Urinary tract infections (UTI) are one of the most common problems in urology clinics. The European Association of Urology (EAU) has been pioneering in its efforts to disseminate the latest clinical findings through the organization of the annual EAU congresses. At this year’s congress (EAU Barcelona 2019), various satellite symposia were organized, focusing on specific issues in the field of urology. “UTI – quo vadis? New alternatives to treat uncomplicated urinary tract infections” was one of the industry-sponsored symposia, organized with the aim of evaluating the current scenario and also throwing light on the paradigm shift in the treatment of acute, uncomplicated lower urinary tract infections (uUTI). Several interlinking topics were presented during this symposium. The topics covered antibiotic resistance, involving a presentation of the current data from the Global Prevalence Study on Infections in Urology (GPIU-study). This discussion was followed by case reports on the impact of antibiotic resistance on the management of patients with UTI/uUTI and treatment options for UTI/uUTI according to current guidelines. The highlight of the symposium was the presentation of very recent data from a gold standard phase III clinical trial (double-blind, double-dummy randomized study), demonstrating the non-inferiority of a herbal medicine( BNO 1045) versus antibiotic therapy (fosfomycin trometamol (FT), as a single dose = 3 g) for the treatment of acute, uncomplicated cystitis., Инфекции мочевыводящих путей (ИМП) являются одними из самых распространенных проблем в урологической практике. Во время этого симпозиума было представлено несколько связанных тем. Темы охватывали антибиотикорезистентность, включая обнародование современных данных Исследования всемирной распространенности инфекционных заболеваний в урологии (исследование GPIU). In approximately one-third of patients with sepsis, the source of infection is the urinary tract. The management of sepsis has rapidly changed over the past two decades, and a review of urosepsis management is paramount. It is estimated that in 30% of patients with severe sepsis and septic shock, the underlying reason is a urinary tract infection (UTI). The prevalence of microbiologically proven urosepsis in urology departments has been reported as 1.5% (quarter of health care–associated UTIs). On a global level, it has been postulated that 5.4 million deaths occur due to sepsis. The main causes of urosepsis are indwelling urinary catheters and urologic interventions (stone treatment, prostate biopsies, and endoscopic urethral stricture treatment). Urosepsis-causative pathogens are primarily gram-negative bacteria; this is different from sepsis overall, which is dominated by gram-positive bacteria. Its been reported that the resistance rates of pathogens in urosepsis are >10% for almost all antibiotics. The main principles of management of urosepsis and sepsis are the same, including early goal-directed treatment and antibiotic administration within the first 45min. Early goal-directed therapy was recently shown not to be superior to standard care; however, these results may not be applicable to settings in which standard care needs improvement. Selection of an appropriate antibiotic for the initial empirical treatment in urosepsis requires knowledge of previous interventions, antibiotic usage, and local resistance rates. Future research on the management of urosepsis should be directed toward identification of groups at risk of developing urosepsis, antibiotic selection, and value of biomarkers in treatment response (eg, lactate, procalcitonin). Patient summary In approximately one-third of patients with sepsis, the source of infection is the urinary tract. This review assessed causes and management of urosepsis and directions for future research.

Background Infection is a serious adverse effect of prostate biopsy (P-Bx), and recent reports suggest an increasing incidence. Objective The aim of this multinational multicentre study was to evaluate prospectively the incidence of infective complications after P-Bx and identify risk factors. Design, setting, and participants The study was performed as an adjunct to the Global Prevalence Study of Infections in Urology (GPIU) during 2010 and 2011. Men undergoing P-Bx in participating centres during the 2-wk period commencing on the GPIU study census day were eligible. Outcome measurements and statistical analysis Baseline data were collected and men were questioned regarding infective complications at 2 wk following their biopsy. The Fisher exact test, Student t test, Mann-Whitney U test, and multivariate regression analysis were used for data analysis. Results and limitations A total of 702 men from 84 GPIU participating centres worldwide were included. Antibiotic prophylaxis was administered prior to biopsy in 98.2% of men predominantly using a fluoroquinolone (92.5%). Outcome data were available for 521 men (74%). Symptomatic urinary tract infection (UTI) was seen in 27 men (5.2%), which was febrile in 18 (3.5%) and required hospitalisation in 16 (3.1%). Multivariate analysis did not identify any patient subgroups at a significantly higher risk of infection after P-Bx. Causative organisms were isolated in 10 cases (37%) with 6 resistant to fluoroquinolones. The small sample size per participating site and in compared with other studies may have limited the conclusions from our study. Conclusions Infective complications after transrectal P-Bx are important because of the associated patient morbidity. Despite antibiotic prophylaxis, 5% of men will experience an infective complication, but none of the possible factors we examined appeared to increase this risk. Our study confirms a high incidence of fluoroquinolone resistance in causative bacteria.
