9. Youth Employment and Training in a Year of Transition: The Outlook for New York City. Trends and Forecasts, Special Issue.

Abstract

The 1983 Federal fiscal year is a year of transition from the Comprehensive Employment and Training Act (CETA) to the Job Training Partnership Act of 1982 (JTPA). Unlike CETA, which included public service employment provisions, the entire thrust of JTPA is on training for unsubsidized employment. The purpose of the new law is to prepare youth and unskilled adults for entry into the labor force, and provide job training to economically disadvantaged individuals and others facing serious barriers to employment. The States and Private Industry Councils (PIC) are keystones in the implementation of JTPA. Governors will designate Service Delivery Areas, establish a State job training coordinating council, and approve local plans. The local plan must be developed and approved by the local government and the PIC, either of which may administer the program. The Department of Employment of the New York City Human Resources Administration is the primary municipal agency for employment and training and has been the local administrative agency under CETA. A wide variety of services currently exist in such categories as pre-employment programs, classroom and on-the-job training programs, and work experience programs. In the future, State legislation is expected to fill gaps in the JTPA, and increased participation of the private sector will be needed to shape employment for youth. (MDK)

10. Collaborative Efforts of Business and the New York City Public High Schools.

Abstract

This document describes collaborative efforts in New York City between public high schools and the business sector. Business involvement with the schools assumes many forms and operates on a number of different levels: (1) forming a special relationship with particular schools or departments, (2) being a resource to provide speakers for classes, (3) acting as role models or mentors for students, (4) providing a work setting for student internships and/or future employment, and (5) serving on advisory committees for occupational education. Examples of each type of relationship as well as some school-specific illustrations are discussed. Some general issues relating to school-business partnerships are raised. These include a mutual understanding of the objectives and how they will be achieved, realistic expectations of each partner, and supportive attitudes accompanied by institutional structures which will ensure continued commitment. Research studies are being conducted to learn more about these joint efforts between schools and businesses. Specific areas of focus include effectiveness of joint efforts in reducing school dropout rates and improving the success of disadvantaged youth in the labor market; the characteristics of successful programs and factors which influence employer participation; and how these relationships might be improved. Abstract

Objective: To investigate mortality in the non-pregnant fertile female population, and mortality during pregnancy and up to 1 year after the end of pregnancy, in Finland from 2001 to 2012 and compare the results with those of 1987-2000.Design: Retrospective cohort study.Setting: Finland.Population: 10 427 deceased women of reproductive age (15-49).Methods: Pregnancy outcomes were categorised into: ongoing pregnancy or birth, miscarriage or ectopic pregnancy and termination of pregnancy. Mortality was calculated per 100 000 pregnancies. The mortality rate of the non-pregnant female population of fertile age was calculated per 100 000 person-years, and the results were compared with those of 1987-2000.Main Outcome Measures: Pregnancy-associated mortality during pregnancy and up to 1 year after the end of pregnancy. Mortality of non-pregnant, fertile-age, female population.Results: The age-adjusted mortality rate during pregnancy and within 1 year after the end of pregnancy was 28.4/100 000 pregnancies, and it had significantly decreased compared with the period 1987-2000 [risk ratio (RR) 0.75 (95% CI, 0.65-0.88)]. Mortality in non-pregnant fertile-age females was 48.1/100 000 person-years. Mortality for diseases and medical conditions during and after pregnancy decreased by 26% [RR 0.74 (95% CI, 0.59-0.92)] and for external causes by 23% [RR 0.77 (95% CI, 0.62-0.95)]. The mortality rate for suicides was 3.3/100 000 in ongoing pregnancies and pregnancies ending in birth while it was 21.8/100 000 after termination of pregnancy and 10.2/100 000 among non-pregnant women.Conclusions: In comparison with earlier decades, pregnancy-associated mortality has decreased in Finland.Tweetable Abstract: Mortality decreases among pregnant women and within 1 year after pregnancy in Finland. Abstract

Atypical teratoid/rhabdoid tumors (AT/RTs) of the central nervous system (CNS) are distinctive, malignant neoplasms of uncertain histogenesis. They are thought to be embryonal and are usually composed of varying amounts of rhabdoid-type cells, small primitive neuroepithelial cells, epithelial tissue and neoplastic mesenchyme. Diagnosis may be difficult due to this morphologic variability and usually depends on demonstrating the presence of a specific mutation on chromosome 22 (the hSNF5/INI1 gene) or by visualizing ultrastructural whorled masses of intracytoplasmic intermediate filaments. The prognosis is poor with the majority of patients dying within one year of diagnosis. We describe a case of an AT/RT in an 8 year old boy who died shortly after presentation and whose tumor at autopsy showed unusual gross and histologic features. Abstract

Objective: The objective was to investigate prevalence, estimate risk factors, and antenatal suspicion of abnormally invasive placenta (AIP) associated with laparotomy in women in the Nordic countries. Design: Population-based cohort study. Setting and population: A 3-year Nordic collaboration among obstetricians to identify and report on uterine rupture, peripartum hysterectomy, excessive blood loss, and AIP from 2009 to 2012 The Nordic Obstetric Surveillance Study (NOSS). Methods: In the NOSS study, clinicians reported AIP cases from maternity wards and the data were validated against National health registries. Main outcome measures: Prevalence, risk factors, antenatal suspicion, birth complications, and risk estimations using aggregated national data. Results: A total of 205 cases of AIP in association with laparotomy were identified, representing 3.4 per 10 000 deliveries. The single most important risk factor, which was reported in 49% of all cases of AIP, was placenta praevia. The risk of AIP increased seven-fold after one prior caesarean section (CS) to 56-fold after three or more CS. Prior postpartum haemorrhage was associated with six-fold increased risk of AIP (95% confidence interval 3.7–10.9). Approximately 70% of all cases were not diagnosed antepartum. Of these, 39% had prior CS and 33% had placenta praevia. Conclusion: Our findings indicate that a lower CS rate in the population may be the most effective way to lower the incidence of AIP. Focused ultrasound assessment of women at high risk will likely strengthen antenatal suspicion. Prior PPH is a novel risk factor associated with an increased prevalence of AIP. Abstract

This study was planned to determine the number of origins of the mutation underlying Huntington's disease (HD) in Sweden. Haplotypes were constructed for 23 different HD families, using six different polymorphisms [(CCG)n, GT70, 674, BS1, E2 and 4.2], including two within the gene. In addition, extensive genealogical investigations were performed, and the geographical origin of the haplotypes was studied. Ten different haplotypes were observed suggesting multiple origins for the HD mutation in Sweden. Analysis of the two polymorphic markers within the HD gene (the CCG repeat and GT70) indicates that there are at least three origins for the HD mutation in Sweden. One of these haplotypes (7/A) accounts for 89% of the families, suggesting that the majority of the Swedish HD families are related through a single HD mutation of ancient origin. Furthermore, three of the families that were previously considered to be unrelated could be traced to a common ancestor in the 15th century, a finding that is consistent with this hypothesis.
