Treatment of Central Pain Syndrome (CPS) is known to be extremely challenging. Current therapies are unsatisfactory as patients report only mild to moderate pain relief. We report a case of using ketamine as a patient-controlled analgesia (PCA) for the treatment of CPS. A 58-year-old male with CPS presented with severe generalized body pain refractory to multiple pharmacological interventions. He was started on a basal infusion rate at 0.3 mg/kg/h with a ketamine PCA bolus of 10 mg with a 10-minute lockout period. Over the next 7 days, the basal infusion rate was titrated up to 2.1 mg/kg/h relative to the number of times the patient pressed the PCA. At the end of the trial, the patient reported 0/10 pain with lightheadedness on the first day being the only side effect reported. He was discharged home with his regular pain regimen, with significant decrease in pain over the next few months. Rather than trying to establish a "one size fits all" protocol for ketamine infusions, this case illustrates a shift in pain management focus by allowing patients to self-titrate and demonstrates the potential for using ketamine PCA as a treatment option for CPS.

3. Rapid and Efficient Waterflood Optimization Using Augmented AI Approach in a Complex Offshore Field

Waterfloods are amongst the most widely implemented methods for oil field development. Despite their vast implementation, operational bottlenecks such as lack of surveillance and optimization tools to guide fast paced decisions render most of these sub-optimal. This paper presents a novel machine-learning, reduced-physics approach to optimize an exceptionally complex off-shore waterflood in the Gulf of Suez. Leveraging a hybrid data-driven and physics approach, the water flooding scheme in Nezzezat reservoir was optimized to improve reservoir voidage replacement, increase oil production, and reduce water production by identifying potential in wells. As a by-product of the study, a better understanding of the complex fault system was also achieved. Including the geological understanding and its uncertainty is one of the key elements that must be preserved. All geological attributes, along with production rates are used to solve for pressure and inter-well communication. This is later supplemented by machine-learning algorithm to solve for the fractional flow of inter-well connections. Combining the inter-well connectivity and fractional flow, an optimization was performed to reach the best possible conditions for oil gains and water-cut reduction. A global optimization is possible thanks to the low computational demand of this approach, as thousands to millions of realizations must be run to reach the best solution while satisfying all constraints. This is all done in a fraction of the time it takes to run a traditional reservoir simulation. For the present case, the paper will present the underlying physics and data-driven algorithms, along with the blind tests performed to validate the results. In addition to the method's inner workings, the paper will focus more on the results to guide operational decisions. This is inclusive of all the complex constraints of an offshore field, as well as the best reservoir management practices, when reaching optimal production and injection rates for each well. An increase in production was achieved with some reduction in water-cut, while honoring well and platform level limitations. While these represent the gains for a particular month, optimization scenarios can be run weekly or monthly to capture the dynamic nature of the problem and any operational limitations that might arise. The ability to update the models and run optimization scenarios effortlessly allows pro-active operational decisions to maximize the value of the asset. The approach followed in this paper solves for the critical physics of the problem and supplements the remaining with machine learning algorithms. This novel and extremely practical approach facilitate the decision making to operate the field optimally.

Known associations with autoimmune hemolytic anemia (AIHA) include lymphoproliferative neoplasms, autoimmune conditions, and viral infections. There are a few case reports that implicate a potential relationship between COVID-19 and either warm or cold AIHA. We present the case of combined warm and cold AIHA in the setting of COVID-19. A 51-year-old male with no known past medical history presented with weakness and jaundice. Initial workup revealed white blood cells 41.4, hemoglobin 3.1, platelets 343, total bilirubin 5.3, direct bilirubin 1.6, and COVID-19 positive. Direct antiglobulin test (DAT) found IgG and C3 antibodies and pathology revealed cold agglutinins, consistent with both warm and cold AIHA. He received a total of five blood transfusions and was started on prednisone 1 mg/kg daily with a gradual taper over months. Hemolysis labs normalized within two weeks after discharge although antibodies remained positive 70 days after admission. Our patient presented with IgG and C3 antibodies as well as cold agglutinins, consistent with both warm and cold AIHA. To our knowledge, this is the first case of both warm and cold AIHA presenting simultaneously in COVID-19 infection. Unlike most cases in the existing literature, this patient had no history of underlying hematologic malignancy and both warm and cold AIHA.

5. Automated Well Production and Gas Lift System Diagnostics and Optimization Using Augmented AI Approach in a Complex Offshore Field

This paper presents the comprehensive analysis and optimization study of the gas lift system of 44 gas lifted wells in QQ Field in the Gulf of Suez. With the least amount of intervention, the optimized operational plan would yield an additional 7 to 10% oil gain over the current production rate, an improvement of gas utilization and operational efficiency, and operating cost reduction. This paper presents an Augmented AI approach, which is an efficient, repeatable and automated workflow to diagnose and optimize gas lift operations, reducing the time required for this process from weeks to hours. This study evaluates historical performance, identification of suboptimal performers using heuristic diagnostics, and subsequent optimization using well models coupled with advanced optimization algorithms. The presented workflow implements a top-down approach beginning with field-level based analytics, then further delves into detailed engineering aspects of well production and individual gas lift operations. It builds from artificial intelligence and engineering-based workflows that compute smart metrics based on the historical well production and performance of gas lift systems. Heuristics and expert-based rules are also considered to perform a fast and smart diagnostics process. These analytics and diagnostics stages help build key metrics to evaluate well performance and monitor gas lift operations from both executive and asset management levels. The study results identified suboptimal wells with inefficient gas lift performance that require detailed evaluation for improvement from both design and operational perspectives. During the optimization phase, reliable well model-based analysis coupled with physical constraints are used to provide remedial actions for the identified wells. The workflow allows automated well model construction and calibration, coupled with an unsupervised learning algorithm to detect and eliminate unreliable datasets. Afterward, specific well-level analysis is performed including gas lift performance curve while honoring existing well configuration, hypervolume-based multi-objective optimization and sensitivity analysis on key parameters for best gas lift performance.

The QQ Field in the Gulf of Suez is a mature, geologically complex with multiple stacked, faulted reservoirs, with commingled production between different reservoirs. This paper illustrates the power of an automated tool to perform systematic, rapid, and detailed assessment of the reservoir performance, identify the key recovery obstacles and prepare remedial plans to enable the reservoir to produce to its full potential. The well and reservoir data were processed to compute a series of metrics and key performance indicators at various levels (well, layer, reservoir, well groups, etc.). The tool has several automated modules to facilitate rapid, metric-driven reservoir assurance and management. These modules include: (i) well production/injection allocation, (ii) wells decline curve analysis including event-detection, (iii) pressure and voidage analysis, and (iv) Contact analysis. Using performance analytics, the study quickly identified ways to improve the health of the reservoir and maximize its value. The QQ Field predominantly produces from two formations: Nubia and Nezzazat. Furthermore, there are multiple sub-layers in each formation. Reliable flow unit allocation is critical to gauge contribution of each layer, identify the undrained areas of the reservoir, and locate future development opportunities. The flow unit allocation module incorporates all available data such as PLT/ILT data, completion history, permeability of each flow unit at well level, relative pressures, and water influx model. Based on the allocated production, the current recovery factors in Nubia and Nezzazat are approximately 60% and 20% respectively. Analysis of RFT data reveals good vertical communication across Nubia. However, in some areas there is clear pressure discontinuity across layers. The reservoir pressure has dropped below the bubble point in both formations. As a result, water injection was initiated. The pressure in all parts of Nubia was restored above bubble point. Aquifer influx is sufficient to support the current withdrawal rates and further water injection is unnecessary. However, Nezzazat has a significantly higher reservoir complexity and therefore, shows a large variation in pressure behavior. It needs water injection to maintain the reservoir pressure above the bubble point in all parts of the reservoir. Based on the flow-unit allocation, the voidage replacement ratio (VRR) was calculated for each area and each layer. Even though the overall VRR in the waterflooded areas is above one, the distribution of the injected water is uneven. Redistributing injected water and ensuring that all the areas and all the layers are adequately supported will help to maximize recovery. The prolonged dip in oil price demands extreme efficiency. Sound reservoir management must not require unreasonable time or manpower. The rapid, automated analysis enables quick identification of the key areas for improvement in reservoir management practices and maximize the value of the asset.

Sandstone reservoirs of the South Caspian basin in Turkmenistan are currently undergoing further development—with the addition of deviated wells. The localized depletion occurring in some of the offshore fields in this area has thus far triggered a host of geomechanics-related challenges—including wellbore instabilities and poor hole quality. In anticipation of further depletion over the remaining fields life, geomechanics effects will become more pronounced and the associated technical and economic challenges facing these fields may increase.To assist in future well planning and field development, and to diagnose the problems already encountered in the existing vertical wells, 3D seismic-driven mechanical earth models (MEMs) were built. These covered the main sandstone reservoirs as well as the shaley formations. This integration of data from drilling operations, open hole logs, core, seismic and formation pressure measurements provided a constrained and consistent description of the prevailing in-situ state of stress, pore pressures and rock mechanical properties. These geomechanical models were further improved by accounting for historical depletion in the fields considered. The depletion modelling was performed numerically using a simulator performing finite difference fluid-flow calculations. The results obtained and understanding gained were then considered in the analyses of wellbore stability for future wells.This paper describes these geomechanical analyses and modelling—including the data integration to assess wellbore stability at the current level of depletion.

The Cheleken area holds a series of hydrocarbon accumulations along a transpressional ridge (Apsheron trend). Seismic data is key to understand each field's structural setting and complexity especially in the inter-well space. Previous data interpretations could not overcome limitations and ambiguities of data quality leading to a range of possible interpretations. This study presents how integration of well data and structural style concepts into the seismic workflow results in a structural model matching all data and field characteristics. Interpretation of band limited thus low-resolution seismic data in context of a complex structural setting across a thick deltaic succession is challenging. In order to avoid over-interpretation and non-systematic patterns all available information was integrated into the interpretation process. From preliminary identification of the master fault and stress regime, a classical Riedel shear system was recognized. Prevailing fault plane geometries at each deformation stage were defined and applied to guide fault delineation at larger scale. Linking interpreted faults to corresponding well markers, identified by layer doubling or missing sections, required considerable effort due to the high number of deviated wells, but strongly reduced ambiguities. The central part of the Lam field forms a positive flower structure across the productive Red Series. A strike slip system with helicoidal fault geometry is the prevailing structural style. Such fault system originates from a single master fault at large depth. Transpressional deformation led to the formation of Riedel faults that root in the main fault. Ongoing deformation resulted in the additional formation of P- faults connecting the Riedel faults at larger depth only. At the same time, Riedel faults steepen while propagating upward, but die out at the top of the Red Series. This resulted in structural highs developed as updomings between the en-echelon patterns of the Riedel faults. These show reverse offsets on the flanks but typically only minor throw when passing the central sections of the updomings. A second, relatively younger set of normal faults was identified which did not match the strike slip system interpretation. These faults crosscut the older faults in the shallow succession and indicate a recent extensional regime. Exceptional situations require exceptional effort across multiple disciplines to arrive at a common understanding for hydrocarbon fields. Success keys were to find the balance between data driven and model driven results and to allow for a certain degree of simplification. The latter was substantial in the construction of a model that captures the main field characteristics but avoids unnecessary detail in terms of structure and subsequently hydrocarbon distribution.

9. An Integrated Geological Model for the Greater Cheleken Area Central Caspian Basin, Turkmenistan; Complex Synsedimentary Transcurrent Faulting and Compartmentalization in Plio-Pleistocene Clastic Reservoirs

The Greater Cheleken Area (GCA) reservoirs, offshore developed by Dragon Oil, are situated on a complex NW-SE dextral transcurrent zone which separates the Northern from the Southern Caspian Basin (SCB), hosting numerous hydrocarbon fields in thick, highly faulted Pliocene-Pleistocene clastic successions of the paleo-Amu-Darya Delta System originating in the N-Balkan Fold belt. We present a new model for the geological evolution of the area, based on elaboration and interpretation of 3d seismic data (643 km2), well logs (ca. 300 wells), and a detailed remote sensing study of the Cheleken peninsula, integrated with available published data. The Apsheron-PreBalkan Fault Zone (APBFZ) separates the Southern from the Northern Caspian Basin, and connects to the SE with the Ashgabat ("Main Kopet-Dagh") Fault Zone (AFZ) which separates the Kopet-Dagh fold belt (SE) from the Balkan fold belt and Turan Block (N). These dextral shear zones show activity from at least the Miocene onwards, evidenced by a minimum of several km's post-Mid Pliocene lateral offset. In our new model a regional dextral left stepping transpressional/reversal bend between APBFZ and AFZ shows internally alternating arrays of interconnected transpressive/transtensional (R; E-W) and transpressional (P; ENE-WSW) positive hydrocarbon bearing flower structures of different types (Lam, Zdhanov-Jygalybek-Yangummez, Cheleken-Aligud-Daghazil, Goturdepe, Barsagalmez, Burun, Nebit-Dag, Urundzhuk, Monzukhly, Karatepe, Kyzylkum, Gumdag, etc.). The GCA shows 3 typical structures: Cheleken-Daghazil (positive Palm-tree flower shape, sigmoidal map shape), Zhdanov (asymmetrical top graben, deeper thrust-block, negative Palm-tree shape), and Lam (3 segments, semi-circular-elliptical-rhomboidal map shapes, positive Tulip-shaped push-ups). Hydrocarbons migrated upwards from the M. Miocene Maikop Fm source rock. Mud-volcanoes dissect the structures and show activity up to present with expulsion of hydrocarbons mixed with deeper Cretaceous and Paleo-Neogene sediments. The major fault structures reach the sea floor/land-surface, and erosion levels reach the mid Pliocene CH2 unconformity; the latest inversion along the structure has taken place during and after Mid Pleistocene times. Unconformity-bound sequences of alternating sandstones / shales with limited limestones, conglomerates, and evaporates (deposited in shallow marine, delta front, delta plain, alluvial, and lacustrine sedimentary environments): Uppermost Miocene - Lower Pliocene Red Series (Lower, Middle, Upper Red Series), Upper Pliocene Akchagyl Fm, Lower Pleistocene Apsheron Fm, and Upper Pleistocene – Holocene, comprise twelve productive levels (ca.1000 – 4000 mssl in the GCA). Hundreds (in subsurface) to thousands (in outcrop) of mappable faults (transpressive / transtensive; m. – 100s m. scale) are connected to deep-seated shear zones, and show fault-sealing (highly compartmentalized reservoirs with vertical and horizontal separations, varying in shape and size both in vertical and horizontal sense), continuous Pliocene-Pleistocene syn-sedimentary activity (sediment thickness heterogeneities, pinch-outs, lateral facies changes, channel structures), and inversion structures. The multi-scaled tectonic and sedimentary complexity described for the GCA provides fundamental insights in what constitutes a major challenge on the different strategies in the Production of the Reservoirs, and opens a gateway to unlock important new Exploration and Appraisal potential.

Human mucosal-associated invariant T (MAIT) cells express the semi-invariant T-cell receptor (TCR) Vα7.2 and are restricted by the major histocompatibility complex-Ib molecule MR1. While MAIT cells share similarities with other innate T cells, the extent to which MAIT cells are innate and their capacity to adapt is unknown. We evaluated the function of Vα7.2(+) T cells from the thymus, cord blood, and peripheral blood. Although antigen-inexperienced MAIT cells displayed a naïve phenotype, these had intrinsic effector capacity in response to Mycobacterium tuberculosis (Mtb)-infected cells. Vα7.2(+) effector thymocytes contained signal joint TCR gene excision circles (sjTRECs) suggesting limited replication and thymic origin. In evaluating the capacity of Mtb-reactive MAIT cells to adapt, we found that those from the peripheral blood demonstrated a memory phenotype and had undergone substantial expansion, suggesting that they responded to antigenic stimulation. MAIT cells, an evolutionarily conserved T-cell subset that detects a variety of intracellular infections, share features of innate and adaptive immunity.
