Stunting in children is a major global issue, especially in rural agricultural areas of Indonesia. Despite ongoing efforts to eliminate stunting, its prevalence remains high, adversely affecting children's cognitive and physical development. This study aimed to determine the factors influencing the nutritional status of stunted children in Indonesia’s agricultural districts. A cross-sectional analytical study was conducted in Jember District, Indonesia, involving 236 malnourished children aged 0 to 59 months. Using stratified random sampling, structured questionnaires were utilized to collect data on family functioning, child feeding attitudes, and practices. The results are Family Function (B=0.038; 95% CI= 0,535 – 1,658, p value= 0.047); Child Feeding Attitude (B=0.030; 95% CI= 0,882 – 1,681, p value= 0.023); and Child Feeding Practice (B=0.120; 95% CI= 0,686 – 1,971, p value= 0.000). There were significant correlations between family function (p value= 0.047), child-feeding attitudes (p value= 0.023), and child-feeding practice (p value= 0.000) with children's nutritional status. This study highlighted a significant relationship among family function, child-feeding attitude, and practices concerning the nutritional status of children experiencing stunting. It emphasizes the importance of improving family function and supporting healthy child-feeding practices. Targeted interventions based on local cultural understanding are critical for effectively reducing stunting.

Food security is a global issue that has received serious attention from various countries, including Indonesia. Indonesia still has problems related to gender inequality, which is a separate cause of food insecurity. This study aimed to analyze the mapping related to the food security of Jember farmers and its relation to household decision-making and the division of tasks related to food management in farming households. The study used a survey method by taking 251 samples of farming families from the working area of the Health Center in the Jember District. This study was conducted from May to July 2023. Data were collected using questionnaires and supported by qualitative data through in-depth interviews. Bivariate data analysis was performed using Spearman’s rho test. The results showed a correlation between gender identity and food security (p= 0,000), and there was a positive relationship pattern: the higher the gender role given, the more food security will increase in July (r= 0,504). In conclusion, there is a positive relationship between gender roles and food security; the higher the gender roles, the higher the farmers' food security.

Introduction: Snakebite Envenoming (SBE) cases are a worldwide health problem. Farmers have a big risk of becoming victims of SBE. SBE management is an issue that needs to be resolved due to a lack of information regarding the appropriate management of SBE. This study aimed to analyze the effectiveness of educational intervention programs on the knowledge, attitude, practice, and self-efficacy of farmers toward SBE management. Methods: The design of this study was a non-blinded cluster randomized controlled trial design, applied to 80 farmers in six subdistricts from rural areas of Indonesia. A multistage sampling was used to select participants. Data collection tools included a three-part questionnaire that consisted of demographics, knowledge, self-efficacy, and an observation checklist for SBE first aid practices for farmers. Education and training sessions were carried out for eight weeks for the intervention group by providing eight sessions of education and training activities. During the COVID-19 pandemic, the elderly with dementia face challenges in the form of changes in daily routines and the absence of social services, making the task of care shift completely to the family. Families who are not ready to provide appropriate and effective care will have an impact on cognitive deterioration and the development of their dementia condition. The purpose of this study was to describe the care of elderly dementia during the COVID-19 pandemic. The research design used in this study is a narrative literature review. The selection of articles is carried out through 4 stages of the PRISMA flowchart. Articles are searched through Pubmed, Springer Link, Plos One and Google Scholar. The results of the study are the findings of 10 articles that describe the care of elderly dementia during the COVID-19 pandemic in the form of home-based and clinical trial-based care. Clinical trials treatment in the form of telehealth, Mario robot, Tv-assist Dem, and a stationary bicycle, while home-based care is in the form of basic care that is not much different from pre-pandemic while still adhering to health protocols. The conclusion of this study is the need for the elderly with dementia to continue to receive care during the COVID-19 pandemic to avoid worsening cognitive function and the development of dementia which can ultimately have an impact on their health status and quality of life.

Loneliness affected sleep disorders among the elderly. A poor sleep pattern will be increasing blood pressure among elderly. The aimed his study was to analyzed the correlation between loneliness and sleep disorder among the elderly with hypertension. A case-control design was conducted among 51 elderlies (33 with hypertension, 18 with non-hypertension). A self-administered questionnaire was used to measure the sociodemographic of elderly while UCLA Loneliness Scale was used to measure loneliness, then the PSQI questionnaire was performed to measure sleep disorder. The correlation between loneliness and sleep disorder was analyzed using Chi-square test. The result showed among 51 elderly were 39,4% of mild loneliness of elderly with systole hypertension, and 75,8% of sleep disorder was mostly experienced by the elderly with systole hypertension. BackgroundFarmers in Indonesia have a high risk for hypertension owing to their lifestyle and working environment. Diet management is a solution to reduce hypertension, and Indonesia has natural resources in the agricultural sector that could help manage hypertension. Optimizing vegetable and fruit intake in a plant-based diet (PBD) could help maintain blood pressure among farmers in Indonesia. ObjectiveThis study aims to explore the health problem of hypertension and the characteristics of local food sources to formulate a PBD menu for treating hypertension, as well as assess the prevalence of hypertension, level of acceptability of a PBD, and associated sociodemographic factors. Further, we want to examine the effectiveness of a community-based nursing program for managing hypertension using a PBD. MethodsWe will use the exploratory sequential mixed methods approach. There will be a qualitative study (phase I) in 2022 and a quantitative study (phase II) in 2023. We will analyze data using a thematic framework in phase I. In phase II, the study will involve (1) questionnaire development and validation; (2) examination of the prevalence of hypertension, the level of acceptability of a PBD, and the associated factors; and (3) a randomized controlled trial. We will recruit farmers with hypertension who meet the study criteria. Moreover, in phase II, we will invite expert nurses and nutritionists to assess the face and content validity of the questionnaire. We will use multiple logistic regression models to estimate the associated sociodemographic factors and the level of acceptability of a PBD. Furthermore, a linear generalized estimating equation will be used to estimate the parameters of a generalized linear model with a possible unmeasured correlation between observations from different time points for systolic and diastolic blood pressure. ResultsA model PBD for hypertension management is expected to be developed. In 2022, we will collect information on hypertension and the characteristics of local food sources for managing hypertension, and will formulate a PBD menu to treat hypertension among farmers. In 2023, we will develop a questionnaire to assess the acceptability of a PBD to manage hypertension among farmers, the prevalence of hypertension, and the sociodemographic factors associated with hypertension among farmers. We will implement a community-based nursing program for managing hypertension using a PBD among farmers. ConclusionsThe PBD model will not be readily available for other agricultural areas since validation of local food variation is required to design the menu. We expect contributions from the local government to implement the intervention as one of the policies in the management of hypertension for farmers in the agricultural plantation areas of Jember. This program may also be implemented in other agricultural countries with similar problems, so that hypertension can be optimally treated among farmers. Hypertension among elderly can decrease the status of cognitive function, that affected for self care activity: daimly activities for the long term. The aimed of this study was to analyzed the relationship between cognitive function and self-care: daily activities in elderly hypertension in UPT PSTW Jember. A cross-sectional study was used among 61 elderlies using total sampling. A self–administred questionnaire was used to identify sociodemography of elderlies, while Mini Mental State Examination (MMSE) was used to measure cognitive function and the Index Katz was used to measure elderly daily self care. The relationship between cognitive function and self-care: daily activities in elderly hypertension using Chi-square test. The results showed that most of the elderly were a normal cognitive function status (37.7%) and independence doing daily activities (47.5%). There were significant relationship between cognitive function and self-care: daily activities in elderly hypertension (p value 0.018, X2 8.005). The conclusion of this study that cognitive function is related to self-care: daily activities in elderly hypertension. Therefore elderly should maintain good cognitive function in order to be independent in performing self-care: daily activities. Keywords: cognitive function, elderly, hypertension, daily activities

10. Validity and Reliability of Indonesian Public Health Nursing Competencies in Achieving Indonesian Healthy Program with a Family Approach: A Pilot Study

Background: Indonesia has the Indonesian Healthy Program with a Family Approach (IHP-FA) to solve various health problems in the country. The public health providers in Indonesia play a very vital role in realizing this program. There have not been clear reference standards regarding the Indonesian Public Health Nursing (IPHN) competencies. This condition causes the provision of nursing services in public health centers (PHCs) to be suboptimal. Purpose: This study aimed to identify the validity and reliability of the core competencies of IPHN standards in a practice setting to achieve the IHP-FA. Methods: A pilot study using a descriptive correlational study was conducted among 55 coordinators of public health nursing (PHN) program from 50 PHCs in Jember, Indonesia. The IPHN practices were accessed using the five PHN core competencies (including activities in PHCs and nursing care for follow-up patients, family, special needs group in the community, and community). The IHP-FA was measured using 12 indicators. Content Validity Index (CVI) was used to examine the validity of core competencies. Internal consistency was explored using Cronbach’ α coefficient. Construct validity using the known-groups technique was explored to measure the correlational between IPHN competencies and indicator of IHP-FA. Results: The CVI indicated adequate content validity (0.80-0.10) and high reliability (Cronbach’s alpha coefficient=0.81). There was a significant correlation between five core IPHN competencies and achievement of IHP-FA (safe birth delivery, immunization, growth and development, management of tuberculosis, smoking, and access to clean water). Conclusion: IPHN competencies contain valid, reliable, and psychometrically robust measures. However, some programs in IHP-FA could not be achieved with five core IPHN competencies, demonstrating the need for developing the IPHN competencies in the future. 11. Identifying Community/Public Health Nursing Competencies in Indonesia: A Modified Delphi Method

Background: Nurses in public health centers, or known as public health nurses (PHNs), have duties in implementing promotive, preventive, curative, and rehabilitative efforts. In Indonesia, there is no basic reference to the competencies that should be performed by PHNs. The provision of health services in the community should cover two areas, namely inside and outside the building (Minister of Health’s Regulation No. 279 Year 2006); it causes nurses in the community to further hone their skills while in the field. Purpose: This study aimed to identify the competencies of the Indonesian PHNs from experts using the C/PHN competencies of the Quad Council Coalition through a Delphi method. Methods: This study was quantitative research with a Delphi method. We employed a purposive sampling technique to recruit the experts of public health nurses. The experts did a Delphi method to identify, analyze, and modify the C/PHN Competencies of the Quad Council Coalition into the Indonesian version of C/PHN Competencies with local cultures. Results: Results showed that from eight domains of the Quad Council Coalition C/PHN competencies, there were changes for priority of the PHN competencies in Indonesia. All priorities were classified into eight groups, namely: (1) Leadership Skills and Thinking Systems; (2) Community Dimensions of Practice; (3) Assessment Analytic; (4) Policy Development and Program Planning; (5) Communication Skills; (6) Financial Planning and Management and Planning; (7) Public Health Sciences Skills; and (8) Cultural Competency. Conclusion: The Quad Council Coalition of C/PHN competencies are appropriate with the Indonesian PHN competencies, although the priority is changed related of local wisdom as stated in the Minister of Health’s Regulation No. 279 Year 2006. Therefore, the Indonesian P/HN competencies should be developed to support the Indonesian health people through family approach.

Children with disabilities often experience physical violence committed by caregivers. This study aims to identify the relationship between stress in caring for physical violence committed against children with disabilities. The study used a cross-sectional design to examine 76 parents with children with disabilities selected by convenience sampling techniques. Of the 76 participants, 35 (46.1%) parents physically abused children with disabilities. The most common type of violence is hitting (74.3%). Parenting stress may be felt by parents because there is a relationship between caregiving stress with physical violence (Z= -2.85; p-value= 0.004). Lack of access to information related to adaptive care makes parents in Indonesia still consider physical violence, such as hitting children, is a natural thing. The research concludes that there is a relationship between parental stress and physical violence against children with disabilities. Health workers are expected to be able to teach parents how to improve coping mechanisms to reduce parenting stress so that parenting behavior becomes adaptive. [Anak difabel sering mengalami kekerasan fisik yang dilakukan oleh pengasuh. Penelitian ini bertujuan untuk mengidentifikasi hubungan antara stres dalam pengasuhan dengan kekerasan fisik yang dilakukan terhadap anakdifabel. Penelitian menggunakan desain cross-sectional untuk meneliti 76 orang tua dengan anak difabel yang dipilih dengan teknik convenience sampling. Dari 76 partisipan, 35 (46,1%) orang tua melakukan kekerasan fisik terhadap anak difabel. Jenis kekerasan yang paling banyak dilakukan adalah memukul (74,3%). Stres pengasuhan mungkin dirasakan oleh orang tua karena terdapat hubungan antara stres pengasuhan dengan kekerasan fisik (Z=-2,85; p-value = 0.004). Kurangnya akses informasi terkait pengasuhan yang adaptif menyebabkan orang tua di Indonesia masih menganggap kekerasan fisik seperti memukul anak merupakan hal yang wajar. Penelitian menyimpulkan bahwa terdapat hubungan antara stres pengasuhan dengan kekerasan fisik terhadap anak difabel. Parenting is an interaction between children and parents, including fulfilling physical, psychological needs, and socializing norms in society. The results of the adaptation of parenting styles include sexuality, gender, and norm perception (SGNP) in adolescent reproductive health. This study aims to analyze the relationship between parenting style with SGNP in adolescent reproductive health in Patrang District, Jember Regency. This study was a correlational analytic with a cross-sectional approach conducted among 158 participants with a cluster random sampling technique. Data collection used a questionnaire to identify the characteristics of participants, parenting with Parenting Style Dimension Questionnaire (PSDQ), and SGNP in adolescent reproductive health with Sexuality, Gender, and Norms Questionnaire. Spearman Rank Correlation as an analysis method was used to answer the objectives of this study. Among 158 participants, 65.8% were authoritative parenting, 20.9% were authoritarian and 13.3% were permissive. Besides, 53.2% of adolescents had positive of SGNP in reproductive health. There is a relationship between parenting style and SGNP in adolescent reproductive health (r = 0.193; p = 0.015). Therefore, a planned intervention is needed such as educating parents and adolescents to be able to apply proper parenting to form positive perceptions.

Tuberculosis (TB) transmission awareness is crucial for TB prevention in the community. However, efforts to prevent TB transmission from TB patients’ perspectives, especially in the rural area, are not well documented. This study aimed to explore the efforts made by TB patients in preventing transmission to the community. This research was qualitative research with a phenomenological approach. Participants in this study were 12 people selected by purposive sampling. Data collection was done by in-depth interviews and recorded. Data analysis was carried out by thematic analysis. This study produced seven themes: perception of TB disease, performing alternative treatments, using personal protective equipment, environmental modification, adhering to treatment, limiting interactions with others, and increasing food intake. Knowledge and awareness of TB patients are still an issue in preventing the transmission of TB in the community. Immediate intervention needs to be made regarding increasing knowledge and awareness of TB patients and the supervision of health workers regularly in handling TB disease in the community.

Objective: to analyze the differences of coping strategies and quality of life among older people with hypertension in nursing homes of Indonesia. Method: a cross-sectional study was conducted among 71 older people during December 2018 in nursing homes. A self-administered questionnaire was used to identify characteristic of participants and quality of life for older people, while Sphygmomanometer was used to measure blood pressure. A Chi-quare test was performed to analyse the data. Results: there were significant differences between coping strategies and quality of life of older people with hypertension (χ2=4.15; p=0.041). Older people who used coping strategies by focusing on problems have more likely moderate quality of life (OR=2.86; 95% CI=1.02-7.97). Conclusion: therefore, coping strategies of older people with hypertension should be improved to achieve their quality of life through caring and counselling.

Penelitian ini bertujuan mengidentifikasi hubungan pekerjaan dan status paritas ibu dengan sikap pemberian ASI (Air Susu Ibu) pada bayi usia 0-6 bulan di Kecamatan Panti Kabupaten Jember. Penelitian dengan desain cross sectional dilakukan pada 273 ibu yang memiliki bayi usia 0-6 bulan dengan stratified random sampling. Kuesioner digunakan untuk mengidentifikasi karakteristik responden dan sikap ibu dalam pemberian ASI eksklusif. Chi-square test digunakan untuk menjawab tujuan penelitian ini. Hasil penelitian menunjukkan sebagian besar pekerjaan ibu adalah ibu rumah tangga (80,2%) dan sebagian besar ibu memiliki sikap pemberian ASI yang cukup (90,8%). Analisis bivariat menunjukkan tidak terdapat hubungan antara pekerjaan ibu dengan sikap pemberian ASI (χ2= 2,089; p-value= 0,352). Akan tetapi teridentifikasi bahwa status paritas ibu berhubungan dengan sikap pemberian ASI (χ2= 14,050; p-value= 0,001). Dapat disimpulkan dari penelitian ini bahwa pekerjaan ibu tidak berhubungan dengan sikap pemberian ASI, tetapi status paritas ibu berhubungan dengan sikap pemberian ASI. Program KB (Keluarga Berencana) perlu ditingkatkan dengan tujuan mendorong ibu memberikan ASI kepada anaknya sesuai usia yang ditentukan. ABSTRACT Double roles for working mothers caused stress which effects on their attitude towards breastfeeding practice. The objective of this study was to identify the correlation between mother’s employment and parity and attitudes towards breastfeeding among mothers in Panti district, Jember regency. A cross-sectional study was conducted among 273 mothers who have infants 0-6 months using stratified random sampling. Respondents’ characteristics and attitudes towards breastfeeding were measured using a questionnaire. A chi-square test was used to analyze the data. The results showed most of the mother was housewives (80,2%), most of the mother were multipara (64,5%) and most of the respondents have sufficient attitudes towards breastfeeding (90,8%). There were no relationship between mother’s employment and attitudes towards breastfeeding (χ2= 2,089; p-value= 0,352). However, there were significant relationship between maternal’s parity and attitudes towards breastfeeding (χ2= 14,050; p-value= 0,001). This study showed there was no relationship between mother’s employment and attitudes towards breastfeeding, although maternal’s parity showed different results. Therefore, family planning program should be improved correctly to support mothers breastfeed their babies until the six month age to achieve exclusive breastfeeding practice.

Methods: This study is a qualitative research with a phenomenological approach. Sampling was done by purposive sampling with a sample of 8 participants. Data is collected through focus group discussions. Thematic analysis is carried out using colaizi step. Results: This study obtained two themes. Theme 1 is the barriers in the aspect of TB patients and Theme 2, which is barriers from the aspect of health care facilities. Conclusion: This study obtained two themes. Background: Public Healthcare Services (PHCs) provide health care activities, including home visits by public health nurses (PHNs), as part of the Indonesian nursing care program. The present study aimed to evaluate the performance of PHNs and the coverage of the nursing care program by CHCs in Jember (East Java, Indonesia). Methods: A cross-sectional study based on secondary analysis, obtained from the Health Department of Jember District (East Java, Indonesia), was conducted in 2016. The data included quality improvement initiatives and outcomes of the nursing care program, which had been collected from 50 CHCs in Jember. The performance of PHNs and the coverage of the nursing care program by CHCs were evaluated based on three categories, namely nursing care for vulnerable families, nursing care for community groups, and family self-care empowerment. The data were analyzed using the SPSS statistical software (version 22.0). Results: The coverage of the nursing care program by PHCs in the category of vulnerable families, community groups, and self-care empowerment was 48.28%, 44.87%, and 49.50%, respectively. The average coverage (low vs. high) by CHCs in the category of vulnerable families, community groups, and self-care empowerment was 50.0% versus 50.0%, 52.0% versus 48.0%, and 52.0% versus 48.0%, respectively. 21. Dating Behaviour and Its Associated Factors among Female Adolescents in Indonesia: A School-Based Survey

Sexuality and reproductive health are sensitive issues for families, schools and communities to discuss with adolescents in Indonesian social, cultural and religious context. This study aims to identify dating and its associated factors in female adolescents in Indonesia. This school-based survey was conducted among 577 students who completed a self-administered questionnaire that included socio-demographic information, discussion on reproductive health with parents, teachers and peers, and physical and psychological development. The results show that 56.5% (95% CI: 51.1%–61.8%) female adolescents had dating experience. Age (OR= 1.63; 95% CI= 1.13-2.35), lack of information on development (OR= 0.39; 95% CI= 0.23-0.67), menstruation (OR= 3.07); 95% CI= 1.54-6.11), lack of discussion with teachers on the topics of puberty (OR= 2.94; 95% CI= 1.58-5.47) and dating (OR= 1.53; 95% CI= 1.03-2.28) are associated with dating behaviour. Characteristics of adolescents and discussion patterns between children and parents and teachers are also associated with dating behaviour. The results indicate that an integrated school-based sex education program that also involves parents is needed in the context of Indonesia.

Objectives: The management of type 2 diabetes mellitus can be improved for individuals by developing relationships with other patients with diabetes. We created the Relationship Consciousness of Japanese Patients with Type 2 Diabetes Mellitus scale to measure the relationship consciousness of type 2 diabetes mellitus patients for other patients based on the Health Belief Model. Methods: This was a cross-sectional study of Japanese patients with type 2 diabetes mellitus (n = 289). Data were analyzed via exploratory factor analyses, reliability tests, concurrent validity. Results: The final scale obtained for the Relationship Consciousness of Japanese Patients with Type 2 Diabetes Mellitus scale comprised a six-factor structure with 36 items. All 36 items had a Cronbach's α coefficient of 0.893 and explained 59.38% of the total variance. The scale was significantly correlated with a related reciprocity consciousness scale. Conclusions: The Relationship Consciousness of Japanese Patients with Type 2 Diabetes Mellitus scale may be an important tool for nurses to assess the relationship consciousness of patients with type 2 diabetes mellitus. In addition, by understanding patients' relationship consciousness for others who share their disease, nurses can begin to recommend ways to establish relationships between patients that suit patients’ particular relationship. consciousness levels and to provide better care in their clinical practice. Abstract: Objectives: In Indonesia, the prevalence of injuries among children aged 5–14 years is high. Such injuries are generally caused by unsafe school-related factors and a lack of education about first aid. Thus, it is necessary to optimize the role of children by introducing first aid education using the First Aid Guideline (FAG) application. The purpose of this study was to develop a first aid education application for Indonesian children based on the Android operating system. Methods: The application was developed in several stages. The initial stage was the selection of the content that would be included in the application. The second and third stages were the making and testing of the application, respectively. The researchers worked with information technology experts during the development of this application. The application feasibility test included experts in the fields of emergency and medical-surgical medicine as well as information technology. The experts assessed the application by completing a questionnaire on learning media. The results were then analysed by kappa tests to assess the experts' perceptions. Results: The application contains emergency call menus, and information on the treatment of choking, burns, wounds, and bleeding. The application provides pictures and video tutorials, making it easier for users to learn about first aid. Statistical analysis showed a sufficient match between Experts 1 and 3 (mean 3.85 ± SD 0.369; kappa = 0.615; p = 0.035) and Experts 1 and 2 (mean 3.85 ± SD 0.369; kappa = 0.615; p = 0.035). Experts 2 and 3 showed good suitability (mean 3.80 ± SD 0.422; kappa = 0.615; p = 0.035). Conclusions: FAG is a breakthrough application for first aid education for children aged 11–14 years. This application can be used as a medium for learning about first aid. Aim: To examine the impact of the complexity of regulations and restrictive policies on the accessibility of healthcare services for immigrants, particularly maternal and child healthcare. Public health nurses can facilitate management of healthcare services for immigrants in the community by preventing mortality and morbidity among the immigrant population. This literature review was used to identify the challenges faced by public health nurses in providing maternal-child immigrant healthcare. Design: A literature review. Methods: Relevant healthcare databases including PubMed, CINAHL, Scopus, and Google Scholar were searched using the following combination of search terms: public health nurses, maternal care, child care, and immigrants. Results: Six articles were included in the review. Immigrants (undocumented immigrant families in particular) have limited access to public healthcare services, due to factors such as: socio-demographics; language barriers; cultural differences; the status of unregistered families; psychological distress; and problems with health insurance. Public health nurses deliver health education, maternal and postnatal check-ups, and vaccination through community and/or home-based service programs. The role of immigrant health volunteers is facilitated by the partnership between public health nurses and the immigrant population in providing comprehensive maternal and child healthcare in the community. Conclusion: There are barriers that currently impact on the development of public health nurses’ provision of maternal and child healthcare services to immigrants. Strategies that can be implemented to improve public health nurse competencies in maternal and child healthcare of immigrants include bicultural or bilingual training, and cultural competencies in visit programs. Objectives: Social stigma surrounding leprosy patients (LPs) in the community is still related to the adherence of these patients to medication and to their internal motivation for healing. Unfortunately, the leprosy case programme has not been optimally established in public health centres (PHCs). The aim of this study was to assess the social stigma towards adherence to medication and motivation for healing among LPs in PHCs in Indonesia. Methods: A cross-sectional study was conducted from March to May 2017 at PHCs in Jember, Indonesia. Data were collected using a self-administered questionnaire, and secondary data related to medical treatment was assessed from PHCs' medical records. The data were analysed using a t-test and a Chi-square test, while linear regression was used to evaluate factors that could influence LPs' adherence to medication and their motivation for healing. Results: Among the 35 LPs in this study, 25.7% and 74.3%, respectively, had paucibacillary and multibacillary types of leprosy. The type of leprosy was associated with the length of time patients were infected with the disease, their adherence to medication and their motivation for healing. The factors that influenced adherence to medication and motivation for healing were the time period that the LP remained infected and the type of leprosy. Conclusions: Infection period and type of leprosy were associated with adherence to medication and motivation for healing in the community. Efforts should be made to find active cases of leprosy at PHCs. Activities at PHCs should include self-care groups to improve LP adherence to medication and their motivation for healing in the community.

PurposeThis study aimed to evaluate a pilot project of the Nursing Feeding Center “Posyandu Plus” (NFCPP) through local food-based complementary feeding (LFCF) program designed to improve the nutritional status of children aged 6–36 months at community health centers in Indonesia.MethodsA quasi-experimental design was used to obtain data regarding the nutritional status of 109 children who participated in the project from 6 rural areas. The NFCPP was conducted for 9 weeks, comprising 2 weeks of preintervention, 6 weeks of intervention, and one week of postintervention. The LFCF intervention consisted of 12 sets of recipes to be made by mothers and given to their children 4 times daily over 6 weeks. 27. Structural model of factors relating to the health promotion behavior of reproductive health among Indonesian adolescents

Objectives: We aim to explore the factors related to the health promotion behavior of reproductive health (RH) among Indonesian adolescents via structural equation modeling. Methods: A cross-sectional quantitative study was used to measure 108 adolescents aged 12â16 years. We employed a self-administered questionnaire developed from the health promotion model (HPM) and the empowerment model (EM) on the basis of self-efficacy to determine the factors related to health promotion in maintaining RH behavior. Structural equation modeling (SEM) was used to examine relationships among the variables. The Analysis of Moment Structures (AMOS) approach was used to evaluate if the proposed model is suitable for the data based on goodness-of-fit indices. Results: The model is appropriate for the data. Three interrelationships emerge among independent variables (P > 0.05), namely, personal and cognitive behaviors (r = â0.01), cognitive and health promotion behaviors (r = 0.09), and affective and health promotion behaviors (r = â0.17). The strongest relationship is between empowerment and health promotion behavior (r = 0.72). Affection (interpersonal and situational influences), immediate competing demands and preferences, and empowerment (participation and control) are directly related to RH behavior maintenance. Meanwhile, willingness of adolescents, prior related behavior, personal factor, and commitment to a plan of action are indirectly related to RH behavior maintenance. The model explains 43.9% of the variance in maintaining RH behavior. Conclusions: The HPM and EM models help determine the factors related to RH behavior maintenance among adolescents. Intervention to promote RH among Indonesian adolescents should begin from the level of childâparentâhealth care professional encounter for empowering the RH of adolescents. 28. Characteristics of heart failure patients incurring high medical costs via matching specific health examination results and medical claim data: a cross-sectional study

Objective We describe the characteristics of patients with high medical costs by matching specific annual medical examination results and medical claim data. Clarifying the relationships between examination items and high medical costs allows the screening of high-risk persons.Design A cross-sectional study.Subjects Subjects were persons insured by national health insurance in Hiroshima City, Hiroshima Prefecture, from April 2016 to March 2017. To identify true heart failure (HF) patients, the disease name listed in the medical claim data was compared with drugs prescribed for HF, with extraction of only subjects whose comparative data matched.Data collection and analysis The specific health examination includes a questionnaire on areas such as lifestyle habits, anthropometry, blood pressure, blood tests and urine tests. The percentage of the total medical costs related to the medical care of subjects with HF was described using Pareto analysis. For specific health examination items, we compared the high-cost and low-cost groups. The normality and homoscedasticity of each variable was checked and Student’s t-tests and χ² tests were applied. Finally, multiple logistic regression analysis was used to detect factors in the health examination items related to high medical costs.Results Pareto analysis showed that 80% of all medical costs were paid by 30% of the HF patient population. The fees for cardiovascular surgery accounted for 54% of the total surgical cost, 64% of which included preventable diseases. Levels of creatinine (Cr) and γ-glutamyl transpeptidase (γ-GTP) and a history of smoking were found to be related to high medical costs.Conclusion Analysis of specific health examination results for HF patients revealed the association between high medical costs, γ-GTP, Cr, and smoking. These results can thus serve as a reference for screening persons at high risk of HF and help prevent the exacerbation of HF.

This study was purposed to gain value and meaning of Using Tribe experience for utilizing complementary therapies in traditional healing practice. Method: Phenomenology study with qualitative methods based on ethnonursing models was used for this research. There were 13 respondents gathered by purposive sampling from three districts: Glagah, Rogojampi and Banyuwangi Kota. The data were obtained by conducting in-depth interviews on respondents and field observations as well as observations while interview. The analysis carried out by sorting and selecting a word or phrase that has value and meaning to the goal related with sub-themes grouped into themes and categories. Result: The results had showed that Using Tribe’s characteristic were social cohesion, appreciate arts, performing daily activities on the river, consume foods of animal and vegetable, not too close with health service, the myth factor was a strong factor in health and physical maintenance’s behavior, the the use of herbs and shaman to treat health problems. Conclusion and discussion: The use of herbs is suitable with modern health science as a complementary therapy, but this point almost faded because it tends to rarely performed. Objective: This study aims to examine the effects of a community-based friendly health clinic (CFHC) program that adopts manual participatory learning (MPL) intervention on the adolescents living in the rural and urban areas of Indonesia to gain further insights into their knowledge, attitudes, and skills related to adolescent reproductive health (ARH). Methods: A quasi-experimental design was used to obtain information on the ARH knowledge, attitudes, and skills of adolescents. Two intervention studies that used similar protocols and measures were conducted. A total of 192 adolescents (96 adolescents from urban and rural areas) participated in the project, and the participants from each area were divided into eight groups. A questionairre was adopted to measure the ARH knowledge, attitudes, and skills of these participants. A content analysis of the logbook entries of these respondents was conducted to identify their ARH-related problems. Objective: This study aims to identify the knowledge, attitude, and behavior of students using eight indicators of Clean and Healthy Living Behavior (CHLB) in relation to the health education and grade levels in Islamic Boarding Schools (IBS) in Jember regency, East Java Province, Indonesia. Methods: A cross-sectional school-based survey design was used. About 114 students (mean age of 13.17 years old) selected through a multi-stage random sampling completed a self-administered questionnaire to measure the eight indicators of CHLB, such as washing hands with running water and soap, eating healthy snacks, using clean and healthy latrines, exercising regularly and measurably, eradicating mosquito larvae, not smoking at school, weighing and height measurement every 6 months, and disposing garbage properly. The data analyses included descriptive and comparative analyses of the difference in knowledge, attitude, and behavior toward CHLB by health education and grade level. Objectives: Underweight remains a health problem among Indonesian children, and the incidence of overweight continuously increases. This study aims to determine factors associated with underweight and overweight in school-aged children in Indonesia. Methods: This study is a cross-sectional investigation on parents and children aged 6–13 years in elementary schools in Makassar, Indonesia. The participants included 877 children and their parents. Anthropometric data were obtained using standardized equipment, and sociodemographic and lifestyle data were determined using a questionnaire. The nutritional status of the children was assessed based on the child growth standard prescribed by the WHO. Results: The prevalence rates of underweight and overweight among the children were 14.5% and 20.4%, respectively. Underweight was more prevalent in boys. Factors such as mother's level of education, having an underweight father, and playing outdoors on weekends for more than 2 h were significantly associated with underweight children. By contrast, mothers with high levels of education, overweight parents, sleeping for less than 9 h, and playing outdoors on weekends for less than 1 h were significantly associated with overweight children. Conclusions: The prevalence of underweight and overweight among school-aged children in Makassar, Indonesia is high. These conditions are associated with the sociodemographic characteristics of children and parents, as well as the lifestyle of children. Parental characteristics and children's lifestyle should be considered when planning prevention and intervention programs for underweight or overweight children.

Background: Medical and allied health professionals should adopt error reduction as a key strategic priority and make patient safety improvement an ongoing, active process within their organization. Objective: The present study investigates the perceptions of healthcare professionals toward patient safety culture in hospitals throughout Hail Region, Kingdom of Saudi Arabia. Methods: This research, which uses a descriptive cross-sectional approach to evaluate the perceptions of healthcare professionals toward patient safety culture, was conducted at four major hospitals in Hail Region, Kingdom of Saudi Arabia and involved 255 healthcare professionals, who served as respondents based on systematic random sampling. Standard deviation and the chi-square test of independence were used for statistical analysis. This study was approved by the Ethics Review Committee (H-2016-057) of the University of Hail. Results: Among the patient safety aspects, patient safety grade received the highest mean value (3.56 ± 0.72), whereas handoffs and transitions received the least consensus (3.00 ± 0.34). Interestingly, hospital work experience significantly correlated to work area (p = 0.026), communications (p = 0.037), and the number of events reported (p = 0.019) in the patient safety dimension. Moreover, the number of years in the area/unit significantly related to the work area (p = 0.047) and supervisor/manager (p = 0.009). Only the number of events reported showed a significant relationship with specialty or profession (p = 0.007). Conclusion: Healthcare professionals have an affirmative view toward patient safety culture aspects, and positive relationships were found between the patient safety dimensions and study participants’ profile. The findings presented herein suggest that healthcare professionals affirm the practice of patient safety culture. However, further research is required to continuously appraise the significance of healthcare-based quality indicators. Overall, the assessment results suggest a platform for better intervention and transformation procedures targeting the promotion of patient safety culture. OBJECTIVES Complex and diverse factors are related to reproductive health (RH) behavior among adolescents according to the social and cultural context of each countries. This study examined the prevalence of active RH and factors related to active RH behavior among Indonesian adolescents. METHODS A cross-sectional study was conducted among 1,040 of students who were selected through a multi-stage random sampling technique. A self-administered questionnaire was developed, including the World Health Organization Illustrative Questionnaire for Interview-Surveys with Young People, pubertal development scale, and sexual activity scale, modified in accordance to the Indonesian context. The data were analyzed using descriptive and comparative statistics, as well as logistic regression analyses. RESULTS The prevalence of active RH behavior were more higher in boys (56.6%; 95% confidence interval [CI], 50.6% to 62.6%) than in girls (43.7%; 95% CI, 37.6% to 49.8%). Negative attitudes towards RH were a factor related to active RH behavior in both boys and girls. Smoking and kind relationship envisioned before marriage (pacaran [courtship] and nikah siri [non-registered marriage]) were factors related to active RH behavior in boys; whereas the absence of access to information on substance abuse was an additional factor in girls. Moreover, an interaction was found between access to information on development and smoking (boys) and attitudes on RH (girls) as independent variables associated with active RH behavior. CONCLUSIONS Sex education for adolescents in Indonesia, particularly in the context of a health promotion program, should be developed based on prevalent social, cultural, and religious values to prevent active RH behavior. Such programs should focus on the kind of relationship envisioned before marriage and smoking for boys and access to information on subtance abuse for girls.

Tujuan penelitian untuk mengidentifikasi pengeruh pojok remaja terhadap pemenuhan kebutuhan kesehatan reproduksi remaja yang mengikuti P2R ditinjau dari pengetahuan, sikap, dan perilaku kesehatan reproduksi remaja. Desain penelitian menggunakan quasi eksperiment dengan rancangan randomized control group design with pretest dan postest. Sampel penelitian 45 responden kelompok perlakuan dan kontrol yang diambil secara cluster sampling. Hasil uji mann whitney dengan alfa 0,05 disimpulkan ada pengaruh pemberian program pojok remaja terhadap pemenuhan kebutuhan kesehatan reproduksi remaja (p 0,022). Berdasarkan hasil penelitian disarankan untuk dilakukan peningkatan pembinaan pelayanan kesehatan remaja di sekolah yang terintegrasi dengan program UKS. Penelitian bertujuan mengaplikasikan terapi keperawatan keluarga terhadap tingkat kemandirian keluarga dengan permasalahan kesehatan reproduksi remaja. Metode penelitian deskriptif analitik dengan rancangan cross sectional. Sample pada penelitian ini adalah 10 keluarga dengan tahap perkembangan remaja yang berisiko mengalami permasalahan kesehatan reproduksi di Kelurahan Ratujaya Kecamatan Pancoran Mas Kota Depok. Hasil penelitian tingkat kemandirian keluarga dalam mengatasi permasalahan kesehatan reproduksi remaja dikaitkan dengan 5 tugas keluarga dalam mengatasi masalah kesehatan keluarga. Lima keluarga dengan kemadirian tingkat III, empat keluarga dengan tingkat kemandirian II, dan satu keluarga dengan tingkat kemandirian I. Kesimpulan penelitian tehnik pemberian pendidikan kesehatan (KIE: komunikasi, informasi dan edukasi), coaching dan conseling dalam pengembangan dan ketrampilan hidup remaja (tanggung jawab, kepercayaan diri, dan penolakan ajakan pergaulan bebas secara asertif), dan pengembangan ketrampilan orang tua dalam berkomunikasi secara efektif dengan remaja. Saran perlu adanya program peer group dan social support group untuk remaja dalam menjalani pertumbuhan dan perkembangan terutama dalam masalah kesehatan reproduksi. - Abstract
The risk of suicide is one of the most vulnerable nursing problems experienced by the elderly. The risk of suicide occurs because of several suicide risk factors experienced by the elderly. This study aims to describe the risk of suicide in the elderly at UPT PSTW (nursing home) Jember. The research design used is a descriptive survey. The sampling technique used is total sampling. The sample used in this study was 76 elderly who were selected based on inclusion and exclusion criteria. Data were collected using the Geriatric Suicide Ideation Scale (GSIS) questionnaire to determine the risk of suicide in the elderly. The One Sample Kolmogorov-Smirnov test was used to achieve the objectives of this study. The results showed that the elderly experienced a significant risk of suicide but in the low category Md (P25-75)= 62(60-65); Z=0.151; p-value=0.000. This study concludes that there is a significant risk of suicide in the elderly at UPT PSTW Jember. Therefore, it is necessary to increase the spirit of life for the elderly by always reminding the elderly that there is still life and hope for them.

40. Grand Parent of Parenting Style and Incidence of Stunting Among Toddlers in Indonesia: A Literature Review

- Abstract
Toddlers with stunting have similar parenting styles, especially parenting style of grandpa and/or grandma. Grand parents have roled for fulfilling toddler nutrition. Parenting style of grand parents is important to facilitate of nutrition among toddlers during their growth and development. The aimed of this study was to determine the parenting style of grandpa and/or grandma and incidence of stunting among toddlers in Indonesia. The research method was used a literature review study. Article search was carried out by searching the database through Google Scholar, PubMed, and Sinta using predetermined keyword combinations, which were then analyzed using a four-stage prism flowchart to obtain 12 articles for review. The results of a review of 12 articles found that authoritative parenting, authoritarian parenting, and permissive parenting are parenting styles adopted by grandparents that can influence the incidence of toddler stunting in Indonesia. Parenting styles of grandparents with authoritative parenting can affect optimal nutritional status and the risk of stunting with toddlers is very low, while parenting styles of grandparents with authoritarian and permissive parenting can affect poor nutritional status and have a high risk of stunting with toddlers. The conclusion from this study is that the parenting style of grandpa and/or grandma influences the incidence of stunting in toddlers.

Stunting is the inadequacy of nutritional intake from the womb to the child at the age of five, which is indicated by the child having less length or height comparing to the age. In this study, the researcher linked family mobility as one of the factors causing the increasing case of stunting. The purpose of this literature study is to describe family mobilization with the stunting case in children under five in Indonesia. The research design used in this study was a traditional literature review. Articles are tracked using several search engines (Google Scholar, PubMed, Springer link, Sinta, and Plos One). The results of this study indicated that family mobilization in reaching health services can affect the nutritional status of children under five. The affordability of health services is caused by several factors such as road access, distance and travel time. It also relates to the family residence, the pattern of care provided, and the socio-economic status of the family which can cause children under five experiencing stunting. The conclusion in this study is that family mobilization such as a lack of family health attention in reaching health services, inadequate needs and a healthier lifestyle or low economic status, as well as a poor or lack parenting knowledge can cause children under five susceptible to infectious diseases so that the children are prone to stunting.

Physical activity is one of the factors that can affect the incidence of hypertension. Individuals who lack physical activity will be at risk for hypertension. Therefore, this study aimed to identify the relationship between physical activity and blood pressure participants in non-communicable diseases of integrated development post (Posbindu PTM) Jenggawah Public Health Center, Jember. The retrospective case-control study design was used to analyze secondary data of Posbindu PTM registered from September to November 2020 among 126 participants. Characteristics of participants (age, gender, education, marital and occupational status), physical activity, and blood pressure were measured based on the health chart (KMS) of Posbindu PTM. Among 126 participants in Posbindu PTM identified 58% of having adequate activity and 60% of normal blood pressure. Meanwhile, there was a relationship between physical activity and the blood pressure of Posbindu PTM (χ2=5.795; p-value=0.016). Furthermore, Posbindu PTM participants who had enough physical activity were 0.4 times maintaining the blood pressure (OR: 0.411; 95% CI = 0.198-0.853). Physical activity is correlated with blood pressure among participants in Posbindu PTM. Therefore, the adequacy of physical activity should be improved to maintain blood pressure and prevent the risk of hypertension.

Obesity is a risk factor for hyperglycemia. Farmers are a group of people at risk of obesity and hyperglycemia due to a lifestyle that consumes foods high in fat and carbohydrates. The aim of this study wants to analyze the relationship between body mass index and blood sugar levels of farmers in the report of Integrated Development Post of Non-Communicable Disease (Posbindu PTM) Pakusari Jember Health Center. The survey analysis study used a retrospective cohort study. A design was used to analyze secondary data of Posbindu PTM Pakusari Jember Health Center registered from January to October 2020 among 345 farmers. A population of 345 farmers was selected by purposive sampling to be used as research samples. Characteristics of participants, height and weight, and random blood sugar levels were measured through a health monitoring book (KMS Posbindu PTM). The chi-Square test was used to answer the objective of the study. Among 146 farmers identified that 46.6% were obese and 45.9% were hyperglycemia. There was a relationship between body mass index and blood sugar levels in farmers (p-value = 0.000). Indirectly, body mass index affects blood sugar levels, manifesting as diabetes mellitus and other complications. Therefore, obesity should be identified as a risk factor associated with farmers' blood sugar levels.

Consumption of less vegetables and fruits in a person risks for overweight and obese. Farmers are one job with a low risk of consuming vegetables and fruit (< 250 grams) in one day. The purpose of this research was to know the association between the consumption of vegetable and fruits and body mass index among farmers in Posts of Non-Communicable Diseases (Posbindu PTM) Jenggawah Public Health Center, Jember Regency. A retrospective case-control study was used to analyze secondary data of Posbindu PTM registered from September to November 2020 among 81 farmers. The characteristics of participants, vegetable and fruit consumption, and body mass index were measured through KMS Posbindu PTM. There was a relationship between the consumption of vegetable and fruit with body mass index among farmers (p-value = 0.006; χ2= 8.725). Among 81 farmers were identified 59.3% less consumption of vegetables and fruit per day and 53,1% of obese. Farmers who consume less vegetables and fruit have a four times greater risk of being obese (OR= 4.00; 95% C = 1.562 – 10.242). Consumption vegetable and fruit is correlated with body mass index among farmers. Therefore, consuming vegetables and fruits should be improved to reduce the risk of obesity among farmers.

The problem of stunting can be immediately overcome by optimizing the local food sources. The purpose of this community service is to optimize the use of agricultural products as nutritious food through Posyandu Plus with an agronursing approach. The approach taken in this service is through participatory action research in Patemon and Bedadung Villages for 3 months. Partners are involved in the establishment, implementation, and evaluation of the Nursing Feeding Center Care. A series of activities were carried out including health education, group process for making additional food, empowerment in the food diversification program, and the implementation of posyandu plus. The measurement of the success of the program is carried out by increasing the knowledge, attitudes, and behavior of mothers in the care of stunting toddlers. Problems in the family can cause temper tantrum behavior in children. The effective function of the family aims to protect and provide psychosocial support for family members. This study aims to determine the relationship between family affective function and temper tantrum behavior in children. The research design is an analytic correlation with a cross-sectional study approach. Respondents in this study were 134 parents who had children aged 3-5 years. Sampling using a cluster sampling method. The questionnaires used in this study were the Family Affective Function Questionnaire and the Temper Tantrum Behavior Questionnaire. The parametric test in this study uses an independent sample t-test because the data is normally distributed. Temper tantrum behavior in children aged 3-5 years is screaming or screaming. Families with children aged 3-5 years can carry out family affective functions effectively. The elderly experience limitations in carrying out physical activities during the COVID-19 pandemic which can reduce the welfare of the elderly. Due to the aging process, physiological changes and decreased immune function in the elderly make the elderly group a group with a high risk of exposure to COVID-19. The purpose of this study was to describe the physical activity of the elderly in the Nursing Home of Banyuwangi. Descriptive quantitative research was performed among 32 elders in May 2021. A self-administered questionnaire was used to identify characteristics of the elderly and their physical activity with the Physical Activity Scale for Elderly (PASE). The One-sample Kolmogorov-Smirnov’s test was used to reach the goal of this study. The result shows that the physical activity of the elderly didn’t significantly change during the pandemic t = 0,119; p-value 0,200. It can be concluded that there is no significant change in elders’ physical activity during the COVID-19 pandemic.

Introduction: The people of Sumba prefer traditional medication from shamans as they believe that diabetes they suffer from the Marapu, hence uncurable by modern medical means. This study aimed to derive a picture regarding diabetes patients’ cultural values, beliefs, and lifeways in the East Sumba ethnic of Indonesia. Methods: This study uses a qualitative design with an ethnographic approach which was conducted in 2018. The main participants in the study were seven diabetic patients, were determined purposively. Focused discussions were held with the person in charge of the non-communicable disease program from seven Public Health Centers. Data were collected from interviews, focus group discussions, field notes and observation sheets. All data were analyzed by content analysis method corresponding to the 4 phases in ethnonursing. Results: Five themes were generated in this study with details of three themes describing the cultural values of diabetic patients (subthemes traditional ceremonies, chewing happa, and traditional medicine), one theme describing the beliefs of diabetic patients (subthemes perceptions about diabetes) and one theme describing the lifestyle of diabetic patients (subthemes eating/drinking patterns, spiritual values, rest/sleep habits, and daily activities). The culture of diabetics depicted in adat ceremonies, happa or betel nut chewing, and traditional medication, which is still thick in East Sumba and their beliefs on the disease they suffer from, exert influence on their daily lives, including their eating or drinking patterns, spiritual values, resting or sleeping patterns, and daily activities. Conclusion: This finding can be helpful for the effort to improve nursing for diabetes patients by cultural approaches.

- Abstract
Hypertension in the elderly is a common problem that is often found. Many factors affect hypertension in the elderly, so it is necessary to empower them elderly. The purpose of this empowerment to control the blood pressure of the elderly through progressive muscle relaxation (PMR) techniques. The method used is counselling and exercise demonstration, which consists of 3 main activities: screening, PMR technique training, and activity evaluation. The empowerment was attended by nine elderly at UPT PSTW Wisma Cempaka Jember. The evaluation of activities is to measure blood pressure with a stethoscope and sphygmomanometer. Wilcoxon statistical test was used to determine changes in blood pressure before and after PMR intervention. The elderly are 100% male, 55.6% are 60-70 years old, 44.4% live in an orphanage for 1-5 years, and 66.7% are from outside Jember and have itching 44.4%. The p-value was 0.020.05 in diastole, which means there was no difference in blood pressure after PMR. PMR focuses on maintaining a relaxed state that can secrete CRH (Corticotropin-releasing Hormone) and ACTH (Adrenocorticotropic releasing hormone) so that adrenaline and non-adrenaline secretion decrease, which has an impact on heart contraction activity and decreased blood pressure. Elderly empowerment with progressive muscle relaxation can control blood pressure, specifically systolic, after one week of exercise. This exercise must be included in the PSTW exercise program, which can be done regularly to control blood pressure.
