Abstract

This paper presents a new active power control algorithm designed to maximize the power reserve of the individual turbines in a farm, in order to improve the tracking accuracy of a power reference signal. The control architecture is based on an open-loop optimal set-point scheduler combined with a feedback corrector, which actively regulate power by both wake steering and induction control. The methodology is compared with a state-of-the-art PI-based controller by means of high-fidelity LES simulations. The new wind farm controller reduces the occurrence of local saturation events, thereby improving the overall tracking accuracy, and limits fatigue loading in conditions of relatively high-power demand.

Abstract

FMR1 premutation cytosine-guanine-guanine repeat expansion alleles are relatively common mutations in the general population that are associated with a neurodegenerative disease (fragile X-associated tremor/ataxia syndrome), reproductive health problems and potentially a wide range of additional mental and general health conditions that are not yet well-characterised. The International Fragile X Premutation Registry (IFXPR) was developed to facilitate and encourage research to better understand the FMR1 premutation and its impact on human health, to facilitate clinical trial readiness by identifying and characterising diverse cohorts of individuals interested in study participation, and to build community and collaboration among carriers, family members, researchers and clinicians around the world. Here, we describe the development and content of the IFXPR, characterise its first 747 registrants from 32 countries and invite investigators to apply for recruitment support for their project(s). With larger numbers, increased diversity and potentially the future clinical characterisation of registrants, the IFXPR will contribute to a more comprehensive and accurate understanding of the fragile X premutation in human health and support treatment studies.

18. Speeding up the transition to a more sustainable food system: New insights into the links in a system of practices

Abstract

More sustainable food choices are part of a complex system that combines practices, which themselves are interconnected in a network of elements made up of competences, meanings and material devices. This article proposes an innovative method for studying this system through a non-deductive quantitative approach. The research adds to the corpus of the ‘theory of practices’ by defining the concepts of ‘central’ practices (i.e. connected to many elements of the system, such as the practice of buying and cooking sustainable products in the sustainable food system) and ‘peripheral’ practices (i.e. connected in a lesser extent to other elements of the system, such as the practices of anti-waste, growing your own food and the consumption of plant proteins). It also highlights the ‘connector’ role between the practices of certain elements. Finally, based on the current structure of the system of sustainable food practices, the article makes several propositions for speeding up change.

26. Wind Tunnel Testing of Combined Derating and Wake Steering

Abstract

This paper describes an experimental assessment of the impact of combined derating and wake steering by yawing on the power output of a cluster of three wind turbines. The test is conducted in a boundary layer wind tunnel, and the flow control strategies are applied to the sole upstream machine. The experiment reveals a relationship, so far never observed experimentally, between the amount of yaw-induced power losses and the implemented level of derating. Furthermore, combining derating and yawing can only be beneficial, in terms of improved power production at the cluster level, for modest derating values.

Abstract

Background. Teratomas demonstrate a benign clinical behavior, however they may recur with malignant components or as teratoma, and in a small group of patients prognosis could be fatal. After the ®rst Italian study, we collected cases of teratoma, alongside the protocol for malignant germ cell tumors. Procedure. Patients with teratoma were collected from 2004 to 2014. Teratomas were classi®ed according to the WHO classi®cations, as mature and immature. Patients with pathological aFP and/or bHCG, and those with a malignant germ cell component were not included. Results. The study enrolled 219 patients (150 mature, 69 immature teratomas) with a median age at diagnosis of 42 months. The primary sites involved were: 118 gonadal and 101 extragonadal teratomas. Two females with ovarian teratoma had a positive family history. Complete and incomplete surgeries were performed in 85% and 9% of cases. Seventeen events occurred: six females had a second metachronous tumor (5 contralateral ovarian teratoma, 1 adrenal neuroblastoma) and 11 teratomas relapsed/progressed (3 mature, 8 immature teratomas). Two patients died, one of progressive immature teratoma and one of surgical complications. At a median follow up of 68 months, the event-free, relapse-free, and overall survival rates were 90.6%, 94.3%, 98.6%, respectively. Conclusions. Teratomas show a good prognosis, especially the mature ones: surgery and follow-up remain the standard approach. Incomplete surgery in immature teratoma is the group at greatest risk of relapse. Bilateral ovarian tumors are a possibility, and the rare family predisposition to ovarian mature teratoma warrants further analyses.

Abstract

Introduction: The health-related quality of life in X-linked agammaglobulinemia was investigated in 25 children and adolescents patients through the Italian version of Pediatric Quality of Life Inventory 4.0 Generic Core Scale for patients aged less then 18 years, comparing child perception to that of the parents and the physician's evaluation. The data were compared with the ones of 80 healthy controls and the literature data of a group of patients with rheumatic diseases. Discussion: The agammaglobulinemia subjects perceived a lower global quality of life than the healthy subjects, but significantly higher than the rheumatic diseases controls. The clinical relevance of health-related quality of life assessment in X-linked agammaglobulinemia pediatric patients is discussed. © 2008 Springer Science+Business Media, LLC.

Abstract

Teratoma is the most common germ cell tumour in childhood; mature (MT) and immature teratomas (IT) are benign tumours, but if they recur, they can be in some cases malignant. The aim of this paper is to evaluate Italian patients with MT and IT enrolled from 1991 to 2001, in a prospective multicentric study. One hundred and eighty-three patients, observed in 15 Italian Centers of Paediatric Oncology and three Paediatric Surgical Units were enrolled. Clinical data, treatment and results were all analysed. Initial evaluation and subsequent follow up included clinical examination, tumour markers and imaging procedures. Surgical resection was recommended for all the tumours. Histology was centrally reviewed and IT was classified as grading 1-3. Chemotherapy (CT) with Vinblastine, D: -actinomycin and cyclophosphamide was indicated for extra-testicular IT grade 2 or 3. MT was diagnosed in 127 patients (93 F and 34 M, age 1-192 months, median 24): 58 patients had gonadic tumour (23 testicular, 35 ovaric), 69 extragonadic (45 sacrococcygeal, 11 mediastinic, 7 retroperitoneal, 6 in other sites). A complete resection was performed in 117 patients, a partial resection in eight patients and biopsy in one. IT was diagnosed in 56 patients (34 F, 22 M, age 1-168 months, median 7). The T grading was 1 in 14 cases, 2 in 26, 3 in 16; 28 had gonadic T (17 ovary, 11 testis), 28 extragonadic (sacrococcygeal 19, mediastinic 3, retroperitoneal 2, other sites 4). CT was administered in eight patients; 15/182 patients relapsed (1 in a metastatic site) and in 5/15 the relapse showed malignant histology. Seven MT (5.5%) relapsed (five sacrococcygeal, one retroperitoneal, one mediastinic): surgery at diagnosis had been complete in five and with residual in two; the relapse was malignant in two patients with sacrococcygeal (sc) tumours, who had a complete resection and a partial resection respectively. Eight IT (14.2%) relapsed (four ovary, three sc, one retroperitoneal). The initial surgical resection had been complete in one, with residual in six, and a biopsy had been performed in one. A malignant recurrence occurred in two patients with sc tumours (after partial resection in one and after biopsy + CT in one) and in one patient with ovarian IT after a partial resection. All the patients underwent surgical excision of the recurred mass; CT according to Protocol for Malignant GCT was administered to those who had malignant recurrence; 122/126 patients with MT and 53/56 with IT are alive without disease with a follow up of 8-144 months (median 56). Two patients with malignant relapse (one with sc MT, one with sc IT) died because of the progression of the disease. Another two died due to severe malformations (one MT, one IT) and three were lost to follow up (two MT, one IT). The overall survival (OS) at 10 years is 98% (95% CI 93.9-99.4); the event free survival (EFS) is 90.4% (95 CI 84.8-94.0). At Cox analysis no significant difference in EFS was found regarding age and site of the primary tumour, while females (P = 0.011), patients with grade 1-3 histology (P = 0.025) and patients with incomplete resection appeared at higher risk of death or relapse (P < 0.001), with a seven, three and eightfold increase in risk, respectively. Our data showed that incomplete resection and female gender are important risk factors for relapse or death, more so than IT histology. The number of patients treated with CT is not sufficient to evaluate the efficacy of CT in avoiding malignant relapse.

Abstract

Objective To investigate the clinical manifestations at diagnosis and during follow-up in patients with 22q11.2 deletion syndrome to better define the natural history of the disease. Study design A retrospective and prospective multicenter study was conducted with 228 patients in the context of the Italian Network for Primary Immunodeficiencies. Clinical diagnosis was confirmed by cytogenetic or molecular analysis. Results The cohort consisted of 112 males and 116 females; median age at diagnosis was 4 months (range 0 to 36 years 10 months). The diagnosis was made before 2 years of age in 71% of patients, predominantly related to the presence of heart anomalies and neonatal hypocalcemia. In patients diagnosed after 2 years of age, clinical features such as speech and language impairment, developmental delay, minor cardiac defects, recurrent infections, and facial features were the main elements leading to diagnosis. During follow-up (available for 172 patients), the frequency of autoimmune manifestations ( P = .015) and speech disorders ( P = .002) increased. After a median follow-up of 43 months, the survival probability was 0.92 at 15 years from diagnosis. Conclusions Our data show a delay in the diagnosis of 22q11.2 deletion syndrome with noncardiac symptoms. This study provides guidelines for pediatricians and specialists for early identification of cases that can be confirmed by genetic testing, which would permit the provision of appropriate clinical management.

Abstract

Particularly in tertiary care centres, there is an increased need for diagnostic procedures, such as CT scans, magnetic resonance, endoscopies, lumbar punctures, biopsies, etc., which require different degrees of anesthesia and/or sedation of the child to ensure safety and performance and to avoid unnecessary pain and discomfort. The limited availability of anestesiologists often represents a problem, leading to unnecessary waiting time. For this reason paediatricians and nurses with specific training in intensive care are increasingly involved in providing sedation for diagnostic procedures. Safety concerns have been voiced and discussed about the use of a variety of drugs and procedures. The Authors report their one-year experience in a tertiary care paediatric hospital regarding in-ward sedation for non invasive (such as diagnostic imaging) or moderately invasive (such as endoscopies and biopsies) procedures. Propofol was used for moderately invasive procedures. A small task force (2 paediatrician and 2 nurses) was established to ensure continuity and good performance. The experience has been satisfactory in terms of efficacy, safety and cost and has been positively evaluated by both patients and health professionals.

43. On the crowdsourcing of macroseismic data to characterize geological settings.

Abstract

Macroseismic data are obtained from observing the effects of an earthquake on people, buildings, and the natural environment. The crowdsourced macroseismic data are collected from a large number of people, often through online platforms or mobile applications. These data can be useful for characterizing geological features, depending on several factors, such as the quality of the data, the representativeness of the sample, and the methods used for data collection and analysis. As a case study, we consider the macroseismic data collected in Trieste (NE Italy) through online questionnaires completed by citizens after the 2020 Mw 6.4 earthquake in Petrinja (Croatia). The campaign was promoted through social media and in a short period of time more than 6000 questionnaires were completed by the citizens of Trieste. The analyzed macroseismic data show good agreement with the expected seismic response of the main soil types of the city. A comparison with a similar project we conducted in Trieste in 2012, following the 2012 Emilia May 20 and 29 earthquakes, shows that also in that case, although a much smaller number of questionnaires was collected, the main characteristics identified correspond well with the soil types of Trieste. Thus, our study proves the importance of collecting macroseismic data even in areas of low damage. Moreover, it shows how people's early engagement, computer skills, social networks, and smartphone popularity can influence the results of such data collection and opens new scenarios for a better understanding of earthquake risks and improved awareness and preparedness through citizen participation. Abstract

OBJECTIVE: to report and analyse time trends in cancer incidence among children (0-14 years of age), adolescents (15-19 years) and young adults (20-24 years) living in the Italian province of Trieste (2003 population, 242,000), between 1972 and 2003. DESIGN: population-based study of descriptive epidemiology. Setting and participants: the new cases of cancer diagnosed to the residents of the province of Trieste below 25 years of age were extracted from the database of the Trieste Cancer Registry (period 1972-1994) and from the database of the Friuli-Venezia Giulia Cancer Registry (period 1995-2003), according to the International Classification of Childhood Cancer (3rd edition). MAIN OUTCOME MEASURES: age-specific and age-standardized (Italian 1981 census population as standard) incidence rates, by diagnostic group, sex and period of diagnosis. Time trend in incidence was analysed by using a Poisson regression model adjusted for calendar year, sex and 5 year age-group, and was expressed as annual percent change (APC) in rates. RESULTS: in the period 1972-2003, the new cases of cancer were 168 in the age-group 0-14 years, 79 in the age-group 15-19 years and 111 in the age-group 20-24 years, while the person-years at risk were respectively: 1,050,027; 431,673; 496,450. The APC in the incidence of all cancers combined was 2.3% (IC 95% 0.6%-3.9%) in children, 4.4% (IC 95% 1.8%-7.1%) in adolescents and 5.1% (IC 95% 2.8%-7.5%) in young adults. Hodgkin lymphomas (APC =12.7%; IC 95% 2.6%-23.7%; 7 cases) in the age-group 0-14 years, skin melanomas and carcinomas (APC =8.2%; IC 95% 4.5%-12.0%; 49 cases) and central nervous system tumours (APC = 6.4%; IC 95% 1.5%-11.5%; 25 cases) in the age-group 15-24 years were the malignancies characterised by the highest increase in incidence. CONCLUSION: the increase in incidence rates observed in this study can be only partly explained by the small number of ascertained cases, by an improvement in diagnostic techniques and by more efficient registration. However, few environmental and hereditary factors are consistently associated with cancers affecting young people. Therefore, it is imperative to continue to carry out descriptive and analytical studies with primary prevention as the ultimate aim.
