Introduction. When using concrete and reinforced concrete structures, it is necessary to overcome problems associated with the quality of concrete to shrink in size and experience shrinkage during hardening and drying. To solve problems with shrinkage, special attention is paid to the materials that, when added to ordinary portland cement, make it possible to obtain non-shrinkable, expanding types of cements. A well-known way of producing expanding cements is to grind cement clinker, gypsum and a special additive together. In addition, the introduction of active mineral additives with pozzolanic properties allows to compact the structure of the cement stone, reduce porosity, increase strength and reduce shrinkage. They also save the clinker for the cement production, help to reduce the cost of production and help to reduce the burden on the environment. One of these mineral additives is tripoli.Materials and methods. The effect of additives content on the deformation property of the composite binger was studied using the experiment planning method. Deformation of cement at the age of 2, 3, 7, 14 and 28 days were determined in accordance with the method of results processing in accordance with GOST R 56727–2015, GOST 11052–74. The research was carried out in the laboratory of the Department of Construction Materials Science of the Moscow State University of Civil Engineering.Results. The results obtained are presented as the surface of a second level regression equation describing the dependence of the blinder deformation on the content of complex expanding and pozzolanic additives. As a result of the experiments conducted, the composition with the maximum expansion and the composition with maximum shrinkage using complex additives were obtained. The results will be further used to predict the deformation properties of the binder.Conclusions. Based on the results of this study, the authors will continue to study the properties of binders based on a complex expanding and pozzolanic additive in the future.

12. Pharmacist perceptions of patient care competency: A survey in Vietnam

Context: In order to perform effectively in professional practice, a pharmacist should demonstrate crucial competencies on delivery of patient care. Aims: To evaluate current pharmacists’ perceptions on their delivery of patient care competencies. Methods: The evaluation divided into 8 domains of competencies with 35 behavioral statements was distributed to hospital and community pharmacists. Respondents were asked to self-evaluate their competencies on a four-point Likert scale. Results: Of 207 responses returned, there were 106 hospital pharmacists and 89 community pharmacists. The mean score ranged from 2.2 to 3.5. Respondents perceived themselves as the most competent in “Drug Specific Issues” and “Provision of drug product” cluster (3.5 ± 0. 68, 3.3 ± 0.84, respectively), whereas the lowest performance was noted in “Patient Consultation” and “Evaluation of Outcomes” (2.8 ± 1.01 and 2.8 ± 0.88, respectively). Pharmacists demonstrated the best performance in the behavioral statements pertaining to the “Appropriate route is ensured” (3.6 ± 0.63). The two behaviors with the lowest performance were “Recording Consultations” (2.2 ± 1.02) and “Record of contributions” (2.7 ± 0.93). There were no statistically significant differences in the competence cluster scores in terms of types of pharmacists, sex, age, education level, years of experience (p>0.05). Conclusions: This research represents the first self-assessment of Vietnamese pharmacists in patient care practice. Areas for additional professional education which were determined include recording patient consultation, obtaining patient consent, prioritization of drug-related problems, referrals to doctors, and assessment of patient outcomes.

13. Searching for Intermediate-mass Black Holes in Globular Clusters through Tidal Disruption Events

- Abstract
Intermediate-mass black holes (IMBHs) may be the link between stellar mass holes and the supermassive variety in the nuclei of galaxies, and globular clusters (GCs) may be one of the most promising environments for their formation. Here, we carry out a pilot study of the observability of tidal disruption events (TDEs) from 103 M ⊙ < M • < 105 M ⊙ IMBHs embedded in stellar cusps at the center of GCs. We model the long super-Eddington accretion phase and ensuing optical flare, and derive the disruption rate of main-sequence stars as a function of black hole mass and GC properties with the help of a 1D Fokker-Planck approach. The photospheric emission of the adiabatically expanding outflow dominates the observable radiation and peaks in the near-ultraviolet/optical bands, outshining the brightness of the (old) stellar population of GCs in Virgo for a period of months to years. A search for TDE events in a sample of nearly 4000 GCs observed at multiple epochs by the Next Generation Virgo Cluster Survey yields null results. Given our model predictions, this sample is too small to set stringent constraints on the present-day occupation fraction of GCs hosting IMBHs. Naturally, better simulations of the properties of the cluster central stellar distribution, TDE light curves, and rates, together with larger surveys of GCs are all needed to gain deeper insights into the presence of IMBHs in GCs.

16. Temporal and spatial requirements of Smoothened in ventral midbrain neuronal development

Background: Several studies have indicated that Sonic hedgehog (Shh) regulates the expansion of dopaminergic (DA) progenitors and the subsequent generation of mature DA neurons. This prevailing view has been based primarily on in vitro culture results, and the exact in vivo function of Shh signaling in the patterning and neurogenesis of the ventral midbrain (vMB) remains unclear.Methods: We characterized the transcriptional codes for the vMB progenitor domains, and correlated them with the expression patterns of Shh signaling effectors, including Shh, Smoothened, Patched, Gli1, Gli2 and Gli3.Results: While Shh and its downstream effectors showed robust expression in the neurogenic niche for DA progenitors at embryonic day (E)8 to E8.5, their expression shifted to the lateral domains from E9.5 to E12.5. Consistent with this dynamic change, conditional mutants with region-specific removal of the Shh receptor Smoothened in the vMB progenitors (Shh-Cre;Smofl/fl) showed a transient reduction in DA progenitors and DA neurons at E10.5, but had more profound defects in neurons derived from the more lateral domains, including those in the red nucleus, oculomotor nucleus, and raphe nuclei. Conversely, constitutive activation of Smoothened signaling in vMB (Shh-Cre;SmoM2) showed transient expansion of the same progenitor population. To further characterize the nature of Shh-Smoothened signaling in vMB, we examined the BAT-GAL reporter and the expression of Wnt1 in vMB, and found that the antagonistic effects of Shh and Wnt signaling critically regulate the development of DA progenitors and DA neurons.Conclusion: These results highlight previously unrecognized effects of Shh-Smoothened signaling in the region-specific neurogenesis within the vMB. © 2013 Tang et al.; licensee BioMed Central Ltd.

- Abstract
The non-treated wastewater from residential areas contains high concentrations of ammonium-nitrogen (NH4+-N). When discharged into the drainage water system, it deteriorates the water quality in urban rivers. This study used two types of materials to form eco-bags, using activated zeolite bead (AZB) and alkaline pretreated straw (APS), in geotextile bags for easy recovery and reuse. The AZB and APS provided the breeding habitat for the microorganisms that promoted biofilm formation on their surface. The immobilization of engineered denitrification microorganisms facilitated the removal of NH4+-N from the urban river water. The NH4+-N removal in the AZB and APS bags were in the range of 64–73%, and 56–61%, respectively, while the chemical oxygen demand (COD) removal in the AZB and APS bags ranged from 33–36%, and 30–31%, respectively. In addition, as evident from DNA and microbial community analysis, the microorganisms demonstrated a greater proclivity to grow and proliferate on the surface of AZB and APS and improved the water quality of urban rivers.

For more than half a century, researchers around the world have been engaged in attempts to achieve fusion ignition as a proof of principle of various fusion concepts. Following the Lawson criterion, an ignited plasma is one where the fusion heating power is high enough to overcome all the physical processes that cool the fusion plasma, creating a positive thermodynamic feedback loop with rapidly increasing temperature. In inertially confined fusion, ignition is a state where the fusion plasma can begin "burn propagation" into surrounding cold fuel, enabling the possibility of high energy gain. While "scientific breakeven" (i.e., unity target gain) has not yet been achieved (here target gain is 0.72, 1.37 MJ of fusion for 1.92 MJ of laser energy), this Letter reports the first controlled fusion experiment, using laser indirect drive, on the National Ignition Facility to produce capsule gain (here 5.8) and reach ignition by nine different formulations of the Lawson criterion.

28. Cochrane systematic review and meta-analysis of the impact of psychological treatment on health-related quality of life in people with epilepsy: an update by the ILAE Psychology Task Force, highlighting methodological changes

- Abstract
Clinical interest in using psychological interventions for people with epilepsy (PWE) aiming at decreasing mental health difficulties, improving health-related quality of life (HRQOL) and seizure-related outcomes, continues to grow. This article summarizes the 2020 update of the 2017 Cochrane Review and meta-analysis of psychological interventions for PWE, highlighting the reasons for major methodological modifications such as the recategorization of interventions and expanded risk of bias assessment. A 2020 literature search yielded 36 RCTs (n=3526) investigating psychological treatments for PWE with a validated HRQOL measure as an outcome. Twenty-seven trials were skills-based psychological interventions, whilst nine studies were education-only interventions. Among skills-based psychological interventions, 11 studies (n=643) used the Quality of Life in Epilepsy-31 (QOLIE-31) or other QOLIE inventories convertible to QOLIE-31 as an outcome measure and were pooled for meta-analysis. Significant mean changes were observed for the QOLIE-31 total score (mean improvement of 5.23 points; p< 0.001) and in six out of seven subscales (emotional well-being, energy and fatigue, overall QoL, seizure worry, medication and cognitive functioning). The mean changes in the QOLIE-31 total score and the overall QoL subscale exceeded the threshold of minimally important change (MIC), indicating clinically meaningful post-intervention improvement. These results provide moderate evidence that psychological treatments for adults and adolescents with epilepsy enhance HRQOL. In addition to the summary of the Cochrane review, we provide a detailed characterization of the interventions and patient populations of the meta-analyzed studies.

34. Managing depression and anxiety in people with epilepsy: A survey of epilepsy health professionals by the ILAE Psychology Task Force

Objectives\ud \ud The Psychology Task Force of the Medical Therapies Commission of the International League Against Epilepsy (ILAE) has been charged with taking steps to improve global mental health care for people with epilepsy. This study aimed to inform the direction and priorities of the Task Force by examining epilepsy healthcare providers’ current practical experiences, barriers, and unmet needs around addressing depression and anxiety in their patients.\ud \ud \ud Methods\ud \ud A voluntary 27‐item online survey was distributed via ILAE chapters and networks. It assessed practices in the areas of screening, referral, management, and psychological care for depression and anxiety. A total of 445 participants, from 67 countries (68% high income), commenced the survey, with 87% completing all components. Most respondents (80%) were either neurologists or epileptologists.\ud \ud \ud Results\ud \ud Less than half of respondents felt adequately resourced to manage depression and anxiety. There was a lack of consensus about which health professionals were responsible for screening and management of these comorbidities. About a third only assessed for depression and anxiety following spontaneous report and lack of time was a common barrier (>50%). Routine referrals to psychiatrists (>55%) and psychologists (>41%) were common, but approximately one third relied on watchful waiting. A lack of both trained mental health specialists (>55%) and standardized procedures (>38%) was common barriers to referral practices. The majority (>75%) of respondents’ patients identified with depression or anxiety had previously accessed psychotropic medications or psychological treatments. However, multiple barriers to psychological treatments were endorsed, including accessibility difficulties (52%).\ud \ud \ud Significance\ud \ud The findings suggest that while the importance of managing depression and anxiety in patients with epilepsy is being recognized, there are ongoing barriers to effective mental health care. Key future directions include the need for updated protocols in this area and the integration of mental health professionals within epilepsy settings.

- Abstract
PURPOSE: Contemporary eye care increasingly recommends the use of advanced retinal imaging technology. Anecdotal evidence suggests that this equipment is widely available in primary eye care settings; however, knowledge regarding how optometrists use this equipment in the context of diabetic retinopathy (DR) is limited. This study aimed to obtain a current overview of optometrists' clinical practice behaviours in the detection, screening, diagnosis and management of patients with diabetes. METHODS: A cross-sectional survey was designed to evaluate optometrists' self-reported clinical practice patterns and perceptions, as well as the availability and impact of retinal imaging equipment specific to DR and diabetic macular oedema (DMO) on optometrists' clinical practice. The survey invited participation from all optometrists practising in Australia. RESULTS: One hundred and sixty-seven optometrists participated. Optometrists' self-reported confidence in assessing DR and DMO was high. Optometrists' referral patterns considered the severity of DR and DMO before initiating referral to secondary ophthalmology care. Nearly all optometrists (98.8%) indicated that they had some form of retinal imaging equipment available to them in clinical practice. An optical coherence tomography (OCT) device was available to 75.5% of optometrists. A significant association between having an OCT device in the practice and higher self-reported confidence levels in the assessment of DMO was found. CONCLUSIONS: Many optometrists are well equipped with sophisticated retinal imaging technology for the provision of high-quality eye care. Enhancing optometric training and education programmes can maximise the community benefit of access to this equipment and improve delivery of eye care in the community.

- Abstract
OBJECTIVES: The Psychology Task Force of the Medical Therapies Commission of the International League Against Epilepsy (ILAE) has been charged with taking steps to improve global mental health care for people with epilepsy. This study aimed to inform the direction and priorities of the Task Force by examining epilepsy healthcare providers' current practical experiences, barriers, and unmet needs around addressing depression and anxiety in their patients. METHODS: A voluntary 27-item online survey was distributed via ILAE chapters and networks. It assessed practices in the areas of screening, referral, management, and psychological care for depression and anxiety. A total of 445 participants, from 67 countries (68% high income), commenced the survey, with 87% completing all components. Most respondents (80%) were either neurologists or epileptologists. RESULTS: Less than half of respondents felt adequately resourced to manage depression and anxiety. There was a lack of consensus about which health professionals were responsible for screening and management of these comorbidities. About a third only assessed for depression and anxiety following spontaneous report and lack of time was a common barrier (>50%). Routine referrals to psychiatrists (>55%) and psychologists (>41%) were common, but approximately one third relied on watchful waiting. A lack of both trained mental health specialists (>55%) and standardized procedures (>38%) was common barriers to referral practices. The majority (>75%) of respondents' patients identified with depression or anxiety had previously accessed psychotropic medications or psychological treatments. However, multiple barriers to psychological treatments were endorsed, including accessibility difficulties (52%). SIGNIFICANCE: The findings suggest that while the importance of managing depression and anxiety in patients with epilepsy is being recognized, there are ongoing barriers to effective mental health care. This paper describes a method o f analyzing potential railgun concepts for their rail heating effects in multishot mode. The method is meant to provide a quick estimate of heating effects based on the geometry of the rails and containment and the performance specifications of each shot. First, a coupled electrical-mechanical simulation is run for the railgun in which the effects of current diffusion are included to calculate the effective rail resistance and associated heating as a function of axial position and time. This analysis is then expanded with an assumed current distribution (relative amount of current on top and back of rail compared with the front) to develop a first order temperature distribution in the rail cross section at any specified axial location. This axial section is then subjected to two-dimensional finite element analysis (FEA) to determine the heat redistribution between shots. A sample set of calculations is compared with a single shot experiment incorporating temperature probes in the rails. Finally, the utility of the method for multishot analysis is shown with two simulations, one with and one without active cooling in the rails. Index Terms - cooling, heating, railguns, thermal.
