2. In vitro modulatory effects of interleukin-3 on macrophage activation induced by interleukin-2

Abstract

Background. The concomitant activation of macrophage-mediated immunosuppressive events might represent one of the most important biologic factors responsible for the decreased efficacy of cancer immunotherapies, including that of interleukin (IL)-2. In previous studies, the authors observed that the increase in the soluble IL-2 receptor (SIL-2R) and neopterin levels was related to the generation of macrophage-mediated immunosuppression and associated with a reduced clinical efficacy during IL-2 cancer immunotherapy. Because both cytokines and neurohormones may influence the macrophage system, the current study was done to evaluate the effects of IL-3 and of the pineal hormone melatonin (MLT) on monocyte response to IL-2 administration. Methods. Peripheral blood monocytes were incubated with different concentrations of IL-2, IL-3, and MLT, either alone or in association. Results. SIL-2R, neopterin, and tumor necrosis factor-alpha mean concentrations in the medium significantly increased during incubation with IL-2 at a concentration of 100 Cetus units/ml. IL-3 alone, at a dose of 10 ng/ml, also stimulated tumor necrosis factor release; no effect was found on SIL-2R and neopterin. The IL-2-induced neopterin rise was blocked by a concomitant incubation with IL-3 at a dose of 10 ng/ml. Finally, the concomitant incubation with IL-3 and MLT further inhibited neopterin release and significantly decreased IL-2-induced SIL-2R secretion. Conclusions. These results suggest that IL-3 alone or in association with MLT may modulate macrophage functions during cancer immunotherapy with IL-2 and decrease the IL-2-induced macrophage activation. 3. Postoperative increase in soluble interleukin-2 receptor serum levels as predictor for early recurrence in non-small cell lung carcinoma

Abstract

It is known that interleukin-2 (IL-2) plays an important role in the activation of host antitumor immune response. In addition to IL-2 cell surface receptor, a soluble form of IL-2 receptor (SIL-2R) may be released in the blood and potentially be involved in the regulation of IL-2 availability. High SIL-2R levels have been found in patients with lung cancer. The current study evaluated the influence of changes in SIL-2R serum levels during the perioperative period on early relapse rate in patients with operable non-small cell lung cancer. The study included 60 patients (epidermoid carcinoma, 33; adenocarcinoma, 27). Serum levels of SIL-2R were measured with an enzyme immunoassay before surgery and 7 and 30 days after surgery. A surgery-induced increase in SIL-2R levels was seen 7 days after surgery in 38 of 60 patients. On the 30th day after surgery, SIL-2R values were lower than the preoperative values in 32 patients (Group A) or still greater in the other 28 patients (Group B). After a median follow-up of 10 months, relapse occurred in 19 of 60 patients. The relapse rate was significantly higher in Group B than in Group A patients (16 of 28 versus 3 of 32, respectively; P < 0.001). This difference also was significant in relation to histotype and node status. This study shows that the persistence of increased CIL2R levels in the postoperative period is associated with a higher early relapse rate in patients with operable on-small cell lung cancer. The impact of SIL-2R levels on relapse suggest that host immune defenses may influence the clinical course of patients with lung cancer. Therefore, the evaluation of SIL-2R in the perioperative period may represent a new prognostic biologic factor in operable non-small cell lung cancer. Cancer 1992;69:2458-24682.

Abstract

Surgery can reduce the body's immune defenses. This immunosuppression may play a role in cancer relapse in patients having surgery to remove tumors. Micrometastases (microscopic spread of cancer) may be present, although undetected, at the time of surgery that is performed to remove the tumor. Production of interleukin-2 (IL-2) is important in the stimulation of most anti-cancer immune reactions which seem to be able to destroy fresh cancer cells. The ability of IL-2 to stimulate the immune system depends on its action on specific IL-2 receptors. Patients with widespread solid cancers have been noted to have abnormally high blood levels of a soluble type of interleukin-2 receptor. A study was undertaken to determine the influence of surgery on soluble interleukin-2 receptor serum levels in patients with solid tumors. There were 48 patients who underwent surgery for cancer and 11 controls without cancer who underwent major surgery. Serum levels of soluble interleukin-2 receptor were evaluated before and seven days after surgery; in both patient groups, the average serum levels were significantly higher after surgery than before. Due to the capacity of soluble interleukin-2 receptors to bind to interleukin-2, the increased levels of soluble interleukin-2 receptors may reduce availability of interleukin-2 and thus have a negative impact on immune function involving interleukin-2 that is directed against tumors. Abstract

Background. Unresectable brain metastases remain an untreatable disease. Because of its antitumor cytostatic action and its anticonvulsant effect, the pineal hormone melatonin could constitute a new effective agent in the treatment of brain metastases. The current study was performed to evaluate the effect of melatonin on the survival time in patients with brain metastases due to solid neoplasms. Methods. The study included 50 patients, who were randomized to be treated with supportive care alone (steroids plus anticonvulsant agents) or with supportive care plus melatonin (20 mg/day at 8:00 p.m. orally). Results. The survival at 1 year, free-from-brain-progression period, and mean survival time were significantly higher in patients treated with melatonin than in those who received the supportive care alone. Conversely, steroid-induced metabolic and infective complications were significantly more frequent in patients treated with supportive care alone than in those concomitantly treated with melatonin. Conclusions. The pineal hormone melatonin may be able to improve the survival time and the quality of life in patients with brain metastases due to solid tumors.

Abstract

Objective:In addition to sex steroids, prolactin (PRL) may also stimulate prostate cancer growth. Abnormally high blood levels of PRL have been noted in metastatic prostate cancer patients. However, most studies have been limited to the evaluation of basal levels of PRL rather than to investigate its secretion in response to classical endocrine dynamic tests. This study was carried out to analyze PRL secretion in metastatic prostate cancer patients both at basal conditions and in response to L–Dopa and metoclopramide, which represents the most classical inhibitory and stimulatory tests for PRL secretion, respectively.Methods:The study included 12 patients with metastatic prostate cancer. On separate occasions, PRL secretion was evaluated in response to L–Dopa (500 mg orally) and to metoclopramide (10 mg i.v. as a bolus). Serum levels of PRL were measured by RIA.Results:Mean PRL concentrations significantly increased after metoclopramide administration, even though no PRL response occurred in 6 of 12 patients. L–Dopa was unable to reduce PRL levels, which, in contrast, paradoxically significantly increased in response to L–Dopa, with mean values comparable to those achieved after metoclopramide injection.Conclusion:By showing a paradoxical stimulatory effect of L–Dopa on PRL secretion and a lack of response to metoclopramide in some patients, this study would suggest the existence of evident alterations in the neuroendocrine regulation of PRL release in advanced prostate cancer.

50. Aminoglutethimide in Postmenopausal Breast Cancer Refractory to Multiple Hormonal and Cytostatic Treatments

Abstract

Eighty-seven consecutive patients with metastatic breast cancer were treated with aminoglutethimide plus hydrocortisone. All patients were postmenopausal and had progressive disease following prior chemotherapy and endocrine therapy. Eighty-five women were evaluable for drug response. One patient showed complete remission and 14 patients partial response, for an overall response rate of 17%. The median duration of response was 11+months. The response rate was highest in the presence of soft tissue involvement (36%). The most common side effects were transient skin rash, lethargy, and dizziness. Two patients discontinued treatment because of cutaneous allergy. Aminoglutethimide can be considered a moderately active agent when utilized as second- or third-line hormonal therapy.
