INTRODUCCIÓN: La principal causa de morbimortalidad en los pacientes diabéticos la constituyen las enfermedades cardiovasculares, y que uno de los principales factores de riesgo son las dislipidemias. El patrón de alteraciones lipídicas está caracterizado por elevación moderada del colesterol LDL, con aumento de las partículas de LDL densas y pequeñas, incremento de triglicéridos y reducción del colesterol HDL (dislipidemia aterogénica). La relación del cLDL y el riesgo cardiovascular, es indudable, así como de los beneficios del tratamiento con estatinas. Una vez conseguido el objetivo de cLDL, son notables las evidencias que demuestran la persistencia de un elevado riesgo cardiovascular, denominado riesgo residual; este, se fundamenta en la dislipidemia aterogénica, caracterizada por un aumento de triglicéridos y de las lipoproteínas ricas en triglicéridos, un descenso del HDL y alteraciones cualitativas de las partículas LDL. Una de las medidas para identificar dicho riesgo es la determinación de los remanentes de colesterol (RC), que representan el contenido de colesterol asociado con lipoproteínas de muy baja densidad (VLDL) y de densidad intermedia (IDLc) en estado de ayunas y el contenido de colesterol de quilomicrón en estado posprandial. Los RC son extremadamente aterogénicos, en gran parte debido a su tamaño pequeño y el contenido de colesterol esterificado encargado de producir inflamación en la pared arterial. Los RC son los responsables; junto con los triglicéridos; del mayor riesgo cardiovascular, en comparación con la concentración de cLDL. Esto se correlacionó en estudios de aleatorización mendeliana y en ensayos epidemiológicos en el período posprandial. OBJETIVO: Determinar la prevalencia de remanentes de colesterol aumentado en pacientes diabéticos tipo2. MATERIAL Y MÉTODO: Se realizó un Estudio Descriptivo de Corte Transversal, Retrospectivo. La muestra fue conformada por 185 sujetos asistidos en un consultorio privado de la Capital de Catamarca que completaron los análisis de laboratorio solicitados en institución acreditada, tomados de historias clínicas de los mismos. La principal variable estudiada fue el Remanente de Colesterol. RESULTADOS: el promedio de edad fue 56 años, de los cuales el 60 % fueron hombres. La prevalencia de remanentes de colesterol aumentado fue de 23 %. Existe una asociación estadísticamente significativa el índice TGS/HDL con la presencia de Remanente de Colesterol Aumentado, así también colesterol No HDL con remanentes de colesterol. No hubo correlación entre estado del grado de control metabólico de la diabetes según parámetro glucémico expresado en Hb a1c y el nivel de Remanentes de Colesterol. CONCLUSIONES: la prevalencia de remanentes de colesterol aumentado en pacientes diabéticos tipo 2 fue del 23 %, se relacionó significativamente con el índice Triglicéridos/HDL y Colesterol no HDL. No hubo correlación con el grado de control metabólico de la diabetes según parámetro glucémico expresado en Hba1c. No hubo relación entre el tratamiento hipolipemiante existente y su potencia, y los individuos con remanentes de colesterol aumentados.

Earth is heading towards a climate that last existed more than three million years ago (Ma) during the 'mid-Pliocene warm period'.sup.1, when atmospheric carbon dioxide concentrations were about 400 parts per million, global sea level oscillated in response to orbital forcing.sup.2,3 and peak global-mean sea level (GMSL) may have reached about 20 metres above the present-day value.sup.4,5. For sea-level rise of this magnitude, extensive retreat or collapse of the Greenland, West Antarctic and marine-based sectors of the East Antarctic ice sheets is required. Yet the relative amplitude of sea-level variations within glacial-interglacial cycles remains poorly constrained. To address this, we calibrate a theoretical relationship between modern sediment transport by waves and water depth, and then apply the technique to grain size in a continuous 800-metre-thick Pliocene sequence of shallow-marine sediments from Whanganui Basin, New Zealand. Water-depth variations obtained in this way, after corrections for tectonic subsidence, yield cyclic relative sea-level (RSL) variations. Here we show that sea level varied on average by 13 [plus or minus] 5 metres over glacial-interglacial cycles during the middle-to-late Pliocene (about 3.3-2.5 Ma). The resulting record is independent of the global ice volume proxy.sup.3 (as derived from the deep-ocean oxygen isotope record) and sea-level cycles are in phase with 20-thousand-year (kyr) periodic changes in insolation over Antarctica, paced by eccentricity-modulated orbital precession.sup.6 between 3.3 and 2.7 Ma. Thereafter, sea-level fluctuations are paced by the 41-kyr period of cycles in Earth's axial tilt as ice sheets stabilize on Antarctica and intensify in the Northern Hemisphere.sup.3,6. Strictly, we provide the amplitude of RSL change, rather than absolute GMSL change. However, simulations of RSL change based on glacio-isostatic adjustment show that our record approximates eustatic sea level, defined here as GMSL unregistered to the centre of the Earth. Nonetheless, under conservative assumptions, our estimates limit maximum Pliocene sea-level rise to less than 25 metres and provide new constraints on polar ice-volume variability under the climate conditions predicted for this century. Genetic instability promotes cancer progression (by increasing the probability of cancerous mutations) as well as hinders it (by imposing a higher cell death rate for cells susceptible to cancerous mutation). With the loss of tumor suppressor gene function known to be responsible for a high percentage of breast and colorectal cancer (and a good fraction of lung cancer and other types as well), it is important to understand how genetic instability can be orchestrated toward carcinogenesis. In this context, this paper gives a complete characterization of the optimal (time-varying) cell mutation rate for the fastest time to a target cancerous cell population through the loss of both copies of a tumor suppressor gene. Similar to the (one-step) oncogene activation model previously analyzed, the optimal mutation rate of the present two-step model changes qualitatively with the convexity of the (mutation rate-dependent) cell death rate. However, the structure of the Hamiltonian for the new model differs significantly and intrinsically from that of the one-step model, and a completely new approach is needed for the solution of the present two-step problem. Considerable insight into the biology of optimal switching (between corner controls) is extracted from numerical results for cases with nonconvex death rates.
