted tumorigenesis. Taken together, our data definitively delineate iron as a potent modifier of intestinal tumorigenesis and have important implications for dietary iron supplementation in patients at high risk of colorectal cancer.

Management of 3794 gunshot wounds of head at disaster scale, who presented to our emergency room in groups continuously over a period of 20 years, by the Department of Neurosurgery at Sher-I-Kashmir Institute of Medical Sciences (SKIMS) Kashmir, India, from September 1988 to November 2008, revealed an overall mortality of 87.69% (3327 out of 3794). Patients were triaged in emergency CT-room. Most of the victims were males. Most of the deaths 79.14% (2633 out of 3327) occurred within 30 minutes of the patient’s arrival to the hospital and only 694 patients lived beyond one hour of arrival. Out Of total (3327) deaths, 2844 patients had admission GCS score of 3 and all of these died. No deaths occurred in the group of patients with GCS score 9-15. Poor and delayed mode of transportation accounted for 89.2% (2133 out of 2391) deaths. Elderly patients above 41 years of age in both sexes had poor outcome. 45 Children (below 18 years) were injured with a mortality of 42.2% (19 out of 45), attributing 0.57% (19 out of 3327) to total deaths. Surgical mortality was 30.9% (181 out of 585) and un-operative patients had 42.2% (46 out of 109) mortality. About 65.87% (276 out of 419) patients with admission-GCS score 4 to 8 were surgically salvageable. Predictors of poor outcome were low admission GCS score, fixed and dilated pupils, poor and delayed mode of transportation, hemodynamic instability, abnormal breathing at admission, coagulopathy and disseminated intravascular coagulation (DIC), CT visualisation of subarachnoid hemorrhage (SAH), ventricular hemorrhage, midline shift, bihemispheric and multilobar injuries and scalp wounds at occipital, temporal and frontal areas. However retained missile and bone fragments were less harmful than retained wooden (pulped mulberry stem) and cardboard wads and pneumocephalus in causing infections, cortical atrophy and seizures in the long run.
