Abstract: A search for proton decay into $e^+/\mu^+$ and a $\eta$ meson has been performed using data from a 0.373 Mton$\cdot$year exposure (6050.3 live days) of Super-Kamiokande. Compared to previous searches this work introduces an improved model of the intranuclear $\eta$ interaction cross section, resulting in a factor of two reduction in uncertainties from this source and $\sim$10\% increase in signal efficiency. No significant data excess was found above the expected number of atmospheric neutrino background events resulting in no indication of proton decay into either mode. Lower limits on the proton partial lifetime of $1.4\times\mathrm{10^{34}~years}$ for $p\rightarrow e^+\eta$ and $7.3\times\mathrm{10^{33}~years}$ for $p\rightarrow \mu^+\eta$ at the 90$\%$ C.L. were set. These limits are around 1.5 times longer than our previous study and are the most stringent to date.

2. Measurement of $\gamma$-rays generated by neutron interaction with ${}^{16}$O at 30 MeV and 250 MeV

Abstract: Deep understanding of $\gamma$-ray production from the fast neutron reaction in water is crucial for various physics studies at large-scale water Cherenkov detectors. We performed test experiments using quasi-mono energetic neutron beams ($E_n = 30$ and 250~MeV) at Osaka University's Research Center for Nuclear Physics to measure $\gamma$-rays originating from the neutron-oxygen reaction with a high-purity germanium detector. Multiple $\gamma$-ray peaks which are expected to be from excited nuclei after the neutron-oxygen reaction were successfully observed. We measured the neutron beam flux by using a liquid scintillator for the cross section measurement. With a spectral fitting analysis based on the tailored $\gamma$-ray signal and background templates, we measured cross sections for each observed $\gamma$-ray component. The results will be useful to validate neutron models employed in the on-going and future water Cherenkov experiments.

Abstract: The Super-Kamiokande and T2K collaborations present a joint measurement of neutrino oscillation parameters from their atmospheric and beam neutrino data. It uses a common interaction model for events overlapping in neutrino energy and correlated detector systematic uncertainties between the two datasets, which are found to be compatible. Using 3244.4 days of atmospheric data and a beam exposure of $19.7(16.3) \times 10^{20}$ protons on target in (anti)neutrino mode, the analysis finds a 1.9$\sigma$ exclusion of CP-conservation (defined as $J_{CP}=0$) and a preference for the normal mass ordering., Comment: 12 pages, 4 figures

Abstract: Preceding a core-collapse supernova, various processes produce an increasing amount of neutrinos of all flavors characterized by mounting energies from the interior of massive stars. Among them, the electron antineutrinos are potentially detectable by terrestrial neutrino experiments such as KamLAND and Super-Kamiokande via inverse beta decay interactions. Once these pre-supernova neutrinos are observed, an early warning of the upcoming core-collapse supernova can be provided. In light of this, KamLAND and Super-Kamiokande, both located in the Kamioka mine in Japan, have been monitoring pre-supernova neutrinos since 2015 and 2021, respectively. Recently, we performed a joint study between KamLAND and Super-Kamiokande on pre-supernova neutrino detection. A pre-supernova alert system combining the KamLAND detector and the Super-Kamiokande detector was developed and put into operation, which can provide a supernova alert to the astrophysics community. Fully leveraging the complementary properties of these two detectors, the combined alert is expected to resolve a pre-supernova neutrino signal from a 15 M$_{\odot}$ star within 510 pc of the Earth, at a significance level corresponding to a false alarm rate of no more than 1 per century. Abstract: Neutrinos from very nearby supernovae, such as Betelgeuse, are expected to generate more than ten million events over 10\,s in Super-Kamokande (SK). At such large event rates, the buffers of the SK analog-to-digital conversion board (QBEE) will overflow, causing random loss of data that is critical for understanding the dynamics of the supernova explosion mechanism. In order to solve this problem, two new DAQ modules were developed to aid in the observation of very nearby supernovae. The first of these, the SN module, is designed to save only the number of hit PMTs during a supernova burst and the second, the Veto module, prescales the high rate neutrino events to prevent the QBEE from overflowing based on information from the SN module. In the event of a very nearby supernova, these modules allow SK to reconstruct the time evolution of the neutrino event rate from beginning to end using both QBEE and SN module data. This paper presents the development and testing of these modules together with an analysis of supernova-like data generated with a flashing laser diode. We demonstrate that the Veto module successfully prevents DAQ overflows for Betelgeuse-like supernovae as well as the long-term stability of the new modules. During normal running the Veto module is found to issue DAQ vetos a few times per month resulting in a total dead time less than 1\,ms, and does not influence ordinary operations. Additionally, using simulation data we find that supernovae closer than 800~pc will trigger Veto module resulting in a prescaling of the observed neutrino data., Comment: 28 pages, 18 figures. Abstract: We present the results of the charge ratio ($R$) and polarization ($P^{\mu}_{0}$) measurements using the decay electron events collected from 2008 September to 2022 June by the Super-Kamiokande detector. Because of its underground location and long operation, we performed high precision measurements by accumulating cosmic-ray muons. We measured the muon charge ratio to be $R=1.32 \pm 0.02$ $(\mathrm{stat.}{+}\mathrm{syst.})$ at $E_{\mu}\cos \theta_{\mathrm{Zenith}}=0.7^{+0.3}_{-0.2}$ $\mathrm{TeV}$, where $E_{\mu}$ is the muon energy and $\theta_{\mathrm{Zenith}}$ is the zenith angle of incoming cosmic-ray muons. This result is consistent with the Honda flux model while this suggests a tension with the $\pi K$ model of $1.9\sigma$. We also measured the muon polarization at the production location to be $P^{\mu}_{0}=0.52 \pm 0.02$ $(\mathrm{stat.}{+}\mathrm{syst.})$ at the muon momentum of $0.9^{+0.6}_{-0.1}$ $\mathrm{TeV}/c$ at the surface of the mountain; this also suggests a tension with the Honda flux model of $1.5\sigma$. This is the most precise measurement ever to experimentally determine the cosmic-ray muon polarization near $1~\mathrm{TeV}/c$. Abstract: The first loading of gadolinium (Gd) into Super-Kamiokande in 2020 was successful, and the neutron capture efficiency on Gd reached 50\%. To further increase the Gd neutron capture efficiency to 75\%, 26.1 tons of $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$ was additionally loaded into Super-Kamiokande (SK) from May 31 to July 4, 2022. As the amount of loaded $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$ was doubled compared to the first loading, the capacity of the powder dissolving system was doubled. We also developed new batches of gadolinium sulfate with even further reduced radioactive impurities. In addition, a more efficient screening method was devised and implemented to evaluate these new batches of $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$. Following the second loading, the Gd concentration in SK was measured to be $333.5\pm2.5$ ppm via an Atomic Absorption Spectrometer (AAS). From the mean neutron capture time constant of neutrons from an Am/Be calibration source, the Gd concentration was independently measured to be 332.7 $\pm$ 6.8(sys.) $\pm$ 1.1(stat.) ppm, consistent with the AAS result. Abstract: Among multi-messenger observations of the next galactic core-collapse supernova, Super-Kamiokande (SK) plays a critical role in detecting the emitted supernova neutrinos, determining the direction to the supernova (SN), and notifying the astronomical community of these observations in advance of the optical signal. On 2022, SK has increased the gadolinium dissolved in its water target (SK-Gd) and has achieved a Gd concentration of 0.033%, resulting in enhanced neutron detection capability, which in turn enables more accurate determination of the supernova direction. Accordingly, SK-Gd's real-time supernova monitoring system (Abe te al. 2016b) has been upgraded. SK_SN Notice, a warning system that works together with this monitoring system, was released on December 13, 2021, and is available through GCN Notices (Barthelmy et al. 2000). When the monitoring system detects an SN-like burst of events, SK_SN Notice will automatically distribute an alarm with the reconstructed direction to the supernova candidate within a few minutes. In this paper, we present a systematic study of SK-Gd's response to a simulated galactic SN. Assuming a supernova situated at 10 kpc, neutrino fluxes from six supernova models are used to characterize SK-Gd's pointing accuracy using the same tools as the online monitoring system. The pointing accuracy is found to vary from 3-7$^\circ$ depending on the models. However, if the supernova is closer than 10 kpc, SK_SN Notice can issue an alarm with three-degree accuracy, which will benefit follow-up observations by optical telescopes with large fields of view., Comment: 38 pages, 29 figures, 6 tables

Abstract: An analysis of solar neutrino data from the fourth phase of Super-Kamiokande~(SK-IV) from October 2008 to May 2018 is performed and the results are presented. The observation time of the data set of SK-IV corresponds to $2970$~days and the total live time for all four phases is $5805$~days. For more precise solar neutrino measurements, several improvements are applied in this analysis: lowering the data acquisition threshold in May 2015, further reduction of the spallation background using neutron clustering events, precise energy reconstruction considering the time variation of the PMT gain. The observed number of solar neutrino events in $3.49$--$19.49$ MeV electron kinetic energy region during SK-IV is $65,443^{+390}_{-388}\,(\mathrm{stat.})\pm 925\,(\mathrm{syst.})$ events. Corresponding $\mathrm{^{8}B}$ solar neutrino flux is $(2.314 \pm 0.014\, \rm{(stat.)} \pm 0.040 \, \rm{(syst.)}) \times 10^{6}~\mathrm{cm^{-2}\,s^{-1}}$, assuming a pure electron-neutrino flavor component without neutrino oscillations. The flux combined with all SK phases up to SK-IV is $(2.336 \pm 0.011\, \rm{(stat.)} \pm 0.043 \, \rm{(syst.)}) \times 10^{6}~\mathrm{cm^{-2}\,s^{-1}}$. Based on the neutrino oscillation analysis from all solar experiments, including the SK $5805$~days data set, the best-fit neutrino oscillation parameters are $\rm{sin^{2} \theta_{12,\,solar}} = 0.306 \pm 0.013 $ and $\Delta m^{2}_{21,\,\mathrm{solar}} = (6.10^{+ 0.95}_{-0.81}) \times 10^{-5}~\rm{eV}^{2}$, with a deviation of about 1.5$\sigma$ from the $\Delta m^{2}_{21}$ parameter obtained by KamLAND. The best-fit neutrino oscillation parameters obtained from all solar experiments and KamLAND are $\sin^{2} \theta_{12,\,\mathrm{global}} = 0.307 \pm 0.012 $ and $\Delta m^{2}_{21,\,\mathrm{global}} = (7.50^{+ 0.19}_{-0.18}) \times 10^{-5}~\rm{eV}^{2}$., Comment: 47 pages, 61 figures

Abstract: We present a measurement of neutrino oscillation parameters with the Super-Kamiokande detector using atmospheric neutrinos from the complete pure-water SK I-V (April 1996-July 2020) data set, including events from an expanded fiducial volume. The data set corresponds to 6511.3 live days and an exposure of 484.2 kiloton-years. Measurements of the neutrino oscillation parameters $\Delta m^2_{32}$, $\sin^2\theta_{23}$, $\sin^2 \theta_{13}$, $\delta_{CP}$, and the preference for the neutrino mass ordering are presented with atmospheric neutrino data alone, and with constraints on $\sin^2 \theta_{13}$ from reactor neutrino experiments. Our analysis including constraints on $\sin^2 \theta_{13}$ favors the normal mass ordering at the 92.3% level., Comment: 24 pages, 18 figures

Abstract: We report the first measurement of the atmospheric neutrino-oxygen neutral-current quasielastic (NCQE) cross section in the gadolinium-loaded Super-Kamiokande (SK) water Cherenkov detector. In June 2020, SK began a new experimental phase, named SK-Gd, by loading 0.011% by mass of gadolinium into the ultrapure water of the SK detector. The introduction of gadolinium to ultrapure water has the effect of improving the neutron-tagging efficiency. Using a 552.2 day data set from August 2020 to June 2022, we measure the NCQE cross section to be 0.74 $\pm$ 0.22(stat.) $^{+0.85}_{-0.15}$ (syst.) $\times$ 10$^{-38}$ cm$^{2}$/oxygen in the energy range from 160 MeV to 10 GeV, which is consistent with the atmospheric neutrino-flux-averaged theoretical NCQE cross section and the measurement in the SK pure-water phase within the uncertainties. Furthermore, we compare the models of the nucleon-nucleus interactions in water and find that the Binary Cascade model and the Liege Intranuclear Cascade model provide a somewhat better fit to the observed data than the Bertini Cascade model. Since the atmospheric neutrino-oxygen NCQE reactions are one of the main backgrounds in the search for diffuse supernova neutrino background (DSNB), these new results will contribute to future studies - and the potential discovery - of the DSNB in SK., Comment: 8 pages, 3 figures

Abstract: We report a search for time variations of the solar $^8$B neutrino flux using 5804 live days of Super-Kamiokande data collected between May 31, 1996, and May 30, 2018. Super-Kamiokande measured the precise time of each solar neutrino interaction over 22 calendar years to search for solar neutrino flux modulations with unprecedented precision. Periodic modulations are searched for in a dataset comprising five-day interval solar neutrino flux measurements with a maximum likelihood method. We also applied the Lomb-Scargle method to this dataset to compare it with previous reports. The only significant modulation found is due to the elliptic orbit of the Earth around the Sun. Abstract: PurposeUreteral access sheaths (UAS) pose the risk of severe ureteral injury. Our prior studies revealed forces ≤6 Newtons (N) prevent ureteral injury. Accordingly, we sought to define the force urologists and residents-in-training typically use when placing a UAS.Materials & methodsAmong urologists and urology residents attending two annual urological conferences in 2022, 121 individuals were recruited for the study. Participants inserted 12Fr, 14Fr, and 16Fr ureteral access sheaths into a male genitourinary model containing a concealed force sensor; they also provided demographic information. Analysis was completed using t-tests and Chi-square tests to identify group differences when passing a 16Fr sheath UAS. Participant traits associated with surpassing or remaining below a minimal force threshold were also explored via polychotomous logistic regression.ResultsParticipant force distributions were: ≤4N (29%), >6N (45%), and >8N (32%). More years of practice were significantly associated with exerting >6N relative to forces between 4-6N; results for >8N relative to 4N-8N were similar. Compared to high-volume ureteroscopists (those performing >20 ureteroscopies/month), physicians performing ≤20 ureteroscopies/month were significantly less likely to exert forces ≤4N (p=0.017 and p=0.041). Of those surpassing 6N and 8N, 15% and 18% respectively were high-volume ureteroscopists.ConclusionsDespite years of practice or volume of monthly ureteroscopic cases performed, most urologists failed to pass 16Fr access sheaths within the ideal range of 4N-6N (74% of participants) or within a predefined safe range of 4N-8N (61% of participants).

20. Computational multiphysics modeling of radioactive aerosol deposition in diverse human respiratory tract geometries

Abstract: Abstract The evaluation of aerosol exposure relies on generic mathematical models that assume uniform particle deposition profiles over the human respiratory tract and do not account for subject-specific characteristics. Here we introduce a hybrid-automated computational workflow that generates personalized particle deposition profiles in 3D reconstructed human airways from computed tomography scans using Computational Fluid and Particle Dynamics simulations. This is the first large-scale study to consider realistic airways variability, where 380 lower and 40 upper human respiratory tract 3D geometries are reconstructed and parameterized. The data is clustered into nine groups using random forest regression. Computational fluid and particle dynamics simulations are conducted on these representative geometries using a realistic heavy-breathing respiratory cycle and radioactive iodine-131 as a source term. Monte Carlo radiation transport simulations are performed to obtain detailed energy deposition maps. Our findings emphasize the importance of personalized studies, as minor respiratory tract variations notably influence deposition patterns rather than global parameters of the lower airways, observing more than 30% variance in the mass deposition fraction.

Abstract: Abstract Ovarian cancer (OvCa) is one of the most lethal gynecological malignancies, and most patients are diagnosed at advanced stage with peritoneal dissemination. Although age at diagnosis is considered an independent prognostic factor, its impact on peritoneal recurrence after combined cytoreductive surgery and chemotherapy is not clear. The objective of this study was to investigate the impact of aging on peritoneal recurrence from stealth dissemination and gain insight of the pathophysiology of OvCa in elderly patients. A total of 243 patients with pT2b-pT3 epithelial ovarian who achieved complete surgery, no-residual tumor at first surgery, were selected to be analyzed the risk of peritoneal seeding and recurrence. We found that age over 65 years was independently associated with an increased risk of peritoneum-specific (PS) recurrence (. Furthermore, pT3 stages and positive ascites cytology also worsen the PS-relapse-free survival. Collectively, our findings suggest that age, especially over 65 years, predicts reduced peritoneum-specific tumor recurrence in patients with advanced ovarian cancer after complete cytoreduction surgery, particularly those with pT3 tumors and positive ascites cytology.

Abstract: We report the first search result for the flux of astrophysical electron antineutrinos for energies O(10) MeV in the gadolinium-loaded Super-Kamiokande (SK) detector. In June 2020, gadolinium was introduced to the ultra-pure water of the SK detector in order to detect neutrons more efficiently. In this new experimental phase, SK-Gd, we can search for electron antineutrinos via inverse beta decay with efficient background rejection and higher signal efficiency thanks to the high efficiency of the neutron tagging technique. In this paper, we report the result for the initial stage of SK-Gd with a $22.5\times552$ $\rm kton\cdot day$ exposure at 0.01% Gd mass concentration. No significant excess over the expected background in the observed events is found for the neutrino energies below 31.3 MeV. Thus, the flux upper limits are placed at the 90% confidence level. The limits and sensitivities are already comparable with the previous SK result with pure-water ($22.5 \times 2970 \rm kton\cdot day$) owing to the enhanced neutron tagging.

Abstract: Abstract Background Few reported studies evaluate the status of those who have a family dentist (FD) by regional differences and the socioeconomic factors associated with this status. This study aimed to assess the prevalence of having an FD among Japanese individuals across three samples of municipality type: urban, intermediate, and rural areas, and determine the factors associated with having an FD. Methods This was a cross-sectional study involving a web-based survey. In total, 2,429 participants (comprising men and women aged 20–69 years) were randomly selected from among the registrants of a web research company: 811 urban residents, 812 intermediate residents, and 806 rural residents. In each area, we categorized the participants into those who had an FD (FD group) and those who did not (non-FD group). A multivariate modified Poisson regression analysis was used to determine the factors associated with the FD group as compared to the non-FD group. Results The proportion of the FD group was lowest in rural areas (42.3%), followed by intermediate (48.6%) and urban areas (49.7%). The regression analysis revealed a statistically significant tendency between associated factors in the two groups; that is, the higher the household income, the more likely that the family belonged to the FD group (prevalence ratio (95%CI), JPY 4–6 million: 1.43 (1.00–2.03), JPY ≥ 8 million: 1.72 (1.21–2.44)). Conclusions Rural areas have the lowest proportion of people with an FD among the three areas, and income inequality is associated with having an FD. Thus, when planning policies to encourage individuals to have an FD to manage their oral health, it is necessary to consider regional differences.

Abstract: Introduction: Multiple sclerosis is a progressive disease that is difficult to predict, originating in cases of disability. Natalizumab is a highly effective disease-modifying therapy but is associated with greater John Cunningham virus (JCV) reactivation and consequent increased risk of developing Progressive Multifocal Leukoencephalopathy (PML). Objective: To analyze JCV seroconversion in patients treated with natalizumab. Methods: A retrospective study was conducted involving patients diagnosed with multiple sclerosis, between January 2012 and December 2021. To assess seroconversion during treatment with natalizumab, patients were considered seronegative at the beginning of treatment and who had at least one result in the period of medication use. The study was approved by the Human Research Ethics Committee under protocol 3,177,442. Results: Sixty-two patients treated with Natalizumab were included, with a seroprevalence of 67.7%. At the start of treatment, 41.9% (26/62) of the patients were negative for anti-JCV, of which 23.1% (6/26) were seroconverted. The mean time to seroconversion was 2.5 years. The baseline index of anti-JCV antibodies was statistically significant with the age of the patients. Among patients with a negative anti-JCV antibody result at baseline, 82.6% (19/23) remained negative throughout monitoring. Treatment was discontinued in 53.2% (33/62) of patients, and 72.7% (24/33) due to anti-JCV positivity with a consumption index >1.5 in 41.9% of cases. Conclusion: Knowing how to monitor the anti-JCV antibody index and treatment approaches in our patient cohort may be useful in future clinical decisions in treating MS.

Abstract: Genital warts (GWs) are the most common sexually transmitted infections worldwide, caused by the human papillomavirus (HPV). In adults, the primary mode of transmission is through sexual contact, whereas in children it can occur through skin-to-skin or skin-to-mucosa contact and be sexual or non-sexual. The increasing prevalence of GWs in children has renewed the interest in therapeutic management which still presents a unique challenge, being influenced by many variables including size, quantity, and location of warts, as well as the presence of comorbidities. Photodynamic therapy (PDT) has already shown encouraging results in treating viral warts in adult patients, but its use is still not standardized in the pediatric population. On this topic, we report the case of an otherwise healthy 5-year-old child affected by GWs, successfully treated with three sessions of PDT with 10% 5-aminolaevulinic acid (ALA), at one-month intervals. Our case is paradigmatic of the potentiality of PDT to treat difficult lesions in a pediatric setting.

Abstract: Introdução: Mesmo com o advento do uso da terapia antirretroviral para tratamento dos pacientes portadores do vírus HIV e com o maior conhecimento que se tem da Aids, ela continua sendo endêmica em várias regiões do mundo, com novos casos diagnosticados diariamente e sendo responsável por inúmeras internações. Entre as doenças oportunistas, a neurotoxoplasmose está entre as mais prevalentes. Em pacientes com baixa contagem de linfócitos T CD4 e carga viral detectável, as chances de adquirir a doença são maiores. Há poucos casos descritos na literatura de neurotoxoplasmose com necessidade de derivação ventricular. Objetivo: Relatar caso de paciente com Aids com neurotoxoplasmose com necessidade de derivação ventricular. Método: Relato de caso. Resultados: A.F., masculino, 71 anos, previamente hígido, com histórico de quedas frequentes há 2 anos e perda de controle esfincteriano há 1 semana, encaminhado ao serviço terciário para investigação de relação com os sintomas neurológicos. Na admissão o paciente apresentava hemiparesia direita associado a desorientação em tempo e espaço, pupilas anisocóricas e candidíase oral. Realizou testagem rápida para HIV com resultado reagente. Contagem de CD4 18 células e carga viral de 33.400 cópias/mL. Realizou tomografia de crânio com lesão extensa à esquerda associada a sinais de hidrocefalia. Introduzido tratamento para neurotoxoplasmose com clindamicina, pirimetamina e ácido folínico. Ressonância de crânio evidenciou estenose de aqueduto com hidrocefalia supratentorial. Neurocirurgia indicou realização de ventriculostomia endoscópica. Introduzido esquema antirretroviral com necessidade de troca para dolutegravir, darunavir e ritonavir devido disfunção renal. Realizou ressonância de crânio de controle, constatando regressão das lesões e diminuição da hidrocefalia. Recebeu alta com melhora significativa do déficit motor. Conclusão: A neurotoxoplasmose é uma doença oportunista que compromete principalmente região próxima dos núcleos da base, apresentando boa resposta ao tratamento clínico. A intervenção neurocirúrgica é raramente empregada, mas deve ser considerada em situações em que o paciente nao apresenta boa resposta terapêutica, para realizar diagnóstico diferencial ou em raros casos com manifestação compressiva, complicando com a hidrocefalia como o caso relatado.

Abstract: Introdução: Em pacientes infectados com HIV, a investigação de doenças oportunistas é imprescindível por serem responsáveis pela elevada morbimortalidade da doença, sendo pneumopatias as principais manifestações dessas afecções. Formas graves de leptospirose comumente evoluem para insuficiência respiratória aguda, com presença de hemorragia, além de quadros de icterícia e insuficiência renal, podendo ser diagnóstico diferencial de doenças oportunistas em pacientes HIV em regiões endêmicas. Objetivo: Relatar quadro de manifestação atípica em paciente com Aids. Método: Relato de caso. Resultados: F. R. S. J., 34 anos, morador de área urbana, trabalhava com lavagem de ônibus, previamente hígido. Quadro de dispneia progressiva ao repouso há 3 dias, e febre, além de episódio de escarro hemoptoico, necessitando de suplementação de oxigênio em máscara de alto fluxo. Apresentou elevação de provas inflamatórias, teste HIV positivo e tomografia de tórax com infiltrado algodonoso difuso. Foi submetido à intubação orotraqueal e iniciado tratamento com sulfametoxazol-trimetoprima, corticosteroides e antibioticoterapia de amplo espectro devido principal hipótese diagnóstica de pneumocistose e pneumonia bacteriana associada. Realizada coleta de CD4 com resultado de 78 células e carga viral de 723.000 cópias/mL. Foi submetido à broncoscopia com visualização de bronquite mucopiosanguinolenta difusa com coágulos, lavado broncoalveolar com aspecto piohemático e pesquisa de Pneumocystis negativa. Apresentou ainda disfunção renal importante, hiperlactatemia, hipercalemia e anemia, sendo levantada a hipótese de leptospirose pois parceira confirmou epidemiologia positiva, com relato de presença de ratos em residência. IgM reagente para Leptospirose. Após longo período de internação em UTI com necessidade de traqueostomia, sendo decanulado, recebeu alta hospitalar em uso de terapia antirretroviral e com recuperação da função renal. Conclusão: Infecções oportunistas devem ser pesquisadas em pacientes com Aids, principalmente naqueles com baixa contagem de CD4, no entanto, sempre devem ser considerados também diagnósticos diferenciais importantes como Leptospirose levando em consideração a epidemiologia, uma vez que a doença apresenta elevada morbimortalidade em casos graves.

Abstract: Introdução: A mucormicose é a terceira causa mais comum de infecção fúngica e acomete principalmente pacientes imunocomprometidos e diabéticos. A apresentação clínica varia e depende da porta de entrada do fungo, incluindo rino-órbita-cerebral, pulmonar, cutâneo, gastrointestinal e formas disseminadas. A forma gastrointestinal é rara (5-10% dos casos) e envolve estômago, seguido de cólon e íleo. Objetivo: Relatar caso de paciente com mucormicose gastrointestinal em cólon, uma condição rara, especialmente entre pacientes imunocompetentes. Método: Relato de caso e revisão da literatura. Resultados: Paciente, 20 anos, em situação de rua, tabagista e usuária de Crack, apresentou tosse seca, calafrios, mialgia e diarreia há uma semana, associados a perda de peso não quantificada. Ao exame físico, encontrava-se em regular estado geral, emagrecida, hipocorada, agitada, taquipneica, boa saturação. Ausculta pulmonar com roncos esparsos. Evoluiu para estado de choque, realizada intubação orotraqueal. Admitida em unidade de cuidados intensivos com antibioticoterapia por sepse de foco pulmonar. Tuberculose afastada. Sorologia IgM para COVID-19 positiva, IgG e teste rápido negativos. Após 8 dias de internação, apresentou melena e enterorragia. A colonoscopia revelou úlcera em cólon ascendente, friável, com coágulo aderido e sangramento ativo e foi realizada escleroterapia e clipagem. Após procedimento, evoluiu com pneumoperitônio e abdome agudo e foi realizada colectomia direita. Hemocultura positiva com presença de leveduras. A biópsia da úlcera do cólon demonstrou tecido necrótico com granuloma supurativo com hifas largas e bulbos, compatível com mucormicose. O produto da colectomia demonstrou presença do fungo. Realizado tratamento com anfotericina B lipossomal por 30 dias. Tomografia de crânio e seios da face e biópsia de lesão de pele não demonstraram alterações significativas. Evoluiu com melhora e, após cerca de 60 dias de internação, teve alta hospitalar. Conclusão: A mucormicose intestinal é uma infecção rara e grave que resulta de ingestão de esporos, causando úlceras gastrointestinais necróticas. Ocorre principalmente em imunodeprimidos e diabéticos. Tem sido também demonstrada em pacientes com diagnóstico anterior de COVID-19, sendo mais comum a rino-órbita-cerebral. O caso demonstra que a mucormicose é um diagnóstico a ser considerado mesmo em indivíduos sem os fatores de risco clássicos, sobretudo em pacientes que, apesar de imunocompetentes, enfrentaram internação prolongada e infecções graves.

Abstract: Introdução: O tétano é uma doença causada por toxinas produzidas pela bactéria Clostridium tetani. É uma doença muito prevalente em países em desenvolvimento e prevenível por vacinação, a qual, no Brasil, é distribuída gratuitamente pelo SUS. Apesar disso, a incidência do tétano tem se mostrado alta e letal, se comparado a outros países. Isso ocorre devido à falha na cobertura vacinal nacional, a qual, não atinge índice de vacinação de 95% preconizado. Sendo assim, esses indivíduos tornam-se suscetíveis à infecção. Objetivo: Analisar o histórico de casos de tétano dos últimos 10 anos no Hospital Universitário de Londrina (HUL). Método: Foi realizado um levantamento dos casos de tétano internados no HUL através de revisão de prontuários médicos. O número de pacientes total foi de 19. Resultados: Nesse sentido, é possível estabelecer que o perfil do paciente com tétano atendido no HUL é: Homem de meia idade (Me: 54 anos, x¯: 58,26 anos e σ: 18,58 anos). A maioria não conhece seu status vacinal ou não é vacinado. Ocupação dos pacientes: pedreiro, eletricista, morador de rua e trabalhador do campo. Todos com contágio acidental, esses pacientes apresentaram um curto período de incubação (cerca de uma semana), desenvolvendo, logo após, sintomas referentes ao tétano generalizado, sendo os mais presentes trismo, febre, rigidez muscular, disfagia e espasmos musculares. O manejo do paciente foi similar em relação ao uso de antibiótico, em todos foram utilizados penicilina e/ou metronidazol. Em contrapartida, o uso de Soro Antitetânico (SAT) e Imunoglobulina (IGHAT) variou entre pacientes: quatro utilizaram SAT e/ou IGHAT e três nenhum. Além disso, durante a internação, 6 pacientes necessitaram de debridamento da ferida contaminada. O Tempo de internação é de 1 a 14 semanas, sendo o tempo de internação em UTI, quando necessário, em média de 48 dias. Houve presença de infecções secundárias na maioria dos pacientes internados. O desfecho da maioria dos casos se dá em alta (57,89%). Conclusão: Portanto, fica clara a importância da conscientização, a fim de despertar a percepção da população acerca da gravidade da doença e da ação da atenção primária no processo de imunização nacional, principalmente entre adultos que não fazem o reforço.

Abstract: Introdução: Dentre as respostas fisiopatológicas da dengue, principalmente entre o segundo e o quarto dia de doença, o extravasamento de líquidos para o interstício pela fragilidade capilar ocasiona grande parte da sintomatologia. A plaquetopenia e as coagulopatias por consumo também fazem parte deste escopo e precisam sempre ser investigadas; são frequentemente associadas a casos de sangramentos em casos de dengue, não sendo comuns manifestações trombóticas. Até o momento, há poucos relatos de tromboembolismo pulmonar associado à dengue. Objetivo: Relatar quadro de tromboembolismo pulmonar associado a dengue. Método: Relato de caso. Resultados: J. D. F., 79 anos, masculino, portador de diabetes mellitus, hipertensão arterial sistêmica, além de episódio de acidente vascular prévio há 5 anos, com sequela de amaurose, apresentava cefaleia, mialgia, artralgia e dor abdominal, além de hematúria macroscópica, episódios de tontura associado à queda da própria altura, sendo necessário internação hospitalar. Recebeu hidratação endovenosa durante a internação. Apresentava plaquetopenia e sorologia IgM reagente para dengue. Evoluiu com episódios de apneia e dessaturação com necessidade de uso de oxigênio suplementar, e ao exame com murmúrios vesiculares abolidos à direita. Foi realizada angiotomografia de tórax confirmando tromboembolismo pulmonar agudo em tronco de artéria pulmonar, além de doppler venoso de membros inferiores com trombose venosa profunda em segmento de veia poplítea direita. Iniciado anticoagulação plena, permitida pelo nível de plaquetopenia, paciente evoluiu com estabilidade do quadro, possibilitando desmame de oxigênio. Recebeu alta hospitalar com resolução dos quadros e seguimento ambulatorial no ambulatório de Pneumologia e Cirurgia Vascular. Conclusão: Casos de tromboembolismo pulmonar em vigência de viremia por dengue são incomuns visto que a doença cursa com anormalidades hematológicas como trombocitopenia, aumento de hematócrito e leucopenia, além de hemorragia, coagulopatia e coagulação intravascular disseminada, promovendo episódios de sangramanto frequentes nos casos graves da doença, sendo incomuns fenômenos trombóticos.

Abstract: Introdução: Em 1897, foi descrito o primeiro caso de ectima gangrenosa associada a sepse por Pseudomonas aeruginosa - caracterizado por vasculite hemorrágica necrotizante. Ele se manifesta com lesões nodulares cutâneas que progridem para úlceras necróticas principalmente em região perineal, mas também em extremidades, tronco e face. Objetivo: Elucidar o raciocínio diagnóstico por trás de uma sepse associada a lesões de pele necrotizantes em imunocomprometidos e ressaltar os desafios em seu tratamento. Método: Relato de caso e revisão de literatura. Resultados: Paciente feminino, 52 anos, Aids, carga viral detectável, CD4 87, má aderente ao tratamento, com queixa de edema e dor em maxila esquerda e dificuldade para mastigar, há dois dias. Exame físico: pápulas eritematosas e crostosas em região malar esquerda e lábios associado à edema e enegrecimento ao redor, além de ulcerações em palato e linfonodomegalia dolorosa em cadeia cervical esquerda. Iniciado amoxicilina + clavulanato empiricamente. Foi realizado biópsia da lesão e iniciado ivermectina. Evoluiu com aumento importante do edema palpebral à esquerda e piora das lesões em face; escalonado antibioticoterapia para piperacilina+tazobactam. TC de face: aumento e infiltração de partes moles de hemiface esquerda. Progrediu com piora do edema e áreas de necrose na maxila esquerda, iniciado corticoterapia. Ao 3° dia de internação, evoluiu com aumento das lesões necróticas e da área de infiltração e insuficiência respiratória aguda, sendo realizado intubação orotraqueal e transferida para leito de UTI. Iniciado linezolida e anfotericina sob hipótese de mucormicose. Houve surgimento de lesão ulcerada em grandes lábios. Hemocultura e urocultura com Pseudomonas aeruginosa e Serratia marcescens sensível à meropenem. Anatomopatológico de pele com necrose da parede vascular em derme, histologicamente compatível com ectima gangrenoso associada ao crescimento de Pseudomonas. Ao fim de 2 meses de internação, realizado desbridamento das lesões, com retirada de tecido necrótico em face e exposição da arcada dentária superior. Alta hospitalar, com seguimento com a cirurgia plástica para correção das deformidades em face e programação de hemodiálise. Conclusão: Ectima gangrenoso é uma manifestação cutânea secundária à bacteremia por Pseudomonas aeruginosa presente em pacientes críticos e imunocomprometidos, sendo confundido com mucormicose pelas características da lesão. O caso apresentado destaca a gravidade da doença associada à sequela importante pós desbridamento de face.

Abstract: Introdução: A Escherichia coli é uma bactéria gram negativa, presente no trato digestivo, todavia podem provocar infecções no trato urinário (ITU), enterites, septicemia e meningite neonatal. A ITU é uma das infecções mais frequentes em pediatria, E. coli ocorre entre 75 a 95% dos casos; é uma bactéria que infecta animais, como aves e bovinos, levando a resistência bacteriana pelo uso inadequado, muitas vezes como fator de crescimento na avicultura. Objetivo: Analisar perfil sensibilidade aos antimicrobianos de uroculturas positivas para E. coli isoladas de crianças com idade. Método: Foi realizado um levantamento das uroculturas de crianças atendidas nas Unidades de Pronto Atendimento na cidade de Londrina/Pr no período de 2016 a 2019. Os resultados foram compilados em um banco de dados Excel e posteriormente analisadas. Utilizou-se como critérios de exclusão pacientes com dados duplicados. Resultados: O número de pacientes foi de 1544, dos quais 156 foram excluídos por terem o prontuário duplicado, restando um total de 1388 uroculturas.: Desse total, 81.2% são do sexo feminino e 18.8% do sexo masculino. Em relação a idade 64.8% apresentavam 0 a 5 anos, 29.8% 6 a 10 anos e 5.4% 11 a 15 anos (Me: 4.45 anos, x¯: 4.00 anos e σ: 3.15 anos). Além disso, 15.9% tiveram que voltar ao serviço de saúde. Cerca de 10.8% dos casos concentram-se nas UBS Santiago e na Armindo Guazzi, entre outras 29 UBS. Em relação ao perfil de sensibilidade a Ampicilina apresentou maior índice de resistência 51.9% dos casos, seguida por Cotrimoxazol 29.2%, depois pela Cefalotina 23.9% e, por fim, o Ácido Nalidíxico que apresentou uma resistência de 17.9%. Apenas 3.9% das cepas são ESBL. Os demais antibióticos da classe dos betalactamicos, cefalosporina, carbapenêmicos, aminoglicosídeos, fluoroquinolonas e nitrofurantoínas apresentaram uma sensibilidade maior de 93%. Conclusão: Na população estudada, a presença de bactérias ESBL é baixa, sendo os betalactâmicos como a cefuroxima-axetil uma alternativa para o tratamento via oral. Como esperado, o cotrimoxazol e a cefalotina possuem baixa sensibilidade, não sendo escolhas no tratamento empírico das ITUs na faixa etária estudada. A importância de se conhecer o perfil de sensibilidade aos antimicrobianos para o tratamento das ITUs, leva a menor índice de recidivas, gastos, internações disseminação de bactérias multirresistentes e o uso indiscriminado de antimicrobianos.
